      Prezado Editor,
    </p>
    <p>Gostar&#237;amos de parabenizar os autores do trabalho "Implante de Stent Guiado por Ultrassom Intracoronariano Melhora Desfechos: Metan&#225;lise de Ensaios Randomizados"1 e fazer os seguintes coment&#225;rios: </p>
    <p>1. H&#225; heterogeneidade significativa entre os estudos da metan&#225;lise, que inclui tanto estudos com a t&#233;cnica de provisional stenting, j&#225; em desuso, quanto um estudo com stent farmacol&#243;gico. Tr&#234;s estudos exclu&#237;ram pacientes com les&#245;es longas e um deles n&#227;o utilizou o USIC para an&#225;lise da les&#227;o antes do implante do stent. </p>
    <p>2. Embora tenha ocorrido redu&#231;&#227;o n&#227;o significativa de eventos cardiovasculares maiores (ECVM) e infarto, o n&#250;mero de mortes foi maior no grupo de interven&#231;&#227;o guiada pelo USIC. Assim, h&#225; vi&#233;s de interpreta&#231;&#227;o dos autores, que sugerem que "com mais estudos e com um n&#250;mero maior de pacientes, o implante de stent guiado por USIC pode reduzir significativamente os casos de ECVM", quando na verdade o n&#250;mero de mortes tamb&#233;m poderia aumentar. </p>
    <p>3. Os autores concluem pelo benef&#237;cio do USIC baseado na an&#225;lise de desfechos substitutivos: reestenose angiogr&#225;fica e revasculariza&#231;&#227;o de vaso alvo, para os quais o cegamento &#233; dif&#237;cil. Apenas 62% dos estudos relataram cegamento dos avaliadores de desfechos. O uso de desfechos substitutivos deve ser visto com cautela, j&#225; que nem sempre se relacionam aos eventos cl&#237;nicos de interesse2. </p>
    <p>Defendemos que ainda a exist&#234;ncia vi&#233;s de publica&#231;&#227;o na presente metan&#225;lise, hip&#243;tese refor&#231;ada aplicando-se o teste de Egger3 aos dados de reestenose (p=0,037) e de ECVM (p=0,023). Pelas figuras apresentadas pelos autores, &#233; n&#237;tido que os estudos menores tiveram resultados "mais positivos" e o teste "trim and fill"4 tamb&#233;m foi positivo em nossa an&#225;lise. </p>
    <p>Sendo assim, acreditamos que a principal conclus&#227;o do estudo &#233; a aus&#234;ncia de provas de benef&#237;cio cl&#237;nico do uso do USIC para guiar o implante de stents. Conclus&#245;es baseadas em metan&#225;lise de desfechos substitutos, especialmente quando h&#225; heterogeneidade dos estudos e suspeita de vi&#233;s de publica&#231;&#227;o, devem ser avaliadas com cautela. </p>
    <p>
      Carta-resposta
    </p>
    <p>Agradecemos os coment&#225;rios referentes ao nosso artigo "Implante de Stent Guiado por Ultrassom Intracoronariano Melhora Desfechos: Metan&#225;lise de Ensaios Randomizados"1 e gostar&#237;amos de esclarecer algumas quest&#245;es levantadas pelos prezados colegas: </p>
    <p>Nosso artigo teve como objetivo revisar sistematicamente o impacto da adi&#231;&#227;o do USIC &#224; angiografia para otimiza&#231;&#227;o do implante de stents sobre os desfechos cl&#237;nicos e angiogr&#225;ficos e, para tal fim, foram inclu&#237;dos oito ensaios cl&#237;nicos randomizados: sete estudos que utilizaram stents convencionais e um que utilizou stent farmacol&#243;gico2. Para o desfecho 'eventos cardiovasculares maiores' (ECVM), analisando todos os estudos em conjunto, foi mostrado que n&#227;o houve redu&#231;&#227;o significativa no mesmo com a adi&#231;&#227;o do USIC &#224; angiografia, o que n&#227;o se modificou quando foi exclu&#237;do o estudo que utilizou stent farmacol&#243;gico, tanto quanto ao resultado principal, quanto em rela&#231;&#227;o &#224; heterogeneidade. O mesmo foi observado em rela&#231;&#227;o ao estudo que utilizou a t&#233;cnica de provisional stenting3: com a exclus&#227;o desse estudo da metan&#225;lise, n&#227;o houve altera&#231;&#227;o no resultado global e nem na heterogeneidade. </p>
    <p>&#201; importante enfatizar que embora os stents farmacol&#243;gicos sejam preferencialmente utilizados na pr&#225;tica contempor&#226;nea, esta abordagem est&#225; associada com maior custo e nem todos os pacientes s&#227;o bons candidatos para receber tais dispositivos, por exemplo, aqueles com contraindica&#231;&#245;es ou baixa ades&#227;o &#224; terapia antiplaquet&#225;ria dupla de longo prazo, cirurgia n&#227;o card&#237;aca planejada e comorbidades associadas com risco elevado de hemorragia representam algumas dessas situa&#231;&#245;es4.</p>
    <p>Em rela&#231;&#227;o &#224; metan&#225;lise que analisou a mortalidade de forma individual, foi observado aumento n&#227;o estatisticamente significativo nesse desfecho, isto &#233;, n&#227;o houve diferen&#231;a significativa entre o grupo USIC e o grupo ANGIO. De fato, como a qualidade dessa evid&#234;ncia foi moderada para esse desfecho e o tamanho &#243;timo da informa&#231;&#227;o n&#227;o foi atingido, novos ensaios cl&#237;nicos podem alterar este resultado, tanto para aumentar, reduzir ou mesmo manter sem diferen&#231;a a associa&#231;&#227;o do uso de USIC com este desfecho. </p>
    <p>O coment&#225;rio realizado sobre a valoriza&#231;&#227;o dos desfechos substitutivos apenas reitera o que j&#225; havia sido abordado na discuss&#227;o, a seguir transcrito: "Esta revis&#227;o sistem&#225;tica e metan&#225;lise demonstra que o implante de stent guiado por USIC reduz a reestenose angiogr&#225;fica e a revasculariza&#231;&#227;o da les&#227;o alvo em compara&#231;&#227;o com o implante de stent guiado por angiografia, por&#233;m n&#227;o reduz os casos de ECVM. Esses dados podem fornecer um suporte maior ao uso de USIC, mas a realiza&#231;&#227;o de ensaios cl&#237;nicos randomizados de maior escala e de alta qualidade &#233; necess&#225;ria a fim de elucidar o poss&#237;vel benef&#237;cio do USIC em rela&#231;&#227;o aos desfechos duros." Al&#233;m disso, apesar da revasculariza&#231;&#227;o do vaso alvo ser considerada um desfecho substituto, ela est&#225; associada &#224; piora da qualidade de vida e aumento dos custos de seguimento5.</p>
    <p>O vi&#233;s de publica&#231;&#227;o identificado pelos autores provavelmente se deve &#224; inclus&#227;o apenas de estudos em ingl&#234;s, portugu&#234;s e espanhol e por n&#227;o ter sido realizada busca na literatura por estudos n&#227;o publicados. A decis&#227;o de n&#227;o incluir este tipo de estudo se deveu a que esses trabalhos n&#227;o passam pelo processo de Peer Review, podendo apresentar pior qualidade metodol&#243;gica. </p>
    <p>Dessa forma, acreditamos nos benef&#237;cios do implante de stent guiado por USIC na redu&#231;&#227;o da reestenose angiogr&#225;fica e revasculariza&#231;&#227;o da les&#227;o alvo em compara&#231;&#227;o com o implante de stent guiado por angiografia, o que n&#227;o repercutiu em redu&#231;&#227;o de desfechos duros. Julgamos fundamental a realiza&#231;&#227;o de novos ensaios cl&#237;nicos randomizados de maior escala e de alta qualidade a fim de elucidar o poss&#237;vel benef&#237;cio do USIC em rela&#231;&#227;o aos desfechos duros.</p>
    <p>Agradecemos os coment&#225;rios do autor e esperamos ter esclarecido seus questionamentos.</p>

      <p>
        Dear Editor,
      </p>
      <p>We would like to congratulate the authors of the study: "Intracoronary ultrasound-guided stenting improves outcomes: a meta-analysis of randomized trials"1 and make the following comments: </p>
      <p>1. There is significant heterogeneity among the meta-analysis studies, which includes both studies with provisional stenting technique, now in disuse, as well as a study with drug-eluting stent. Three studies excluded patients with long lesions and one of them did not use IVUS for lesion analysis prior to stent implantation. </p>
      <p>2. Although there was no significant reduction in major cardiovascular events (MACE) and myocardial infarction, the number of deaths was higher in the intervention group guided by IVUS. Thus, there is an interpretation bias by the authors, who suggest that "with more studies and a larger number of patients, IVUS-guided stenting can significantly reduce MACE cases," when in fact the number of deaths could also increase.</p>
      <p>3. The authors conclude on the benefit of IVUS, based analysis of surrogate outcomes: angiographic restenosis and target vessel revascularization, for which blinding is difficult. Only 62% of the studies reported blinding of outcome examiners. The use of surrogate outcomes should be viewed with caution, as it does not always relate to the clinical events of interest2.</p>
      <p>We point out the existence of publication bias in this meta-analysis, a hypothesis reinforced by applying the test of Egger3 to the data on restenosis (p = 0.037) and MACE (p = 0.023). Considering the figures presented by the authors, it is clear that the smaller studies had "more positive" results and the "trim and fill"4 test was also positive in our analysis.</p>
      <p>Thus, we believe that the main conclusion of the study is the absence of evidence of clinical benefit from the use of IVUS to guide stent implantation.</p>
      <p>Conclusions based on meta-analyses of surrogate endpoints, especially when there is heterogeneity of the studies and suspected publication bias should be assessed with caution.</p>
      <p>
        Response Letter
      </p>
      <p>We would like to thank you for the comments regarding our article: "Intracoronary ultrasound-guided stenting improves outcomes: a meta-analysis of randomized trial"1 and would like to clarify some questions raised by our colleagues: </p>
      <p>Our article aimed to systematically review the impact of adding the IVUS angiography for optimal stent implantation on clinical and angiographic outcomes, and for this purpose, we included eight randomized clinical trials: seven studies using conventional stents and one that used drug-eluting stent (DES)2. For the outcome 'major cardiovascular events' (MACE), when analyzing all the studies together, it was shown that there was no significant reduction for it by adding IVUS to the angiography, which did not change even when the study that used DES was excluded, both regarding the main outcome, and the heterogeneity. The same was observed in the study using the provisional stenting technique3: the exclusion of this study from the meta-analysis did not change either the overall result or the heterogeneity. </p>
      <p>It is important to stress that although DES are preferably used in current practice, this approach is associated with higher costs and not all patients are good candidates to receive such devices, such as, for instance, those with contraindications or low adherence to long-term double antiplatelet therapy, planned non-cardiac surgery and comorbidities associated with increased risk of bleeding4. </p>
      <p>Regarding the meta-analysis that assessed mortality individually, we observed an increase in this outcome that was not statistically significant, i.e., no significant difference between the IVUS group and the ANGIO group. In fact, as the quality of this evidence was moderate for this outcome and the optimal size of the information was not reached, further clinical trials may alter this result, to increase, reduce or even maintain the association with no difference in the use of IVUS with this outcome. </p>
      <p>The comment about the appraisal of surrogate outcomes merely reiterates what has already been addressed in the discussion, as follows: "This systematic review and meta-analysis demonstrates that IVUS-guided stenting reduces angiographic restenosis and target-lesion revascularization when compared with stenting guided by angiography, but does not reduce MACE cases. These data may provide further support to the use of IVUS, but further randomized larger-scale and high-quality clinical trials are needed to elucidate the possible benefit of IVUS in relation to hard outcomes. Furthermore, although target-vessel revascularization can be considered a surrogate endpoint, it is associated with decreased quality of life and increased follow-up costs5. </p>
      <p>The publication bias identified by the authors is probably due to the inclusion of studies in English, Portuguese and Spanish only and because a literature search for unpublished studies was not carried out. The decision to not include this type of study was due to the fact that they do not go through the peer-review process, and may have worse methodological quality.</p>
      <p>Therefore, we believe in the benefits of IVUS-guided stenting in reducing angiographic restenosis and target-lesion revascularization when compared to stenting guided by angiography, which did not lead to reduction in hard endpoints. We believe that carrying out new larger-scale and high-quality randomized controlled trials is crucial in order to elucidate the possible benefit of IVUS in relation to hard outcomes.</p>
      <p>We appreciate the comments of the author and hope to have clarified their questions. </p>
      <p>Sincerely,</p>
      <p>Graciele Sbruzzi</p>
