  <body>
    <p>O acidente vascular cerebral (AVC) continua sendo uma das principais causas de morte e incapacidade em todo o mundo, com eventos isqu&#234;micos respons&#225;veis por aproximadamente 87% dos casos. Apesar da investiga&#231;&#227;o etiol&#243;gica exaustiva, at&#233; 25% dos AVCs isqu&#234;micos permanecem sem explica&#231;&#227;o &#8212; o chamado AVC criptog&#234;nico &#8212; uma designa&#231;&#227;o que acarreta implica&#231;&#245;es significativas para a preven&#231;&#227;o secund&#225;ria, dada uma taxa m&#233;dia de recorr&#234;ncia de 3&#8211;6% ao ano.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref> A fibrila&#231;&#227;o atrial (FA) tem sido, por muito tempo, o principal fator de risco cardioemb&#243;lico nesse contexto; no entanto, evid&#234;ncias crescentes sugerem que anormalidades atriais estruturais e funcionais &#8212; coletivamente englobadas pelo conceito de cardiomiopatia atrial (CA) &#8212; podem impulsionar eventos tromboemb&#243;licos por meio de uma via independente, mesmo na aus&#234;ncia de arritmia manifesta.<xref ref-type="bibr" rid="B3">3</xref> A declara&#231;&#227;o de consenso atualizada de 2024 da EHRA, HRS, APHRS e LAHRS consolida um sistema de estadiamento para CA integrando biomarcadores, geometria atrial e par&#226;metros eletrofisiol&#243;gicos, estabelecendo firmemente a CA como uma entidade patol&#243;gica distinta da &#8212; embora intimamente associada &#224; &#8212; FA.<xref ref-type="bibr" rid="B4">4</xref> Nesse contexto, o eletrocardiograma (ECG) de 12 deriva&#231;&#245;es &#8212; simples, barato e universalmente dispon&#237;vel &#8212; oferece uma abordagem pr&#225;tica para a detec&#231;&#227;o de CA por meio de dois marcadores estabelecidos: dura&#231;&#227;o prolongada da onda P (&gt;120 ms) e for&#231;a terminal da onda P elevada na deriva&#231;&#227;o V1 (PTFV1 &gt;4.000 &#956;V&#183;ms). Estudos anteriores do Cardiovascular Health Study (CHS), do Multi-Ethnic Study of Atherosclerosis (MESA) e da coorte Atherosclerosis Risk in Communities (ARIC) demonstraram que esses marcadores predizem independentemente o AVC isqu&#234;mico;<xref ref-type="bibr" rid="B5">5</xref>&#8211;<xref ref-type="bibr" rid="B7">7</xref> no entanto, sua relev&#226;ncia em popula&#231;&#245;es latino-americanas, com um perfil cardiovascular e sociodemogr&#225;fico distinto, permaneceu uma importante lacuna de evid&#234;ncias.</p>
    <p>Nesta edi&#231;&#227;o dos Arquivos Brasileiros de Cardiologia, o estudo de L&#234;u et al.<xref ref-type="bibr" rid="B8">8</xref> aborda essa lacuna com not&#225;vel rigor. Utilizando o conjunto de dados CODE-BH &#8212; um subconjunto da coorte Clinical Outcomes in Digital Electrocardiography (CODE), um dos maiores bancos de dados de ECG digital anotado do mundo<xref ref-type="bibr" rid="B9">9</xref> &#8212; os autores reuniram uma coorte retrospectiva de 245.588 pacientes com idade &#8805;40 anos em ritmo sinusal, que realizaram ECGs em Belo Horizonte entre 2006 e 2018. Os dados eletrocardiogr&#225;ficos foram vinculados probabilisticamente aos bancos de dados do Sistema &#218;nico de Sa&#250;de (SUS) &#8212; o Sistema de Informa&#231;&#245;es sobre Mortalidade (SIM) e o Sistema de Informa&#231;&#245;es Hospitalares (SIH) &#8212; permitindo a identifica&#231;&#227;o de &#243;bitos e interna&#231;&#245;es por AVC, mortalidade cardiovascular, FA incidente e s&#237;ndrome coronariana aguda durante um acompanhamento m&#233;dio de 3,5 anos. Ap&#243;s ajuste incremental para idade, sexo, fatores de risco cardiovascular e hipertrofia ventricular esquerda, tanto a dura&#231;&#227;o prolongada da onda P (HR 1,24; IC 95% 1,12&#8211;1,36) quanto a PTFV1 elevada (HR 1,20; IC 95% 1,05&#8211;1,38) foram associadas independentemente ao desfecho composto de morte ou hospitaliza&#231;&#227;o por AVC. A dura&#231;&#227;o prolongada da onda P tamb&#233;m foi associada &#224; mortalidade cardiovascular e &#224; incid&#234;ncia de FA, enquanto a PTFV1 elevada foi associada &#224; mortalidade por todas as causas e &#224; mortalidade cardiovascular.</p>
    <p>Diversos pontos fortes metodol&#243;gicos merecem reconhecimento. O tamanho da amostra &#8212; o maior entre estudos compar&#225;veis at&#233; o momento &#8212; confere consider&#225;vel poder estat&#237;stico, compensando efetivamente o per&#237;odo m&#233;dio de acompanhamento relativamente curto. A plataforma CODE j&#225; gerou contribui&#231;&#245;es de influ&#234;ncia internacional, incluindo trabalhos pioneiros sobre interpreta&#231;&#227;o de ECG baseada em redes neurais profundas<xref ref-type="bibr" rid="B10">10</xref> e idade biol&#243;gica derivada do ECG como preditor de mortalidade,<xref ref-type="bibr" rid="B11">11</xref> posicionando esta coorte como uma plataforma de relev&#226;ncia cient&#237;fica global. A ades&#227;o &#224;s diretrizes de relato STROBE fortalece ainda mais a transpar&#234;ncia metodol&#243;gica, e o uso da an&#225;lise automatizada de ECG de Glasgow garante a reprodutibilidade em todos os contextos de aten&#231;&#227;o prim&#225;ria de onde a coorte foi extra&#237;da. &#201; importante destacar que este estudo constitui a primeira investiga&#231;&#227;o de marcadores de CA baseados em ECG e desfechos cardiovasculares adversos em uma popula&#231;&#227;o brasileira, conferindo-lhe particular relev&#226;ncia para a pr&#225;tica cardiol&#243;gica regional.</p>
    <p>Como em todos os estudos que utilizam bases de dados administrativas de sa&#250;de, certas caracter&#237;sticas do desenho do estudo tamb&#233;m definem oportunidades para pesquisas futuras. O uso de c&#243;digos da CID-10 do SIM e do SIH para identificar eventos de AVC &#8212; abrangendo diagn&#243;sticos isqu&#234;micos, hemorr&#225;gicos e cerebrovasculares n&#227;o especificados &#8212; reflete uma caracter&#237;stica inerente &#224; metodologia de coorte eletr&#244;nica em larga escala e abre uma via de pesquisa promissora: estudos futuros que relacionem esses marcadores de ECG a subtipos de AVC confirmados por neuroimagem e a sistemas de classifica&#231;&#227;o etiol&#243;gica validados, como o TOAST<xref ref-type="bibr" rid="B12">12</xref> ou os crit&#233;rios ESUS,<xref ref-type="bibr" rid="B13">13</xref> permitiriam aos pesquisadores determinar se a associa&#231;&#227;o com a CA &#233; impulsionada especificamente por eventos cardioemb&#243;licos &#8212; como a plausibilidade biol&#243;gica da hip&#243;tese preveria &#8212; e quantificar a contribui&#231;&#227;o relativa da CA para cada mecanismo de AVC. Da mesma forma, o contexto de aten&#231;&#227;o prim&#225;ria da coorte, embora limite a generaliza&#231;&#227;o para popula&#231;&#245;es cl&#237;nicas de maior risco, ilustra precisamente a amplitude e a efici&#234;ncia com que o rastreamento de CA baseado em ECG poderia ser implementado em n&#237;vel populacional. A subestima&#231;&#227;o da incid&#234;ncia de FA devido a registros de ECG em um &#250;nico momento destaca o potencial de estrat&#233;gias de monitoramento cont&#237;nuo &#8212; incluindo gravadores de eventos implant&#225;veis e ECG ambulatorial aprimorado por IA &#8212; para refinar a estratifica&#231;&#227;o de risco quando combinadas com marcadores da onda P em futuros protocolos prospectivos.</p>
    <p>O panorama terap&#234;utico para a preven&#231;&#227;o de AVC relacionado &#224; anticoagula&#231;&#227;o permanece uma fronteira ativa. O estudo ARCADIA, que randomizou pacientes com AVC criptog&#234;nico e evid&#234;ncia de cardiopatia atrial para apixabana versus aspirina, n&#227;o demonstrou redu&#231;&#227;o significativa na recorr&#234;ncia de AVC com anticoagula&#231;&#227;o na aus&#234;ncia de FA documentada,<xref ref-type="bibr" rid="B14">14</xref> ressaltando que marcadores atriais estruturais isoladamente podem ser insuficientes para orientar as decis&#245;es de anticoagula&#231;&#227;o na pr&#225;tica atual e que a identifica&#231;&#227;o de fen&#243;tipos espec&#237;ficos de alto risco de anticoagula&#231;&#227;o permanece uma necessidade cr&#237;tica n&#227;o atendida. Uma revis&#227;o sistem&#225;tica e meta-an&#225;lise recentes confirmam que a dura&#231;&#227;o da onda P basal &#233; significativamente prolongada em sobreviventes de AVC criptog&#234;nico que posteriormente desenvolvem FA, refor&#231;ando o ECG como uma ferramenta pr&#225;tica de triagem para selecionar pacientes que podem se beneficiar de monitoramento card&#237;aco prolongado.<xref ref-type="bibr" rid="B15">15</xref> As Diretrizes da ESC de 2024 sobre FA, que introduziram a estrutura AF-CARE centrada no paciente, reconhecem explicitamente o papel crescente da avalia&#231;&#227;o do substrato atrial na preven&#231;&#227;o de AVC, fornecendo um contexto regulat&#243;rio oportuno no qual os presentes achados s&#227;o particularmente relevantes.<xref ref-type="bibr" rid="B16">16</xref></p>
    <p>Em resumo, o estudo<xref ref-type="bibr" rid="B8">8</xref> representa uma contribui&#231;&#227;o significativa para a compreens&#227;o crescente da CA como um fator de risco independente para AVC e mortalidade cardiovascular. Seus achados corroboram o valor progn&#243;stico de par&#226;metros eletrocardiogr&#225;ficos amplamente dispon&#237;veis e refor&#231;am a hip&#243;tese de que a doen&#231;a do substrato atrial pode representar uma via distinta e pass&#237;vel de interven&#231;&#227;o na preven&#231;&#227;o do AVC cardioemb&#243;lico. O estudo tamb&#233;m abre caminho para futuras investiga&#231;&#245;es prospectivas capazes de alcan&#231;ar a resolu&#231;&#227;o etiol&#243;gica e a granularidade cl&#237;nica necess&#225;rias para traduzir essas associa&#231;&#245;es em estrat&#233;gias de preven&#231;&#227;o eficazes &#8212; e, em &#250;ltima an&#225;lise, reduzir a carga de AVC criptog&#234;nico na popula&#231;&#227;o brasileira e latino-americana em geral.</p>

    <body>
      <p>Stroke remains one of the foremost causes of death and disability globally, with ischemic events accounting for approximately 87% of cases. Despite exhaustive etiological investigation, up to 25% of ischemic strokes remain unexplained &#8212; the so-called cryptogenic stroke &#8212; a designation that carries significant implications for secondary prevention, given an average recurrence rate of 3&#8211;6% per year.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref> Atrial fibrillation (AF) has long been the dominant cardioembolic risk factor in this setting; however, mounting evidence suggests that structural and functional atrial abnormalities &#8212; collectively encompassed by the concept of atrial cardiomyopathy (AC) &#8212; may drive thromboembolic events through an independent pathway, even in the absence of overt arrhythmia.<xref ref-type="bibr" rid="B3">3</xref> The 2024 updated consensus statement from EHRA, HRS, APHRS, and LAHRS consolidates a staging system for AC integrating biomarkers, atrial geometry, and electrophysiological parameters, firmly establishing AC as a pathological entity distinct from &#8212; though intimately associated with &#8212; AF.<xref ref-type="bibr" rid="B4">4</xref> In this context, the 12-lead electrocardiogram (ECG) &#8212; simple, inexpensive, and universally available &#8212; offers a practical approach to AC detection through two established markers: prolonged P-wave duration (&gt;120 ms) and elevated P-wave terminal force in lead V1 (PTFV1 &gt;4,000 &#956;V&#183;ms). Prior studies from the Cardiovascular Health Study (CHS), the Multi-Ethnic Study of Atherosclerosis (MESA), and the Atherosclerosis Risk in Communities (ARIC) cohort demonstrated that these markers independently predict ischemic stroke;<xref ref-type="bibr" rid="B5">5</xref>&#8211;<xref ref-type="bibr" rid="B7">7</xref> yet their relevance in Latin American populations, with a distinct cardiovascular and sociodemographic profile, had remained an important evidence gap.</p>
      <p>In this issue of Arquivos Brasileiros de Cardiologia, the study by L&#234;u et al.<xref ref-type="bibr" rid="B8">8</xref> addresses this gap with notable rigour. Drawing upon the CODE-BH dataset &#8212; a subset of the Clinical Outcomes in Digital Electrocardiography (CODE) cohort, one of the world's largest annotated digital ECG databases<xref ref-type="bibr" rid="B9">9</xref> &#8212; the authors assembled a retrospective cohort of 245,588 patients aged &#8805;40 years in sinus rhythm, who underwent ECGs in Belo Horizonte between 2006 and 2018. Electrocardiographic data were probabilistically linked to the Brazilian Unified Health System (SUS) databases &#8212; the Mortality Information System (SIM) and the Hospital Information System (SIH) &#8212; enabling ascertainment of stroke-related deaths and hospitalizations, cardiovascular mortality, incident AF, and acute coronary syndrome over a mean follow-up of 3.5 years. After incremental adjustment for age, sex, cardiovascular risk factors, and left ventricular hypertrophy, both prolonged P-wave duration (HR 1.24; 95% CI 1.12&#8211;1.36) and elevated PTFV1 (HR 1.20; 95% CI 1.05&#8211;1.38) were independently associated with the composite endpoint of stroke-related death or hospitalization. Prolonged P-wave duration was further associated with cardiovascular mortality and incident AF, whilst elevated PTFV1 was linked to all-cause and cardiovascular mortality.</p>
      <p>Several methodological strengths deserve recognition. The sample size &#8212; the largest among comparable studies to date &#8212; confers considerable statistical power, effectively offsetting the relatively short mean follow-up. The CODE platform has already yielded internationally influential contributions, including pioneering work on deep neural network-based ECG interpretation<xref ref-type="bibr" rid="B10">10</xref> and ECG-derived biological age as a mortality predictor,<xref ref-type="bibr" rid="B11">11</xref> positioning this cohort as a platform of global scientific relevance. Adherence to STROBE reporting guidelines further strengthens methodological transparency, and the use of automated Glasgow ECG analysis ensures reproducibility across the primary care settings from which the cohort was drawn. Importantly, this study constitutes the first investigation of ECG-based AC markers and adverse cardiovascular outcomes in a Brazilian population, lending it particular significance for regional cardiology practice.</p>
      <p>As with all studies leveraging administrative health databases, certain design characteristics also define opportunities for future research. The use of ICD-10 codes from SIM and SIH to identify stroke events &#8212; encompassing ischemic, hemorrhagic, and unspecified cerebrovascular diagnoses &#8212; reflects an inherent feature of large-scale electronic cohort methodology, and opens a compelling research avenue: future studies linking these ECG markers to neuroimaging-confirmed stroke subtypes and validated etiological classification systems, such as TOAST<xref ref-type="bibr" rid="B12">12</xref> or the ESUS criteria,<xref ref-type="bibr" rid="B13">13</xref> would allow investigators to determine whether the association with AC is driven specifically by cardioembolic events &#8212; as the biological plausibility of the hypothesis would predict &#8212; and to quantify the relative contribution of AC to each stroke mechanism. Similarly, the primary care setting of the cohort, whilst limiting generalizability to higher-risk clinical populations, illustrates precisely how widely and efficiently ECG-based AC screening could be deployed at a population level. The underestimation of incident AF due to single-timepoint ECG recordings highlights the potential of continuous monitoring strategies &#8212; including implantable loop recorders and AI-enhanced ambulatory ECG &#8212; to refine risk stratification when combined with P-wave markers in future prospective protocols.</p>
      <p>The therapeutic landscape for AC-related stroke prevention remains an active frontier. The ARCADIA trial, which randomized patients with cryptogenic stroke and evidence of atrial cardiopathy to apixaban versus aspirin, did not demonstrate a significant reduction in recurrent stroke with anticoagulation in the absence of documented AF,<xref ref-type="bibr" rid="B14">14</xref> underscoring that structural atrial markers alone may be insufficient to guide anticoagulation decisions in current practice, and that identification of specific high-risk AC phenotypes remains a critical unmet need. A recent systematic review and meta-analysis confirms that baseline P-wave duration is significantly prolonged in cryptogenic stroke survivors who subsequently develop AF, reinforcing the ECG as a practical triage tool for selecting patients who may benefit from extended cardiac monitoring.<xref ref-type="bibr" rid="B15">15</xref> The 2024 ESC Guidelines on AF, which introduced the patient-centered AF-CARE framework, explicitly acknowledge the growing role of atrial substrate assessment in stroke prevention, providing a timely regulatory context within which the present findings are particularly resonant.<xref ref-type="bibr" rid="B16">16</xref></p>
      <p>In summary, the study<xref ref-type="bibr" rid="B8">8</xref> constitutes a meaningful contribution to the evolving understanding of AC as an independent risk factor for stroke and cardiovascular mortality. Its findings substantiate the prognostic value of widely available ECG parameters and reinforce the hypothesis that atrial substrate disease may represent a distinct, targetable pathway in cardioembolic stroke prevention. The study also paves the way for future prospective investigations capable of achieving the etiological resolution and clinical granularity needed to translate these associations into actionable prevention strategies &#8212; and ultimately to reduce the burden of cryptogenic stroke in the Brazilian and broader Latin American population.</p>
