      Prezado Editor,
    </p>
    <p>A real preval&#234;ncia de hipertens&#227;o resistente (HR) &#233; desconhecida, ao menos em parte, porque sua defini&#231;&#227;o n&#227;o &#233; universalmente aceita. N&#243;s lemos, com grande interesse, o artigo publicado por Massierer e cols.1, demonstrando que a frequ&#234;ncia de pacientes com HR verdadeira &#233; mais baixa (3,0-4,5% da popula&#231;&#227;o geral de hipertensos) no atendimento cl&#237;nico quando comparada com a preval&#234;ncia de 14,5% estimada com base em dados do Spanish ABPM Registry2. A nosso ver, esta &#250;ltima estimativa n&#227;o leva em conta dados relacionados a hipertens&#227;o n&#227;o controlada por tratamento n&#227;o otimizado e falta de ader&#234;ncia, por exemplo. Assim, a preval&#234;ncia de HR observada por Massierer e cols.1 &#233;, provavelmente, muito mais pr&#243;xima da real, tendo-se em vista que alguns erros em virtude de inconsist&#234;ncias na defini&#231;&#227;o dessa condi&#231;&#227;o, falta de crit&#233;rios r&#237;gidos e aus&#234;ncia de controle da qualidade de dados foram corretamente abordados.</p>
    <p>
      Carta-resposta
    </p>
    <p>Agradecemos o interesse dos colegas em nosso trabalho1. Conhecer-se a real frequ&#234;ncia de doen&#231;as &#233; indispens&#225;vel para o planejamento de cuidados e aloca&#231;&#227;o de recursos. Por vezes, estimativas s&#227;o artificialmente aumentadas em virtude de vieses de sele&#231;&#227;o. &#201; o que se configura no presente caso, a preval&#234;ncia de hipertens&#227;o resistente. Concordo com os colegas quanto ao entendimento de que essa preval&#234;ncia est&#225; superestimada. Defini&#231;&#227;o de diagn&#243;stico e bases amostrais n&#227;o representativas contribuem para isso, mas cabe destacar que muitos pacientes est&#227;o nessa condi&#231;&#227;o por insuficiente ades&#227;o ao tratamento. Em termos pragm&#225;ticos, pacientes com press&#227;o elevada porque n&#227;o tomam medicamentos est&#227;o sob risco similar aos que tomam e n&#227;o t&#234;m adequada resposta. Para os primeiros cabe empregar todas as t&#233;cnicas e energias para convenc&#234;-los a tomar os medicamentos, deixando para os segundos, bem menos frequentes, o emprego de m&#233;todos diagn&#243;sticos e terap&#234;uticos avan&#231;ados.</p>

        Dear Editor,
      </p>
      <p>The true prevalence of resistant hypertension (RHT) is unknown, at least in part because its definition is not consistent worldwide. We have read with great interest the article by Massierer et al. (1) showing that the frequency of patients with true RHT is lower (3.0-4.5% of the general hypertensive population) in clinical settings when compared to 14.5% estimated in the Spanish ABPM Registry (2). In our opinion, the latter estimates do not completely account for uncontrolled hypertension secondary to undertreatment and poor adherence, for example. Thus, the present findings are probably much closer to the real prevalence of RHT considering that errors due to inconsistencies in defining RHT, lack of strict criteria for inclusion of patients and absence of quality control of the data were correctly taken into account.</p>
      <p>
        Reply
      </p>
      <p>We appreciate your interest in our work1. To establish the real frequency of diseases is essential for the planning of care and resource allocation. Sometimes the estimates are artificially increased due to selection biases. This appears to be the present issue on the prevalence of resistant hypertension. I agree with the colleagues regarding the understanding that this prevalence is overestimated. Variable definitions and case selection in unrepresentative samples contribute to that, but it is worth noting that many patients have this condition because of insufficient treatment adherence. In pragmatic terms, patients with high blood pressure because they are not using blood pressure drugs are at similar risk to those who use the medication and show inadequate response. Regarding the former, we should employ all the techniques and efforts to convince them to take the medication; as for the latter, which have a less frequently observed condition, advanced diagnostic and therapeutic methods should be used to treat them.</p>
      <p>
        Sincerely,
