	<body>
		<p>Dorsalis Pedis Artery Aneurysms (DPAAs) are rare, and few cases have been described in the literature. Pseudoaneurysms are believed to be more common than true aneurysms, but little is known about their clinical behavior.<xref ref-type="bibr" rid="B1">1</xref> The present study shows a case report of a patient with a DPAA, diagnosed by vascular ultrasound (VUS) and surgically treated at our facility.</p>
		<p>We treated a 59-year-old female patient, who was a saleswoman. She was asymptomatic until one year ago, when she noticed the appearance of a pulsatile mass on the dorsum of her right foot, which was occasionally painful, especially when wearing closed-toe shoes. She reported trauma five years prior to the onset of symptoms, caused by an iron bar falling on her feet.</p>
		<p>Medical History: hypertension, diabetes mellitus, and dyslipidemia.</p>
		<p>At the time of treatment, she was continuously taking simvastatin, metformin, and losartan. Physical examination was normal, except for the presence of a pulsatile mass on the dorsum of the right foot, varicose veins, and ochre dermatitis on both lower limbs. BP: 120X85 mmHg; HR=76 bpm.</p>
		<p>Laboratory tests, an electrocardiogram, chest X-ray, and transthoracic echocardiogram were performed, all of which were normal.</p>
		<p>The VUS of the arteries of the right lower limb showed an image suggestive of a fusiform aneurysm of the right DPAA, with thrombi measuring approximately 1.9 cm x 0.7 cm. Flow was present and multiphasic. Other arteries of the right lower limb showed flow to be present and multiphasic, without stenosis or other dilations (<xref ref-type="fig" rid="f1">Figure 1</xref>).</p>

		</fig>
		<p>An arteriography of the lower right limb demonstrated saccular dilation of the DPAA, near the metatarsal branches, with a maximum diameter of 1.0 cm. Arteriography suggested the diagnosis of a pseudoaneurysm (<xref ref-type="fig" rid="f2">Figure 2</xref>).</p>

		</fig>
		<p>The patient underwent open surgery, and the intraoperative findings, upon opening the aneurysm sac, showed no evidence of a pseudoaneurysm orifice; however, evidence of a true aneurysm was observed.</p>
		<p>An end-to-end graft with an inverted great saphenous vein was performed, preserving the arterial flow.</p>
		<p>The patient was discharged the following day after a control ultrasound demonstrated preserved, multiphasic flow in the dorsal artery of the right foot (<xref ref-type="fig" rid="f3">Figure 3</xref>).</p>

			<title>Discussion</title>
			<p>The DPAA (pedal artery) was first described in 1907.<xref ref-type="bibr" rid="B2">2</xref> Since then, other authors have reported this rare type of aneurysm, but its clinical manifestations are still relatively unknown.<xref ref-type="bibr" rid="B1">1</xref> With only 24 cases reported in the literature until 2017, according to Aragão et al.,<xref ref-type="bibr" rid="B1">1</xref> the DPAA aneurysm is more common among men (63%) with a mean age of 55.4%.<xref ref-type="bibr" rid="B3">3</xref> In general, it appears as a pulsatile mass, which may lead to microembolization, hemorrhage, rupture, and nerve compression.<xref ref-type="bibr" rid="B4">4</xref> Physical examination usually reveals a pulsatile mass that can cause pain, paresthesia, and discomfort when walking or wearing shoes, complaints similar to the case reported in our study.<xref ref-type="bibr" rid="B5">5</xref></p>
			<p>The pathophysiology of these aneurysms is still uncertain, but it appears to be related to two types of mechanisms: an intrinsic mechanism, related to weakness in the vessel wall structure, such as collagen diseases, Marfan and Ehlers-Danlos syndromes, syphilis, diabetes, infections, trauma, atherosclerosis, and fibrodysplasia; and an extrinsic mechanism, caused by mechanical stress on the arterial wall, such as trauma.<xref ref-type="bibr" rid="B3">3</xref>–<xref ref-type="bibr" rid="B6">6</xref> However, according to the literature, many DPAAs are pseudoaneurysms and occur after trauma or iatrogenic injuries secondary to orthopedic or vascular procedures.<xref ref-type="bibr" rid="B1">1</xref> Although trauma is a cause of DPAAs, patients are often unable to remember the acute event, and symptoms arise when they experience a compression of local structures or specific events, including embolism or rupture.<xref ref-type="bibr" rid="B5">5</xref> There have also been cases of repeated, low-impact trauma that have led to aneurysmal degeneration, such as tight shoes in patients with congenitally markedly high arches.<xref ref-type="bibr" rid="B5">5</xref></p>
			<p>True aneurysms appear with all three layers of the arterial wall, while pseudoaneurysms refer to hematomas with flow resulting from a rupture of the vessel wall.<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B7">7</xref> In our case, contrast angiography diagnosed the dilation as a pseudoaneurysm, but the VUS characteristics were those of a true DPAA with mural thrombus, which proved to be consistent with the surgical finding, which also presented as a true DPAA. Other authors have described a similar situation, and in some cases, only histopathological examination can differentiate between pseudoaneurysms and true DPAAs.<xref ref-type="bibr" rid="B5">5</xref>,<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B8">8</xref></p>
			<p>Diagnosis must be made promptly to avoid complications, such as thrombosis, distal embolism, hemorrhage, and rupture.<xref ref-type="bibr" rid="B1">1</xref> In a review by Kato et al.,<xref ref-type="bibr" rid="B3">3</xref> 4% of infrapopliteal aneurysms progress to rupture and 12.5% to thrombosis. VUS should be the first diagnostic test performed due to its high sensitivity and specificity in detecting aneurysms and pseudoaneurysms, as well as its noninvasive nature and lack of iodinated contrast.<xref ref-type="bibr" rid="B7">7</xref> However, other tests, such as computed tomography, conventional angiography, or magnetic resonance imaging, may also be performed for further diagnostic investigation or in doubtful cases.<xref ref-type="bibr" rid="B9">9</xref> The diagnostic investigation is usually similar in patients with pseudoaneurysms and true DPAAs.<xref ref-type="bibr" rid="B9">9</xref> Our patient initially underwent vascular ultrasonography, which diagnosed the DPAA, and conventional arteriography was subsequently performed to demonstrate the arterial arch and better evaluate other vascular structures.</p>
			<p>Treatment of a DPAA should be tailored to the individual case, with the options being endovascular or open surgical treatment.<xref ref-type="bibr" rid="B9">9</xref> Open surgical methods include simple ligation with or without resection, reconstruction of the artery with primary anastomosis, or interposition of a vein graft.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B10">10</xref> In the present case, an end-to-end graft with an inverted great saphenous vein and preservation of arterial flow were performed. The patient is currently asymptomatic and is being treated through outpatient follow-up.</p>

			<p>Os aneurismas da artéria dorsal do pé (ADP) são raros e poucos casos foram descritos na literatura. Acredita-se que os pseudoaneurismas são mais comuns do que os aneurismas verdadeiros, mas há pouco conhecimento acerca do seu comportamento clínico.<xref ref-type="bibr" rid="B1">1</xref> Relatamos o caso de uma paciente com aneurisma da ADP diagnosticado pela ultrassonografia vascular e tratado cirurgicamente em nosso serviço.</p>
			<p>Tratamos de uma paciente do sexo feminino, de 59 anos e comerciante. Estava assintomática até há 1 ano, quando observou surgimento de massa pulsátil no dorso do pé direito, eventualmente dolorosa, especialmente com uso de calçados fechados. Refere trauma 5 anos antes do surgimento dos sintomas, por queda de uma barra de ferro sobre os pés.</p>
			<p>Os antecedentes: incluem hipertensão arterial, diabetes mellitus e dislipidemia.</p>
			<p>No atendimento, estava em uso contínuo de Sinvastatina, Metformina e Losartana. O exame físico foi normal, exceto pela presença de massa pulsátil no dorso do pé direito, varizes e dermatite ocre em ambos os membros inferiores. PA: 120 X 85 mmHg; FC = 76 bpm.</p>
			<p>Foram realizados exames laboratoriais, eletrocardiograma, RX de tórax e ecocardiograma transtorácico, que estavam dentro da normalidade.</p>
			<p>A ultrassonografia vascular (USV) das artérias do membro inferior direito revelou imagem sugestiva de aneurisma fusiforme da ADP à direita, com presença de trombos, medindo cerca de 1,9 cm X 0,7 cm, o fluxo estava presente, de padrão multifásico. As demais artérias do membro inferior direito apresentavam fluxo presente e de padrão multifásico, sem estenoses ou outras dilatações – <xref ref-type="fig" rid="f4">Figura 1</xref>.</p>

			<p>Foi realizada arteriografia do membro inferior direito, que demonstrou dilatação sacular da ADP, próxima aos ramos metatarsianos, com diâmetro máximo de 1,0 cm. A arteriografia sugeriu o diagnóstico de pseudoaneurisma – <xref ref-type="fig" rid="f5">Figura 2</xref>.</p>

			<p>A paciente foi submetida a procedimento cirúrgico aberto, cujo achado intraoperatório, na abertura do saco aneurismático, não demonstrou evidência de orifício de pseudoaneurisma e revelou configuração de aneurisma verdadeiro.</p>
			<p>Foi realizado enxerto término-terminal com veia safena magna invertida e preservação do fluxo arterial.</p>
			<p>A paciente teve alta hospitalar no dia seguinte, após realização de USV de controle que demonstrou fluxo preservado, multifásico, na artéria dorsal do pé direito – <xref ref-type="fig" rid="f6">Figura 3</xref>.</p>

				<title>Discussão</title>
				<p>O aneurisma da artéria dorsal do pé (artéria pediosa) foi descrito pela primeira vez em 1907.<xref ref-type="bibr" rid="B2">2</xref> Desde então, outros autores relataram esse raro tipo de aneurisma, porém suas manifestações clínicas ainda são pouco conhecidas.<xref ref-type="bibr" rid="B1">1</xref> Com apenas 24 casos relatados na literatura até 2017, segundo Aragão et al.,<xref ref-type="bibr" rid="B1">1</xref> o aneurisma da ADP é mais comum em homens (63%) com média de idade de 55,4%.<xref ref-type="bibr" rid="B3">3</xref> Em geral, apresenta-se como uma massa pulsátil, podendo evoluir com microembolização, hemorragia, ruptura e compressão nervosa.<xref ref-type="bibr" rid="B4">4</xref> O exame físico geralmente apresenta uma massa pulsátil que pode causar dor, parestesia e desconforto ao caminhar ou usar calçados, queixas semelhantes às do caso aqui relatado.<xref ref-type="bibr" rid="B5">5</xref></p>
				<p>A fisiopatologia desses aneurismas ainda é incerta, mas parece relacionar-se a dois tipos de mecanismo: um intrínseco, que se relaciona à fraqueza na estrutura da parede do vaso, como doenças do colágeno, as síndromes de Marfan e de Ehlers-Danlos, sífilis, diabetes, infecções, trauma, aterosclerose e fibrodisplasia, e outro extrínseco, causado por estresse mecânico da parede arterial, como nos traumas.<xref ref-type="bibr" rid="B3">3</xref>–<xref ref-type="bibr" rid="B6">6</xref> No entanto, de acordo com a literatura, muitos aneurismas da ADP são pseudoaneurismas e ocorrem após traumas ou lesões iatrogênicas secundárias a procedimentos ortopédicos ou vasculares.<xref ref-type="bibr" rid="B1">1</xref> Apesar de o trauma ser uma causa de aneurisma da ADP, algumas vezes os pacientes não se lembram do evento agudo e os sintomas surgem quando apresentam compressão de estruturas locais ou eventos como embolia ou ruptura.<xref ref-type="bibr" rid="B5">5</xref> Também há casos de traumas repetidos, de baixo impacto, que levam à degeneração aneurismática, como sapatos apertados em pacientes com arcos acentuadamente altos de forma congênita.<xref ref-type="bibr" rid="B5">5</xref></p>
				<p>Os aneurismas verdadeiros apresentam as três camadas da parede arterial, enquanto os pseudoaneurismas se referem aos hematomas com fluxo, resultantes de uma ruptura da parede do vaso.<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B7">7</xref> No nosso caso, a angiografia contrastada diagnosticou a dilatação como pseudoaneurisma, porém, as características demonstradas pela USV eram de aneurisma verdadeiro da ADP com trombo mural, concordando com o achado cirúrgico que também apresentou como aneurisma verdadeiro da ADP. Outros autores descreveram situação semelhante e, eventualmente, somente o exame histopatológico é capaz de diferenciar entre pseudoaneurismas e aneurismas verdadeiros da ADP.<xref ref-type="bibr" rid="B5">5</xref>,<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B8">8</xref></p>
				<p>O diagnóstico deve ser feito prontamente a fim de evitar complicações como trombose, embolia distal, hemorragia e ruptura.<xref ref-type="bibr" rid="B1">1</xref> Em revisão feita por Kato et al.,<xref ref-type="bibr" rid="B3">3</xref> 4% dos aneurismas infrapoplíteos evoluem para ruptura e 12,5%, para trombose. A ultrassonografia vascular deve ser o primeiro exame diagnóstico a ser realizado, pela sua alta sensibilidade e especificidade na detecção de aneurismas e pseudoaneurismas, além de não ser invasivo e não utilizar contraste iodado.<xref ref-type="bibr" rid="B7">7</xref> Porém, outros exames também podem ser realizados, como a tomografia computadorizada, a angiografia convencional ou por ressonância magnética, seja para complementação diagnóstica ou em casos duvidosos.<xref ref-type="bibr" rid="B9">9</xref> A investigação diagnóstica costuma ser semelhante tanto em pacientes com pseudoaneurismas como aneurismas verdadeiros da ADP.<xref ref-type="bibr" rid="B9">9</xref> Nossa paciente inicialmente passou pela ultrassonografia vascular, que fez o diagnóstico do aneurisma da ADP. Posteriormente, foi realizada a arteriografia convencional a fim de demonstrar o arco arterial e melhor avaliar outras estruturas vasculares.</p>
				<p>O tratamento do aneurisma da ADP deve ser instituído conforme o caso, podendo-se optar por tratamento endovascular ou cirúrgico aberto.<xref ref-type="bibr" rid="B9">9</xref> Os métodos cirúrgicos abertos incluem ligadura simples, com ou sem ressecção, reconstrução da artéria com anastomose primária ou interposição de enxerto venoso.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B10">10</xref> No presente caso, optou-se pela realização de enxerto término-terminal com veia safena magna invertida e preservação do fluxo arterial. A paciente encontra-se assintomática em acompanhamento ambulatorial.</p>
