  <body>
    <p>A fibrila&#231;&#227;o atrial (FA) &#233; a arritmia card&#237;aca mais comum na pr&#225;tica cl&#237;nica, com incid&#234;ncia crescente nos indiv&#237;duos mais idosos. &#201; bem conhecido que a FA &#233; um fator de risco substancial para eventos cardiovasculares, aumentando a morbidade e mortalidade entre os pacientes afetados, al&#233;m de provocar redu&#231;&#227;o da capacidade f&#237;sica, cognitiva e da qualidade de vida.<xref ref-type="bibr" rid="B1">1</xref>-<xref ref-type="bibr" rid="B3">3</xref> O tratamento atual da FA baseia-se na modifica&#231;&#227;o dos fatores cl&#237;nicos agravantes, na introdu&#231;&#227;o de anticoagulantes baseada na rela&#231;&#227;o risco-benef&#237;cio individualizada, de medicamentos para controle da frequ&#234;ncia card&#237;aca e de antiarr&#237;tmicos para controle do ritmo, al&#233;m da cardiovers&#227;o el&#233;trica naqueles com FA persistente.<xref ref-type="bibr" rid="B1">1</xref></p>
    <p>Nos pacientes refrat&#225;rios ou intolerantes ao tratamento cl&#237;nico, a abla&#231;&#227;o por cateter com isolamento el&#233;trico das veias pulmonares (VPs) vem sendo recomendada com entusiasmo crescente pelas diretrizes internacionais. De fato, os ajustes tecnol&#243;gicos obtidos nos &#250;ltimos anos na abla&#231;&#227;o por radiofrequ&#234;ncia ponto a ponto e na crioabla&#231;&#227;o por bal&#227;o promoveram melhor efetividade do procedimento e maior seguran&#231;a com ambas as tecnologias, demonstradas em numerosos trabalhos cient&#237;ficos comparativos recentes.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B5">5</xref></p>
    <p>A radiofrequ&#234;ncia e a crioenergia s&#227;o classificadas como energias t&#233;rmicas, pois os seus efeitos terap&#234;uticos resultam da libera&#231;&#227;o de calor na abla&#231;&#227;o com radiofrequ&#234;ncia, ou do acentuado resfriamento durante a crioabla&#231;&#227;o. Embora com a&#231;&#245;es antag&#244;nicas, ambas condi&#231;&#245;es provocam necrose de coagula&#231;&#227;o bastante precisa do tecido atrial em contato com o cateter.<xref ref-type="bibr" rid="B6">6</xref> A libera&#231;&#227;o da energia ao redor das VPs tem como objetivo isolar eletricamente os focos deflagradores das VPs do &#225;trio esquerdo e assim evitar a ocorr&#234;ncia da FA. O grande desafio ao longo dos anos foi a obten&#231;&#227;o de uma cicatriz circunferencial, transmural definitiva com resultados cl&#237;nicos satisfat&#243;rios de longo prazo.<xref ref-type="bibr" rid="B7">7</xref>,<xref ref-type="bibr" rid="B8">8</xref></p>
    <p>Uma das principais limita&#231;&#245;es no uso dessas tecnologias &#233; o ajuste preciso da entrega de energia. Esse ajuste deve garantir que a energia liberada provoque a les&#227;o linear atrial necess&#225;ria, periostial, cont&#237;gua e transmural, sem causar estenose das VPs ou atingir tecidos adjacentes de forma indesejada, como les&#245;es no nervo fr&#234;nico, quando a energia &#233; aplicada nas VPs direitas, ou les&#245;es t&#233;rmicas no es&#244;fago, durante aplica&#231;&#245;es na regi&#227;o antral ou no isolamento da parede posterior do &#225;trio esquerdo.<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B10">10</xref></p>
    <p>A abla&#231;&#227;o por campo pulsado (PFA, do ingl&#234;s &#8211; pulsed field ablation) &#233; a mais nova forma de energia usada para isolamento el&#233;trico das VPs que cria les&#245;es no tecido mioc&#225;rdico por meio de eletropora&#231;&#227;o irrevers&#237;vel, com efeito t&#233;rmico m&#237;nimo nos tecidos afetados. Os estudos biof&#237;sicos e cl&#237;nicos para o ajuste dessa energia em eletrofisiologia se iniciaram h&#225; aproximadamente 15 anos, com resultados cl&#237;nicos em pacientes com FA nos &#250;ltimos 5 anos.<xref ref-type="bibr" rid="B11">11</xref></p>
    <p>Essa nova energia para abla&#231;&#227;o caracteriza-se pela libera&#231;&#227;o de pulsos el&#233;tricos de alta energia pelo cateter, com dura&#231;&#227;o de pulso extremamente curta, de poucos microssegundos. Esses pulsos provocam abertura de poros da membrana celular em contato com os eletrodos do cateter, gerando inunda&#231;&#227;o de subst&#226;ncias do l&#237;quido tissular no seu interior e sua inativa&#231;&#227;o. Dependendo das caracter&#237;sticas do pulso el&#233;trico e energia do campo el&#233;trico as altera&#231;&#245;es podem ser transit&#243;rias ou definitivas com morte celular mais intensa em tecidos espec&#237;ficos, caracter&#237;stica interessante, apresentando seletividade na les&#227;o tecidual.<xref ref-type="bibr" rid="B11">11</xref></p>
    <p>Os equipamentos que est&#227;o sendo introduzidos atualmente para uso cl&#237;nicos foram testados e ajustados em avalia&#231;&#245;es experimentais e em estudos pr&#233;-cl&#237;nicos e mostraram-se muito efetivos para provocar eletropora&#231;&#227;o irrevers&#237;vel no tecido atrial, sem afetar significativamente a parede das veias pulmonares, o nervo fr&#234;nico justaposto &#224;s VPs direitas e ao es&#244;fago.<xref ref-type="bibr" rid="B11">11</xref></p>
    <p>V&#225;rios ensaios cl&#237;nicos demonstraram a efic&#225;cia do PFA em pacientes com FA parox&#237;stica refrat&#225;ria a medicamentos, com aus&#234;ncia de arritmias atriais variando de 66% a 87% um ano ap&#243;s o isolamento das VPs, com taxas de incid&#234;ncia variando de 0% a 2,5% para eventos adversos graves.<xref ref-type="bibr" rid="B12">12</xref>-<xref ref-type="bibr" rid="B16">16</xref> Adicionalmente, apresentam menor tempo de procedimento e r&#225;pida curva de aprendizado. Esses aspectos t&#233;cnicos tornam a abla&#231;&#227;o de FA mais acess&#237;vel &#224; popula&#231;&#227;o, envolvendo pacientes mais fr&#225;geis.</p>
    <p>Um registro rec&#233;m-publicado envolvendo 17.642 pacientes de centros europeus confirmou as observa&#231;&#245;es de seguran&#231;a preliminares, sem nenhum registro de casos de les&#245;es esof&#225;gicas, estenose de veias pulmonares ou paralisia fr&#234;nica, gerando grande entusiasmo mundial.<xref ref-type="bibr" rid="B17">17</xref> Outra informa&#231;&#227;o desse registro foi de que o n&#250;mero de complica&#231;&#245;es reduziu entre os 1.700 casos iniciais at&#233; os 17.000. Adicionalmente, um estudo randomizado recente de n&#227;o inferioridade demonstrou equival&#234;ncia no sucesso do procedimento quando comparou as formas convencionais de energias t&#233;rmicas (radiofrequ&#234;ncia e crio) &#224; eletropora&#231;&#227;o irrevers&#237;vel.<xref ref-type="bibr" rid="B18">18</xref></p>
    <p>Apesar de segura, a utiliza&#231;&#227;o dessa tecnologia est&#225; associada a alguns efeitos adversos espec&#237;ficos desse tipo de energia. Um deles &#233; o espasmo coron&#225;rio, principalmente em aplica&#231;&#245;es em istmo cavotricusp&#237;deo e istmo mitral,<xref ref-type="bibr" rid="B19">19</xref> e les&#227;o renal aguda<xref ref-type="bibr" rid="B20">20</xref> nos pacientes com n&#250;mero elevado de aplica&#231;&#245;es quando se busca o isolamento da parede posterior. Entretanto, algumas estrat&#233;gias j&#225; foram descritas para prevenir essas complica&#231;&#245;es com o uso de nitrato e hidrata&#231;&#227;o.</p>
    <p>Os pa&#237;ses europeus v&#234;m usando a nova tecnologia na pr&#225;tica cl&#237;nica desde 2020, os EUA a partir de fevereiro de 2024 ap&#243;s a aprova&#231;&#227;o do Food and Drug Administration (FDA), e o PFA encontra-se tamb&#233;m dispon&#237;vel no Brasil ap&#243;s a aprova&#231;&#227;o da Ag&#234;ncia Nacional de Vigil&#226;ncia Sanit&#225;ria (ANVISA) em abril de 2024.</p>
    <p>O maior desafio das institui&#231;&#245;es no Brasil &#233; tornar essas novas tecnologias acess&#237;veis para a popula&#231;&#227;o assistida pelo Sistema &#218;nico de Sa&#250;de (SUS). At&#233; o momento, n&#227;o existe aprova&#231;&#227;o pelo SUS das t&#233;cnicas citadas e usadas amplamente na rede de sa&#250;de suplementar. Assim, estudos cl&#237;nicos avaliando o custo-efetividade das atuais tecnologias se fazem necess&#225;rios, assim como estudos de viabilidade econ&#244;mica para sua implanta&#231;&#227;o sist&#234;mica.</p>

    <body>
      <p>Atrial fibrillation (AF) is the most common cardiac arrhythmia in clinical practice, with an increasing incidence in more elderly patients. It is well known that AF is a substantial risk factor for cardiovascular events, increasing morbidity and mortality among affected patients, in addition to causing reduced physical and cognitive capacity and quality of life.<xref ref-type="bibr" rid="B1">1</xref>-<xref ref-type="bibr" rid="B3">3</xref> Current treatment of AF is based on modification of aggravating clinical factors, introduction of anticoagulants based on individualized risk-benefit ratio, medications to control heart rate and antiarrhythmics to control rhythm, and electrical cardioversion in patients with persistent AF.<xref ref-type="bibr" rid="B1">1</xref></p>
      <p>In patients who are refractory or intolerant to medical treatment, catheter ablation with electrical pulmonary vein isolation has been recommended with increasing enthusiasm by international guidelines. In fact, technological adjustments achieved in recent years in point-to-point radiofrequency ablation and balloon cryoablation have promoted greater effectiveness of the procedure and increased safety with both technologies, as demonstrated in numerous recent comparative scientific studies.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B5">5</xref></p>
      <p>Radiofrequency and cryoenergy are classified as thermal energies, as their therapeutic effects result from releasing heat during radiofrequency ablation or pronounced cooling during cryoablation. Although they have opposite effects, both conditions cause precise coagulation necrosis of the atrial tissue in contact with the catheter.<xref ref-type="bibr" rid="B6">6</xref> The goal of releasing energy around pulmonary veins is to electrically isolate the triggering foci of the pulmonary veins from the left atrium and thus prevent the occurrence of AF. The main challenge over the years has been to achieve a definitive circumferential, transmural scar with satisfactory long-term clinical results.<xref ref-type="bibr" rid="B7">7</xref>,<xref ref-type="bibr" rid="B8">8</xref></p>
      <p>One of the main limitations to the use of these technologies is the precise adjustment of energy delivery. This adjustment must guarantee that the energy released causes the necessary linear atrial, periosteal, contiguous, and transmural lesion, without causing stenosis of the pulmonary veins or reaching adjacent tissues in an undesirable manner, for example, phrenic nerve lesions, when the energy is applied to the right pulmonary veins, or thermal lesions in the esophagus during applications in the antral region or in isolation of the posterior wall of the left atrium.<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B10">10</xref></p>
      <p>Pulsed field ablation (PFA) is the newest form of energy used for electrical pulmonary vein isolation that creates lesions in the myocardial tissue through irreversible electroporation, with minimal thermal effect on the affected tissues. Biophysical and clinical studies on the adjustment of this energy in electrophysiology began approximately 15 years ago, with clinical results in patients with AF in the past 5 years.<xref ref-type="bibr" rid="B11">11</xref></p>
      <p>This new form of energy for ablation is characterized by the release of high-energy electrical pulses by the catheter, with an extremely short pulse duration lasting only a few microseconds. These pulses cause the opening of pores in the cell membrane in contact with the catheter electrodes, generating a flood of substances from the tissue fluid inside and their inactivation. Depending on the characteristics of the electrical pulse and the energy of the electrical field, the changes may be transient or permanent, with more intense cell death in specific tissues, an interesting characteristic that shows selectivity in tissue injury.<xref ref-type="bibr" rid="B11">11</xref></p>
      <p>The equipment that is currently being introduced for clinical use has been tested and adjusted in experimental evaluations and preclinical studies, and it has shown to be very effective in causing irreversible electroporation in atrial tissue, without significantly affecting the pulmonary vein walls, the phrenic nerve juxtaposed to the right pulmonary veins, or the esophagus.<xref ref-type="bibr" rid="B11">11</xref></p>
      <p>Various clinical trials have demonstrated the efficacy of PFA in patients with drug-refractory paroxysmal AF, with the absence of atrial arrhythmias ranging from 66% to 87% 1 year after pulmonary vein isolation and incidence rates ranging from 0% to 2.5% for severe adverse events.<xref ref-type="bibr" rid="B12">12</xref>-<xref ref-type="bibr" rid="B16">16</xref> Additionally, they demonstrated shorter procedure times and a quick learning curve. These technical aspects make AF ablation more accessible to the population, involving more fragile patients.</p>
      <p>A recently published registry involving 17,642 patients from European centers confirmed the preliminary safety observations, with no reported cases of esophageal lesions, pulmonary vein stenosis, or phrenic paralysis, generating great enthusiasm worldwide.<xref ref-type="bibr" rid="B17">17</xref> Another finding from this registry was that the number of complications decreased from the initial 1,700 cases to the subsequent 17,000. Additionally, a recent randomized non-inferiority study demonstrated equivalence in the success of the procedure when comparing the three forms of thermal energy (radiofrequency and cryoablation) and irreversible electroporation.<xref ref-type="bibr" rid="B18">18</xref></p>
      <p>Despite its safety, the use of this technology has been associated with some specific adverse effects related to this type of energy. One of them is coronary spasm, mainly in applications in the cavotricuspid isthmus and mitral isthmus,<xref ref-type="bibr" rid="B19">19</xref> and acute kidney injury<xref ref-type="bibr" rid="B20">20</xref> in patients with an elevated number of applications aiming to isolate the posterior wall. Nonetheless, some strategies have already been described to prevent these complications with nitrate use and hydration.</p>
      <p>European countries have been using this new technology in clinical practice since 2020. It has been used in the United States since February 2024 after it was approved by the Food and Drug Administration (FDA), and PFA is also available in Brazil after its approval by the Brazilian National Health Surveillance Agency (ANVISA) in April 2024.</p>
      <p>The greatest challenge that institutions in Brazil face is to make this new technology accessible to the population treated through the Unified Health System (SUS). To date, the SUS has not approved the techniques discussed herein, which are widely used in the supplementary health network. Therefore, clinical studies evaluating the cost-effectiveness of current technologies are necessary, as are economic feasibility studies for their systemic implementation.</p>
