      RESUMO
    </p>
    <p>Paciente de 52 anos de idade, do sexo masculino, com diagn&#243;stico de angina inst&#225;vel p&#243;s-infarto. A angiografia coronariana revelou obstru&#231;&#227;o luminal de 90% no ter&#231;o m&#233;dio da art&#233;ria coron&#225;ria direita e de 90% no ramo marginal da art&#233;ria circunflexa. Ap&#243;s o uso de clopidogrel 300 mg associado ao &#225;cido acetilsalic&#237;lico, o paciente foi submetido a implante de stent com elui&#231;&#227;o de sirolimus (CYPHER; Johnson&amp;Johnson-Cordis) 2,5 x 18 mm, na les&#227;o do ramo marginal esquerdo. Ap&#243;s um ano e cinco meses do implante do stent CYPHER, foi observada em nova angiografia a presen&#231;a de aneurisma coronariano intra-stent, no ramo marginal esquerdo. O presente relato sugere que o implante de stent coronariano revestido com sirolimus pode ocasionar, tardiamente, a forma&#231;&#227;o de aneurisma da art&#233;ria coron&#225;ria.</p>
    <p>Os estudos randomizados com o uso de stents com elui&#231;&#227;o de drogas demonstraram inibi&#231;&#227;o da hiperplasia neointimal na grande maioria dos pacientes1,2. Com o crescente uso desses stents, informa&#231;&#245;es sobre os efeitos dos mesmos em longo prazo s&#227;o de extrema import&#226;ncia. A presen&#231;a de aneurisma coronariano um ano e cinco meses p&#243;s-implante de stent com elui&#231;&#227;o de sirolimus suscita grande interesse pela aus&#234;ncia de relato de caso anterior.</p>

      RELATO DO CASO
    </p>
    <p>Paciente de 52 anos de idade do sexo masculino foi admitido com hist&#243;ria de dor precordial opressiva de forte intensidade, em repouso. Apresentava como fatores de risco para doen&#231;a coronariana hipertens&#227;o arterial sist&#234;mica e tabagismo, e fazia uso de captopril 12,5 mg duas vezes ao dia. Foram realizados eletrocardiograma e dosagem de enzimas, diagnosticando-se infarto agudo do mioc&#225;rdio sem supradesnivelamento do segmento ST. Durante a evolu&#231;&#227;o hospitalar, apresentou dor precordial e diagnosticou-se angina p&#243;s-infarto. Ao exame f&#237;sico, apresentava press&#227;o arterial de 120/70 mmHg e freq&#252;&#234;ncia card&#237;aca de 80 bpm. A ausculta cardiopulmonar era normal.</p>
    <p>O eletrocardiograma mostrou ritmo sinusal, sem anormalidades. A radiografia do t&#243;rax mostrou &#237;ndice cardiotor&#225;cico de 0,5, aorta alongada e campos pleuro-pulmonares normais.</p>
    <p>O paciente foi submetido a angiografia coronariana pela t&#233;cnica de Sones que revelou obstru&#231;&#227;o luminal de 90% no ter&#231;o m&#233;dio da art&#233;ria coron&#225;ria direita (ACD) e de 90% no primeiro ramo marginal da art&#233;ria circunflexa (MG) (<xref ref-type="fig" rid="fig1">fig. 1</xref>). A ventriculografia esquerda mostrou hipocontratilidade &#237;nfero-lateral discreta.</p>

    <p>O paciente foi medicado, no mesmo dia da angiografia, com clopidogrel 300 mg e &#225;cido acetilsalic&#237;lico (AAS) 200 mg, e no dia seguinte, ap&#243;s pr&#233;-dilata&#231;&#227;o com cateter-bal&#227;o Wordpass (Johnson&amp;Johnson-Cordis) 2,0 x 20 mm, foi submetido a implante de stent com elui&#231;&#227;o de sirolimus (CYPHER; Johnson&amp;Johnson-Cordis) 2,5 x 18 mm na les&#227;o do ramo MG com sucesso (<xref ref-type="fig" rid="fig2">fig. 2</xref>). O stent foi liberado com press&#227;o de 12 atm e a via utilizada foi a art&#233;ria femoral direita (AFD).</p>

    <p>A an&#225;lise angiogr&#225;fica quantitativa pelo Sistema Phyllips das medidas do ramo marginal esquerdo revelou, no pr&#233;-implante, di&#226;metro de refer&#234;ncia 2,50 mm, porcentual da estenose em di&#226;metro 81,01%, di&#226;metro luminal m&#237;nimo 0,47 mm e extens&#227;o da les&#227;o 8,21 mm. Ap&#243;s o implante, obteve-se di&#226;metro de refer&#234;ncia 2,50 mm e di&#226;metro luminal m&#237;nimo 2,54 mm (<xref ref-type="fig" rid="fig3">fig. 3</xref>).</p>

    <p>Os exames laboratoriais p&#243;s-procedimento revelaram creatinofosfoquinase de 60U/l com fra&#231;&#227;o MB 5U/l e desidrogenase l&#225;ctica 227 U/ml. O introdutor da AFD foi retirado ap&#243;s 4 horas do procedimento e foi realizada compress&#227;o manual local por 15 minutos. N&#227;o ocorreu sangramento ou hematoma ap&#243;s a compress&#227;o.</p>
    <p>Durante a interna&#231;&#227;o, o paciente evoluiu sem precordialgia, sem altera&#231;&#245;es eletrocardiogr&#225;ficas ou enzim&#225;ticas. Ap&#243;s dois dias do implante de stent, recebeu alta hospitalar com prescri&#231;&#227;o de clopidrogel 75 mg/dia at&#233; completar sessenta dias e AAS 200 mg indefinidamente.</p>
    <p>Ap&#243;s 36 dias do implante do primeiro stent, foi realizado o implante de stent Bx Velocity (Johnson&amp;Johnson-Cordis) 4,0 x 23 mm com sucesso na les&#227;o da ACD.</p>
    <p>O paciente apresentou novo epis&#243;dio de dor precordial um ano e cinco meses p&#243;s-implante do stent com elui&#231;&#227;o de sirolimus. Foi ent&#227;o submetido a nova angiografia coronariana, que revelou a presen&#231;a de aneurisma coronariano no ramo marginal, na por&#231;&#227;o proximal intra-stent, sem obstru&#231;&#227;o luminal em ambos os stents (<xref ref-type="fig" rid="fig4">fig. 4</xref>).</p>

    <p>
      DISCUSS&#195;O
    </p>
    <p>A forma&#231;&#227;o de aneurisma da art&#233;ria coron&#225;ria, definida como uma dilata&#231;&#227;o na art&#233;ria coron&#225;ria que excede 1,5 vez o di&#226;metro de refer&#234;ncia dos segmentos coron&#225;rios adjacentes angiograficamente normais3, foi relatada ap&#243;s angioplastia coronariana, aterectomia coronariana direcionada e angioplastia por laser com uma freq&#252;&#234;ncia que varia de 2% a 10%4. No estudo STRESS3 foi observada a presen&#231;a de aneurisma da art&#233;ria coron&#225;ria em 3,9% dos pacientes submetidos a implante de stent de Palmaz-Schatz.</p>
    <p>O implante de stent associado a agentes anti-inflamat&#243;rios foi relacionado a aumento da freq&#252;&#234;ncia de aneurismas coronarianos. Rab e cols.5 relataram a ocorr&#234;ncia de aneurisma da art&#233;ria coron&#225;ria em 32% dos pacientes p&#243;s-implante de stent, quando cortic&#243;ide e colchicina foram administrados ap&#243;s o procedimento. Esses investigadores conclu&#237;ram que a forma&#231;&#227;o de aneurisma estava provavelmente aumentada pelo uso concomitante de agentes anti-inflamat&#243;rios.</p>
    <p>Os novos stents com elui&#231;&#227;o de drogas aprovados para a pr&#225;tica cl&#237;nica, tais como o paclitaxel6 e o sirolimus1,2, s&#227;o capazes de inibir a hiperplasia neointimal. O sirolimus age especificamente na fase G1 do ciclo celular. Por sua a&#231;&#227;o precoce no ciclo celular, o sirolimus pode bloquear a prolifera&#231;&#227;o celular, com potencial seq&#252;ela vascular tardia2. Em adi&#231;&#227;o, o sirolimus estimula a apoptose e reduz a inflama&#231;&#227;o7. Assim, os stents com elui&#231;&#227;o de drogas podem levar &#224; forma&#231;&#227;o de aneurisma coronariano e dentre os achados tardios p&#243;s-implante, a aposi&#231;&#227;o incompleta das hastes do stent poderia ser o mecanismo respons&#225;vel.</p>
    <p>A aposi&#231;&#227;o incompleta tardia das hastes do stent, observada durante o seguimento com ultra-som intracoronariano em pacientes que receberam stents com elui&#231;&#227;o de sirolimus, &#233; um fen&#244;meno local, definida como a separa&#231;&#227;o de uma ou mais hastes do stent da intima, n&#227;o sobrepondo ramo lateral e com evid&#234;ncia de fluxo sang&#252;&#237;neo atr&#225;s da haste. Ocorre em 4% a 5% dos stents n&#227;o revestidos com drogas e em n&#250;mero desconhecido nos revestidos. Muitas hip&#243;teses foram formuladas para explicar sua origem, tais como remodelamento vascular regional positivo, regress&#227;o da placa, dissolu&#231;&#227;o tardia do material tromb&#243;tico aprisionado pelas hastes do stent, necrose celular, apoptose e rea&#231;&#227;o al&#233;rgica ao sirolimus8.</p>
    <p>Nos estudos RAVEL1 e SIRIUS2, E-SIRIUS3 e C-SIRIUS4, a presen&#231;a de aneurisma n&#227;o foi relatada, nos pacientes avaliados pela angiografia e ultra-som intracoronariano, seis a oito meses p&#243;s-implante de stent revestido com sirolimus.</p>
    <p>Recentemente, foram avaliados 13 pacientes com aposi&#231;&#227;o incompleta, por meio do ultra-som intracoronariano seis e doze meses p&#243;s-implante de stent com elui&#231;&#227;o de sirolimus e observou-se que a &#225;rea de aposi&#231;&#227;o incompleta n&#227;o se alterou nesse per&#237;odo, exceto para um paciente, em que se observou a presen&#231;a de aneurisma coronariano no seguimento aos doze meses, provavelmente preexistente na &#233;poca do implante e n&#227;o observado por estar preenchido por trombo. Baseado no fato que a elui&#231;&#227;o de sirolimus das hastes do stent permanece por apenas seis semanas, com uma meia-vida do sirolimus no tecido de 60 horas, o sirolimus por si &#233; improv&#225;vel induzir altera&#231;&#227;o na parede do vaso a longo prazo. Foi ent&#227;o levantada a possibilidade de que esse aneurisma provavelmente estivesse presente desde a primeira avalia&#231;&#227;o, mascarado por um trombo9.</p>
    <p>No presente caso, uma poss&#237;vel explica&#231;&#227;o para a forma&#231;&#227;o do aneurisma, n&#227;o relacionada ao uso do stent com elui&#231;&#227;o de sirolimus, &#233; que, se tratando de infarto agudo do mioc&#225;rdio sem supradesnivelamento do segmento ST, tenha ocorrido uma dissec&#231;&#227;o espont&#226;nea, n&#227;o vis&#237;vel pela angiografia, e que foi respons&#225;vel pelo desencadeamento da s&#237;ndrome coronariana aguda. A presen&#231;a de dissec&#231;&#227;o j&#225; foi observada por ultra-som intracoronariano previamente ao implante de Palmaz-Schatz, e a imagem n&#227;o desapareceu completamente logo ap&#243;s o implante. A angiografia foi repetida ap&#243;s seis meses e observou-se a presen&#231;a de aneurisma intra-stent. O ultra-som coronariano revelou que a mesma dissec&#231;&#227;o apresentava continuidade com o aneurisma coronariano, sugerindo ser esta a respons&#225;vel pela sua forma&#231;&#227;o10.</p>
    <p>Uma limita&#231;&#227;o no estudo do presente caso foi a n&#227;o-realiza&#231;&#227;o do ultra-som intracoronariano, que &#233; a &#250;nica maneira de detectar aposi&#231;&#227;o incompleta do stent.</p>
    <p>N&#227;o foi realizada nenhuma outra terap&#234;utica nesse caso, pois os pacientes com aneurisma da art&#233;ria coron&#225;ria p&#243;s-interven&#231;&#227;o coronariana percut&#226;nea t&#234;m mostrado curso cl&#237;nico benigno, sem associa&#231;&#227;o com aumento da mortalidade, infarto agudo do mioc&#225;rdio ou ruptura arterial3.</p>

      </p>
      <p>A 52-year-old man with diagnosis of post-infarction unstable angina. Coronary angiography revealed 90% luminal obstruction in the middle third of the right coronary artery and 90% in the marginal branch of the circumflex artery. After the administration of clopidogrel 300 mg associated with acetylsalicylic acid, the patient underwent the implantation of a sirolimus-eluting stent (CYPHER; Johnson &amp; Johnson Cordis) 2.5 x 1.8 mm in the lesion located at the left marginal branch. One year and five months after the CYPHER stent implantation, a new angiography showed intrastent coronary aneurysm at the left marginal branch. This case report suggests that the implantation of a sirolimus-coated coronary stent may cause late formation of coronary artery aneurysm.</p>
      <p>Randomized studies with the use of drug-eluting stents have demonstrated an inhibition of neointimal hyperplasia in the majority of patients1,2. With the increasing use of these stents, information on their long-term effect is extremely important. The presence of a coronary aneurysm one year and five months after the implantation of a sirolimus-eluting stent is of utmost interest due to the lack of previously reported case reports.</p>
      <p>
        CASE REPORT
      </p>
      <p>The patient was a 52-year-old male admitted with a history of very intense oppressive precordial pain at rest. He presented risk factors for coronary disease, systemic arterial hypertension and was a smoker; he also used captopril 12.5 mg twice a day. An electrocardiogram and enzyme measurements were carried out and an acute myocardial infarction was diagnosed, with no upper unleveling of the ST segment. During hospital stay, he presented precordial pain and post-infarction angina was diagnosed. At the physical examination he presented arterial pressure of 120/70 mmHg and cardiac frequency of 80 bpm. Cardiopulmonary auscultation was normal.</p>
      <p>The electrocardiogram showed a sinusal rhythm, with no abnormalities. Thorax X-ray showed a cardiothoracic index of 0.5, elongated aorta and normal pleuropulmonary fields.</p>
      <p>The patient underwent a coronary angiography by the Sones technique, which disclosed a luminal obstruction of 90% in the middle third of the right coronary artery (RCA) and 90% in the first marginal branch (MG) (<xref ref-type="fig" rid="fig1-body2">fig. 1</xref>) of the circumflex artery (CA). The venticulography showed a slight lower lateral hypocontractility.</p>

      <p>The patient was medicated with clopidogrel 300 mg and acetylsalicylic acid (AAS) 200 mg on the same day the angiography was carried out, and on the following day, after a pre-dilation with a Wordpass balloon-catheter (Johnson&amp;Johnson-Cordis) 2.0 x 20 mm, he underwent the implantation of a sirolimus-eluting stent (CYPHER; Johnson&amp;Johnson-Cordis) 2.5 x 18 mm in the lesion at the MG branch with success (<xref ref-type="fig" rid="fig2-body2">fig. 2</xref>). The stent was released with a 12 ATM pressure and the right femoral artery (RFA) was the access via used.</p>

      <p>The use of the Philips System for the quantitative angiographic analysis of the measurements of the left marginal branch disclosed, at the pre-implantation assessment, a reference diameter of 2.50 mm, a percent of stenosis in diameter of 81.01%, minimal luminal diameter of 0.47 mm and lesion extension of 8.21 mm. After the implant, the reference diameter was 2.50 mm and the minimal luminal diameter was 2.54 mm (<xref ref-type="fig" rid="fig3-body2">fig. 3</xref>).</p>

      <p>Post-procedure laboratory assessment showed levels of creatine phosphokinase of 60 U/L with a 5U/L MB fraction and 227 U/mL lactate dehydrogenase. The RFA introductory device was removed 4 hours after procedure completion and a manual compression was performed for 15 minutes. There was no post-compression bleeding or hematoma.</p>
      <p>During hospital stay, the patient evolved with no precordial pain and no electrocardiographic or enzymatic alterations were observed. Two days after stent implantation, he was released from the hospital with a clopidrogel prescription of 75 mg/day for 60 days and AAS 200 mg from the release date on, indefinitely.</p>
      <p>Thirty-six days after the first stent placement, a Bx Velocity stent (Johnson&amp;Johnson-Cordis) 4.0 x 23 mm was successfully implanted at the RCA lesion.</p>
      <p>The patient presented a new episode of precordial pain one year and five months after the implantation of the sirolimus-eluting stent. He then underwent a new coronary angiography, which disclosed the presence of a coronary aneurysm in the marginal branch, in the intra-stent proximal portion, with no luminal obstruction in both stents (<xref ref-type="fig" rid="fig4-body2">fig. 4</xref>).</p>

        DISCUSSION
      </p>
      <p>The formation of a coronary artery aneurysm, defined as a dilation of the coronary artery that exceeds 1.5 times the reference diameter of the adjacent coronary segments that are angiographically normal3, has been reported after coronary angioplasty, directed coronary atherectomy, and laser angioplasty at a frequency that varies from 2 to 10%4. At the STRESS3 study, a the presence of a coronary artery aneurysm was observed in 3.9% of the patients submitted to Palmaz-Schatz stent implant.</p>
      <p>Stent placement associated to anti-inflammatory agents has been correlated to an increased frequency of coronary aneurysms. Rab et al5 have reported the occurrence of coronary artery aneurysms in 32% of patients submitted to stent placement, when corticoid drugs and colchicine were administered after the implant. These investigators concluded that the aneurysm formation was probably increased due to the concomitant use of anti-inflammatory agents.</p>
      <p>The new drug-eluting stents approved for medical practice, such as paclitaxel6 and sirolimus1,2, are capable of inhibiting neointimal hyperplasia. Sirolimus acts specifically at the G1 phase of the cell cycle. Due to its early action on the cell cycle, sirolimus can block cell proliferation with late vascular sequelae potential2. Additionally, sirolimus stimulates apoptosis and reduces inflammation7. Thus, drug-eluting stents can lead to the formation of coronary aneurysms and among the late post-implantation findings, the incomplete apposition of the stent filaments could be the responsible mechanism.</p>
      <p>The late incomplete apposition of the stent filaments, observed during the follow-up with intracoronary ultrasound, is defined as a separation of one or more stent filaments from the intima, with no overlap on the lateral branch and evidence of blood flow behind the filament. It occurs in 4 to 5% of non-eluting stents and at an unknown frequency in drug-eluting ones. Many hypothesis have been formulated to explain its origin, such as positive regional vascular remodeling, plaque regression, late dissolution of the thrombotic material captured by the stent filaments, cell necrosis, apoptosis, and allergic reaction to sirolimus8.</p>
      <p>At the RAVEL1, SIRIUS2, E-SIRIUS3 and C-SIRIUS4 studies, the presence of aneurysms was not reported, in patients evaluated by angiography and intra-coronary ultrasound six to eight months after sirolimus-eluting stent placement.</p>
      <p>Recently, 13 patients with incomplete apposition were evaluated by intra-coronary ultrasound, six to twelve months after sirolimus-eluting stent placement and it was observed that the area of incomplete apposition did not show alterations during this period, except in one patient in whom the presence of a coronary aneurysm was observed during the follow-up period of twelve months; the condition was probably pre-existing at the time of the stent placement and failed to be detected due to the fact that it was filled with a thrombus. Based on the fact that sirolimus elution of the stent filaments remains for only six weeks, with a half-life of sirolimus in tissue for 60 hours, it is unlikely that sirolimus itself cam induce long-term alterations of the vessel wall. The possibility that this aneurysm was probably present since the first evaluation, having been masked by a thrombus, was then raised9.</p>
      <p>Regarding the present case, a possible explanation for the aneurysm formation, which is unrelated to use of a sirolimus-eluting stent, is that, being an acute myocardial infarction with no upper unleveling of the ST segment, there was a spontaneous dissection that was not visible through the angiography, which triggered the acute coronary syndrome. The presence of a dissection has been observed by intra-coronary ultrasound prior to Palmaz-Schatz stent placement, and the image did not disappear entirely immediately after the implant. The angiography was repeated six months later and the presence of an intra-stent aneurysm was observed. The coronary ultrasound disclosed that the same dissection presented continuity with the coronary aneurysm, suggesting that the dissection was responsible for the aneurysm formation10.</p>
      <p>A limitation observed in the present study was the lack of an intra-coronary ultrasound, which is the only way to detect the incomplete stent apposition.</p>
      <p>No other therapeutic measures were applied in this case, as patients with coronary artery aneurysm post-percutaneous coronary intervention have shown a benign clinical course, with no association with an increase in mortality, acute myocardial infarction or artery rupture3.</p>
