Pulmonary Embolism and Light Chain Cardiac Amyloidosis with Progression to Heart Transplant

Thales Quintella Drummond et al. Lígia Lopes Balsalobre Trevizan Sandrigo Mangini Fernando Bacal

Introduction

Cardiac amyloidosis results from the deposition of amyloid fibrils in the heart’s extracellular space. This chronic and progressive disease significantly reduces quality of life and carries a high mortality rate if not diagnosed and treated early. Myocardial damage occurs due to direct injury by blood peptides, as well as architectural distortion caused by the extracellular infiltrate, leading primarily to diastolic ventricular dysfunction but also systolic dysfunction, as the disease progresses and cardiac arrhythmias emerge. The most common subtypes include abnormal transthyretin peptides (ATTR) and immunoglobulin light chain (AL), with AL having a worse prognosis and heart transplantation being a potential treatment option. In the United States, the estimated incidence of light chain amyloidosis cardiomyopathy is 1 in 100,000 individuals., The primary risk factor for the development of light chain amyloidosis is a personal history of monoclonal gammopathy.

Cardiac manifestations often involve orthostatic hypotension, arrhythmias, ventricular hypertrophy inconsistent with electrically inactive regions on the electrocardiogram, and heart failure with preserved ejection fraction predominantly, though reduced ejection fraction may occur in advanced cases.

Abstract

Amyloidosis is characterized by the localized or systemic deposition of proteins with unstable tertiary structure, which aggregate and form amyloid fibrils. Cardiac amyloidosis is a frequently underdiagnosed condition and it is an important cause of heart failure. There are more than 30 known types of amyloid proteins, but only five often infiltrate the heart, causing cardiac amyloidosis. light chain immunoglobulin, heavy chain immunoglobulin, transthyretin, serum amyloid A and apolipoprotein AI, mostly in light chain immunoglobulin or transthyretin forms. According to the type of fibrillar protein deposited, cardiac amyloidosis has different clinical courses, prognosis and different forms of treatment. In this review, we address new techniques that allow the diagnosis of this entity, especially in situations of heart failure with preserved ejection fraction and restrictive heart diseases. Early diagnosis is fundamental in defining the best therapeutic approach and in the prognosis of these patients.

Three-dimensional Speckle Tracking Echocardiography in Amyloidosis: A New Assessment Method for a Rare Disease

Brivaldo Markman Filho

Immunoglobulin deposition in the myocardium characterizes involvement of cardiac amyloidosis (CA). Fibrillary infiltration, which may happen in every heart cavity, leads to the restrictive cardiomyopathy phenotype, with complex pathophysiological mechanisms, which will result in the syndromic diagnosis of congestive heart failure. Diastolic dysfunction is dominant in most cases, and it may or may not, follow diverse levels of systolic dysfunction in the most advanced phases of the disease. Atrial remodeling by amyloid infiltration may contribute to cardiac output decrease by means of insufficient or nonexistent telediastolic atrial contraction. The onset of atrial electrical instability, ending in atrial fibrillation, highlights symptomatic worsening and these patients’ reserved prognosis.

Historically, the right cavities of the heart have been neglected in echocardiographic assessments. The complex morphology of the right ventricle (RV) has possibly contributed for the lack of reproducible data on echocardiographic cutting plans, diversely from the left ventricle (LV). The development of three-dimensional echocardiography (3DE) has allowed for a more accurate calculation of right ventricle volume and function in the diverse pathologies involving that chamber.
