			<title>Introduction</title>
			<p>The interruption of the aortic arch is a rare congenital cardiopathy, whose surgical correction generally requires reconstruction of the aortic arch. This intervention can cause distortion of the aortopulmonary space and results in bronchial compression with a consequent pulmonary atelectasis and dependence on ventilatory assistance. The most commonly used therapeutic options for bronchial decompression are not always enough to make pulmonary expansion possible. Our study presents a case of the implant of an endoluminal stent in an infant who, in the post-operative stage of the correction of the interruption of the aortic arch, developed complete atelectasis of the left lung secondary to bronchial compression, with no response after posterior aortopexy</p>
		</sec>

			<title>Case Report</title>
			<p>Our study presents the case of a 3-year-old, preschool, female patient with a diagnosis of the interruption of the aortic arch and a major interventricular communication, submitted to the reconstruction of the aortic arch at 13 days of life, with end-to-side anastomosis of the descending aorta with the ascending aorta, due to the large distance from the stumps and difference in the diameter of the aortic extremities.</p>
			<p>In post-op, the patient presented complete and persistent atelectasis of the left lung (<xref ref-type="fig" rid="f01">Figure 1A</xref>) and was kept on mechanical ventilation with high-level parameters, with no possibility of extubation.</p>
			<p>

			</p>
			<p>Bronchoscopy suggested an extrinsic compression of the proximal portion of the left bronchus, a hypothesis confirmed by angiotomography, which demonstrated an external vascular compression of the main left bronchus between the left atrium and the descending aorta (<xref ref-type="fig" rid="f02">Figure 2A</xref>).</p>

			</p>
			<p>Despite the tracheostomy performed at two months of age and positive pressure ventilation, the child continued with a collapsed left lung and frequent episodes of hypoxia. At three months of age, she was submitted to aortopexy, which promoted an adequate distancing of the aorta from the bronchus, but this did not result in lung aeration, demonstrating damage to the bronchial structure. A resection of the altered tracheal segment was contraindicated due to technical difficulties and the severity of the ventilatory condition.</p>
			<p>At four months, after a multidisciplinary discussion, an expandable mechanical stent (Cordis Palmaz® BlueTM 5x15 mm) was implanted in the left bronchus by the hemodynamic service team (Video 1). Hence, progressive aeration of the affected lung (<xref ref-type="fig" rid="f01">Figure 1B</xref>) led to a decrease in the ventilatory parameters. The child evolved with refractory heart failure due to the drug treatment, and was thus submitted to surgical correction of the ventricular septal defect and an implant of a stent in the anastomosis region of the aorta (Video 1), which proved to be stenotic, with good results.</p>
			<p>During bronchoscopy follow-up, an excessive proliferation of the granulation tissue was observed (<xref ref-type="fig" rid="f03">Figure 3A</xref>), causing a progressive obstruction of the bronchial light, with no response to the three attempts of balloon dilatation and consequent total obstruction after three months of stent implant.</p>

			</p>
			<p>As the child presented a good respiratory pattern, using oxygen inhaled through the tracheostomy, she was released at eight months of life, with the proposal of the evaluation of the implant of a new bronchial stent (Dynamic Renal® 5x15 mm).</p>
			<p>At approximately one year of life, five months after bronchoscopic confirmation of the total obstruction of the stent, a chest X-ray, performed within an infectious context, showed left lung aeration (<xref ref-type="fig" rid="f01">Figure 1c</xref>).</p>
			<p>At one year and six months, the child was admitted to an intensive care unit (ICU) with a severe respiratory failure. The chest X-ray showed a diaphragmatic hernia on the right side and complete aeration of the left lung (<xref ref-type="fig" rid="f01">Figure 1D</xref>), confirmed through chest computed tomography (CT) (<xref ref-type="fig" rid="f02">Figure 2B</xref>) and bronchoscopy (<xref ref-type="fig" rid="f03">Figure 3B</xref>, video 2). After having performed surgery to correct the Morgani hernia, the child was released in good condition, with planning to be gradually weaned off of the tracheostomy.</p>

			<title>Discussion</title>
			<p>Bronchial compression is a potentially severe complication described in both the pre-operative and post-operative stages of congenital cardiopathies with an obstruction of the aortic arch, such as the coarctation of the aorta, the interruption of the aortic arch, and left heart syndrome. The altered relation between the blood vessels of the arch and the tracheobronchial tree found in these pathologies causes a distortion and straightening of the aortopulmonary space, and can lead to an extrinsic bronchial compression.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref> In this case, this alteration occurred in the geometry and position of the aortic arch after the surgical correction of the interruption.</p>
			<p>The knowledge of a possible compression of the airways is of utmost importance, as its incidence can reach 27.2% in the pre-operative and 33.3% in the post-operative stages of cardiopathies that require the reconstruction of the aortic arch, according to findings from Jhang et al.<xref ref-type="bibr" rid="B1">1</xref></p>
			<p>The respiratory symptoms are generally unclear and may be absent in many cases.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref> In a study conducted by An et al., only 50% of the patients with airway compression found in CT images presented symptoms.<xref ref-type="bibr" rid="B2">2</xref> The bronchoscopy enabled the direct viewing of the compression of the tracheobronchial tree, while the CT enabled the evaluation of the spatial region between the airway and cardiovascular structures, and should therefore be considered, especially in patients with persistent atelectasis and failure to wean off of mechanical ventilation.<xref ref-type="bibr" rid="B3">3</xref>-<xref ref-type="bibr" rid="B5">5</xref></p>
			<p>The early diagnosis of this medical condition is essential, since a prolonged bronchial compression can cause significant functional involvement and tracheobronchomalacia, which can persist even after the release of extrinsic compression<xref ref-type="bibr" rid="B2">2</xref>-<xref ref-type="bibr" rid="B5">5</xref>, as observed in the case described above.</p>
			<p>If on the one hand the diagnosis of airway compression can be easily performed, the therapeutic handling can be challenging, especially in cases of severe residual malacia. The choice between a surgical option, such as posterior aortopexy, the remodeling of the aortic arch, and transverse extension of the aortic arch using an autograft of the pulmonary artery, as compared to a conservative conduct,<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B6">6</xref> should be based on the severity of the obstruction and the clinical condition of the patient.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B5">5</xref> The tracheostomy with prolonged mechanical ventilation has been proposed when one considers the tracheomalacia to be self-limited, with progressive improvement during childhood.<xref ref-type="bibr" rid="B5">5</xref> In this case report, even if one had removed the bronchial compression, posterior aortopexy did not result in lung aeration, illustrating damage to the bronchial structure.</p>
			<p>In recent years, attention has been geared toward placing the endoluminal stent in airways in severe cases of bronchomalacia that did not respond to the implemented therapy. Barnes et al. described the successful use of the stent in a case of severe bronchomalacia after stage I of the Norwood procedure, which was not reverted after posterior aortopexy.<xref ref-type="bibr" rid="B7">7</xref> Arcieri et al. also reported the use of the left bronchial stent in six patients with severe residual bronchomalacia after posterior aortopexy; however, these authors only analyzed the results of aortopexy.<xref ref-type="bibr" rid="B4">4</xref> In the study conducted by Serio et al., the stent implant restored the spontaneous respiration in 20 patients with severe residual malacia after surgery.<xref ref-type="bibr" rid="B8">8</xref>Based on these descriptions and faced with the complexity of the case and absence of new surgical proposals, we chose the endobronchial stent implant.</p>
			<p>Although the stent implant in airways is a tempting conduct, as it can quickly restore the permeability of the bronchus and enable pulmonary aeration, it is not free of complications, such as migration, fracture, obstruction caused by mucus and granulation tissue, the rupture of the airways, and infection.<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B10">10</xref> The formation of the granulation tissue is a constant, since the airway stent did not become epithelated like the endovascular stent.<xref ref-type="bibr" rid="B5">5</xref> In the case described herein, we detected the proliferation of serial dilatations with a balloon catheter, but with a partial and spontaneous resolution after nearly five months, most likely after a decrease in the reactional inflammatory process.</p>
			<p>Therefore, a recommendation must first undergo a detailed analysis, since the stents are easy to implant, but difficult to remove, especially metallic stents, which should be considered permanent.<xref ref-type="bibr" rid="B5">5</xref></p>
			<p>Complication related to the use of the stent indicate the need for protocols to maintain the permeability of the lumen<xref ref-type="bibr" rid="B9">9</xref> and for regular follow-up by bronchoscopy and/or CT in order to achieve an early identification of the complications.<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B10">10</xref></p>
		</sec>
		<sec sec-type="conclusions">
			<title>Conclusions</title>
			<p>Bronchial compression after the reconstruction of the aortic arch is a severe complication, which one may not be able to solve with available surgical interventions. In these cases, the stent implant in the location where the bronchial light is tapered can represent a successful alternative, though reserved for children in a critical condition, whose airway decompression surgery did not result in affective lung aeration.</p>

				<title>Introdução</title>
				<p>A interrupção do arco aórtico é uma cardiopatia congênita rara, cuja correção cirúrgica geralmente requer reconstrução do arco aórtico. Essa intervenção pode causar distorção do espaço aortopulmonar e resultar em compressão brônquica com consequente atelectasia pulmonar e dependência de assistência ventilatória. As opções terapêuticas mais utilizadas para descompressão brônquica nem sempre são suficientes para possibilitar expansão pulmonar. Relatamos um caso de implante de stent brônquico endoluminal em um lactente que, no pós-operatório de correção de interrupção do arco aórtico, desenvolveu atelectasia total do pulmão esquerdo secundária a compressão brônquica, sem resposta após aortopexia posterior.</p>
			</sec>
			<sec sec-type="cases">
				<title>Relato do caso</title>
				<p>Apresentamos o caso de uma paciente do sexo feminino, de três anos com diagnóstico de interrupção do arco aórtico e grande comunicação interventricular, submetida a reconstrução do arco aórtico aos treze dias de vida, com anastomose término-lateral da aorta descendente com a ascendente, devido à grande distância dos cotos e diferença de diâmetro das extremidades aórticas.</p>
				<p>No pós-operatório, apresentou atelectasia completa e persistente do pulmão esquerdo (<xref ref-type="fig" rid="f01002">Figura 1A</xref>), sendo mantida em ventilação mecânica com altos parâmetros, sem possibilidade de extubação.</p>
				<p>

					</fig>
				</p>
				<p>A broncoscopia sugeriu compressão extrínseca da porção proximal do brônquio esquerdo, hipótese confirmada por angiotomografia que demonstrou compressão vascular externa do brônquio principal esquerdo entre o átrio esquerdo e a aorta descendente (<xref ref-type="fig" rid="f02002">Figura 2A</xref>).</p>
				<p>

					</fig>
				</p>
				<p>Apesar da traqueostomia realizada aos dois meses e ventilação com pressão positiva, a criança manteve colapso pulmonar esquerdo e episódios frequentes de hipóxia, sendo submetida, aos três meses, a aortopexia posterior que promoveu um adequado afastamento da aorta do brônquio, mas não resultou em aeração pulmonar, evidenciando um dano à estrutura do brônquio. Uma ressecção do segmento traqueal alterado foi contraindicada por dificuldades técnicas e gravidade do quadro ventilatório.</p>
				<p>Aos quatro meses, após discussão multidisciplinar, um stent metálico expansível (Cordis Palmaz® BlueTM 5x15 mm) foi implantado no brônquio esquerdo pela equipe de hemodinâmica (Vídeo 1). Houve, assim, aeração progressiva do pulmão acometido (<xref ref-type="fig" rid="f01002">Figura 1B</xref>), permitindo diminuição dos parâmetros ventilatórios. A criança evoluiu com insuficiência cardíaca refratária ao tratamento medicamentoso, sendo realizada correção cirúrgica da comunicação interventricular e implante de stent na região de anastomose da aorta (Vídeo 1) que se apresentava estenótica, com bom resultado.</p>
				<p>Durante acompanhamento broncoscópico, foi observada proliferação excessiva de tecido de granulação (<xref ref-type="fig" rid="f03002">Figura 3A</xref>) causando obstrução progressiva da luz bronquial, sem resposta a três tentativas de dilatação com balão e consequente obstrução total após três meses de implante do stent.</p>

				</p>
				<p>Como a criança apresentava bom padrão respiratório, em uso de oxigênio inalatório pela traqueostomia, recebeu alta aos 8 meses de vida, com proposta de avaliação de implante de um novo stent no brônquio (Dynamic Renal® 5x15 mm).</p>
				<p>Com cerca de um ano de vida, cinco meses após confirmação broncoscópica de obstrução total do stent, uma radiografia de tórax realizada durante um contexto infeccioso mostrou aeração do pulmão esquerdo (<xref ref-type="fig" rid="f01002">Figura 1C</xref>).</p>
				<p>Com um ano e seis meses, a criança foi admitida na unidade de terapia intensiva com insuficiência respiratória grave. A radiografia de tórax mostrou uma hérnia diafragmática à direita e aeração completa do pulmão esquerdo (<xref ref-type="fig" rid="f01002">Figura 1D</xref>), confirmado por tomografia computadorizada de tórax (<xref ref-type="fig" rid="f02002">Figura 2B</xref>) e broncoscopia (<xref ref-type="fig" rid="f03002">Figura 3B</xref>, vídeo 2). Após realização da correção cirúrgica da hérnia de Morgani, a criança recebeu alta em boas condições, com programação de desmame da traqueostomia.</p>

				<title>Discussão</title>
				<p>A compressão brônquica é uma complicação potencialmente grave descrita tanto no pré como no pós-operatório de cardiopatias congênitas com obstrução do arco aórtico, como coarctação de aorta, interrupção do arco aórtico, síndrome de hipoplasia do coração esquerdo. A relação alterada entre os vasos do arco e a árvore traqueobrônquica encontrada nessas patologias causa distorção e estreitamento do espaço aortopulmonar, podendo levar a compressão extrínseca do brônquio.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref> Nesse caso houve essa alteração na geometria e posição do arco aórtico após correção cirúrgica da interrupção.</p>
				<p>O conhecimento da possibilidade de compressão das vias aéreas é de grande importância, pois a incidência pode chegar a 27,2% no pré-operatório e 33,3% no pós-operatório de cardiopatias que necessitam de reconstrução do arco aórtico, conforme relatado por Jhang et al.<xref ref-type="bibr" rid="B1">1</xref></p>
				<p>Os sintomas respiratórios são geralmente inespecíficos e podem estar ausentes em muitos casos.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref> No estudo de An et al., apenas 59% dos pacientes com compressão aérea confirmada por tomografia computadorizada apresentavam sintomas.<xref ref-type="bibr" rid="B2">2</xref> A broncoscopia possibilita visibilização direta da compressão da árvore traqueobrônquica e a tomografia computadorizada permite avaliação da relação espacial entre as estruturas das vias aéreas e cardiovasculares e devem ser consideradas principalmente nos pacientes com atelectasia persistente e falha no desmame da ventilação mecânica.<xref ref-type="bibr" rid="B3">3</xref>-<xref ref-type="bibr" rid="B5">5</xref></p>
				<p>O diagnóstico precoce desse quadro é fundamental, pois uma compressão brônquica prolongada pode causar comprometimento funcional significativo e traqueobroncomalácia que pode persistir mesmo após liberação da compressão extrínseca,<xref ref-type="bibr" rid="B2">2</xref>-<xref ref-type="bibr" rid="B5">5</xref> como observado no caso descrito.</p>
				<p>Se por um lado o diagnóstico da compressão das vias aéreas pode ser facilmente realizado, o manejo terapêutico pode ser desafiador, especialmente nos casos de grave malácia residual. A escolha entre uma opção cirúrgica, como aortopexia posterior, remodelação do arco aórtico e extensão transversa do arco aórtico usando autoenxerto da artéria pulmonar, e uma conduta conservadora,<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B6">6</xref> deve ser baseada na gravidade da obstrução e na condição clínica do paciente.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B5">5</xref> A traqueostomia com ventilação mecânica prolongada tem sido proposta ao considerar a traqueomalácia autolimitada, com melhora progressiva durante a infância.<xref ref-type="bibr" rid="B5">5</xref> No caso relatado, a aortopexia posterior, mesmo removendo a compressão brônquica, não resultou em aeração pulmonar, evidenciando dano à estrutura do brônquio.</p>
				<p>Nos últimos anos, a atenção tem se voltado para colocação de stent endoluminal em vias aéreas em casos de grave broncomalácia que não responderam à terapêutica instituída. Barnes et al. descreveram uso de stent com sucesso em um caso de broncomalácia grave após Estágio I de Norwood, não revertida após aortopexia posterior.<xref ref-type="bibr" rid="B7">7</xref> Arcieri et al. também relataram uso de stent no brônquio esquerdo em seis pacientes com broncomalácia residual grave após aortopexia posterior, porém só analisaram os resultados da aortopexia.<xref ref-type="bibr" rid="B4">4</xref> No estudo de Serio et al., o implante de stent restaurou a respiração espontânea em 20 pacientes com malácia residual grave após cirurgia.<xref ref-type="bibr" rid="B8">8</xref> Baseados nessas descrições e diante da complexidade do caso e ausência de novas propostas cirúrgicas, optamos por implante de stent endobronquial.</p>
				<p>Embora o implante de stent em vias aéreas seja uma conduta tentadora por rapidamente restaurar a permeabilidade do brônquio e possibilitar aeração pulmonar, não é isenta de complicações, como migração, fratura, obstrução por muco ou tecido de granulação, ruptura de vias aéreas, infecção.<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B10">10</xref> A formação de tecido de granulação é uma constante, pois os stents das vias aéreas não se tornam epitelizados como os endovasculares.<xref ref-type="bibr" rid="B5">5</xref> No caso descrito, foi detectada proliferação de tecido de granulação com obstrução total do stent, sem resposta a dilatações seriadas com cateter balão, mas com resolução parcial e espontânea após cerca de cinco meses, provavelmente após redução do processo inflamatório reacional.</p>
				<p>Portanto, a indicação deve ser criteriosa, pois os stents são de fácil implantação, mas de difícil remoção, especialmente os metálicos que devem ser considerados permanentes.<xref ref-type="bibr" rid="B5">5</xref></p>
				<p>Complicações relacionadas ao uso do stent indicam necessidade de protocolos para manter a permeabilidade do lúmen<xref ref-type="bibr" rid="B9">9</xref> e acompanhamento regular por broncoscopia e/ou tomografia para identificação precoce das complicações.<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B10">10</xref></p>
			</sec>
			<sec sec-type="conclusions">
				<title>Conclusões</title>
				<p>A compressão brônquica após reconstrução do arco aórtico é uma complicação grave, podendo não ser solucionada com as intervenções cirúrgicas disponíveis. Nesses casos, a colocação de stent no local de estreitamento da luz brônquica pode ser uma alternativa bem-sucedida, embora reservada para crianças em estado crítico, cuja cirurgia para descompressão das vias aéreas não resultou em aeração pulmonar efetiva.</p>
