        <sec>
          <title>Fundamento</title>
          <p> A doen&#231;a cardiovascular &#233; a principal causa de mortalidade mundial. Estrat&#233;gias que priorizem o diagn&#243;stico precoce podem reduzir a incid&#234;ncia de complica&#231;&#245;es e custo relacionados.</p>
        </sec>
        <sec>
          <title>Objetivo</title>
          <p> Avaliar a custo-efetividade da angiotomografia coronariana (AngioTC) como estrat&#233;gia de investiga&#231;&#227;o inicial de dor tor&#225;cica est&#225;vel em pacientes com probabilidade pr&#233;-teste intermedi&#225;ria de doen&#231;a arterial coronariana (DAC) est&#225;vel, em compara&#231;&#227;o com a angiografia coronariana invasiva (ACI).</p>
        </sec>
        <sec>
          <title>M&#233;todos</title>
          <p> Foi realizada uma an&#225;lise de custo-efetividade comparando AngioTC e ACI considerando dados da sa&#250;de suplementar brasileira. O modelo considerou os custos diretos dos exames diagn&#243;sticos, insumos, custo de interna&#231;&#227;o por infarto e custo de revasculariza&#231;&#227;o mioc&#225;rdica nas 5 regi&#245;es do Brasil. Foi realizada uma an&#225;lise de impacto or&#231;ament&#225;rio referente &#224; incorpora&#231;&#227;o gradual da AngioTC ao longo de 5 anos, utilizando como popula&#231;&#227;o eleg&#237;vel um universo de 100.000 vidas.</p>
        </sec>
        <sec>
          <title>Resultados</title>
          <p> A an&#225;lise de custo-efetividade com AngioTC comparada &#224; ACI, estimada para uma popula&#231;&#227;o de 100.000 vidas, mostrou uma economia de R$ 1.021,00 por vida ou de R$ 102.069.703,00 ao final do quinto ano. Quando considerado o valor m&#233;dio da AngioTC por regi&#227;o, observamos, na popula&#231;&#227;o de 100.000, por vida e em 5 anos, respectivamente, uma economia m&#233;dia de: R$ 1.226,00 e R$ 122.577.793,00 na Regi&#227;o Norte; R$ 1.460,00 e R$ 145.988.367,00 na Regi&#227;o Nordeste; R$ 1.625,00 e R$ 162.502.626,00 na Regi&#227;o Centro-Oeste; R$ 1.313,00 e R$ 131.270.230,00 na Regi&#227;o Sudeste; e R$ 1.043,00 e R$ 104.268.937,00 na Regi&#227;o Sul.</p>
        </sec>
        <sec>
          <title>Conclus&#227;o</title>
          <p> A AngioTC como estrat&#233;gia inicial na investiga&#231;&#227;o da dor tor&#225;cica est&#225;vel &#233; custo-efetiva quando comparada &#224; ACI e est&#225; associada &#224; redu&#231;&#227;o de custos significativos na sa&#250;de suplementar brasileira.</p>

      <title>Introdu&#231;&#227;o</title>
      <p>A doen&#231;a cardiovascular &#233; a principal causa de morte no mundo, maior do que todas as causas de c&#226;ncer somadas, matando 3 vezes mais mulheres do que o c&#226;ncer de mama e 7 vezes mais do que c&#226;ncer de c&#243;lon.<xref ref-type="bibr" rid="B1">1</xref>-<xref ref-type="bibr" rid="B3">3</xref> No Brasil, h&#225; mais de 350.000 mortes por ano, mais de 1.100 mortes por dia, cerca de 46 mortes por hora, 1 morte a cada 90 segundos.<xref ref-type="bibr" rid="B4">4</xref> Diante dessa elevada carga de mortalidade, fornecer o diagn&#243;stico precoce da doen&#231;a arterial coronariana (DAC) torna-se essencial para redu&#231;&#227;o de complica&#231;&#245;es relacionadas &#224; ocorr&#234;ncia de eventos cardiovasculares maiores.</p>
      <p>A DAC representa um impacto significativo tanto no sistema p&#250;blico de sa&#250;de quanto na economia nacional. Estima-se que, no Brasil, as doen&#231;as cardiovasculares gerem custos diretos e indiretos que ultrapassam R$ 50 bilh&#245;es por ano.<xref ref-type="bibr" rid="B5">5</xref> Os gastos abrangem desde consultas, exames diagn&#243;sticos e terapias farmacol&#243;gicas at&#233; procedimentos de alta complexidade, como angioplastia e cirurgia de revasculariza&#231;&#227;o mioc&#225;rdica, que vem aumentado nos &#250;ltimos anos no Brasil.<xref ref-type="bibr" rid="B3">3</xref> Al&#233;m disso, os custos indiretos relacionados &#224; perda de produtividade, aposentadoria precoce e interna&#231;&#245;es recorrentes acentuam ainda mais o &#244;nus financeiro. Dessa forma, investir em estrat&#233;gias diagn&#243;sticas que possam fornecer o diagn&#243;stico precoce e preciso da DAC tem o potencial n&#227;o apenas de melhorar o progn&#243;stico cl&#237;nico dos pacientes, mas tamb&#233;m de mitigar poss&#237;veis custos a longo prazo, ao possibilitar interven&#231;&#245;es oportunas que previnam desfechos adversos e hospitaliza&#231;&#245;es evit&#225;veis.</p>
      <p>A estrat&#233;gia diagn&#243;stica da DAC &#233; por vezes complexa, sendo discutida em v&#225;rias situa&#231;&#245;es cl&#237;nicas de acordo com presen&#231;a de sintomas e fatores de risco cardiovascular. A angiografia coron&#225;ria invasiva (ACI) &#233; o m&#233;todo padr&#227;o-ouro no diagn&#243;stico anat&#244;mico da DAC.<xref ref-type="bibr" rid="B6">6</xref> Entretanto, trata-se de um m&#233;todo invasivo, n&#227;o isento de complica&#231;&#245;es relacionadas.<xref ref-type="bibr" rid="B7">7</xref> A angiotomografia coronariana (AngioTC) &#233; o &#250;nico teste n&#227;o invasivo capaz de visualizar a placa ateroscler&#243;tica coronariana, em diversas fases evolutivas, incluindo as n&#227;o obstrutivas, permitindo a detec&#231;&#227;o e o tratamento mais precoce da DAC, reduzindo o n&#250;mero absoluto de infartos.<xref ref-type="bibr" rid="B8">8</xref>,<xref ref-type="bibr" rid="B9">9</xref> Ap&#243;s a s&#233;rie de resultados do Ischemia Trial Group, parece consolidado o conceito de que a detec&#231;&#227;o e caracteriza&#231;&#227;o da carga ateroscler&#243;tica tem valor progn&#243;stico superior &#224; detec&#231;&#227;o de isquemia mioc&#225;rdica.<xref ref-type="bibr" rid="B10">10</xref>,<xref ref-type="bibr" rid="B11">11</xref> Diretrizes brasileiras atuais recomendam este exame como estrat&#233;gia diagn&#243;stica inicial em paciente com dor tor&#225;cica, sem anatomia conhecida.<xref ref-type="bibr" rid="B12">12</xref> Mas n&#227;o h&#225; dados brasileiros que mostrem a custo-efetividade desta abordagem na sa&#250;de suplementar.</p>
      <p>Diante disso, este estudo conduziu uma an&#225;lise de custo-efetividade comparando as estrat&#233;gias dispon&#237;veis para diagn&#243;stico anat&#244;mico da DAC (AngioTC versus ACI) como procedimento inicial para avalia&#231;&#227;o de pacientes com angina est&#225;vel no sistema de sa&#250;de suplementar. O objetivo foi estimar o potencial de redu&#231;&#227;o de custos e o impacto cl&#237;nico da ado&#231;&#227;o sistem&#225;tica da AngioTC, com modelagem espec&#237;fica para diferentes regi&#245;es geogr&#225;ficas do Brasil, a fim de refletir varia&#231;&#245;es nos custos e na infraestrutura dispon&#237;vel.</p>

      <title>M&#233;todos</title>
      <p>Foi realizada uma avalia&#231;&#227;o de custo-efetividade e de impacto or&#231;ament&#225;rio que levou em considera&#231;&#227;o a AngioTC como estrat&#233;gia inicial na investiga&#231;&#227;o da DAC em compara&#231;&#227;o &#224; ACI. A escolha da ACI como comparador baseou-se no fato de que esta representa o m&#233;todo diagn&#243;stico padr&#227;o-ouro na estratifica&#231;&#227;o da DAC, sendo amplamente dispon&#237;vel no cen&#225;rio nacional da sa&#250;de suplementar. Essa compara&#231;&#227;o se justifica pelo fato de a AngioTC ser o &#250;nico m&#233;todo n&#227;o invasivo dispon&#237;vel para o diagn&#243;stico anat&#244;mico da DAC e com alta acur&#225;cia diagn&#243;stica.<xref ref-type="bibr" rid="B13">13</xref></p>
      <p>Com a finalidade de aumentar a transpar&#234;ncia do estudo proposto, os principais aspectos das an&#225;lises foram sumarizados conforme o checklist Consolidated Health Economic Evaluation Reporting Standards (CHEERS), sob um horizonte de 5 anos, como recomendado pelo Minist&#233;rio da Sa&#250;de.<xref ref-type="bibr" rid="B14">14</xref></p>
      <p>A an&#225;lise focou na interven&#231;&#227;o da AngioTC como diagn&#243;stico inicial de pacientes com suspeita de DAC est&#225;vel em compara&#231;&#227;o com a ACI. A popula&#231;&#227;o avaliada na modelagem corresponde a uma demanda simulada de uma operadora de sa&#250;de de m&#233;dio porte, estimada em uma coorte anual de 100.000 benefici&#225;rios. Adicionalmente, foi aplicada &#224; proje&#231;&#227;o populacional a taxa de crescimento demogr&#225;fico anual definida pelo Instituto Brasileiro de Geografia e Estat&#237;stica (IBGE).</p>
      <p>A avalia&#231;&#227;o econ&#244;mica foi conduzida com base em valores praticados no sistema de sa&#250;de suplementar brasileiro, utilizando como refer&#234;ncia a Classifica&#231;&#227;o Brasileira Hierarquizada de Procedimentos M&#233;dicos (CBHPM), elaborada pela Associa&#231;&#227;o M&#233;dica Brasileira (AMB) em conjunto com as sociedades de especialidades m&#233;dicas, com apoio t&#233;cnico da Funda&#231;&#227;o Instituto de Pesquisas Econ&#244;micas (FIPE). De modo complementar, utilizou-se a lista oficial de pre&#231;os de medicamentos publicada pela C&#226;mara de Regula&#231;&#227;o do Mercado de Medicamentos (CMED), &#243;rg&#227;o interministerial composto por representantes dos Minist&#233;rios da Sa&#250;de, da Economia, da Justi&#231;a e da Casa Civil, sendo a Ag&#234;ncia Nacional de Vigil&#226;ncia Sanit&#225;ria (Anvisa) respons&#225;vel por sua Secretaria-Executiva.<xref ref-type="bibr" rid="B15">15</xref>,<xref ref-type="bibr" rid="B16">16</xref></p>
      <p>O modelo considerou os custos diretos relacionados aos exames diagn&#243;sticos (incluindo o exame em si, taxas operacionais e insumos), bem como os custos do manejo cl&#237;nico de eventos, como infarto agudo do mioc&#225;rdio e hospitaliza&#231;&#227;o. Os valores atribu&#237;dos a cada procedimento diagn&#243;stico foram obtidos por meio dos c&#243;digos oficiais da CBHPM e da CMED.<xref ref-type="bibr" rid="B15">15</xref>,<xref ref-type="bibr" rid="B16">16</xref></p>
      <p>Para a ACI, o custo total estimado foi de R$ 1.900,79, valor que inclui o exame, a taxa hospitalar pelo uso da sala de hemodin&#226;mica e os custos com a monitoriza&#231;&#227;o do paciente durante o procedimento. Para a AngioTC o valor utilizado foi de R$ 1.311,95, conforme refer&#234;ncia da Ag&#234;ncia Nacional de Sa&#250;de Suplementar (ANS).</p>
      <p>Os custos relacionados &#224; hospitaliza&#231;&#227;o e ao tratamento cl&#237;nico subsequente (incluindo exames complementares e medica&#231;&#245;es), em pacientes com infarto agudo do mioc&#225;rdio, foram estimados com base nas tabelas oficiais dispon&#237;veis e sumarizados na <xref ref-type="sec" rid="suppl">Tabela S1</xref>. Os custos relacionados aos procedimentos de revasculariza&#231;&#227;o mioc&#225;rdica cir&#250;rgica e &#224; angioplastia com stent foram obtidos conforme detalhado nas <xref ref-type="sec" rid="suppl">Tabelas Suplementares S2 e S3</xref>, respectivamente. Para componentes como di&#225;rias e taxas hospitalares, utilizaram-se valores m&#233;dios praticados por hospitais em diferentes regi&#245;es do pa&#237;s, diante da aus&#234;ncia de uma tabela nacional padronizada. &#201; importante destacar que esses valores costumam ser negociados diretamente entre operadoras de sa&#250;de e institui&#231;&#245;es prestadoras de servi&#231;os. Para a an&#225;lise por subgrupos regionais, calcularam-se as m&#233;dias de custos por regi&#227;o, com o objetivo de refletir as varia&#231;&#245;es econ&#244;micas e estruturais observadas nas diferentes regi&#245;es do Brasil.</p>
      <p>Para estimativa das incid&#234;ncias dos percentuais da probabilidade de infarto nos grupos encaminhados &#224; AngioTC ou ACI, da probabilidade de revasculariza&#231;&#227;o mioc&#225;rdica nos dois grupos e probabilidade de interna&#231;&#227;o nos dois grupos, foram utilizadas as incid&#234;ncias de eventos baseadas no estudo DISCHARGE TRIAL,<xref ref-type="bibr" rid="B17">17</xref> recente estudo multic&#234;ntrico randomizado, que comparou as estrat&#233;gias AngioTC e ACI, durante seguimento m&#233;dio de 3,5 anos. <p>Para garantir a robustez dos resultados, adicionalmente, foi conduzida uma an&#225;lise de sensibilidade probabil&#237;stica multivariada, por meio de simula&#231;&#245;es de coorte de Monte Carlo de segunda ordem, utilizando 1.000 intera&#231;&#245;es com base nos par&#226;metros definidos no modelo. Os resultados dessa an&#225;lise est&#227;o representados na <xref ref-type="fig" rid="f01">Figura Central</xref>. Para os par&#226;metros com distribui&#231;&#227;o cont&#237;nua e ilimitada superiormente, como os custos, foram aplicadas distribui&#231;&#245;es do tipo gama. J&#225; para par&#226;metros restritos ao intervalo entre 0 e 1, como as probabilidades de eventos adversos, adotaram-se distribui&#231;&#245;es beta. As distribui&#231;&#245;es beta e gama foram parametrizadas com intervalos de varia&#231;&#227;o entre 80% e 120% dos valores determin&#237;sticos, permitindo avaliar a influ&#234;ncia de varia&#231;&#245;es de at&#233; 20% sobre os resultados do modelo.</p>
      <p>Al&#233;m da an&#225;lise de custo-efetividade, foi conduzido a an&#225;lise de impacto or&#231;ament&#225;rio referente &#224; incorpora&#231;&#227;o da AngioTC em pacientes com suspeita de DAC est&#225;vel, utilizando como popula&#231;&#227;o eleg&#237;vel estimada em 100.000 benefici&#225;rios no primeiro ano e um aumento anual proporcional &#224; taxa de crescimento demogr&#225;fico da popula&#231;&#227;o brasileira em um horizonte de 5 anos. Esse n&#250;mero de vidas buscou refletir o esperado de uma operadora de sa&#250;de de porte m&#233;dio e, por consequ&#234;ncia, a realidade do sistema de sa&#250;de suplementar atual.</p>
      <p>A implanta&#231;&#227;o da estrat&#233;gia com AngioTC foi modelada de forma conservadora, em conformidade com diretrizes do Minist&#233;rio da Sa&#250;de, considerando uma implementa&#231;&#227;o progressiva ao longo de 5 anos. No primeiro ano, estimou-se que 5% da popula&#231;&#227;o eleg&#237;vel (5.000 indiv&#237;duos) seriam submetidos &#224; AngioTC. A partir da&#237;, adotou-se um incremento anual de 5 pontos percentuais, alcan&#231;ando 25% da popula&#231;&#227;o eleg&#237;vel ao final do quinto ano. Essa propor&#231;&#227;o corresponde a 25.608 indiv&#237;duos, considerando a proje&#231;&#227;o de crescimento populacional derivada das estimativas demogr&#225;ficas do IBGE (<xref ref-type="sec" rid="suppl">Tabela S5</xref>).</p>

      <title>Discuss&#227;o</title>
      <p>No nosso estudo, a estrat&#233;gia de utiliza&#231;&#227;o da AngioTC como m&#233;todo diagn&#243;stico inicial em pacientes est&#225;veis com suspeita de DAC e probabilidade pr&#233;-teste intermedi&#225;ria demonstrou superioridade econ&#244;mica em rela&#231;&#227;o &#224; ACI, tradicionalmente utilizada como padr&#227;o-ouro para diagn&#243;stico anat&#244;mico nessa popula&#231;&#227;o. A an&#225;lise de custo-efetividade revelou que a AngioTC &#233; uma estrat&#233;gia dominante, ou seja, mais eficaz e menos onerosa, nos desfechos cl&#237;nicos avaliados: infarto agudo do mioc&#225;rdio, hospitaliza&#231;&#245;es e necessidade de revasculariza&#231;&#227;o mioc&#225;rdica. Essa domin&#226;ncia foi consistente nas an&#225;lises de sensibilidade, com 91,2% das simula&#231;&#245;es refor&#231;ando a superioridade econ&#244;mica da AngioTC. Al&#233;m disso, os dados por regi&#227;o do Brasil demonstraram robustez da estrat&#233;gia mesmo frente &#224;s varia&#231;&#245;es regionais de custos, mantendo a AngioTC como custo-efetiva nos cen&#225;rios avaliados. A an&#225;lise de impacto or&#231;ament&#225;rio projeta uma expressiva economia acumulada para uma operadora de porte m&#233;dio no pa&#237;s. O emprego da AngioTC gera uma economia de R$ 776,00 por vida ou uma redu&#231;&#227;o de R$ 25.727.985,00 ao final do quinto ano quando a estrat&#233;gia fosse empregada para 25% da popula&#231;&#227;o.</p>
      <p>A utiliza&#231;&#227;o da AngioTC tem crescido nas &#250;ltimas d&#233;cadas, em virtude das in&#250;meras evid&#234;ncias cient&#237;ficas que refor&#231;am seu emprego na estratifica&#231;&#227;o da doen&#231;a coron&#225;ria. Estudos utilizando AngioTC na investiga&#231;&#227;o inicial diagn&#243;stica de dor tor&#225;cica em pacientes est&#225;veis em compara&#231;&#227;o com a estrat&#233;gia habitual, que utiliza fatores de risco e sintomas e/ou exames funcionais (teste ergom&#233;trico/cintilografia mioc&#225;rdica), demonstraram n&#227;o inferioridade da AngioTC em rela&#231;&#227;o a exames funcionais, no desfecho morte cardiovascular<xref ref-type="bibr" rid="B18">18</xref> e diminui&#231;&#227;o da incid&#234;ncia de infartos n&#227;o fatais em at&#233; 40%, com resultados que persistiram por 10 anos de seguimento.<xref ref-type="bibr" rid="B19">19</xref> Esses dados levaram as recentes diretrizes europeias, estadunidense e brasileira a elevarem o n&#237;vel de recomenda&#231;&#227;o da AngioTCc como classe I, n&#237;vel de evid&#234;ncia A, na investiga&#231;&#227;o inicial de dor tor&#225;cica est&#225;vel.<xref ref-type="bibr" rid="B12">12</xref>,<xref ref-type="bibr" rid="B20">20</xref></p>
      <p>Uma metan&#225;lise publicada recentemente, que incluiu estudos comparando a estrat&#233;gia diagn&#243;stica para DAC est&#225;vel utilizando a AngioTC e a ACI, exibiu n&#227;o inferioridade da estrat&#233;gia da AngioTC no desfecho de mortalidade cardiovascular e infarto n&#227;o fatal, por&#233;m, com menos revasculariza&#231;&#227;o mioc&#225;rdica e com menor incid&#234;ncia de acidente vascular cerebral. Al&#233;m disso, a metan&#225;lise mostrou que utilizar a estrat&#233;gia de AngioTC preferencialmente permitiria evitar a necessidade de ACI em at&#233; 77% dos pacientes.<xref ref-type="bibr" rid="B21">21</xref></p>
      <p>Apesar das crescentes evid&#234;ncias das vantagens da investiga&#231;&#227;o da dor tor&#225;cica est&#225;vel com AngioTC, com a incorpora&#231;&#227;o desta estrat&#233;gia nas principais diretrizes mundiais, ainda n&#227;o t&#237;nhamos um estudo de custo-efetividade na sa&#250;de suplementar brasileira que inclu&#237;sse AngioTC como estrat&#233;gia inicial comparada com a ACI. Apesar da heterogeneidade dos valores de exames diagn&#243;sticos, hospitaliza&#231;&#227;o e tratamento praticados pelas diversas operadoras de sa&#250;de no Brasil, balizamos os valores utilizados para o c&#225;lculo de custo-efetividade considerando 100% dos valores recomendados pelas &#250;ltimas atualiza&#231;&#245;es das tabelas CBHPM e CMED. Considerando-se o c&#225;lculo de implanta&#231;&#227;o desta &#8220;nova&#8221; tecnologia, seguindo recomenda&#231;&#227;o conservadora e progressiva do Minist&#233;rio da Sa&#250;de que recomenda iniciar o uso em somente 5% da popula&#231;&#227;o, aumentando progressivamente 5% ao ano em um horizonte de 5 anos, o nosso estudo mostrou que a estrat&#233;gia de AngioTC &#233; dominante e custo-efetiva, com os valores praticados na sa&#250;de suplementar brasileira.</p>
      <p>Segundo a ANS, 24,5% da popula&#231;&#227;o brasileira possu&#237;a acesso &#224; sa&#250;de suplementar em janeiro de 2025, cerca de 52 milh&#245;es de brasileiros.<xref ref-type="bibr" rid="B22">22</xref> Segundo esta mesma fonte, cerca de 26% dos usu&#225;rios de sa&#250;de suplementar est&#227;o acima de 40 anos, ou seja, estariam, devido &#224; idade, mais prop&#237;cios a ter doen&#231;a ateroscler&#243;tica coronariana, com a ressalva que mulheres teriam maior preval&#234;ncia de aterosclerose coronariana 10 anos ap&#243;s a climat&#233;rio.<xref ref-type="bibr" rid="B23">23</xref> Desta forma, se em 5 anos, em uma popula&#231;&#227;o de 100.000 vidas, foi demonstrada custo-efetividade utilizando AngioTC em uma fra&#231;&#227;o desta popula&#231;&#227;o, &#233; prov&#225;vel que, se a estrat&#233;gia fosse disponibilizada para as milh&#245;es de pessoas com potencial risco de aterosclerose coronariana que utilizam a sa&#250;de suplementar, tiv&#233;ssemos um impacto econ&#244;mico ainda mais significativo.</p>
      <p>H&#225; 10 anos, o &#243;rg&#227;o de qualidade e decis&#227;o baseada em evid&#234;ncia do servi&#231;o de sa&#250;de p&#250;blica do Reino Unido (NICE), recomendou a ado&#231;&#227;o da AngioTC como teste diagn&#243;stico preferencial para investiga&#231;&#227;o de dor tor&#225;cica em todo o seu territ&#243;rio.<xref ref-type="bibr" rid="B24">24</xref> Depois de mais de meia d&#233;cada de experi&#234;ncia, os resultados dessa decis&#227;o foram apresentados e se revelaram extremamente positivos naquelas regi&#245;es do Reino Unido que conseguiram, de fato, adotar as referidas recomenda&#231;&#245;es, resultando em redu&#231;&#227;o significativa da mortalidade cardiovascular e uma tend&#234;ncia de redu&#231;&#227;o na mortalidade por todas as causas.<xref ref-type="bibr" rid="B25">25</xref> Parece razo&#225;vel citar uma importante premissa em medicina, de que o diagn&#243;stico precoce como principal estrat&#233;gia permite resultados melhores, com grande economia na an&#225;lise do impacto or&#231;ament&#225;rio. <title>Limita&#231;&#245;es</title>
        <p>Este estudo apresenta algumas limita&#231;&#245;es importantes que devem ser consideradas na interpreta&#231;&#227;o de seus resultados. Primeiramente, a modelagem de custo-efetividade foi baseada exclusivamente nos dados do estudo DISCHARGE, o que pode limitar a generaliza&#231;&#227;o dos achados, uma vez que os resultados de um &#250;nico ensaio cl&#237;nico randomizado, ainda que robusto, n&#227;o contemplam todas as variabilidades cl&#237;nicas e populacionais da pr&#225;tica cotidiana. Em segundo lugar, a an&#225;lise foi restrita a pacientes com probabilidade pr&#233;-teste intermedi&#225;ria de DAC, o que impossibilita extrapola&#231;&#245;es para indiv&#237;duos com baixa ou alta probabilidade pr&#233;-teste, limitando sua aplicabilidade universal. Por fim, o modelo adotado compara a AngioTC diretamente com a ACI, sem considerar compara&#231;&#245;es com testes funcionais amplamente utilizados na pr&#225;tica cl&#237;nica, como cintilografia mioc&#225;rdica ou ecocardiograma com estresse, os quais continuam sendo recomendados nas diretrizes atuais para avalia&#231;&#227;o funcional da isquemia mioc&#225;rdica. A aus&#234;ncia dessa compara&#231;&#227;o impede uma an&#225;lise mais abrangente sobre o posicionamento da AngioTC frente a todas as op&#231;&#245;es diagn&#243;sticas dispon&#237;veis.</p>

      <title>Conclus&#227;o</title>
      <p>A escolha da AngioTC como estrat&#233;gia padr&#227;o para o diagn&#243;stico de DAC est&#225;vel parece ser custo-efetiva quando comparada &#224; realiza&#231;&#227;o da ACI, no sistema de sa&#250;de suplementar brasileiro. <title>Introduction</title>
        <p>Cardiovascular disease is the leading cause of death worldwide, surpassing all causes of cancer combined, killing 3 times more women than breast cancer and 7 times more than colon cancer.<xref ref-type="bibr" rid="B1">1</xref>-<xref ref-type="bibr" rid="B3">3</xref> In Brazil, they are responsible for more than 350,000 deaths per year, more than 1,100 deaths per day, approximately 46 deaths per hour, or 1 death every 90 seconds.<xref ref-type="bibr" rid="B4">4</xref> Given this high burden of mortality, it is essential to provide early diagnosis of coronary artery disease (CAD) in order to reduce complications related to major cardiovascular events.</p>
        <p>CAD represents a significant impact on both public health systems and national economies. It is estimated that, in Brazil, cardiovascular diseases generate direct and indirect costs exceeding R$50 billion per year.<xref ref-type="bibr" rid="B5">5</xref> These costs range from consultations, diagnostic tests, and pharmacological therapies to highly complex procedures such as angioplasty and myocardial revascularization, which have increased in recent years in Brazil.<xref ref-type="bibr" rid="B3">3</xref> Furthermore, indirect costs related to loss of productivity, early retirement, and recurrent hospitalizations further increase the financial burden. Accordingly, investments in diagnostic strategies that can provide early and accurate diagnosis of CAD have the potential not only to improve patients&#8217; clinical prognosis but also to mitigate potential long-term costs by enabling timely interventions that prevent adverse outcomes and avoidable hospitalizations.</p>
        <p>The diagnostic strategy for CAD is often complex and is discussed in various clinical situations according to the presence of symptoms and cardiovascular risk factors. Invasive coronary angiography (ICA) is the gold standard method for anatomical diagnosis of CAD.<xref ref-type="bibr" rid="B6">6</xref> However, it is an invasive method, which is not free from related complications.<xref ref-type="bibr" rid="B7">7</xref> Coronary computed tomography angiography (CCTA) is the only non-invasive test capable of visualizing coronary atherosclerotic plaque in various phases of development, including non-obstructive ones, allowing for earlier detection and treatment of CAD and reducing the absolute number of infarctions.<xref ref-type="bibr" rid="B8">8</xref>,<xref ref-type="bibr" rid="B9">9</xref> A series of results from the Ischemia Trial Group seems to have consolidated the concept that the detection and characterization of atherosclerotic burden has a prognostic value superior to the detection of myocardial ischemia.<xref ref-type="bibr" rid="B10">10</xref>,<xref ref-type="bibr" rid="B11">11</xref> Current Brazilian guidelines recommend this exam as an initial diagnostic strategy in patients with chest pain and unknown anatomy.<xref ref-type="bibr" rid="B12">12</xref> Nevertheless, there are no Brazilian data demonstrating the cost-effectiveness of this approach in the private healthcare system.</p>
        <p>Given the above, this study conducted a cost-effectiveness analysis comparing available strategies for anatomical diagnosis of CAD (CCTA versus ICA) as the initial procedure for evaluating patients with stable angina in the Brazilian private healthcare system. The objective was to estimate the potential cost reduction and clinical impact of systematically adopting CCTA, with specific modeling for different geographic regions of Brazil in order to reflect variations in costs and available infrastructure.</p>

      <sec sec-type="methods">
        <title>Methods</title>
        <p>A cost-effectiveness and budget impact assessment was conducted, considering CCTA as the initial strategy for investigating CAD compared to ICA. ICA was chosen as the comparator due to the fact that it is considered the gold standard diagnostic method for CAD stratification and is widely available in the Brazilian private healthcare system. This comparison is justified by the fact that CCTA is the only non-invasive method available for anatomical diagnosis of CAD as well as its high diagnostic accuracy.<xref ref-type="bibr" rid="B13">13</xref></p>
        <p>With the aim of increasing the transparency of the proposed study, the main aspects of the analyses were summarized according to the Consolidated Health Economic Evaluation Reporting Standards (CHEERS) checklist, over a 5-year horizon, as recommended by the Brazilian Ministry of Health.<xref ref-type="bibr" rid="B14">14</xref></p>
        <p>The analysis focused on the use of CCTA as the initial diagnostic tool for patients with suspected stable CAD, in comparison with ICA. The population assessed in the model corresponds to the simulated demand of a mid-sized healthcare provider, estimated at an annual cohort of 100,000 patients. Additionally, the annual demographic growth rate defined by the Brazilian Institute of Geography and Statistics (IBGE) was applied to the population projection.</p>
        <p>The economic assessment was conducted based on prices charged within the Brazilian private healthcare system, applying the Brazilian Hierarchical Classification of Medical Procedures (CBHPM), put forth by the Brazilian Medical Association (AMB) in conjunction with medical specialty societies, with technical support from the Economic Research Institute Foundation (FIPE), as a reference. Additionally, we used the official drug price list published by the Drug Market Regulation Chamber (CMED), an interministerial body composed of representatives from the Brazilian Ministries of Health, Economy, Justice, and the Chief of Staff, with the National Health Surveillance Agency (ANVISA) responsible for its Executive Secretariat.<xref ref-type="bibr" rid="B15">15</xref>,<xref ref-type="bibr" rid="B16">16</xref></p>
        <p>The model considered direct costs related to diagnostic tests (including the test itself, operating fees, and medical supplies), as well as the costs of clinical management of events, such as acute myocardial infarction and hospitalization. The values assigned to each diagnostic procedure were obtained using official codes from the CBHPM and CMED.<xref ref-type="bibr" rid="B15">15</xref>,<xref ref-type="bibr" rid="B16">16</xref></p>
        <p>For ICA, the total estimated cost was BRL 1,900.79, which includes the test, the hospital fee for use of the catheterization room, and the costs of patient monitoring during the procedure. For CCTA, the cost of BRL 1,311.95 was considered, according to the reference provided by the National Supplementary Health Agency (ANS), which is the regulatory body responsible for private healthcare in Brazil.</p>
        <p>The costs related to hospitalization and subsequent clinical treatment (including additional tests and medications) for patients with acute myocardial infarction were estimated based on official tables made available and summarized in <xref ref-type="sec" rid="suppl">Supplementary Table S1</xref>. The costs related to surgical myocardial revascularization procedures and stent angioplasty were obtained as detailed in <xref ref-type="sec" rid="suppl_en">Supplemental Tables S2 and S3</xref>, respectively. For components such as hospital fees and daily rates, average values charged by hospitals in different regions of the country were used, given the lack of a standardized national price list. It is important to note that these values are often negotiated directly between health insurance companies and service providers. For analysis by regional subgroups, average costs were calculated by region to reflect the economic and structural variations observed in different Brazilian regions.</p>
        <p>To estimate the incidence of the percentage probability of infarction in the groups referred for CCTA or ICA, the probability of myocardial revascularization in both groups, and the probability of hospitalization in both groups, we used the event incidences based on the DISCHARGE TRIAL study,<xref ref-type="bibr" rid="B17">17</xref> a recent multicenter randomized trial that compared CCTA and ICA strategies over a mean follow-up of 3.5 years. <xref ref-type="sec" rid="suppl_en">Supplementary Table S4</xref> summarizes the probabilities of acute myocardial infarction, revascularization, and hospitalization events identified in the original study and applied in the present analysis.</p>
        <p>To guarantee the robustness of the results, a multivariate probabilistic sensitivity analysis was also conducted, by means of second-order Monte Carlo simulations, using 1,000 interactions based on the parameters defined in the model. The results of this analysis are shown in <xref ref-type="fig" rid="f01001">Central Illustration</xref>. For parameters with continuous and upper incomplete distribution, such as costs, gamma distributions were applied. For parameters restricted to the range between 0 and 1, such as the probabilities of adverse events, beta distributions were adopted. The beta and gamma distributions were parameterized with variation intervals between 80% and 120% of the deterministic values, allowing assessment of the influence of variations of up to 20% on the model results.</p>
        <p>In addition to the cost-effectiveness analysis, a budget impact analysis was conducted regarding the incorporation of CCTA in patients with suspected stable CAD, using an estimated eligible population of 100,000 patients in the first year and an annual increase proportional to the demographic growth rate of the Brazilian population over a 5-year horizon. This number of lives sought to reflect the expectations of a mid-sized health insurance provider and, consequently, the reality of the current private healthcare system in Brazil.</p>
        <p>The implementation of the CCTA strategy was modeled conservatively, in accordance with Ministry of Health guidelines, considering progressive implementation over 5 years. In the first year, it was estimated that 5% of the eligible population (5,000 individuals) would undergo CCTA. Thereafter, an annual increase of 5% was adopted, reaching 25% of the eligible population by the end of the fifth year. This proportion corresponds to 25,608 individuals, considering the population growth projection derived from IBGE demographic estimates (Table S5).</p>
      </sec>
      <sec sec-type="results">
        <title>Results</title>
        <p>Cost-effectiveness assessment demonstrated that the CCTA strategy in stable patients was superior to the standard strategy with ICA for diagnostic confirmation in three significant outcomes: acute myocardial infarction, hospitalizations, and myocardial revascularization, as a consequence of diagnosis of CAD with significant stenosis. The CCTA strategy demonstrated savings of BRL 686.86 per life for the outcome of acute myocardial infarction. For the outcome of hospitalization, the savings with the CCTA strategy reached a decreasing value of BRL 684.41 per life, and for the outcome of myocardial revascularization, the CCTA strategy achieved a reduction of BRL 811.09 per life (<xref ref-type="table" rid="t1001">Table 1</xref>).</p>

        <title>Discussion</title>
        <p>In our study, the strategy of using CCTA as the initial diagnostic method in stable patients with suspected CAD and intermediate pre-test probability demonstrated economic superiority over ICA, traditionally used as the gold standard for anatomical diagnosis in this population. Cost-effectiveness analysis revealed CCTA as a dominant strategy, i.e., more effective and less costly, for the following clinical outcomes assessed: acute myocardial infarction, hospitalizations, and the need for myocardial revascularization. This dominance was consistent in the sensitivity analyses, with 91.2% of simulations reinforcing the economic superiority of CCTA. Furthermore, data by Brazilian region demonstrated the robustness of the CCTA strategy, even considering regional cost variations, maintaining its cost-effectiveness in the scenarios assessed. The budget impact analysis projects significant cumulative savings for a mid-sized operator in Brazil. The use of CCTA would generate savings of 776.00 BRL per life or a reduction of 25,727,985.00 BRL at the end of the fifth year when implementing the strategy for 25% of the population.</p>
        <p>The use of CCTA has grown in recent decades, due to the large volume of scientific evidence that has reinforced its use in the stratification of CAD. Studies assessing CCTA in the initial diagnostic investigation of chest pain in stable patients compared with the standard strategy, which uses risk factors and symptoms and/or functional tests (exercise stress testing/myocardial scintigraphy), demonstrated non-inferiority of CCTA in relation to functional tests for the outcome of cardiovascular death<xref ref-type="bibr" rid="B18">18</xref> and a reduction in the incidence of non-fatal infarctions by up to 40%, with results that persisted for 10 years of follow-up.<xref ref-type="bibr" rid="B19">19</xref> These data led recent European, United States, and Brazilian guidelines to raise the recommendation level of CCTA to class I, level of evidence A, in the initial investigation of stable chest pain.<xref ref-type="bibr" rid="B12">12</xref>,<xref ref-type="bibr" rid="B20">20</xref></p>
        <p>A recently published meta-analysis, which compared the diagnostic strategy for stable CAD using CCTA and ICA, demonstrated non-inferiority of the CCTA strategy for the outcomes of cardiovascular mortality and non-fatal myocardial infarction, but with fewer myocardial revascularization procedures and a lower incidence of stroke. Furthermore, the meta-analysis demonstrated that preferentially using CCTA could avoid the need for ICA in 77% of patients.<xref ref-type="bibr" rid="B21">21</xref></p>
        <p>Despite the growing evidence of the advantages of investigating stable chest pain by means of CCTA, with the incorporation of this strategy into major global guidelines, there were no cost-effectiveness studies in the context of Brazilian private healthcare that included CCTA as an initial strategy compared to ICA. Despite the heterogeneity in costs of diagnostic tests, hospitalization, and treatment charged by different health insurance providers in Brazil, we based the values used to calculate cost-effectiveness on 100% of those recommended by the most recent updates of the CBHPM and CMED tables. Considering the calculation of implementing this &#8220;new&#8221; technology, following the conservative progressive recommendation of the Ministry of Health, which recommends starting use in only 5% of the population, progressively increasing 5% per year over a 5-year horizon, our study showed that the CCTA strategy is dominant and cost-effective, considering the values practiced in the Brazilian private healthcare system.</p>
        <p>According to the ANS, 24.5% of the Brazilian population had access to private healthcare in January 2025, corresponding to approximately 52 million Brazilians.<xref ref-type="bibr" rid="B22">22</xref> According to the same source, approximately 26% of private healthcare users are over 40 years old, meaning they would, due to their age, be more likely to have coronary atherosclerotic disease, also taking into account the fact that women have a higher prevalence of coronary atherosclerosis 10 years after menopause.<xref ref-type="bibr" rid="B23">23</xref> Thus, if in 5 years, in a population of 100,000 lives, cost-effectiveness was demonstrated using CCTA in a fraction of this population, it is likely that, were the strategy made available to the millions of people at potential risk of coronary atherosclerosis who use private healthcare, the economic impact would be even more significant.</p>
        <p>Ten years ago, in the United Kingdom, the National Institute for Health and Care Excellence (NICE) recommended the adoption of CCTA as the preferred diagnostic test for investigating chest pain throughout the territory.<xref ref-type="bibr" rid="B24">24</xref> After more than half a decade of experience, the results of this decision were presented, and they proved extremely positive in the regions of the United Kingdom that were able to effectively adopt the recommendations, resulting in a significant reduction in cardiovascular mortality and a downward trend in all-cause mortality.<xref ref-type="bibr" rid="B25">25</xref> It seems reasonable to cite an important premise in medicine, namely, that early diagnosis as the primary strategy allows for better outcomes, with significant savings in budget impact analysis. Patients with early diagnosis of CAD can receive appropriate drug treatment and reduce future events by effectively treating atherosclerotic disease.<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B26">26</xref></p>

          <title>Limitations</title>
          <p>There are some important study limitations that should be considered when interpreting these findings. First, the cost-effectiveness model was based exclusively on data from the DISCHARGE study, which may limit the generalizability of the findings, seeing that the results of a single randomized clinical trial, while robust, do not encompass all clinical and population variability in daily practice. Second, the analysis was restricted to patients with intermediate pre-test probability of CAD, making it impossible to extrapolate to patients with low or high pre-test probability, thus limiting universal applicability. Finally, the adopted model compares CCTA directly with ICA, without considering comparisons with functional tests that are widely used in clinical practice, for example, myocardial scintigraphy or stress echocardiography, which are still recommended in current guidelines for functional assessment of myocardial ischemia. The absence of this comparison prevents a more comprehensive analysis of the positioning of CCTA compared to all available diagnostic options.</p>

        <title>Conclusion</title>
        <p>The choice of CCTA as the standard strategy for diagnosing stable CAD appears to be cost-effective when compared to ICA in the context of the Brazilian private healthcare system. The budget impact of implementing this intervention in Brazil would result in significant cost savings for private healthcare.</p>
