    </p>
    <p>Cianose como sinal isolado surgiu com cinco meses de idade, tendo progredido recentemente, conjuntamente com processo infeccioso.</p>
    <p>Ao exame f&#237;sico, a paciente apresentou-se eupn&#233;ica, com cianose acentuada e pulsos normalmente palpados nos quatro membros. Seu peso era de 10 kg; altura, 81 cm; press&#227;o arterial, 90/60 mmHg; freq&#252;&#234;ncia card&#237;aca, 120 bpm; e satura&#231;&#227;o de oxig&#234;nio em repouso, 50%. A aorta n&#227;o era palpada na f&#250;rcula.</p>
    <p>No prec&#243;rdio, n&#227;o havia deformidades ou impuls&#245;es. O ictus cordis n&#227;o foi palpado. As bulhas card&#237;acas eram normofon&#233;ticas e n&#227;o foram auscultados sopros card&#237;acos. </p>
    <p>Nos pulm&#245;es, havia discreta diferen&#231;a de timbre do murm&#250;rio vesicular de um e de outro lado do t&#243;rax. O abdome n&#227;o apresentava anormalidades. O eletrocardiograma demonstrou ritmo sinusal e sinais de sobrecarga da cavidade ventricular esquerda, dada a presen&#231;a de ondas S profundas em V2 e V3 (&#237;ndice de Sokollof de 40 mm). &#194;QRS: +100&#186;; &#194;P: +60&#186;; &#194;T: +30&#186;.</p>

    <p>
      Confirma&#231;&#227;o diagn&#243;stica
    </p>
    <p>Em vista da aus&#234;ncia de sinais de cardiopatia, a cianose observada pode ser explicada por desvio arteriovenoso de sangue em supostas f&#237;stulas pulmonares. A magnitude desse desvio pode explicar o aumento do ventr&#237;culo esquerdo na radiografia de t&#243;rax e a sobrecarga ventricular esquerda no eletrocardiograma. O ecocardiograma confirmou a normalidade cardiovascular e o estudo hemodin&#226;mico revelou a presen&#231;a de gigantes f&#237;stulas arteriovenosas limitadas &#224; parte m&#233;dia do pulm&#227;o esquerdo, correspondendo ao lobo da l&#237;ngula e, talvez, a parte do lobo superior, dada a presen&#231;a de tr&#234;s troncos arteriais. O lobo inferior esquerdo estava preservado. No lobo m&#233;dio direito havia tamb&#233;m uma f&#237;stula arteriovenosa pulmonar, moderada (<xref ref-type="fig" rid="fig02">fig. 2</xref>).</p>
    <fig id="fig02">
      <graphic xlink:href="1678-4170-abc-90-06-0441-00009-gf02.jpg"/></fig>
    <p>
      Conduta
    </p>
    <p>No mesmo procedimento hemodin&#226;mico diagn&#243;stico, foi realizada emboliza&#231;&#227;o com mola efetivamente na f&#237;stula do lobo m&#233;dio direito, tendo da&#237; a satura&#231;&#227;o de oxig&#234;nio aumentado para cerca de 65%. Como a f&#237;stula &#224; esquerda era muito pronunciada, foi por isso idealizada a lobectomia do lobo da l&#237;ngula, a fim de se evitar n&#227;o s&#243; a progress&#227;o da hip&#243;xia, mas suas conseq&#252;&#234;ncias delet&#233;rias.</p>

      <p>Cyanosis as an isolated sign appeared at 5 months of age, having recently progressed accompanied by an infectious process.</p>
      <p>
        Physical examination
      </p>
      <p>Patient was eupneic, with marked cyanosis, and pulses were palpated normally on all 4 limbs. Weight: 10 Kg, Height: 81 cm, BP: 90/60 mm Hg, HR: 120 bpm. O2 saturation, at rest: 50%. Aorta was not palpable at the supraesternal notch.</p>
      <p>There were no deformities or impulsions in the precordium. Ictus cordis was not palpated. Heart sounds were normal and no heart murmurs were identified. </p>
      <p>Lung auscultation evidenced a slight difference in characteristics [timbre] of vesicular breath sounds from one side to the other of the chest. The abdomen showed no abnormalities. </p>
      <p>The electrocardiogram showed sinus rhythm and signs of left ventricular chamber overload with deep S waves in V2 and V3 (Sokolof Index: 40 mm) with &#194;QRS: +100o, &#194;P: +60o, and &#194;T: +30o.</p>

      <p>
        Diagnostic impression
      </p>
      <p>this image is consistent with the diagnosis of possible significant pulmonary arteriovenous fistulae.</p>
      <p>
        Differential diagnosis
      </p>
      <p>image of condensation in pulmonary parenchyma demands differential diagnosis with an infectious and/or tumoral pulmonary process.</p>
      <p>
        Diagnostic confirmation
      </p>
      <p>in light of the absence of signs of cardiopathy, the cyanosis observed may be explained by an arteriovenous shift of blood through the supposed pulmonary fistulae. The magnitude of this shift may justify the left ventricular increase seen on the chest X-ray and the left ventricular overload observed on the electrocardiogram. The echocardiogram confirmed cardiovascular normality and the hemodynamic study showed the presence of giant arteriovenous fistulae limited to the middle portion of the left lung corresponding to the lingula of the lobe and possibly part of the upper lobe because of the presence of 3 arterial trunks. The left lower lobe was preserved. In the right middle lobe there was also a moderate pulmonary arteriovenous fistula (<xref ref-type="fig" rid="fig02-body2">Fig. 2</xref>).</p>
      <fig id="fig02-body2">
        <graphic xlink:href="1678-4170-abc-90-06-0441-00009-gf02-en.jpg"/></fig>
      <p>
        Treatment
      </p>
      <p>During the same diagnostic hemodynamic procedure, coil embolization was performed of the middle right lobe and oxygen saturation increased to around 65%. Since the fistula on the left was pronounced, a lobectomy of the lobe-lingula was considered in order to avoid progression of the hypoxia along with its deleterious consequences.</p>
