        <title>Resumo</title>
        <sec>
          <title>Fundamento</title>
          <p>Pacientes com idade superior a 50 anos requerem quatro vezes mais interven&#231;&#245;es cir&#250;rgicas que o grupo mais jovem. Muitas diretrizes recomendam a realiza&#231;&#227;o do eletrocardiograma pr&#233;-operat&#243;rio nessa faixa et&#225;ria.</p>
        </sec>
        <sec>
          <title>Objetivos</title>
          <p>Determinar a import&#226;ncia do ECG pr&#233;-operat&#243;rio em pacientes com idade superior a 50 anos e com classifica&#231;&#227;o de risco cir&#250;rgico ASA I e II.</p>
        </sec>
        <sec>
          <title>M&#233;todos</title>
          <p>Foram recrutados pacientes com idade superior a 50 anos, sem comorbidades, submetidos &#224; interven&#231;&#227;o cir&#250;rgica sob anestesia geral. Os pacientes foram randomizados para a realiza&#231;&#227;o (grupo A n=214) ou n&#227;o (grupo B n=213) do ECG pr&#233;-operat&#243;rio. Foram analisadas as vari&#225;veis: sexo, idade, resultado do ECG, da radiografia do t&#243;rax e dos exames laboratoriais, risco cir&#250;rgico, dura&#231;&#227;o do procedimento, eventos adversos e mortalidade intra-hospitalar. O n&#237;vel de signific&#226;ncia estat&#237;stica adotado foi de 5%.</p>
        </sec>
        <sec>
          <title>Resultados</title>
          <p>Houve ocorr&#234;ncia de desfechos adversos em 23 (5,4%) pacientes, com um n&#250;mero significante de eventos adversos nos pacientes do sexo masculino (OR=7,91, IC95% 3,3-18,90, p&lt;0,001) e naqueles com interven&#231;&#245;es de maior porte cir&#250;rgico (OR=30,02, IC95% 4,01-224,92, p&lt;0,001). N&#227;o houve diferen&#231;a entre os grupos que realizaram ou n&#227;o o ECG (OR=1,59, IC95% 0,67-3,75, p=0,289). As demais vari&#225;veis n&#227;o mostraram diferen&#231;as significantes. Na regress&#227;o log&#237;stica multivariada o sexo masculino (OR=6,49; IC95% 2,42-17,42, p&lt;0,001) e o porte cir&#250;rgico (OR=22,62; IC95% 2,95-173,41, p=0,002) foram preditores independentes de desfechos adversos, enquanto realizar ou n&#227;o ECG (OR=1,09; IC95% 0,41-2,90, p=0,867) permaneceu sem signific&#226;ncia estat&#237;stica.</p>
        </sec>
        <sec>
          <title>Conclus&#245;es</title>
          <p>Os resultados sugerem que o ECG pr&#233;-operat&#243;rio n&#227;o foi capaz de predizer aumento do risco de desfechos adversos nos pacientes estudados, durante a fase hospitalar.</p>

      <title>Introdu&#231;&#227;o</title>
      <p>O n&#250;mero de procedimentos cir&#250;rgicos n&#227;o card&#237;acos est&#225; em franca ascens&#227;o, ultrapassando a barreira das 300 milh&#245;es de interven&#231;&#245;es ao ano. <xref rid="B1" ref-type="bibr">1</xref> , <xref rid="B2" ref-type="bibr">2</xref>  Nos pa&#237;ses desenvolvidos, a mortalidade hospitalar varia de 0,4 a 0,8% e a taxa de complica&#231;&#245;es entre 3% e 16%, das quais 40% est&#227;o relacionadas ao sistema cardiovascular. <xref rid="B3" ref-type="bibr">3</xref> , <xref rid="B4" ref-type="bibr">4</xref>  Al&#233;m disso, a popula&#231;&#227;o acima dos 50 anos de idade requer quatro vezes mais interven&#231;&#245;es cir&#250;rgicas que o grupo mais jovem. Ainda, com o envelhecimento crescente da popula&#231;&#227;o, gra&#231;as principalmente a uma melhor aten&#231;&#227;o dada &#224;s enfermidades cr&#244;nicas, &#233; estimado que exista um aumento exponencial dos procedimentos nesse grupo nos pr&#243;ximos anos. <xref rid="B5" ref-type="bibr">5</xref> </p>
      <p>Atualmente recomenda-se a realiza&#231;&#227;o de um eletrocardiograma (ECG) pr&#233;-operat&#243;rio em indiv&#237;duos com idade superior a 40 anos, visto que estudos na popula&#231;&#227;o cir&#250;rgica mostram que altera&#231;&#245;es eletrocardiogr&#225;ficas aumentam com a idade e muitas delas podem ter implica&#231;&#245;es cl&#237;nicas para o manuseio anest&#233;sico. <xref rid="B6" ref-type="bibr">6</xref> , <xref rid="B7" ref-type="bibr">7</xref>  </p>
      <p>Devido a isso, na maioria das institui&#231;&#245;es, faz-se rotineiramente avalia&#231;&#227;o cl&#237;nica e laboratorial antes de uma cirurgia para determinar a condi&#231;&#227;o pr&#233;-operat&#243;ria do paciente com o objetivo de reduzir a morbimortalidade. E no &#226;mbito da avalia&#231;&#227;o cl&#237;nica, um dos exames realizados &#233; o ECG. Esse &#233; particularmente solicitado em pacientes acima dos 40 anos de idade independente da condi&#231;&#227;o cl&#237;nica, com grau de recomenda&#231;&#227;o vari&#225;vel dependendo da diretriz adotada, mas sempre com um n&#237;vel de evid&#234;ncia fraco, exatamente pela escassez de estudos com n&#250;mero de pacientes, desenho e qualidade suficientes para uma recomenda&#231;&#227;o mais robusta. <xref rid="B8" ref-type="bibr">8</xref> - <xref rid="B12" ref-type="bibr">12</xref> </p>
      <p>O presente estudo visa avaliar pela primeira vez, em um desenho prospectivo e randomizado, a necessidade da realiza&#231;&#227;o de um ECG na rotina pr&#233;-operat&#243;ria para pacientes com idade superior a 50 anos, sem comorbidades, e que foram submetidos &#224; interven&#231;&#227;o cir&#250;rgica n&#227;o card&#237;aca de forma eletiva sob anestesia geral.</p>

      <title>M&#233;todos</title>
      <p>Entre abril 2020 e fevereiro 2022 foram avaliados 427 pacientes com idade superior a 50 anos sem comorbidades, que foram submetidos &#224; interven&#231;&#227;o cir&#250;rgica sob anestesia geral. Todos possu&#237;am exame f&#237;sico normal no momento da avalia&#231;&#227;o cl&#237;nica pr&#233;-operat&#243;ria. Os pacientes que preencheram os crit&#233;rios de inclus&#227;o e aceitaram participar do estudo ap&#243;s assinar o termo de consentimento livre e esclarecido foram randomizados na propor&#231;&#227;o 1:1 para a realiza&#231;&#227;o ou n&#227;o do ECG.</p>
      <p>Todos os pacientes realizaram exames laboratoriais (hemograma, glicemia, ureia, creatinina e coagulograma) e radiografia de t&#243;rax na posi&#231;&#227;o posteroanterior, e foram submetidos a procedimento cir&#250;rgico sob anestesia geral para tratamento de doen&#231;a neopl&#225;sica.</p>
      <p>Foram coletadas de cada paciente as seguintes vari&#225;veis: sexo, idade, resultado do ECG (para aqueles que realizaram), da radiografia do t&#243;rax e dos exames laboratoriais (hemograma, glicemia, ureia, creatinina e coagulograma). Informa&#231;&#245;es sobre o risco cir&#250;rgico pr&#233;-operat&#243;rio (American Society of Anesthesiology - ASA), dura&#231;&#227;o do procedimento, eventos adversos e mortalidade intra-hospitalar tamb&#233;m foram contabilizados. A morbimortalidade intra-hospitalar dos pacientes que fizeram o ECG (grupo A) foi comparada com aqueles que foram submetidos &#224; interven&#231;&#227;o cir&#250;rgica sem o exame (grupo B). Os pacientes do grupo A ainda foram divididos em dois subgrupos: A1 (com ECG normal) e A2 (com ECG apresentando alguma anormalidade) que foram comparados entre si e com os pacientes do grupo B. O ECG foi interpretado pelo cardiologista s&#234;nior da institui&#231;&#227;o participante e foi considerado anormal qualquer tra&#231;ado que apresentasse um ritmo diferente do sinusal, presen&#231;a de sobrecargas de c&#226;maras card&#237;acas, bloqueios intraventriculares ou atrioventriculares, invers&#227;o da polaridade da onda T em pelo menos duas deriva&#231;&#245;es cont&#237;guas, mais que tr&#234;s batimentos prematuros atriais ou ventriculares, presen&#231;a de pr&#233;-excita&#231;&#227;o ventricular ou QTc &gt; 470 ms. Radiografia de t&#243;rax foi avaliada por dois radiologistas do departamento de imagem e foi considerada anormal a presen&#231;a de infiltrados parenquimatosos, sequela de tuberculose, presen&#231;a de derrame pleural, &#225;rea card&#237;aca aumentada ou presen&#231;a de met&#225;stases. Foi considerado exame laboratorial alterado qualquer resultado diferente dos limites considerados normais adotados pelo laborat&#243;rio central da Institui&#231;&#227;o. Nenhum paciente foi exclu&#237;do da randomiza&#231;&#227;o devido a alguma anormalidade observada nos exames laboratoriais, na radiografia de t&#243;rax ou no ECG.</p>
      <p>O porte cir&#250;rgico foi dividido em dois grandes grupos: baixo e moderado/alto risco. Para efeito desse estudo foi considerada cirurgia de moderado/alto risco qualquer procedimento intracavit&#225;rio (cr&#226;nio, t&#243;rax, abdome ou pelve) ou aqueles cuja reposi&#231;&#227;o de fluidos durante o ato cir&#250;rgico excederam 30 mL/Kg.</p>
      <p>Desfecho adverso foi considerado qualquer tipo de complica&#231;&#227;o (cl&#237;nica e/ou cir&#250;rgica) que aumentasse o tempo de interna&#231;&#227;o previsto para cada procedimento realizado ou &#243;bito por qualquer causa os quais foram estudados de forma individual e em conjunto sob denomina&#231;&#227;o de morbimortalidade.</p>
      <p>Esse estudo foi aprovado pelo Comit&#234; de &#201;tica e Pesquisa do Instituto Brasileiro de Controle do C&#226;ncer e pelos comit&#234;s das institui&#231;&#245;es coparticipantes sob o n o CAAE 20728719.3.0000.0072</p>

        <p>As caracter&#237;sticas qualitativas foram descritas em frequ&#234;ncias absolutas e relativas segundo a realiza&#231;&#227;o ou n&#227;o do ECG, e verificada a associa&#231;&#227;o pelo teste do qui-quadrado ou testes exatos (teste exato de Fisher ou teste da raz&#227;o de verossimilhan&#231;as). As caracter&#237;sticas quantitativas foram descritas segundo realiza&#231;&#227;o do ECG com uso de medidas resumo (m&#233;dia, desvio padr&#227;o, mediana e quartis) e comparadas pelo teste t-Student n&#227;o pareado ou teste Mann-Whitney, conforme normalidade de distribui&#231;&#227;o avaliada por meio do teste de Kolmogorov-Smirnov.</p>
        <p>A ocorr&#234;ncia de morbimortalidade foi descrita segundo cada caracter&#237;stica qualitativa e quantitativa pelos mesmos testes mencionados anteriormente.</p>
        <p>As vari&#225;veis que foram significativas nas an&#225;lises pelos testes exatos de Fisher/raz&#227;o de verossimilhan&#231;a ou teste t-Student/Mann-Whitney relativamente &#224; morbimortalidade foram inseridas na an&#225;lise multivariada com uso de regress&#227;o log&#237;stica, inserindo o uso de ECG na an&#225;lise para avaliar se a realiza&#231;&#227;o do exame teve influ&#234;ncia na morbimortalidade.</p>
        <p>As an&#225;lises foram realizadas com uso do software SPSS for Windows vers&#227;o 22.0 e os testes foram realizados com n&#237;vel de signific&#226;ncia de 5%.</p>

      <p>Payne et al. <xref rid="B16" ref-type="bibr">16</xref> avaliaram 345 pacientes em uma coorte prospectiva e conclu&#237;ram que o ECG &#233; um exame &#250;til para predizer eventos cardiovasculares perioperat&#243;rios. Outros estudos <xref rid="B17" ref-type="bibr">17</xref> - <xref rid="B19" ref-type="bibr">19</xref> mostraram que anormalidades no ECG pr&#233;-operat&#243;rio foi capaz de predizer complica&#231;&#245;es cardiovasculares principalmente quando existe a presen&#231;a de um QT longo. Entretanto, mais tarde, esses resultados favor&#225;veis foram questionados por v&#225;rios outros autores, <xref rid="B6" ref-type="bibr">6</xref> , <xref rid="B20" ref-type="bibr">20</xref> , <xref rid="B21" ref-type="bibr">21</xref> e essa controv&#233;rsia tem se mantido atualmente. Muitas dessas d&#250;vidas poderiam ter sido sanadas se tiv&#233;ssemos evid&#234;ncias robustas advindas de ensaios prospectivos e randomizados em pacientes submetidos &#224; interven&#231;&#227;o cir&#250;rgica eletiva sob anestesia geral.</p>
      <p>O principal achado do nosso estudo foi mostrar que pacientes sem comorbidades mesmo acima de 50 anos (idade m&#233;dia de 58 anos) submetidos &#224; interven&#231;&#227;o cir&#250;rgica sob anestesia geral parecem n&#227;o se beneficiar da realiza&#231;&#227;o do ECG pr&#233;-operat&#243;rio. N&#227;o encontramos diferen&#231;a no percentual de eventos adversos entre o grupo A, que realizou o ECG quando comparado com o grupo B, sem ECG. Houve at&#233; uma tend&#234;ncia de maior morbimortalidade no grupo A, o que pode ser explicado pelo fato de serem mais velhos, possuir mais anormalidade nos exames laboratoriais e de terem realizado procedimentos com maior tempo cir&#250;rgico.</p>
      <p>Alguns estudos corroboram nossos achados. Richardson et al., <xref rid="B21" ref-type="bibr">21</xref> avaliando uma coorte de 152 479 pacientes de forma retrospectiva concluiu que o ECG pr&#233;-operat&#243;rio n&#227;o possui valor para predizer infarto ou mortalidade cardiovascular p&#243;s-operat&#243;ria. Da mesma forma, Liu et al., <xref rid="B6" ref-type="bibr">6</xref> analisando 513 pacientes observaram que anormalidades eletrocardiogr&#225;ficas n&#227;o era capaz de predizer complica&#231;&#245;es cardiovasculares na popula&#231;&#227;o geri&#225;trica e que esse exame n&#227;o era &#250;til nesses indiv&#237;duos. Na mesma linha, van Klei et al. <xref rid="B20" ref-type="bibr">20</xref> avaliaram o ECG de 2967 pacientes acima de 50 anos em um estudo observacional e notaram que a presen&#231;a de bloqueios intraventriculares apesar de correlacionar com o risco de infarto p&#243;s-operat&#243;rio e &#243;bito, n&#227;o melhorou a predi&#231;&#227;o al&#233;m dos fatores de risco identificados na hist&#243;ria cl&#237;nica do paciente, questionando a necessidade da realiza&#231;&#227;o do ECG pr&#233;-operat&#243;rio.</p>
      <p>Em nosso estudo, o sexo masculino e cirurgia de moderado/alto risco foram preditores independentes para desfechos adversos p&#243;s-operat&#243;rios na fase hospitalar. Uma significante maior preval&#234;ncia de complica&#231;&#245;es em cirurgias de maior porte &#233; facilmente compreens&#237;vel, visto que os eventos adversos s&#227;o devido a fatores inerentes ao paciente, mas tamb&#233;m est&#227;o fortemente relacionados a complexidade dos procedimentos cir&#250;rgicos. A explica&#231;&#227;o do g&#234;nero como fator de risco para eventos adversos &#233; mais dif&#237;cil. Acreditamos que pelo fato de os pacientes do sexo masculino serem mais velhos e haverem se submetido a interven&#231;&#245;es de maior porte pode em parte explicar esses resultados. Ainda, podemos observar que n&#227;o houve diferen&#231;a de g&#234;nero entre o grupo A e o grupo B. Tamb&#233;m n&#227;o foi observada maior preval&#234;ncia de anormalidades eletrocardiogr&#225;ficas no sexo masculino.</p>
      <p>Nesse ensaio, n&#243;s tamb&#233;m avaliamos a import&#226;ncia das altera&#231;&#245;es eletrocardiogr&#225;ficas no desfecho p&#243;s-operat&#243;rio e n&#227;o encontramos nenhuma diferen&#231;a significante. Outros estudos tamb&#233;m t&#234;m demonstrado esses mesmos resultados. <xref rid="B6" ref-type="bibr">6</xref> , <xref rid="B21" ref-type="bibr">21</xref>  </p>
      <p>At&#233; onde n&#243;s conhecemos, este &#233; o primeiro estudo desenhado de forma prospectiva e randomizada a avaliar o papel do ECG pr&#233;-operat&#243;rio sobre os desfechos adversos p&#243;s-operat&#243;rios durante a fase hospitalar em indiv&#237;duos acima de 50 anos submetidos &#224; interven&#231;&#227;o cir&#250;rgica sob anestesia geral. Nossos resultados podem ter implica&#231;&#245;es pr&#225;ticas imediatas, ao ajudar algumas sociedades m&#233;dicas a se reposicionarem sobre as recomenda&#231;&#245;es de diretrizes em solicitar o ECG pr&#233;-operat&#243;rio baseado apenas na idade dos pacientes.</p>
      <p>As limita&#231;&#245;es desse estudo s&#227;o: primeiro, consideramos o n&#250;mero de pacientes ainda pequeno frente o universo de procedimentos cir&#250;rgicos realizados; segundo, esse estudo foi realizado em uma &#250;nica institui&#231;&#227;o o que pode dificultar a extrapola&#231;&#227;o desses resultados para outras institui&#231;&#245;es com estrutura e profissionais distintos; terceiro, estudamos apenas uma popula&#231;&#227;o que possu&#237;a como diagn&#243;stico de base uma doen&#231;a neopl&#225;sica e sem alta preval&#234;ncia de comorbidades, e em quarto, n&#227;o foi feito o seguimento tardio desses pacientes ap&#243;s a alta hospitalar, escopo para outra an&#225;lise.</p>

      <title>Conclus&#227;o</title>
      <p>Os achados desse estudo sugerem que em pacientes com idade superior a 50 anos, sem comorbidades, submetidos &#224; interven&#231;&#227;o cir&#250;rgica sob anestesia geral, o ECG pr&#233;-operat&#243;rio n&#227;o acrescenta benef&#237;cio em predizer complica&#231;&#245;es p&#243;s-operat&#243;rias durante a fase hospitalar, sugerindo uma profunda reflex&#227;o sobre a real necessidade da solicita&#231;&#227;o de forma rotineira desse exame baseado apenas na faixa et&#225;ria dos pacientes.</p>

        <title>Abstract</title>
        <sec>
          <title>Background</title>
          <p>Patients aged over 50 years require four times more surgical interventions than younger groups. Many guidelines recommend the performance of preoperative electrocardiogram (ECG) in this population.</p>
        </sec>
        <sec>
          <title>Objectives</title>
          <p>To determine the value of preoperative ECG in patients aged over 50 years and classified as ASA I&#8211;II (surgical risk).</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p>Patients older than 50 years, without comorbidities, who underwent surgical intervention and general anesthesia were included in the study. Patients were randomized to undergo ECG (group A, n=214) or not (group B, n=213) in the preoperative period. <title>Results</title>
          <p>Adverse outcomes were reported in 23 (5.4%) patients, with a significant number of adverse events in male patients (OR=7.91 95%CI 3.3-18.90, p&lt;0.001) and in those undergoing major surgeries (OR=30.02 95%CI 4.01-224.92, p&lt;0.001). No differences were observed between patients who underwent ECG and those who did not (OR=1.59, 95%CI, 0.67-3.75, p=0.289). No significant differences were found in the other variables. In multivariate logistic regression, male sex (OR = 6.49; 95%CI 2.42-17.42, p&lt;0.001) and major surgery (OR=22.62; 95%CI 2.95-173.41, p=0.002) were independent predictors of adverse outcomes, whereas undergoing (or not) ECG (OR=1.09; IC95% 0.41-2.90, p=0.867) remained without statistical significance.</p>

        <title>Introduction</title>
        <p>The number of non-cardiac surgeries has been increasing, surpassing 300 million interventions per year. <xref rid="B1" ref-type="bibr">1</xref> , <xref rid="B2" ref-type="bibr">2</xref> In developed countries, mortality rates have varied from 0.4 to 0.8% and complication rates from 3% to 16%, of which 40% seem to be related to the cardiovascular system. <xref rid="B3" ref-type="bibr">3</xref> , <xref rid="B4" ref-type="bibr">4</xref> People over the age of 50 require four times more surgical interventions than younger groups and, with the increasing aging of the population, especially due to greater attention paid to chronic diseases, it is estimated that these procedures exponentially increase in the next years. <xref rid="B5" ref-type="bibr">5</xref> </p>
        <p>Today, it is recommended a preoperative electrocardiogram (ECG) for people older than 40 years, since studies on surgical populations have shown that electrocardiographic changes increase with age, and several of them may have clinical implications in the anesthetic management. <xref rid="B6" ref-type="bibr">6</xref> , <xref rid="B7" ref-type="bibr">7</xref> </p>
        <p>In this context, in most institutions, clinical and laboratory evaluation has been routinely performed to determine patients&#8217; preoperative status aiming at reducing morbidity and mortality. One of the tests used for clinical evaluation is ECG. This test is particularly ordered for patients aged older than 40, regardless of their clinical condition, at a variable level of recommendation (depending on the guideline used), but always with a weak level of evidence because of the scarcity of studies with sufficient design, quality, and sample size to allow a stronger recommendation. <xref rid="B8" ref-type="bibr">8</xref> - <xref rid="B12" ref-type="bibr">12</xref> </p>
        <p>The present study aims to assess, for the first time, using a prospective, randomized design, the need for a routine preoperative ECG in patients older than 50 years, without comorbidities, who had undergone non-cardiac surgery under general anesthesia.</p>

        <title>Methods</title>
        <p>Between April 2020 and February 2022, 427 patients aged older than 50 years, without comorbidities, underwent non-cardiac surgery under general anesthesia. All patients had normal physical examination in the preoperative evaluation. Those who met the inclusion criteria and accepted to participate in the study after signing the consent form were randomized in a 1:1 proportion to undergo ECG or not.</p>
        <p>Laboratory tests (complete blood count, glycemia, urea, creatinine and coagulogram) were performed in all patients, and all participants underwent posteroanterior chest X-ray and antineoplastic surgical procedure (under general anesthesia).</p>
        <p>The following variables were analyzed &#8211; sex, age, ECG results (for those who underwent ECG), chest X-ray results, laboratory tests (complete blood count, glycemia, urea, creatinine and coagulogram). Data on preoperative surgical risk ( American Society of Anesthesiology - ASA ), duration of procedure, adverse events and in-hospital mortality were also analyzed. In-hospital mortality and morbidity rates of patients who underwent ECG (group A) were compared with those who underwent surgery without the ECG (group B). Group A patients were divided into two subgroups A1 (with normal ECG) and A2 (with abnormal ECG findings); these groups were compared with each other and with group B. The ECG was interpreted by senior cardiologist of the participating institution and was considered abnormal if there was abnormal (other than sinus) rhythm, presence of cardiac chamber overload, intraventricular or atrioventricular block, reversal of the T wave polarity in at least two contiguous leads, more than three atrial or ventricular premature beats, ventricular preexcitation or QTc&gt;470ms. Chest x-ray was assessed by two radiologists at the radiology department; the presence of parenchyma infiltration, tuberculosis sequelae, pleural effusion, increased cardiac area or metastasis. Laboratory test values that were out of the normal range adopted by the central laboratory of the institution was considered abnormal. No patient was excluded from randomization due to any abnormal finding in the laboratory tests, chest x-ray or ECG.</p>
        <p>Surgical procedures were divided into two groups, of low risk and of moderate/high risk. In this study, a high/moderate-risk surgery was defined as an intracavitary (cranial, chest, abdominal or pelvic) surgery and those procedures in which fluid replacement exceeded 30mL/Kg.</p>
        <p>Adverse outcome was any type of complication (clinical and/or surgical) that prolonged the hospital length of stay predicted for each procedure, or death due to any cause. This was assessed both individually and combined (named as morbidity and mortality).</p>
        <p>This study was approved by the ethics committee of the Instituto Brasileiro de Controle de Cancer (Brazilian Institute for Cancer Control) and by the ethics committees of participating institutions (CAAE 20728719.3.0000.0072)</p>

          <title>Statistical analysis</title>
          <p>Qualitative characteristics were described as absolute and relative frequencies, by group (with or without ECG), and their associations were evaluated by chi-square test or by exact tests (Fisher&#8217;s exact test or the likelihood-ratio test). Quantitative features were described as summary measures (mean, standard deviation, median and interquartile range) and compared by unpaired Student&#8217;s t-test or Mann-Whitney test, according to normality of distribution assessed by Kolmogorov-Smirnov test.</p>
          <p>Morbidity and mortality were described according to each quantitative and qualitative feature using the same tests above mentioned.</p>
          <p>The variables that showed statistical significance by Fisher exact test or the likelihood-ratio test, or by the Student&#8217;s t-test/Mann-Whitney test regarding morbidity and mortality were included in the multivariate analysis by logistic regression. ECG was also included in the analysis to evaluate whether the test affected morbimortality.</p>
          <p>The software SPSS for Windows, version 22.0, was used for statistical analysis, and the significance level was set at 5%.</p>

        <title>Discussion</title>
        <p>Prognostic meaning of preoperative ECG has changed in the last decades. Since the end of the 70s, resting ECG has been widely used as a marker of cardiovascular risk in individuals undergoing elective surgery. Electrocardiographic changes, such as the presence of pathological Q waves and arrythmias are included in the Goldman risk score. <xref rid="B13" ref-type="bibr">13</xref> These results were repeated, confirming the prognostic value of ECG in the preoperative period. <xref rid="B14" ref-type="bibr">14</xref> , <xref rid="B15" ref-type="bibr">15</xref> </p>
        <p>Payne et al. <xref rid="B16" ref-type="bibr">16</xref> evaluated 345 patients of a prospective cohort and concluded that ECG is a useful test to predict perioperative cardiovascular events. Other studies <xref rid="B17" ref-type="bibr">17</xref> - <xref rid="B19" ref-type="bibr">19</xref> showed that abnormal ECG in the preoperative period could predict cardiovascular complications especially in the presence of prolonged QT interval. However, these favorable results were later questioned by other authors, <xref rid="B6" ref-type="bibr">6</xref> , <xref rid="B20" ref-type="bibr">20</xref> , <xref rid="B21" ref-type="bibr">21</xref> and such controversy still remains today. Many of these questions could have been answered if there were robust evidence from prospective, randomized trials with patients undergoing elective surgery and general anesthesia.</p>
        <p>The main finding of our study was that patients without comorbidities, even those aged over 50 years (mean age of 58 years) undergoing surgical intervention and general anesthesia may not benefit from preoperative ECG. We did not observe differences in the percentage of adverse events between group A (with ECG) and group B (without ECG). There was a trend for higher morbidity and mortality in group A, which may be explained by the fact that these patients were older, had more abnormal laboratory findings and having been undergone longer surgeries.</p>
        <p>Some studies corroborate our findings. Richardson et al., <xref rid="B21" ref-type="bibr">21</xref> evaluating retrospectively a cohort of 152,479 patients, concluded that preoperative ECG is not valuable to predict postoperative infarction or cardiovascular mortality. Similarly, Liu et al., <xref rid="B6" ref-type="bibr">6</xref> in a study with 513 patients, observed that electrocardiographic abnormalities could not predict cardiovascular complications in the elderly population and hence ECG was not useful in these patients. In addition, van Klein et al., <xref rid="B20" ref-type="bibr">20</xref> in an observational study, evaluated the use of ECG in 2967 patients older than 50 years and found that although the presence of intraventricular block is correlated with the risk of postoperative infarction and death, it did not improve prediction beyond risk factors identified on patient history, thereby questioning the need for a preoperative ECG.</p>
        <p>In our study, male sex and moderate/high-risk surgery were independent predictors of adverse postoperative outcomes during hospitalization. A significantly higher prevalence of complications in major surgeries is easily understandable, since adverse events not only result from factors inherent to the patient, but also are strongly correlated with the complexity of surgeries. The explanation of the role of male sex as a risk factor for adverse events, however, is more difficult. We believe that the fact that male patients were older and that they had undergone more extensive surgeries may, in part, explain these results. Also, there was no difference in sex distribution between group A and group B. We also did not observe higher prevalence of electrocardiographic abnormalities among men than women.</p>
        <p>In this study, we also evaluated the role of electrocardiographic changes in the postoperative outcome and did not find any statistically significant difference. This is in accordance with results of previous studies. <xref rid="B6" ref-type="bibr">6</xref> , <xref rid="B21" ref-type="bibr">21</xref> </p>
        <p>To our knowledge, this is the first prospective and randomized study to evaluate the role of preoperative ECG in adverse postoperative outcomes in the hospital phase in individuals older than 50 years who underwent surgical intervention under general anesthesia. Our results may have immediate practical implications by helping some medical societies in positioning themselves about guidelines recommendations for indicating preoperative ECG based on patients&#8217; age only.</p>
        <p>Limitations of the study include the relatively small number of patients in face of the multitude of surgical interventions; second, this was a single-center study, which may make extrapolation of results to other institutions with different infrastructure and staff difficult; third, we studied a single population with underlying diagnosis of cancer and without a high prevalence of comorbidities; and finally, a long-term follow-up of these patients after discharge was not performed, which may be the scope of future research.</p>

        <title>Conclusion</title>
        <p>The findings of the present study suggest that in patients older than 50 years, without comorbidities, undergoing surgical intervention and general anesthesia, preoperative ECG does not add value in predicting postoperative complications in the hospital phase, suggesting the necessity for a deep analysis on the real need for ordering this test in a routine basis, considering patients&#8217; age only.</p>
