			<title>Introduction</title>
			<p>There has been an increased incidence of coronavirus-associated thromboembolic events during the coronavirus disease 2019 (COVID-19) pandemic.<xref ref-type="bibr" rid="B1">1</xref> However, most cases presented with venous thrombotic events, mainly deep vein thrombosis and pulmonary thromboembolism,<xref ref-type="bibr" rid="B2">2</xref> whereas arterial and coronary phenomena are rare.<xref ref-type="bibr" rid="B3">3</xref></p>
			<p>Several pathophysiological factors can explain the relationship between COVID-19 infection and increased embolic phenomena. The main factors are increased inflammatory mediators, the release of prothrombotic factors, and endothelial dysfunction.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B5">5</xref></p>
			<p>Cardiovascular presentations are also common in patients with COVID-19, such as atypical chest pain, dyspnea, arrhythmias, and myocardial injury with increased troponin levels.<xref ref-type="bibr" rid="B6">6</xref> However, chest pain and increased troponin levels in the absence of coronary obstruction are rare and restricted to a few case reports.<xref ref-type="bibr" rid="B7">7</xref>,<xref ref-type="bibr" rid="B8">8</xref> In addition, cardiovascular signs and symptoms associated with coronavirus infection occur in the acute stage of the disease with a symptomatic condition. Late presentation after complete symptom resolution is rarely reported.</p>
		</sec>
		<sec>
			<title>Objective</title>
			<p>Here we describe the case of a patient with the late presentation of chest pain associated with increased myocardial injury biomarkers after hospital discharge from a COVID-19 admission. The patient underwent coronary cineangiography and cardiac magnetic resonance imaging (MRI), and the syndromic diagnosis was myocardial infarction with non-obstructive coronary atherosclerosis (MINOCA). These results highlight the possibility of cardiovascular presentations in a subacute COVID-19 infection context.</p>
			<sec>
				<title>Clinical case description</title>
				<p>A 55-year-old man was admitted to the Emergency Department of our hospital with complaints of sudden-onset severe chest pain unrelated to exertion and was not treated with irradiation. The patient denied other associated symptoms.</p>
				<p>Before this clinical condition, the patient had been admitted to an inpatient bed in our hospital for 13 days with the diagnosis of COVID-19 and had used a nasal oxygen cannula and the following medications: dexamethasone, codeine, colchicine, and prophylactic enoxaparin. One day after hospital discharge, he started complaining of chest pain.</p>
				<p>A physical examination revealed a heart rate of 92 bpm, blood pressure of 122/80 mmHg, no significant changes on cardiac and pulmonary auscultation, good peripheral perfusion, and symmetrical pulses. His chest pain did not worsen with deep inspiration or palpation.</p>
			</sec>
			<sec>
				<title>Personal history</title>
				<p>The patient had a normal weight and no known risk factors or other diseases, nor was he regularly taking any medication. At the time of admission, he was no longer in pain.</p>
			</sec>
			<sec>
				<title>Differential diagnosis</title>
				<p>The following diagnostic hypotheses were proposed due to the recent COVID-19 infection and his clinical presentation: myocarditis, pericarditis, acute coronary syndrome, pulmonary thromboembolism, and Takotsubo syndrome.</p>
			</sec>
			<sec>
				<title>Diagnostic investigation</title>
				<p>After being admitted to the Emergency Department, the patient underwent a 12-lead electrocardiography (ECG) and troponin collection. The ECG showed sinus rhythm and an anterosuperior divisional block without changes suggestive of ischemia (<xref ref-type="fig" rid="f1">Figure 1</xref>). The high-sensitivity troponin levels were: 555.7 ng/L, 1,038 ng/L, 1,029 ng/L, and 853.5 ng/L. The other biochemical tests showed no significant changes.</p>

					</caption>
					<graphic xlink:href="2675-312X-dic-35-2-eabc290-gf01.tif"/>
				</fig>
				<p>Transthoracic echocardiography (TTE) showed no segmental contractility or valve changes.</p>
				<p>Next, as he reported feeling chest pain and his biomarker levels were increased, he underwent coronary angiography, which showed no significant coronary obstructions (<xref ref-type="fig" rid="f2">Figure 2</xref>).</p>
				<fig id="f2">

					</caption>
					<graphic xlink:href="2675-312X-dic-35-2-eabc290-gf02.tif"/>
				</fig>
				<p>Based on the diagnostic hypothesis of myocarditis, a cardiac MRI was performed and showed an area of late transmural enhancement located in the apical region of the left ventricle compatible with acute myocardial infarction (<xref ref-type="fig" rid="f3">Figure 3</xref>, <xref ref-type="fig" rid="m01">Videos 1</xref> and <xref ref-type="fig" rid="m02">2</xref>). The test was performed 6 days after the event with no signs of high signal intensity on T2-weighted sequences suggestive of myocardial edema.</p>

					<label>Video 2 (RM_4_Camaras)</label>
					<caption>
						<title>Cine-resonance - four-chamber view </title>
					</caption>
				</media>
				<p>The coronary angiogram was reviewed by two interventional cardiologists. However, no coronary obstruction that could have gone unnoticed was identified.</p>
				<p>Thus, the diagnostic hypotheses were ruled out due to absence of enhancement suggestive of myocarditis on cardiac MRI; the absence of electrocardiographic and echocardiographic signs suggestive of pericarditis (as well as significant biomarker increase with chest pain not characteristic of pericarditis); the absence of electrocardiographic (sinus tachycardia and/or S1Q3T3 pattern) and echocardiographic signs (no right ventricular dysfunction and/or signs of pulmonary hypertension) suggestive of pulmonary thromboembolism; and the absence of echocardiographic segmental contractility changes suggestive of Takotsubo syndrome.</p>
				<p>Accordingly, we started the patient on dual antiplatelet therapy and a statin. He developed no new symptoms and was discharged from the hospital.</p>
			</sec>
		</sec>
		<sec sec-type="discussion">
			<title>Discussion</title>
			<p>In this case, in the absence of coronary obstructions, ventricular dysfunction, and/or segmental contractility changes, the main diagnostic hypotheses were myocarditis and MINOCA. In this scenario, MRI was an essential differential diagnosis tool.<xref ref-type="bibr" rid="B9">9</xref></p>
			<p>After the complementary MRI findings, we believe that COVID-19-related factors may have influenced the formation of coronary thrombosis or distal emboli and resulted in the transmural pattern of late enhancement in the left ventricle.</p>
			<p>In this sense, we observed a rare cardiovascular complication potentially associated with COVID-19 infection that has a temporally atypical clinical presentation. Thus, we must pay attention to the possibility of late coronary events in COVID-19 patients and consider the possible persistence of inflammatory injury secondary to COVID-19 in the medium term. New studies may be necessary to identify the real incidence and clinical impact of these changes.</p>

				<title>Introdução</title>
				<p>Durante a pandemia da doença pelo coronavírus 2019 (Covid-19), observou-se um aumento da incidência de eventos tromboembólicos associados à infecção pelo coronavírus.<xref ref-type="bibr" rid="B1">1</xref> A maioria dos casos, no entanto, é composta de eventos trombóticos venosos, principalmente trombose venosa profunda e tromboembolismo pulmonar.<xref ref-type="bibr" rid="B2">2</xref> A ocorrência de fenômenos arteriais e, principalmente, coronarianos é rara.<xref ref-type="bibr" rid="B3">3</xref></p>
				<p>Vários fatores fisiopatológicos foram enumerados para explicar a relação entre a infecção por coronavírus e o aumento dos fenômenos embólicos. O aumento dos mediadores inflamatórios, a liberação de fatores protrombóticos e a disfunção endotelial são os principais fatores envolvidos.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B5">5</xref></p>
				<p>Também são comuns as manifestações cardiovasculares em pacientes infectados com coronavírus, como dor torácica atípica, dispneia, arritmias e injúria miocárdica com aumento de troponina.<xref ref-type="bibr" rid="B6">6</xref> A apresentação clínica com dor torácica e aumento de troponina na ausência de obstrução coronariana, no entanto, é rara e restrita a alguns relatos de caso.<xref ref-type="bibr" rid="B7">7</xref>,<xref ref-type="bibr" rid="B8">8</xref> Além disso, temporalmente, as manifestações cardiovasculares associadas à infecção por coronavírus ocorrem durante o quadro agudo da doença, na fase sintomática da doença. A apresentação tardia após a resolução completa dos sintomas é pouco relatada.</p>
				<p>Trata-se de um relato de caso de paciente com apresentação tardia, após a alta hospitalar de internação por Covid-19, de dor torácica associada à elevação de biomarcadores de injúria miocárdica. Após a investigação com cineangiografia coronariana e ressonância cardíaca, o diagnóstico sindrômico foi infarto do miocárdio sem lesões coronarianas obstrutivas (MINOCA, sigla do inglês myocardial infarction with nonobstructive coronary arteries). Esses resultados destacam a possibilidade de manifestações cardiovasculares em um contexto subagudo da infecção pelo coronavírus.</p>

				<title>Relato do caso</title>
				<p>Paciente, 55 anos, sexo masculino, deu entrada no Departamento de Emergência do nosso hospital com queixas de dor torácica, precordial, intensa, de início súbito, sem relação com esforços, sem irradiação. Negava outros sintomas associados.</p>
				<p>Antes desse quadro clínico, paciente esteve internado no nosso hospital por 13 dias com o diagnóstico de Covid-19. Durante a internação, paciente esteve em enfermaria, em uso de cateter nasal de oxigênio, fazendo uso das seguintes medicações: dexametasona, codeína, colchicina e enoxaparina profilática. Um dia após a alta hospitalar, iniciou a queixa de dor torácica descrita.</p>
				<p>Ao exame físico, frequência cardíaca de 92bpm, pressão arterial de 122x80mmHg, auscultas cardíaca e pulmonar sem alterações significativas, boa perfusão periférica e pulsos simétricos. Não havia piora da dor com inspiração profunda ou palpação.</p>
				<sec>
					<title>Antecedentes pessoais</title>
					<p>Peso era normal e não apresentava fatores de risco ou outras doenças conhecidas, assim como não fazia uso de nenhuma medicação de forma regular. No momento da admissão, o paciente já não apresentava mais dor.</p>
				</sec>
				<sec>
					<title>Diagnóstico diferencial</title>
					<p>Em virtude da infecção recente pela Covid-19 e da apresentação clínica, as seguintes hipóteses diagnósticas foram aventadas: miocardite, pericardite, síndrome coronariana aguda, tromboembolismo pulmonar e síndrome de Takotsubo.</p>
				</sec>
				<sec>
					<title>Investigação diagnóstica</title>
					<p>Após a admissão no Departamento de Emergência, eletrocardiograma (ECG) de 12 derivações e coleta de troponina foram realizados. O ECG mostrou ritmo sinusal, bloqueio divisional anterossuperior, sem alterações sugestivas de isquemia (<xref ref-type="fig" rid="f4">Figura 1</xref>). Os níveis da troponina de alta sensibilidade foram: 555,7ng/L, 1.038ng/L, 1.029ng/L e 853,5ng/L. Os demais testes bioquímicos não mostraram alterações significativas.</p>

					</fig>
					<p>O paciente foi submetido ao ecocardiograma transtorácico, que não mostrou alterações na contratilidade segmentar, assim como não havia alterações em valvas.</p>
					<p>Em seguida, uma vez que se tratava de dor torácica na presença de elevação de biomarcadores, uma angiografia coronariana foi realizada, a qual não mostrou obstruções coronarianas significativas (<xref ref-type="fig" rid="f5">Figura 2</xref>).</p>

					</fig>
					<p>Pela hipótese diagnóstica de miocardite, foi realizada uma ressonância magnética cardíaca (RMC), a qual revelou área de realce tardio transmural localizada na região apical do ventrículo esquerdo, compatível com infarto agudo do miocárdio (<xref ref-type="fig" rid="f6">Figura 3</xref>, <xref ref-type="fig" rid="m01_pt">Vídeos 1</xref> e <xref ref-type="fig" rid="m02_pt">2</xref>). Exame foi realizado após 6 dias do evento, sem sinais de hipersinal em sequências ponderadas em T2, sugestivas de edema miocárdico.</p>

					</media>
					<p>Após o resultado da RMC, a angiografia coronariana foi revisada por dois cardiologistas intervencionistas, mas foi observada ausência de obstrução coronariana, que poderia ter passada despercebido no momento do exame inicial.</p>
					<p>Dessa forma, foram descartadas as hipóteses diagnósticas por ausência de realce sugestivo de miocardite em ressonância cardíaca e de sinais eletrocardiográficos e ecocardiográficos sugestivos de pericardite (assim como elevação significativa de biomarcador com dor torácica não característica de pericardite), ausência de sinais eletrocardiográficos (taquicardia sinusal e/ou padrão S1Q3T3) e ecocardiográficos (ausência de disfunção de ventrículo direito e/ou sinais de hipertensão pulmonar) sugestivos de tromboembolismo pulmonar e ausência de alterações em contratilidade segmentar no ecocardiograma sugestivas de síndrome de Takotsubo.</p>
					<p>Após os resultados dos exames, iniciou-se o tratamento com dupla antiagregação plaquetária e estatina. O paciente seguiu sem novos sintomas, recebendo alta hospitalar.</p>

			<sec sec-type="discussion">
				<title>Discussão</title>
				<p>No caso deste paciente, na ausência de obstruções coronarianas e de disfunção ventricular e/ou alterações da contratilidade segmentar, as principais hipóteses diagnósticas eram miocardite e MINOCA. Nesse cenário, a utilização da RMC como ferramenta de auxílio no diagnóstico diferencial é fundamental.<xref ref-type="bibr" rid="B9">9</xref></p>
				<p>Após os achados complementares da RMC, acredita-se que, mesmo tardiamente, os fatores envolvidos na infecção pelo coronavírus podem ter influenciado a formação de trombose coronariana ou formação de êmbolos distais, ocasionando o padrão transmural de realce tardio no ventrículo esquerdo.</p>
				<p>Neste sentido, observa-se uma complicação cardiovascular potencialmente associada à infecção por coronavírus, além de rara, com manifestação clínica temporalmente atípica. Dessa forma, deve se atentar para a possibilidade de eventos coronarianos tardios em pacientes infectados por coronavírus, levantando a possibilidade da persistência da injúria inflamatória secundária à Covid-19 no médio prazo. Novos estudos podem ser necessários para identificar a real incidência e o impacto clínico dessas alterações.</p>
