          <title>Resultados</title>
          <p> Dos 203 pacientes identificados, 54 foram exclu&#237;dos devido a dados incompletos. A coorte final foi composta por 149 pacientes (89,9% do sexo feminino), com mediana de idade ao diagn&#243;stico de 31 anos. Ao diagn&#243;stico, 92,6% apresentavam sintomas. A claudica&#231;&#227;o dos membros superiores (36,2%) e inferiores (30,9%) foi frequente, juntamente com les&#227;o vascular avan&#231;ada, como estenose (85,9%) e oclus&#245;es (52,3%). A claudica&#231;&#227;o dos membros superiores foi predita independentemente pela redu&#231;&#227;o dos pulsos nos membros superiores (OR=4,83; IC 95%=2,08-11,24; p=0,001) e pela oclus&#227;o da art&#233;ria subcl&#225;via direita (OR=8,06; IC 95%=1,94-33,44; p=0,004). A claudica&#231;&#227;o dos membros inferiores foi prevista pela oclus&#227;o da art&#233;ria subcl&#225;via direita (OR=6,65; IC 95%=2,05-21,61; p=0,002), espessamento da art&#233;ria subcl&#225;via direita (OR=5,12; IC 95%=1,18-22,71; p=0,029) e estenose da art&#233;ria subcl&#225;via esquerda (OR=2,71; IC 95%=1,21-60,56; p=0,016). Durante um acompanhamento mediano de 10 anos, apesar da remiss&#227;o em 91,3% dos casos, houve aumento das comorbidades cardiovasculares e 26,8% dos pacientes necessitaram de cirurgia.</p>
        </sec>
        <sec>
          <title>Conclus&#245;es</title>
          <p> A claudica&#231;&#227;o intermitente dos membros &#233; um importante indicador progn&#243;stico de les&#227;o radiol&#243;gica avan&#231;ada e atraso no diagn&#243;stico. Isso &#233; refor&#231;ado pela dissocia&#231;&#227;o a longo prazo entre alta remiss&#227;o cl&#237;nica e doen&#231;a vascular progressiva, o que exige monitoramento vigilante.</p>

      <title>Introdu&#231;&#227;o</title>
      <p>A arterite de Takayasu (AT) &#233; uma forma rara de vasculite sist&#234;mica prim&#225;ria que afeta predominantemente art&#233;rias de grande calibre, incluindo a aorta e seus principais ramos. A inflama&#231;&#227;o cr&#244;nica desses vasos pode levar a estenoses, oclus&#245;es, ectasias ou aneurismas.<xref ref-type="bibr" rid="B1">1</xref> Nesse contexto, o sistema de classifica&#231;&#227;o proposto por Hata<xref ref-type="bibr" rid="B2">2</xref> para AT facilita a caracteriza&#231;&#227;o anat&#244;mica das altera&#231;&#245;es vasculares e sua correla&#231;&#227;o com os sinais e sintomas cl&#237;nicos observados em pacientes com AT.<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B4">4</xref></p>
      <p>As manifesta&#231;&#245;es cl&#237;nicas mais comuns incluem febre, perda de peso, hipertens&#227;o arterial sist&#234;mica, claudica&#231;&#227;o vascular dos membros, press&#227;o arterial sist&#243;lica reduzida ou assim&#233;trica entre os membros, vertigem e artralgia, entre outras.<xref ref-type="bibr" rid="B5">5</xref>,<xref ref-type="bibr" rid="B6">6</xref> Os achados laboratoriais frequentemente revelam um aumento nos reagentes de fase aguda.<xref ref-type="bibr" rid="B5">5</xref>,<xref ref-type="bibr" rid="B7">7</xref></p>
      <p>No entanto, essas manifesta&#231;&#245;es iniciais s&#227;o heterog&#234;neas, o que dificulta sua caracteriza&#231;&#227;o. Al&#233;m disso, os estudos existentes na literatura n&#227;o estabeleceram correla&#231;&#245;es consistentes entre as manifesta&#231;&#245;es cl&#237;nicas e os achados radiol&#243;gicos iniciais,<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B8">8</xref>-<xref ref-type="bibr" rid="B20">20</xref> principalmente devido &#224; aus&#234;ncia de uma an&#225;lise temporal precisa que permitisse uma distin&#231;&#227;o clara entre manifesta&#231;&#245;es precoces e tardias, bem como entre diferentes tipos de les&#245;es vasculares. Quando avaliadas, essas altera&#231;&#245;es s&#227;o frequentemente descritas de forma gen&#233;rica, sem detalhes quanto ao tipo de les&#227;o (estenose, oclus&#227;o, aneurisma etc.), o que limita a associa&#231;&#227;o entre os achados cl&#237;nicos e as altera&#231;&#245;es vasculares.</p>
      <p>Portanto, o presente estudo teve como objetivo caracterizar as manifesta&#231;&#245;es cl&#237;nicas e radiol&#243;gicas iniciais em uma coorte representativa de pacientes com AT e explorar as associa&#231;&#245;es entre esses par&#226;metros. Al&#233;m disso, o estudo avaliou a progress&#227;o da doen&#231;a e o progn&#243;stico dos pacientes.</p>

        <p>A avalia&#231;&#227;o da normalidade das vari&#225;veis foi realizada por meio do teste de Shapiro-Wilk. Os dados foram apresentados como m&#233;dia e desvio padr&#227;o, mediana e intervalo interquartil, ou frequ&#234;ncias absolutas e relativas, conforme apropriado. Para a an&#225;lise univariada, as compara&#231;&#245;es entre grupos independentes foram realizadas utilizando o teste t de Student n&#227;o pareado para vari&#225;veis cont&#237;nuas com distribui&#231;&#227;o normal e o teste U de Mann-Whitney para vari&#225;veis cont&#237;nuas sem distribui&#231;&#227;o normal. As vari&#225;veis categ&#243;ricas foram comparadas utilizando o teste qui-quadrado ou o teste exato de Fisher, dependendo da distribui&#231;&#227;o de frequ&#234;ncia esperada, ou o teste de McNemar para dados categ&#243;ricos pareados, dependendo do tipo de vari&#225;vel. Todas as vari&#225;veis que apresentaram diferen&#231;a significativa nesta an&#225;lise foram selecionadas para ajuste. As raz&#245;es de chances (OR) ajustadas por sexo e idade, com intervalos de confian&#231;a (IC) de 95%, foram calculadas utilizando um modelo de regress&#227;o log&#237;stica incondicional. O n&#237;vel de signific&#226;ncia foi definido em p &lt; 0,05. O software estat&#237;stico SPSS vers&#227;o 21.0 (Chicago, EUA) foi utilizado para as an&#225;lises.</p>

      <p>Entre 2000 e 2024, foi identificada uma coorte de 203 pacientes diagnosticados com AT. Destes, 54 pacientes foram exclu&#237;dos devido a dados cl&#237;nicos incompletos, 32 devido &#224; falta de informa&#231;&#245;es sobre os sintomas iniciais e manifesta&#231;&#245;es cl&#237;nicas ao diagn&#243;stico e 22 devido a dados radiol&#243;gicos vasculares insuficientes. Consequentemente, a an&#225;lise final incluiu 149 pacientes.</p>
      <p>A idade mediana ao diagn&#243;stico foi de 31,0 anos (intervalo interquartil: 27,0-34,3 anos), e o intervalo mediano entre o in&#237;cio dos sintomas e o diagn&#243;stico foi de 1,0 ano (intervalo interquartil: 0,0-2,3 anos), com 68,0% dos casos diagnosticados em at&#233; 12 meses ap&#243;s o in&#237;cio dos sintomas. Dos 149 pacientes, 80,5% eram do sexo feminino e 81,2% eram de etnia branca.</p>

      <title>Discuss&#227;o</title>
      <p>Nossos achados revelam que, no diagn&#243;stico inicial de AT, 7,4% dos pacientes eram assintom&#225;ticos. Entre aqueles que apresentavam sintomas, as manifesta&#231;&#245;es cl&#237;nicas foram predominantemente inespec&#237;ficas. Aproximadamente metade dos pacientes demonstrou comprometimento vascular significativo, como pulsos diminu&#237;dos e discrep&#226;ncia na press&#227;o arterial sist&#243;lica nos membros superiores. Os exames de imagem revelaram diversas altera&#231;&#245;es relacionadas a sequelas, incluindo estenoses e oclus&#245;es, potencialmente indicativas de diagn&#243;stico tardio da doen&#231;a. A claudica&#231;&#227;o dos membros superiores foi prevista pela redu&#231;&#227;o dos pulsos nos membros superiores e pela oclus&#227;o da art&#233;ria subcl&#225;via direita, enquanto a claudica&#231;&#227;o dos membros inferiores n&#227;o foi prevista por les&#245;es nos vasos infrarrenais, mas sim pelo comprometimento grave das art&#233;rias subcl&#225;vias.</p>
      <p>Os pontos fortes deste estudo incluem uma amostra representativa de pacientes com AT, recrutados de acordo com os crit&#233;rios de classifica&#231;&#227;o EULAR/ACR de 2022.<xref ref-type="bibr" rid="B22">22</xref> Embora este estudo seja retrospectivo, os dados foram coletados sistematicamente a partir de registros eletr&#244;nicos de sa&#250;de, utilizando instrumentos padronizados e pr&#233;-parametrizados, minimizando assim o vi&#233;s de medi&#231;&#227;o e garantindo a qualidade e uniformidade dos dados.</p>
      <p>Notavelmente, diferentemente de outros estudos na literatura,<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B8">8</xref>-<xref ref-type="bibr" rid="B12">12</xref>,<xref ref-type="bibr" rid="B14">14</xref>-<xref ref-type="bibr" rid="B20">20</xref> nossa pesquisa avaliou especificamente as manifesta&#231;&#245;es iniciais detalhadas e sua correla&#231;&#227;o com o progn&#243;stico, considerando tamb&#233;m os eventos cir&#250;rgicos. Por exemplo, Alnabwani et al.<xref ref-type="bibr" rid="B6">6</xref> se focaram exclusivamente em estenoses sem especificar o momento da aquisi&#231;&#227;o das imagens, enquanto Gloor et al.<xref ref-type="bibr" rid="B8">8</xref> identificaram os vasos afetados sem detalhar as altera&#231;&#245;es anat&#244;micas. Da mesma forma, Cong et al.<xref ref-type="bibr" rid="B9">9</xref> e Vanoli et al.<xref ref-type="bibr" rid="B10">10</xref> ou n&#227;o definiram claramente o per&#237;odo da aquisi&#231;&#227;o das imagens ou n&#227;o diferenciaram consistentemente entre os achados radiol&#243;gicos iniciais e tardios.</p>
      <p>Numerosos estudos<xref ref-type="bibr" rid="B11">11</xref>,<xref ref-type="bibr" rid="B12">12</xref>,<xref ref-type="bibr" rid="B14">14</xref>-<xref ref-type="bibr" rid="B16">16</xref> n&#227;o especificaram o momento das altera&#231;&#245;es radiol&#243;gicas, o que impede uma correla&#231;&#227;o precisa com os sintomas iniciais. Mesmo estudos que consideraram achados precoces, como o de Karabacak et al.,<xref ref-type="bibr" rid="B17">17</xref> n&#227;o especificaram o tipo de anormalidades radiol&#243;gicas detectadas. Al&#233;m disso, as pesquisas conduzidas por Thakare et al.<xref ref-type="bibr" rid="B18">18</xref> e Misra et al.,<xref ref-type="bibr" rid="B19">19</xref> embora abordassem aspectos iniciais, limitaram suas an&#225;lises progn&#243;sticas a compara&#231;&#245;es espec&#237;ficas &#8212; envolvimento renal ou estado do pulso, respectivamente &#8212; apresentando dados em formato de raz&#227;o, o que restringe a correla&#231;&#227;o abrangente das caracter&#237;sticas cl&#237;nicas e radiol&#243;gicas iniciais com o progn&#243;stico geral. Notavelmente, diferentemente do nosso estudo, esses trabalhos n&#227;o detalharam eventos cir&#250;rgicos. Ademais, o estudo de Uchida et al.,<xref ref-type="bibr" rid="B20">20</xref> apesar de fornecer dados iniciais e progn&#243;sticos, foi limitado por um curto per&#237;odo de acompanhamento de apenas dois anos e tamb&#233;m omitiu relatos de interven&#231;&#245;es cir&#250;rgicas.</p>
      <p>Nossos achados revelaram que, no momento do diagn&#243;stico de AT, 92,6% dos pacientes apresentavam sintomas. No entanto, estes exibiam sintomas cl&#237;nicos variados e altamente inespec&#237;ficos, como febre e perda de peso. Em contrapartida, sintomas t&#237;picos, como claudica&#231;&#227;o vascular dos membros superiores e inferiores, embora mais espec&#237;ficos, estavam presentes em menos da metade dos pacientes. A claudica&#231;&#227;o dos membros superiores foi associada exclusivamente &#224; diminui&#231;&#227;o dos pulsos nos membros superiores e &#224; oclus&#227;o da art&#233;ria subcl&#225;via direita, apesar de sua correla&#231;&#227;o inicial com par&#226;metros como tontura e dist&#250;rbios visuais. Este achado sugere que a claudica&#231;&#227;o dos membros superiores serve como um marcador cl&#237;nico significativo de doen&#231;a vascular avan&#231;ada, correlacionando-se com indicadores de dano vascular estabelecido e ressaltando a import&#226;ncia do diagn&#243;stico tardio da doen&#231;a. Nossos resultados indicam ainda que a claudica&#231;&#227;o n&#227;o reflete necessariamente as manifesta&#231;&#245;es cl&#237;nicas iniciais, mas sim altera&#231;&#245;es vasculares espec&#237;ficas, enfatizando a import&#226;ncia do exame f&#237;sico e da avalia&#231;&#227;o radiol&#243;gica na avalia&#231;&#227;o do paciente.</p>
      <p>A claudica&#231;&#227;o dos membros inferiores foi inicialmente associada a sopro card&#237;aco e diversas altera&#231;&#245;es radiol&#243;gicas; a an&#225;lise multivariada n&#227;o revelou associa&#231;&#227;o com les&#245;es dos vasos da aorta infrarrenal ou com os sintomas iniciais. Em vez disso, foi relacionada a les&#245;es vasculares em territ&#243;rios suprarrenais, com preditores independentes incluindo estenose da art&#233;ria subcl&#225;via esquerda, oclus&#227;o da art&#233;ria subcl&#225;via direita e espessamento da art&#233;ria subcl&#225;via direita. Essa correla&#231;&#227;o inesperada sugere que a claudica&#231;&#227;o dos membros inferiores pode n&#227;o refletir um envolvimento arterial localizado, mas sim indicar um comprometimento grave da art&#233;ria subcl&#225;via como marcador do est&#225;gio da doen&#231;a, refletindo um comprometimento vascular significativo com repercuss&#245;es hemodin&#226;micas globais. Assim como na claudica&#231;&#227;o dos membros superiores, esses achados corroboram a hip&#243;tese de um diagn&#243;stico tardio, que ocorre quando o dano vascular j&#225; &#233; funcionalmente significativo, ressaltando a import&#226;ncia dos exames de imagem no manejo do paciente.</p>
      <p>Al&#233;m disso, a preval&#234;ncia de manifesta&#231;&#245;es hemodin&#226;micas em aproximadamente metade dos pacientes no momento do diagn&#243;stico, juntamente com altera&#231;&#245;es vasculares como estenoses e oclus&#245;es na maioria dos casos, fornece evid&#234;ncias para o diagn&#243;stico tardio da AT.</p>
      <p>A an&#225;lise estat&#237;stica se concentrou nos sintomas de claudica&#231;&#227;o vascular devido &#224; sua relev&#226;ncia cl&#237;nica e preval&#234;ncia em nossa coorte. Embora manifesta&#231;&#245;es sist&#234;micas como febre, fadiga e cefaleia fossem prevalentes no momento do diagn&#243;stico, sua natureza inespec&#237;fica limita a capacidade de estabelecer correla&#231;&#245;es diretas e robustas com achados radiol&#243;gicos espec&#237;ficos. Em contrapartida, sintomas mais espec&#237;ficos foram raros, o que impossibilitou uma an&#225;lise estat&#237;stica com poder suficiente para gerar conclus&#245;es v&#225;lidas. Consequentemente, a an&#225;lise foi conduzida com base na claudica&#231;&#227;o dos membros superiores e inferiores para correlacionar as manifesta&#231;&#245;es cl&#237;nicas iniciais e os achados de imagem na AT.</p>
      <p>Nossos resultados revelaram ainda que, embora a grande maioria dos pacientes tenha alcan&#231;ado a remiss&#227;o, houve um aumento not&#225;vel nas comorbidades cardiovasculares e na necessidade de interven&#231;&#245;es cir&#250;rgicas complexas. Isso indica que a remiss&#227;o cl&#237;nica n&#227;o equivale invariavelmente &#224; estabilidade anat&#244;mica ou &#224; revers&#227;o de danos vasculares j&#225; estabelecidos, evidenciando uma dissocia&#231;&#227;o cr&#237;tica entre o controle da atividade inflamat&#243;ria e a progress&#227;o a longo prazo dos danos vasculares.</p>
      <p>A gravidade e a progress&#227;o da doen&#231;a s&#227;o evidenciadas pela alta incid&#234;ncia de interven&#231;&#245;es cir&#250;rgicas, como o implante de endopr&#243;teses em mais de um quarto dos pacientes, al&#233;m de cirurgias de revasculariza&#231;&#227;o e substitui&#231;&#245;es da v&#225;lvula a&#243;rtica. A ocorr&#234;ncia de complica&#231;&#245;es vasculares graves, como dissec&#231;&#227;o a&#243;rtica durante o acompanhamento, acentua ainda mais essa progress&#227;o, indicando que a doen&#231;a pode avan&#231;ar com eventos vasculares extremamente graves. Isso &#233; comprovado pelas causas de &#243;bito, que inclu&#237;ram eventos como acidente vascular cerebral isqu&#234;mico extenso e choque s&#233;ptico. Esses dados ressaltam que, mesmo com o controle da inflama&#231;&#227;o, o dano vascular acumulado continua resultando em morbidade significativa, exigindo monitoramento rigoroso e cont&#237;nuo.</p>
      <p>Este estudo apresenta algumas limita&#231;&#245;es metodol&#243;gicas. Primeiro, seu desenho retrospectivo pode introduzir vi&#233;s de informa&#231;&#227;o e depende da precis&#227;o e completude dos registros m&#233;dicos eletr&#244;nicos. Segundo, por se tratar de um estudo unic&#234;ntrico, os resultados podem n&#227;o ser generaliz&#225;veis para popula&#231;&#245;es mais amplas com diferentes origens &#233;tnicas ou gen&#233;ticas. Essas limita&#231;&#245;es ressaltam a necessidade de futuros estudos prospectivos multic&#234;ntricos para validar e ampliar nossos resultados.</p>

      <title>Conclus&#245;es</title>
      <p>Este estudo elucida que a AT &#233; caracterizada por sintomas inespec&#237;ficos que coincidem com les&#245;es vasculares avan&#231;adas, como estenoses e oclus&#245;es. A correla&#231;&#227;o direta entre manifesta&#231;&#245;es cl&#237;nicas espec&#237;ficas, como claudica&#231;&#227;o, e essas les&#245;es ressalta a import&#226;ncia progn&#243;stica dos achados iniciais. A longo prazo, apesar da alta taxa de remiss&#227;o cl&#237;nica, a doen&#231;a apresentou progress&#227;o, marcada pelo aumento de comorbidades cardiovasculares e pela necessidade de interven&#231;&#245;es cir&#250;rgicas, preenchendo assim uma lacuna na literatura sobre a progress&#227;o da AT. Estudos prospectivos multic&#234;ntricos s&#227;o necess&#225;rios para validar esses achados.</p>

          <title>Methods</title>
          <p> A single-center retrospective cohort study (2000 to 2024) included patients diagnosed with TAK. The significance level was set at p&lt;0.05.</p>

        <sec>
          <title>Results</title>
          <p> Among 203 patients identified, 54 were excluded due to incomplete data. The final cohort comprised 149 patients (89.9% female), with a median age at diagnosis of 31 years. At diagnosis, 92.6% were symptomatic. Claudication of upper (36.2%) and lower (30.9%) limbs was frequent, alongside advanced vascular damage like stenosis (85.9%) and occlusions (52.3%). Upper limb claudication was independently predicted by reduced upper limb pulses (OR=4.83; 95%CI=2.08-11.24; p=0.001) and right subclavian artery occlusion (OR=8.06; 95%CI=1.94-33.44; p=0.004). Lower limb claudication was predicted by right subclavian artery occlusion (OR=6.65; 95%CI=2.05-21.61; p=0.002), right subclavian artery thickening (OR=5.12; 95%CI=1.18-22.71; p=0.029), and left subclavian artery stenosis (OR=2.71; 95%CI=1.21-60.56; p=0.016). Over a median 10-year follow-up, despite 91.3% remission, cardiovascular comorbidities increased, and 26.8% required surgery.</p>
        </sec>
        <sec>
          <title>Conclusions</title>
          <p> Limb claudication is a key prognostic indicator of advanced radiological damage and diagnostic delay. This is reinforced by a long-term dissociation between high clinical remission and progressive vascular disease, demanding vigilant monitoring.</p>

        <title>Introduction</title>
        <p>Takayasu&#8217;s arteritis (TAK) is a rare form of primary systemic vasculitis that predominantly affects large-caliber arteries, including the aorta and its major branches. Chronic inflammation of these vessels can lead to stenosis, occlusions, ectasias, or aneurysms.<xref ref-type="bibr" rid="B1">1</xref> In this context, the classification system proposed by Hata<xref ref-type="bibr" rid="B2">2</xref>for TAK facilitates the anatomical characterization of vascular alterations and their correlation with the clinical signs and symptoms observed in patients with TAK.<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B4">4</xref></p>
        <p>The most common clinical manifestations include fever, weight loss, systemic arterial hypertension, limb vascular claudication, reduced or asymmetric systolic blood pressure between limbs, vertigo, and arthralgia, among others.<xref ref-type="bibr" rid="B5">5</xref>,<xref ref-type="bibr" rid="B6">6</xref> Laboratory findings often reveal an increase in acute phase reactants.<xref ref-type="bibr" rid="B5">5</xref>,<xref ref-type="bibr" rid="B7">7</xref></p>
        <p>However, these initial manifestations are heterogeneous, complicating their characterization. Moreover, existing studies in the literature have not established consistent correlations between clinical manifestations and initial radiological findings,<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B8">8</xref>-<xref ref-type="bibr" rid="B20">20</xref> primarily due to the absence of precise temporal analysis that would allow for a clear distinction between early and late manifestations, as well as between different types of vascular lesions. When evaluated, these changes are frequently described generically, lacking detail regarding the type of lesion (stenosis, occlusion, aneurysm, etc.), which limits the association between clinical findings and vascular alterations.</p>
        <p>Therefore, the present study aimed to characterize the initial clinical and radiological manifestations in a representative cohort of patients with TAK and to explore the associations between these parameters. Additionally, the study evaluated the disease progression and prognosis of the patients.</p>

          <title>Statistical analysis</title>
          <p>It involved evaluating the normality of variables using the Shapiro-Wilk tests. Data were presented as mean and standard deviation, median and interquartile range, or absolute and relative frequencies, as appropriate. For the univariate analysis, comparisons between independent groups were performed using the unpaired Student&#8217;s t-test for normally distributed continuous variables, and the Mann-Whitney U test for non-normally distributed continuous variables. Categorical variables were compared using the chi-square test or Fisher&#8217;s exact test, depending on the expected frequency distribution, or McNemar&#8217;s test for paired categorical data were employed, depending on the variable type. All variables that differed significantly in this analysis were selected for adjustment. The sex-age-adjusted odds ratios (OR) with 95% confidence intervals (CI) were calculated using an unconditional logistic model. The significance level was set at p &lt; 0.05. SPSS version 21.0 statistics software (Chicago, USA) was utilized for the analyses.</p>

Approximately half of the patients exhibited decreased pulses in the upper limbs; a similar proportion showed a difference in systolic pressure between the upper limbs and systemic arterial hypertension. Less frequent manifestations included complete absence of arterial pulses, aortic insufficiency, stroke, and congestive heart failure. Among vascular murmurs, the most prevalent were carotid, abdominal, and cardiac, followed by subclavian and aortic murmurs. Acute phase markers were elevated in approximately 40% of patients.</p>

Lesions predominated in the cervical vessels, particularly in the carotid and subclavian arteries, with a predominance on the left. In the thoracic region, aortic arch aneurysms and thickening of the thoracic aorta were notable. Among the abdominal vessels, the renal arteries were most affected, followed by alterations in the abdominal aorta and celiac trunk artery. In the pelvic region, the most common lesions were stenosis of the iliac arteries.</p>

        <p>In addition, we conducted a comprehensive analysis of the association between the presence of upper or lower limb claudication and the initial clinical and radiological presentations previously detailed in <xref ref-type="table" rid="t1001">Tables 1</xref>, <xref ref-type="table" rid="t2001">2</xref>, and <xref ref-type="table" rid="t3001">3</xref>. The parameters demonstrating a statistical association are presented in <xref ref-type="table" rid="t4001">Table 4</xref>. For the upper limb, significant correlations were identified with dizziness, visual disturbances, diminished upper limb pulses, systemic arterial hypertension, left subclavian artery stenosis, right subclavian artery occlusion, and right subclavian artery thickening (all p&lt;0.05). Regarding the lower limb, notable associations were observed with cardiac murmur, left subclavian artery stenosis, right subclavian artery occlusion, and right subclavian artery thickening (all p&lt;0.05). In the multivariate analysis, the independent predictors for upper limb vascular claudication were reduced upper limb pulses (OR=4.83; 95%CI=2.08-11.24; p&lt;0.001) and right subclavian artery occlusion (OR=8.06; 95%CI=1.94-33.44; p=0.004). For lower limb claudication, the primary predictors were left subclavian artery stenosis (OR=2.71; 95%CI=1.21-60.56; p=0.016), right subclavian artery occlusion (OR=6.65; 95%CI=2.05-21.61; p=0.002), and right subclavian artery thickening (OR=5.12; 95%CI=1.18-22.71; p=0.029).</p>

        <title>Discussion</title>
        <p>Our findings reveal that at the initial diagnosis of TAK, 7.4% of patients were asymptomatic. Among those presenting with symptoms, the clinical manifestations were predominantly nonspecific. Approximately half of the patients demonstrated significant vascular involvement, such as diminished pulses and a systolic blood pressure discrepancy in the upper limbs. Imaging revealed several alterations related to sequelae, including stenoses and occlusions, potentially indicative of the disease&#8217;s late diagnosis. The upper limb claudication was predicted by reduced upper limb pulses and right subclavian artery occlusion, whereas lower limb claudication was not predicted by lesions in infrarenal vessels but rather by severe involvement of the subclavian arteries.</p>
        <p>The strengths of this study include a representative sample of patients with TAK, enrolled according to the most recent 2022 EULAR/ACR classification criteria.<xref ref-type="bibr" rid="B22">22</xref> Although this study was retrospective, data were systematically collected from electronic health records using pre-parameterized, standardized instruments, thereby minimizing measurement bias and ensuring data quality and uniformity.</p>
        <p>Notably, unlike other studies in the literature,<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B8">8</xref>-<xref ref-type="bibr" rid="B12">12</xref>,<xref ref-type="bibr" rid="B14">14</xref>-<xref ref-type="bibr" rid="B20">20</xref> our research specifically evaluated detailed initial manifestations and their correlation with prognosis, while also considering surgical events. For example, Alnabwani et al.<xref ref-type="bibr" rid="B6">6</xref>focused solely on stenoses without specifying imaging timing, whereas Gloor et al.<xref ref-type="bibr" rid="B8">8</xref> identified affected vessels without detailing anatomical alterations. Similarly, Cong et al.<xref ref-type="bibr" rid="B9">9</xref> and Vanoli et al.<xref ref-type="bibr" rid="B10">10</xref>either did not clearly define the imaging timeframe or failed to consistently differentiate between initial and late radiological findings.</p>
        <p>Numerous studies <xref ref-type="bibr" rid="B11">11</xref>,<xref ref-type="bibr" rid="B12">12</xref>,<xref ref-type="bibr" rid="B14">14</xref>-<xref ref-type="bibr" rid="B16">16</xref> have failed to specify the timing of radiological changes, thus impeding precise correlation with initial symptoms. Even studies that considered early findings, such as that by Karabacak M et al.,<xref ref-type="bibr" rid="B17">17</xref> did not specify the type of radiological abnormalities detected. Furthermore, the research conducted by Thakare et al.<xref ref-type="bibr" rid="B18">18</xref>and Misra et al.,<xref ref-type="bibr" rid="B19">19</xref> while addressing initial aspects, limited their prognostic analyses to specific comparisons&#8212;renal involvement or pulse status, respectively&#8212;presenting data in ratio formats, which restricts the comprehensive correlation of initial clinical and radiological features with overall prognosis. Notably, unlike our study, these works did not detail surgical events. Additionally, the study by Uchida et al.,<xref ref-type="bibr" rid="B20">20</xref> despite providing initial and prognostic data, was constrained by a short follow-up period of only two years and also omitted reports of surgical interventions.</p>
        <p>Our findings revealed that at the time of TAK diagnosis, 92.6% of patients were symptomatic. However, they exhibited varied and highly nonspecific clinical symptoms, such as fever and weight loss. In contrast, typical symptoms like vascular claudication of the upper and lower limbs, although more specific, were present in less than half of the patients. The upper limb claudication was exclusively associated with diminished upper limb pulses and right subclavian artery occlusion, despite its initial correlation with parameters such as dizziness and visual disturbances. This finding suggests that upper limb claudication serves as a significant clinical marker of advanced vascular disease, correlating with indicators of established vascular damage and underscoring the late diagnosis of the condition. Our results further indicate that claudication does not necessarily reflect initial clinical manifestations but rather specific vascular alterations, emphasizing the importance of physical examination and radiological assessment in patient evaluation.</p>
        <p>The lower limb claudication was initially associated with cardiac murmur and various radiological alterations; multivariate analysis revealed no association with infrarenal aortic vessel lesions or initial symptoms. Instead, it was linked to vascular lesions in suprarenal territories, with independent predictors including left subclavian artery stenosis, right subclavian artery occlusion, and right subclavian artery thickening. This unexpected correlation suggests that lower claudication may not reflect localized arterial involvement but rather indicates severe subclavian artery involvement as a marker of disease stage, reflecting significant vascular compromise with global hemodynamic repercussions. As with upper claudication, these findings support the hypothesis of a late diagnosis, occurring when vascular damage is already functionally significant, highlighting the importance of imaging studies in patient management.</p>
        <p>Additionally, the prevalence of hemodynamic manifestations in approximately half of the patients at diagnosis, along with vascular alterations such as stenoses and occlusions in the majority, provides evidence for the late diagnosis of TAK.</p>
        <p>The statistical analysis concentrated on vascular claudication symptoms due to their clinical significance and prevalence within our cohort. While systemic manifestations such as fever, fatigue, and headache were prevalent at diagnosis, their nonspecific nature limits the ability to establish direct and robust correlations with specific radiological findings. In contrast, more specific symptoms were rare, precluding a statistical analysis with sufficient power to yield valid conclusions. Consequently, the analysis was conducted based on upper and lower limb claudication to correlate initial clinical manifestations and imaging findings in TAK.</p>
        <p>Our findings further revealed that although a substantial majority of patients achieved remission, there was a notable increase in cardiovascular comorbidities and the necessity for complex surgical interventions. This indicates that clinical remission does not invariably equate to anatomical stability or the reversal of established vascular damage, highlighting a critical dissociation between the management of inflammatory activity and the long-term progression of vascular damage.</p>
        <p>The severity and advancement of the disease are underscored by the high incidence of surgical interventions, such as the implantation of endoprostheses in over a quarter of the patients, alongside revascularization surgeries and aortic valve replacements. The occurrence of severe vascular complications, such as aortic dissection during follow-up, further accentuates this progression, indicating that the disease can advance with extremely severe vascular events. This is evidenced by the causes of death, which included events such as extensive ischemic stroke and septic shock. These data underscore that, even with the control of inflammation, the accumulated vascular damage continues to result in significant morbidity, necessitating rigorous and continuous monitoring.</p>
        <p>This study has some methodological limitations. First, its retrospective design may introduce information bias and depends on the accuracy and completeness of electronic medical records. Second, being a single-center study, the findings may lack generalizability to broader populations with different ethnic or genetic backgrounds. These limitations underscore the need for future prospective, multicenter studies to validate and expand upon our results.</p>

        <title>Conclusions</title>
        <p>This study elucidates that TAK is characterized by nonspecific symptoms that coincide with advanced vascular lesions, such as stenoses and occlusions. The direct correlation between specific clinical manifestations, such as claudication, and these lesions underscores the prognostic significance of the initial findings. Over the long term, despite a high rate of clinical remission, the disease exhibited progression, marked by an increase in cardiovascular comorbidities and the necessity for surgical interventions, thereby addressing a gap in the literature concerning the progression of TAK. Prospective, multicenter studies are warranted to validate these findings.</p>
