  <body>
    <p>Desde 1983, dados de mais de 220 000 receptores de transplante de cora&#231;&#227;o, pulm&#227;o, e cora&#231;&#227;o e pulm&#227;o de 481 centros de transplante card&#237;aco e 260 centros de transplante pulmonar foram submetidos ao registro do ISHLT (International Society for Heart and Lung Transplantation). A miss&#227;o do registro do ISHLT &#233; melhorar o cuidado do paciente com doen&#231;a card&#237;aca e pulmonar avan&#231;ada. O registro tem atra&#237;do membros de diversas regi&#245;es, fornecido dados de transplantes tor&#225;cicos de todo o mundo e catalisado colabora&#231;&#245;es cl&#237;nicas e cient&#237;ficas. Dez hospitais brasileiros participavam do registro ISHL no passado e, durante esse per&#237;odo, foram observados alguns problemas. Primeiramente, havia um comprometimento heterog&#234;neo dos centros participantes para completar o conjunto de dados. Segundo, havia uma escassez de recursos humanos em alguns hospitais dedicados ao registro. Terceiro, a coleta de dados era responsabilidade de cada centro, e a submiss&#227;o dos dados (mediante pagamento) ao registro ISHLT n&#227;o era obrigat&#243;ria.<xref ref-type="bibr" rid="B1">1</xref></p>
    <p>Em 2021, o registro ISHLT foi interrompido devido aos novos regulamentos de compartilhamento de dados e novas abordagens de coleta de dados. Os campos e o processo de carregamento dos dados foram atualizados. Em 2024, o registro ISHLT come&#231;ou a trocar dados somente com registros nacionais e regionais, tais como o United Network for Organ Sharing e o Eurotransplant, e n&#227;o mais com hospitais ou institui&#231;&#245;es individuais.</p>
    <p>Para o Brasil continuar sua participa&#231;&#227;o no novo formato do registro ISHLT, era compreens&#237;vel que a Associa&#231;&#227;o Brasileira de Transplante de &#211;rg&#227;os (ABTO) fosse escolhida como a sociedade nacional representante por j&#225; possuir seu pr&#243;prio registro, o Registro Brasileiro de Transplantes (RBT). O RBT foi iniciado em 1996, e se trata do relat&#243;rio oficial de todos os transplantes realizados no Brasil, incluindo transplantes de rim, cora&#231;&#227;o, p&#226;ncreas, pulm&#227;o, medula &#243;ssea, c&#243;rnea e osso. O registro inclui tamb&#233;m cap&#237;tulos de transplante pedi&#225;trico, informa&#231;&#245;es sobre lista de espera e doa&#231;&#227;o de &#243;rg&#227;os. Seus relat&#243;rios s&#227;o publicados trimestralmente com dados parciais e anualmente com dados completos. Tais publica&#231;&#245;es t&#234;m um papel crucial em decis&#245;es estrat&#233;gicas sobre transplante e &#243;rg&#227;os pelo governo brasileiro. Atualmente, todas as equipes de transplante card&#237;aco participam voluntariamente do RBT que inclui 11 vari&#225;veis: sexo, idade e tipo doador, identidade do receptor, sexo do receptor, idade do receptor, data da cirurgia de transplante, data da perda do enxerto (se aplic&#225;vel), causa da perda de enxerto (se aplic&#225;vel), data do &#250;ltimo acompanhamento, data e causa de morte (se aplic&#225;vel). O RBT fornece um selo de qualidade aos centros que tiverem obtido uma taxa de submiss&#227;o de 100%. O comprometimento ao banco de dados por parte dos centros de transplante card&#237;aco &#233; heterog&#234;neo. No ano passado, somente 43% dos centros de transplante card&#237;aco receberam um selo de qualidade, o que representa 50% de dados faltantes de pacientes considerando o total de transplantes card&#237;acos realizados. Em 2023, foram realizados 424 transplantes no Brasil, em 13 estados por 48 equipes de transplante.<xref ref-type="bibr" rid="B2">2</xref> Isso foi um grande feito considerando o per&#237;odo inicial de transplante de &#243;rg&#227;os s&#243;lidos. A <xref ref-type="fig" rid="f1">Figura 1</xref> mostra o aumento progressivo de transplante card&#237;aco no Brasil; 2023 foi um marco no n&#250;mero de transplantes card&#237;acos (<xref ref-type="fig" rid="f2">Figura 2</xref>), bem como no n&#250;mero de doa&#231;&#245;es efetivas de &#243;rg&#227;os realizadas no Brasil.</p>

    <p>N&#227;o h&#225; d&#250;vidas de que precisamos melhorar o banco de dados de transplante card&#237;aco no RBT. Trata-se de uma velha demanda de nossa comunidade de transplante, a Sociedade Brasileira de Cardiologia<xref ref-type="bibr" rid="B3">3</xref> e a Sociedade Brasileira de Cirurgia Cardiovascular. Isso nos dar&#225; uma informa&#231;&#227;o mais clara e significativa sobre as atividades e desfechos de transplante card&#237;aco em nosso pa&#237;s, para promover iniciativas de melhoria de qualidade e para discutir novos sistemas de reembolso aos usu&#225;rios do sistema de sa&#250;de p&#250;blico e privado que contribuir&#227;o para a sustentabilidade financeira de nossos programas. A participa&#231;&#227;o de centros brasileiros de transplante tor&#225;cico no registro ISHLT pode ser crucial para o avan&#231;o em nosso registro. As lideran&#231;as da ABTO e da ISHLT j&#225; assinaram um acordo de compartilhamento de dados do registro. Isso significa que todos os centros de transplante continuar&#227;o a preencher o RBT, que est&#225; em portugu&#234;s, com seus pr&#243;prios dados de maneira online. Uma vez ao ano, a ABTO ir&#225; transferir todos os dados em ingl&#234;s para o registro ISHLT de maneira segura e an&#244;nima, sem identifica&#231;&#227;o de pacientes e/ou hospitais. Isso nos dar&#225; a oportunidade de participar ativamente em um banco de dados global muito importante, no qual teremos padr&#245;es e refer&#234;ncias internacionais de desfecho para comparar. Finalmente, isso abre a oportunidade para pesquisa e educa&#231;&#227;o com foco em nossas necessidades nacionais.</p>
    <p>Nesse sentido, o RBT foi modificado em uma nova plataforma, vers&#227;o 2.0. Isso foi particularmente importante para alcan&#231;ar padr&#245;es legais de prote&#231;&#227;o, confidencialidade, privacidade dos dados, e integridade da informa&#231;&#227;o. Durante esse per&#237;odo de moderniza&#231;&#227;o de nosso registro, novas vari&#225;veis foram inclu&#237;das com as mesmas codifica&#231;&#245;es do registro ISHLT, o que facilitar&#225; o processo de exporta&#231;&#227;o de dados. Uma vez que o conjunto completo de dados do registro ISHLT possui mais de 100 vari&#225;veis, seria pr&#225;tico adicionar neste momento somente nove vari&#225;veis al&#233;m daquelas inclu&#237;das na vers&#227;o original da ABTO e progressivamente incorporar novas vari&#225;veis.</p>
    <p>Para o sucesso da parceria / acordo entre a ABTO e a ISHLT, &#233; fundamental que todos os centros brasileiros de transplante card&#237;aco participem ativamente no registro. Uma conscientiza&#231;&#227;o universal sobre a import&#226;ncia do registro &#233; extremamente relevante para nossos pacientes para todos n&#243;s</p>

      <p>Since 1983, data of more than 220,000 heart, lung and heart-lung transplant recipients from 481 heart transplant centers and 260 lung transplant centers in 47 countries have been submitted to the International Society for Heart and Lung Transplantation (ISHLT) Registry.<xref ref-type="bibr" rid="B1">1</xref> The ISHLT Registry mission is to improve the care of patients with advanced heart and lung disease. The ISHLT Registry has engaged members from diverse regions, provided outcome data from thoracic transplants worldwide and catalyzed scientific and clinical collaboration. Ten Brazilian hospitals used to participate in the ISHLT Registry in the past. During this period, some problems were observed. First, there was heterogenous commitment among participating centers to the complete dataset. Second, there was lack of human resources in some hospitals dedicated to the Registry. Third, data collection was each center's responsibility and submission of the data to the ISHLT registry was not mandatory.<xref ref-type="bibr" rid="B1">1</xref></p>
      <p>In 2021, the ISHLT Registry was interrupted due to new data sharing regulations and data collection approaches. Data fields and data upload process were updated. In 2024, the ISHLT Registry began to exchange data only with national and regional registries, such as United Network for Organ Sharing and Eurotransplant, and no longer with individual hospitals or institutions.</p>
      <p>In order for Brazil to continue its participation in the new ISHLT Registry format, it was understandable that the Brazilian Association for Organ Transplantation (ABTO) was chosen as the national representative society since it had already established its own registry, The Brazilian Transplant Registry. The Brazilian Transplant Registry started in 1996, and it is the official report of all transplants performed in Brazil, including kidney, liver, heart, pancreas, lung, bone marrow, cornea and bone. It also includes chapters on pediatric transplantation, information about waiting list and organ donation. Its reports are published quarterly and yearly, with partial and complete data, respectively. These publications have a crucial role in strategic decisions on organ transplantation by the Brazilian government. Currently, all heart transplant teams participate voluntarily in the Brazilian Transplant Registry which includes 11 variables: donor sex, donor age, type of donor, recipient identification, recipient sex, recipient age, date of transplant surgery, date of graft loss (if applicable), cause of graft loss (if applicable), date of last follow-up, date and cause of death (if applicable). The Brazilian Transplant Registry provides a quality label for those centers that have had 100% data submission rate. Commitment to the database is heterogeneous among heart transplant centers. Last year, only 43% of heart transplant centers received a quality label, that represents 50% patient missing data regarding the total number of heart transplants performed. In 2023, 424 heart transplants were performed in Brazil, in 13 states by 48 transplant teams.<xref ref-type="bibr" rid="B2">2</xref> That has been a huge achievement since the early period of solid organ transplantation. <xref ref-type="fig" rid="f3">Figure 1</xref> shows the progressive increase of heart transplantation in Brazil; 2023 was a milestone in the number of heart transplants (<xref ref-type="fig" rid="f4">Figure 2</xref>), as well as in the number of effective organ donations performed in Brazil.</p>

      <p>There is no question that we need to improve the heart transplant database within the Brazilian Transplant Registry. This is an old demand of our transplant community, the Brazilian Society of Cardiology<xref ref-type="bibr" rid="B3">3</xref> and the Brazilian Society of Cardiovascular Surgery. That will give us a clearer and meaningful information about heart transplant activities and outcomes in our country, to promote quality improvement initiatives and to discuss new reimbursement systems to public and private health users that will contribute to the financial sustainability of our programs. The participation of Brazilian thoracic transplant centers in the ISHLT Registry may be key to advance our own registry. Both the ABTO and the ISHLT leadership have already signed a Registry Data Sharing Agreement. That means that all transplant centers will continue to fill their own data in the web-based Brazilian Transplant Registry that is in Portuguese. Once a year, ABTO will transfer all data in English to the ISHLT Registry securely, anonymously, with no identification of patients and/or hospitals. That will provide us the opportunity to actively participate in a very relevant global database, in which we will have international outcome standards/benchmarks to compare. Finally, it opens the opportunity for education and research, focused on our national needs.</p>
      <p>In this regard, the Brazilian Transplant Registry was modified into a new platform version 2.0. That was particularly important to reach legal standards of data protection, confidentiality, privacy and information's integrity. During this period of modernization of our Registry, new variables were included with the same codifications of the ISHLT Registry, which will facilitate the data exporting process. Since the ISHLT Registry complete dataset has more than 100 variables, it would be practical to add at this point only nine variables in addition to those included in the ABTO original version and progressively incorporate new ones.</p>
      <p>In order for the ABTO and ISHLT partnership/agreement to be successful, it is fundamental that all Brazilian heart transplant centers actively participate in the Registry. Universal awareness of the importance of the Registry is extremely relevant for our patients and for all of us!</p>
