	<body>
		<p>A 73-year-old woman with a history of hypertension was referred to the vascular surgery department for severe chest pain and signs of shock, including tachycardia and a tendency toward hypotension. An angiotomography revealed a thoracoabdominal aneurysm (Crawford Classification type 2 – <xref ref-type="fig" rid="f1">Figure 1</xref>) with rupture signs (<xref ref-type="fig" rid="f2">Figures 2A</xref> and <xref ref-type="fig" rid="f2">2B</xref>) and anatomical variations in the supra-aortic trunks, including an aberrant right subclavian artery (ARSA) (<xref ref-type="fig" rid="f3">Figures 3A</xref> and <xref ref-type="fig" rid="f3">3B</xref>) and a bicarotid trunk (<xref ref-type="fig" rid="f4">Figure 4</xref>). She underwent emergency surgery with a hybrid approach, which included visceral branching via laparotomy and endovascular repair of the aneurysm. Unfortunately, she developed hemodynamic instability and refractory shock during the procedure, resulting in death. Her family authorized the publication and signed the consent form.</p>

		<p>An ARSA is a rare anomaly with a 0.2% to 13.3% prevalence.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref> It is more common among females, individuals with congenital anomalies (up to 3%), and those with Down syndrome (up to 35%).<xref ref-type="bibr" rid="B3">3</xref> ARSA may be associated with a bicarotid trunk in 0.8% to 20% of cases, where common carotid arteries originate from the same point on the aortic arch.<xref ref-type="bibr" rid="B2">2</xref> Thoracoabdominal Aortic Aneurysms (TAAAs) represent 10% of all aortic aneurysms. These aneurysms are potentially fatal if not treated early, as they may progress to rupture or dissection.<xref ref-type="bibr" rid="B4">4</xref> In a São Paulo epidemiological study, the mortality rate for emergency TAAA repairs was nearly double that of elective repairs (42.10% vs. 26.78%). This difference is often attributed to the hemodynamic instability of ruptured aneurysms and the lack of timely preoperative preparation in urgent cases.<xref ref-type="bibr" rid="B5">5</xref> Anatomical assessment is essential for treating TAAAs, whether through open surgery, endovascular surgery, or hybrid techniques. Anatomical variations in the supra-aortic trunks increase surgical complexity, regardless of the chosen technique.<xref ref-type="bibr" rid="B6">6</xref></p>
		<p>This &quot;hybrid&quot; approach, was first proposed by Quinones-Baldrich et al. in 1999 and involves a sequential revascularization of the visceral vessels, followed by endovascular aortic repair. Blood supply to the visceral organs is maintained by connecting an uncovered donor artery (often the iliac arteries).<xref ref-type="bibr" rid="B7">7</xref> This approach limits open surgery time to laparotomy, theoretically reducing the physiological stress associated with a larger thoracoabdominal incision and repair. By sparing patients from some disadvantages of open surgeries, such as thoracotomy, one-lung ventilation, extracorporeal pump perfusion, aortic cross-clamping, and ischemia of the visceral organs, spine, and limbs, it can be a valuable option.<xref ref-type="bibr" rid="B8">8</xref> In emergencies, there is insufficient time to customize stents for a fully endovascular repair in complex cases. Therefore, one alternative is surgeon-modified stents to create scallops or fenestrae. Yang et al. compared outcomes for TAAA patients treated with surgeon-modified stents and the hybrid technique, and reported low overall mortality (3.4% at 30 days) but a higher reintervention rate in patients who underwent endovascular surgery (13% vs. 0%).<xref ref-type="bibr" rid="B9">9</xref> Here, we report a rare case of anatomic variations in the supra-aortic trunks associated with a ruptured Crawford type 2 thoracoabdominal aneurysm.</p>

			<p>Uma mulher de 73 anos, com histórico de hipertensão arterial, foi encaminhada ao serviço de cirurgia vascular por dor torácica intensa e sinais de choque (taquicardia e tendência à hipotensão). Foi realizada uma angiotomografia com achado de aneurisma toracoabdominal (Classificação de Crawford tipo 2 – <xref ref-type="fig" rid="f5">Figura 1</xref>) com sinais de ruptura (<xref ref-type="fig" rid="f6">Figura 2A</xref> e <xref ref-type="fig" rid="f6">2B</xref>) e variação anatômica dos troncos supra-aórticos, artéria subclávia direita aberrante (ASDA) (<xref ref-type="fig" rid="f7">Figura 3A</xref> e <xref ref-type="fig" rid="f7">3B</xref>) e tronco bicarotídeo (<xref ref-type="fig" rid="f8">Figura 4</xref>). Ela foi submetida a cirurgia de emergência com reparo híbrido, envolvendo ramificação visceral via laparotomia e correção endovascular do aneurisma. Infelizmente, durante o procedimento, ela apresentou instabilidade hemodinâmica, choque refratário culminando em óbito. A família autorizou a publicação e assinou o termo de consentimento.</p>

			<p>A ASDA é uma anomalia rara com prevalência de 0,2% a 13,3%.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref> Há um predomínio no sexo feminino, indivíduos com anomalias congênitas (até 3%) e na síndrome de Down (até 35%).<xref ref-type="bibr" rid="B3">3</xref> A ASDA pode estar associada a um tronco bicarotídeo em 0,8 até 20% dos casos, no qual as duas artérias carótidas comuns se originam do mesmo local no arco aórtico.<xref ref-type="bibr" rid="B2">2</xref> Os Aneurismas da Aorta Toracoabdominal (AATA) representam 10% de todos aneurismas da aorta. São potencialmente fatais se não forem tratados precocemente, podendo evoluir com ruptura e/ou dissecção.<xref ref-type="bibr" rid="B4">4</xref> A taxa de mortalidade dos aneurismas toracoabdominais em um levantamento epidemiológico na cidade de São Paulo foram duas vezes maiores para reparos de emergência, do que em reparo eletivo (42,10% vs. 26,78%). Alguns autores atribuem essa diferença à instabilidade hemodinâmica dos aneurismas rotos, bem como a falta de preparo pré-operatório em tempo hábil para os pacientes em situações de urgência ou emergência.<xref ref-type="bibr" rid="B5">5</xref> O estudo anatômico é imprescindível para o tratamento dos AATAs seja por cirurgia aberta, endovascular ou técnicas hibridas. Variações anatômicas dos troncos supra-aórticos aumentam a complexidade cirúrgica, independente das técnicas empregadas.<xref ref-type="bibr" rid="B6">6</xref></p>
			<p>A técnica &quot;híbrida&quot; foi proposta por Quinones-Baldrich et al., em 1999 e envolve a revascularização sequencial dos vasos viscerais, seguindo para o reparo endovascular com exclusão da aorta. O suprimento visceral é garantido pelas pontes confeccionadas de uma artéria doadora não coberta (frequentemente as artérias ilíacas).<xref ref-type="bibr" rid="B7">7</xref> O tempo cirúrgico aberto é limitado a laparotomia e, teoricamente, reduz o estresse fisiológico de uma incisão e reparo toracoabdominal maior. Essa abordagem também poupa o paciente de vários pontos negativos associados à cirurgia aberta, incluindo toracotomia, ventilação monopulmonar, necessidade de perfusão por bomba extracorpórea, pinçamento aórtico e isquemia visceral, medular e dos membros.<xref ref-type="bibr" rid="B8">8</xref> Em situações de emergência, não há tempo hábil para customização de endopróteses para o reparo endovascular em casos complexos. Assim, uma alternativa são as próteses modificadas pelo cirurgião, para confecção de scallops ou fenestras. Yang et al., comparou os resultados de pacientes com AATA tratados com endopróteses modificadas pelo médico e a técnica híbrida. Foi relatado uma mortalidade geral baixa (3,4% em 30 dias), com uma taxa de reintervenção maior nos pacientes submetidos a cirurgia endovascular (13% vs. 0%).<xref ref-type="bibr" rid="B9">9</xref> Descrevemos um caso raro de variações anatômicas dos troncos supra-aórticos associado a aneurisma toracoabdominal Crawford 2 roto.</p>
