    <p>A doen&#231;a ateroscler&#243;tica coron&#225;ria tem v&#225;rias facetas de apresenta&#231;&#227;o cl&#237;nica, sendo a oclus&#227;o coron&#225;ria cr&#244;nica (OCC), aquela que produz obstru&#231;&#227;o total da luz do vaso conhecida ou presumida com tr&#234;s meses ou mais. Est&#225; presente em 16-18% dos pacientes que possuem doen&#231;a coron&#225;ria significativa.<xref ref-type="bibr" rid="B1">1</xref></p>
    <p>O tratamento percut&#226;neo deste tipo de les&#227;o &#233; quase t&#227;o antigo quanto a primeira angioplastia coron&#225;ria realizada por Andreas Gruntzig em 1979. Foi realizada pela primeira vez por Martin Kaltenbach em Frankfurt no final da d&#233;cada de 70 e por Geoffrey Hartzler em Kansas City nas d&#233;cadas seguintes.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref> Desde ent&#227;o, a crescente e espetacular incorpora&#231;&#227;o de tecnologia &#224; medicina, especialmente na Cardiologia Intervencionista, atrelada ao conhecimento e expertise de novas t&#233;cnicas dedicadas &#224;s interven&#231;&#245;es coron&#225;rias complexas, onde est&#227;o inclu&#237;das as OCC, permitiram grande, seguro e efetivo avan&#231;o na abordagem destas les&#245;es.</p>
    <p>Existem registros, documentos, artigos e consensos internacionais dedicados a este cen&#225;rio.<xref ref-type="bibr" rid="B4">4</xref>-<xref ref-type="bibr" rid="B6">6</xref> Conhecer os dados nacionais se faz de extrema import&#226;ncia, pois nos posiciona perante a comunidade internacional. Tamb&#233;m possibilita o desenvolvimento de medidas assertivas e efetivas e programas dedicados &#224; educa&#231;&#227;o m&#233;dica continuada e o desenvolvimento de pol&#237;ticas tanto na esfera p&#250;blica quanto na sa&#250;de suplementar para este tipo de procedimento, o qual requer n&#227;o s&#243; conhecimento t&#233;cnico, mas tamb&#233;m dispositivos dedicados de alto custo, nem sempre dispon&#237;veis.</p>
    <p>No artigo aqui publicado por Botelho da Silva, et al.<xref ref-type="bibr" rid="B7">7</xref> &#8220;Panorama das interven&#231;&#245;es coronarianas percut&#226;neas em oclus&#245;es totais cr&#244;nicas em centros participantes do LATAM CTO REGISTRY no Brasil&#8221;, temos a grata felicidade de observar que nosso pa&#237;s est&#225; muito bem posicionado e representado frente &#224;s mais importantes institui&#231;&#245;es do mundo que se dedicam ao tratamento destas obstru&#231;&#245;es, com similares taxas de sucesso t&#233;cnico (84% dos procedimentos), eventos adversos (2,3%) e de mortalidade (0,75%). Este primeiro relato da pr&#225;tica m&#233;dica acerca da recanaliza&#231;&#227;o das OCC em 26 centros no Brasil mostra tamb&#233;m um fator primordial para nossa pr&#225;tica cl&#237;nica di&#225;ria, que &#233; a excepcional ader&#234;ncia &#224;s indica&#231;&#245;es cl&#237;nicas para este tipo de procedimento: cerca de 95% dos pacientes apresentavam ou angina limitante ou isquemia moderada/severa documentada.</p>
    <p>Cabe aqui registrar, n&#227;o s&#243; para este registro bem como para todos os outros dedicados &#224; esta estrat&#233;gia, a baixa utiliza&#231;&#227;o de imagem intravascular, principalmente do ultrassom intracoron&#225;rio, utilizado em 10% dos casos desta s&#233;rie, provavelmente por problemas de reembolso tanto na esfera p&#250;blica quanto privada. Estudos<xref ref-type="bibr" rid="B8">8</xref> t&#234;m demonstrado uma redu&#231;&#227;o significativa de eventos combinados (morte e infarto) e da revasculariza&#231;&#227;o do vaso alvo quando esta ferramenta &#233; utilizada com o objetivo de otimizar os resultados da interven&#231;&#227;o e tamb&#233;m para auxiliar na identifica&#231;&#227;o e localiza&#231;&#227;o da capa proximal quando esta &#233; amb&#237;gua, bem como nas t&#233;cnicas de dissec&#231;&#227;o-reentrada, aumentando as taxas de sucesso deste procedimento.</p>
    <p>A Cardiologia Intervencionista Brasileira continua sendo equiparada aos maiores e melhores centros mundiais, a despeito de toda e qualquer limita&#231;&#227;o que porventura possa existir em nosso complexo sistema de reembolso, quer seja ele p&#250;blico ou privado.</p>

    <body>
      <p>The coronary atherosclerotic disease has several facets of clinical presentation; chronic coronary occlusion (CCO) is one that produces total obstruction of the lumen of the vessel, known or presumed for three months or more. It is present in 16-18% of patients with significant coronary disease.<xref ref-type="bibr" rid="B1">1</xref></p>
      <p>Percutaneous treatment of this type of lesion is almost as old as the first coronary angioplasty performed by Andreas Gruntzig in 1979. Martin Kaltenbach first performed it in Frankfurt in the late 1970s and by Geoffrey Hartzler in Kansas City in the following decades.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref> Since then, the growing and spectacular incorporation of technology into medicine, especially in Interventional Cardiology, associated with the knowledge and expertise of new techniques dedicated to complex coronary interventions, which include CCO, have allowed great, safe, and effective advances in the approach to these lesions.</p>
      <p>Records, documents, articles, and international consensuses are dedicated to this scenario.<xref ref-type="bibr" rid="B4">4</xref>-<xref ref-type="bibr" rid="B6">6</xref> Knowing the national data is extremely important, as it positions us before the international community. It also enables the development of assertive and effective measures and programs dedicated to continuing medical education and the development of policies both in the public sphere and in supplementary health for this type of procedure, which requires not only technical knowledge but also dedicated high-cost devices, not always available.</p>
      <p>In the article published here by Botelho da Silva AC et al.<xref ref-type="bibr" rid="B7">7</xref> &#8220;Overview of percutaneous coronary interventions in chronic total occlusions in centers participating in the LATAM CTO REGISTRY in Brazil&#8221;, we have grateful happiness to observe that our country is very well positioned and represented in the face of the most important institutions in the world that are dedicated to the treatment of these obstructions, with similar technical success rates (84% of procedures), adverse events (2.3%) and mortality (0.75%). This first report from medical practice about the recanalization of CCO in 26 centers in Brazil also shows a key factor for our daily clinical practice, which is the exceptional adherence to the clinical indications for this type of procedure: about 95% of the patients had either angina or limiting or documented moderate/severe ischemia.</p>
      <p>It is worth noting here, not only for this registry as well as for all others dedicated to this strategy, the low use of intravascular imaging, mainly intravascular ultrasound, used in 10% of the cases in this series, probably due to reimbursement problems both in the public sphere how private. Studies<xref ref-type="bibr" rid="B8">8</xref> have shown a significant reduction in combined events (death and infarction) and target vessel revascularization when this tool is used to optimize intervention results and also to help identify and locate the proximal layer when it is ambiguous, as well as in dissection-reentry techniques, increasing the success rates of this procedure.</p>
      <p>Brazilian Interventional Cardiology continues to be equated with the largest and best centers in the world, despite any and all limitations that may exist in our complex reimbursement system, whether public or private.</p>
      <p>Happy reading to everyone.</p>
