	<body>
		<p>Prosthetic Heart Valves, despite their improvements over the last few decades, can be subject to complications and dysfunction. Mechanical prosthetic valves have an expected lifespan of approximately 10 to 20 years, with many lasting longer. For biological valves, the lifespan is generally shorter (5 to 15 years). However, the durability varies according to individual factors, valve type, and patient health. Although their greater durability, Mechanical Heart Valves are more pro-thrombotic than biological valves, and their dysfunction may be associated, for example, with thrombotic events and the formation of pannus.<xref ref-type="bibr" rid="B1">1</xref> Pannus is an inflammatory tissue composed of inflammatory cells, fibroblasts, and blood vessels, and its presence can lead to dysfunction or other complications such as valve obstruction or endocarditis. The prevalence of pannus in cardiac valve prostheses varies depending on several factors, such as the type of prosthesis (biological or mechanical) and the patients’ clinical conditions. It is estimated that the formation of pannus occurs in about 1% to 5% of patients with valve prostheses, although it may be more common in biological ones. The annual rate of thrombus formation can reach 5.7-8%, being higher in tricuspid and mitral valve prostheses.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref> A multimodality-based approach can provide useful data for differential diagnosis, complementing clinical assessment and offering an appropriate and optimized therapeutic approach. Valve dysfunction, in case of delayed diagnosis, can progress to cardiogenic shock.<xref ref-type="bibr" rid="B1">1</xref> Valvular dysfunctions can be structural (valve dysfunction due to changes related to the valve itself) or non-structural (dysfunctions not related to the valve resulting in stenosis or regurgitation, which include the formation of pannus), which arises from the accumulation of fibrous tissue in the valve, causing its dysfunction. It forms less frequently than thrombi and its risk is higher in the mitral position compared to the tricuspid.<xref ref-type="bibr" rid="B2">2</xref></p>
		<p>The authors present a case of mechanical mitral valve prosthesis dysfunction due to pannus. A Caucasian 43 years-old woman, with dyslipidaemia and hypertension with a history of infectious endocarditis of the mitral valve in 04/2022, presented with signs of infectious infiltration of both leaflets and severe mitral regurgitation. At this point, a cycle of antibiotic therapy was performed with ampicillin and flucloxacillin, with no agent being isolated (neither in blood cultures nor microbiological examination of the explanted valve). A double-disc mechanical valve prosthesis was implanted in the mitral position. The patients were hypocoagulated with a vitamin K antagonist (warfarin, with target INR levels of 2.5-3.5) and discharged from the hospital for outpatient follow-up.</p>
		<p>About a year after the intervention, she reported fatigue with moderate exertion. The INR value was 4.56, with adequate hypocoagulation in previous evaluations. A transthoracic echocardiogram was performed showing signs of prosthesis obstruction (transprosthetic mean gradient of 8 mmHg), due to a fixed disc in a closed position, and preserved Left Ventricular Ejection Fraction (58%). Faced with an abnormally high gradient, fluoroscopy was performed to assess valve function. Fluoroscopy, despite not being able to distinguish whether the valve dysfunction is due to pannus or thrombus, is extremely important because it allows us to understand that the valve movement is anomalous.<xref ref-type="bibr" rid="B3">3</xref> Fluoroscopy was performed confirming immobility of one of the discs (<xref ref-type="fig" rid="f01">Figure 1</xref>). The histological examination of the surgical material was compatible with pannus.</p>

		<p>Pannus formation is usually indolent, occurring several years later. In this case, the authors present a case in which pannus formation occurred with significant obstruction shortly after surgery. In the case of hemodynamically significant pannus, the only therapeutic choice is surgery, and a differential diagnosis with thrombosis must be made. In this case, although the INR was at supra-therapeutic values, it was not expected that a pannus dysfunction would occur so early. The timely mechanism evaluation and its contextualization are essential, in order to personalize and optimize potential interventions. The use of an integrated strategy (clinical, laboratory, and imaging) is essential for the adequate guidance of these complex cases.</p>

			<p>Apesar das melhorias observadas nas últimas décadas, as válvulas cardíacas protéticas ainda podem apresentar complicações e disfunções. Possuindo uma vida útil estimada entre 10 e 20 anos, com muitas superando esse período. Para válvulas biológicas, a vida útil é geralmente menor (5 a 15 anos). No entanto, a durabilidade varia de acordo com fatores individuais, tipo de válvula e estado de saúde do paciente. Embora sejam mais duráveis, as válvulas cardíacas mecânicas apresentam maior propensão à trombose em comparação com as válvulas biológicas, e sua disfunção pode estar associada a eventos trombóticos e à formação de pannus.<xref ref-type="bibr" rid="B1">1</xref> O pannus é um tecido inflamatório composto por células inflamatórias, fibroblastos e vasos sanguíneos, podendo causar disfunção valvular ou outras complicações, como obstrução valvular e endocardite. A prevalência de pannus em próteses valvulares cardíacas varia conforme o tipo de prótese (biológica ou mecânica) e as condições clínicas dos pacientes. Estima-se que o pannus se forme em 1% a 5% dos pacientes com próteses valvulares, sendo potencialmente mais comum em próteses biológicas. A taxa anual de formação de trombos pode variar de 5,7% a 8%, com maior incidência em próteses tricúspide e mitral.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref> Uma abordagem multimodal pode fornecer informações úteis para o diagnóstico diferencial, complementando a avaliação clínica e otimizando o tratamento. O pannus resulta do acúmulo de tecido fibroso na válvula, levando à disfunção. Forma-se menos frequentemente do que os trombos, sendo mais comum na posição mitral em comparação com a tricúspide.<xref ref-type="bibr" rid="B2">2</xref>

			<p>O presente relato apresenta um caso de disfunção de prótese valvular mitral mecânica devido à formação de pannus. Uma mulher caucasiana de 43 anos, com dislipidemia e hipertensão, apresentou endocardite infecciosa da válvula mitral em 04/2022, com infiltração dos folhetos e regurgitação mitral grave. Um ciclo de antibioticoterapia com ampicilina e flucloxacilina foi administrado, sem que nenhum agente fosse identificado em hemoculturas ou exames microbiológicos da válvula explantada. Uma prótese valvar mecânica de disco duplo foi implantada na posição mitral. A paciente foi tratada com antagonista da vitamina K (varfarina, alvo INR 2,5-3,5) e recebeu alta para acompanhamento ambulatorial.</p>
			<p>Aproximadamente um ano após a intervenção, ela relatou fadiga aos esforços moderados. O valor do INR foi de 4,56, com hipocoagulação adequada nas avaliações anteriores. Foi submetida a um ecocardiograma transtorácico, que revelou obstrução protética (gradiente médio de 8 mmHg), devido a um disco fixo em posição fechada, e fração de ejeção do ventrículo esquerdo preservada (58%). Diante de gradiente anormalmente alto, foi realizada fluoroscopia para avaliar a função valvar. A fluoroscopia, embora não diferencie entre pannus e trombo como causa da disfunção, é crucial para identificar movimentos anômalos da válvula.<xref ref-type="bibr" rid="B3">3</xref> Neste caso, a fluoroscopia confirmou a imobilidade de um dos discos (<xref ref-type="fig" rid="f01002">Figura 1</xref>). O exame histológico do material cirúrgico foi compatível com pannus.</p>

			<p>A formação de pannus costuma ser indolente, ocorrendo vários anos depois. No caso apresentado, a formação de pannus ocorreu com obstrução significativa, pouco depois da cirurgia. Em casos em que há pannus hemodinamicamente significativo, a cirurgia é a única opção terapêutica, exigindo diagnóstico diferencial com trombose. No caso apresentado, apesar do INR supraterapêutico, não era esperado que a disfunção causada pelo pannus ocorresse tão precocemente. A avaliação precoce do mecanismo e sua contextualização são essenciais, a fim de personalizar e otimizar potenciais intervenções. Uma estratégia integrada, envolvendo abordagens clínicas, laboratoriais e de imagem, é essencial para o manejo de casos complexos como este.</p>
