        <sec>
          <title>Fundamento</title>
          <p> Mulheres submetidas &#224; cirurgia de revasculariza&#231;&#227;o do mioc&#225;rdio (CRM) apresentam piores desfechos intra-hospitalares, mas n&#227;o est&#225; claro se essas diferen&#231;as s&#227;o atribu&#237;das ao sexo ou a fatores cl&#237;nicos.</p>
        </sec>
        <sec>
          <title>Objetivo</title>
          <p> Comparar os desfechos intra-hospitalares entre mulheres e homens submetidos &#224; CRM.</p>
        </sec>
        <sec>
          <title>M&#233;todos</title>
          <p> Este estudo observacional, unic&#234;ntrico e retrospectivo analisou dados de 9.845 pacientes submetidos &#224; CRM entre 1995 e 2022; dentre eles, 1.947 (19,8%) eram mulheres. Para avaliar o sexo feminino como fator progn&#243;stico de mortalidade intra-hospitalar, foram realizadas an&#225;lises estat&#237;sticas descritivas, regress&#245;es log&#237;sticas univariada e multivariada e pareamento por escore de propens&#227;o. O n&#237;vel de signific&#226;ncia foi estabelecido em 5%.</p>
        </sec>
        <sec>
          <title>Resultados</title>
          <p> Mulheres apresentaram maior idade (66,7 vs 62,19 anos, p&lt;0,001), menor &#237;ndice de massa corporal (26,91 vs 27,64, p&lt;0,001) e maior preval&#234;ncia de diabetes melito (34,0% vs 31,6%, p=0,045). Elas tamb&#233;m tiveram maior tempo de interna&#231;&#227;o (14,84 vs 13,13 dias, p&lt;0,001) e maior mortalidade operat&#243;ria (4,8% vs 2,4%, p&lt;0,001). Ap&#243;s regress&#227;o log&#237;stica, o sexo feminino foi associado &#224; maior mortalidade (OR: 1,51, IC 95%: 1,15-1,99, p=0,003). Ap&#243;s o pareamento, n&#227;o houve diferen&#231;a significativa na mortalidade (OR: 1,20, IC 95%: 0,88-1,64, p=0,241), por&#233;m o tempo de interna&#231;&#227;o permaneceu maior entre as mulheres.</p>
        </sec>
        <sec>
          <title>Conclus&#227;o</title>
          <p> Ao se igualarem os fatores cl&#237;nicos entre homens e mulheres por meio do pareamento, as diferen&#231;as na mortalidade desaparecem. Isso sugere que interven&#231;&#245;es voltadas &#224; redu&#231;&#227;o das disparidades podem ser eficazes para melhorar os desfechos de mortalidade em mulheres submetidas &#224; CRM.</p>

      <title>Introdu&#231;&#227;o</title>
      <p>A doen&#231;a cardiovascular &#233; a principal causa de morbimortalidade no mundo, sendo a doen&#231;a arterial coronariana (DAC) sua manifesta&#231;&#227;o mais comum.<xref ref-type="bibr" rid="B1">1</xref> Em pa&#237;ses em desenvolvimento, a interpreta&#231;&#227;o dos resultados de registros nacionais &#233; desafiadora devido &#224; heterogeneidade das popula&#231;&#245;es e &#224;s diferen&#231;as significativas entre hospitais p&#250;blicos e privados.<xref ref-type="bibr" rid="B2">2</xref>-<xref ref-type="bibr" rid="B4">4</xref></p>
      <p>Ainda s&#227;o escassos os dados dispon&#237;veis sobre esse tema na popula&#231;&#227;o brasileira. Atualmente, 58% dos procedimentos de revasculariza&#231;&#227;o s&#227;o realizados por angioplastia, e a propor&#231;&#227;o entre angioplastia e cirurgia de revasculariza&#231;&#227;o aumentou significativamente entre 2008 e 2019. Essa mudan&#231;a reflete um novo perfil de pacientes, com caracter&#237;sticas menos favor&#225;veis &#224; cirurgia, o que pode impactar os resultados da cirurgia de revasculariza&#231;&#227;o do mioc&#225;rdio (CRM).<xref ref-type="bibr" rid="B5">5</xref>-<xref ref-type="bibr" rid="B8">8</xref></p>
      <p>As doen&#231;as do aparelho circulat&#243;rio foram respons&#225;veis por mais de 170.000 mortes de mulheres em 2019.<xref ref-type="bibr" rid="B1">1</xref> O sexo tem se mostrado um fator importante no tratamento da cardiopatia isqu&#234;mica. De modo geral, as mulheres tendem a desenvolver a doen&#231;a mais tardiamente devido &#224; prote&#231;&#227;o hormonal, o que contribui para uma maior mortalidade na CRM.<xref ref-type="bibr" rid="B9">9</xref>-<xref ref-type="bibr" rid="B13">13</xref> No entanto, alguns estudos n&#227;o identificaram essa diferen&#231;a.<xref ref-type="bibr" rid="B14">14</xref> Essa discrep&#226;ncia pode ser explicada pela exclus&#227;o de fatores espec&#237;ficos ao sexo &#8212; como menopausa precoce, disfun&#231;&#227;o microvascular e diferen&#231;as no di&#226;metro arterial &#8212; em grandes registros e estudos randomizados.<xref ref-type="bibr" rid="B15">15</xref>-<xref ref-type="bibr" rid="B17">17</xref></p>
      <p>Dada a alta incid&#234;ncia de DAC, especialmente entre mulheres, &#233; essencial gerar mais dados nacionais para preencher a lacuna de conhecimento sobre essas diferen&#231;as. Assim, o presente estudo teve como objetivo comparar as caracter&#237;sticas e os desfechos demogr&#225;ficos, cl&#237;nicos e cir&#250;rgicos entre homens e mulheres, al&#233;m de avaliar o impacto do sexo feminino como fator progn&#243;stico independente para a mortalidade na CRM.</p>

        <p>Trata-se de um estudo observacional e retrospectivo, no qual foi analisada uma coorte de pacientes submetidos &#224; CRM em um centro card&#237;aco privado (<xref ref-type="fig" rid="f01">Figura Central</xref>).</p>
        <p>Desde 1995, os dados de todos os procedimentos cir&#250;rgicos realizados em pacientes com mais de 18 anos submetidos &#224; CRM &#8212; eletiva ou n&#227;o, isolada ou associada a procedimento valvar, aneurisma ventricular ou cirurgia da aorta ascendente &#8212; s&#227;o coletados diariamente por profissionais de sa&#250;de. Os dados foram armazenados em uma plataforma eletr&#244;nica integrada ao sistema Tasy&#174; de prontu&#225;rio hospitalar (Philips, Blumenau, SC, Brasil). Os dados foram armazenados no servidor do hospital, com acesso restrito aos profissionais da pr&#243;pria institui&#231;&#227;o.</p>
        <p>Para garantir a fidelidade e a qualidade dos dados, foi realizada uma auditoria indireta por meio da avalia&#231;&#227;o de consist&#234;ncia, completude e acur&#225;cia das informa&#231;&#245;es. Al&#233;m disso, a incid&#234;ncia de caracter&#237;sticas perioperat&#243;rias foi analisada ano a ano para assegurar a credibilidade das vari&#225;veis. Como resultado, 136 pacientes com dados incompletos foram exclu&#237;dos da an&#225;lise final (<xref ref-type="fig" rid="f02">Figura 1</xref>).</p>

        <title>An&#225;lise estat&#237;stica</title>
        <p>Todas as an&#225;lises foram realizadas utilizando o software R, vers&#227;o 4.0.1.</p>
        <p>As vari&#225;veis categ&#243;ricas foram descritas em frequ&#234;ncias absolutas e percentuais, enquanto as vari&#225;veis cont&#237;nuas foram apresentadas como m&#233;dia e desvio padr&#227;o (DP). A normalidade das vari&#225;veis cont&#237;nuas foi avaliada por meio do teste de Anderson-Darling; a homogeneidade das vari&#226;ncias, pelo teste de Levene.</p>
        <p>Na compara&#231;&#227;o entre os grupos, todas as vari&#225;veis cont&#237;nuas apresentaram distribui&#231;&#227;o normal e foram avaliadas por meio do teste t de Student n&#227;o pareado para compara&#231;&#227;o das m&#233;dias. As vari&#225;veis categ&#243;ricas foram comparadas utilizando o teste exato de Fisher ou o teste do qui-quadrado, conforme apropriado.</p>
        <p>Uma an&#225;lise de sensibilidade adicional foi realizada com o uso do pareamento por escore de propens&#227;o, com o objetivo de minimizar poss&#237;veis vieses entre os grupos analisados. As vari&#225;veis inclu&#237;das no pareamento foram idade, &#237;ndice de massa corporal (IMC), cirurgia associada, doen&#231;a pulmonar obstrutiva cr&#244;nica (DPOC), urg&#234;ncia ou emerg&#234;ncia, infarto do mioc&#225;rdio (IM) nos &#250;ltimos 30 dias, classe de angina segundo classifica&#231;&#227;o da Canadian Cardiovascular Society (CCS), classe de dispneia segundo classifica&#231;&#227;o da New York Heart Association (NYHA), doen&#231;a arterial perif&#233;rica, CRM pr&#233;via, insufici&#234;ncia renal cr&#244;nica e classe de fra&#231;&#227;o de eje&#231;&#227;o.</p>
        <p>Por fim, regress&#245;es log&#237;sticas univariada e multivariada foram conduzidas para identificar fatores de risco independentes para mortalidade intra-hospitalar, considerando o sexo feminino como uma das vari&#225;veis de risco, juntamente com os demais fatores. O n&#237;vel de signific&#226;ncia foi estabelecido em 5%.</p>

      <title>Discuss&#227;o</title>
      <p>Os principais achados deste estudo evidenciaram diferen&#231;as entre homens e mulheres submetidos &#224; CRM quanto &#224;s caracter&#237;sticas pr&#233;-operat&#243;rias, complica&#231;&#245;es p&#243;s-operat&#243;rias e taxas de mortalidade. Ap&#243;s o controle da heterogeneidade das caracter&#237;sticas pr&#233;-operat&#243;rias por meio do pareamento por escore de propens&#227;o, a dura&#231;&#227;o da interna&#231;&#227;o hospitalar manteve-se maior entre as mulheres, enquanto as taxas de mortalidade n&#227;o apresentaram diferen&#231;a significativa entre os grupos. No entanto, na identifica&#231;&#227;o dos fatores progn&#243;sticos por meio da an&#225;lise de regress&#227;o log&#237;stica multivariada, o sexo feminino demonstrou forte associa&#231;&#227;o com maior risco de mortalidade intra-hospitalar.</p>
      <p>Comparar dois grupos com diferentes caracter&#237;sticas basais ajuda a explicar a observa&#231;&#227;o de resultados distintos. Neste estudo, observamos que as mulheres apresentaram maior mortalidade, mas tamb&#233;m tinham maior preval&#234;ncia de outros fatores progn&#243;sticos. Para isolar o efeito do sexo de poss&#237;veis fatores de confus&#227;o, utilizamos duas abordagens: o pareamento por escore de propens&#227;o e a regress&#227;o log&#237;stica multivariada. Ao aplicar o pareamento por escore de propens&#227;o para equilibrar as caracter&#237;sticas basais entre homens e mulheres, n&#227;o encontramos diferen&#231;a significativa na mortalidade intra-hospitalar. No entanto, ao isolar o sexo como vari&#225;vel de interesse na regress&#227;o log&#237;stica multivariada, o sexo feminino se mostrou um fator progn&#243;stico significativo para mortalidade operat&#243;ria. Esse resultado controverso nos leva &#224; reflex&#227;o sobre qual m&#233;todo deve ser priorizado na interpreta&#231;&#227;o dos achados. Cepeda et al.<xref ref-type="bibr" rid="B18">18</xref> realizaram uma compara&#231;&#227;o entre essas duas estrat&#233;gias e demonstraram que o pareamento por escore de propens&#227;o apresenta maior poder emp&#237;rico do que a regress&#227;o log&#237;stica multivariada apenas quando h&#225; sete ou menos eventos por fator de confus&#227;o. No presente estudo, o n&#250;mero de eventos por fator de confus&#227;o foi significativamente superior a esse limiar, o que torna a regress&#227;o log&#237;stica multivariada o m&#233;todo mais apropriado para sustentar nossa conclus&#227;o.</p>
      <p>A associa&#231;&#227;o entre o sexo feminino e a mortalidade operat&#243;ria em cirurgias card&#237;acas, especialmente na CRM, &#233; reconhecida h&#225; d&#233;cadas.<xref ref-type="bibr" rid="B19">19</xref>,<xref ref-type="bibr" rid="B20">20</xref> Modelos consagrados de estimativa de risco, como o modelo europeu EuroSCORE II e o score americano da Society of Thoracic Surgeons (STS), j&#225; consideram o sexo feminino como um fator de risco.<xref ref-type="bibr" rid="B21">21</xref>,<xref ref-type="bibr" rid="B22">22</xref> No entanto, as opini&#245;es sobre essa associa&#231;&#227;o permanecem divididas.</p>
      <p>Por exemplo, no modelo chin&#234;s de estimativa de risco (SinoSCORE), o sexo n&#227;o &#233; inclu&#237;do no c&#225;lculo de risco.<xref ref-type="bibr" rid="B23">23</xref> Uma an&#225;lise com mais de 35.000 pacientes submetidos &#224; CRM em Pequim<xref ref-type="bibr" rid="B24">24</xref> mostrou que as mulheres apresentaram maiores taxas de mortalidade intra-hospitalar em compara&#231;&#227;o aos homens (1,62% vs 1,30%, p=0,0248). No entanto, ap&#243;s a aplica&#231;&#227;o da regress&#227;o log&#237;stica multivariada, a idade avan&#231;ada &#8212; e n&#227;o o sexo feminino &#8212; foi identificada como fator progn&#243;stico independente para mortalidade intra-hospitalar p&#243;s-CRM. Esses achados sugerem que fatores locais, gen&#233;ticos e/ou sociais podem influenciar os desfechos, ressaltando a import&#226;ncia de se realizarem estudos semelhantes em diferentes contextos ao redor do mundo.</p>
      <p>Outro estudo realizou uma an&#225;lise secund&#225;ria do ensaio GOPCABE, utilizando o conjunto de dados da popula&#231;&#227;o original para avaliar a influ&#234;ncia do sexo feminino nos desfechos perioperat&#243;rios.<xref ref-type="bibr" rid="B25">25</xref> A regress&#227;o log&#237;stica multivariada indicou que o sexo feminino n&#227;o parece ser um fator progn&#243;stico para mortalidade em 30 dias ap&#243;s a CRM (OR: 0,703, IC 95%: 0,397-1,244, p=0,279). No entanto, essa an&#225;lise apresenta uma limita&#231;&#227;o comum a todas as an&#225;lises secund&#225;rias de ensaios cl&#237;nicos randomizados: a maioria desses estudos recruta apenas uma pequena parcela dos pacientes atendidos na pr&#225;tica cl&#237;nica cotidiana. Por isso, os achados podem n&#227;o ser representativos do restante da popula&#231;&#227;o, limitando a generaliza&#231;&#227;o das compara&#231;&#245;es entre homens e mulheres.</p>
      <p>A idade geralmente &#233; considerada um fator de risco, em conjunto com o sexo. Em um estudo recente que analisou 30 anos de experi&#234;ncia com revasculariza&#231;&#227;o do mioc&#225;rdio em um &#250;nico pa&#237;s &#8212; a Dinamarca &#8212;, observou-se que as mulheres apresentaram maior mortalidade em 30 dias, 1 ano e 10 anos, em compara&#231;&#227;o aos homens.<xref ref-type="bibr" rid="B13">13</xref> Um achado marcante foi que mulheres mais jovens apresentaram estimativas de mortalidade mais elevadas do que mulheres mais velhas. Esse resultado remete &#224; influ&#234;ncia de componentes biol&#243;gicos, como a menopausa e a terapia de reposi&#231;&#227;o hormonal, que podem estar associados &#224; mortalidade cardiovascular e ter um impacto compar&#225;vel ao da idade.<xref ref-type="bibr" rid="B25">25</xref></p>
      <p>No presente estudo, o IMC apresentou-se como um fator protetor, embora com limite superior do IC 95% de 0,99. Alguns estudos, no entanto, conclu&#237;ram que pacientes obesos submetidos &#224; CRM apresentam maior morbidade p&#243;s-operat&#243;ria e pior sobrevida em longo prazo.<xref ref-type="bibr" rid="B26">26</xref> O IMC elevado est&#225; fortemente associado a condi&#231;&#245;es como DM, dislipidemia, HAS e fatores inflamat&#243;rios, como a leptina.<xref ref-type="bibr" rid="B27">27</xref> Esses fatores est&#227;o relacionados &#224; DAC, que h&#225; d&#233;cadas se mant&#233;m como a principal causa de morte em adultos em todo o mundo.<xref ref-type="bibr" rid="B1">1</xref> Entretanto, quando se observa apenas pacientes com DAC submetidos &#224; CRM, estudos apontam que indiv&#237;duos com baixo peso s&#227;o mais vulner&#225;veis a complica&#231;&#245;es p&#243;s-operat&#243;rias que evoluem para &#243;bito operat&#243;rio.<xref ref-type="bibr" rid="B28">28</xref> Al&#233;m disso, ao incluir o IMC em modelos de regress&#227;o juntamente com outros fatores progn&#243;sticos, isolando sua influ&#234;ncia, os resultados podem indicar que, quanto maior o IMC, menor o risco de mortalidade operat&#243;ria.<xref ref-type="bibr" rid="B23">23</xref> Por outro lado, uma terceira linha de evid&#234;ncia aponta uma perspectiva diferente: uma an&#225;lise retrospectiva do banco de dados cir&#250;rgico do Royal Papworth Hospital demonstrou que o IMC n&#227;o se associa &#224; mortalidade operat&#243;ria, mas sim &#224; redu&#231;&#227;o da sobrevida tardia dos pacientes submetidos &#224; CRM.<xref ref-type="bibr" rid="B29">29</xref></p>
      <p>Este estudo apresenta algumas limita&#231;&#245;es. A an&#225;lise incluiu quase 10.000 pacientes cir&#250;rgicos, mas os dados foram obtidos de um &#250;nico hospital. Para alcan&#231;ar esse n&#250;mero de pacientes, foram utilizados dados acumulados ao longo de 27 anos de pr&#225;tica cir&#250;rgica. Testes laboratoriais e dados de exames complementares n&#227;o foram inclu&#237;dos, devido &#224; aus&#234;ncia dessas informa&#231;&#245;es no banco de dados. Trata-se de um estudo observacional, com foco exclusivo nos dados intra-hospitalares, o que o torna suscet&#237;vel a fatores de confus&#227;o. Al&#233;m disso, n&#227;o houve acompanhamento ap&#243;s a alta hospitalar, de modo que os desfechos avaliados se restringem ao per&#237;odo intra-hospitalar. Os resultados tamb&#233;m n&#227;o permitem afirmar, de forma definitiva, que o IMC seja um fator progn&#243;stico protetor para mortalidade operat&#243;ria em cirurgia card&#237;aca. O IC 95% pr&#243;ximo de 1 sugere a possibilidade de erro estat&#237;stico tipo II, especialmente quando considerado em conjunto com as demais limita&#231;&#245;es mencionadas.</p>
      <p>Apesar dessas limita&#231;&#245;es, os dados apresentados neste estudo s&#227;o consistentes com outras evid&#234;ncias dispon&#237;veis na literatura. A an&#225;lise de grandes bancos de dados continua sendo uma abordagem v&#225;lida, pois permite compara&#231;&#245;es com a popula&#231;&#227;o geral e contribui com informa&#231;&#245;es relevantes para este importante campo de estudo na cardiologia.<xref ref-type="bibr" rid="B30">30</xref>,<xref ref-type="bibr" rid="B31">31</xref></p>

      <title>Conclus&#227;o</title>
      <p>Mulheres apresentaram maior tempo de interna&#231;&#227;o e maior mortalidade operat&#243;ria. Embora a regress&#227;o log&#237;stica multivariada tenha identificado o sexo feminino como fator progn&#243;stico para mortalidade, ap&#243;s o pareamento por escore de propens&#227;o, essa diferen&#231;a n&#227;o se manteve significativa. Esses achados sugerem que a redu&#231;&#227;o das disparidades pode contribuir para a melhora dos desfechos em mulheres submetidas &#224; CRM.</p>

        <title>Abstract</title>
        <sec>
          <title>Background</title>
          <p> Women undergoing coronary artery bypass grafting (CABG) tend to have worse in-hospital outcomes, but it is unclear whether these differences are due to gender or to clinical factors.</p>
        </sec>
        <sec>
          <title>Objective</title>
          <p> To compare in-hospital outcomes between women and men undergoing CABG.</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p> This was a single-center, retrospective observational study analyzing data from 9,845 patients who underwent CABG between 1995 and 2022, of whom 1,947 (19.8%) were women. To evaluate female gender as a prognostic factor for in-hospital mortality, we used descriptive statistics, univariate and multivariate logistic regression, and propensity score matching. The significance level was set at 5%.</p>
        </sec>
        <sec>
          <title>Results</title>
          <p> Women were older (66.7 vs 62.19 years, p&lt;0.001), had lower body mass index (26.91 vs 27.64, p&lt;0.001), and had a higher prevalence of diabetes mellitus (34.0% vs 31.6%, p=0.045). They also had longer hospital stays (14.84 vs 13.13 days, p&lt;0.001) and higher operative mortality (4.8% vs 2.4%, p&lt;0.001). In logistic regression, female gender was associated with higher mortality (OR=1.51; 95% CI: 1.15-1.99; p=0.003). After matching, there was no significant difference in mortality (OR=1.20; 95% CI: 0.88-1.64; p=0.241), but length of hospital stay remained longer in women.</p>
        </sec>
        <sec>
          <title>Conclusion</title>
          <p> When clinical factors were matched between men and women, the mortality difference disappeared. This suggests that targeted interventions to reduce disparities may help improve mortality outcomes in women undergoing CABG.</p>

        <title>Introduction</title>
        <p>Cardiovascular disease is the leading cause of morbidity and mortality worldwide, with coronary artery disease (CAD) being the most common form.<xref ref-type="bibr" rid="B1">1</xref> In developing countries, interpretation of data from national registries is challenging due to population heterogeneity and significant differences between public and private hospitals.<xref ref-type="bibr" rid="B2">2</xref>-<xref ref-type="bibr" rid="B4">4</xref></p>
        <p>Data on this issue in the Brazilian population are still scarce. Currently, 58% of revascularization procedures are performed by angioplasty, and the ratio between angioplasty and coronary artery bypass grafting (CABG) increased significantly from 2008 to 2019. This shift reflects a new patient profile, with characteristics that are less favorable for surgery, which may impact CABG outcomes.<xref ref-type="bibr" rid="B5">5</xref>-<xref ref-type="bibr" rid="B8">8</xref></p>
        <p>Circulatory system diseases caused more than 170,000 deaths in women in 2019.<xref ref-type="bibr" rid="B1">1</xref> Gender has been shown to be an important factor in the treatment of ischemic heart disease. Overall, women tend to develop the disease later in life because of hormonal protection, which contributes to higher mortality rates in CABG.<xref ref-type="bibr" rid="B9">9</xref>-<xref ref-type="bibr" rid="B13">13</xref> However, some studies have not found this difference.<xref ref-type="bibr" rid="B14">14</xref><xref ref-type="bibr" rid="B1">1</xref>4&#8308; This discrepancy may be explained by the exclusion of gender-specific factors &#8212; such as early menopause, microvascular dysfunction, and differences in arterial diameter &#8212; in large registries and randomized trials.<xref ref-type="bibr" rid="B15">15</xref>-<xref ref-type="bibr" rid="B17">17</xref></p>
        <p>Because of the high incidence of CAD, especially in women, more national data are needed to fill the knowledge gap regarding these differences. Therefore, the aim of this study was to compare demographic, clinical, and surgical characteristics and outcomes between men and women as well as to assess the impact of female gender as an independent prognostic factor for mortality in CABG.</p>

        <title>Methods</title>
        <sec>
          <title>Study design</title>
          <p>This was a retrospective observational study that analyzed a cohort of patients who underwent CABG at a private cardiac center (<xref ref-type="fig" rid="f01001">Central Illustration</xref>).</p>
          <p>Since 1995, data from all surgical procedures performed in patients over 18 years of age who underwent CABG &#8212; whether elective or not, performed alone or combined with valve surgery, ventricular aneurysm repair, or ascending aorta surgery &#8212; have been collected daily by health care professionals. Data were stored in an electronic platform integrated with the Tasy&#174; hospital information system (Philips, Blumenau, SC, Brazil). They were stored on the hospital&#8217;s server, with access restricted to authorized staff from the institution.</p>
          <p>To ensure data accuracy and quality, an indirect audit was performed by evaluating the consistency, completeness, and accuracy of information. In addition, the incidence of perioperative characteristics was analyzed year by year to confirm the reliability of variables. As a result, 136 patients with incomplete data were excluded from the final analysis (<xref ref-type="fig" rid="f02001">Figure 1</xref>).</p>

          <title>Statistical analysis</title>
          <p>All analyses were performed using R software, version 4.0.1.</p>
          <p>Categorical variables were reported as absolute numbers and percentages, while continuous variables were presented as means and standard deviations (SD). The normality of continuous variables was assessed using the Anderson-Darling test, and the homogeneity of variances was tested using Levene&#8217;s test.</p>
          <p>For group comparisons, all continuous variables had a normal distribution and were analyzed using the unpaired Student&#8217;s t-test to compare means. Categorical variables were compared using Fisher&#8217;s exact test or the chi-square test as appropriate.</p>
          <p>An additional sensitivity analysis was performed using propensity score matching to minimize potential bias between the groups. Variables included in the matching process were age, body mass index (BMI), combined surgery, chronic obstructive pulmonary disease (COPD), urgent or emergency status, myocardial infarction (MI) within the past 30 days, angina class according to the Canadian Cardiovascular Society (CCS), dyspnea class according to the New York Heart Association (NYHA), peripheral artery disease, prior CABG, chronic kidney disease, and ejection fraction class.</p>
          <p>Univariate and multivariate logistic regressions were then performed to identify independent risk factors for in-hospital mortality, considering female gender as one of the risk variables, along with the other factors. The significance level was set at 5%.</p>

        <title>Results</title>
        <p>Significant differences were observed in baseline characteristics between men and women (<xref ref-type="table" rid="t1001">Table 1</xref>). Women were older, had lower BMI, and showed a higher prevalence of SAH and diabetes mellitus (DM). They also presented with more advanced functional classes of dyspnea and angina. However, compared to men, they had lower rates of recent MI, chronic kidney disease, and prior cardiac surgeries.</p>

        <title>Discussion</title>
        <p>The main findings of this study showed differences between men and women who underwent CABG in terms of preoperative characteristics, postoperative complications, and mortality rates. After adjusting for differences in baseline characteristics using propensity score matching, women still had longer hospital stays, but mortality rates were no longer significantly different between the groups. However, in the multivariable logistic regression analysis used to identify prognostic factors, female gender remained strongly associated with a higher risk of in-hospital mortality.</p>
        <p>Comparing two groups with different baseline characteristics helps explain why their outcomes may differ. In this study, we found that women had higher mortality, but they also had a higher prevalence of other risk factors. To isolate the effect of gender from potential confounding factors, we used two approaches: propensity score matching and multivariable logistic regression. When we applied propensity score matching to balance the baseline characteristics between men and women, there was no significant difference in in-hospital mortality. However, when gender was analyzed as an independent variable in the multivariable logistic regression model, the female gender emerged as a significant predictor of operative mortality. This conflicting result raises the question of which method should be prioritized when interpreting the findings. Cepeda et al.<xref ref-type="bibr" rid="B18">18</xref> compared these two strategies and showed that propensity score matching only has greater empirical power than multivariable logistic regression when there are seven or fewer events per confounding variable. In our study, the number of events per confounder was well above that threshold, making multivariable logistic regression the more appropriate method to support our conclusion.</p>
        <p>The link between female gender and operative mortality in cardiac surgery &#8212; particularly in CABG &#8212; has been acknowledged for decades.<xref ref-type="bibr" rid="B19">19</xref>,<xref ref-type="bibr" rid="B20">20</xref> Established risk prediction models, such as the European EuroSCORE II and the U.S. Society of Thoracic Surgeons (STS) score, already include female gender as a risk factor.<xref ref-type="bibr" rid="B21">21</xref>,<xref ref-type="bibr" rid="B22">22</xref> However, opinions on this association remain divided.</p>
        <p>For example, in the Chinese risk prediction model (SinoSCORE), gender is not included as a risk factor.<xref ref-type="bibr" rid="B23">23</xref> An analysis of more than 35,000 patients who underwent CABG in Beijing<xref ref-type="bibr" rid="B24">24</xref> showed that women had higher in-hospital mortality rates compared to men (1.62% vs 1.30%, p=0.0248). However, after applying multivariable logistic regression, increased age &#8212; not female gender &#8212; was found to be an independent predictor of in-hospital mortality after CABG. Such findings suggest that local, genetic, and/or social factors may influence outcomes, underscoring the need for similar studies across diverse populations worldwide.</p>
        <p>Another study conducted a secondary analysis of the GOPCABE trial using data from the original study population to assess the impact of female gender on perioperative outcomes.<xref ref-type="bibr" rid="B25">25</xref> The multivariable logistic regression showed that female gender did not appear to be a predictor of 30-day mortality after CABG (OR: 0.703, 95% CI: 0.397-1.244, p=0.279). However, this analysis shares a common limitation with all secondary analyses of randomized clinical trials: most trials enroll only a small portion of the patients typically treated in everyday clinical practice. As a result, the findings may not represent the broader population, limiting the generalizability of comparisons between men and women.</p>
        <p>Age is usually considered a risk factor alongside gender. In a recent study analyzing 30 years of experience with CABG in a single country &#8212; Denmark &#8212; women showed higher mortality rates at 30 days, 1 year, and 10 years compared to men.<xref ref-type="bibr" rid="B13">13</xref> A striking finding was that younger women had higher estimated mortality rates than older women. This points to the possible influence of biological factors, such as menopause and hormone replacement therapy, which may be linked to cardiovascular mortality and have an impact comparable to that of age.<xref ref-type="bibr" rid="B25">25</xref></p>
        <p>In the present study, BMI appeared as a protective factor, although the upper limit of the 95% CI was 0.99. However, some studies have found that obese patients undergoing CABG experience higher postoperative morbidity and worse long-term survival.<xref ref-type="bibr" rid="B26">26</xref> Increased BMI is strongly associated with conditions such as DM, dyslipidemia, SAH, and inflammatory markers such as leptin.<xref ref-type="bibr" rid="B27">27</xref> These factors are linked to CAD, which has remained the leading cause of death among adults worldwide for decades.<xref ref-type="bibr" rid="B1">1</xref> When focusing specifically on patients with CAD undergoing CABG, some studies have shown that underweight individuals are more vulnerable to postoperative complications that can lead to operative death.<xref ref-type="bibr" rid="B28">28</xref> Moreover, when BMI is included in regression models along with other prognostic factors &#8212; allowing its effect to be isolated &#8212; the results may suggest that higher BMI is associated with a lower risk of operative mortality.<xref ref-type="bibr" rid="B23">23</xref> On the other hand, a third line of evidence offers a unique perspective: a retrospective analysis of the surgical database from the Royal Papworth Hospital found no association between BMI and operative mortality, but did show reduced long-term survival among patients with higher BMI undergoing CABG.<xref ref-type="bibr" rid="B29">29</xref></p>
        <p>This study has several limitations. Although it included nearly 10,000 surgical patients, data came from just one hospital. To reach that number, data were gathered over 27 years of surgical practice. Laboratory tests and imaging data were not included because they were not available in the database. Since this was an observational study based only on in-hospital data, the results may have been affected by other confounders not accounted for. In addition, there was no follow-up after patients were discharged from the hospital, so all outcomes are limited to the in-hospital period. The results also do not allow us to say for certain that BMI is a protective factor for operative mortality in cardiac surgery. The 95% CI being close to 1 suggests there may be a type II statistical error, especially when considered along with the other limitations already mentioned.</p>
        <p>Despite these limitations, data in this study align with other evidence in literature. Analyzing large databases is still a valid approach because it allows for comparisons with the general population and offers valuable insights into this important area of cardiology research.<xref ref-type="bibr" rid="B30">30</xref>,<xref ref-type="bibr" rid="B31">31</xref></p>

        <title>Conclusion</title>
        <p>Women had longer hospital stays and higher operative mortality. Although multivariable logistic regression identified female gender as a prognostic factor for mortality, this difference was no longer significant after propensity score matching. These findings suggest that reducing disparities may help improve outcomes in women undergoing CABG.</p>
