      RESUMO
    </p>
    <p>Mulher de 43 anos, sintom&#225;tica (dispn&#233;ia e palpita&#231;&#245;es), apresentava m&#250;ltiplas f&#237;stulas de alto d&#233;bito de ambas coron&#225;rias para a art&#233;ria pulmonar, embolizadas percutaneamente com micro-molas de libera&#231;&#227;o controlada e bal&#245;es destac&#225;veis, com sucesso. </p>
    <p> As f&#237;stulas coron&#225;rias s&#227;o comunica&#231;&#245;es entre essas art&#233;rias e as cavidades card&#237;acas ou outros vasos mediastinais; s&#227;o, freq&#252;entemente, desvios do desenvolvimento embrion&#225;rio1. M&#250;ltiplas f&#237;stulas de ambas coron&#225;rias para a art&#233;ria pulmonar, como na nossa paciente, s&#227;o entidades cl&#237;nicas raras2. F&#237;stulas com acentuada repercuss&#227;o hemodin&#226;mica, levando ao surgimento de sintomas de insufici&#234;ncia card&#237;aca e dor tor&#225;cica anginosa s&#227;o, classicamente, tratadas por liga&#231;&#227;o cir&#250;rgica3. Oclus&#227;o por cateterismo intervencionista tem se tornado cada vez mais freq&#252;ente4-8.</p>
    <p>
      Relato do Caso
    </p>
    <p>Mulher de 43 anos de idade procurou atendimento cardiol&#243;gico por palpita&#231;&#245;es taquic&#225;rdicas freq&#252;entes e dispn&#233;ia aos m&#233;dios esfor&#231;os, com sintomas progredindo nos &#250;ltimos anos. Exame cl&#237;nico espec&#237;fico revelou sopro card&#237;aco cont&#237;nuo, bem aud&#237;vel, na borda esternal esquerda na altura do 3&#186; e 4&#186; espa&#231;os intercostais. O eletrocardiograma de 12 deriva&#231;&#245;es revelou bloqueio completo de ramo esquerdo. O ecodoppler realizado em outro servi&#231;o diagnosticou persist&#234;ncia de canal arterial de grande calibre e hipertens&#227;o arterial pulmonar. Com este diagn&#243;stico a paciente foi encaminhada a estudo hemodin&#226;mico. A manometria revelou discreta eleva&#231;&#227;o das press&#245;es nas c&#226;maras direitas e art&#233;ria pulmonar. Havia salto oxim&#233;trico ao n&#237;vel do tronco da art&#233;ria pulmonar. N&#227;o foi encontrada persist&#234;ncia de canal arterial. A cinecorona riogradia revelou volumosas f&#237;stulas da coron&#225;ria direita (<xref ref-type="fig" rid="fig01">fig.1</xref>) e da coron&#225;ria esquerda (<xref ref-type="fig" rid="fig02">fig. 2</xref>) para a art&#233;ria pulmonar. </p>

    <p>Decidiu-se por emboliza&#231;&#227;o percut&#226;nea, utilizando-se como via de acesso pun&#231;&#227;o da art&#233;ria femoral direita, com cateter-guia JR 4 6F (Cordis, brite tip) para a coron&#225;ria direita e JL 4 6F para coron&#225;ria esquerda. Ap&#243;s administra&#231;&#227;o endovenosa de 5000 U de heparina n&#227;o fracionada, volumosa f&#237;stula do &#243;stio da coron&#225;ria direita foi microcateterizada seletivamente (Excelsior microcatheter, Boston Scientific). Foram liberados coils de libera&#231;&#227;o controlada at&#233; oclus&#227;o da f&#237;stula (MicroPlex, Microvention), usando-se microcateteres para a catereriza&#231;&#227;o seletiva de duas volumosas f&#237;stulas nos ter&#231;os proximal e m&#233;dio da art&#233;ria descendente anterior, sucessivamente oclu&#237;das com o mesmo tipo de micro-molas. Esses coils s&#227;o produzidos pela Microvention, novas micro-molas de platina introduzidas no mercado em 2002 para emboliza&#231;&#227;o de aneurismas cerebrais. Foram usadas ao todo 11 desses dispositivos para emboliza&#231;&#227;o das f&#237;stulas, 2 microplex 4mm/8cm, 2 de 6mm/15cm, 2 de 8mm/20cm, 1 de 4mm/10cm, 1 de 5mm/12cm, 1 de 7mm/18cm, 1 de 7mm/30cmm e 1 de 9/30cm (<xref ref-type="fig" rid="fig03">fig. 3</xref>). </p>

    <p>Restou um fluxo residual na f&#237;stula da coron&#225;ria direita por um ramo lateral que emergia antes do local de onde as molas foram liberadas. A paciente tolerou bem o procedimento sob anestesia local. N&#227;o se observou emboliza&#231;&#227;o distal para os ramos das coron&#225;rias. </p>
    <p>Ap&#243;s o procedimento a paciente relatou uma grande melhora dos sintomas, recebendo alta hospitalar ap&#243;s dois dias do procedimento em uso de atenolol 25mg/dia. </p>
    <p>O ecodopplercardiograma realizado oito meses ap&#243;s o procedimento, apresentou cavidades normais sem fluxos an&#244;malos. O eletrocardiograma mostrou revers&#227;o do bloqueio completo do ramo esquerdo. A cinecoronariogradia tardia de controle revelou oclus&#227;o completa das duas f&#237;stulas da coron&#225;ria esquerda (<xref ref-type="fig" rid="fig04">fig 4</xref>). O ramo lateral da f&#237;stula da coron&#225;ria direita n&#227;o trombosou, como esperado, persistiu e desenvolveu-se. Apesar da paciente se encontrar assintom&#225;tica, optamos por novo procedimento de emboliza&#231;&#227;o com bal&#245;es destac&#225;veis. Ap&#243;s cateteriza&#231;&#227;o seletiva do ramo fistuloso com JR 4 8F, a f&#237;stula foi oclu&#237;da com dois bal&#245;es (GVB 16, Cathnet-Science) (<xref ref-type="fig" rid="fig05">fig. 5</xref>). </p>

    <p>
      Discuss&#227;o
    </p>
    <p>As f&#237;stulas coron&#225;rias podem ocorrer entre essas art&#233;rias e as c&#226;maras card&#237;acas ou outros vasos, podendo ter origem cong&#234;nita, traum&#225;tica ou iatrog&#234;nica1. A repercuss&#227;o hemodin&#226;mica depende do s&#237;tio de origem, t&#233;rmino e tamanho da f&#237;stula2. F&#237;stulas de ambas coron&#225;rias representam apenas 5% do total. </p>
    <p>A maioria das f&#237;stulas coron&#225;rias &#233; pequena, sem repercuss&#227;o hemodin&#226;mica. Nas f&#237;stulas maiores pode haver roubo de fluxo, com isquemia resultante no segmento do mioc&#225;rdio perfundido pela coron&#225;ria distal &#224; f&#237;stula. H&#225; dilata&#231;&#227;o compensat&#243;ria do segmento proximal. </p>
    <p>As f&#237;stulas de alto d&#233;bito podem resultar em sintomas ou seq&#252;elas3. Nas altera&#231;&#245;es que podem ser encontradas incluem-se: isquemia mioc&#225;rdica cr&#244;nica e angina, insufici&#234;ncia card&#237;aca e cardiomiopatia, infarto do mioc&#225;rdio, hipertens&#227;o pulmonar, endocardite, arritmias, trombose da f&#237;stula e ruptura, que &#233; rara. </p>
    <p>Muitos pacientes s&#227;o referidos devido a sopro cont&#237;nuo na borda esternal esquerda. Nas f&#237;stulas grandes pode haver altera&#231;&#245;es eletrocardiogr&#225;ficas. O ecocardiograma transtor&#225;cico n&#227;o consegue, na maioria dos casos em adultos, determinar os locais de in&#237;cio e t&#233;rmino do shunt, sendo o ecotransesof&#225;gico mais adequado para um diagn&#243;stico preciso. </p>
    <p>O tamanho e caracter&#237;stica anat&#244;mica das f&#237;stulas podem ser estabelecidos com precis&#227;o pela angiografia coronariana. </p>
    <p>F&#237;stulas sem repercuss&#227;o, clinicamente silenciosas n&#227;o merecem tratamento. A profilaxia para endocardite &#233; controversa. </p>
    <p>F&#237;stulas volumosas devem ser ligadas3. Os m&#233;todos cir&#250;rgicos t&#234;m bons resultados9,10. </p>
    <p>Nos &#250;ltimos anos, a despeito dos bons resultados com a cirurgia, os m&#233;todos percut&#226;neos menos invasivos t&#234;m se tornado o tratamento de escolha. V&#225;rios dispositivos t&#234;m sido empregados, incluindo coils de Gianturco, coils destac&#225;veis, bal&#245;es destac&#225;veis, polyvinil &#225;lcool, double umbrellas, duto oclusor de Amplatzer4-8. O risco do tratamento percut&#226;neo inclui infarto do mioc&#225;rdio e emboliza&#231;&#227;o dos dispositivos para estruturas vasculares extracard&#237;acas. </p>
    <p>No caso presente, a cinecoronariogradia de controle tardio mostrou completa oclus&#227;o das f&#237;stulas da coron&#225;ria esquerda e a f&#237;stula residual da coron&#225;ria direita p&#244;de ser tratada com sucesso com bal&#245;es destac&#225;veis. O uso de dispositivos percut&#226;neos variados para oclus&#227;o de m&#250;ltiplas f&#237;stulas de ambas coron&#225;rias, cursando com sintomas de insufici&#234;ncia card&#237;aca, torna este caso peculiar.</p>

      </p>
      <p> A 43-year-old symptomatic woman (dyspnea and palpitation) had multiple coronary-pulmonary artery fistulae with high output; percutaneous embolization was successfully performed using controlled-release microcoils and disposable balloons. </p>
      <p>Coronary fistulae are communications between these arteries and the cardiac cavities or other mediastinal vessels; frequently, they are alterations in embryo development 1. Several fistulae arising from both coronaries and ending in the pulmonary artery, such as in our patient's case, are rare clinical entities 2. Fistulae with marked hemodynamic repercussions leading to symptoms of heart failure and thoracic pain are, classically, treated with surgery 3; however, catheter occlusion is currently increasing in frequency 4-8.</p>
      <p>
        Case Report
      </p>
      <p>A 43-year-old woman was referred to the hospital because of frequent episodes of palpitations and exercise-related dyspnea, with symptoms progressing in recent years. Specific clinical examination revealed a continuous cardiac murmur, heard at the left sternal border at the third and fourth intercostal spaces. Twelve-lead electrocardiography revealed complete left bundle-branch block. Echocardiographic Doppler performed at another hospital showed large patent ductus arteriosus and pulmonary artery hypertension. The patient was referred for hemodynamic study because of this diagnosis. Manometry revealed a mild increase in pressure in the right chamber and pulmonary artery. A pulse oximetric recording peaked at the level of the pulmonary artery trunk. Patent ductus arteriosus was not found. Cardiac scintigraphy demonstrated large right coronary (<xref ref-type="fig" rid="fig01-body2">fig.1</xref>) and left coronary fistulae (<xref ref-type="fig" rid="fig02-body2">fig. 2</xref>) in the pulmonary artery. </p>

      <p>Percutaneous embolization was performed, using the right femoral artery for Access with a JR 4 6F guidewire catheter (Cordis, brite tip) for the right coronary and a JL 4 6F for the left coronary. After venous administration of 5000 U unfractionated heparin, a large fistula of the right coronary ostium was catheterized selectively (Excelsior microcatheter, Boston Scientific). Controlled-release coils were used to completely occlude fistula (MicroPlex, Microvention). Microcatheters were used to perform catheterization of the 2 large fistulae in the proximal and medium third of the anterior descending artery, the fistula being successively occluded with the same type of microcoils. These coils are produced by Microvention. New platinum microcoils were introduced into the market in 2002 for the embolization of brain aneurysms. We used 11 devices for the embolization of the fistulae: 2 microplex 4mm/8cm, 2 microplex 6mm/15cm, 2 microplex 8mm/20cm, 1 microplex 4mm/10cm, 1 microplex 5mm/12cm, 1 microplex 7mm/18cm, 1 microplex 7mm/30cmm, and 1 microplex 9/30cm (<xref ref-type="fig" rid="fig03-body2">fig. 3</xref>). </p>

      <p>A residual flow was left in the right coronary fistula by a lateral branch emerging before the site where the coils were released. The patient under local anesthesia tolerated the procedure well. Distal embolization was not observed for the coronary branches. </p>
      <p>After the procedure, the patient reported a sizeable improvement in symptoms and was discharged 2 days after the procedure, receiving atenolol 25mg/day. </p>
      <p>Doppler echocardiography performed 8 months after the procedure revealed normal cavities without anomalous flows. An electrocardiogram demonstrated complete reversion of the left bundle-branch block. Late coronary angiography revealed complete occlusion of the 2 fistulae of the left coronary artery (<xref ref-type="fig" rid="fig04-body2">fig 4</xref>). The lateral branch of the fistula in the right coronary artery did not have the expected embolization, persisting and evolving. Although the patient was asymptomatic, we opted for a new embolization procedure with disposable balloons. After selective catheterization of the fistulous branch by using JR 4 8F, the fistula was occluded with 2 balloons (GVB 16, Cathnet-Science) (<xref ref-type="fig" rid="fig05-body2">fig. 5</xref>).</p>

      <p>
        Discussion
      </p>
      <p>Coronary fistulae may occur between these arteries and the cardiac chamber or other vessels; they may be congenital, traumatic, or iatrogenic 1. Hemodynamic repercussion depends on the site of origin, ending, and size of the fistula 2. Fistulae from both coronaries represent only 5% of the total. </p>
      <p>Most coronary fistulae are small, without hemodynamic repercussions. In larger fistulae, myocardial ischemia may be due to a "coronary steal phenomenon", resulting in ischemia of the myocardial segment caused by the coronary distal to the fistula. Compensatory dilation occurs in the proximal segment. </p>
      <p>High-debt fistulae may result in symptoms or sequelae 3. Among the alterations that may be found are chronic myocardial ischemia and angina, heart failure and cardiomyopathy, myocardial infarction, pulmonary hypertension, endocarditis, arrhythmias, fistula thrombosis, and rupture, which is rare. </p>
      <p>Many patients are referred to the hospital due to continuous murmur in the left sternal border. In larger fistulae, electrocardiography alterations may occur. Transthoracic echocardiography, in most adult cases, cannot determine the beginning and the end of the shunt, and transesophageal echocardiography is more suitable for an accurate diagnosis. </p>
      <p> The size and anatomical characteristics of the fistulae may be accurately established by coronary angiography. </p>
      <p>Fistulae without complications and clinically silent should not be treated. Prophylaxis for endocarditis is controversial. Large fistulae should be linked 3, and surgical methods have good results 9,10. </p>
      <p>In recent years, despite the satisfactory results of surgery, less invasive percutaneous methods have become the treatment of choice. Several devices have been used, including Gianturco's coils, disposable coils, disposable balloons, alcohol polyvinyl, double umbrellas, and the Amplatzer occluder 4-8. The risks of percutaneous treatment include myocardial infarction and embolization of the devices used for other extracardiac vascular structures. </p>
      <p>In the present case, coronary angiography for late control demonstrated complete occlusion of the left coronary fistulae, and the residual fistula of the right coronary may be successfully treated with disposable balloons. The use of several percutaneous devices for occlusion of several fistulae from both coronaries, occurring with symptoms of heart failure makes this a peculiar case.</p>
