        <sec>
          <title>M&#233;todos</title>
          <p> Um total de 53 pacientes foram inclu&#237;dos e alocados aleatoriamente em um programa de treinamento supervisionado (grupo TREINO, n=27) ou em um grupo CONTROLE (n=26) que recebeu recomenda&#231;&#245;es usuais sobre a pr&#225;tica de exerc&#237;cio f&#237;sico ap&#243;s um IAM. Todos os pacientes realizaram um teste cardiopulmonar e um ecocardiograma com speckle tracking para medir v&#225;rios par&#226;metros da mec&#226;nica de contra&#231;&#227;o do VE em um m&#234;s e cinco meses ap&#243;s o IAM. Um valor de p &lt;0,05 foi considerado para signific&#226;ncia estat&#237;stica nas compara&#231;&#245;es das vari&#225;veis.</p>
        </sec>
        <sec>
          <title>Resultados</title>
          <p> N&#227;o foram encontradas diferen&#231;as nas an&#225;lises dos par&#226;metros de strain circunferencial, radial ou longitudinal do VE entre os grupos ap&#243;s o per&#237;odo de treinamento. Ap&#243;s o programa, a an&#225;lise da mec&#226;nica de tor&#231;&#227;o revelou uma redu&#231;&#227;o na rota&#231;&#227;o basal do VE no grupo TREINO em compara&#231;&#227;o ao grupo CONTROLE (5,9&#177;2,3 vs. 7,5&#177;2.9o; p=0,03), bem como na velocidade rotacional basal (53,6&#177;18,4 vs. 68,8&#177; 22,1 &#186;/s; p=0,01), velocidade de twist (127,4&#177;32,2 vs. 149,9&#177;35,9 &#186;/s; p=0,02) e na tor&#231;&#227;o (2,4&#177;0,4 vs. 2,8&#177;0, &#186;/cm; p=0,02).</p>
        </sec>
        <sec>
          <title>Conclus&#245;es</title>
          <p> A atividade f&#237;sica n&#227;o causou melhora significativa nos par&#226;metros de deforma&#231;&#227;o longitudinal, radial ou circunferencial do VE. No entanto, o exerc&#237;cio teve um impacto significativo sobre a mec&#226;nica de tor&#231;&#227;o do VE, que consistiu em uma redu&#231;&#227;o na rota&#231;&#227;o basal, na velocidade de twist, na tor&#231;&#227;o, e na velocidade de tor&#231;&#227;o, que pode ser interpretada como uma &#8220;reserva&#8221; de tor&#231;&#227;o ventricular nessa popula&#231;&#227;o.</p>

      <p>Os benef&#237;cios do exerc&#237;cio ap&#243;s um infarto agudo do mioc&#225;rdio (IAM) t&#234;m sido investigados por muitos anos, e a maioria das publica&#231;&#245;es demonstraram um melhor progn&#243;stico com essa pr&#225;tica.<xref ref-type="bibr" rid="B1">1</xref> - <xref ref-type="bibr" rid="B8">8</xref> A atividade f&#237;sica ap&#243;s um IAM tem sido relacionada &#224; preven&#231;&#227;o do remodelamento card&#237;aco e da progress&#227;o da insufici&#234;ncia card&#237;aca, e est&#225; associada com aumento da capacidade funcional.<xref ref-type="bibr" rid="B4">4</xref> , <xref ref-type="bibr" rid="B9">9</xref> - <xref ref-type="bibr" rid="B13">13</xref> Esses fatores est&#227;o associados a um menor risco de IAM e redu&#231;&#227;o na mortalidade e na interna&#231;&#227;o por todas as causas.<xref ref-type="bibr" rid="B14">14</xref> , <xref ref-type="bibr" rid="B15">15</xref></p>
      <p>Tais benef&#237;cios t&#234;m sido demonstrado em casos de IAM com maior envolvimento contr&#225;til do ventr&#237;culo esquerdo (VE), marcado por disfun&#231;&#227;o sist&#243;lica moderada e importante [fra&#231;&#227;o de eje&#231;&#227;o do ventr&#237;culo esquerdo (FEVE) &#8804; 40% e &#8804; 30%, respectivamente].<xref ref-type="bibr" rid="B16">16</xref> - <xref ref-type="bibr" rid="B23">23</xref> Contudo, com a maior dissemina&#231;&#227;o de informa&#231;&#245;es quanto ao reconhecimento dos sintomas &#8220;de alerta&#8221;, maior rapidez no atendimento, evolu&#231;&#227;o das terapias empregadas, tem-se observado um menor comprometimento ap&#243;s o IAM.<xref ref-type="bibr" rid="B9">9</xref> Assim, a cardiomiopatia isqu&#234;mica com fun&#231;&#227;o sist&#243;lica do VE preservada ou disfun&#231;&#227;o leve (FEVE 41-51% em homens e 41-53% em mulheres) tem se tornado mais comum na popula&#231;&#227;o geral.<xref ref-type="bibr" rid="B9">9</xref> Nesse contexto, pouco se conhece sobre os benef&#237;cios do exerc&#237;cio sobre o remodelamento do VE e os efeitos sobre os mecanismos de contra&#231;&#227;o do VE nessa popula&#231;&#227;o.</p>
      <p>A ecocardiografia com speckle tracking pode realizar um estudo abrangente da mec&#226;nica da contra&#231;&#227;o do VE, caracterizada por um encurtamento longitudinal da base para o &#225;pice, associado a rota&#231;&#245;es segmentares e tor&#231;&#227;o ventricular.<xref ref-type="bibr" rid="B24">24</xref> Essa an&#225;lise fornece dados importantes que podem ser acrescidas &#224; determina&#231;&#227;o da FEVE.<xref ref-type="bibr" rid="B24">24</xref> - <xref ref-type="bibr" rid="B30">30</xref></p>
      <p>O presente estudo tem como objetivo testar a hip&#243;tese de que a reabilita&#231;&#227;o card&#237;aca, promovida por um programa supervisionado de exerc&#237;cios, teria um impacto sobre a mec&#226;nica da contra&#231;&#227;o do VE em pacientes com IAM n&#227;o complicado.</p>

        <title>Delineamento e popula&#231;&#227;o do estudo</title>
        <p>Este foi um estudo prospectivo, longitudinal, randomizado e controlado. Pacientes com um IAM n&#227;o complicado, admitidos na Unidade de Doen&#231;a Coronariana Aguda do Instituto do Cora&#231;&#227;o da Faculdade de Medicina da Universidade de S&#227;o Paulo (InCor/HCFMUSP), que concordaram em participar do programa, foram separados aleatoriamente em dois grupos (TREINO e CONTROLE), na propor&#231;&#227;o de 1:1 de acordo com o seguinte protocolo: Tempo 0: durante interna&#231;&#227;o, todos os pacientes, ap&#243;s devidas explica&#231;&#245;es, assinaram um termo de consentimento para inclus&#227;o no estudo; tempo 1: todos os participantes retornaram um m&#234;s ap&#243;s o IAM e foram submetidos a um ecocardiograma e a um teste cardiopulmonar (TCP). Em seguida, os participantes alocados no grupo TREINO foram inclu&#237;dos em um programa de treino f&#237;sico supervisionado, duas vezes por semana, por quatro meses, no Laborat&#243;rio de Reabilita&#231;&#227;o, e os indiv&#237;duos alocados no grupo CONTROLE receberam recomenda&#231;&#245;es usuais para a pr&#225;tica da atividade f&#237;sica na resid&#234;ncia. Em resumo, os pacientes foram orientados &#224; pr&#225;tica de exerc&#237;cios aer&#243;bicos de intensidade leve a moderada, por no m&#237;nimo 30 minutos, pelo menos tr&#234;s vezes por semana nos primeiros dois meses. No terceiro m&#234;s, os participantes foram orientados a aumentar a intensidade do exerc&#237;cio para moderada, tr&#234;s a cinco vezes por semana, 30 a 60 minutos. N&#227;o foi realizado monitoramento espec&#237;fico. O programa de treinamento supervisionado consistiu em: cinco minutos de exerc&#237;cio de alongamento, 40 minutos em uma bicicleta ergom&#233;trica, dez minutos de exerc&#237;cios de fortalecimento, e cinco minutos de resfriamento com exerc&#237;cios de alongamento. A intensidade do exerc&#237;cio foi estabelecida por n&#237;veis de frequ&#234;ncia card&#237;aca correspondentes ao limitar anaer&#243;bico e ao ponto de compensa&#231;&#227;o respirat&#243;ria. Tempo 2: ao final do quarto m&#234;s (quinto m&#234;s ap&#243;s o evento de IAM), todos os pacientes repetiram o ecocardiograma e o TCP ( <ext-link ext-link-type="uri" xlink:href="http://abccardiol.org/supplementary-material/2023/12004/2022-0185_AO_Supplementary%20Data%20I.pdf">material suplementar I</ext-link> ).</p>
        <p>Os crit&#233;rios de inclus&#227;o foram: idade acima de 18 anos, interna&#231;&#227;o por IAM espont&#226;neo com ou sem eleva&#231;&#227;o do segmento ST, estabelecido de acordo com a terceira defini&#231;&#227;o universal de IAM;<xref ref-type="bibr" rid="B31">31</xref> pacientes est&#225;veis clinicamente e hemodinamicamente; FEVE &gt; 0,40 e classes Killip I ou II. Os crit&#233;rios de exclus&#227;o foram: qualquer condi&#231;&#227;o que contraindicava atividade f&#237;sica, praticantes de atividade f&#237;sica regular antes do evento (confirmado por entrevista para a inclus&#227;o); FEVE &#8804; 40%; classe Killip III ou IV; ritmo card&#237;aco irregular (tais como fibrila&#231;&#227;o atrial, contra&#231;&#245;es atriais ou ventriculares prematuras frequentes); janela ecocardiogr&#225;fica limitada para an&#225;lise.</p>
        <p>O presente estudo foi conduzido de acordo com a Declara&#231;&#227;o de Helsinki e aprovado pelo comit&#234; de &#233;tica e pelo comit&#234; cient&#237;fico da institui&#231;&#227;o. Todos os pacientes assinaram o termo de consentimento antes de serem inclu&#237;dos no estudo.</p>

      <title>Discuss&#227;o</title>
      <p>Quanto &#224;s deforma&#231;&#245;es longitudinal, circunferencial e radial do mioc&#225;rdio, em geral, o grupo TREINO n&#227;o apresentou um desempenho superior de contra&#231;&#227;o em compara&#231;&#227;o ao grupo CONTROLE at&#233; quatro meses ap&#243;s o IAM. Embora o grupo TREINO tenha mostrado um aumento significativo na dura&#231;&#227;o do exerc&#237;cio, carga m&#225;xima alcan&#231;ada e pico de VO2 ap&#243;s o per&#237;odo de treinamento, n&#227;o foi observado aumento na mec&#226;nica longitudinal ou transversal do VE. Contudo, identificamos um resultado muito interessante em rela&#231;&#227;o &#224; mec&#226;nica de tor&#231;&#227;o. Comparativamente ao grupo controle, o grupo TREINO mostrou valores significativamente mais baixos de rota&#231;&#227;o e de velocidade de rota&#231;&#227;o dos segmentos basais do VE, bem como valores mais baixos de tor&#231;&#227;o, e de velocidade de twist e de tor&#231;&#227;o ap&#243;s as 16 semanas de treinamento. McGregor et al.<xref ref-type="bibr" rid="B42">42</xref> observaram resultados similares em seu elegante estudo explorat&#243;rio.<xref ref-type="bibr" rid="B42">42</xref> Esses autores tamb&#233;m descreveram uma redu&#231;&#227;o no twist e na velocidade de twist ap&#243;s 10 semanas de sess&#245;es de treinamento f&#237;sico, duas vezes por semana, em uma popula&#231;&#227;o similar que sofreu um IAM e manteve a fun&#231;&#227;o ventricular esquerda preservada (FEVE &gt; 50%). Em seu estudo, esse resultado final relacionado ao twist do VE foi associado a uma redu&#231;&#227;o tanto na rota&#231;&#227;o basal como na rota&#231;&#227;o apical. Finalmente, similar ao nosso estudo, os autores n&#227;o encontraram um impacto positivo significativo do exerc&#237;cio sobre o strain do VE (longitudinal, circunferencial ou radial).</p>
      <p>Extrapolando para atletas de alto rendimento, apesar de controv&#233;rsias nos achados, os estudos apontam para um impacto real final e comum do exerc&#237;cio sobre a mec&#226;nica de tor&#231;&#227;o do VE. St&#246;hr et al. descreveram uma redu&#231;&#227;o da rota&#231;&#227;o apical e do twist do VE em indiv&#237;duos com alta capacidade aer&#243;bica.<xref ref-type="bibr" rid="B43">43</xref> O mesmo foi observado por Nottin et al.,<xref ref-type="bibr" rid="B44">44</xref> estudando ciclistas de elite, e Z&#243;calo et al.<xref ref-type="bibr" rid="B45">45</xref> avaliando jogadores de futebol profissionais. Foi descrita uma redu&#231;&#227;o nas velocidades de rota&#231;&#227;o apical e basal do VE, e na velocidade de tor&#231;&#227;o. Weiner et al.<xref ref-type="bibr" rid="B46">46</xref> tamb&#233;m descreveram achados interessantes com atletas de remo de competi&#231;&#227;o. Em um programa de atividade f&#237;sica de alto n&#237;vel, os autores descreveram um &#8220;fen&#244;meno f&#225;sico&#8221;, compreendendo uma fase aguda de aumento no twist do VE, seguido de uma redu&#231;&#227;o subsequente cr&#244;nica nesse par&#226;metro.<xref ref-type="bibr" rid="B46">46</xref> Com base nesse fato, &#233; v&#225;lido supor que um maior aumento no twist do VE durante o exerc&#237;cio pode representar maior efici&#234;ncia sist&#243;lica nesses indiv&#237;duos. Esse desfecho pode ser interpretado como uma &#8220;reserva&#8221; de tor&#231;&#227;o do VE em atletas e em indiv&#237;duos que se exercitaram ap&#243;s IAM, representando uma mec&#226;nica fisiologicamente mais eficiente, o que levaria a um aumento na capacidade funcional, e possivelmente a uma melhor evolu&#231;&#227;o cl&#237;nica.</p>
      <p>O delineamento mais aceito da arquitetura do m&#250;sculo card&#237;aco &#233; o proposto por Torrent-Guasp et al.,<xref ref-type="bibr" rid="B47">47</xref> , <xref ref-type="bibr" rid="B48">48</xref> que descreveram o cora&#231;&#227;o como uma banda muscular &#8220;dobrada&#8221; em dupla h&#233;lice. Em termos de gasto energ&#233;tico, tal arquitetura prov&#234; uma forma mais eficiente de contra&#231;&#227;o, bem como uma distribui&#231;&#227;o mais homog&#234;nea do estresse da parede, com menos consumo de oxig&#234;nio pelo mioc&#225;rdio, em compara&#231;&#227;o a uma simples deforma&#231;&#227;o da cavidade do VE.<xref ref-type="bibr" rid="B49">49</xref></p>
      <p>O efeito do exerc&#237;cio sobre o strain do VE ainda n&#227;o est&#225; claro. Em uma revis&#227;o sistem&#225;tica e meta-an&#225;lise, Murray et al.<xref ref-type="bibr" rid="B50">50</xref> estudaram o efeito do exerc&#237;cio sobre o strain longitudinal global do VE em uma ampla gama de popula&#231;&#245;es sadias, em risco de doen&#231;a cardiovascular, e com doen&#231;as cr&#244;nicas. Naqueles com doen&#231;a card&#237;aca, observou-se um efeito moderado do exerc&#237;cio em compara&#231;&#227;o a controles n&#227;o praticantes de atividade f&#237;sica, e n&#227;o houve efeito significativo do exerc&#237;cio nos indiv&#237;duos sadios ou em risco de doen&#231;a cardiovascular em compara&#231;&#227;o aos controles que n&#227;o praticaram atividade f&#237;sica. Similar ao nosso estudo, McGregor et al.<xref ref-type="bibr" rid="B42">42</xref> n&#227;o encontraram um impacto positivo do exerc&#237;cio sobre o strain longitudinal. Assim, apesar da boa acur&#225;cia e dados mais robustos na literatura sobre o strain longitudinal, o twist do VE poderia ser um par&#226;metro mais sens&#237;vel para avaliar a resposta sist&#243;lica global do VE ao exerc&#237;cio f&#237;sico nessa popula&#231;&#227;o.</p>
      <sec>
        <title>Limita&#231;&#245;es</title>
        <p>Primeiro, o tamanho relativamente pequeno da amostra, a curta dura&#231;&#227;o do programa de exerc&#237;cio, o pequeno n&#250;mero de sess&#245;es de treinamento para os pacientes no grupo TREINO (duas vezes por semana), e a baixa ades&#227;o de alguns pacientes ao programa pode ter diminu&#237;do o poder do estudo em demonstrar poss&#237;veis diferen&#231;as entre grupos. Os indiv&#237;duos eram incentivados a participar das sess&#245;es e, aqueles que compareceram a menos de uma sess&#227;o por semana, ou pararam as sess&#245;es, foram exclu&#237;dos. Um tamanho amostral de 72 participantes, como calculado, n&#227;o foi alcan&#231;ado, o que consiste em mais uma limita&#231;&#227;o. Ainda, al&#233;m da pr&#225;tica de exerc&#237;cios, n&#227;o foi dada nenhuma outra orienta&#231;&#227;o relacionada &#224; sa&#250;de, como suporte diet&#233;tico ou psicol&#243;gico, o que poderia ter afetado o resultado final. N&#227;o foram coletados dados sobre s&#237;ndromes coron&#225;rias cr&#244;nicas ou doen&#231;a arterial perif&#233;rica, que poderiam ter tido um impacto sobre o exerc&#237;cio. Por fim, nem todos os dados derivados do TCP como dados sobre a taxa de troca respirat&#243;ria, foram abordados neste estudo, o que poderia ter revelado diferen&#231;as entre os grupos.</p>
        <p>Outro aspecto importante &#233; a subjetividade do ecocardiograma, que pode levar ao vi&#233;s de quantifica&#231;&#227;o. Pequenas varia&#231;&#245;es na aquisi&#231;&#227;o no n&#237;vel apical do VE, que n&#227;o possui um marcador anat&#244;mico, podem resultar na distor&#231;&#227;o dos valores. Neste sentido, criamos outro crit&#233;rio para confirmar uma aquisi&#231;&#227;o correta, que consistiu na visualiza&#231;&#227;o de pelo menos uma tend&#234;ncia a uma rota&#231;&#227;o anti-hor&#225;rio dos segmentos, o que era fisiologicamente esperado. Finalmente, &#233; importante lembrar que o examinador era cego para a aloca&#231;&#227;o dos pacientes nos grupos.</p>

      <title>Conclus&#227;o</title>
      <p>Em nosso estudo, o exerc&#237;cio n&#227;o causou melhora significativa nos par&#226;metros de deforma&#231;&#227;o longitudinal, radial ou circunferencial. Contudo, o exerc&#237;cio foi associado a uma redu&#231;&#227;o na rota&#231;&#227;o basal, na velocidade de twist, na tor&#231;&#227;o e na velocidade de tor&#231;&#227;o. Esse desfecho pode ser interpretado como uma &#8220;reserva&#8221; da tor&#231;&#227;o do VE em indiv&#237;duos que se exercitaram ap&#243;s o IAM, sugerindo uma mec&#226;nica de tor&#231;&#227;o fisiologicamente mais eficiente.</p>

          <p>Exercise plays a positive role in the course of the ischemic heart disease, enhancing functional capacity and preventing ventricular remodeling.</p>
        </sec>
        <sec>
          <title>Objective</title>
          <p>To investigate the impact of exercise on left ventricular (LV) contraction mechanics after an uncomplicated acute myocardial infarction (AMI).</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p>A total of 53 patients was included, 27 of whom were randomized to a supervised training program (TRAINING group), and 26 to a CONTROL group, who received usual recommendations on physical exercise after AMI. All patients underwent cardiopulmonary stress testing and a speckle tracking echocardiography to measure several parameters of LV contraction mechanics at one month and five months after AMI. A p value &lt; 0.05 was considered statistically significant for the comparisons of the variables.</p>
        </sec>
        <sec>
          <title>Results</title>
          <p>No significant difference were found in the analysis of LV longitudinal, radial and circumferential strain parameters between groups after the training period. After the training program, analysis of torsional mechanics demonstrated a reduction in the LV basal rotation in the TRAINING group in comparison to the CONTROL group (5.9&#177;2.3 vs. 7.5&#177;2.9o; p=0.03), and in the basal rotational velocity (53.6&#177;18.4 vs.68.8&#177;22.1 &#186;/s; p=0.01), twist velocity (127.4&#177;32.2 vs. 149.9&#177;35.9 &#186;/s; p=0.02) and torsion (2.4&#177;0.4 vs. 2.8&#177;0.8 &#186;/cm; p=0.02).</p>
        </sec>
        <sec>
          <title>Conclusions</title>
          <p>Physical activity did not cause a significant improvement in LV longitudinal, radial and circumferential deformation parameters. However, the exercise had a significant impact on the LV torsional mechanics, consisting of a reduction in basal rotation, twist velocity, torsion and torsional velocity which can be interpreted as a ventricular &#8220;torsion reserve&#8221; in this population.</p>

        <title>Introduction</title>
        <p>The benefits of exercise after acute myocardial infarction (AMI) have been investigated for several years and most publications have demonstrated a better prognosis related to this practice.<xref ref-type="bibr" rid="B1">1</xref> - <xref ref-type="bibr" rid="B8">8</xref> Physical activity post-AMI has been related to prevention of cardiac remodeling and of heart failure progression, and associated with increased functional capacity.<xref ref-type="bibr" rid="B4">4</xref> , <xref ref-type="bibr" rid="B9">9</xref> - <xref ref-type="bibr" rid="B13">13</xref> These are linked to lower risk of AMI and reduction in all-cause hospitalization and mortality.<xref ref-type="bibr" rid="B14">14</xref> , <xref ref-type="bibr" rid="B15">15</xref></p>
        <p>Benefits have been demonstrated in cases of AMI with greater impairment of LV contraction, causing moderate and severe systolic dysfunction (left ventricular ejection fraction [LVEF] &#8804; 40% and &#8804; 30%, respectively).<xref ref-type="bibr" rid="B16">16</xref> - <xref ref-type="bibr" rid="B23">23</xref> However, with broader dissemination of information regarding the recognition of AMI &#8220;alarm symptoms&#8221;, greater promptness of care, evolution of the therapies employed, less impairment of cardiac contraction after AMI has become more frequent.<xref ref-type="bibr" rid="B9">9</xref> So, ischemic cardiomyopathy with preserved left ventricular (LV) systolic function or mild dysfunction (LVEF 41-51% in men and 41-53% in women) has become more frequent in the general population.<xref ref-type="bibr" rid="B9">9</xref> In this context, little is known about the overall benefit of exercise on LV remodeling and the effects on the mechanisms of left ventricular contraction in this population.</p>
        <p>Speckle tracking echocardiography (STE) can perform a comprehensive study of LV contraction mechanics, which is characterized by longitudinal apex-to-base shortening in association to segmental rotations and ventricular torsion.<xref ref-type="bibr" rid="B24">24</xref> This analysis adds important data to the simple determination of LVEF.<xref ref-type="bibr" rid="B24">24</xref> - <xref ref-type="bibr" rid="B30">30</xref></p>
        <p>The present study aims to test the hypothesis that cardiovascular rehabilitation, promoted by a supervised exercise program, would have a positive impact on the mechanics of LV contraction in patients with uncomplicated AMI.</p>

          <title>Study design and population</title>
          <p>This was a prospective, longitudinal, randomized and controlled study. Patients with uncomplicated AMI admitted to the Acute Coronary Disease Unit of the Heart Institute of the University of Sao Paulo Medical School (InCor/HCFMUSP), who agreed to participate in the study, were randomized to two groups (TRAINING and CONTROL), in the proportion 1:1 according to the following protocol: Time 0: still hospitalized, all patients, after due explanations, signed the informed consent for inclusion in the study. Time 1: all participants returned one month after AMI and were submitted to an echocardiogram and cardiopulmonary exercising testing (CPX). After this, participants randomized to the TRAINING group were included in a supervised physical training program, twice a week, for four months, at the Rehabilitation Lab, while individuals randomized to the CONTROL group received usual recommendations for physical activity at home. Briefly, patients were instructed to engage in aerobic exercises of low-to-moderate intensity, with a minimum duration of 30 minutes, at least three times a week in the first two months. In the third month, they were instructed to increase the intensity to a moderate level 3-5 times a week, 30 to 60 minutes. No specific monitoring was carried out. Supervised training program consisted of two 60-minute exercise sessions per week. Each exercise session consisted of 5 minutes of stretching exercises, 40 minutes of cycling on a stationary bicycle, 10 minutes of local strengthening exercises, and five minutes of cool down with stretching exercises. The exercise intensity was established by heart rate levels that corresponded to the anaerobic threshold and respiratory compensation point. Time 2: at the end of the fourth month (fifth month after the AMI event), all patients repeated the echocardiogram and CPX (<ext-link ext-link-type="uri" xlink:href="http://abccardiol.org/supplementary-material/2023/12004/2022-0185_AO_Supplementary%20Data%20I.pdf">supplementary data I</ext-link>).</p>
          <p>Inclusion criteria were: age over 18; hospitalization for spontaneous AMI with or without ST-segment elevation, established according to the 3rdUniversal Definition of AMI;<xref ref-type="bibr" rid="B31">31</xref> clinically and hemodynamically stable patients; LVEF &gt; 0.40 and Killip classes I or II. The exclusion criteria were: any condition that contraindicated physical activity; regular physical activity practitioners prior to the event (confirmed by interview for inclusion); LVEF &#8804; 40%; Killip classes III or IV; irregular heart rhythm (such as atrial fibrillation, frequent premature atrial or ventricular contractions); limited echocardiographic window for analysis.</p>
          <p>The present study was conducted in accordance with the Declaration of Helsinki and was approved by the institutional ethics and scientific committees. All the patients signed the informed consent form before inclusion in the study.</p>

          <title>Conventional echocardiogram</title>
          <p>The echocardiogram was performed by a single experienced operator blind to the allocation group. Commercially available echocardiographic equipment (Vivid E9; GE Medical Systems, Milwaukee, WI, USA) was used, equipped with linear broadband transducers with a frequency of 5-2 MHz. Measurements, and analysis of valve flow and LV diastolic function were performed according to current guidelines.<xref ref-type="bibr" rid="B32">32</xref> - <xref ref-type="bibr" rid="B35">35</xref></p>

          <title>STE image acquisition and analysis</title>
          <p>For image acquisition for speckle tracking analysis, the machine was adjusted to acquire three cardiac cycles, with frame rate between 40-80 frames/s. Images were obtained in the parasternal short axis (PSAX) view in its three main levels: basal (mitral valve), mid (papillary muscles) and apical. LV apical window was composed by: longitudinal (APLAX), four (A4C) and two chambers (A2C) views.</p>
          <p>Off-line analyses were performed using the EchoPAC software, version v20.1 (GE Medical Systems, Milwaukee, WI, USA). The parameters evaluated were: strain (&#949;,%) and strain rate (SR) (&#949;&#8217;,s-1); maximum basal and apical rotations (LVrot,o) and their peak absolute rotational velocities (LVrot-v,o/s), LV twist (LVtw,o) and torsion (LVtor,o/cm), and their velocities (LVtw-v,o/s; LVtor-v,o/s.cm). LV twist was calculated as the absolute difference between peak systolic apical and basal rotations (LVtw = apical LVrot - basal LVrot) and torsion as the twist normalized to LV longitudinal length.<xref ref-type="bibr" rid="B24">24</xref> By convention, baseline rotation values were negative, and apical, positive ( <xref ref-type="fig" rid="f02001">Figure 1</xref> ).<xref ref-type="bibr" rid="B24">24</xref> In this paper, the negative strain values will be expressed as modulus (positive) for a better understanding.</p>

          <p>The CPX was performed on an electromagnetic cycle ergometer (Medfit 400L, Medical Fitness Equipment, Maarn, Netherlands), following a ramp protocol, with a speed of 60 revolutions per minute (rpm) and workload increments from 10w to 20w per minute, until achieving physical exhaustion. Participants were connected to a ventilator (SensorMedics Corp, CA, USA), and pulmonary ventilation, oxygen (O2) and carbon dioxide (CO2) concentrations were measured with calculation of oxygen consumption (VO2) and CO2production. When the patient achieved exhaustion, the anaerobic threshold and respiratory compensation point (i.e. points when blood lactate starts to increase rapidly due to high exercise intensity and tissue anaerobiosis) were determined, both used to prescribe the intensity of physical training.<xref ref-type="bibr" rid="B36">36</xref></p>

Categorical data were presented as numbers and percentages. To calculate the sample size, the mean value of global longitudinal strain of 15.9% (&#177; 2.3) was considered.<xref ref-type="bibr" rid="B37">37</xref> For an expected gain of 10% in longitudinal strain in the TRAINING group, considering the effect size of this intervention (d = 0.65), a power (1 - &#946;) 0f 80% to detect differences and alpha index of 0.05, it was necessary to include 36 patients in each group (a sample size of 72 participants). Randomization of the sample was made through the website &#8220;randomization.com&#8221;. Comparisons of continuous variables between groups were performed using the Student&#8217;s t-test (in case of normal distribution) or Mann-Whitney test (in case of non-normal distribution) for independent samples. For categorical variables, the Chi-square test (&#967;2) or Fisher&#8217;s exact test was used, as appropriate. The within-group comparison of means was performed using the paired Student&#8217;s t test. Finally, comparison of the &#8220;delta&#8221; (&#916;) of values obtained from the follow-up of each group was done by the unpaired Student&#8217;s t test.</p>

        <title>Discussion</title>
        <p>This present study investigated the hypothesis that cardiovascular rehabilitation by a supervised exercise program would have a positive impact on the LV contraction mechanics in a population after an uncomplicated AMI. There is a lack of knowledge, with few studies investigating the real benefit of exercise on LV remodeling, with analysis of LV contraction mechanics in this population. To date, to the best of our knowledge, this is the first study to investigate this hypothesis in such a detailed way, considering the large number of LV contraction parameters including those of LV systolic mechanics analyzed in our study. In this work, we used the speckle tracking to further investigate LV systolic function. This technique has being proved superior to LVEF in myocardial contraction assessment, as it is less susceptible to hemodynamic conditions, more reproducible,<xref ref-type="bibr" rid="B38">38</xref> and also a better prognostic factor for cardiovascular events in a wide spectrum of cardiac diseases.<xref ref-type="bibr" rid="B39">39</xref> - <xref ref-type="bibr" rid="B41">41</xref> This is particularly important after a myocardial ischemic event where an accurate LV systolic analysis is essential.</p>
        <p>Regarding longitudinal, circumferential and radial myocardial deformations, in general, the TRAINING group did not have a superior contraction performance over the CONTROL group up to 4 months post-AMI. Although the TRAINING group showed a significant increase in exercise duration, maximum workload achieved and peak VO2after the training period, no increment in longitudinal or transversal LV mechanics was observed. However, we identified a very interesting finding in relation to the LV torsional mechanics. Compared to the CONTROL group, the TRAINING group showed significantly lower values of rotation and rotational velocity of the LV basal segments, as well as lower values of twist velocity, torsion and torsional velocity after the 16-week training period. McGregor et al.<xref ref-type="bibr" rid="B42">42</xref> observed similar results in their elegant exploratory study.<xref ref-type="bibr" rid="B42">42</xref> These authors also described a reduction in LV twist and twist velocity after 10-weeks of physical training sessions, twice a week, in a similar population who had an AMI and still maintained a preserved LV function (LVEF &gt; 50%). In their study, this final result on LV twist was linked to a reduction in both basal and apical rotations. Finally, as similar to our study, they did not find a significant positive impact of exercise on LV strain (longitudinal, circumferential or radial).</p>
        <p>Extrapolating to highly-trained athletes, despite some contrasts in the findings, studies point to a common and final real impact of exercise on LV torsional mechanics. St&#246;hr et al. described a reduction of apical rotation and LV twist in individuals with high aerobic fitness.<xref ref-type="bibr" rid="B43">43</xref> The same was found by Nottin et al.<xref ref-type="bibr" rid="B44">44</xref> studying elite cyclists, and Z&#243;calo et al.<xref ref-type="bibr" rid="B45">45</xref> assessing professional soccer players. A reduction of LV basal and apical rotations rates, and torsional rate, were described. Weiner et al.<xref ref-type="bibr" rid="B46">46</xref> also showed interesting findings with competitive rowing athletes. They described a so called &#8220;phasic phenomenon&#8221; in a program of high level physical activity, comprised by an acute phase of augmentation of LV twist, followed by a subsequent and chronic reduction of this parameter.<xref ref-type="bibr" rid="B46">46</xref> Based on this, one may postulate that a greater enhancement of LV twist during exercise may represent greater systolic efficiency systole in these individuals. This outcome may be interpreted as a &#8220;LV torsional reserve&#8221; in athletes and in individuals who exercised post-AMI, representing a physiologically more efficient torsional mechanics, which could lead to an increment in functional capacity, and perhaps to a better clinical course.</p>
        <p>The most accepted design of the myocardial muscle architecture is that proposed by Torrent-Guasp et al.,<xref ref-type="bibr" rid="B47">47</xref> , <xref ref-type="bibr" rid="B48">48</xref> who described the heart as a muscle band &#8220;folded&#8221; in double helix. In terms of energy expenditure, it provides a more efficient form of contraction, and a more homogeneous distribution of cavity wall stress, with less myocardial oxygen consumption, compared to a simple radial LV cavity deformation.<xref ref-type="bibr" rid="B49">49</xref> , <xref ref-type="bibr" rid="B50">50</xref></p>
        <p>The effect of exercise on LV strain is still unclear. In a systematic review and meta-analysis, Murray et al.51studied the effect exercise on LV global longitudinal strain across a wide range of healthy, at risk of cardiovascular disease, and chronic diseased populations. A moderate effect of exercise was observed in individuals with heart disease compared to non-exercising controls, and no significant effect of exercise was observed in individuals at risk or healthy populations. As in our study, McGregor et al.<xref ref-type="bibr" rid="B42">42</xref> did not find a positive impact of exercise on longitudinal strain. So, despite the good accuracy and more robust data in the literature on longitudinal strain, LV twist could be a more sensitive parameter for evaluating LV global systolic response to physical exercise in this population.</p>

          <title>Limitations</title>
          <p>First, the relatively small sample size, the short duration of the exercise program, the small number of training sessions for patients in the TRAINING group (twice a week), and the low adherence of some patients to the program may have decreased the power of the study to demonstrate possible intergroup differences. Subjects were encouraged to attend the sessions, and those who attended less than one session per week, or stopped the sessions, were excluded. A sample size of 72 participants, as calculated, was not achieved, and this is also a limitation. In addition, besides exercise, no other health instructions, such as nutritional or psychological support were given, which could have affected the final result. No data concerning chronic coronary syndromes or peripheral artery disease were collected, which may have had an impact on exercise. Finally, not all data derived from the CPX, including data on the respiratory exchange ratio, were addressed in this study - these could have revealed some differences between groups.</p>
          <p>Another important point is the subjectivity of the echocardiogram which can lead to quantification bias. Small variations in the acquisition at LV apical level, which do not have an anatomical marker, can result in distorted values. Images were acquired from the apical short-axis view when the visibility of the myocardial segments was clear. Additionally, we created another criterion to ratify the correct acquisition, which was the visualization of at least a trend of a counterclockwise rotation of these segments, which would be physiologically expected. Finally, it is important to remind that the examiner was blinded to the patient&#8217;s group allocation.</p>

        <title>Conclusion</title>
        <p>In our study, exercise led to no significant improvement in the LV longitudinal, radial and circumferential myocardial deformation parameters. However, exercise was associated to a reduced basal rotation, twist velocity, torsion and torsional velocity. This outcome may be interpreted as a &#8220;LV torsional reserve&#8221; in individuals who exercised in post-AMI, suggesting a physiologically more efficient torsional mechanics.</p>
