          <title>Fundamento</title>
          <p>As &#250;ltimas d&#233;cadas t&#234;m assistido ao r&#225;pido desenvolvimento do tratamento invasivo de arritmias por procedimentos de abla&#231;&#227;o por cateter. Apesar da sua seguran&#231;a e efic&#225;cia bem estabelecida em adultos, at&#233; o momento, h&#225; poucos dados nos cen&#225;rios pedi&#225;tricos. Uma das principais preocupa&#231;&#245;es &#233; a poss&#237;vel expans&#227;o da cicatriz do procedimento de abla&#231;&#227;o nessa popula&#231;&#227;o e suas consequ&#234;ncias ao longo dos anos.</p>

          <title>M&#233;todos</title>
          <p>Este &#233; um estudo retrospectivo de 20 pacientes pedi&#225;tricos com tratamento pr&#233;vio de arritmia supraventricular com abla&#231;&#227;o, submetidos &#224; resson&#226;ncia magn&#233;tica card&#237;aca e angiografia coron&#225;ria para avalia&#231;&#227;o de fibrose mioc&#225;rdica e da integridade das art&#233;rias coron&#225;rias durante o acompanhamento.</p>
        </sec>
        <sec>
          <title>Resultados</title>
          <p>A idade mediana no procedimento de abla&#231;&#227;o foi 15,1 anos (Q1 12,9, Q3 16,6) e 21 anos (Q1 20, Q3 23) quando a resson&#226;ncia magn&#233;tica card&#237;aca foi realizada. Quatorze dos pacientes eram mulheres. Taquicardia por reentrada nodal e s&#237;ndrome de Wolf-Parkinson-White foram os principais diagn&#243;sticos (19 pacientes), com um paciente com taquicardia atrial. Tr&#234;s pacientes apresentaram fibrose mioc&#225;rdica ventricular, mas com um volume inferior a 0,6 cm 3 . Nenhum deles desenvolveu disfun&#231;&#227;o ventricular e nenhum paciente apresentou les&#245;es coronarianos na angiografia.</p>

      <title>Introdu&#231;&#227;o</title>
      <p>A efic&#225;cia e a seguran&#231;a do tratamento das arritmias por abla&#231;&#227;o por cateter de radiofrequ&#234;ncia (RF) est&#225; bem estabelecida na pr&#225;tica cl&#237;nica atual. <xref rid="B1" ref-type="bibr">1</xref> , <xref rid="B2" ref-type="bibr">2</xref> Na popula&#231;&#227;o pedi&#225;trica acima de cinco anos de idade e peso maior que 15Kg, a conduta se tornou procedimento de rotina nos &#250;ltimos anos. <xref rid="B3" ref-type="bibr">3</xref> Por&#233;m, existem poucos estudos e falta de evid&#234;ncia sobre o poss&#237;vel impacto da aplica&#231;&#227;o da RF no desenvolvimento do mioc&#225;rdio em crian&#231;as na idade escolar e adolescentes. A maior parte do que se pratica hoje s&#227;o infer&#234;ncias de estudos com animais. <xref rid="B4" ref-type="bibr">4</xref> , <xref rid="B5" ref-type="bibr">5</xref> As principais preocupa&#231;&#245;es incluem o poss&#237;vel envolvimento das art&#233;rias coron&#225;rias e a progress&#227;o da fibrose no mioc&#225;rdio durante a fase adulta. Existem poucos dados na literatura que nos permitem estimar o comportamento dessas les&#245;es durante o crescimento do mioc&#225;rdio, se elas tendem a aumentar ou n&#227;o ao longo dos anos e quais seriam as consequ&#234;ncias no desenvolvimento card&#237;aco da crian&#231;a.</p>
      <p>A maioria dos estudos na literatura demonstrou as consequ&#234;ncias em curto prazo, desde horas at&#233; a alguns meses ap&#243;s a aplica&#231;&#227;o de RF. Nosso estudo teve como objetivo identificar a presen&#231;a de les&#245;es na art&#233;ria coron&#225;ria e a extens&#227;o da fibrose no mioc&#225;rdio ap&#243;s a aplica&#231;&#227;o de RF em pacientes pedi&#225;tricos durante um acompanhamento de longo prazo.</p>
    </sec>
    <sec sec-type="methods">
      <title>M&#233;todos</title>
      <p>Todos os 187 pacientes que foram submetidos &#224; abla&#231;&#227;o por cateter com idade igual ou inferior a 15 anos em um centro terci&#225;rio de cardiologia e que apresentavam 18 anos ou mais no in&#237;cio do estudo (2015) foram contatados e convidados para participar. Ap&#243;s o convite, 20 pacientes aceitaram a participar e um consentimento informado foi obtido dos pais ou representantes legais. Os protocolos de pesquisa foram revisados e aprovados (protocolo n&#250;mero SDC 3776/12/032).</p>
      <p>Os procedimentos de abla&#231;&#227;o por RF foram realizados entre julho de 2002 e novembro de 2012. Por meio de pun&#231;&#227;o da veia femoral direita e/ou esquerda e sob anestesia geral, cateteres de eletrodos multipolares foram introduzidos guiados por fluoroscopia para mapeamento eletrofisiol&#243;gico e aplica&#231;&#227;o de pulsos de RF. A energia da RF foi aplicada usando cateteres de 4mm na maioria dos casos, exceto um paciente com taquicardia de Mahaim, em que um cateter com ponta de 8mm foi necess&#225;rio ap&#243;s falha da abla&#231;&#227;o com um cateter com ponta de 4mm. A temperatura foi estabelecida em 60 o C e pot&#234;ncia de 50W foi usada em todos os casos.</p>
      <p>Para avalia&#231;&#227;o de fibrose mioc&#225;rdica, resson&#226;ncia magn&#233;tica card&#237;aca (RMC) foi realizada, e a angiografia coron&#225;ria por tomografia computadorizada (AngioTC) foi usada para investiga&#231;&#227;o da integridade da art&#233;ria coron&#225;ria. Todos os exames foram conduzidos com o equipamento Philips Achieva 1.5T MRI (Holanda). A fun&#231;&#227;o e os volumes ventriculares foram avaliados por cine-resson&#226;ncia usando a sequ&#234;ncia de pulsos de precess&#227;o livre no estado estacion&#225;rio (SSFP, Steady State Free Precession ), e obtidos pelo m&#233;todo de Simpson com imagens no eixo curto do cora&#231;&#227;o. Fibrose mioc&#225;rdica foi avaliada usando a t&#233;cnica do realce tardio do mioc&#225;rdio, tamb&#233;m conhecida por realce tardio pelo gadol&#237;nio (RTG), em aquisi&#231;&#245;es em 2D e em 3D, avaliando os &#225;trios e os ventr&#237;culos. Todas as an&#225;lises foram realizadas usando o programa CVi42 (Circle CVi, Calgary, Canad&#225;). A AngioTC foi realizada com um tom&#243;grafo 320 x 0.5mm (Aquilion ONE, Canon Medical Systems, Otawara, Jap&#227;o). O protocolo incluiu a aquisi&#231;&#227;o de imagens sem contraste para o escore de c&#225;lcio e a aquisi&#231;&#227;o com AngioTC com inje&#231;&#227;o de contraste iodado (50-70mL) e sincroniza&#231;&#227;o eletrocardiogr&#225;fica prospectiva para avalia&#231;&#227;o do l&#250;men da art&#233;ria coron&#225;ria.</p>

        <p>Foi realizada uma an&#225;lise descritiva. Os dados num&#233;ricos s&#227;o apresentados em m&#233;dia e desvio padr&#227;o ou mediana e intervalo interquartil, de acordo com a normalidade dos dados. Os dados categ&#243;ricos s&#227;o apresentados em frequ&#234;ncia absoluta e porcentagens. A distribui&#231;&#227;o dos dados foi testada quanto &#224; normalidade pelo teste de Shapiro-Wilk. Um valor de p&lt;0,05 foi considerado estatisticamente significativo. O programa IBM-SPSS para Windows, vers&#227;o 25.0, foi usado para as an&#225;lises, e o programa Microsoft Excel 2013 usado para tabular os dados.</p>

      <title>Resultados</title>
      <p>Vinte pacientes foram submetidos &#224; AngioTC e &#224; RMC. A maioria dos pacientes (70%) era do sexo feminino, idade mediana de 15,1 (Q1: 12,9; Q3: 16,67) anos na ocasi&#227;o da abla&#231;&#227;o e 20,8 (Q1: 20 e Q3: 23,5) anos no momento da avalia&#231;&#227;o. O per&#237;odo mediano entre a abla&#231;&#227;o e a avalia&#231;&#227;o foi 6,7 (Q1: 5,37; Q3: 9,12) anos. Peso m&#233;dio dos participantes foi 65,7 &#177; 9Kg, altura 167,6&#177;7,5cm e &#237;ndice de massa corporal 23,4&#177;2,3Kg/m 2 . A fra&#231;&#227;o de eje&#231;&#227;o m&#233;dia foi 61&#177;8% e somente um paciente diagnosticado com taquicardia atrial (TA) apresentava doen&#231;a card&#237;aca estrutural: um defeito septal atrial, corrigido cirurgicamente por atriosseptoplastia com coloca&#231;&#227;o do retalho.</p>
      <p>Dezoito pacientes foram submetidos &#224; abla&#231;&#227;o na regi&#227;o septal direita: 13 com taquicardia por reentrada nodal e quatro com s&#237;ndrome de Wolff-Parkinson-White (WPW). Os locais dessas vias acess&#243;rias (VAs) foram anterosseptal (n=1) e posterosseptal (n=3); um paciente apresentava TA localizada na regi&#227;o posterosseptal ( <xref rid="t1" ref-type="table">Tabela 1</xref> , Figuras <xref ref-type="fig" rid="f02">1</xref> e <xref ref-type="fig" rid="f03">2</xref>).</p>

      <title>Discuss&#227;o</title>
      <p>A abla&#231;&#227;o com cateter tornou-se o tratamento padr&#227;o para arritmias sintom&#225;ticas e fatais, com uma alta taxa de sucesso, inclusive na popula&#231;&#227;o pedi&#225;trica. Embora sua baixa taxa de complica&#231;&#245;es esteja bem estabelecida em pacientes adultos, isso ainda &#233; question&#225;vel em crian&#231;as. <xref rid="B1" ref-type="bibr">1</xref> </p>
      <p>As taxas de mortalidade em crian&#231;a s&#227;o baixas, aproximadamente 2/1000 casos no geral e 1/1000 casos em pacientes com cora&#231;&#227;o com estrutura normal, <xref rid="B2" ref-type="bibr">2</xref> e pode ser ainda mais baixa, considerando t&#233;cnicas e tecnologias recentes. Apesar da baixa mortalidade, h&#225; pouca evid&#234;ncia da evolu&#231;&#227;o ou complica&#231;&#245;es em longo prazo e preocupa&#231;&#245;es acerca do aumento das les&#245;es da abla&#231;&#227;o foram relatadas. <xref rid="B1" ref-type="bibr">1</xref> </p>
      <p>Um estudo <xref rid="B4" ref-type="bibr">4</xref> experimental n&#227;o mostrou altera&#231;&#227;o da dimens&#227;o da les&#227;o ao longo do tempo em c&#227;es. Por outro lado, Saul et al. <xref rid="B5" ref-type="bibr">5</xref> conduziram um experimento com ovelhas jovens e observaram um aumento nas &#225;reas de fibrose relacionada &#224; aplica&#231;&#227;o de RF com o aumento do animal e do mioc&#225;rdio, apesar do per&#237;odo de acompanhamento mais curto (8,5 &#177; 0,5 meses) quando comparado ao nosso. Contudo, n&#227;o existem dados comparando a faixa et&#225;ria entre diferentes esp&#233;cies e poss&#237;veis diferen&#231;as na fisiologia.</p>
      <p>O resfriamento convectivo &#233; um efeito bem conhecido da perfus&#227;o coron&#225;ria que previne complica&#231;&#245;es vasculares. Por&#233;m, se uma art&#233;ria estiver localizada dentro da &#225;rea de abla&#231;&#227;o, sua efici&#234;ncia pode ser comprometida. <xref rid="B6" ref-type="bibr">6</xref> Por outro lado, aplica&#231;&#245;es repetidas de RF para evitar o efeito de resfriamento pode levar ao dano vascular.</p>
      <p>Paul et al. <xref rid="B7" ref-type="bibr">7</xref> demonstraram, em porcos jovens, o poss&#237;vel envolvimento da art&#233;ria coron&#225;ria direita pelo espessamento significativo da camada &#237;ntima, relacionado &#224; aplica&#231;&#227;o de RF na face atrial do anel da v&#225;lvula tric&#250;spide, observado 48 horas ou seis meses depois. <xref rid="B8" ref-type="bibr">8</xref> Em um estudo consecutivo envolvendo 212 pacientes, com idade mediana de 12 (0,3 &#8211; 20,4) anos, submetidos &#224; angiografia coron&#225;ria antes e 30 minutos ap&#243;s a abla&#231;&#227;o supraventricular com cateter, uma redu&#231;&#227;o aguda no di&#226;metro luminal da art&#233;ria coron&#225;ria foi encontrada em dois pacientes. Ambos os pacientes apresentaram uma VA posterosseptal. Em um dos casos, observou-se uma diminui&#231;&#227;o de 40% no di&#226;metro luminal da art&#233;ria coron&#225;ria direita e, no outro, observou-se uma diminui&#231;&#227;o de 35% no di&#226;metro luminal da art&#233;ria circunflexa esquerda. Esses achados podem ser explicados por espasmo ou edema da art&#233;ria coron&#225;ria, o que pode ser autolimitado, embora representem um dano arterial permanente relacionado &#224; trombose, hiperplasia da camada &#237;ntima ou advent&#237;cia e necrose medial.</p>
      <p>O RTG na RMC tornou-se uma ferramenta promissora na eletrofisiologia. A avalia&#231;&#227;o de les&#245;es por RF pode potencialmente demonstrar a efic&#225;cia e a seguran&#231;a desse tratamento, explicada pela coagula&#231;&#227;o grave e necrose de bandas de contra&#231;&#227;o, com total perda da arquitetura celular e vascular nas les&#245;es por RF, <xref rid="B9" ref-type="bibr">9</xref> , <xref rid="B10" ref-type="bibr">10</xref> mesmo se algumas das propriedades eletrofisiol&#243;gicas possam ser revertidas ap&#243;s a fase aguda da aplica&#231;&#227;o da RF. <xref rid="B11" ref-type="bibr">11</xref> </p>
      <p>Na ocasi&#227;o da RMC, a idade mediana dos pacientes era 20,5 anos, e a avalia&#231;&#227;o por imagem foi realizada 6,7 anos (mediana) ap&#243;s a abla&#231;&#227;o. Esse longo per&#237;odo ap&#243;s o procedimento sem RMC ou sinais de eventos adversos sugere que a t&#233;cnica &#233; segura nos pacientes com idade pedi&#225;trica. Al&#233;m disso, a fibrose do mioc&#225;rdio na RMC em pacientes ap&#243;s a abla&#231;&#227;o mostrou somente uma extens&#227;o limitada da les&#227;o. Essa pequena quantidade de fibrose est&#225; provavelmente associada &#224; les&#227;o causada por abla&#231;&#227;o por RF e &#233; necess&#225;ria para o efeito terap&#234;utico; por&#233;m, n&#227;o provocou nenhum efeito negativo no desenvolvimento do mioc&#225;rdio em nossa coorte.</p>
      <p>Em nossos casos, a abla&#231;&#227;o foi realizada com cateteres n&#227;o irrigados, mas, ao contr&#225;rio do que era esperado, os cateteres com pontas irrigadas podem n&#227;o resultar em les&#245;es de volumes maiores em compara&#231;&#227;o a cateteres com pontas n&#227;o irrigada na abla&#231;&#227;o de pacientes pedi&#225;tricos. Isso pode ser explicado pelo fato do uso de que a pot&#234;ncia aplicada &#233; geralmente mais baixa, embora os cateteres irrigados possam resultar em melhores desfechos devido &#224; maior precis&#227;o do mapeamento que eles possibilitam. <xref rid="B10" ref-type="bibr">10</xref> </p>
      <p>Observamos uma baixa taxa de fibrose atrial e ventricular, e aus&#234;ncia de disfun&#231;&#227;o ventricular, sete anos ap&#243;s a abla&#231;&#227;o por RF realizada durante meados da inf&#226;ncia e adolesc&#234;ncia.</p>
      <p>Esses dados sugerem a hip&#243;tese de que a abla&#231;&#227;o por RF n&#227;o aumenta as chances de arritmias ou disfun&#231;&#227;o ventricular esquerda durante o acompanhamento e seguimento dessas crian&#231;as.</p>
      <p>Nesse cen&#225;rio, a RMC &#233; mais adequada na fase adulta, pois na maioria dos casos n&#227;o h&#225; necessidade de seda&#231;&#227;o. Mais estudos s&#227;o necess&#225;rios para melhor avaliar pacientes mais jovens.</p>
      <p>Embora estudos maiores com pacientes mais jovens sejam necess&#225;rios, acreditamos que nossos dados refor&#231;a a seguran&#231;a da abla&#231;&#227;o da taquicardia supraventricular por cateter de RF na popula&#231;&#227;o pedi&#225;trica, especialmente quanto &#224; aus&#234;ncia de progress&#227;o da fibrose mioc&#225;rdica e de les&#245;es na art&#233;ria coron&#225;ria ap&#243;s a abla&#231;&#227;o por RF nessa popula&#231;&#227;o.</p>
      <sec>
        <title>Limita&#231;&#245;es</title>
        <p>A principal limita&#231;&#227;o de nosso estudo &#233; o fato de ele haver sido conduzido com crian&#231;as em idade escolar e adolescentes, em que o impacto da expans&#227;o da fibrose seria menos esperada em compara&#231;&#227;o a abla&#231;&#245;es em neonatos, lactentes e pr&#233;-escolares.</p>
        <p>Os pacientes n&#227;o foram submetidos nem &#224; tomografia nem a uma RMC antes da abla&#231;&#227;o para fins de compara&#231;&#227;o. No entanto, nos tr&#234;s pacientes em que o realce tardio ventricular (fibrose no mioc&#225;rdio) foi observado na RMC, a &#225;rea de fibrose sobrep&#244;s-se aos locais de aplica&#231;&#227;o de RF.</p>
        <p>Infelizmente, tamb&#233;m tivemos um n&#250;mero limitado de pacientes e detalhes sobre o procedimento de abla&#231;&#227;o, tais como o n&#250;mero de aplica&#231;&#245;es e a pot&#234;ncia mediana alcan&#231;ada, n&#227;o puderam ser recuperados. Mais estudos s&#227;o necess&#225;rios para corroborar o fato de que a abla&#231;&#227;o por RF &#233; um procedimento seguro quando realizado em centros especializados.</p>

      <title>Conclus&#245;es</title>
      <p>Nosso estudo demonstrou que o uso de RF no mioc&#225;rdio em desenvolvimento pode de fato levar &#224; fibrose, mas na minoria dos casos e em quantidade n&#227;o significativa. No acompanhamento em longo prazo, n&#243;s n&#227;o detectamos envolvimento coronariano e n&#227;o observamos massa de fibrose significativa ap&#243;s o per&#237;odo de acompanhamento de 7&#177;4 anos.</p>

          <title>Background</title>
          <p>The past decades have seen the rapid development of the invasive treatment of arrhythmias by catheter ablation procedures. Despite its safety and efficacy being well-established in adults, to date there has been little data in pediatric scenarios. One of the main concerns is the possible expansion of the ablation procedure scar in this population and its consequences over the years.</p>
        </sec>
        <sec>
          <title>Objectives</title>
          <p>This study aimed to analyze the risk of myocardial injury progression after radiofrequency catheter ablation in pediatric patients.</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p>This is a retrospective study of 20 pediatric patients with previous ablation for treatment of supraventricular arrhythmia that underwent cardiac magnetic resonance and coronary angiography for evaluation of myocardial fibrosis and the integrity of the coronary arteries during follow-up.</p>
        </sec>
        <sec>
          <title>Results</title>
          <p>The median age at ablation procedure was 15.1 years (Q1 12.9, Q3 16.6) and 21 years (Q1 20, Q3 23) when the cardiac magnetic resonance was performed. Fourteen of them were women. Nodal reentry tachycardia and Wolf-Parkinson-White Syndrome were the main diagnosis (19 patients), with one patient with atrial tachycardia. Three patients had ventricular myocardial fibrosis, but with a volume &lt; 0.6 cm 3 . None of them developed ventricular dysfunction and no patient had coronary lesions on angiography.</p>
        </sec>
        <sec>
          <title>Conclusion</title>
          <p>Radiofrequency catheter ablation did not show to increase the risk of myocardial injury progression or coronary artery lesions.</p>

        <title>Introduction</title>
        <p>The effectiveness and safety of treating arrhythmias using radiofrequency (RF) ablation is well established in the current medical practice. <xref rid="B1" ref-type="bibr">1</xref> , <xref rid="B2" ref-type="bibr">2</xref> In the pediatric population above five years of age and weight greater than 15Kg, it has become a routine procedure over the past few years. <xref rid="B3" ref-type="bibr">3</xref> But there are few studies and lack of evidence of the possible impact of RF application on the development of the myocardium in school-age children and adolescents. Most current assumptions are inferences from animal studies. <xref rid="B4" ref-type="bibr">4</xref> , <xref rid="B5" ref-type="bibr">5</xref> Main concerns include the possible involvement of coronary arteries and progression of myocardial fibrosis during adulthood. There is little data in the literature that allows us to estimate the behavior of these injuries during the growth of the myocardium, whether they tend to increase or not over the years and what would be the consequences on the child&#8217;s cardiac development.</p>
        <p>Most studies in the literature demonstrated the consequences in the short term, from hours to a few months after the RF application. Our study aimed to identify the presence of coronary artery lesions and extent of myocardial fibrosis after application of RF in pediatric patients during a long-term follow-up.</p>

      <sec sec-type="methods">
        <title>Methods</title>
        <p>All the 187 patients that underwent RF catheter ablation in a tertiary cardiology center at the age of 15 years or younger and were 18 years old or older when the study was started (2015) were contacted and invited for participation. After the invitation, 20 patients consented the participation and informed consent was obtained from the parents or legally authorized representative. Research protocols were reviewed and approved by protocol number SDC 3776/12/032.</p>
        <p>The RF catheter ablation procedures were performed between July 2002 and November 2012. Through puncture of the right and / or left femoral vein and under general anesthesia, multipolar electrode catheters were introduced using fluoroscopic guidance for electrophysiological mapping and application of RF pulses. RF energy was delivered using 4mm tip catheters in most cases, except for one patient with Mahaim tachycardia in whom an 8mm tip catheter was necessary, after failure of ablation with a 4mm tip catheter. The temperature was set at 60&#186;C and power at 50W for all cases.</p>
        <p>For myocardial fibrosis evaluation, a cardiac magnetic resonance (CMR) was performed, and coronary angiography (Cor) by computed tomography (CT) (CorCTA) was used for coronary artery integrity investigation. All examinations were conducted in a Philips Achieva 1.5T MRI scanner (Netherlands). Ventricular function and volumes were assessed by cine resonance, using the sequence, and subsequently obtained by the Simpson&#8217;s method with images on the short axis of the heart. Myocardial fibrosis was evaluated using the myocardial delayed enhancement technique, also known as late gadolinium enhancement (LGE), in 2D and 3D acquisitions, assessing the atria and ventricles. All analyzes were performed using the CVi42 software (Circle CVi, Calgary, Canada). CorCTA was performed on a 320 x 0.5mm detector (Aquilion ONE, Canon Medical Systems, Otawara, Japan). The protocol included the acquisition of non-contrasted images for calcium score and the acquisition of CorCTA with iodinated contrast injection (50-70mL) and prospective electrocardiographic synchronization for evaluation of coronary artery lumen.</p>

          <title>Statistical analysis</title>
          <p>A descriptive analysis was performed. The numerical data are presented as means and standard deviations or medians and interquartile ranges, according to normality of the data. Categorized data are presented as absolute frequency and percentages. Data were tested for distribution normality using the Shapiro-Wilk test of normality. A P-value of &lt;0.05 was considered statistically significant. The IBM-SPSS for Windows version 25.0 software was used for the analyzes and the Microsoft Excel 2013 software was used to tabulate the data.</p>

        <title>Results</title>
        <p>We performed CT and CRM in 20 patients, most were female (70%), median age of 15.1 (Q1: 12.9; Q3: 16.67) years old at the time of ablation and 20.8 (Q1: 20 and Q3: 23.5) years-old at the time of evaluation. The median time from ablation to evaluation was 6.7 (Q1: 5.37; Q3: 9.12) years. Mean body weight, height and body mass index were 65.7&#177;9Kg, 167.6&#177;7.5cm and 23.4&#177;2.3 Kg/m 2 , respectively. The ejection fraction was 61&#177;8% and only one patient diagnosed with atrial tachycardia (AT) had structural heart disease: an atrial septal defect, surgically corrected by atrioseptoplasty with patch placement.</p>
        <p>Eighteen patients underwent ablation in the right septal region: 13 with atrioventricular nodal reentry tachycardia and four with Wolff-Parkinson-White Syndrome. The locations of these accessory pathways (AP) were anteroseptal (n=1) e posteroseptal (n=3). One patient had AT located in the posteroseptal region ( <xref rid="t1001" ref-type="table">Table 1</xref> , <xref rid="f02001" ref-type="fig">Figure 1</xref> and <xref ref-type="fig" rid="f03001">2</xref>).</p>

Right atrium mapping did not show potential of the AP and ablation in the tricuspid ring from the right atrium was unsuccessful. RF applications were successfully performed in the ventricular insertion of the AP, at the midseptal portion of the right ventricle.</p>

        <title>Discussion</title>
        <p>Catheter ablation has become the standard treatment for symptomatic and life-threatening arrhythmias, with a high procedural success rate, also in the pediatric population. Its low rate of complications is well established in adult patients, but it is still questionable in pediatrics. <xref rid="B1" ref-type="bibr">1</xref> </p>
        <p>Mortality rates in children are low, approximately 2/1000 cases overall and 1/1000 cases for patients with structurally normal hearts <xref rid="B2" ref-type="bibr">2</xref> and can be even lower, considering recent techniques and technologies. Despite its low mortality, there is a lack of evidence of long-term evolution or complications and concerns about the enlargement of ablation lesions has been postulated. <xref rid="B1" ref-type="bibr">1</xref> </p>
        <p>An experimental study <xref rid="B4" ref-type="bibr">4</xref> showed no lesion dimension change over time in dogs. Differently, Saul et al. <xref rid="B5" ref-type="bibr">5</xref> carried out an experiment with young sheeps and observed an increase in the areas of fibrosis related to RF application along the growth of the animal and the myocardium, although their follow-up period was shorter (8.5 &#177; 0.5 months), when compared to ours. Nevertheless, there are no data comparing age range between different species and possible differences in their physiology.</p>
        <p>Convective cooling is a well-known effect of coronary perfusion that prevents vascular complications. But, if an artery is located inside the ablative target area, the efficiency of the ablation may be compromised. <xref rid="B6" ref-type="bibr">6</xref> On the other hand, repeated RF applications to avoid the cooling effect can lead to vascular damage.</p>
        <p>Paul et al. <xref rid="B7" ref-type="bibr">7</xref> demonstrated, in young pigs, the possible involvement of the right coronary artery by significant intimal thickening related to RF application in the atrial face of the tricuspid valve ring, observed 48h or 6 months later.~ <xref rid="B8" ref-type="bibr">8</xref> In a consecutive study involving 212 patients, with a median age of 12 (0.3 - 20.4) years old, who underwent coronary angiography before and 30 minutes after supraventricular catheter ablation, an acute reduction in the luminal diameter of the coronary artery was found in two patients. Both patients had a posteroseptal AP. A 40% decrease in the luminal diameter of the distal right coronary artery was observed in one case and a 35% decrease in the luminal diameter of left circumflex coronary artery in the other. These findings may be explained by coronary spasm or edema, which might be self-limited, although they can represent permanent arterial damage related to thrombosis, intimal or adventitial hyperplasia and medial necrosis. <xref rid="B8" ref-type="bibr">8</xref> </p>
        <p>LGE in CMR has become a promising tool in electrophysiology scenarios. Assessment of RF lesions can potentially demonstrate efficacy and security of this treatment, explained by the severe coagulation and contraction band necrosis with complete loss of cellular and vascular architecture in RF lesions, <xref rid="B9" ref-type="bibr">9</xref> , <xref rid="B10" ref-type="bibr">10</xref> even though some of the electrophysiologic properties can be reversed after the acute phase of RF energy delivery. <xref rid="B11" ref-type="bibr">11</xref> </p>
        <p>At the time of our CMR study, patients had a median age of 20.5 years old, and imaging evaluation was performed 6.7 years (median) after ablation. This long period after the procedure without CMR or clinical signs of adverse events suggest that the technique is safe in the pediatric age group. Additionally, myocardial fibrosis on CMR in patients after ablation showed only limited lesion extension. This small amount of myocardial fibrosis is probably related to the lesion caused by RF ablation and necessary for the therapeutic effect but did not provoke any negative effect on myocardial development in this cohort.</p>
        <p>In our cases, ablation was performed with non-irrigated catheters, but contrary to what should be expected, irrigated-tip catheters may not result in larger lesion volume than non-irrigated-tip catheters in pediatrics ablation. The reason for that may be the fact that the power is usually decreased, although irrigated catheters may result in better successful outcomes due to the more precise mapping they can enable. <xref rid="B10" ref-type="bibr">10</xref> </p>
        <p>We observed only a small rate of ventricular and atrial fibrosis and no ventricular dysfunction, seven years after RF ablation, performed during middle childhood and adolescence.</p>
        <p>These data raise the hypothesis that RF ablation does not increase the chances of arrhythmias or left ventricular dysfunction during follow-up and development of these children.</p>
        <p>In this scenario, CMR is best suited in adulthood, in whom in most cases, there is no longer need of sedation. Further studies are necessary to better evaluate younger aged patients.</p>
        <p>Although larger studies in younger patients are necessary, we believe that our data reassures the safety of RF ablation of supraventricular tachycardia in the pediatric population, especially regarding the absence of myocardial fibrosis progression and coronary artery lesions after RF application in this population in long term follow-up.</p>
        <sec>
          <title>Limitations</title>
          <p>The main limitation of our study is that it was carried out in school-age children and adolescents in whom the impact of fibrosis expansion would be less expected when compared to ablations in neonates, infants and preschoolers.</p>
          <p>The patients did not undergo a pre-ablation tomography or CMR images for comparison. However, in the three patients in whom late ventricular enhancement (myocardial fibrosis) was observed by CMR, the area of fibrosis overlapped the sites of RF application.</p>
          <p>Unfortunately, we also had a limited number of patients and details of the ablation procedure such as number of applications and median power reached could not be retrieved. Further studies are necessary to reinforce the fact that the RF ablation is safe when performed in specialized centers.</p>

        <title>Conclusions</title>
        <p>Our study demonstrated that the use of RF in the developing myocardium might indeed lead to fibrosis, but in the minority of cases and in no significant quantity. In the long-term follow-up, we did not detect coronary involvement and there was no significant fibrosis mass after the follow-up period of 7&#177;4 years.</p>
