Unfolding Type B Aortic Dissection Controversies – Piecing Together the Evidence

Walter J. Gomes et al. Eduardo N. Gomes Nelson A. Hossne Jr.

Unlike type A aortic dissection, many aspects of the natural history of type B aortic dissection (TBAD) remain blurred and unsettled, leading to uncertainties in its prognosis, recommendations, and management., Among the critical factors impacting TBAD outcomes, the thrombosis of the false lumen seems to be an associated finding exhibiting a robust correlation with late prognosis and conveying significant clinical implications. In addition to being a critical prognostic indicator for TBAD patients, the presence or absence of false lumen thrombosis has been employed to guide treatment decisions, affecting the choice of therapeutic interventions, the thoracic endovascular aortic repair (TEVAR) and adjusting post-treatment follow-up.

In this way, Tang et al. report an interesting aspect of the correlation between imaging and clinics in this issue. Analyzing a large cohort of consecutive patients with TBAD, they investigated the factors affecting false lumen thrombosis in patients with TBAD, focusing on understanding the role of aortic morphology in the thrombosis of the false lumen. Computed tomographic angiography images were used for measurements. Specialized software reconstructed three-dimensional models of aortic dissection, and measurements of true and false lumen diameters were performed at different zones of the aorta. The incidence of thrombotic false lumen was higher in older patients with normal renal function than in younger patients or those with compromised kidney function.

Chronic Aortic Dissection and Pregnancy: Clinical Case Report

Giulia de Miranda Taglialegna et al. Leila Katz Larissa Mayara Aristóteles de Albuquerque Milena Máximo de Freitas … Alexandre Jorge Gomes de Lucena Melania Maria Ramos de Amorim

Introduction

Aortic complications during pregnancy can occur in women without risk factors, but they are more often associated with collagen diseases (Marfan and Ehlers-Danlos syndromes) and structural cardiac malformations (aortic coarctation and bivalve aortic valve)., Approximately half of these events occur during pregnancy in women under 40 years of age, particularly in the third trimester, or in the postpartum period. It is an extremely rare event, accounting for 0.1% to 0.4% of all aortic dissections, affecting 5.5 (95% CI: 4.0 – 7.8) women per 1 million during pregnancy and puerperium.,, It is believed that the physiological hemodynamic changes during pregnancy, structural changes determined by compression of the gravid uterus and the hormonal action can aggravate this aortic pathology.,

The most commonly used classifications are based on the affected anatomical portion. The Stanford group considers: type A (dissection involving ascending aorta) and type B (descending aorta only). Depending on the duration of symptoms until the clinical presentation, aortic dissection can be classified as chronic, if it occurs two weeks or more after symptom onset.

Case 1/2019 – A 51-year-old Man with Arterial Hypertension, Aortic Dissection and Aortic Valve Regurgitation, in Addition to Heart Failure with Unchanged Clinical Course After Surgical Intervention

Desiderio Favarato et al. Vera Demarchi Aiello

Clinical aspects

This was a male patient, who presented with ascending aortic dissection, and who, even after surgery for dissection repair, developed a picture of severe heart failure and died.

The International Registry of Acute Aortic Dissection (IRAD) showed that patients with aortic dissection were older than our patient, 61 years; and a majority of males (63%). Regarding the diseases related to the dissection, they were: Marfan syndrome (6.7%), hypertension (69.3%), atherosclerosis (24.4%), previously known aortic aneurysm (12.4%), previous aortic dissection 3.9%) and diabetes mellitus (4.3%). Also, 15.9% had a history of previous cardiac surgery and iatrogenic cause in 4.8% (1.7% coronary angiography and 3.1% after cardiac surgery). The 2015 update of the same registry, with a ten-fold higher number of patients, showed an increase in hypertension (75.5%) and a decrease in the presence of Marfan syndrome (4.5%), (75.5%), atherosclerosis (19.6%) and previous cardiac surgery (10.6%).
