      <title>Introduction</title>
      <p>Hypertrophic cardiomyopathy (HCM) is an intrinsic myocardial disorder characterized by cardiac hypertrophy (wall thickness &#8805; 15 mm), that is not explained by conditions of pressure overload (eg, hypertension, severe aortic stenosis).<xref ref-type="bibr" rid="B1">1</xref> HCM is the most common genetic primary cardiomyopathy, with a prevalence estimated to be about one in 500 adults in the general population.<xref ref-type="bibr" rid="B2">2</xref>
			</p>
      <p>More than 450 mutations have been identified in the 20 genes that cause different phenotypes. In most cases, HCM is associated with sarcomere protein gene mutations, and exhibits multiple phenotypic expressions. We present a case that combines all phenotypes.<xref ref-type="bibr" rid="B3">3</xref>

      <title>Case Report</title>
      <p>A 58-year-old hypertensive woman was admitted to the coronary care unit because of acute heart failure syndrome. The patient denied chest discomfort, illicit drug use or previous disease. The patient noted progressive dyspnea, abdominal swelling, edema of both legs and weight gain. Bilateral edema, ascites, jugular venous distention and (a 3-sound) gallop rhythm were evident on physical examination. The electrocardiogram (ECG) showed sinus rhythm, low QRS amplitude and a pseudoinfarction pattern (<xref ref-type="fig" rid="f1">Figure 1</xref>). The echocardiogram depicted global severe hipokinesis with preservation of lateral wall motion, and increased wall thickness with left chamber enlargement. Moderate pericardial effusion was also present. A continuous infusion of loop diuretics was administered.</p>

      </p>
      <p>Thyroid hormones, iron tests and free light chain proteins were negative. Coronary angiography showed normal coronary arteries.</p>
      <p>Cardiac magnetic resonance revealed maximal wall thickness of 15 mm, left ventricular (LV) mass 262 g and LV mass index 178 g/m2, LV diastolic volume 194 mL, LV systolic volume 167 mL and ejection fraction 14%. A marked, diffuse transmural late gadolinium enhancement was also detected (<xref ref-type="fig" rid="f2">Figure 2. A</xref>-<xref ref-type="fig" rid="f2">F</xref>) (<xref ref-type="fig" rid="f2">Figure 2. A</xref>-<xref ref-type="fig" rid="f2">F</xref>)..</p>

Diffuse and transmural late gadolinium enhancement is showed (D, E, F) with preservation of basal segments. (G) Endomyocardial biopsy showed myocyte disarray.</title>

      <p>With the picture of severe congestive heart failure in addition to an inverse relationship of ECG amplitude with wall thickness, an infiltrative cardiomyopathy was suspected. Right heart catheterization showed high filling pressures and low cardiac output, and endomyocardial biopsy showed diffuse fibrosis without specific changes. As the patient became refractory to optimal medical treatment, she underwent orthotopic heart transplantation. She recovered uneventfully and biopsy of the explanted heart was positive for HCM, showing severe interstitial fibrosis and extensive foci of myocyte disarray affecting the LV (<xref ref-type="fig" rid="f2">Figure 2.G</xref>).</p>

      <title>Discussion</title>
      <p>HCM is a heterogeneous disease in terms of both genetics and phenotypes. For instance, it has been reported that distribution of hypertrophy in hypertrophic cardiomyopathy by troponin T gene differs no only among families but also within families.<xref ref-type="bibr" rid="B4">4</xref>
			</p>
      <p>The information available about the genotype - phenotype correlation in HCM is sparse.</p>
      <p>Sometimes HCM exhibits a "restrictive phenotype" characterized by restrictive filling and minimal or no left ventricular hypertrophy, which resembles idiopathic restrictive cardiomyopathy.<xref ref-type="bibr" rid="B5">5</xref>
			</p>
      <p>A smaller number of patients with HCM (5-10%) will progress to an end-stage form of the disease that is characterized by LV dilation, wall thinning and systolic dysfunction.<xref ref-type="bibr" rid="B6">6</xref> Despite the absence of systemic disease, the relationship between increased wall thickness, low ECG amplitude and severe diastolic dysfunction favors the diagnosis of a restrictive cardiomyopathy. Gadolinium enhancement was also typical for cardiac amyloidosis. In addition, there was no history of familial cardiac disease. Although endomyocardial biopsy even ruled out the diagnosis of cardiac amyloidosis, it also failed to confirm HCM, probably because the right ventricle was less affected.</p>
      <p>In this case, we reported increased left ventricular dimensions, depressed systolic function, ventricular hypertrophy and restrictive physiology, which are typical of a dilated, restrictive, hypertrophic phenotype. It is known that multiple genetic mutations can be present in the same person, which might explain the combination of three different features in this case.</p>
      <p>There is no similar case report in the literature.</p>
      <p>The limitation of this report is that we could not perform genetic testing as it is not currently available at our institution. In addition to the HCM diagnosis provided by biopsy it would have been of great value to know the specific gene mutation in order to puzzle out this "intriguing phenotype".</p>

        <p>Uma mulher de 58 anos, hipertensa, foi admitida &#224; unidade coron&#225;ria por insufici&#234;ncia card&#237;aca aguda. A paciente negou desconforto no peito, uso de drogas il&#237;citas ou doen&#231;as pr&#233;vias. Percebeu dispneia progressiva, incha&#231;o do abd&#244;men e em ambas as pernas, al&#233;m de ganho de peso. No exame f&#237;sico, detectou-se edema bilateral, ascite, distens&#227;o da veia jugular, e ritmo de galope com 3 sons. O eletrocardiograma (ECG) mostrou ritmo sinusal, QRS com amplitude diminu&#237;da, e padr&#227;o de pseudo-infarto (<xref ref-type="fig" rid="f3">Figura 1</xref>). O ecocardograma mostrou hipocinesia grave, com movimenta&#231;&#227;o da parede lateral preservada, e aumento da espessura da parede e do ventr&#237;culo esquerdo (VE). Tamb&#233;m foi detectada efus&#227;o peric&#225;rdica moderada. Foi administrado diur&#233;tico de al&#231;a em infus&#227;o cont&#237;nua. Testes de horm&#244;nios tireoideanos, ferro e prote&#237;nas de cadeia livre leve deram negativos. A angiografia coron&#225;ria mostrou art&#233;rias coron&#225;rias normais.</p>

        <p>Diante do quadro de insufici&#234;ncia card&#237;aca congestiva e da rela&#231;&#227;o inversa da amplitude do ECG com a espessura da parede, suspeitou-se de cardiomiopatia restritiva (infiltrativa). O cateterismo card&#237;aco esquerdo mostrou press&#227;o de enchimento aumentado e baixo d&#233;bito card&#237;aco, e a bi&#243;psia endomioc&#225;rdica revelou fibrose difusa, sem altera&#231;&#245;es espec&#237;ficas. Como a paciente tornou-se resistente ao tratamento m&#233;dico, foi submetida ao transplante card&#237;aco ortot&#243;pico. A paciente recuperou-se sem intercorr&#234;ncias e a bi&#243;psia do cora&#231;&#227;o explantado foi positiva para CMH, com fibrose intersticial grave e focos extensos de desorganiza&#231;&#227;o dos mi&#243;citos, acometendo o VE (<xref ref-type="fig" rid="f4">Figura 2.G</xref>).</p>

        <p>A CMH &#233; uma doen&#231;a heterog&#234;nea em termos gen&#233;ticos e fenot&#237;picos. Por exemplo, a distribui&#231;&#227;o da hipertrofia pelo gene da troponina T na cardiomiopatia hipertr&#243;fica difere-se n&#227;o s&#243; entre fam&#237;lias como tamb&#233;m dentro de uma mesma fam&#237;lia.<xref ref-type="bibr" rid="B4">4</xref>
				</p>
        <p>Informa&#231;&#245;es acerca da correla&#231;&#227;o genot&#237;pica-fenot&#237;pica s&#227;o escassas. &#192;s vezes, a CMH apresenta um fen&#243;tipo "restritivo", caracterizado por enchimento restritivo e hipertrofia do VE m&#237;nima ou ausente, semelhante &#224; cardiomiopatia restritiva idiop&#225;tica.<xref ref-type="bibr" rid="B5">5</xref>
				</p>
        <p>Um menor n&#250;mero de pacientes com CMH (5-10%) evoluir&#225; para o est&#225;gio terminal da doen&#231;a, que &#233; caracterizado por dilata&#231;&#227;o do VE, redu&#231;&#227;o na espessura da parede, e disfun&#231;&#227;o sist&#243;lica.<xref ref-type="bibr" rid="B6">6</xref>
				</p>
        <p>Apesar da aus&#234;ncia de doen&#231;a sist&#234;mica, a presen&#231;a de espessura da parede aumentada, baixa amplitude no ECG e disfun&#231;&#227;o diast&#243;lica grave &#233; sugestiva de cardiomiopatia restritiva. O realce com gadolinium tamb&#233;m foi caracter&#237;stico de amiloidose card&#237;aca. Al&#233;m disso, n&#227;o havia hist&#243;ria de doen&#231;a card&#237;aca familiar. Apesar de a bi&#243;psia endomioc&#225;rdica haver descartado o diagn&#243;stico de amiloidose card&#237;aca, tamb&#233;m n&#227;o confirmou o de CMH, provavelmente pelo fato de o ventr&#237;culo direito ter sido pouco afetado.</p>
        <p>No presente caso, encontramos dimens&#245;es aumentadas do VE, fun&#231;&#227;o sist&#243;lica diminu&#237;da, hipertrofia ventricular e fisiologia restritiva, que caracterizam um fen&#243;tipo dilatado, restritivo, hipertr&#243;fico.</p>
        <p>Sabe-se que muitas muta&#231;&#245;es gen&#233;ticas podem estar presentes em um mesmo indiv&#237;duo, o que pode explicar a combina&#231;&#227;o de tr&#234;s caracter&#237;sticas diferentes no presente caso.</p>
        <p>N&#227;o existe relato de caso similar na literatura.</p>
        <p>A limita&#231;&#227;o deste estudo deve-se a n&#227;o realiza&#231;&#227;o de um teste gen&#233;tico, uma vez que esse ainda n&#227;o est&#225; dispon&#237;vel em nossa institui&#231;&#227;o. Al&#233;m do diagn&#243;stico de CMH obtida por bi&#243;psia, seria de grande valor conhecer a muta&#231;&#227;o gen&#233;tica espec&#237;fica a fim de se esclarecer tal fen&#243;tipo "intrigante".</p>
