  <body>
    <p>Primeiro o RX, e depois o eletrocardiograma, revolucionaram a abordagem diagn&#243;stica na cardiologia. Aparelhos m&#233;dicos se sofisticam a cada dia, com o uso cada vez maior de ferramentas diagn&#243;sticas, agora dispondo da possibilidade de transmiss&#227;o &#224; dist&#226;ncia dos exames, antecipando diagn&#243;stico, e permitindo a aplica&#231;&#227;o de protocolos de atendimento e de decis&#227;o terap&#234;utica.</p>
    <p>Na cardiologia, com a transmiss&#227;o do tra&#231;ado eletrocardiogr&#225;fico para centros cardiol&#243;gicos em tempo real, o que a princ&#237;pio era feito atrav&#233;s de aparelhos de fac-s&#237;mile, e depois, com dispositivos com transmiss&#227;o pela internet, foi poss&#237;vel a ado&#231;&#227;o de protocolos de tratamento, com impacto na letalidade e no progn&#243;stico funcional. Programas de teleconsultoria no contexto do infarto agudo do mioc&#225;rdio, com transmiss&#227;o de eletrocardiograma feito no primeiro local de atendimento, com este fim, foram desenvolvidos no Rio de Janeiro,<xref ref-type="bibr" rid="B1">1</xref> (Projeto TIET), Salvador,<xref ref-type="bibr" rid="B2">2</xref> S&#227;o Paulo<xref ref-type="bibr" rid="B3">3</xref> e Minas Gerais<xref ref-type="bibr" rid="B4">4</xref> (Minas Telecardio), todos dentro do Sistema &#218;nico de Sa&#250;de (SUS), onde ficou demonstrado a seguran&#231;a e o benef&#237;cio de utilizar a teleconsultoria, abreviando o tempo para realizar o m&#233;todo de revasculariza&#231;&#227;o mais adequado.</p>
    <p>Neste n&#250;mero dos ABC Cardiol, temos os dados de um protocolo piloto dentro do Estudo PROVAR+, realizado dentro da mesma Rede Minas, no qual o objetivo dos autores foi avaliar a acur&#225;cia da Tele-ECG em locais distantes para prever anormalidades no ecocardiograma de triagem, realizados por profissionais de sa&#250;de n&#227;o m&#233;dicos em Centros de Aten&#231;&#227;o Prim&#225;ria com laudo &#224; dist&#226;ncia por especialistas.<xref ref-type="bibr" rid="B5">5</xref> Apesar das limita&#231;&#245;es citadas no pr&#243;prio artigo, anormalidades no Tele-ECG aumentaram a probabilidade de doen&#231;a card&#237;aca importante na ecografia de triagem, gerando boas perspectivas em agilizar o processo de estratifica&#231;&#227;o e regula&#231;&#227;o no cen&#225;rio da aten&#231;&#227;o prim&#225;ria.</p>
    <p>A telemedicina tem grande potencial como fator de transforma&#231;&#227;o da rede de assist&#234;ncia &#224; sa&#250;de, inclusive na aten&#231;&#227;o prim&#225;ria. Este modelo de presta&#231;&#227;o de cuidados de sa&#250;de &#224; dist&#226;ncia oferece uma s&#233;rie de benef&#237;cios e desafios que devem ser cuidadosamente avaliados, um problema n&#227;o s&#243; do Brasil, mas presente em diversos outros pa&#237;ses do mundo.<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B7">7</xref></p>
    <p>Mesmo com projetos em andamento, h&#225; barreiras. O artigo de Damasceno et al.,<xref ref-type="bibr" rid="B8">8</xref> de 2019, com dados obtidos de question&#225;rio com 385 m&#233;dicos que atuavam na Estrat&#233;gia de Sa&#250;de da Fam&#237;lia (ESF) em 73 munic&#237;pios no Norte do Estado de Minas Gerais, quanto &#224; utiliza&#231;&#227;o da Teleconsultoria ofertada pela citada Rede de Teleassist&#234;ncia de Minas Gerais que desenvolve o PROVAR+, mostrou que 55,8% dos m&#233;dicos n&#227;o a utilizavam, sendo identificadas como raz&#245;es, desde a n&#227;o disponibilidade de computadores e acesso &#224; Internet nas Unidades B&#225;sicas de Sa&#250;de at&#233; a falta de informa&#231;&#227;o e de forma&#231;&#227;o em consultoria em telessa&#250;de. Os resultados demonstram o grande desafio de implementa&#231;&#227;o deste tipo de programa em todo o pa&#237;s e todo m&#233;rito de quem a desenvolve.</p>
    <p>H&#225; ainda um grande entrave para o Sistema de Sa&#250;de, que &#233; o processo de regula&#231;&#227;o ap&#243;s o diagn&#243;stico inicial, quando se teria de garantir equidade ao acesso &#224; assist&#234;ncia m&#233;dica de qualidade. Os Sistemas de Regula&#231;&#227;o n&#227;o contemplam as necessidades de sa&#250;de da popula&#231;&#227;o. S&#227;o grandes filas de espera, com impacto direto na preven&#231;&#227;o e sucesso de tratamento.<xref ref-type="bibr" rid="B9">9</xref> Novamente, isso n&#227;o &#233; exclusivo do Brasil e h&#225; preocupa&#231;&#227;o da pr&#243;pria Organiza&#231;&#227;o Mundial da Sa&#250;de com o represamento de pacientes na Aten&#231;&#227;o Prim&#225;ria, impedindo o objetivo da proposta atual da Cobertura Universal da Sa&#250;de.<xref ref-type="bibr" rid="B10">10</xref></p>
    <p>Embora a telemedicina possa vir a representar um acesso mais amplo aos cuidados de sa&#250;de, especialmente para popula&#231;&#245;es em &#225;reas remotas ou com dificuldades para atendimento em centros m&#233;dicos tradicionais, existem desafios a serem superados. Para maximizar os benef&#237;cios da telemedicina na aten&#231;&#227;o prim&#225;ria, &#233; essencial integr&#225;-la de forma hol&#237;stica aos sistemas de sa&#250;de existentes. Isso inclui a forma&#231;&#227;o de profissionais de sa&#250;de para a utiliza&#231;&#227;o eficaz das tecnologias de telemedicina, o desenvolvimento de diretrizes e padr&#245;es de pr&#225;tica cl&#237;nica espec&#237;ficos para consultas remotas, e a garantia de que a infraestrutura tecnol&#243;gica seja robusta o suficiente para suportar a demanda crescente por servi&#231;os de sa&#250;de digitalizados. O crescimento tecnol&#243;gico &#233; extremamente r&#225;pido, com a perspectiva de a Intelig&#234;ncia Artificial ocupar boa parte do processo de estratifica&#231;&#227;o de risco e diagn&#243;stico, com potencial de revolucionar os Sistemas de Sa&#250;de, mas sua implementa&#231;&#227;o plena vai exigir extensa regulamenta&#231;&#227;o legal e &#233;tica.<xref ref-type="bibr" rid="B11">11</xref></p>
    <p>Em resumo, a telemedicina tem grande potencial dentro dos diversos n&#237;veis de aten&#231;&#227;o &#224; sa&#250;de, ao permitir acesso mais amplo e eficiente para o diagn&#243;stico e o tratamento. No entanto, para que isso seja alcan&#231;ado com sucesso, &#233; crucial abordar os desafios e garantir uma implementa&#231;&#227;o cuidadosa e integrada dentro dos sistemas de sa&#250;de existentes, com financiamento adequado e a fundamental integra&#231;&#227;o multiprofissional, que j&#225; ocorre em v&#225;rios pa&#237;ses desenvolvidos com legisla&#231;&#227;o n&#227;o restritiva. Sem isso ser&#225; mais uma boa ideia, perdida em meio a burocracia e ao desperd&#237;cio. Os dados apresentados nesta etapa do PROVAR+, s&#227;o promissores e merecem aten&#231;&#227;o.</p>

      <p>First, the X-ray, and then the electrocardiogram, revolutionized the diagnostic approach in cardiology. Medical devices become more sophisticated every day, with the increasing use of diagnostic tools, now offering the possibility of remote transmission of exams, anticipating diagnosis, and allowing the application of care protocols and therapeutic decisions.</p>
      <p>In cardiology, with the transmission of the electrocardiographic tracing to Cardiological Centers in real-time, which at first was done through facsimile devices, and later, with devices with internet transmission, it was possible to adopt treatment protocols, with impact on lethality and function prognosis. Teleconsulting programs in the context of acute myocardial infarction, with transmission of an electrocardiogram performed at the first place of care, for this purpose, were developed in Rio de Janeiro<xref ref-type="bibr" rid="B1">1</xref> (TIET Project), Salvador,<xref ref-type="bibr" rid="B2">2</xref> S&#227;o Paulo<xref ref-type="bibr" rid="B3">3</xref> and Minas Gerais<xref ref-type="bibr" rid="B4">4</xref> (Minas Telecardio), all within the public health system (Sistema &#218;nico de Sa&#250;de - SUS), where the safety and benefit of using Teleconsultation was demonstrated, shortening the time to perform the most appropriate revascularization method.</p>
      <p>In this issue of ABC Cardiol, we have data from a pilot protocol within the PROVAR+ Study, carried out within the same Rede Minas, in which the authors&#8217; objective was to evaluate the accuracy of Tele-ECG in distant locations to predict abnormalities in the screening echocardiogram, carried out by non-medical health professionals in Primary Care Centers with remote reports by specialists.<xref ref-type="bibr" rid="B5">5</xref> Despite the limitations mentioned in the article itself, abnormalities in the Tele-ECG increased the probability of significant Heart Disease on screening ultrasound, generating good prospects for streamlining the stratification and regulation process in the primary care setting.</p>
      <p>Telemedicine has great potential as a factor in transforming the health care network, including primary care. This model of providing healthcare remotely offers a series of benefits and challenges that must be carefully evaluated, a problem not only in Brazil but present in several other countries around the world.<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B7">7</xref></p>
      <p>Even with projects underway, there are barriers. The article by Damasceno et al.<xref ref-type="bibr" rid="B8">8</xref> from 2019, with data obtained from a questionnaire with 385 doctors who worked in the Family Health Strategy (ESF) in 73 municipalities in the North of the State of Minas Gerais, regarding the use of Teleconsulting offered by the aforementioned Health Network Teleassistance in Minas Gerais, which develops PROVAR+, showed that 55.8% of doctors did not use it, with reasons being identified as ranging from the non-availability of computers and Internet access in Basic Health Units to the lack of information and training in telehealth consultancy. The results demonstrate the great challenge of implementing this type of program across the country and all the merit of those who develop it.</p>
      <p>There is still a major obstacle for the Health System, which is the regulation process after the initial diagnosis when it would be necessary to guarantee equal access to quality medical care. Regulatory Systems do not address the health needs of the population. There are long waiting lists, with a direct impact on prevention and treatment success.<xref ref-type="bibr" rid="B9">9</xref> Again, this is not exclusive to Brazil and there is concern from the World Health Organization itself about the backlog of patients in Primary Care, impeding the objective of the current proposal for Universal Health Coverage.<xref ref-type="bibr" rid="B10">10</xref></p>
      <p>Although telemedicine may represent broader access to healthcare, especially for populations in remote areas or those with difficulty accessing traditional medical centers, there are challenges to be overcome. To maximize the benefits of telemedicine in primary care, it is essential to integrate it holistically into existing healthcare systems. This includes training healthcare professionals to effectively utilize telemedicine technologies, developing specific clinical practice guidelines and standards for remote consultations, and ensuring that technological infrastructure is robust enough to support the growing demand for services. digitized healthcare. Technological growth is extremely rapid, with the prospect of Artificial Intelligence occupying a large part of the risk stratification and diagnosis process, with the potential to revolutionize Health Systems, but its full implementation will require extensive legal and ethical regulation.<xref ref-type="bibr" rid="B11">11</xref></p>
      <p>In summary, telemedicine has great potential within the different levels of health care, by allowing broader and more efficient access to diagnosis and treatment. However, for this to be achieved successfully, it is crucial to address the challenges and ensure careful and integrated implementation within existing health systems, with adequate financing and the fundamental multidisciplinary integration, which already occurs in several developed countries with non-restrictive legislation. Without this, it will be another good idea, lost amid bureaucracy and waste. The data presented in this stage of PROVAR+ are promising and deserve attention.</p>
