Beyond Limits: What 366 Consecutive Marathons Reveal about the Human Heart

Fabricio Braga et al. Ricardo Stein

Western culture has always celebrated the pursuit of physical extremes — from ancient epics to modern ultramarathons, endurance has always been more than sport: it is narrative, identity, and defiance of perceived limits. Completing a single marathon already places substantial stress on the cardiovascular, metabolic, and musculoskeletal systems. Doing a Marathon 366 times over 366 consecutive days shifts the focus from physiology to philosophy: Just how far can the human heart truly go?

The case study published in this issue of Arquivos Brasileiros de Cardiologia documents a feat previously unimaginable: a Brazilian amateur athlete completed a full marathon every single day for a year — all under systematic medical supervision. More than a Guinness World Record, this is a real-time exploration of human adaptability. The athlete’s cardiovascular system remained remarkably stable throughout the year, with maximal oxygen uptake (VO2 max), cardiac structure, and biomarkers within physiological ranges, despite the extreme exercise volume.

My Approach to “Athlete's Heart”: Evaluation of the Different Types of Adaptation to Exercise

Frederico José Neves Mancuso

Abstract

Exercise-induced adaptation may occur in amateur and professional athletes. This condition is commonly named “athlete’s heart”. The alterations observed include dilation of the heart chambers, increased myocardial thickness, improved ventricular filling, increased left ventricular trabeculation, dilation of the inferior vena cava, among others. These changes can also be observed in some heart diseases, such as dilated, hypertrophic and other cardiomyopathies (CMP). Thus, cardiac imaging tests are fundamental in identifying these alterations and in differentiating between “athlete’s heart” and possible heart disease.

PURPOSE: To assess the prevalence of the athlete’s, heart syndrome in elite disabled Brazilians athletes. METHODS: Seventy-five athletes, age 27.8±6.7 years, 56 men, with various disabilities (47 physical, 12 visual and 16 cerebral paralysis) underwent clinical, electrocardiographic, vectorcardiographic, ergometric and echocardiographic evaluations. RESULTS: Athlete’s heart signs occurred in 33% of the clinical evaluations, in 55% of the electrocardiograms, in 15% of the vectorcardiograms, and in 5% of the echocardiograms. At least one of these signs was presented in 51% of the athletes. There were 2 or more abnormalities in 46% of the athletes and 4 or more signs in 12%. Exercise test was considered not ischemic in 77% of the subjects. There was right bundle branch block in 23% of the tests. CONCLUSION: There were two or more athlete’s heart syndrome signs in 46% of Brazilian disabled athletes.
