				<title>Abstract</title>
				<p>Takayasu’s Arteritis (TA) is a disease of multiple presentation, which can involve many different systems and mainly affects women in the first decades of life. The present case reports on a 19-year-old patient with TA, with a diagnosis of ascending aortic aneurysm and severe coronary artery disease in the right coronary artery (RCA) and left coronary trunk (LCT), who underwent an implant of a supracoronary Dacron graft in the ascending aorta and myocardial revascularization with a double internal thoracic artery. The patient evolved satisfactorily in post-op and is today undergoing clinical follow-up in our medical services.</p>

			<title>Introduction</title>
			<p>Takayasu’s Arteritis (TA) is a rare disease,<xref ref-type="bibr" rid="B1">1</xref> described for this first time in 1908 by Mikito Takayasu as a case of retinal vasculitis with the absence of a pulse in a young adult.<xref ref-type="bibr" rid="B2">2</xref> In an overall perspective, this disease is more common in women (80-90% of the cases), between the 2nd and 4th decades of life.<xref ref-type="bibr" rid="B1">1</xref> Its incidence varies and, according to statistical data from Japan, Europe, and the USA, around 1-3 new cases/one million inhabitants, are diagnosed each year.<xref ref-type="bibr" rid="B3">3</xref></p>
			<p>TA is defined as a progressive and granulomatous chronic inflammatory disease of the large blood vessels (aorta and its main supra-aortic branches, as well as the mid-sized pulmonary and coronary arteries), characterized by stenosis, occlusion, and/or dilation/aneurysm.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B5">5</xref> Of unknown etiology and poorly understood pathogenesis, it is presumed that genetic factors and infectious agents can play key roles in the origin of the disease.<xref ref-type="bibr" rid="B6">6</xref></p>
			<p>There are no specific laboratory exams for the diagnosis and follow-up of the disease’s activity. Angiotomography (Angio-CT) and the magnetic resonance angiography (MRA) represent the gold standards for the diagnosis of TA.<xref ref-type="bibr" rid="B7">7</xref>,<xref ref-type="bibr" rid="B8">8</xref> According to angiographic findings, TA has been classified in 6 types of alterations, based on the affected region, which is useful for surgical planning.<xref ref-type="bibr" rid="B9">9</xref></p>
			<p>The clinical treatment of this entity can be found in the guidelines, whose objective is the control of active inflammation and the prevention of new vascular damage. However, when in the presence of critical stenotic lesions, dilation, and/or aneurysm, percutaneous angioplasty and surgical treatment are recommended, on a case by case basis.<xref ref-type="bibr" rid="B8">8</xref> In this context, this work aims to describe a rare clinical manifestation of TA</p>

			<title>Case report</title>
			<p>Our study reports on a 19-year-old female patient, weighing 57 kg, with TA diagnosed at 3 years of age, who was medicated with applications of pulse therapy and methotrexate until 12 years of age, at which time she ceased to continue her medical follow-up until she was 18 years of age. The patient resumed her follow-up treatment one year ago at the same medical institution, at which time she was diagnosed with ectasia of the ascending aorta, with 35 x 34 mm in the transthoracic echocardiogram (TEE). The patient was then sent to the medical reference service.</p>
			<p>She underwent an MRA, which revealed a thickening of the artic wall and dilation of the ascending aorta of 37x35 mm (<xref ref-type="fig" rid="f01">Figure 1</xref>). Six months later, she returned to the outpatient clinic, reporting a medical condition of typical chest pain lasting 30 minutes, followed by syncope, but she did not go to the hospital on that occasion. An Angio-CT of the aorta and coronary arteries was requested, which revealed an aneurysm of the ascending aorta, 46 x 44 mm, with signs of circumferential parietal thickening and parietal calcification along its entire extension, as well as severe coronary artery involvement by extrinsic compression related to the parietal thickening of the aorta (<xref ref-type="fig" rid="f02">Figure 2</xref>).</p>

			</p>
			<p>The coronary cineangiography showed severe lesions in the right coronary artery (RCA), 80% proximal and 80% proximal of the left coronary trunk (LCT). The case was discussed with the aorta Heart Team, and it was decided that the patient should be hospitalized for surgical treatment. The patient was asymptomatic at the time of hospitalization.</p>
			<p>Upon physical exam, a systolic murmur appeared at a II/IV aortic focus. Laboratory data presented no significant alterations. During the pre-operative period, the patient’s routine medication was maintained. Given the evidence of disease progression, the patient underwent a supracoronary Dacron graft implant in the ascending aorta and myocardial revascularization with double internal thoracic artery (ATIE-DA and ATID-CD, in Portuguese) (<xref ref-type="fig" rid="f02">Figure 2</xref>). With no complications during hospitalization, the patient was released on the 8th post-operative day. She is currently undergoing optimized clinical treatment and is taking medicine to control the base disease, as advised by the healthcare team.</p>

			<title>Discussion</title>
			<p>The description of this case refers to the severity of TA, as well as to its complexity in the diagnostic approach and treatment. TA is a rare disease, most commonly found in women before 50 years of age, but many studies also show a high prevalence at more advanced ages.<xref ref-type="bibr" rid="B10">10</xref></p>
			<p>The diagnosis is based on diagnostic criteria from EULAR/PRINTO/PRES.<xref ref-type="bibr" rid="B11">11</xref> In this case, the clinical means of presentation was of coronary artery disease. Coronary artery involvement is uncommon, showing only a 9% incidence.<xref ref-type="bibr" rid="B12">12</xref> Although it is rare, myocardial ischemia is one of the main causes of death. The inflammatory process of the aorta has been identified as being responsible for these lesions, in which the occlusion of the ostium of the coronary arteries occurs more often, though there are many cases in the literature that describe coronary arteritis with no occlusion of the ostium.<xref ref-type="bibr" rid="B13">13</xref>,<xref ref-type="bibr" rid="B14">14</xref> Coronary artery involvement in this patient is similar to that described by Endo et al., in which 24 patients presented coronary artery stenosis, of whom 83.3% were women.<xref ref-type="bibr" rid="B12">12</xref></p>
			<p>According to angiographic findings, the TA was classified in six types of alterations based on the affected region, which is useful for surgical planning.<xref ref-type="bibr" rid="B7">7</xref>,<xref ref-type="bibr" rid="B8">8</xref> The decision of when to use an endovascular approach or open surgery can be influenced by a series of factors.<xref ref-type="bibr" rid="B15">15</xref> Percutaneous coronary interventions (PCI) have been used in the initial treatment of the obstructive lesions, presenting uncertain results and a long-term survival that is still not fully understood.<xref ref-type="bibr" rid="B16">16</xref></p>
			<p>Due to a serious chance of intra-stent restenosis, cardiac magnetic resonance (CMR) is the procedure of choice to treat coronary artery lesions.<xref ref-type="bibr" rid="B14">14</xref> In a retrospective case series involving 106 Japanese patients, Miyata et al.<xref ref-type="bibr" rid="B17">17</xref> observed a 15-year increase in the survival rate, rising from 43% (among those treated with medical therapy) to 67.5% after the addition of surgical intervention.</p>
			<p>Yang et al.,<xref ref-type="bibr" rid="B18">18</xref> in an analysis of 31 patients with TA who underwent revascularization by CMR (12) and PCI (19), in an average follow-up of 101 months, observed higher restenosis rates in the percutaneous group. The inflammatory process can affect different parts of the coronary artery, but with a greater prevalence of ostial/proximal lesions, in accordance with that reported in the present study. The authors also highlight that the myocardial ischemia resulting from coronary artery involvement constitutes the main cause of death among TA patients. In this sense, the importance of studying the coronary arteries of these patients when there is a clinical suspicion is crucial.</p>
			<p>Huang et al.,<xref ref-type="bibr" rid="B19">19</xref>in a study involving 90 patients comparing medicine vs. revascularization treatments in TA patients (CMR and PCI) were unable to identify any difference in mortality due to cardiovascular causes among the groups. Nevertheless, in the analysis of the subgroup through a revascularization strategy, the findings corroborated with those reported by Yang et al.<xref ref-type="bibr" rid="B18">18</xref> with higher restenosis rates in the percutaneous approach. Moreover, in this study, heart failure was an independent predictor of mortality.</p>
			<p>Modalities of revascularization were also the focus of the study published by Wang et al.,<xref ref-type="bibr" rid="B20">20</xref> in an analysis of 46 patients with TA and coronary artery involvement. The authors found that the MACE rates (all-causes mortality, myocardial infarction, and revascularization) were greater in the group submitted to PCI. The multivariate analysis for the MACE outcome viewed the disease activity and the type of revascularization (CMR vs. PCI) as predictive factors. Such data reinforce the individualized indication and the importance of the Heart Team in these patients’ medical evaluations.</p>
		</sec>
		<sec sec-type="conclusions">
			<title>Conclusion</title>
			<p>Coronary artery lesions vary from patient to patient, especially proximal involvement, and therefore careful planning in all stages of the treatment through revascularization, be it percutaneous or surgical, is of utmost importance. The data point to higher restenosis rates after the percutaneous approach, with the CMR being the preferred method when no contraindications were presented, such as the availability of grafts and the possibility of using the internal thoracic artery.</p>
			<p>The adequate suppression of the inflammatory activity from the pre-operative stage and the choice of technique to be used guarantee better end results. Thus, the evaluation of the patient’s stage of the disease is essential, given that it is a factor associated with higher MACE rates, as well as the shared decision on the case. The rigorous and specialized clinical follow-up in a reference center should be encouraged.</p>

				<title>Resumo</title>
				<p>A Arterite de Takayasu (AT) constitui doença de múltipla apresentação, podendo acometer diversos sistemas. Acomete principalmente mulheres nas primeiras décadas de vida. Reportamos um caso de paciente com 19 anos de idade com AT, com diagnóstico de aneurisma de aorta ascendente e doença arterial coronariana grave em artéria coronária direita (ACD) e tronco de coronária esquerda (TCE), sendo submetida a implante de enxerto de Dacron supracoronariano em aorta ascendente e revascularização miocárdica com dupla artéria torácica interna. Evoluiu de forma satisfatória no pós-operatório, seguindo em acompanhamento clínico em nosso serviço.</p>

				<title>Introdução</title>
				<p>Arterite de Takayasu (AT) é uma doença rara,<xref ref-type="bibr" rid="B1">1</xref> descrita pela primeira vez em 1908 por Mikito Takayasu como um caso de vasculite retiniana com ausência de pulso em uma jovem.<xref ref-type="bibr" rid="B2">2</xref> De distribuição mundial, ocorre mais frequentemente em mulheres, em 80 a 90% dos casos, entre a 2ª e 4ª décadas de vida.<xref ref-type="bibr" rid="B1">1</xref> Sua incidência varia e, de acordo com dados estatísticos do Japão, Europa e EUA, são diagnosticados cerca de 1-3 novos casos por ano, por milhão de habitante.<xref ref-type="bibr" rid="B3">3</xref></p>
				<p>É definida como uma doença inflamatória crônica progressiva e granulomatosa dos grandes vasos (aorta e seus principais ramos supra-aórticos, bem como as artérias pulmonares e coronárias de médio porte), caracterizada por estenose, oclusão ou dilatação/aneurisma.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B5">5</xref> De etiologia desconhecida e a patogênese mal compreendida, presume-se que fatores genéticos ou agentes infecciosos podem ter algum papel na origem da doença.<xref ref-type="bibr" rid="B6">6</xref></p>
				<p>Não existem exames laboratoriais específicos para o seu diagnóstico e o acompanhamento da atividade da doença. A angiotomografia e angiorresonância constituem o padrão ouro para o seu diagnóstico.<xref ref-type="bibr" rid="B7">7</xref>,<xref ref-type="bibr" rid="B8">8</xref> De acordo com os achados angiográficos, a AT foi classificada em 6 tipos de alterações com base na região de acometimento, o que é útil para o planejamento cirúrgico.<xref ref-type="bibr" rid="B9">9</xref></p>
				<p>O tratamento clínico dessa entidade já se encontra devidamente descrito nas diretrizes, cujo objetivo é o controle da inflamação ativa e a prevenção de novos danos vasculares. Porém, quando em presença de lesões estenóticas críticas, dilatação e/ou aneurisma importante, a angioplastia percutânea e o tratamento cirúrgico estão indicados conforme cada caso.<xref ref-type="bibr" rid="B8">8</xref> Nesse contexto, este trabalho tem como objetivo descrever uma apresentação clínica rara da AT.</p>

					<title>Apresentação do caso</title>
					<p>Apresentamos o caso de uma paciente feminina de 19 anos de idade, 57 kg, com AT diagnosticada aos 3 anos, sendo medicada com cursos de pulsoterapia e metotrexato até os 12 anos, momento em que perdeu seu seguimento médico até os 18 anos. Há um ano a paciente retomou seguimento na mesma instituição, sendo então diagnosticada ectasia de aorta ascendente com 35x34 mm ao ecocardiograma transtorácico, e sendo a paciente encaminhada a serviço de referência.</p>
					<p>Realizou angio-RNM que revelou espessamento da parede aórtica, dilatação da aorta ascendente 37x35 mm (<xref ref-type="fig" rid="f01002">Figura 1</xref>). Seis meses depois, retornou ao ambulatório, referindo quadro de dor torácica típica com duração de 30 minutos seguida de síncope, porém não procurou hospital na ocasião. Solicitou-se Angio-TC de aorta e coronárias que evidenciaram aneurisma de aorta ascendente 46x44 mm, com sinais de espessamento parietal circunferencial e calcificação parietal em toda sua extensão e comprometimento coronariano importante por compressão extrínseca relacionada a espessamento parietal da aorta (<xref ref-type="fig" rid="f02002">Figura 2</xref>).</p>

					</p>
					<p>A cineangiocoronariografia evidenciou lesões graves em artéria coronária direita (ACD) 80% proximal e 80% proximal de tronco de coronária esquerda (TCE). Discutido o caso em Heart Team de aorta, decidiu-se internação para abordagem cirúrgica. A paciente estava assintomática no momento da sua admissão.</p>
					<p>Ao exame físico, apresentava sopro sistólico em foco aórtico II/IV. Dados laboratoriais não apresentavam alterações significativas. Durante o pré-operatório, foi mantida a medicação em uso habitual. Perante as evidências de progressão da doença, foi submetida a implante de enxerto de Dacron supracoronariano em aorta ascendente e revascularização miocárdica com dupla artéria torácica interna (ATIE-DA e ATID-CD) (<xref ref-type="fig" rid="f02002">Figura 2</xref>). Sem intercorrências durante a internação, recebeu alta no 8º dia de pós-operatório. Atualmente mantém-se em tratamento clínico otimizado, assim como em uso de medicações para o controle da doença de base orientado pela equipe assistente.</p>

				<title>Discussão</title>
				<p>A descrição desse caso refere-se à gravidade da AT, bem como à sua complexidade na abordagem diagnóstica e tratamento. Trata-se de uma doença rara, mais frequente em mulheres antes dos 50 anos, mas vários estudos mostram uma elevada prevalência em idades superiores.<xref ref-type="bibr" rid="B10">10</xref></p>
				<p>O diagnóstico baseia-se nos critérios diagnósticos da EULAR/PRINTO/PRES.<xref ref-type="bibr" rid="B11">11</xref> Neste caso, a forma de apresentação clínica foi a doença coronária. O acometimento coronário é incomum e apresenta incidência de 9%.<xref ref-type="bibr" rid="B12">12</xref> Embora seja rara, a isquemia miocárdica é uma das principais causas de morte. O processo inflamatório da aorta tem sido identificado como responsável por essas lesões em que a oclusão dos óstios das coronárias ocorre com maior frequência, embora existam vários casos descritos de arterite coronária sem oclusão do óstio.<xref ref-type="bibr" rid="B13">13</xref>,<xref ref-type="bibr" rid="B14">14</xref> O envolvimento das coronárias nesta paciente é similar ao descrito por Endo et al., em que 24 pacientes tiveram estenose coronária, sendo que 83,3% destes eram mulheres.<xref ref-type="bibr" rid="B12">12</xref></p>
				<p>De acordo com os achados angiográficos, a AT foi classificada em 6 tipos de alterações com base na região de acometimento, o que é útil para o planejamento cirúrgico.<xref ref-type="bibr" rid="B7">7</xref>,<xref ref-type="bibr" rid="B8">8</xref> A decisão de quando usar uma abordagem endovascular ou cirurgia aberta pode ser influenciada por uma série de fatores.<xref ref-type="bibr" rid="B15">15</xref> Intervenções coronárias percutâneas (ICP) têm sido utilizadas no tratamento inicial das lesões obstrutivas, apresentando resultado incerto e sobrevida em longo prazo ainda desconhecida.<xref ref-type="bibr" rid="B16">16</xref></p>
				<p>Devido à grande chance de reestenoses intra-stents, a CRM é o procedimento de escolha para o tratamento das lesões coronarianas.<xref ref-type="bibr" rid="B14">14</xref> Em uma série de casos retrospectivos envolvendo 106 pacientes japoneses, Miyata et al.,<xref ref-type="bibr" rid="B17">17</xref> observaram um aumento na taxa de sobrevida em 15 anos de 43% (naqueles tratados com terapia médica) para 67,5% após a adição de cirurgia intervenção.</p>
				<p>Yang et al.<xref ref-type="bibr" rid="B18">18</xref> em análise de 31 pacientes com AT submetidos à revascularização por CRM (12) e ICP (19), em seguimento médio de 101 meses, observaram maiores taxas de reestenoses no grupo percutâneo. O processo inflamatório pode afetar diferentes partes da artéria coronária; no entanto, com maior prevalência de lesões ostiais/proximais conforme evidenciado no presente estudo. Os autores ainda destacam que a isquemia miocárdica resultante do comprometimento coronariano constitui a principal causa de morte em pacientes com AT. Dessa forma, a importância do estudo coronariano desses pacientes na suspeita clínica é fundamental.</p>
				<p>Huang et al.,<xref ref-type="bibr" rid="B19">19</xref>em estudo envolvendo 90 pacientes comparando tratamento medicamentoso vs. revascularização em pacientes com AT (CRM e ICP), não identificaram diferença de mortalidade por causa cardiovascular entre os grupos. Contudo, na análise de subgrupo por estratégia de revascularização, seus achados corroboraram aqueles de Yang et al.<xref ref-type="bibr" rid="B18">18</xref> com maiores taxas de reestenoses na abordagem percutânea. Ademais, nesse estudo a insuficiência cardíaca foi um preditor independente de mortalidade.</p>
				<p>Modalidades de revascularização também foram foco de estudo publicado por Wang et al.,<xref ref-type="bibr" rid="B20">20</xref> em análise de 46 pacientes com AT e comprometimento coronariano. Os autores identificaram que as taxas de MACE (mortalidade por todas as causas, infarto do miocárdio e revascularização) foram superiores no grupo submetido à ICP. A análise multivariada para o desfecho de MACE observou como fatores preditivos a atividade da doença e o tipo de revascularização (CRM vs. ICP). Tais dados reforçam a indicação individualizada e a importância do Heart Team na avaliação desses pacientes.</p>
			</sec>
			<sec sec-type="conclusions">
				<title>Conclusão</title>
				<p>As lesões coronarianas variam em cada paciente, especialmente o comprometimento proximal e, diante disso, o planejamento cuidadoso em todas as etapas do tratamento mediante revascularização, seja percutânea ou cirúrgica, é de extrema importância. Dados apontam para maiores taxas de reestenoses após abordagem percutânea, sendo a CRM o método de preferência na ausência de contraindicações, como a disponibilidade de enxertos e a possibilidade do uso da artéria torácica interna.</p>
				<p>A adequada supressão da atividade inflamatória desde a fase pré-operatória e a escolha da técnica a ser utilizada garantem melhores resultados tardios. Assim, é fundamental a avaliação do estágio da doença do paciente, visto ser um fator associado a maiores taxas de MACE, bem como a decisão compartilhada do caso. O acompanhamento clínico rigoroso e especializado em centro de referência deve ser estimulado.</p>
