        <title>Resumo</title>
        <sec>
          <title>Fundamento</title>
          <p> O padr&#227;o-ouro atual dos stents farmacol&#243;gicos (SF) coron&#225;rios consiste em ligas met&#225;licas com hastes mais finas e pol&#237;meros bioabsorv&#237;veis.</p>
        </sec>
        <sec>
          <title>Objetivos</title>
          <p> Nosso objetivo foi comparar um stent eluidor de sirolimus de hastes ultrafinas (Inspiron&#174;) com outras plataformas de SF de terceira gera&#231;&#227;o em pacientes com infarto do mioc&#225;rdio com supradesnivelamento do segmento ST (IAMCSST) submetidos &#224; interven&#231;&#227;o coron&#225;ria percut&#226;nea (ICP) prim&#225;ria.</p>
        </sec>
        <sec>
          <title>M&#233;todos</title>
          <p> Analisamos dados de um registro multic&#234;ntrico de IAMCSST de centros de refer&#234;ncia da Regi&#227;o Sul do Brasil. Todos os pacientes foram submetidos &#224; ICP prim&#225;ria, seja com Inspiron&#174; ou outro SF de segunda ou terceira gera&#231;&#227;o. Foi calculado pareamento por escore de propens&#227;o (PEP) para gerar grupos semelhantes (Inspiron&#174; versus outros stents) em rela&#231;&#227;o &#224;s caracter&#237;sticas cl&#237;nicas e do procedimento. Todos os testes de hip&#243;tese tiveram um n&#237;vel de signific&#226;ncia bilateral de 0,05.</p>
        </sec>
        <sec>
          <title>Resultados</title>
          <p> De janeiro de 2017 a janeiro de 2021, 1.711 pacientes foram submetidos &#224; ICP prim&#225;ria, e 1.417 pacientes preencheram nossos crit&#233;rios de inclus&#227;o (709 pacientes no grupo Inspiron&#174; e 708 pacientes no grupo dos outros SF de segunda ou terceira gera&#231;&#227;o). Ap&#243;s PEP, a amostra do estudo foi composta por 706 pacientes (353 pacientes no grupo Inspiron&#174; e 353 pacientes no grupo dos demais SF de segunda ou terceira gera&#231;&#227;o). As taxas de revasculariza&#231;&#227;o do vaso alvo (odds ratio [OR] 0,52; intervalo de confian&#231;a [IC] 0,21 a 1,34; p = 0,173), trombose de stent (OR 1,00; IC 0,29 a 3,48;p = 1,000), mortalidade (hazard ratio 0,724; IC 0,41 a 1,27; p = 0,257) e os desfechos cardiovasculares maiores (OR 1,170; IC 0,77 a 1,77; p = 0,526) foram semelhantes entre os grupos ap&#243;s um acompanhamento mediano de 17 meses.</p>
        </sec>
        <sec>
          <title>Conclus&#227;o</title>
          <p> Nossos achados mostram que o stent Inspiron&#174; foi eficaz e seguro quando comparado a outros SF de segunda ou terceira gera&#231;&#227;o em uma coorte contempor&#226;nea do mundo real de pacientes com IAMCSST submetidos &#224; ICP prim&#225;ria.</p>

      <title>Introdu&#231;&#227;o</title>
      <p>Os stents farmacol&#243;gicos (SF) coron&#225;rios est&#227;o em constante evolu&#231;&#227;o, e os dispositivos mais novos devem apresentar seguran&#231;a e efic&#225;cia para serem utilizados na pr&#225;tica di&#225;ria. O infarto do mioc&#225;rdio com supradesnivelamento do segmento ST (IAMCSST) &#233; provavelmente o cen&#225;rio cl&#237;nico mais desafiador para comparar os SF mais recentes com aqueles j&#225; estabelecidos, devido aos riscos hospitalares e de longo prazo mais elevados de trombose de stent, infarto do mioc&#225;rdio recorrente e morte.</p>
      <p>O stent eluidor de sirolimus Inspiron&#174; (Scitech MedicalTM, Goi&#226;nia, Brasil) utiliza uma liga de cobalto-cromo L-605 ultrafina com uma plataforma de haste de 75 &#956;m de espessura revestida com um pol&#237;mero abluminal biodegrad&#225;vel. Os registros da popula&#231;&#227;o do tipo &#8220;all-comers&#8221; demonstraram seguran&#231;a e excelente desempenho, com baixa taxa de eventos card&#237;acos adversos.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref> Em um ensaio cl&#237;nico randomizado comparando Inspiron&#174; com o stent eluidor de biolimus Biomatrix Flex&#174;, n&#227;o houve diferen&#231;a nos resultados em pacientes submetidos &#224; interven&#231;&#227;o coron&#225;ria percut&#226;nea (ICP) eletiva ou urgente ap&#243;s cinco anos de seguimento. Al&#233;m disso, n&#227;o houve trombose de stent em pacientes tratados com Inspiron&#174; durante o per&#237;odo do estudo.<xref ref-type="bibr" rid="B3">3</xref></p>
      <p>Com o uso amplamente difundido do dispositivo, s&#227;o necess&#225;rios dados de um n&#250;mero maior de pacientes em um cen&#225;rio de alto risco. Nosso objetivo foi comparar o stent eluidor de sirolimus Inspiron&#174; com outras plataformas de SF de terceira gera&#231;&#227;o seguras, bem estudadas e estabelecidas em pacientes com IAMCSST submetidos &#224; ICP prim&#225;ria.</p>

        <title>Desenho do estudo e sele&#231;&#227;o de pacientes</title>
        <p>Este &#233; um registro prospectivo, no qual inclu&#237;mos pacientes consecutivos admitidos com IAMCSST e tratados com ICP prim&#225;ria com SF de segunda e terceira gera&#231;&#227;o em dois hospitais terci&#225;rios (o Hospital de Cl&#237;nicas de Porto Alegre, um hospital geral, e o Instituto de Cardiologia do Rio Grande do Sul, um centro de cardiologia) na Regi&#227;o Sul do Brasil entre os anos de 2017 e 2021. Esta an&#225;lise espec&#237;fica foi realizada retrospectivamente, ou seja, n&#227;o havia sido pr&#233;-definida no momento em que o registro foi iniciado. O IAMCSST foi definido como dor tor&#225;cica t&#237;pica em repouso associada &#224; eleva&#231;&#227;o do segmento ST de pelo menos 1 mm em duas deriva&#231;&#245;es cont&#237;guas no plano frontal ou de 2 mm no plano horizontal, ou como dor t&#237;pica em repouso em pacientes com um bloqueio de ramo esquerdo novo ou presumivelmente novo. Os crit&#233;rios de exclus&#227;o foram aus&#234;ncia de uso de SF de segunda e terceira gera&#231;&#227;o e aus&#234;ncia de acompanhamento. O presente estudo foi aprovado pelo Comit&#234; Institucional de Pesquisa e &#201;tica de ambas as institui&#231;&#245;es e obteve consentimento informado de todos os pacientes. Os dados foram registrados prospectivamente em formul&#225;rios adequados, armazenados em planilhas eletr&#244;nicas e posteriormente coletados do banco de dados.</p>

        <title>Aspectos do procedimento</title>
        <p>Amostras de sangue foram coletadas por pun&#231;&#227;o venosa antes do procedimento, como parte da rotina de atendimento ao paciente. Todos os pacientes foram pr&#233;-tratados com dose de ataque de &#225;cido acetilsalic&#237;lico (300 mg) e clopidogrel (300 a 600 mg), e heparina n&#227;o fracionada foi usada durante o procedimento (70 a 100 UI/kg). As estrat&#233;gias t&#233;cnicas de ICP e a sele&#231;&#227;o do stent foram realizadas de acordo com a escolha do operador. O fluxo coronariano antes e ap&#243;s o procedimento foi avaliado e descrito de acordo com os crit&#233;rios Thrombolysis in Myocardial Infarction (TIMI). Os anticoagulantes foram suspensos ap&#243;s o t&#233;rmino do procedimento, e a dura&#231;&#227;o da terapia antiplaquet&#225;ria dupla foi recomendada a crit&#233;rio do cardiologista. O sucesso da ICP prim&#225;ria foi definido como obten&#231;&#227;o do fluxo TIMI 2 ou 3 e pat&#234;ncia do vaso com estenose residual &lt; 30%.</p>

        <p>Todos os pacientes inclu&#237;dos receberam um SF Inspiron&#174; ou outro de segunda ou terceira gera&#231;&#227;o. A decis&#227;o de implantar qual tipo de stent foi baseada no crit&#233;rio do operador e na disponibilidade do centro. Al&#233;m do Inspiron&#174;, os outros SF utilizados foram Xience (laborat&#243;rios Abbott, Chicago, EUA), Resolute Integrity (Medtronic, Minneapolis, EUA), Supraflex (SMT, Mumbai, &#205;ndia), Orsiro (Biotronic, Berlim, Alemanha) e Ultimaster (Terumo, T&#243;quio, Jap&#227;o). O n&#250;mero de pacientes tratados com cada tipo de SF, material de haste e espessura, droga antiproliferativa e tipo de pol&#237;mero est&#227;o resumidos na <xref ref-type="fig" rid="f02">Figura 1</xref>.</p>

        <title>An&#225;lise estat&#237;stica</title>
        <p>As vari&#225;veis cont&#237;nuas foram descritas como m&#233;dia e desvio padr&#227;o. As vari&#225;veis categ&#243;ricas foram apresentadas como n&#250;meros absolutos e percentuais e comparadas usando o teste qui-quadrado ou teste exato de Fisher, quando apropriado. Os grupos de pacientes foram comparados usando o teste t de Student para amostras independentes para vari&#225;veis cont&#237;nuas e qui-quadrado ou testes exatos de Fisher para vari&#225;veis categ&#243;ricas. A normalidade da distribui&#231;&#227;o de cada vari&#225;vel foi avaliada pelo teste de Shapiro&#8211;Wilk.</p>
        <p>Para limitar os vieses, foi usada a an&#225;lise de pareamento por escore de propens&#227;o (PEP). Visto que as caracter&#237;sticas basais dos dois grupos eram bastante diferentes e seu tamanho da amostra semelhante, selecionamos aleatoriamente 50% dos pacientes do grupo Inspiron&#174; para reduzir a dist&#226;ncia do escore de propens&#227;o e, portanto, reduzir grandes discrep&#226;ncias entre os grupos. A sele&#231;&#227;o aleat&#243;ria foi realizada na plataforma Statistical Package for the Social Sciences (SPSS), com os seguintes comandos: selecionar casos &#8805; amostra aleat&#243;ria de casos &#8805; 50% de todos os casos. A regress&#227;o log&#237;stica foi realizada com Inspiron&#174; como vari&#225;vel dependente e as seguintes vari&#225;veis independentes: idade, diabetes, creatinina na admiss&#227;o, parada card&#237;aca pr&#233;-ICP e classifica&#231;&#227;o de Killip. A validade da regress&#227;o log&#237;stica foi avaliada usando o teste de Hosmer&#8211;Lemeshow. Subsequentemente, foi realizado PEP usando m&#233;todos do vizinho mais pr&#243;ximo, onde foram criados 2 grupos de 353 pacientes cada. A regress&#227;o de Cox para as taxas de eventos de acompanhamento de longo prazo de infarto do mioc&#225;rdio, trombose de stent, revasculariza&#231;&#227;o, morte e MACCE foi calculada para popula&#231;&#227;o n&#227;o pareada e grupos pareados.</p>
        <p>Um modelo linear generalizado com regress&#227;o log&#237;stica bin&#225;ria tamb&#233;m foi realizado em pacientes gerais (e n&#227;o em cima do PEP), e as mesmas vari&#225;veis do PEP foram inclu&#237;das como covari&#225;veis em um modelo multivariado. Nosso objetivo foi mostrar resultados de dois modelos estat&#237;sticos diferentes comumente usados. Todos os testes de hip&#243;tese tiveram um n&#237;vel de signific&#226;ncia bilateral de 0,05. Todos os dados foram analisados usando SPSS, vers&#227;o 17.0.</p>

      <title>Discuss&#227;o</title>
      <p>Em um registro contempor&#226;neo do mundo real de pacientes com IAMCSST representativo da pr&#225;tica di&#225;ria de hospitais terci&#225;rios, um stent eluidor de sirolimus, de pol&#237;mero biodegrad&#225;vel de cobalto-cromo, de hastes finas, demonstrou efic&#225;cia e seguran&#231;a com baixa incid&#234;ncia de desfechos adversos em 12 meses. Embora as diferen&#231;as de linha de base fossem pronunciadas e os pacientes tratados com Inspiron&#174; apresentassem um perfil de risco mais baixo, ap&#243;s PSM, Inspiron&#174; n&#227;o foi inferior em compara&#231;&#227;o com outros SF de segunda e terceira gera&#231;&#227;o bem estabelecidos em rela&#231;&#227;o a MACCE e seus componentes individuais.</p>
      <p>Desde a introdu&#231;&#227;o dos stents coron&#225;rios no final de d&#233;cada de 80, houve cont&#237;nuas melhorias t&#233;cnicas e dos dispositivos visando reduzir os desfechos adversos relacionados tanto &#224; apresenta&#231;&#227;o cl&#237;nica quanto &#224;s complica&#231;&#245;es relacionadas aos stents. Em compara&#231;&#227;o com os SF de primeira gera&#231;&#227;o, os SF contempor&#226;neos de segunda e terceira gera&#231;&#227;o t&#234;m hastes mais finas, e a mudan&#231;a na plataforma do stent de ligas de cromo de a&#231;o inoxid&#225;vel (de 130&#8211;149 para 81&#8211;91 &#956;m) reduziu o infarto do mioc&#225;rdio do vaso alvo tardio e durante o procedimento.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B5">5</xref> Hastes mais finas produzem menos les&#245;es nos vasos, inflama&#231;&#227;o, prolifera&#231;&#227;o neointimal e forma&#231;&#227;o de trombos em compara&#231;&#227;o com stents com hastes mais espessas.<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B7">7</xref> Al&#233;m disso, a espessura das hastes tem sido um elemento-chave no desenho do stent, pois hastes mais finas est&#227;o relacionadas a uma maior capacidade de entrega do stent. Por outro lado, hastes mais finas podem ter efeitos indesej&#225;veis, como menor for&#231;a radial e maior risco de deforma&#231;&#227;o do stent ao negociar anatomias dif&#237;ceis, o que destaca a import&#226;ncia de avaliar seus resultados em coortes contempor&#226;neas de pacientes tratados na pr&#225;tica do mundo real.</p>
      <p>Outra caracter&#237;stica dos stents de terceira gera&#231;&#227;o &#233; a presen&#231;a (em alguns deles) de pol&#237;mero bioabsorv&#237;vel. Isso possibilita a libera&#231;&#227;o controlada do f&#225;rmaco e posterior dissolu&#231;&#227;o do material polim&#233;rico, evitando est&#237;mulos para inflama&#231;&#227;o cr&#244;nica e risco de posterior trombose do stent. Em um ensaio amplo do tipo &#8220;all-comers&#8221; comparando stents eluidores de sirolimus com pol&#237;mero bioabsorv&#237;vel e stents eluidores de everolimus com pol&#237;mero dur&#225;vel, a ocorr&#234;ncia de eventos cl&#237;nicos foi semelhante entre os grupos, embora o IAMCSST tenha sido a apresenta&#231;&#227;o cl&#237;nica em apenas 19% dos pacientes inclu&#237;dos.<xref ref-type="bibr" rid="B8">8</xref> Mais tarde, em um estudo randomizado envolvendo 1.334 pacientes (50% apresentando s&#237;ndrome coronariana aguda), um stent eluidor de sirolimus, ultrafino e bioabsorv&#237;vel teve menor falha na les&#227;o alvo (6% versus 10%; intervalo de confian&#231;a 95% &#8211;6,84 a &#8211;0,29; p = 0,0399 ) e infarto do mioc&#225;rdio no vaso alvo (5% versus 8%, p = 0,0155) em 12 meses em compara&#231;&#227;o com um stent eluidor de everolimus de pol&#237;mero dur&#225;vel.<xref ref-type="bibr" rid="B9">9</xref> Outro estudo comparando os mesmos SF acima, mas exclusivamente em pacientes com IAMCSST encontrou resultados semelhantes, com taxas mais baixas de falha da les&#227;o alvo com um stent eluidor de sirolimus de pol&#237;mero bioabsorv&#237;vel.<xref ref-type="bibr" rid="B10">10</xref> Essas diferen&#231;as, no entanto, podem ser causadas pela diferen&#231;a na espessura da haste e n&#227;o pela durabilidade do pol&#237;mero, uma vez que o pol&#237;mero do stent Orsiro&#174; se degrada em um per&#237;odo de dois anos.</p>
      <p>O pol&#237;mero bioabsorv&#237;vel isoladamente n&#227;o garante a qualidade do SF. Metan&#225;lises anteriores indicaram um risco excessivo de eventos adversos com pol&#237;meros bioabsorv&#237;veis em compara&#231;&#227;o com stents de pol&#237;meros dur&#225;veis, com alta heterogeneidade de dispositivos nos grupos de pol&#237;meros bioabsorv&#237;veis.<xref ref-type="bibr" rid="B11">11</xref>,<xref ref-type="bibr" rid="B12">12</xref> Por outro lado, uma metan&#225;lise mais recente envolvendo pacientes submetidos &#224; ICP de art&#233;ria coron&#225;ria principal esquerda desprotegida usando stents ultrafinos (hastes mais finas que 81 &#956;m) mostrou resultados semelhantes em termos de MACCE com pol&#237;mero bioabsorv&#237;vel e stents de pol&#237;mero dur&#225;vel, e diferen&#231;as na trombose de stent n&#227;o foram evidentes entre os grupos.<xref ref-type="bibr" rid="B13">13</xref> Em les&#245;es de bifurca&#231;&#227;o tratadas com dois stents, no entanto, os pacientes tratados com SF de pol&#237;mero biodegrad&#225;vel apresentaram melhor desfecho em termos de MACCE e revasculariza&#231;&#227;o do vaso alvo. Esses dados sugerem que evitar o est&#237;mulo inflamat&#243;rio prolongado &#233; especialmente importante em cen&#225;rios mais trombog&#234;nicos, como s&#237;ndromes coronarianas agudas e les&#245;es de bifurca&#231;&#227;o. &#201; not&#225;vel que o Inspiron&#174; suporta um acesso suave ao ramo lateral com seu design de c&#233;lula aberta, e uma an&#225;lise dedicada nesta configura&#231;&#227;o tamb&#233;m &#233; merecida.</p>
      <p>Estudos avaliando o stent Inspiron&#174; relataram propriedades promissoras de cicatriza&#231;&#227;o dos vasos, com pouca hiperplasia neointimal por ultrassom intravascular (percentual de obstru&#231;&#227;o por hiperplasia neointimal de 4,9% &#177; 4,1%) e altas taxas de cobertura das hastes por tomografia de coer&#234;ncia &#243;ptica (99,49% &#177; 1,01%).<xref ref-type="bibr" rid="B14">14</xref> Embora o Inspiron&#174; tenha demonstrado ser seguro em um ensaio cl&#237;nico randomizado pr&#233;vio com uma popula&#231;&#227;o &#8220;all-comers&#8221; com um longo acompanhamento,<xref ref-type="bibr" rid="B3">3</xref> o uso generalizado de dispositivos mais novos pode levar tempo, especialmente em pacientes e anatomias de risco elevados. Em nosso estudo, o risco basal da popula&#231;&#227;o Inspiron&#174; foi claramente menor, e uma das hip&#243;teses &#233; que os operadores tendem a escolher dispositivos bem estabelecidos em casos mais complexos. O grupo Inspiron&#174; apresentou taxas mais baixas de acidente vascular cerebral de longo prazo, mesmo ap&#243;s o PEP, possivelmente devido a outras vari&#225;veis de confus&#227;o n&#227;o inclu&#237;das no modelo.</p>
      <p>O presente estudo tem limita&#231;&#245;es, em primeiro lugar, as limita&#231;&#245;es inerentes aos estudos observacionais, onde a escolha do tratamento foi baseada na prefer&#234;ncia do operador. O vi&#233;s de sele&#231;&#227;o foi altamente prov&#225;vel, embora a an&#225;lise estat&#237;stica possa ter atenuado esse problema. Em segundo lugar, a an&#225;lise retrospectiva pode ter influenciado a qualidade e a consist&#234;ncia dos dados coletados. No entanto, este foi um registro representativo de IAMCSST de dois centros com crit&#233;rios de inclus&#227;o amplos e dados cl&#237;nicos altamente aplic&#225;veis.</p>

      <title>Conclus&#245;es</title>
      <p>Nossos achados sustentam que o Inspiron&#174; &#233; seguro e eficaz em pacientes com IAMCSST, com desfechos semelhantes em compara&#231;&#227;o com SF de terceira gera&#231;&#227;o bem estabelecidos no tratamento com ICP prim&#225;ria em acompanhamento de curto e longo prazo.</p>

        <title>Abstract</title>
        <sec>
          <title>Background</title>
          <p> The current gold standard of coronary drug-eluting stents (DES) consists of metal alloys with thinner struts and bioresorbable polymers.</p>
        </sec>
        <sec>
          <title>Objectives</title>
          <p> Our aim was to compare an ultrathin strut, sirolimus-eluting stent (Inspiron&#174;) with other third-generation DES platforms in patients with ST-elevation myocardial infarction (STEMI) submitted to primary percutaneous coronary intervention (PCI).</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p> We analyzed data from a STEMI multicenter registry from reference centers in the South Region of Brazil. All patients were submitted to primary PCI, either with Inspiron&#174; or other second- or third-generation DES. Propensity score matching (PSM) was computed to generate similar groups (Inspiron&#174; versus other stents) in relation to clinical and procedural characteristics. All hypothesis tests had a two-sided significance level of 0.05.</p>
        </sec>
        <sec>
          <title>Results</title>
          <p> From January 2017 to January 2021, 1711 patients underwent primary PCI, and 1417 patients met our entry criteria (709 patients in the Inspiron&#174; group and 708 patients in the other second- or third-generation DES group). After PSM, the study sample was comprised of 706 patients (353 patients in the Inspiron&#174; group and 353 patients in the other the other second- or third-generation DES group). The rates of target vessel revascularization (OR 0.52, CI 0.21 &#8211; 1.34, p = 0.173), stent thrombosis (OR 1.00, CI 0.29 &#8211; 3.48, p = 1.000), mortality (HR 0.724, CI 0.41 &#8211; 1.27, p = 0.257), and major cardiovascular outcomes (OR 1.170, CI 0.77 &#8211; 1.77, p = 0.526) were similar between groups after a median follow-up of 17 months.</p>

        <title>Introduction</title>
        <p>Coronary drug-eluting stents (DES) are continuously evolving, and newer devices should present safety and efficacy in order to be used in daily practice. ST-elevation myocardial infarction (STEMI) is probably the most challenging clinical scenario to compare newer DES with established ones, because of higher in-hospital and long-term risks of stent thrombosis, recurrent myocardial infarction, and death.</p>
        <p>The Inspiron&#174; sirolimus-eluting stent (Scitech MedicalTM, Goi&#226;nia, Brazil) uses an ultrathin L-605 cobalt-chromium alloy with a 75 &#956;m rod thickness platform coated with a biodegradable abluminal polymer. Registries of all-comers population demonstrated safety and excellent performance, with a low rate of adverse cardiac events.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref> In a randomized clinical trial comparing Inspiron&#174; with Biomatrix Flex&#174; biolimus-eluting stent, there was no difference in outcomes in patients undergoing elective or urgent percutaneous coronary intervention (PCI) after a five-year follow-up. Also, there was no stent thrombosis in patients treated with Inspiron&#174; during the study period.<xref ref-type="bibr" rid="B3">3</xref></p>
        <p>With the widespread use of the device, data from a greater number of patients in a high-risk scenario are needed. Our aim was to compare the Inspiron&#174; sirolimus-eluting stent with other safe, well studied and established third-generation DES platforms in patients with STEMI submitted to primary PCI.</p>

        <title>Methods</title>
        <sec>
          <title>Study design and patient selection</title>
          <p>This is a prospective registry, in which we included consecutive patients admitted with STEMI and treated with primary PCI using second- and third-generation DES in two tertiary hospitals (Hospital de Clinicas de Porto Alegre, a general hospital, and Instituto de Cardiologia do Rio Grande do Sul, a cardiology center) in the South Region of Brazil between the years of 2017 and 2021. This specific analysis was conducted retrospectively; that is, it had not been pre-defined at the time registration was started. STEMI was defined as typical chest pain at rest associated with ST-segment elevation of at least 1 mm in two contiguous leads in the frontal plane or 2 mm in the horizontal plane, or typical pain at rest in patients with a new, or presumably new, left bundle-branch block. Exclusion criteria were absence of second- and third-generation DES use and lack of follow-up. This study was approved by the Institutional Research and Ethics Committee of both institutions, and informed consent was obtained from all patients. The data were prospectively recorded in appropriate forms, stored in electronic spreadsheets and later collected from the database.</p>
        </sec>
        <sec>
          <title>Procedural aspects</title>
          <p>Blood samples were collected by venipuncture before the procedure, as part of routine patient care. All patients were pre-treated with a loading dose of acetylsalicylic acid (300 mg) and clopidogrel (300 to 600 mg), and unfractioned heparin was used during the procedure (70 to 100 IU/kg). PCI technical strategies and stent selection were performed according to the operator&#8217;s choice. Coronary flow before and after the procedure was assessed and described according to the Thrombolysis in Myocardial Infarction (TIMI) criteria. Anticoagulants were suspended after the end of procedure, and dual antiplatelet therapy duration was recommended at the cardiologist&#8217;s discretion. Primary PCI success was defined as achievement of TIMI flow 2 or 3 and vessel patency with residual stenosis &lt; 30%.</p>

          <title>Stents</title>
          <p>All patients included received Inspiron&#174; or a second- or third-generation DES. The decision to implant which type of stent was based on operator discretion and center availability. Besides Inspiron&#174;, the other platforms used were Xience (Abbott laboratories, Chicago, USA), Resolute Integrity (Medtronic, Minneapolis, USA), Supraflex (SMT, Mumbai, India), Orsiro (Biotronic, Berlin, Germany), and Ultimaster (Terumo, Tokyo, Japan). The number of patients treated with each type of DES, strut material and thickness, antiproliferative drug, and polymer type are summarized in <xref ref-type="fig" rid="f02001">Figure 1</xref>.</p>

          <title>Study outcomes</title>
          <p>The clinical outcomes analyzed were the occurrence of the following isolated or combined major cardiovascular outcomes (MACCE): a new myocardial infarction, stent thrombosis, target vessel revascularization, stroke, and death. Clinical follow-up was carried out through outpatient consultation or telephone contact.</p>

          <title>Statistical analysis</title>
          <p>Continuous variables were described as mean and standard deviation. Categorical variables were presented as absolute and percentage numbers and compared using the chi-square test or Fisher&#8217;s exact test, when appropriate. Patient groups were compared using Student&#8217;s t test for independent samples for continuous variables and chi-square or Fisher&#8217;s exact tests for categorical variables. The normality of the distribution of each variable was assessed by the Shapiro&#8211;Wilk test.</p>
          <p>To limit biases, propensity score matching (PSM) analysis was used. Because the baseline characteristics of the two groups were quite different and their sample size were similar, we randomly selected 50% of the Inspiron&#174; patients in order to reduce the propensity score distance and therefore reduce large score discrepancies between groups. The random selection was performed in the Statistical Package for the Social Sciences (SPSS) platform, with the following commands: select cases &#8805; random sample of cases &#8805; 50% of all the cases. The logistic regression was performed with Inspiron&#174; as a dependent variable and the following as independent variables: age, diabetes, admission creatinine, pre-PCI cardiac arrest, and Killip classification. The validity of logistic regression was assessed using the Hosmer&#8211;Lemeshow test. Subsequently, PSM was performed using nearest neighbor methods, where 2 groups of 353 patients each were created. Cox regression for long-term follow-up event rates of myocardial infarction, stent thrombosis, revascularization, death, and MACCE was calculated for unmatched population and matched groups.</p>
          <p>A generalized linear model with binary logistic regression was also performed in overall patients (and not on top of PSM), and the same variables of PSM were included as covariates in a multivariate model. Our objective was to show results of two different statistical models commonly used. All hypothesis tests had a two-sided significance level of 0.05 All data were analyzed using SPSS, version 17.0.</p>

        <title>Discussion</title>
        <p>In a contemporary, real-world registry of STEMI patients representative of the daily practice of tertiary hospitals, a thin strut, cobalt-chromium, biodegradable polymer sirolimus-eluting stent demonstrated effectiveness and safety with a low incidence of adverse outcomes at 12 months. Although baseline differences were pronounced and patients treated with Inspiron&#174; displayed a lower risk profile, after PSM, Inspiron&#174; was not inferior compared to other well established second- and third-generation DES regarding MACCE and its individual components.</p>
        <p>Since the introduction of coronary stents in late 80s, there have been continuing technical and device improvements aimed at reducing adverse outcomes related to both clinical presentation and stent-related complications. Compared to first-generation DES, contemporary second- and third-generation DES have thinner struts, and the change in stent platform from stainless steel chromium alloys (130&#8211;149 to 81&#8211;91 &#956;m) reduced both procedural and late target vessel myocardial infarction.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B5">5</xref> Thinner struts produce less vessel injury, inflammation, neointimal proliferation, and thrombus formation compared with thicker strut stents.<xref ref-type="bibr" rid="B6">6</xref>,<xref ref-type="bibr" rid="B7">7</xref> Moreover, strut thickness has been a key element in stent design, as thinner struts are related to a greater stent deliverability. On the other hand, thinner struts may have undesirable effects, such as lower radial force and higher risk of stent deformation when negotiating difficult anatomies, which highlights the importance of assessing its results on contemporary cohorts of patients treated in real-world practice.</p>
        <p>Another characteristic of third-generation stents is the presence (in some of them) of bioresorbable polymer. It enables controlled drug release and subsequent dissolution of the polymer material, avoiding stimulus for chronic inflammation risk of further stent thrombosis. In a large, all-comers trial comparing bioresorbable polymer sirolimus-eluting stents and durable polymer everolimus-eluting stents, the occurrence of clinical events was similar between groups, although STEMI was the clinical presentation in only 19% of included patients.<xref ref-type="bibr" rid="B8">8</xref> Later, in a randomized trial involving 1334 patients (50% presenting with acute coronary syndrome), an ultrathin, bioresorbable polymer sirolimus-eluting stent had lower target lesion failure (6% versus 10%, 95% CI &#8211;6.84 to &#8211;0.29, p = 0.0399) and target vessel myocardial infarction (5% versus 8%, p = 0.0155) at 12 months compared to a durable polymer everolimus-eluting stent.<xref ref-type="bibr" rid="B9">9</xref> Another study comparing the same DES above, but exclusively in patients with STEMI found similar results, with lower rates of target lesion failure with a bioresorbable polymer sirolimus-eluting stent.<xref ref-type="bibr" rid="B10">10</xref> These differences, however, may be driven by the difference in strut thickness rather than polymer durability, since the Orsiro&#174; stent polymer degrades over a two-year span.</p>
        <p>Bioresorbable polymer alone does not guarantee the quality of the DES. Previous meta-analyses have indicated an excess risk of adverse events with bioresorbable polymer compared with durable polymer stents, with a high heterogeneity of devices in the bioresorbable polymer groups.<xref ref-type="bibr" rid="B11">11</xref>,<xref ref-type="bibr" rid="B12">12</xref> Conversely, a more recent meta-analysis involving patients submitted to PCI of unprotected left main coronary artery using ultrathin stents (struts thinner than 81 &#956;m) showed similar results in terms of MACCE with bioresorbable polymer and durable polymer stents, and no differences in stent thrombosis were evident between groups.<xref ref-type="bibr" rid="B13">13</xref> In bifurcation lesions treated with two stents, however, patients treated with biodegradable polymer DES showed a better outcome in terms of MACCE and target-vessel revascularization. These data suggest that avoiding prolonged inflammatory stimulus is especially important in more thrombogenic settings, such as acute coronary syndromes and bifurcation lesions. It is noteworthy that Inspiron&#174; supports a smooth side branch access with its open-cell design, and dedicated analysis in this setting is also warranted.</p>
        <p>Studies evaluating Inspiron&#174; stent have reported reassuring vessel healing properties, with very little neointimal hyperplasia either by intravascular ultrasound (percent of neointimal hyperplasia obstruction of 4.9% &#177; 4.1%) and high rates of strut coverage by optical coherence tomography (99.49% &#177; 1.01%).<xref ref-type="bibr" rid="B14">14</xref> Although Inspiron&#174; was demonstrated safe in a previous randomized clinical trial with an all-comers population with a long follow-up,<xref ref-type="bibr" rid="B3">3</xref> widespread use of newer devices may take time, especially in higher risk patients and anatomy. In our study, baseline risk of the Inspiron&#174; population was clearly lower, and one of the hypotheses is that operators tend to choose well established devices in more complex cases. The Inspiron&#174; group had lower rates of long-term stroke, even after PSM, possibly because of other confounding variables not included in the model.</p>
        <p>This study has limitations, first, the limitations that are inherent in observational studies, where choice of treatment was based on the operator&#8217;s preference. Selection bias was highly probable, although statistical analysis may have mitigated this issue. Second, the retrospective analysis may have influenced the quality and consistency of the data collected. However, this was a representative two-center STEMI registry with broad inclusion criteria and highly applicable clinical data.</p>

        <p>Our findings support that Inspiron&#174; is safe and effective in patients with STEMI, with similar outcomes compared to well established third-generation DES in treatment with primary PCI at short- and long-term follow-up.</p>
