				<title>Abstract</title>
				<p>Cardiac masses, though rare, can lead to significant hemodynamic and arrhythmic issues, even if benign. Improved multimodal imaging has refined the diagnosis and understanding of these masses. We present an 85-year-old woman with New York Heart Association class II-III dyspnea for six months. Echocardiography showed a mass in the basal inferolateral wall of the left ventricle, along with severe left atrial dilation and moderate mitral regurgitation. Initially thought to be a pericardial cyst, further cardiac magnetic resonance (CMR) imaging identified an intramyocardial mass, and computed tomography (CT) suggested a hydatid cyst. Given the non-limiting symptoms and high surgical risk, the decision was made for expectant management. This case illustrates the rarity of cardiac cysts and the essential role of multimodal imaging in diagnosis and treatment planning.</p>

			<title>Introduction</title>
			<p>Cardiac masses, despite uncommon, are relevant due to their potential to cause hemodynamic and arrhythmic complications, even when benign.<xref ref-type="bibr" rid="B1">1</xref> The availability of multimodal imaging has increased the detection of these masses and provided more precise insights into their etiology and relationship with other cardiac structures.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref></p>

			<title>Case report</title>
			<p>An 85-year-old woman presented with dyspnea of New York Heart Association class II-III lasting for six months. A transthoracic echocardiogram revealed a left ventricle of normal dimensions and function, severe dilation of the left atrium, moderate mitral regurgitation, and a mass in the basal inferolateral wall of the left ventricle. The suggested diagnosis was a pericardial cyst. Cardiac magnetic resonance (CMR) imaging was requested, revealing an intramyocardial mass. Tissue characterization pointed towards a probable diagnosis of a hydatid cyst (<xref ref-type="fig" rid="f1">Figure 1</xref>). Computed tomography (CT) showed a mass with a calcified wall and hypodense content, findings suggestive of a hydatid cyst (<xref ref-type="fig" rid="f2">Figure 2</xref>). The course of action was discussed with the attending cardiologist and cardiac surgeon. Given the non-limiting symptoms and high surgical risk, a joint decision with the patient was made to pursue an expectant management approach.</p>

			<title>Discussion</title>
			<p>Cystic hydatid disease is caused by the larval stage of Echinococcus granulosus or Echinococcus multilocularis that may affect various organs of the body. Cardiac cysts are rare in adults and present a broad differential diagnosis encompassing both congenital and acquired conditions.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B3">3</xref> Pericardial cysts are the most common and distinguishing them from a hydatid cyst can be challenging, especially when the transthoracic ultrasound window is limited.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref></p>
			<p>Cardiac involvement in hydatid disease is observed in less than 2% of cases.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B5">5</xref> After invading the myocardium through the coronary artery, the left ventricle, which has the highest blood flow, is most frequently affected (50-60%). The interventricular septum is reported to be involved in 10-20% of cases.<xref ref-type="bibr" rid="B5">5</xref>,<xref ref-type="bibr" rid="B6">6</xref></p>
			<p>Typical characteristics of a hydatid cyst include mixed hypointensity on T1-weighted magnetic resonance imaging (MRI), mixed hyperintensity on T2-weighted MRI, cyst wall enhancement with gadolinium on MRI, along with variable hypodensity and wall calcification on CT.<xref ref-type="bibr" rid="B7">7</xref>,<xref ref-type="bibr" rid="B8">8</xref></p>
			<p>Cyst involution and/or complete calcification have been reported in 12% of cases; however, the cyst generally exhibits gradual expansion.<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B9">9</xref> Ruptured intracardiac cysts can have potentially fatal consequences. In the treatment of hydatid disease, surgery remains the method of choice.</p>
			<p>In this case, multimodal imaging played a crucial role in both diagnosing and locating the cyst, influencing the therapeutic decision.</p>
		</sec>
		<sec sec-type="conclusions">
			<title>Conclusion</title>
			<p>Cardiac masses are rare, and cardiac hydatid cysts are particularly uncommon. However, they can be found in countries with high prevalence of hydatidosis, such as those in Latin America, making it important to recognize their characteristics. Multimodal imaging plays a crucial role in identifying the etiology, locating the cyst, and planning surgical intervention. In the presented case, multimodal imaging was instrumental in determining the optimal approach based on the patient's vital prognosis.</p>

				<title>Resumo</title>
				<p>Massas cardíacas, embora raras, podem levar a problemas hemodinâmicos e arrítmicos importantes, mesmo quando benignas. O avanço do exame multimodal refinou o diagnóstico e o entendimento dessas massas. Apresentamos um caso de uma mulher de 85 anos, com dispneia classe II-III (New York Heart Association) há seis meses. O ecocardiograma mostrou uma massa na parede ínfero-lateral basal do ventrículo esquerdo, dilatação grave do átrio esquerdo, e regurgitação mitral moderada. A primeira hipótese era cisto pericárdico, mas o exame de ressonância magnética cardíaca (RMC) revelou uma massa intramiocárdica, e a tomografia computadorizada (TC) sugeriu um cisto hidático. Dados os sintomas não limitantes e o alto risco cirúrgico, decidiu-se pela vigilância ativa. Este caso ilustra a raridade dos cistos cardíacos e o papel essencial do exame multimodal no planejamento diagnóstico e terapêutico.</p>

				<title>Relato de caso</title>
				<p>Uma mulher de 85 anos de idade apresentou-se com dispneia classe II-III (New York Heart Association) há seis meses. Um ecocardiograma transtorácico revelou um ventrículo esquerdo com dimensões e função normais, dilatação grave do átrio esquerdo, regurgitação mitral moderada, e uma massa na parede ínfero-lateral do ventrículo esquerdo. O diagnóstico sugerido foi de cisto pericárdico. Um exame de ressonância magnética cardíaca (RMC) foi solicitado, que revelou uma massa intramiocárdica. A caracterização tecidual indicou para um provável diagnóstico de cisto hidático (<xref ref-type="fig" rid="f3">Figura 1</xref>). A tomografia computadorizada (TC) mostrou uma massa com uma parede calcificada e conteúdo hipodenso, sugestivo de um cisto hidático (<xref ref-type="fig" rid="f4">Figura 2</xref>). As condutas foram discutidas com o cardiologista responsável e o cirurgião cardíaco. Dados os sintomas não limitantes e o alto risco cirúrgico, a decisão foi tomada em conjunto com o paciente a fim de se obter uma abordagem de vigilância ativa.</p>

				<title>Discussão</title>
				<p>A hidatiose é causada por larvas do Echinococcus granulosus ou do Echinococcus multilocularis e pode afetar vários órgãos do corpo. Cistos cardíacos são raros em adultos e apresenta uma ampla gama de diagnósticos diferenciais, abrangendo tanto condições congênitas como adquiridas.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B3">3</xref> Cistos pericárdicos são os mais comuns, e os distinguir dos cistos hidáticos pode ser desafiador, principalmente quando a janela transtorácica no ultrassom é limitada.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref></p>
				<p>O envolvimento cardíaco na hidatiose é observado em menos de 2% dos casos.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B5">5</xref> Após invadir o miocárdio pela artéria coronária, o ventrículo esquerdo, que possui o maior fluxo sanguíneo, é frequentemente o mais afetado (50-60%). O septo interventricular está envolvido em 10-20% dos casos.<xref ref-type="bibr" rid="B5">5</xref>,<xref ref-type="bibr" rid="B6">6</xref></p>
				<p>Características típicas de um cisto hidático incluem hipointensidade mista em imagens ponderadas em T1 na Ressonância Magnética (RM), hiperintensidade em T2, realce pelo gadolínio da parede do cisto na RM, além de hipodensidade e calcificação variáveis da parede na TC.<xref ref-type="bibr" rid="B7">7</xref>,<xref ref-type="bibr" rid="B8">8</xref></p>
				<p>A regressão e/ou completa calcificação do cisto foram descritas em 12% dos casos; no entanto, o cisto geralmente exibe uma expansão gradual.<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B9">9</xref> Cistos intracardíacos rompidos podem ter consequências potencialmente fatais. No tratamento da hidatiose, a cirurgia continua sendo o tratamento de escolha.</p>
				<p>Neste caso, o exame de imagens multimodal exerceu um papel essencial tanto no diagnóstico como na localização do cisto, influenciando a decisão terapêutica.</p>
			</sec>
			<sec sec-type="conclusions">
				<title>Conclusão</title>
				<p>As massas cardíacas são raras, e cistos hidáticos cardíacos são particularmente comuns. No entanto, eles podem ser encontrados em países com uma alta prevalência de hidatiose, como em países da América Latina, sendo importante o reconhecimento de suas características. O exame multimodal exerce um papel crucial na identificação da etiologia, localização do cisto, e planejamento da intervenção cirúrgica. No caso apresentado, a imagem multimodal foi fundamental na definição de uma abordagem ótima baseada no prognóstico</p>
