			<title>Introduction</title>
			<p>The Takotsubo Syndrome, also known as “broken heart syndrome”, is characterized by a severe and transitory left ventricular dysfunction, with the presence of alterations in the contraction of the basal, medial, or apical segments of the left ventricular walls.<xref ref-type="bibr" rid="B1">1</xref> The syndrome can be caused by an emotional or physical trigger, or by a combination of both; however, this is not a mandatory criterion for its diagnosis.<xref ref-type="bibr" rid="B2">2</xref></p>
			<p>This study describes a case of Takotsubo Syndrome after mitral valve surgery to replace the valve with a mechanical prosthesis.</p>
		</sec>
		<sec sec-type="cases">
			<title>Case report</title>
			<p>A 56-year-old, female patient, with a history of smoking, atrial fibrillation, rheumatoid arthritis, and mitral stenosis, underwent a percutaneous mitral valvuloplasty in 2007, using warfarin, atenolol, digoxin, amiodarone, prednisone (intermittent), hydroxychloroquine, and leflunomide. The patient was hospitalized to undergo elective mitral valve replacement surgery.</p>
			<p>The transthoracic echocardiogram showed an ejection fraction of 64%, with an increase in the left atrium (indexed volume of 122ml/m<xref ref-type="bibr" rid="B2">2</xref>) and eccentric left ventricular hypertrophy. The mitral vale presented thickened cusps, with the posterior leaflet fixed and the anterior with a cup-shaped opening. The Doppler imaging showed an important reflux with a maximum and average gradient of 14 and 4 mmHg, respectively. The planimetry indicated a valve area of 1.9 cm<xref ref-type="bibr" rid="B2">2</xref>. The previously conducted cinecoronariography showed a 40% obstruction in the anterior descending artery (<xref ref-type="fig" rid="f01">Figure 1</xref>).</p>

			</p>
			<p>The patient underwent mitral valve surgery to replace the valve with a mechanical prosthesis, and to close the left atrial auricle, without complications. On the second post-operative day, the patient evolved with a hypertensive acute pulmonary edema and atrial fibrillation with a good ventricular response, introducing a treatment with intravenous amiodarone, nitroglycerin, and furosemide. The electrocardiogram presented a T-wave inversion in the V2-V4 derivations (<xref ref-type="fig" rid="f02">Figure 2</xref>). A transthoracic echocardiogram was performed, which presented hypercontractility of the basal segments and akinesia of the medial, apical, and apex segments (standard suggestive of Takotsubo cardiomyopathy), as well as a high-degree systolic dysfunction of the left ventricle (ejection fraction of the left ventricle = 22% by the Simpson method). The mitral mechanical prosthesis showed a normal function, with no obstruction of the left ventricle outflow (<xref ref-type="fig" rid="f03">Figure 3</xref> and <xref ref-type="fig" rid="f04">4</xref>, <xref ref-type="fig" rid="m01">Videos 1</xref> and <xref ref-type="fig" rid="m02">2</xref>). Due to a suspected coronary complications resulting from the heart surgery, a dose of troponin was applied, which showed a value of 3,305 ng/L (reference: &lt; 11 ng/L). However, the patient’s coronary anatomy was only understood after the decision by the clinical-surgical team, along with the results from the imaging exams (echocardiographic standard) and the given surgical context. For this reason, we opted to manage this situation clinically.</p>

			<p>Due to the worsening of her medical condition, the patient underwent orotracheal intubation and therapy with noradrenaline (0.25 mcg/kg/min), and dobutamine (8 mcg/kg/min) was applied in the context cardiogenic shock. After 96 horas, The patient’s medical condition improved to a hemodynamic state, leading to the gradual suspension of the support measures. The echocardiogram showed a recovery of the systolic function of the left ventricle, with a left ventricle ejection fraction of 61%, with anomalous movement of the intraventricular septum and myocardial contractility preserved in the other segments of the left ventricle. The mechanical prosthesis was in the mitral position, with a good excursion of its mobile elements.</p>
			<p>After an excellent clinical evolution, the patient was released from the ICU, with subsequent outpatient follow-up.</p>

			<title>Discussion</title>
			<p>The Takotsubo Syndrome has been described in some cases of patients submitted to gynecological, urological, and abdominal surgeries.<xref ref-type="bibr" rid="B3">3</xref> The cardiovascular procedures are also reported as triggers of the Takotsubo Syndrome, including stress tests with dobutamine, pericardiocentesis, cardioversion of atrial fibrillation, insertion of a pacemaker, electrophysiological tests, ablation and valve replacement surgery, and the use of a transcatheter.<xref ref-type="bibr" rid="B4">4</xref></p>
			<p>Takotsubo cardiomyopathy in the perioperative period has gained recognition, but its association with heart surgery is, to a great extent, mostly unknown and rarely described.<xref ref-type="bibr" rid="B5">5</xref> There are insufficient data in the literature on which to base the hypothesis of the prevalence of a cardiothoracic procedure or other procedure concerning the increased risk of the development of the syndrome. However, what is important to note here is the increase in the number of cases that describe the development of Takotsubo Syndrome after heart surgery.<xref ref-type="bibr" rid="B5">5</xref></p>
			<p>The possible mechanisms associated with the event in patients submitted to heart surgery are likely multiple, such as a coronary vasospasm (epicardial and microvascular ischemia), toxicity mediated by catecholamine, and excessive sympathetic activation.<xref ref-type="bibr" rid="B6">6</xref></p>
			<p>Lyon et al.,<xref ref-type="bibr" rid="B7">7</xref> in a review of the physical-pathological mechanisms involved in the genesis of this syndrome, highlight the intense sympathetic activation, as well as the high levels of catecholamines, as central triggers. These findings are related to micro and macrovascular dysfunctions and, consequently, to the dysfunction of the cardiomyocytes and their respective consequences evidenced in the manifestation of the syndrome.</p>
			<p>Thus, the development of electrocardiographic abnormalities, circumferential dysfunction of the ventricular contraction, clinical signs/symptoms (precordial pain and/or hypotension/circulatory shock) in the postoperative period of cardiovascular surgery are possible manifestations of the Takotsubo Syndrome, likely caused by extracorporeal circulation and surgical trauma, factors associated with the rise in catecholamines in this post-procedural period. Fundao et al., in their study, showed that these patients represented 10.4% of the cases.<xref ref-type="bibr" rid="B8">8</xref></p>
			<p>Many entities around the world have created diagnostic criteria to recognize this syndrome.<xref ref-type="bibr" rid="B6">6</xref> In an attempt to define a consensus, a new group of diagnostic criteria (InterTAK criteria) was proposed by specialists in 2018, including:</p>
			<list list-type="bullet">
				<list-item>
					<p>The patients show a transitory left ventricular dysfunction (hypokinesia, akinesia, or dyskinesia), appearing with an apical or medial ventricular ballooning, basal, or focal abnormalities of wall movement. The involvement of the right ventricle may appear;</p>
				</list-item>
				<list-item>
					<p>An emotional, physical, or combined trigger can precede the Takotsubo Syndrome event, but this is not a mandatory factor;</p>
				</list-item>
				<list-item>
					<p>New abnormalities in the ECG are present (ST-segment elevation, ST-segment depression, T-wave inversion, and QTc extension); however, there are rare cases with no alterations in the ECG.</p>
				</list-item>
			</list>
			<p>The InterTAK criteria mention the occurrence of the syndrome in the postoperative period of heart surgery, that is, they do not confirm nor exclude the diagnosis of the Takotsubo Syndrome in this case; however, with the increase in the number of cases reported in the literature, these criteria have been used for diagnoses in these groups of patients.<xref ref-type="bibr" rid="B8">8</xref> Findings from Fundao et al. suggest that, for future updates of the diagnostic criteria, one should consider the inclusion of heart surgery as a trigger for Takotsubo Syndrome.<xref ref-type="bibr" rid="B8">8</xref></p>
			<p>The differential diagnosis should mainly include acute coronary syndromes, since cardiomyopathies caused by stress overlap with acute coronary syndromes in their clinical and electrocardiographic manifestations.<xref ref-type="bibr" rid="B3">3</xref> In this scenario, in which the patient develops a cardiogenic shock in the postoperative period of heart surgery with acute alterations in the electrocardiogram and a left ventricle dysfunction with an anomalous movement of the wall, the first possibility to be discarded is coronary disease.<xref ref-type="bibr" rid="B9">9</xref></p>
			<p>In myocardiopathy caused by stress, the left ventricular function returns to normal within a few weeks; however, some complications can occur before fully recovering the ventricular systolic function. The main complications include: cardiogenic shock, obstruction of the left ventricular outflow, arrhythmias, systemic thromboembolism, and intramyocardial rupture.<xref ref-type="bibr" rid="B3">3</xref></p>
			<p>Treatment of the Takotsubo Syndrome is controversial, since no randomized prospective clinical trials have been conducted on the theme. Thus, the treatments are based on clinical experience and the consensus of specialists. One of the main questions in the treatment is to determine the presence or absence of the obstruction of the left ventricular outflow, which can change the therapeutic approach. The detection of the obstruction of left ventricular outflow is generally achieved through echocardiography or hemodynamic measures during the cardiac catheterization.<xref ref-type="bibr" rid="B2">2</xref> In the case reported here, the obstruction of the left ventricular outflow was discarded, and treatment using inotropics was implemented. Another possibility would be the temporary use of the intra-aortic balloon to minimize the use of inotropics, which was not necessary in the present case.</p>
		</sec>
		<sec sec-type="conclusions">
			<title>Conclusion</title>
			<p>The Takotsubo Syndrome should be remembered as a rare complication in the context of cardiogenic shock during the postoperative period of cardiovascular surgery. This syndrome stems from conditions directly related to stress factors, with an abrupt rise in catecholamine levels. The characteristic echocardiographic standard, associated with the clinical scenario presented above, can aid in the diagnostic definition.</p>
			<p>There are still many aspects concerning Takotsubo Syndrome that are still not fully understood. Clinical research is necessary to evaluate the treatment strategies in the acute stage and their impacts upon the patients’ recovery of their ventricular function.</p>

				<title>Introdução</title>
				<p>A Síndrome de Takotsubo, também conhecida como “síndrome do coração partido”, é caracterizada por uma disfunção ventricular esquerda grave e transitória, com presença de alterações da contração dos segmentos basal, médio ou apical das paredes no ventrículo esquerdo.<xref ref-type="bibr" rid="B1">1</xref> A síndrome pode ser causada por um gatilho emocional, físico, ou por uma combinação dos dois, porém esse não é um critério obrigatório para o seu diagnóstico.<xref ref-type="bibr" rid="B2">2</xref></p>
				<p>Neste artigo, descrevemos um caso de Síndrome de Takotsubo após cirurgia de troca valvar mitral por prótese mecânica.</p>
			</sec>
			<sec sec-type="cases">
				<title>Apresentação do caso</title>
				<p>Paciente de 56 anos, do sexo feminino, com antecedente de tabagismo, fibrilação atrial, artrite reumatoide e estenose mitral, foi submetida à valvoplastia mitral percutânea no ano de 2007, em uso de varfarina, atenolol, digoxina, amiodarona, prednisona (intermitente), hidroxicloroquina e leflunomida. Foi admitida de forma eletiva para realização de cirurgia de troca valvar mitral.</p>
				<p>O ecocardiograma transtorácico demonstrou uma fração de ejeção de 64%, com aumento do átrio esquerdo (volume indexado de 122 ml/m<xref ref-type="bibr" rid="B2">2</xref>) e hipertrofia ventricular esquerda excêntrica. A valva mitral apresenta cúspides espessadas com folheto posterior fixo e anterior com abertura em cúpula. Ao Doppler, observa-se refluxo importante com gradiente máximo e médio de 14 e 4 mmHg respectivamente. A planimetria indicou área valvar de 1,9 cm<xref ref-type="bibr" rid="B2">2</xref>. A cinecoronariografia realizada previamente à cirurgia demonstrou obstrução de 40% em artéria descendente anterior (<xref ref-type="fig" rid="f01002">Figura 1</xref>).</p>
				<p>

				</p>
				<p>A paciente foi submetida à cirurgia de troca valvar mitral por prótese mecânica e fechamento de aurícula atrial esquerda, sem intercorrências. No segundo dia de pós-operatório, evoluiu com edema agudo de pulmão hipertensivo e fibrilação atrial de alta resposta ventricular, sendo introduzido o tratamento com amiodarona, nitroglicerina e furosemida endovenosas. O eletrocardiograma apresentou inversão de onda T em derivações V2-V4 (<xref ref-type="fig" rid="f02002">Figura 2</xref>). Foi realizado ecocardiograma transtorácico que apresentou hipercontratilidade dos segmentos basais e acinesia dos segmentos médio, apicais e apex (padrão sugestivo de cardiomiopatia de Takotsubo), disfunção sistólica de ventrículo esquerdo de grau importante (fração de ejeção de ventrículo esquerdo = 22% pelo método Simpson). A prótese mecânica mitral apresentava função normal, sem obstrução da via de saída do ventrículo esquerdo. (<xref ref-type="fig" rid="f03002">Figura 3</xref> e <xref ref-type="fig" rid="f04002">4</xref>, <xref ref-type="fig" rid="m01002">Vídeos 1</xref> e <xref ref-type="fig" rid="m02002">2</xref>). Por suspeita de complicação coronariana secundária à cirurgia cardíaca, foi realizada a dosagem de troponina, que apresentou o valor de 3.305 ng/L (referência: &lt; 11 ng/L). No entanto, a anatomia coronariana do paciente era conhecida, desta forma, após decisão da equipe clínica-cirúrgica face aos exames de imagem (padrão ecocardiográfico) e ao contexto cirúrgico apresentado, optou-se por manejo clínico.</p>

				<p>Devido à piora do quadro, a paciente foi submetida à intubação orotraqueal e foi iniciada a terapia com noradrenalina (0,25 mcg/kg/min) e dobutamina (8 mcg/kg/min) no contexto de choque cardiogênico. Após 96 horas, paciente evoluiu com melhora do estado hemodinâmico, levando à suspensão progressiva das medidas de suporte. O ecocardiograma demonstrou recuperação da função sistólica do ventrículo esquerdo, com fração de ejeção de ventrículo esquerdo de 61%, com movimento anômalo do septo interventricular e contratilidade miocárdica preservados nos demais segmentos do ventrículo esquerdo. A prótese mecânica estava em posição mitral, com boa excursão de seus elementos móveis.</p>
				<p>Após excelente evolução clínica, a paciente recebeu alta da UTI, seguindo em acompanhamento ambulatorial.</p>

				<title>Discussão</title>
				<p>A Síndrome de Takotsubo tem sido descrita em alguns casos de pacientes submetidos a cirurgias (ginecológicas, urológicas e abdominais).<xref ref-type="bibr" rid="B3">3</xref> Os procedimentos cardiovasculares são também relatados como desencadeantes de Síndrome de Takotsubo, incluindo teste de estresse com dobutamina, pericardiocentese, cardioversão de fibrilação atrial, inserção de marcapasso, teste eletrofisiológico e ablação e substituição valvar cirúrgica e transcateter.<xref ref-type="bibr" rid="B4">4</xref></p>
				<p>A cardiomiopatia de Takotsubo no período perioperatório é cada vez mais reconhecida, mas sua associação com a cirurgia cardíaca é, em sua maioria, pouco conhecida e não descrita de forma frequente.<xref ref-type="bibr" rid="B5">5</xref> Não há dados suficientes na literatura que embasem a hipótese de prevalência de um procedimento cardiotorácico ou outro acerca do aumentado risco de desenvolvimento da síndrome, porém, é notório o aumento do número de casos descritos de desenvolvimento de Síndrome de Takotsubo pós-cirurgia cardíaca.<xref ref-type="bibr" rid="B5">5</xref></p>
				<p>Os possíveis mecanismos associados à sua ocorrência nos pacientes submetidos à cirurgia cardíaca são provavelmente múltiplos, tais como, vasoespasmo coronariano (isquemia epicárdica e microvascular), toxicidade mediada por catecolamina e ativação simpática excessiva.<xref ref-type="bibr" rid="B6">6</xref></p>
				<p>Lyon et al.,<xref ref-type="bibr" rid="B7">7</xref> em revisão dos mecanismos fisiopatológicos envolvidos na gênese dessa síndrome, destacam a ativação simpática intensa, bem como os níveis elevados de catecolaminas, como gatilhos centrais. Tais achados estão relacionados à disfunção micro e macrovascular coronariana e, por conseguinte, à disfunção dos cardiomiócitos e suas respectivas consequências evidenciadas na apresentação da síndrome.</p>
				<p>Assim, o desenvolvimento de anormalidades eletrocardiográficas, disfunção circunferencial da contração ventricular, sinais/sintomas clínicos (dor precordial e/ou hipotensão/choque circulatório) no pós-operatório de cirurgia cardiovascular são possíveis manifestações da Síndrome de Takotsubo, provavelmente causadas pela circulação extracorpórea e trauma cirúrgico, fatores associados à elevação de catecolaminas nesse período pós-procedimento. Fundão et al. em seu estudo demonstraram que esses pacientes representaram 10,4% dos casos.<xref ref-type="bibr" rid="B8">8</xref></p>
				<p>Várias entidades ao redor do mundo criaram critérios diagnósticos para o reconhecimento da síndrome.<xref ref-type="bibr" rid="B6">6</xref> Na tentativa de definir um consenso, um novo conjunto de critérios diagnósticos (critérios InterTAK) foi proposto por especialistas no ano de 2018, entres os quais destacam-se:</p>
				<list list-type="bullet">
					<list-item>
						<p>Os pacientes apresentam disfunção ventricular esquerda transitória (hipocinesia, acinesia ou discinesia) apresentando-se com balonamento apical ou medioventricular, basal, ou anormalidades focais do movimento da parede. O envolvimento do ventrículo direito pode estar presente;</p>
					</list-item>
					<list-item>
						<p>Um gatilho emocional, físico ou combinado pode preceder o evento da síndrome de Takotsubo, mas isso não é obrigatório;</p>
					</list-item>
					<list-item>
						<p>Novas anormalidades no ECG estão presentes (elevação do segmento ST, depressão do segmento ST, inversão da onda T e prolongamento do QTc); no entanto, existem casos raros sem qualquer alteração no ECG;</p>
					</list-item>

				<p>Os critérios do InterTAK não mencionam a ocorrência da síndrome no pós-operatório de cirurgia cardíaca, ou seja, não confirmam ou excluem o diagnóstico de Síndrome de Takotsubo nesse caso; no entanto, com o aumento do número de casos relatados na literatura, têm-se utilizado tais critérios para o diagnóstico nesses grupos de pacientes.<xref ref-type="bibr" rid="B8">8</xref> Fundão et al. sugerem em seu estudo que, para as próximas atualizações dos critérios diagnósticos, deve ser considerada a inclusão da cirurgia cardíaca como gatilho para Síndrome de Takotsubo.<xref ref-type="bibr" rid="B8">8</xref></p>
				<p>O diagnóstico diferencial deve incluir, principalmente, síndromes coronarianas agudas, pois a cardiomiopatia de estresse se sobrepõe a síndromes coronarianas agudas em sua apresentação clínica, bem como alterações eletrocardiográficas<xref ref-type="bibr" rid="B3">3</xref> Em situações como essa, em que o paciente desenvolve choque cardiogênico no pós-operatório de cirurgia cardíaca com alterações agudas do eletrocardiograma e disfunção do ventrículo esquerdo com movimento anômalo de parede, a primeira possibilidade a descartar é a doença coronariana.<xref ref-type="bibr" rid="B9">9</xref></p>
				<p>Na miocardiopatia por estresse, a função ventricular esquerda retorna ao normal dentro de poucas semanas; no entanto, algumas complicações podem ocorrer antes da recuperação da função sistólica ventricular. As principais complicações são: choque cardiogênico, obstrução da via de saída do ventrículo esquerdo, arritmias, tromboembolismo sistêmico e rotura intramiocárdica.<xref ref-type="bibr" rid="B3">3</xref></p>
				<p>O tratamento da Síndrome de Takotsubo é controverso, pois não foram realizados ensaios clínicos prospectivos randomizados sobre o tema. Assim, os tratamentos são baseados na experiência clínica e no consenso de especialistas. Uma das principais questões no tratamento é determinar a presença ou ausência de obstrução da via de saída do ventrículo esquerdo, a qual altera a abordagem terapêutica. A detecção de obstrução da via de saída do ventrículo esquerdo é geralmente feita por ecocardiografia ou medidas hemodinâmicas durante o cateterismo cardíaco.<xref ref-type="bibr" rid="B2">2</xref> No caso aqui relatado, a obstrução de via de saída do ventrículo esquerdo foi descartada, sendo realizado tratamento com uso de inotrópicos. Outra possibilidade seria o uso temporário do balão intra-aórtico para minimizar o uso de inotrópicos, não necessário no presente caso.</p>

				<title>Conclusão</title>
				<p>A Síndrome de Takotsubo deve ser lembrada como rara complicação no contexto de choque cardiogênico em pós-operatório de cirurgia cardiovascular. Constitui condição diretamente relacionada a fatores de estresse com elevação abrupta dos níveis de catecolamina. O padrão ecocardiográfico característico, associado ao quadro clínico apresentado, auxilia na definição diagnóstica.</p>
				<p>Persistem muitos aspectos acerca da síndrome de Takotsubo ainda não compreendidos. Pesquisas clínicas são necessárias a fim de avaliar estratégias de tratamento na fase aguda e seus reflexos na recuperação da função ventricular dos pacientes.</p>
