        <title>Abstract</title>
        <p>In the pathophysiology of heart failure (HF), attention has been directed to sympathetic hyperactivity and low-intensity systemic inflammation, aspects also present in erectile dysfunction (ED). Physical exercise is a strategy that improves these parameters, which makes plausible the hypothesis that, when practiced through dance, exercise would provide better results. By combining physical activity and music, dance would be able to improve the functioning of central and peripheral neural networks additionally to other favorable mechanisms, contributing to the restoration of neurohormonal function and reduction of the inflammatory response, which would increase the efficacy of HF and ED treatment.</p>

      <title>Introduction</title>
      <p>Heart failure (HF) is a complex clinical syndrome of high morbidity and mortality rates, representing one of the major contemporary health problems. In the pathophysiology of HF, attention has been directed to sympathetic hyperactivity, ventricular remodeling and, more recently, inflammatory and prothrombotic factors. These physiopathological aspects have a close relationship to each other and have an influence on disease perpetuation, prognosis and severity.<xref ref-type="bibr" rid="B1">1</xref>
			</p>
      <p>A high prevalence of sexual dysfunction, particularly erectile dysfunction (ED) has been reported among HF patients. ED has been considered a predictor of cardiovascular risk, and of cardiovascular and all-cause mortality. For this reason, ED has a major role in public health, by negatively affecting health and quality of life of patients,<xref ref-type="bibr" rid="B2">2</xref> with common characteristics of HF.</p>
      <p>In this context, physical exercise, such as dancing and its own particularities, is an intervention with great potential to benefit both HF and ED conditions.</p>
      <sec>
        <title>Development</title>
        <p>In ED and HF physiopathology, high levels of pro-inflammatory cytokines, associated with impaired autonomic nervous system have been reported,<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B4">4</xref> which may be characterized as a phenomenon primarily inflammatory and neurovascular.</p>
        <p>Sympathetic hyperactivity has an important compensatory role in the initial phase of HF. However, its maintenance contributes to further aggravation of cardiac dysfunction, which coexists with a significant increase in central and peripheral vasoconstriction,<xref ref-type="bibr" rid="B1">1</xref> which in turn increases penile smooth muscle tone and vasoconstriction of penile blood vessels, characteristic of ED.<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B4">4</xref>
				</p>
        <p>Similar to sympathetic hyperactivity, the systemic, low-degree inflammatory process, characterized as &#8216;subclinical&#8217; in ED patients, contributes to endothelial dysfunction<xref ref-type="bibr" rid="B3">3</xref> and consequent reduction in penile arterial dilation capacity.<xref ref-type="bibr" rid="B2">2</xref> Both abnormal autonomic modulation and subclinical inflammation are equally present in ED and HF.<xref ref-type="bibr" rid="B3">3</xref>
				</p>
        <p>Recently, neuroscience studies have identified a neural basis, which combined with humoral and immune activities, is able to reflexively regulate and adjust the inflammatory response under physiological conditions, referred to as the "inflammatory reflex".<xref ref-type="bibr" rid="B5">5</xref> In this respect, the nervous system, which receives information from the immune system by means of soluble mediators and sensory neurons, is also able to inhibit inflammation,<xref ref-type="bibr" rid="B5">5</xref> which is associated with both HF and ED.</p>
        <p>Nevertheless, in sympathetic hyperactivity and/or vagal suppression conditions, as in HF, such inflammatory reflex is impaired. In this case, physical exercise may play an important role by contributing to anatomical modulation and control of inflammation. These exercise effects can be beneficial to erectile function in ED.<xref ref-type="bibr" rid="B6">6</xref>-<xref ref-type="bibr" rid="B8">8</xref>
				</p>
        <p>Traditionally, aerobic exercise prescribed to HF patients is of moderate intensity and constant load, i.e., continuous training protocols that normally include conventional cyclic exercises. Such exercise programs may be monotonous, leading to low motivation and low adherence of patients to cardiac rehabilitation. In this regard, dancing is a playful, joyful activity that may be easily incorporated to social life, and a great potential alternative.<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B10">10</xref>
				</p>
        <p>Dance combines physical activity with music and thereby potentiates the physical, cognitive and emotional benefits, and promotes social integration.<xref ref-type="bibr" rid="B9">9</xref> Thus, dance enables a true synergism of positive effects promoted by physical exercise and music.<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B10">10</xref> Similar to physical activity, music activates body functioning, particularly the nervous system. When music penetrates human ears, sound waves are converted into electrical activity that sends information to thalamic and subthalamic regions and to the limbic system, which is considered the unit responsible for emotions and social behaviors.<xref ref-type="bibr" rid="B9">9</xref>
				</p>
        <p>The hypothalamus, which belongs to the limbic system and is considered the core of emotional expressions and sexual behavior, is also a central structure involved in the inflammatory reflex. This makes plausible the hypothesis that dancing can be an intervention capable to positively modulate inflammatory response by activation of cholinergic anti-inflammatory pathways, in a similar or even superior manner than conventional physical exercise, while promoting an increase in vagal activation.<xref ref-type="bibr" rid="B5">5</xref>
				</p>
        <p>Considering similarities in the physiopathology that explain the high prevalence of ED in HF patients, a plausible hypothesis is that dancing may be effective in the concomitant treatment of both conditions. However, so far only one observational, case-control study performed by our group showed that patients with cardiovascular diseases (coronary diseases and hypertension), who were ballroom dancing practitioners, had a lower risk to develop sexual dysfunction, and no other study on the effect of dance in HF patients was found.<xref ref-type="bibr" rid="B10">10</xref> This represents a gap in the literature.</p>

      <title>Conclusions</title>
      <p>Considering the similarities in the physiopathology of HF and ED, particularly regarding autonomic modulation and inflammation, dance is shown as a promising therapy for both conditions. The lack of studies investigating the effects of dance in the concomitant treatment of both HF and ED is a gap in the literature to be filled.</p>

        <title>Resumo</title>
        <p>Na fisiopatologia da Insufici&#234;ncia Card&#237;aca (IC) tem-se dado destaque &#224; hiperatividade simp&#225;tica e &#224; inflama&#231;&#227;o sist&#234;mica de baixa intensidade, aspectos tamb&#233;m presentes na disfun&#231;&#227;o er&#233;til (DE). O exerc&#237;cio f&#237;sico constitui-se estrat&#233;gia que proporciona melhora desses par&#226;metros sendo plaus&#237;vel a hip&#243;tese de que quando exercido por meio da dan&#231;a proporcionaria melhores resultados. Ao unir atividade f&#237;sica e m&#250;sica, a dan&#231;a seria capaz, dentre outros mecanismos favor&#225;veis, aprimorar o funcionamento das redes neurais centrais e perif&#233;ricas, contribuindo para o restabelecimento da normalidade da fun&#231;&#227;o neuro-hormonal e redu&#231;&#227;o da resposta inflamat&#243;ria, o que aumentaria a efic&#225;cia do tratamento tanto da IC quanto da DE.</p>

        <title>Introdu&#231;&#227;o</title>
        <p>A Insufici&#234;ncia Card&#237;aca (IC) &#233; uma s&#237;ndrome clinica complexa, de grande morbidade e alta taxa de mortalidade, constituindo-se em um dos maiores problemas contempor&#226;neos de sa&#250;de. Na sua fisiopatologia, tem-se dado destaque &#224; hiperatividade simp&#225;tica, ao remodelamento ventricular e, mais recentemente, aos fatores inflamat&#243;rios e pr&#243; tromb&#243;ticos, aspectos fisiopatol&#243;gicos que mant&#233;m forte v&#237;nculo entre si, influindo na perpetua&#231;&#227;o, progn&#243;stico e gravidade da doen&#231;a.<xref ref-type="bibr" rid="B1">1</xref>
				</p>
        <p>Nos pacientes com IC, tem sido observada alta preval&#234;ncia de disfun&#231;&#227;o sexual, e mais especificamente de disfun&#231;&#227;o er&#233;til (DE), a qual &#233; considerada preditora de risco cardiovascular, e de morte por causa cardiovascular e por todas as causas. Portanto, a DE possui grande import&#226;ncia em termos de sa&#250;de p&#250;blica, sendo capaz de prejudicar gravemente a sa&#250;de e a qualidade de vida,<xref ref-type="bibr" rid="B2">2</xref> com caracter&#237;sticas fisiopatol&#243;gicas em comum com a IC.</p>
        <p>Neste contexto, o exerc&#237;cio f&#237;sico, especialmente a dan&#231;a por suas peculiaridades, representa uma interven&#231;&#227;o com grande potencial de beneficiar ambas as situa&#231;&#245;es, IC e DE.</p>
        <sec>
          <title>Desenvolvimento</title>
          <p>Nas fisiopatologias da DE e da IC, t&#234;m sido observados elevados n&#237;veis de citocinas pr&#243;-inflamat&#243;rias, associados a um desequil&#237;brio do sistema nervoso auton&#244;mico,<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B4">4</xref> o que pode ser caracterizado como um fen&#244;meno primariamente inflamat&#243;rio e neurovascular.</p>
          <p>A hiperatividade simp&#225;tica tem importante fun&#231;&#227;o compensat&#243;ria na fase inicial da IC. Sua manuten&#231;&#227;o, no entanto, contribui para posterior agravamento da disfun&#231;&#227;o card&#237;aca que coexiste com o aumento significativo da vasoconstri&#231;&#227;o central e perif&#233;rica,<xref ref-type="bibr" rid="B1">1</xref> que promove o aumento do t&#244;nus da musculatura lisa do p&#234;nis e vasoconstri&#231;&#227;o dos vasos penianos, presentes na DE.<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B4">4</xref>
					</p>
          <p>Assim como a hiperatividade simp&#225;tica, o sist&#234;mico processo inflamat&#243;rio de baixo grau, caracterizado como subcl&#237;nico nos pacientes com DE,<xref ref-type="bibr" rid="B1">1</xref> contribui para a disfun&#231;&#227;o endotelial<xref ref-type="bibr" rid="B3">3</xref> e consequente redu&#231;&#227;o da capacidade de dilata&#231;&#227;o arterial peniana.<xref ref-type="bibr" rid="B2">2</xref> Ambas as situa&#231;&#245;es, modula&#231;&#227;o auton&#244;mica anormal e inflama&#231;&#227;o subcl&#237;nica, est&#227;o igualmente presentes na DE e IC.<xref ref-type="bibr" rid="B3">3</xref>

          <p>Recentemente, estudos de neuroci&#234;ncia identificaram uma base neural, que somada &#224; a&#231;&#227;o humoral e imune, &#233; capaz de reflexamente monitorar e ajustar a resposta inflamat&#243;ria em condi&#231;&#245;es fisiol&#243;gicas, denominado de "reflexo inflamat&#243;rio".<xref ref-type="bibr" rid="B5">5</xref> Neste contexto, o sistema nervoso, que recebe informa&#231;&#227;o do sistema imunit&#225;rio na forma de mediadores sol&#250;veis e de neur&#244;nios sensoriais, tamb&#233;m &#233; capaz de suprimir a inflama&#231;&#227;o,<xref ref-type="bibr" rid="B5">5</xref> presente tanto na IC quanto na DE.</p>
          <p>Entretanto, em condi&#231;&#245;es patol&#243;gicas caracterizadas por uma hiperexcita&#231;&#227;o simp&#225;tica e/ou supress&#227;o vagal, como no caso da IC, o reflexo inflamat&#243;rio encontra-se comprometido. Nesse caso, o exerc&#237;cio f&#237;sico pode desempenhar papel importante, contribuindo para a adequa&#231;&#227;o da modula&#231;&#227;o auton&#244;mica e o controle da inflama&#231;&#227;o. O exerc&#237;cio f&#237;sico tem potencial para produzir algo semelhante tamb&#233;m na DE, sendo capaz de favorecer a fun&#231;&#227;o er&#233;til pelos mesmos mecanismos, ou seja, aprimoramento da modula&#231;&#227;o auton&#244;mica e controle da inflama&#231;&#227;o.<xref ref-type="bibr" rid="B6">6</xref>-<xref ref-type="bibr" rid="B8">8</xref>

          <p>Tradicionalmente, o exerc&#237;cio aer&#243;bico prescrito para pacientes com IC &#233; de intensidade moderada com carga constante. Na pr&#225;tica cl&#237;nica, este tipo de treinamento pode gerar monotonia, resultando em baixa motiva&#231;&#227;o e pequena ader&#234;ncia dos pacientes aos programas de reabilita&#231;&#227;o card&#237;aca. <p>Considerando-se aspectos semelhantes na fisiopatologia, que explicam a grande preval&#234;ncia de DE em pacientes com IC, &#233; plaus&#237;vel a hip&#243;tese de que a dan&#231;a seja eficaz no tratamento concomitante de ambas as situa&#231;&#245;es. Entretanto, at&#233; o momento, um &#250;nico estudo observacional (caso controle), realizado pelo nosso grupo, demonstrou que pacientes com doen&#231;as cardiovasculares (coronariopatas e hipertensos) praticantes de dan&#231;a de sal&#227;o apresentam menor chance de desenvolver disfun&#231;&#227;o sexual, sendo que nenhum estudo foi encontrado sobre a influ&#234;ncia da dan&#231;a nos pacientes com IC.<xref ref-type="bibr" rid="B10">10</xref> Algo que representa uma lacuna na literatura.</p>
