			<title>Introduction</title>
			<p>The superior vena cava syndrome (SVCS) is caused by the obstruction or severe occlusion of the superior vena cava (SVC) and can result in significant morbidity or mortality.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref> Malignancy is the most common cause of this obstruction, corresponding to approximately 70% of the cases.<xref ref-type="bibr" rid="B3">3</xref> However, more recently, the incidence of SVCS related to devices, such as central venous catheters and electrodes of a pacemaker or defibrillator, have increased.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B5">5</xref></p>
			<p>These devices, together with states of hypercoagulability, can precipitate the formation of thrombi.<xref ref-type="bibr" rid="B4">4</xref> In extreme situations, in the context of bacteremia, these can lead to the formation of a complex thrombus-vegetation association.</p>
			<p>The traditional clinical findings include: facial, periorbital, cervical, and upper limb edemas, followed by venous dilations of the anterior thoracic wall, illustrating collateral circulation.<xref ref-type="bibr" rid="B1">1</xref>-<xref ref-type="bibr" rid="B3">3</xref> Although it generally does not represent an imminent risk, SVCS is often associated with symptoms of high morbidity-mortality, such as dyspnea, dysphagia, and even intracranial hypertension, which can result in a coma.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref></p>
			<p>To diagnose this condition, the transesophageal echocardiogram (TEE) plays a complementary role in a bedside multimodal image to demonstrate the presence of thrombus in SVC topography.<xref ref-type="bibr" rid="B6">6</xref> New tools, such as the real-time three-dimensional TEE (3D TEE), has enabled a greater quantification of the lesions in the right atrium and the SVC, with the risk of evolving into SVCS.</p>

			<title>Case Report</title>
			<p>A 52-year-old male patient, who was hypertensive, diabetic, and diagnosed with prior coronary artery disease and chronic dialytic kidney disease, underwent a kidney transplant in 2021, evolving with graft loss. The patient was hospitalized in February 2022 due to a fungal endocarditis and remained hospitalized during antifungal treatment. After six months, he developed a new medical condition of fever and chills associated with the use of the dialysis catheter and the poor functioning of this device, in addition to redness and facial edema, dysphagia, and dyspnea. The blood cultures showed the growth of S. epidermidis, and the patient had been taking vancomycin, staggered with teicoplanin and meropenem, and continued to use fluconazole. The two-dimensional (2D) TEE showed the presence of a large hyperechogenic image within the right atrium, protruding to the SVC, measuring approximately 40 x 08 mm (<xref ref-type="fig" rid="f01">Figure 1</xref>). A 3D technique was used, which allowed for a better characterization of the lesion and its extension, occupying approximately 67% of the circumferential diameter of the SVC, associated with the presence of a mobile filament image in its outer portions, which can correspond to a thrombus or to vegetation (<xref ref-type="fig" rid="f02">Figure 2</xref>). An angiotomography of the cervical and thoracic blood vessels was conducted, with signs of a partial thrombosis of the internal jugular veins, the left subclavian vein, the brachiocephalic veins, and the SVC. The dialysis catheter was moved by the vascular surgery team to the left femoral vein, and anticoagulation was begun.</p>

		<sec sec-type="discussion">
			<title>Discussion</title>
			<p>The incidence of SVCS related to devices has increased considerably due to the increased use of catheters, pacemakers, and defibrillators.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref> In a recent study, 28% of all SVCS were related to devices.<xref ref-type="bibr" rid="B3">3</xref> Venous thrombosis, which is normally clinically silent, occurred in nearly 30% of all patients using pacemaker electrodes.<xref ref-type="bibr" rid="B3">3</xref> However, the obstruction of the SVC is rare and observed in only 0.1% to 3.3% of all patients.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref> The stenosis in the SVC-atrial junction occurs due to the deposit of fibrin on the surface of the stimulating electrodes and their incorporation in a person’s reproductive organs, followed by the inflammation of the blood vessel wall, fibrosis, the formation of thrombosis, and stenosis.<xref ref-type="bibr" rid="B3">3</xref> The chronic mechanical irritation and reaction of the foreign body are the main mechanisms of the formation of thrombi and vegetations in this location. The extraction of devices and posterior re-implant also caused mechanical trauma and can be considered additional risk factors for the emergence of thrombi and venous occlusion.<xref ref-type="bibr" rid="B4">4</xref></p>
			<p>The diagnosis of this complication can be difficult. Although the computed tomography can show the location of the catheter, it is quite limited in revealing the structural details concerning the implanted device itself.<xref ref-type="bibr" rid="B5">5</xref> The proximal portion of the SCV can be viewed in the TEE through the subcostal window. However, the views are generally quite limited in many patients.<xref ref-type="bibr" rid="B6">6</xref> The TEE in this scenario is an excellent diagnostic approach as it is minimally invasive and has few contra-indications.<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B7">7</xref> The SVC can be easily observed, as demonstrated in our case, especially through the transesophageal acoustic window in a medium esophagus with an angulation of about 90° (bicaval incidence). The good spatial resolution of the echocardiographic images allows for an adequate definition of the catheter and associated pathology. In addition, follow-up using this imaging method is feasible, presents a low morbidity-mortality, and can guide additional treatment.<xref ref-type="bibr" rid="B8">8</xref></p>
			<p>A prior study evaluated the role of the TEE in the diagnosis of central venous thrombi associated with the implant of intracardiac devices (pacemaker electrodes), using the TEE and venography. After 06 months, the transesophageal method enabled the detection of most of the episodes of thrombosis in the intracardiac devices. By contrast, the venogram detected only thrombi in the subclavian vein and/or innominate vein. The main finding of this prospective study was that the thrombosis associated with the pacemaker electrode in the right atrium and with SVC is not uncommon six months after the implant, and that the 3D TEE was superior in the diagnosis of these thrombi when compared to the venogram with peripheral intravenous contrast and the TEE.<xref ref-type="bibr" rid="B4">4</xref></p>
			<p>The study of these emboligenic sources is essential, as it can reduce the rate of pulmonary embolism, the emergence of infectious endocarditis, and even rare cases of the SVCS due to the subtotal/total occlusion of this blood vessel, as seen in the present case report. In this context, the TEE is extremely useful, as it is a semi-invasive method, which is readily available, inexpensive, and capable of detecting thrombi in devices and/or long-term catheters, even in asymptomatic patients.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B5">5</xref></p>
			<p>The venogram by digital subtraction, by contrast, represents the gold standard in this syndrome, defining the severity of the obstruction and enabling the interventionist to develop the strategy for revascularization.<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B10">10</xref> However, the viewing of the superior right vena cava is still a challenge due to the acoustic shadow imposed by the ribs and the lungs. In this context, it is important to perform not only the fluoroscopy, but also the TEE, which has the power to evaluate the insertion of the SVC in the right atrium.<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B7">7</xref> The inadequate observation of the SVC junction – right atrium by the fluoroscopy enhances the accuracy of the TEE, making it an excellent ally in the visualization of this portion through the image obtained in the bicaval acoustic window, which is a quick, safe, and technically precise method for the differential diagnosis of SVCS.<xref ref-type="bibr" rid="B7">7</xref></p>
			<p>In the spectrum of the TEE, the use of the 3D TEE is an additional tool capable of bringing relevant information provided by an adequate spatial relationship.<xref ref-type="bibr" rid="B7">7</xref>,<xref ref-type="bibr" rid="B8">8</xref> In this sense, the 3D TEE allows for an improved evaluation of the SVC and its relation with the thrombus area, determining the occupation rate and the respective hemodynamic effects resulting from this obstruction, characterizing lesions with a greater risk of developing SVCS.<xref ref-type="bibr" rid="B7">7</xref>,<xref ref-type="bibr" rid="B8">8</xref></p>
		</sec>
		<sec sec-type="conclusions">
			<title>Conclusion</title>
			<p>The incidence of SVCS related to devices, such as central venous catheters and electrodes from pacemakers and defibrillators, have increased progressively. In this light, the echocardiogram, especially the transesophageal analysis, emerges as a quick, safe, and technically precise method to evaluate the presence of thrombi and/or vegetations that may well be related to SVCS, as seen in the present case report. The good spatial relation of the 3D TEE allows for an additional value in the topographical evaluation of the lesion in relation to the adjacent planes and a better evaluation of its dimensions.</p>

				<title>Introdução</title>
				<p>A síndrome de veia cava superior (SVCS) é causada pela obstrução ou oclusão severa da veia cava superior (VCS) e pode resultar em significativa morbidade e mortalidade.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref> A malignidade é a causa mais comum dessa obstrução, correspondendo a aproximadamente 70% dos casos.<xref ref-type="bibr" rid="B3">3</xref> Entretanto, recentemente, a incidência de SVCS relacionada a dispositivos, como cateteres venosos centrais e eletrodos de marcapasso ou desfibrilador, tem aumentado.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B5">5</xref></p>
				<p>Esses dispositivos juntamente com estados de hipercoagulabilidade podem precipitar a formação de trombos.<xref ref-type="bibr" rid="B4">4</xref> Em situações extremas, no contexto de bacteremia, pode ocorrer a formação de uma complexa associação de trombo-vegetação.</p>
				<p>Os achados clínicos classicamente descritos incluem: edemas faciais, periorbitários, cervicais e de membros superiores, seguidos de dilatações venosas da parede torácica anterior, caracterizando circulação colateral.<xref ref-type="bibr" rid="B1">1</xref>-<xref ref-type="bibr" rid="B3">3</xref> Embora geralmente não represente risco de morte iminente, a SVCS está frequentemente associada a sintomas com alta morbimortalidade como dispneia, disfagia e até hipertensão intracraniana, podendo resultar em coma.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref></p>
				<p>Para diagnosticar essa condição, a ecocardiografia transesofágica (ETE) tem papel complementar em imagem multimodal à beira leito para demonstrar a presença de trombo em topografia de VCS.<xref ref-type="bibr" rid="B6">6</xref> Novas ferramentas, como a ecocardiografia tridimensional em tempo real (ETE 3D), têm possibilitado melhor quantificação das lesões em átrio direito e VCS, com risco de evolução para SVCS.</p>

				<title>Relato de caso</title>
				<p>Paciente sexo masculino, 52 anos, hipertenso, diabético, portador de doença arterial coronariana prévia e doença renal crônica dialítica, foi submetido a transplante renal em 2021, evoluindo com perda do enxerto. Esteve internado em fevereiro de 2022, devido à endocardite fúngica e permaneceu hospitalizado durante tratamento antifúngico. Após 06 meses, evoluiu com novo quadro de febre e calafrios associado ao uso do cateter de diálise e mau funcionamento deste, além de rubor e edema facial, disfagia e dispneia. As hemoculturas com crescimento de S. epidermidis, e o paciente vinha em uso de vancomicina, escalonado para teicoplanina, meropenem, e mantido o uso do fluconazol. O ecocardiograma transesofágico bidimensional demonstrou a presença de extensa imagem hiperecogênica no interior do átrio direito, projetando-se para a VCS, medindo cerca de 40 x 08 mm (<xref ref-type="fig" rid="f01002">Figura 1</xref>). Foi realizada técnica tridimensional, a qual permitiu melhor caracterização da lesão, bem como sua extensão, ocupando aproximadamente 67% do diâmetro circunferencial da VCS, associada à presença de imagem filamentar móvel em sua extremidade, podendo corresponder a trombo ou à vegetação (<xref ref-type="fig" rid="f02002">Figura 2</xref>). Foi realizada angiotomografia de vasos cervicais e torácicos com sinais de trombose parcial das veias jugulares internas, veia subclávia esquerda, veias braquiocefálicas e VCS. Foi realizada a troca de cateter de diálise pela equipe da cirurgia vascular para veia femoral esquerda e foi iniciada a anticoagulação.</p>

			<sec sec-type="discussion">
				<title>Discussão</title>
				<p>A incidência da SVCS relacionada a dispositivos tem aumentado devido ao uso cada vez mais frequente de cateteres, marcapassos e desfibriladores.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref> Em um estudo recente, 28% de todas as SVCS foram relacionadas a dispositivos.<xref ref-type="bibr" rid="B3">3</xref> A trombose venosa clinicamente silenciosa pode ocorrer em até 30% dos pacientes com eletrodos de marcapasso.<xref ref-type="bibr" rid="B3">3</xref> No entanto, a obstrução da VCS é rara e observada em apenas 0,1% a 3,3% dos pacientes.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref> A estenose na junção VCS-atrial ocorre devido à deposição de fibrina na superfície dos eletrodos de estimulação e sua incorporação na íntima, seguida de inflamação da parede do vaso, fibrose, formação de trombo e estenose.<xref ref-type="bibr" rid="B3">3</xref> A irritação mecânica crônica e reação de corpo estranho são os principais mecanismos de formação de trombos e de vegetações nesse local. A extração de dispositivos e posterior reimplante também causam trauma mecânico e podem ser considerados fatores de risco adicionais para o surgimento de trombos e oclusão venosa.<xref ref-type="bibr" rid="B4">4</xref></p>
				<p>O diagnóstico dessa complicação pode ser, às vezes, difícil. Embora a tomografia computadorizada possa demonstrar a localização do cateter, ela é bastante limitada em demonstrar detalhes estruturais sobre o próprio dispositivo implantado.<xref ref-type="bibr" rid="B5">5</xref> A porção proximal da VCS pode ser visualizada na ecocardiografia transtorácica pela janela subcostal. Entretanto, as visualizações geralmente são bastante limitadas em muitos pacientes.<xref ref-type="bibr" rid="B6">6</xref> A ETE nesse cenário é uma excelente abordagem diagnóstica, por ser minimamente invasiva e com poucas contraindicações.<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B7">7</xref> A VCS costuma ser bem visualizada como demonstrado em nosso caso, especialmente pela janela acústica transesofágica em esôfago médio com angulação em torno de 90° (incidência bicaval). A boa resolução espacial das imagens ecocardiográficas permite a definição adequada do cateter e da patologia associada. Além disso, o acompanhamento por esse método de imagem é viável, apresenta baixa morbimortalidade e pode orientar o tratamento adicional.<xref ref-type="bibr" rid="B8">8</xref></p>
				<p>Estudo prévio avaliou o papel da ETE no diagnóstico de trombos venosos centrais associados a implante de dispositivos intracardíacos (eletrodos de marcapasso) utilizando ETE e venografia. . Após 06 meses, o método transesofágico permitiu detectar a maioria dos episódios de trombose nos dispositivos intracardíacos. Em contrapartida, a venografia detectou apenas trombos em veia subclávia e/ou inominada. O principal achado desse estudo prospectivo foi que a trombose associada ao eletrodo de marcapasso no átrio direito e à VCS não é infrequente seis meses após o implante, e que o ETE foi superior no diagnóstico desses trombos em comparação com a venografia com contraste intravenoso periférico e o ETT.<xref ref-type="bibr" rid="B4">4</xref></p>
				<p>A pesquisa dessas fontes emboligênicas é fundamental, pois pode reduzir a taxa de embolia pulmonar, o surgimento de endocardite infecciosa e até mesmo os raros casos de SVCS devido à oclusão subtotal/total desse vaso, como no nosso relato de caso descrito. Nesse contexto, a ETE é extremamente útil por ser um método semi-invasivo, relativamente disponível, de custo não elevado e capaz de detectar trombos em dispositivos e/ou cateteres de longa permanência, mesmo em pacientes assintomáticos.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B5">5</xref></p>
				<p>Por sua vez, a venografia por subtração digital representa o padrão-ouro nessa síndrome, definindo inclusive a gravidade da obstrução e permitindo ao intervencionista o desenvolvimento da estratégia de revascularização.<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B10">10</xref> No entanto, a visualização direta da VCS é um desafio, devido à sombra acústica imposta pelas costelas e pulmão, sendo nesse contexto importante realizar não só a fluoroscopia, mas também o ecocardiograma transesofágico, o qual tem poder de avaliar a inserção da VCS no átrio direito.<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B7">7</xref> Essa visualização inadequada da junção VCS - átrio direito pela fluoroscopia potencializa a acurácia do ecocardiograma transesofágico tornando-o excelente aliado na visualização dessa porção através de imagem obtida na janela acústica bicaval, sendo um método rápido, seguro e tecnicamente preciso para o diagnóstico diferencial da SVCS.<xref ref-type="bibr" rid="B7">7</xref></p>
				<p>No espectro da ETE, a utilização da modalidade tridimensional (ETE 3D) é uma ferramenta adicional capaz de trazer informações relevantes propiciadas pela sua adequada relação espacial.<xref ref-type="bibr" rid="B7">7</xref>,<xref ref-type="bibr" rid="B8">8</xref> Dessa forma, o ETE 3D possibilita a melhor avaliação da VCS e sua relação com a área do trombo, determinando a taxa de proporção da sua ocupação e os respectivos efeitos hemodinâmicos advindos dessa obstrução, caracterizando lesões com maior risco de desenvolvimento de SVCS.<xref ref-type="bibr" rid="B7">7</xref>,<xref ref-type="bibr" rid="B8">8</xref></p>
			</sec>
			<sec sec-type="conclusions">
				<title>Conclusão</title>
				<p>A incidência de SVCS relacionada a dispositivos, como cateteres venosos centrais e eletrodos de marcapasso ou desfibrilador, tem aumentado progressivamente. Diante desse cenário, a ecocardiografia, sobretudo a análise transesofágica, se constitui como um método rápido, seguro e tecnicamente preciso para avaliação da presença de trombos e/ou vegetações que podem, inclusive, estar relacionados a SVCS, como no caso descrito neste relato. A boa resolução espacial do ETE 3D permite valor adicional na avaliação topográfica da lesão em relação aos planos adjacentes e melhor avaliação das suas dimensões.</p>
