      Prezado Editor,

    <p>Gostar&#237;amos de parabenizar os autores pelo relevante relato de caso "Linfoma Card&#237;aco: Um Relato de Caso", publicado nos Arquivos Brasileiros de Cardiologia (ABC Cardiol).<xref ref-type="bibr" rid="B1">1</xref> O artigo aborda uma condi&#231;&#227;o cl&#237;nica rara e desafiadora, o linfoma card&#237;aco prim&#225;rio (LCP), que, como bem destacado no artigo, &#233; um tumor agressivo com progn&#243;stico ruim e sintomas inespec&#237;ficos, o que dificulta o diagn&#243;stico precoce.<xref ref-type="bibr" rid="B1">1</xref> Entre os pontos fortes do artigo, destacamos a raridade do caso, por se tratar de linfoma difuso de grandes c&#233;lulas B (LDGC) com envolvimento card&#237;aco prim&#225;rio, dada a baixa incid&#234;ncia desses tumores (aproximadamente 0,04% dos tumores card&#237;acos prim&#225;rios malignos, com linfomas representando cerca de 1,3% destes) sendo o LDGC o subtipo mais comum, frequentemente localizado nas c&#226;maras direitas do cora&#231;&#227;o.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref></p>
    <p>A documenta&#231;&#227;o detalhada de um caso t&#227;o incomum contribui significativamente para a literatura m&#233;dica, sobretudo devido &#224; sua raridade e aos sintomas inespec&#237;ficos que podem mimetizar outras condi&#231;&#245;es como insufici&#234;ncia card&#237;aca (IC), o diagn&#243;stico do LCP &#233; complexo. Um relato de caso com descri&#231;&#227;o minuciosa dos achados de imagem &#8212; ecocardiograma, angiotomografia, resson&#226;ncia magn&#233;tica card&#237;aca &#8212; e confirma&#231;&#227;o histopatol&#243;gica, serve como um guia valioso para m&#233;dicos que se deparam com massas card&#237;acas at&#237;picas.</p>
    <p>Casos cl&#237;nicos detalhados s&#227;o ferramentas pedag&#243;gicas essenciais para estudantes e residentes, ajudando-os a reconhecer, investigar e gerenciar doen&#231;as raras que n&#227;o s&#227;o abordadas extensivamente em livros-texto ou aulas. Al&#233;m disso, incentivam o desenvolvimento acad&#234;mico e o pensamento cient&#237;fico, como destacam Florek e Dellavalle,<xref ref-type="bibr" rid="B4">4</xref> sendo frequentemente a primeira experi&#234;ncia de escrita cient&#237;fica para estudantes. Al&#233;m disso, Danish et al.<xref ref-type="bibr" rid="B5">5</xref> enfatizam que os relatos de caso podem apresentar casos novos e raros de maneira que levem &#224; expans&#227;o da pesquisa e &#224; gera&#231;&#227;o de novas hip&#243;teses inovadoras.</p>
    <p>O artigo descreve a evolu&#231;&#227;o cl&#237;nica do paciente, as complica&#231;&#245;es p&#243;s-operat&#243;rias (como edema agudo de pulm&#227;o e necessidade de marca-passo) e a resposta ao tratamento quimioter&#225;pico, incluindo a intercorr&#234;ncia de hemorragia digestiva e a reca&#237;da da doen&#231;a, al&#233;m de explorar a import&#226;ncia de exames de imagem n&#227;o invasivos, como o ecocardiograma transtor&#225;cico e, principalmente, a resson&#226;ncia magn&#233;tica cardiovascular (RMC) para a identifica&#231;&#227;o e caracteriza&#231;&#227;o das massas card&#237;acas.<xref ref-type="bibr" rid="B1">1</xref></p>
    <p>O tratamento com R-CHOP, esquema padr&#227;o para LDGC, demonstrou benef&#237;cio terap&#234;utico com taxa de remiss&#227;o de at&#233; 70%, embora a presen&#231;a de antraciclinas represente risco significativo de cardiotoxicidade, exigindo acompanhamento rigoroso.<xref ref-type="bibr" rid="B6">6</xref> Ao apresentar a descri&#231;&#227;o do tratamento, incluindo a ressec&#231;&#227;o cir&#250;rgica do tumor com reconstru&#231;&#227;o de veia cava superior e rafia do &#225;trio direito, e a subsequente quimioterapia com o esquema R-CHOP, o artigo de Pamplona et al.,<xref ref-type="bibr" rid="B1">1</xref> 2025 demonstra a complexidade do manejo desses pacientes. A discuss&#227;o das complica&#231;&#245;es p&#243;s-operat&#243;rias, como bradicardia juncional, fibrila&#231;&#227;o atrial e edema agudo de pulm&#227;o, al&#233;m da intercorr&#234;ncia de hemorragia digestiva durante o tratamento, ressalta os desafios cl&#237;nicos enfrentados. A conclus&#227;o do artigo, que refor&#231;a a necessidade de incluir o LCP como diagn&#243;stico diferencial de massas card&#237;acas, &#233; crucial. O diagn&#243;stico precoce pode melhorar significativamente o progn&#243;stico e aumentar a sobrevida dos pacientes, permitindo um encaminhamento r&#225;pido para tratamento espec&#237;fico.</p>
    <p>No entanto, alguns pontos poderiam ser considerados para aprimoramento. O artigo menciona que o paciente optou por participar de um ensaio cl&#237;nico para testar um novo quimioter&#225;pico e que a evolu&#231;&#227;o do paciente n&#227;o pode ser divulgada. Embora compreens&#237;vel por quest&#245;es &#233;ticas e de confidencialidade de ensaios cl&#237;nicos, a falta de informa&#231;&#245;es sobre o desfecho final do paciente limita a compreens&#227;o completa da efic&#225;cia do tratamento a longo prazo e do progn&#243;stico do caso espec&#237;fico. Diretrizes como a CARE (CAse REport) recomendam a apresenta&#231;&#227;o completa do seguimento, incluindo resposta ao tratamento, efeitos adversos e progn&#243;stico, elementos que enriquecem o relato e oferecem subs&#237;dios importantes &#224; pr&#225;tica m&#233;dica.<xref ref-type="bibr" rid="B7">7</xref>-<xref ref-type="bibr" rid="B9">9</xref></p>
    <p>Para um relato de caso, a evolu&#231;&#227;o completa &#233; um elemento importante para extrair futuras li&#231;&#245;es. As complica&#231;&#245;es p&#243;s-operat&#243;rias e durante o tratamento quimioter&#225;pico (edema agudo de pulm&#227;o, necessidade de marcapasso, hemorragia digestiva) s&#227;o mencionadas. Uma discuss&#227;o um pouco mais detalhada sobre a poss&#237;vel rela&#231;&#227;o dessas intercorr&#234;ncias com o tipo de tumor, o tratamento, ou fatores espec&#237;ficos do paciente, poderia enriquecer o caso. Por exemplo, a colite infecciosa que levou &#224; hemorragia digestiva, um evento grave, poderia ter uma breve reflex&#227;o sobre manejo ou preven&#231;&#227;o em pacientes oncol&#243;gicos sob tratamento quimioter&#225;pico, o que tornaria o caso ainda mais valioso para os leitores.</p>
    <p>Em suma, o presente relato de caso &#233; uma valiosa adi&#231;&#227;o ao conhecimento sobre o LCP, destacando os desafios diagn&#243;sticos e terap&#234;uticos. As observa&#231;&#245;es acima s&#227;o sugest&#245;es para potencializar ainda mais o impacto e a completude de futuros relatos de caso sobre condi&#231;&#245;es t&#227;o complexas.</p>

      <p>
        Dear Editor,
      </p>
      <p>We would like to congratulate the authors on their relevant case report, "Cardiac Lymphoma: A Case Report," published in the Arquivos Brasileiros de Cardiologia (ABC Cardiol).<xref ref-type="bibr" rid="B1">1</xref> The article addresses a rare and challenging clinical condition, primary cardiac lymphoma (PCL), which, as highlighted in the article, is an aggressive tumor with a poor prognosis and nonspecific symptoms, making early diagnosis difficult.<xref ref-type="bibr" rid="B1">1</xref> Among the article's strengths, we highlight the rarity of the case, as it is a diffuse large B-cell lymphoma (DLBCL) with primary cardiac involvement, given the low incidence of these tumors (approximately 0.04% of primary malignant cardiac tumors, with lymphomas representing about 1.3% of these), with DLBCL being the most common subtype, frequently located in the right chambers of the heart.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref></p>
      <p>The detailed documentation of such an unusual case contributes significantly to the medical literature, especially due to its rarity and the nonspecific symptoms that can mimic other conditions, such as heart failure (HF), making the diagnosis of PCL complex. A case report with a meticulous description of the imaging findings&#8212;echocardiogram, computed tomography angiography, cardiac magnetic resonance imaging&#8212;and histopathological confirmation serves as a valuable guide for physicians who encounter atypical cardiac masses.</p>
      <p>Detailed clinical cases are essential pedagogical tools for students and residents, helping them to recognize, investigate, and manage rare diseases that are not extensively covered in textbooks or lectures. Furthermore, they encourage academic development and scientific thinking, as highlighted by Florek &amp; Dellavalle,<xref ref-type="bibr" rid="B4">4</xref> often being the first scientific writing experience for students. In addition, Danish et al.<xref ref-type="bibr" rid="B5">5</xref> emphasize that case reports can present new and rare cases in a way that leads to the expansion of research and the generation of innovative new hypotheses.</p>
      <p>The article describes the patient's clinical evolution, postoperative complications (such as acute pulmonary edema and the need for a pacemaker), and response to chemotherapy treatment, including the occurrence of gastrointestinal bleeding and disease relapse, as well as exploring the importance of non-invasive imaging exams, such as transthoracic echocardiography and, especially, cardiovascular magnetic resonance imaging (CMR) for the identification and characterization of cardiac masses.<xref ref-type="bibr" rid="B1">1</xref></p>
      <p>Treatment with R-CHOP, the standard regimen for DLBCL, has demonstrated therapeutic benefit with remission rates as high as 70%, although the presence of anthracyclines poses a significant risk of cardiotoxicity, requiring close monitoring.<xref ref-type="bibr" rid="B6">6</xref> In presenting the treatment description, including surgical resection of the tumor with reconstruction of the superior vena cava and repair of the right atrium, and subsequent chemotherapy with the R-CHOP regimen, the article by Pamplona et al.<xref ref-type="bibr" rid="B1">1</xref> demonstrates the complexity of managing these patients. The discussion of postoperative complications, such as junctional bradycardia, atrial fibrillation, and acute pulmonary edema, in addition to the occurrence of gastrointestinal bleeding during treatment, highlights the clinical challenges faced. The article's conclusion, which reinforces the need to include PCL as a differential diagnosis for cardiac masses, is crucial. Early diagnosis can significantly improve prognosis and increase patient survival, allowing for rapid referral for specific treatment.</p>
      <p>However, some points could be considered for improvement. The article mentions that the patient chose to participate in a clinical trial to test a new chemotherapy drug and that the patient's progress cannot be disclosed While understandable due to ethical and confidentiality considerations in clinical trials, the lack of information on the patient's final outcome limits a complete understanding of the long-term effectiveness of treatment and the prognosis of the specific case. Guidelines such as CARE (CAse REport) recommend the complete presentation of follow-up, including treatment response, adverse effects, and prognosis, elements that enrich the report and offer important support to medical practice.<xref ref-type="bibr" rid="B7">7</xref>-<xref ref-type="bibr" rid="B9">9</xref></p>
      <p>For a case report, the complete evolution is an important element for drawing future lessons. Postoperative complications and those occurring during chemotherapy (acute pulmonary edema, need for a pacemaker, gastrointestinal bleeding) are mentioned. A slightly more detailed discussion of the possible relationship of these complications with the type of tumor, the treatment, or specific patient factors could enrich the case. For example, the infectious colitis that led to gastrointestinal bleeding, a serious event, could be briefly reflected upon in terms of management or prevention in cancer patients undergoing chemotherapy, which would make the case even more valuable for readers.</p>
      <p>In summary, this case report is a valuable addition to the knowledge about PCL, highlighting the diagnostic and therapeutic challenges. The observations above are suggestions to further enhance the impact and completeness of future case reports on such complex conditions.</p>
