    <p>Se o Professor Jorge Pinto Ribeiro estivesse vivo, sem d&#250;vida seria quem que estaria escrevendo esse minieditorial em vez de n&#243;s. Infelizmente, n&#243;s o perdemos em 2013, mas sua mem&#243;ria, e palavras e artigos inspiradores continuam presentes. O Professor Ribeiro era um defensor obstinado do Teste Cardiopulmonar de Exerc&#237;cio (TCPE). Ele escreveu v&#225;rios artigos sobre o tema, incluindo um com t&#237;tulo instigante: "Beyond Peak Oxygen Uptake: New Prognostic Markers from Gas Exchange Exercise Tests in Chronic Heart Failure".<xref ref-type="bibr" rid="B1">1</xref> &#201; uma honra mencionar que o Professor Chiappa e o Professor Stein tiveram a sorte de colaborar com ele nesse artigo.</p>
    <p>O que aconteceu h&#225; 18 anos<xref ref-type="bibr" rid="B1">1</xref> est&#225; diretamente relacionado ao que houve em seguida: apesar da riqueza de dados valiosos dispon&#237;veis, o TCPE continua sendo subutilizado. Tal subutiliza&#231;&#227;o deve-se n&#227;o &#224; falta de capacidade, e sim &#224; pouca explora&#231;&#227;o dos dados que ele gera. Com os avan&#231;os da obten&#231;&#227;o de dados e da anal&#237;tica, estamos no limiar de uma nova era, em que o potencial m&#225;ximo do TCPE, tanto para o progn&#243;stico quanto para o diagn&#243;stico, poder&#225; ser revelado.</p>
    <p>A efici&#234;ncia ventilat&#243;ria exemplifica uma &#225;rea espec&#237;fica em que a total explora&#231;&#227;o dos dados do TCPE poderia revolucionar a maneira como tratamos a doen&#231;a cardiopulmonar e cuidamos do paciente.<xref ref-type="bibr" rid="B2">2</xref> Uma an&#225;lise mais detalhada do TCPE revela que a inclina&#231;&#227;o VE/VCO2 (raz&#227;o entre a ventila&#231;&#227;o minuto e a produ&#231;&#227;o de di&#243;xido de carbono) &#233; uma ferramenta valiosa para navegar entre as complexidades de v&#225;rias condi&#231;&#245;es cardiopulmonares. Essa m&#233;trica, ao quantificar a efici&#234;ncia ventilat&#243;ria, serve como um importante indicador cl&#237;nico para doen&#231;as como insufici&#234;ncia card&#237;aca cr&#244;nica (ICC), doen&#231;a pulmonar obstrutiva cr&#244;nica (DPOC), hipertens&#227;o arterial pulmonar, e doen&#231;a pulmonar intersticial. Ainda, considerando a rela&#231;&#227;o entre ventila&#231;&#227;o e demanda metab&#243;lica, a inclina&#231;&#227;o VE/VCO2 auxilia na classifica&#231;&#227;o da gravidade da doen&#231;a e na predi&#231;&#227;o de risco de morbidade e de mortalidade. Assim, essa vari&#225;vel fornece informa&#231;&#245;es importantes tanto sobre a efici&#234;ncia do sistema respirat&#243;rio quanto sobre a intera&#231;&#227;o entre as fun&#231;&#245;es pulmonar e card&#237;aca, levando ao direcionamento de estrat&#233;gias personalizadas de manejo dos pacientes com condi&#231;&#245;es cardiopulmonares.<xref ref-type="bibr" rid="B3">3</xref></p>
    <p>A inclina&#231;&#227;o VE/VCO2 exibe um comportamento vari&#225;vel entre as diferentes condi&#231;&#245;es cl&#237;nicas. Na DPOC, a obstru&#231;&#227;o das vias a&#233;reas e as limita&#231;&#245;es mec&#226;nicas podem evitar a eleva&#231;&#227;o esperada na inclina&#231;&#227;o com a progress&#227;o da doen&#231;a.<xref ref-type="bibr" rid="B4">4</xref> Por outro lado, na ICC, o valor da inclina&#231;&#227;o &#233; geralmente mais alto devido &#224; inefici&#234;ncia metab&#243;lica e ao desequil&#237;brio entre a ventila&#231;&#227;o e a perfus&#227;o. Esses comportamentos contrastantes requerem uma interpreta&#231;&#227;o cuidadosa da inclina&#231;&#227;o VE/VCO2 para assegurar a avalia&#231;&#227;o precisa da fun&#231;&#227;o cardiopulmonar e do estado de sa&#250;de do paciente, j&#225; que refletem mecanismos fisiopatol&#243;gicos distintos de cada condi&#231;&#227;o. &#201; importante destacar que, embora tanto a DPOC como a ICC tenham um impacto sobre a efici&#234;ncia ventilat&#243;ria, seus mecanismos diferem-se significativamente. A DPOC exibe uma diminui&#231;&#227;o paradoxal na inclina&#231;&#227;o VE/VCO2 devido &#224;s limita&#231;&#245;es mec&#226;nicas e dist&#250;rbios de troca gasosa. Por outro lado, na ICC, o valor da inclina&#231;&#227;o &#233; bem elevado, refletindo sua pr&#243;pria fisiopatologia.<xref ref-type="bibr" rid="B5">5</xref></p>
    <p>Orro et al.,<xref ref-type="bibr" rid="B6">6</xref> em um artigo fascinante publicado nesta edi&#231;&#227;o dos Arquivos Brasileiros de Cardiologia, apresentam uma nova abordagem para avaliar a efici&#234;ncia ventilat&#243;ria: o &#951;VE. Os autores destacam v&#225;rias vantagens associadas a esse m&#233;todo:</p>
    <sec>
      <title>Melhor an&#225;lise comparativa:</title>
      <p>o m&#233;todo permite compara&#231;&#245;es mais diretas da efici&#234;ncia ventilat&#243;ria entre pacientes com ICC e pacientes com DPOC, acomodando as diferen&#231;as na fisiopatologia dessas condi&#231;&#245;es.</p>
    </sec>
    <sec>
      <title>&#218;til na doen&#231;a obstrutiva avan&#231;ada:</title>
      <p>&#951;VE &#233; particularmente ben&#233;fico para avaliar pacientes com superposi&#231;&#227;o de DPOC e ICC, em que m&#233;todos tradicionais como a inclina&#231;&#227;o VE/VCO2 podem ser menos eficaz dada &#224; complexa intera&#231;&#227;o entre as doen&#231;as.</p>
    </sec>
    <sec>
      <title>Supera limita&#231;&#245;es de m&#233;todos tradicionais:</title>
      <p>Na DPOC, em que os mecanismos respirat&#243;rios e as doen&#231;as das vias a&#233;reas afetam significativamente a efici&#234;ncia ventilat&#243;ria, a inclina&#231;&#227;o VE/CO2 pode levar a falsas interpreta&#231;&#245;es. O &#951;VE oferece uma avalia&#231;&#227;o mais precisa nesses cen&#225;rios.</p>
      <p>O &#951;VE, conforme apresentado por Orro et al.,<xref ref-type="bibr" rid="B6">6</xref> &#233; uma ferramenta potencialmente mais refinada e precisa para avaliar a efici&#234;ncia ventilat&#243;ria no contexto das doen&#231;as cardiopulmonares. Isso &#233; particularmente relevante quando m&#233;todos tradicionais podem ser inadequados pela complexidade inerente dessas condi&#231;&#245;es. Os dados multifacetados obtidos pelo TCPE, incluindo vari&#225;veis bem estabelecidas, tais como a inclina&#231;&#227;o VE/VCO2 bem como o recentemente apresentado &#951;VE, parecem promissores na identifica&#231;&#227;o de novos marcadores progn&#243;sticos.</p>
      <p>Finalmente, a capacidade do TCPE em avaliar de maneira abrangente os diversos aspectos da sa&#250;de cardiopulmonar destaca seu valor significativo nos cen&#225;rios cl&#237;nicos. Ele transcende m&#233;tricas convencionais como o consumo de oxig&#234;nio de pico &#8211; VO2 pico &#8211; ao fornecer insights mais profundos sobre a capacidade funcional e a progress&#227;o da doen&#231;a dos pacientes.</p>

      <p>If Professor Jorge Pinto Ribeiro were still alive, he would undoubtedly be the one writing this Mini Editorial instead of us. Sadly, we lost him in 2013, but his memory and insightful words and articles live on. Professor Ribeiro was a passionate advocate for cardiopulmonary exercise testing (CPET). He authored numerous articles on the subject, including one with the thought-provoking title: "Beyond Peak Oxygen Uptake: New Prognostic Markers from Gas Exchange Exercise Tests in Chronic Heart Failure".<xref ref-type="bibr" rid="B1">1</xref> It is an honor to mention that Professor Chiappa and Professor Stein were fortunate to collaborate with him on this piece.</p>
      <p>So, what happened 18 years ago<xref ref-type="bibr" rid="B1">1</xref> is directly related to what come next: although possessing a wealth of valuable data, the CPET remains underutilized. This underutilization stems not from a lack of capability, but rather from limited exploration of the data it generates. With advancements in data mining and analytics, we stand on the threshold of a new era where the CPET&#8217;s full potential, for both prognosis and diagnosis, can be unlocked.</p>
      <p>Ventilation efficiency exemplifies a specific area where fully exploring CPET data could revolutionize the way we manage cardiopulmonary disease and deliver patient care.<xref ref-type="bibr" rid="B2">2</xref> Delving deeper into the realm of CPET reveals the VE/VCO2 slope (minute ventilation to carbon dioxide production ratio) as a valuable tool for navigating the complexities of various cardiopulmonary conditions. This metric, quantifying ventilatory efficiency, serves as an important clinical indicator for diseases like chronic heart failure (CHF), chronic obstructive pulmonary disease (COPD), pulmonary arterial hypertension, and interstitial lung disease. In addition, considering the connection between ventilation and metabolic demand, the VE/VCO2 slope aids in classifying disease severity and predicting morbidity and mortality risks. Thus, this variable provides crucial insights into both respiratory system efficiency and the interplay between pulmonary and cardiac function, ultimately guiding personalized management strategies for patients with cardiopulmonary conditions.<xref ref-type="bibr" rid="B3">3</xref></p>
      <p>The VE/VCO2 slope exhibits changeable behavior across different clinical conditions. In COPD, airway obstruction and mechanical limitations can prevent the expected rise in the slope with disease progression.<xref ref-type="bibr" rid="B4">4</xref> Conversely, CHF often presents with a higher slope due to metabolic inefficiencies and ventilation-perfusion mismatches. These contrasting behaviors require a nuanced interpretation of the VE/VCO2 slope to ensure accurate assessment of patient&#8217;s cardiopulmonary function and health status, as they reflect the distinct pathophysiological mechanisms underlying each condition. It is important to point out that, although both COPD and CHF impact ventilatory efficiency, their mechanisms differ significantly. COPD exhibits a paradoxical decrease in the VE/VCO2 slope due to mechanical limitations and gas exchange disruptions. Conversely, CHF presents with a distinctively higher slope, reflecting its unique pathophysiology.<xref ref-type="bibr" rid="B5">5</xref></p>
      <p>Orro et al.<xref ref-type="bibr" rid="B6">6</xref> in a fascinating article published in this issue of Arquivos Brasileiros de Cardiologia, introduce a novel approach for assessing ventilatory efficiency: &#951;VE. The authors highlight several advantages associated with this method:</p>
      <sec>
        <title>Enhanced Comparative Analysis:</title>
        <p>It allows for more direct comparisons of ventilatory efficiency between CHF and COPD patients, accommodating the pathophysiological differences between these conditions.</p>
      </sec>
      <sec>
        <title>Useful in Advanced Obstructive Disease:</title>
        <p>&#951;VE is particularly beneficial for assessing patients with overlapping COPD and CHF, where traditional methods like the VE/VCO2 slope might be less effective due to the complex interplay of diseases.</p>
      </sec>
      <sec>
        <title>Overcoming Traditional Method Limitations:</title>
        <p>In COPD, where respiratory mechanics and airway diseases significantly affect ventilatory efficiency, the VE/CO2 slope can be misleading. &#951;VE offers a more accurate assessment in such scenarios.</p>
        <p>&#951;VE, as presented by Orro et al.,<xref ref-type="bibr" rid="B6">6</xref> offers a potentially more refined and accurate tool for assessing ventilatory efficiency in the context of cardiopulmonary diseases. This is particularly relevant when traditional methods may prove inadequate due to the inherent complexities of these conditions. The multifaceted data obtained via CPET, including well-established variables like the VE/VCO2 slope and the recently introduced &#951;VE, hold promise for uncovering novel prognostic markers.</p>
        <p>Finally, CPET&#8217;s ability to comprehensively evaluate diverse aspects of cardiopulmonary health underscores its significant value in clinical settings. It transcends conventional metrics like peak oxygen consumption - VO2 peak - by providing deeper insights into patients&#8217; functional capacity and disease progression.</p>
