        <sec>
          <title>Fundamentos</title>
          <p> Estudos recentes associaram a repolariza&#231;&#227;o precoce (RP) com um risco de fibrila&#231;&#227;o ventricular aumentado, especialmente nas deriva&#231;&#245;es V1-V3 e nas regi&#245;es inferior e lateral. No entanto, dados sobre a popula&#231;&#227;o brasileira s&#227;o limitados.</p>

        <sec>
          <title>Objetivos</title>
          <p> Estimar o impacto da RP na sobrevida e na ocorr&#234;ncia de arritmias ventriculares (AVs) em um per&#237;odo de 10 anos em um hospital universit&#225;rio.</p>
        </sec>
        <sec>
          <title>M&#233;todos</title>
          <p> Este estudo retrospectivo do tipo coorte incluiu pacientes com RP no eletrocardiograma a partir do banco de dados do hospital. An&#225;lise descritiva foi conduzida para descrever o perfil e as caracter&#237;sticas dos pacientes. O m&#233;todo de Kaplan-Meier foi usado para avaliar as diferen&#231;as entre os tipos de RP. Modelos de regress&#227;o de Cox foram aplicados para avaliar os riscos de morte e AV. calculando os hazard ratios brutos e ajustados. O n&#237;vel de signific&#226;ncia adotado na an&#225;lise estat&#237;stica foi de 5%.</p>
        </sec>
        <sec>
          <title>Resultados</title>
          <p> A popula&#231;&#227;o do estudo foi predominantemente do sexo masculino, com idade m&#233;dia de 45,6 anos; 2,7% apresentaram AV (cinco no grupo com RP lateral, quatro no grupo com RP inferior e quatro no grupo com RP &#237;nfero-lateral). Foram observadas diferen&#231;as significativas na idade, sexo, e ocorr&#234;ncia de insufici&#234;ncia card&#237;aca com fra&#231;&#227;o de eje&#231;&#227;o reduzida entre os grupos. Em rela&#231;&#227;o &#224; mortalidade por todas as causas, 2,3% dos pacientes foram a &#243;bito (cinco no grupo com RP lateral, um no grupo com RP inferior e cinco no grupo com RP &#237;nfero-lateral). Somente idade mostrou uma diferen&#231;a estatisticamente significativa. Observou-se diferen&#231;as significativas na ocorr&#234;ncia de morte e de AV entre os grupos (p=0,7 e p=0,5, respectivamente).</p>
        </sec>
        <sec>
          <title>Conclus&#227;o</title>
          <p> A RP n&#227;o levou a uma maior incid&#234;ncia de AV nem de mortalidade por todas as causas na popula&#231;&#227;o estudada.</p>

      <title>Introdu&#231;&#227;o</title>
      <p>A preval&#234;ncia de repolariza&#231;&#227;o precoce (RP) varia entre 2,3% e 29,3%, variando conforme idade, ra&#231;a e sexo.<xref ref-type="bibr" rid="B1">1</xref> A RP &#233; comumente observada em atletas de alto desempenho, com uma incid&#234;ncia de cerca de 1/3. <p>Ha&#239;ssaguerre et al.<xref ref-type="bibr" rid="B4">4</xref>encontraram uma alta preval&#234;ncia de RP em pacientes que foram ressuscitados ap&#243;s parada card&#237;aca causada por fibrila&#231;&#227;o ventricular (FV) abortada, e os pacientes com RP apresentaram um risco duas vezes maior de recorr&#234;ncia de FV.<xref ref-type="bibr" rid="B4">4</xref> No estudo CASPER, os pacientes que apresentaram morte s&#250;bita abortada e cora&#231;&#227;o estruturalmente normal, a RP foi a causa em 7,9% da amostra.<xref ref-type="bibr" rid="B6">6</xref> O grau de eleva&#231;&#227;o do ponto J nas deriva&#231;&#245;es inferiores pode se correlacionar om a magnitude do risco. O risco relativo de morte foi 1,3 naqueles com 0,1mV e tr&#234;s vezes naqueles com 0,2mV;<xref ref-type="bibr" rid="B1">1</xref> o risco de arritmia ventricular foi maior nos pacientes com 0,2 mV, com a inclina&#231;&#227;o do segmento ST resultando em um progn&#243;stico significativo, com uma orienta&#231;&#227;o horizontal ou descendente associada com um risco maior de arritmia.<xref ref-type="bibr" rid="B7">7</xref></p>

        <title>An&#225;lise dos dados</title>
        <p>Foi realizada uma an&#225;lise explorat&#243;ria das caracter&#237;sticas da amostra, e os resultados apresentados em propor&#231;&#245;es ou m&#233;dias, dependendo da natureza da vari&#225;vel. Subsequentemente, as caracter&#237;sticas da amostra foram estratificadas de acordo com mortes e arritmias ventriculares para an&#225;lise posterior. Diferen&#231;as entre os grupos foram avaliadas pelo teste do qui-quadrado para dados categ&#243;ricos e pelo teste t para dados cont&#237;nuos, com signific&#226;ncia estat&#237;stica estabelecida em p &#8804; 0,05. Uma popula&#231;&#227;o controle foi usada para an&#225;lise comparativa.</p>
        <p>Para a an&#225;lise de sobrevida considerando os v&#225;rios tipos de RP, o tempo foi calculado em meses (0-120 meses), e o evento (falha) foi definido como &#243;bito do paciente durante o seguimento de 10 anos. Os pacientes que sobreviveram al&#233;m do per&#237;odo de acompanhamento foram censurados &#224; direita. Para a an&#225;lise do tempo at&#233; a ocorr&#234;ncia de arritmia ventricular, o tempo foi calculado em meses, e o evento (falha) foi definido como ocorr&#234;ncia de arritmia durante o seguimento de 10 anos. Os pacientes com arritmia ventricular ap&#243;s o per&#237;odo de seguimento foram considerados dados censurados &#224; direita.</p>
        <p>Para estimar a probabilidade de sobrevida e o risco cumulativo ao longo do tempo para morte e arritmia ventricular em rela&#231;&#227;o aos tipos de RP, foi empregado o m&#233;todo de Kaplan-Meier. Diferen&#231;as entre curvas de sobrevida para ambos os desfechos foram avaliadas usando o teste de log-rank, com signific&#226;ncia estabelecida em p&#8804;0,05.</p>
        <p>Para analisar a rela&#231;&#227;o entre RP e o tempo at&#233; ocorr&#234;ncia de morte ou arritmia na popula&#231;&#227;o do estudo, foi usada a regress&#227;o de Cox. Hazard ratios (HRs) brutos e ajustados foram obtidos para as vari&#225;veis de confus&#227;o, al&#233;m do Intervalo de Confian&#231;a de 95% (IC95%). A an&#225;lise dos dados foi realizada usando o programa R vers&#227;o 4.3.0.</p>

      <title>Discuss&#227;o</title>
      <p>Achados recentes de Liu et al.<xref ref-type="bibr" rid="B11">11</xref> sugerem que a RP seja um fen&#244;meno transiente, aparecendo em ECGs basais, mas potencialmente se resolvendo em medidas subsequentes. Tal natureza transiente poderia indicar um curso benigno, conforme discutido em seu estudo coorte.</p>
      <p>A eleva&#231;&#227;o do ponto J no ECG nas 12 deriva&#231;&#245;es, compat&#237;vel com RP, tem sido tradicionalmente vista como um achado inocente em indiv&#237;duos sadios.<xref ref-type="bibr" rid="B12">12</xref>,<xref ref-type="bibr" rid="B13">13</xref></p>
      <p>A amostra do estudo, como esperado, consistiu em indiv&#237;duos de meia idade com um n&#250;mero menor de comorbidades em compara&#231;&#227;o aos tipicamente relatados em estudos de cardiologia. Embora n&#227;o seja um estudo randomizado, as caracter&#237;sticas dos grupos foram muito similares, e somente o grupo com RP lateral mostrou uma incid&#234;ncia mais alta de paciente hipertensos, reduzindo o efeito de vi&#233;s de sele&#231;&#227;o, embora ainda presente. Estudos pr&#233;vios relataram idades m&#233;dias de popula&#231;&#245;es com RP igual ou muito similar &#224; da amostra deste estudo.<xref ref-type="bibr" rid="B1">1</xref></p>
      <p>Ao analisar as caracter&#237;sticas dos pacientes que foram a &#243;bito, a idade resultou em uma diferen&#231;a estatisticamente significativa, confirmando a associa&#231;&#227;o esperada entre idade mais avan&#231;ada e maior risco de mortalidade. Quanto &#224; incid&#234;ncia de arritmias ventriculares, sexo e Insufici&#234;ncia Card&#237;aca com Fra&#231;&#227;o de Eje&#231;&#227;o Reduzida (ICFEr) resultaram em diferen&#231;as estatisticamente significativas. Os pacientes com arritmia ventricular consistiram em pacientes jovens, do sexo masculino, com uma not&#225;vel porcentagem de pacientes com ICFEr, indicando uma predisposi&#231;&#227;o &#224; arritmog&#234;nese por meio de v&#225;rios mecanismos de cardiomiopatia. Idade, ap&#243;s ajuste no modelo Cox, apresentou-se como um fator protetor contra arritmia ventricular.<xref ref-type="bibr" rid="B3">3</xref></p>
      <p>Curvas de sobrevida analisadas usando o m&#233;todo de Kaplan-Meier revelaram altas taxas de sobrevida em todos os grupos, mas sem diferen&#231;a estatisticamente significativa (p=0,7). No entanto, o grupo com RP &#237;nfero-lateral mostrou o risco cumulativo mais alto. Consistente com os achados do estudo de Tikkanen et al.<xref ref-type="bibr" rid="B1">1</xref> em 2009, os pacientes com RP inferior mostraram uma taxa mais alta de mortalidade cardiovascular, principalmente quando o ponto J era maior que 2mm. No entanto, ao analisar a mortalidade por todas as causas, n&#227;o foi observada diferen&#231;a estatisticamente significativa entre os grupos com RP lateral e RP inferior. A porcentagem da amostra sem arritmia ventricular foi alta em todos os grupos, sem diferen&#231;as estatisticamente significativas (p=0,5). Diferentemente da mortalidade, os pacientes com RP inferior mostraram um risco cumulativo mais alto de arritmia ventricular, enquanto os pacientes om RP &#237;nfero-lateral apresentaram o risco mais baixo. Achados similares foram relatados por Tikkanen et al.<xref ref-type="bibr" rid="B1">1</xref> em 2009 sobre arritmia ventricular, com uma maior incid&#234;ncia observada no grupo com RP inferior quando o ponto J era maior que 2mm.<xref ref-type="bibr" rid="B1">1</xref></p>
      <p>Embora a mortalidade n&#227;o tenha sido estatisticamente diferente entre os grupos, foi interessante notar a diferen&#231;a observada no HR ajustado para idade e sexo no grupo com RP &#237;nfero-lateral. Esse resultado sugere um fator de risco potencial, com uma chance 1,2 vezes maior de morte. Contudo, o amplo intervalo de confian&#231;a, incluindo o zero, pode ser atribu&#237;do ao pequeno n&#250;mero de mortes. Mais recentemente, Ha&#239;ssaguerre et al.<xref ref-type="bibr" rid="B4">4</xref> observaram uma frequ&#234;ncia mais alta de RP &#237;nfero-lateral em pacientes com morte s&#250;bita abortada por taquicardia ventricular, corroborando os achados deste estudo, que a categoria de RP est&#225; associada com um risco maior de arritmia ventricular e morte.<xref ref-type="bibr" rid="B4">4</xref></p>
      <p>Apesar das importantes contribui&#231;&#245;es a esta &#225;rea do conhecimento, o presente estudo possui v&#225;rias limita&#231;&#245;es. Os dados foram obtidos retrospectivamente por meio da an&#225;lise dos prontu&#225;rios m&#233;dicos, e muitos detalhes dependeram das descri&#231;&#245;es de terceiros, que &#224;s vezes resultou em informa&#231;&#245;es incompletas ou faltantes. A extra&#231;&#227;o de dados dos prontu&#225;rios m&#233;dicos tamb&#233;m foi limitada por quest&#245;es t&#233;cnicas e acesso restrito a esses documentos. Ainda, o uso de dados secund&#225;rios, tais como o SIM, dadas as informa&#231;&#245;es escassas sobre &#243;bitos, dificulta a categoriza&#231;&#227;o das causas de &#243;bito. Outra limita&#231;&#227;o foi o pequeno n&#250;mero de desfechos (morte e arritmia ventricular), o que pode comprometer o poder da amostra para detectar diferen&#231;as estatisticamente significativas. &#201; essencial que m&#233;dicos cl&#237;nicos, particularmente cardiologistas, reconhe&#231;am a import&#226;ncia desta nova evid&#234;ncia, e se atentem quando os pacientes apresentarem esses achados eletrocardiogr&#225;ficos. O tamanho amostral relativamente pequeno e o n&#250;mero limitado de desfechos, particularmente morte e arritmias ventriculares, pode haver impactado o poder estat&#237;stico do estudo e a capacidade de detectar diferen&#231;as significativa entre os grupos com RP. Ainda, a natureza unic&#234;ntrica do estudo e os crit&#233;rios de exclus&#227;o podem limitar a possibilidade de extrapolar os achados para popula&#231;&#245;es mais amplas. A qualidade e a disponibilidade dos dados, bem como os potenciais fatores de confus&#227;o n&#227;o completamente controlados, adicionam complexidade &#224; interpreta&#231;&#227;o dos resultados. A natureza observacional do estudo impede o estabelecimento de rela&#231;&#245;es causais entre RP e os desfechos. Per&#237;odos mais longos de acompanhamento, e estudos prospectivos multic&#234;ntricos s&#227;o necess&#225;rios para validar os achados e abordar essas limita&#231;&#245;es.</p>

      <title>Conclus&#245;es</title>
      <p>No presente estudo, observamos que os pacientes com repolariza&#231;&#227;o ventricular precoce, particularmente nas deriva&#231;&#245;es inferiores e laterais, n&#227;o apresentaram uma incid&#234;ncia significativamente mais alta de arritmias ventriculares ou mortalidade. Esses resultados sugerem que a presen&#231;a isolada de RP pode n&#227;o ser um forte preditor de desfechos adversos na popula&#231;&#227;o de nosso estudo. Contudo, outros estudos prospectivos e randomizados s&#227;o necess&#225;rios para validar esses resultados e direcionar a pr&#225;tica cl&#237;nica quanto ao manejo dos pacientes com repolariza&#231;&#227;o ventricular precoce.</p>

        <title>Abstract</title>
        <sec>
          <title>Background</title>
          <p> Recent research has linked early repolarization (ER) with increased ventricular fibrillation risk, especially in leads V1-V3 and in inferior and lateral regions. However, data on the Brazilian population are limited.</p>
        </sec>
        <sec>
          <title>Objective</title>
          <p> To estimate the impact of ER on survival and the occurrence of ventricular arrhythmias (VA) in patients over a 10-year period at a university hospital.</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p> This retrospective cohort study included patients with ER on electrocardiogram from the hospital database. Descriptive analysis was conducted to describe patients&#8217; profile and characteristics. Kaplan-Meier method was utilized to analyze survival curves, with the log-rank test employed to assess differences between ER types. Cox regression models were applied to evaluate the risks of death and VA, calculating both gross and adjusted hazard ratios. The level of significance adopted in the statistical analysis was 5%.</p>
        </sec>
        <sec>
          <title>Results</title>
          <p> The study population was predominantly male, average age of 45.6 years; 2.7% experienced VA (five in the group with lateral ER, four in the group with inferior ER group, and four in the ER inferolateral group). Significant differences were observed in age, sex, and HFrEF between the groups. Regarding all-cause mortality, 2.3% of patients died (five in the group with lateral ER, one in the group with inferior ER group, and five in the ER inferolateral group). Only age showed a statistically significant difference. There were significant differences in both death and VA between the groups (p=0.7 and p=0.5, respectively).</p>
        </sec>
        <sec>
          <title>Conclusion</title>
          <p> ER did not lead to a higher incidence of VA or all-cause mortality in this cohort.</p>

        <title>Introduction</title>
        <p>The prevalence of early repolarization (ER) ranges from 2.3% to 29.3%, varying with age, race, and sex.<xref ref-type="bibr" rid="B1">1</xref> ER is also commonly found in high-performance athletes, with an incidence is around 1/3. However, the yearly incidence of sudden cardiac death in this population is very low.<xref ref-type="bibr" rid="B2">2</xref> For many years, ER was considered a benign finding on the electrocardiogram (ECG), but recent studies suggest that this pattern in leads other than V1-V3 (especially inferior leads) is associated with a higher risk of ventricular arrhythmias and sudden death.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B3">3</xref>-<xref ref-type="bibr" rid="B5">5</xref></p>
        <p>Ha&#239;ssaguerre et al.<xref ref-type="bibr" rid="B4">4</xref> found a high prevalence of ER in patients who were resuscitated after cardiac arrest due to ventricular fibrillation (VF), and patients with ER presented twice the risk of VF recurrence.<xref ref-type="bibr" rid="B4">4</xref> In the CASPER study, patients who had aborted sudden death with structurally normal hearts, ER was found as the cause in 7.9% of the sample.<xref ref-type="bibr" rid="B6">6</xref> The degree of elevation of point J in the inferior leads can correlate with the magnitude of risk. The relative risk of death was 1.3 in those with 0.1 mV and three times in those with 0.2 mV;<xref ref-type="bibr" rid="B1">1</xref> the risk of ventricular arrhythmia was bigger in those with 0.2 mV, with the inclination of the ST segment resulting in a meaningful prognosis, with a horizontal or downward orientation associated with a higher arrhythmic risk.<xref ref-type="bibr" rid="B7">7</xref></p>
        <p>Screening and management of patients with ER and the risk of sudden death are a challenge.<xref ref-type="bibr" rid="B8">8</xref> The American Heart Association (AHA) recommends that, when detected, standard ER should be investigated in case of unexplained syncope or family history of sudden death and arrhythmogenic ECG features.<xref ref-type="bibr" rid="B8">8</xref> The low incidence of arrhythmic events also presents a major challenge for effective interventions that can reduce arrhythmic risk.<xref ref-type="bibr" rid="B2">2</xref></p>
        <p>Although quinidine and isoproterenol can be effective in managing patients with VF, the data supporting their use remains limited to small samples.<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B10">10</xref> Due to the limited data in the literature, especially concerning the Brazilian population, this study aims to evaluate whether patients with different types of ER present a higher risk of ventricular arrhythmias and death.</p>

        <title>Methods</title>
        <p>Temporal changes in the ER pattern were not assessed in this study; only a single ECG was evaluated. ER was classified based on the morphology observed in leads: lateral (leads I, aVL, V5-V6), inferior (leads II, III, aVF), and infero-lateral (combination of these leads).</p>
        <p>This retrospective cohort study included 478 patients from a university hospital with an electrocardiographic diagnosis of early ventricular repolarization between 2011 and 2022. The study was conducted in accordance with regulatory guidelines involving human subjects, and the research project was approved by the Institutional Research Ethics Committee (number 2021/0585).</p>
        <p>Inclusion criteria encompassed all patients diagnosed (by cardiologists) with early ventricular repolarization (J point elevation &#8805; 1 mm in at least two contiguous leads, with a slurring or notching pattern, excluding leads V1 to V3 &#8211; <xref ref-type="fig" rid="f02001">Figure 1</xref>) who underwent ECG evaluation at the university hospital and were older than 18 years between 2011 and 2022. Exclusion criteria comprised the impossibility of performing an electrocardiogram for measures of ventricular repolarization parameters, secondary changes from ventricular repolarization, and patients with pacemakers and other cardiac devices.</p>

        <p>The search for the term &#8220;early repolarization&#8221; through the hospital database retrieved 2,172 patients. Of those, 923 were selected for review based on the inclusion criteria. After thorough evaluation, 478 patients were diagnosed with ER.</p>

          <title>Variables of the study</title>
          <p>Exposure variables included early ventricular repolarization, classified into three groups: lateral, inferior, and inferolateral. Outcome variables included sustained ventricular arrhythmia and all-cause mortality. Confounding variables controlled included age and sex.</p>
        </sec>
        <sec>
          <title>Data analysis</title>
          <p>An exploratory analysis of sample characteristics was conducted, and the results were presented as proportions or averages, depending on the nature of the variable. Subsequently, the characteristics of the sample were stratified according to deaths and ventricular arrhythmias for further analysis. Group differences were assessed using the chi-square test for categorical data and the t-test for continuous data, with statistical significance set at p-value &#8804; 0.05. A control population was used for comparative analysis.</p>
          <p>For survival analysis concerning various types of ER, time was calculated in months (0-120 months), and the event (failure) was defined as patient death during the 10-year follow-up period. Patients who survived beyond the follow-up period were censored on the right. Similarly, for analysis of time to ventricular arrhythmia, time was calculated in months, and the event (failure) was defined as the occurrence of arrhythmia during the 10-year follow-up. Patients without ventricular arrhythmia after the follow-up period were considered censored data on the right.</p>
          <p>To estimate survival probability and cumulative risk over time for death and ventricular arrhythmia in relation to the types of ER, the Kaplan-Meier method was employed. Differences between survival curves for both outcomes were assessed using the log-rank test, with significance set at p-value &#8804; 0.05.</p>
          <p>To analyze the relationship between ER and time until death or arrhythmia occurrence in the study population, Cox regression was utilized. Gross and adjusted hazard ratios (HR) were obtained for confounding variables, along with 95% confidence intervals (95 CI). Data analysis was performed using R version 4.3.0.</p>

        <title>Results</title>
        <p>During follow-up, ventricular arrhythmias included sustained ventricular tachycardia and ventricular fibrillation as defined by ICD-10. Causes of death were primarily cardiac conditions, including sudden cardiac arrest.</p>
        <p>A total of 2172 patients with a previous diagnosis of ER on the ECG were initially identified from the hospital database. Of these, 941 patients were selected for review based on specific inclusion criteria. After thorough evaluation by three cardiologists, 478 patients were confirmed to have a genuine diagnosis of ER and were included in the study. The remaining patients who did not fulfill the criteria for ER were used as controls (<xref ref-type="fig" rid="f03001">Figure 2</xref> and <xref ref-type="fig" rid="f01001">Central Illustration</xref>).</p>

        <title>Discussion</title>
        <p>Recent findings from Liu et al.<xref ref-type="bibr" rid="B11">11</xref> suggest that ER is a transient phenomenon, appearing in baseline ECGs but potentially resolving in subsequent measurements. This transient nature could indicate a benign course, as discussed in their cohort study.</p>
        <p>The elevation of the J point in the ECG in 12 leads, compatible with ER, has traditionally been viewed as an innocent finding in healthy individuals.<xref ref-type="bibr" rid="B12">12</xref>,<xref ref-type="bibr" rid="B13">13</xref></p>
        <p>The study sample, as expected, consisted of middle-aged individuals with fewer comorbidities than typically seen in cardiology studies. Although not a randomized study, the characteristics of the groups were very similar, with only the lateral ER group showing a higher incidence of hypertensive patients, reducing the effect of selection bias, although it still exists. Previous studies have reported average ages of ER populations equal to or very similar to the sample in this study.<xref ref-type="bibr" rid="B1">1</xref></p>
        <p>When analyzing the characteristics of the sample that died, age resulted in a statistically significant difference, confirming the expected association between older age and higher mortality risk. Regarding the incidence of ventricular arrhythmias, sex and heart failure with reduced ejection fraction (HFrEF) resulted in statistically significant differences. Patients with ventricular arrhythmia consisted of young males, with a notable percentage presenting HFrEF, indicating a predisposition to ventricular arrhythmogenesis through various cardiomyopathy mechanisms. Age, after adjustment in the Cox model, emerged as a protective factor against ventricular arrhythmia.<xref ref-type="bibr" rid="B3">3</xref></p>
        <p>Survival curves analyzed using the Kaplan-Meier method revealed high survival rates across all groups, with no statistically significant differences (p=0.7). However, the group with inferolateral ER exhibited the highest cumulative risk. Consistent with the findings from Tikkanen et al.<xref ref-type="bibr" rid="B1">1</xref> in 2009, patients with inferior ER showed a higher rate of cardiovascular mortality, especially when the J point was higher than 2mm. However, when analyzing mortality from all causes, no significant difference was found between the groups with lateral ER and inferior ER. The percentage of the sample free of ventricular arrhythmia was high across all groups, with no statistically significant differences (p=0.5). Unlike mortality, patients with inferior ER exhibited a greater cumulative risk of ventricular arrhythmia, while the infero-lateral group showed the lowest risk. Similar findings were reported by Tikkanen et al.<xref ref-type="bibr" rid="B1">1</xref> in 2009 regarding ventricular arrhythmia, with a greater incidence observed in the inferior ER group when the J point was higher than 2mm.<xref ref-type="bibr" rid="B1">1</xref></p>
        <p>Although mortality did not significantly differ between groups, it was interesting to note the difference observed in the HR adjusted for age and sex in the group with inferolateral ER. This finding suggests a potential risk factor with 1.2 times more chance of death. However, the wide confidence interval, including zero, may be attributed to the small number of deaths. More recently, Ha&#239;ssaguerre et al.<xref ref-type="bibr" rid="B4">4</xref> observed a higher frequency of inferolateral ER in patients with sudden death aborted by ventricular tachycardia, corroborating the findings of this study that ER category is associated with a greater risk of ventricular arrhythmia and death.<xref ref-type="bibr" rid="B4">4</xref></p>
        <p>Despite making important contributions to this field of knowledge, this study has several limitations. Data were obtained retrospectively through the analysis of medical records, and many details depended on third-party descriptions, which sometimes resulted in incomplete or missing information. Data extraction from medical records was also limited by technical issues and restricted access to these documents. Additionally, the use of secondary data, such as the SIM, due to scarce information regarding deaths made it challenging to categorize the cause of death. Another limitation was the low number of outcomes (death and ventricular arrhythmia), which could compromise the sample&#8217;s power to detect statistically significant differences. It is essential for clinicians, particularly cardiologists, to recognize the significance of this new evidence and pay close attention to patients with these electrocardiographic findings.</p>
        <p>Also, the design of a retrospective cohort study has several limitations that warrant consideration. Firstly, its retrospective design relying on pre-existing data from medical records introduces potential biases and limitations in data completeness and accuracy. The relatively small sample size and limited number of outcomes, particularly deaths and ventricular arrhythmias, may have impacted the study&#8217;s statistical power and ability to detect significant differences between ER groups. Additionally, the study&#8217;s single-center nature and exclusion criteria may limit the generalizability of findings to broader populations. Data quality and availability, and potential confounding factors not fully controlled, further add complexity to the interpretation of results. The observational nature of the study precludes the establishment of causal relationships between ER and outcomes. Longer follow-up periods and prospective, multicenter studies are needed to validate findings and address these limitations comprehensively.</p>

        <title>Conclusion</title>
        <p>In this study, we observed that patients with early ventricular repolarization, particularly in the lateral and inferior leads, did not exhibit a significantly higher incidence of ventricular arrhythmias or mortality. These findings suggest that the presence of ER alone may not be a strong predictor of adverse outcomes in our study population. However, further prospective and randomized studies are warranted to validate these results and inform clinical practice regarding the management of patients with early ventricular repolarization.</p>
