    <p>
      Caro editor,
    </p>
    <p>O estudo publicado por Tang et al., intitulado "Fatores que Afetam a Trombose da Falsa Luz na Dissec&#231;&#227;o A&#243;rtica Tipo B", destacou a influ&#234;ncia dos aspectos morfol&#243;gicos da dissec&#231;&#227;o a&#243;rtica do tipo B (DATB), de acordo com a classifica&#231;&#227;o de Stanford, na trombose da falsa luz previamente ao tratamento com corre&#231;&#227;o endovascular da aorta tor&#225;cica. Foi demonstrado que a trombose da falsa luz acontece com frequ&#234;ncia significativamente menor em pacientes com DATB e insufici&#234;ncia renal, quando comparados com aqueles com DATB com fun&#231;&#227;o renal normal.<xref ref-type="bibr" rid="B1">1</xref></p>
    <p>Do ponto de vista terap&#234;utico, a identifica&#231;&#227;o de fatores predisponentes &#224; trombose da falsa luz pode auxiliar na personaliza&#231;&#227;o do tratamento. O estudo de Evangelista et al. demonstrou que a persist&#234;ncia de uma falsa luz patente est&#225; associada a um progn&#243;stico desfavor&#225;vel, com maior risco de complica&#231;&#245;es e necessidade de interven&#231;&#245;es futuras.&#178; A an&#225;lise de caracter&#237;sticas cl&#237;nicas e morfol&#243;gicas, como as realizadas por Tang et al., pode auxiliar na melhoria de estrat&#233;gias terap&#234;uticas, reduzindo a morbimortalidade e complica&#231;&#245;es graves, como ruptura e aneurismas.<xref ref-type="bibr" rid="B1">1</xref></p>
    <p>O estudo de Tsai et al. observou a presen&#231;a de trombose parcial da falsa luz em 68% dos pacientes, a qual estava associada a um aumento de at&#233; 2,69 vezes no risco de mortalidade ap&#243;s a alta hospitalar. Em um per&#237;odo de tr&#234;s anos, a taxa de mortalidade desses pacientes alcan&#231;ou 31,6%.<xref ref-type="bibr" rid="B3">3</xref> A partir disso, al&#233;m de abordar o impacto da rela&#231;&#227;o morfol&#243;gica e tromb&#243;tica na aorta acometida, se mostra relevante destacar o impacto cl&#237;nico da trombose parcial na mortalidade dos pacientes.</p>
    <p>Fatores morfol&#243;gicos e cl&#237;nicos, como o di&#226;metro da aorta e a presen&#231;a de insufici&#234;ncia renal respectivamente, foram abordados no artigo como influenciadores da trombose da falsa luz. No entanto, fatores hemodin&#226;micos, como a taxa de cisalhamento e tempo de resid&#234;ncia do fluido nos l&#250;mens n&#227;o foram relatados, os quais s&#227;o cr&#237;ticos para forma&#231;&#227;o de trombos.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B5">5</xref> Estudos como o de Menichini et al.<xref ref-type="bibr" rid="B4">4</xref> sugerem a utiliza&#231;&#227;o de modelo computacional para avaliar par&#226;metros hemodin&#226;micos. Dessa forma, a incorpora&#231;&#227;o de simula&#231;&#245;es computacionais pode buscar associa&#231;&#227;o e assim justificar como a insufici&#234;ncia renal interfere em fatores hemodin&#226;micos e promove o desenvolvimento de trombose da falsa luz.</p>
    <p>Parker et al. ressalta que saber a propor&#231;&#227;o da trombose do falso l&#250;men, definido como a raz&#227;o entre o volume do trombo e o volume total do falso l&#250;men, &#233; essencial para categorizar poss&#237;vel fator protetivo. Al&#233;m disso, o estudo descartou tomografia computadorizada com falsos l&#250;mens perfundidos inadequadamente pelo contraste, visto que impedem a modelagem a&#243;rtica e an&#225;lise hemodin&#226;mica adequadas,<xref ref-type="bibr" rid="B6">6</xref> ao contr&#225;rio do que aborda Tang et al.<xref ref-type="bibr" rid="B1">1</xref> que considerou aus&#234;ncia de contraste como falso l&#250;men trombosado. Outrossim, se destaca a import&#226;ncia de ter dados sobre o estado de anticoagula&#231;&#227;o dos pacientes, pois isso afeta a presen&#231;a de trombose no falso l&#250;men.</p>

        Dear editor,
      </p>
      <p>The study published by Tang et al., entitled "Factors Affecting False Lumen Thrombosis in Type B Aortic Dissection," highlighted the influence of morphological aspects of type B aortic dissection (TBAD), according to the Stanford classification, on false lumen thrombosis prior to treatment with endovascular repair of the thoracic aorta. It has been shown that false lumen thrombosis occurs significantly less frequently in patients with TBAD and renal failure, when compared with those with TBAD and normal renal function.<xref ref-type="bibr" rid="B1">1</xref></p>
      <p>From a therapeutic perspective, identifying predisposing factors for false lumen thrombosis can help personalize treatment. The study by Evangelista et al. demonstrated that persistent patent false lumen is associated with a poor prognosis, with a higher risk of complications and the need for future interventions.&#178; The analysis of clinical and morphological characteristics, such as those performed by Tang et al., can help improve therapeutic strategies, reducing morbidity and mortality and serious complications, such as rupture and aneurysms.<xref ref-type="bibr" rid="B1">1</xref></p>
      <p>The study by Tsai et al. observed partial thrombosis of the false lumen in 68% of patients, which was associated with a 2.69-fold increase in the risk of mortality after hospital discharge. Over a three-year period, the mortality rate in these patients reached 31.6%.<xref ref-type="bibr" rid="B3">3</xref> Therefore, in addition to addressing the impact of the morphological and thrombotic relationship in the affected aorta, it is important to highlight the clinical impact of partial thrombosis on patient mortality.</p>
      <p>Morphological and clinical factors, such as aortic diameter and the presence of renal failure, respectively, were discussed in the article as influencing false lumen thrombosis. However, hemodynamic factors, such as shear rate and fluid residence time in the lumens, which are critical for thrombus formation, were not reported.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B5">5</xref> Studies such as that by Menichini et al.<xref ref-type="bibr" rid="B4">4</xref> suggest the use of a computational model to evaluate hemodynamic parameters. Thus, incorporating computational simulations can seek associations and thus explain how renal failure interferes with hemodynamic factors and promotes the development of false lumen thrombosis.</p>
      <p>Parker et al. emphasize that knowing the proportion of false lumen thrombosis, defined as the ratio of thrombus volume to total false lumen volume, is essential for categorizing a possible protective factor. Furthermore, the study ruled out computed tomography scans with false lumens inadequately perfused with contrast, as they impede adequate aortic modeling and hemodynamic analysis,<xref ref-type="bibr" rid="B6">6</xref> contrary to what Tang et al.,<xref ref-type="bibr" rid="B1">1</xref> who considered the absence of contrast as a thrombosed false lumen, argue. Furthermore, they emphasize the importance of having data on patients&#8217; anticoagulation status, as this affects the presence of false lumen thrombosis.</p>
