          <title>M&#233;todos</title>
          <p> Os pacientes inclu&#237;dos foram submetidos &#224; ICP de OTC em centros participantes do LATAM CTO Registry, um registro multic&#234;ntrico latino-americano dedicado &#224; coleta prospectiva desses dados. Os crit&#233;rios de inclus&#227;o foram procedimentos realizados no Brasil, idade acima de 18 anos e presen&#231;a de OTC com tentativa de ICP. A defini&#231;&#227;o de OTC foi les&#227;o de 100% em uma art&#233;ria coron&#225;ria epic&#225;rdica, conhecida ou estimada como tendo pelo menos 3 meses de evolu&#231;&#227;o.</p>

        <sec>
          <title>Resultados</title>
          <p> Foram inclu&#237;dos dados de 1.196 ICPs de OTC. Os procedimentos foram realizados principalmente para controle da angina (85%) e/ou tratamento de uma grande &#225;rea isqu&#234;mica (24%). A taxa de sucesso t&#233;cnico foi de 84% e foi alcan&#231;ada com t&#233;cnicas de fios anter&#243;grados em 81%, dissec&#231;&#227;o/reentrada anter&#243;grada em 9% e retr&#243;grada em 10% dos procedimentos. Os eventos cardiovasculares adversos intra-hospitalares ocorreram em 2,3% dos casos, sendo a mortalidade de 0,75%.</p>

        <sec>
          <title>Conclus&#245;es</title>
          <p> As OTC podem ser tratadas no Brasil por interven&#231;&#227;o coron&#225;ria percut&#226;nea de forma efetiva e com baixas taxas de complica&#231;&#245;es. O desenvolvimento cient&#237;fico e tecnol&#243;gico observado nessa &#225;rea na &#250;ltima d&#233;cada reflete-se na pr&#225;tica cl&#237;nica de centros brasileiros dedicados a essa t&#233;cnica.</p>

      <title>Introdu&#231;&#227;o</title>
      <p>A oclus&#227;o coronariana cr&#244;nica (OTC) tem grande preval&#234;ncia, sendo encontrada em aproximadamente um em cada tr&#234;s pacientes submetidos &#224; angiografia coron&#225;ria diagn&#243;stica e podendo chegar at&#233; 50% em estudos de pacientes com cirurgia de revasculariza&#231;&#227;o do mioc&#225;rdio (CRM) pr&#233;via. A OTC &#233; uma das causas mais frequente de revasculariza&#231;&#227;o incompleta. As interven&#231;&#245;es coron&#225;rias percut&#226;neas (ICP) em OTC t&#234;m sido tradicionalmente associadas com menores taxas de sucesso e mais complica&#231;&#245;es quando comparadas &#224;s ICP em estenoses com fluxo anter&#243;grado. Esses aspectos est&#227;o relacionados &#224;s dificuldades t&#233;cnicas em cruzar a oclus&#227;o com o fio-guia, mas tamb&#233;m &#224; maior complexidade angiogr&#225;fica, ao maior perfil de risco e &#224; presen&#231;a de comorbidades.<xref ref-type="bibr" rid="B1">1</xref>-<xref ref-type="bibr" rid="B3">3</xref> Nos &#250;ltimos anos, tem sido observado um grande avan&#231;o nas t&#233;cnicas e nos dispositivos para realizar a recanaliza&#231;&#227;o de uma OTC, com taxas de sucesso pr&#243;ximas de 90% em registros internacionais de ICP em OTC.<xref ref-type="bibr" rid="B4">4</xref> No entanto, existem poucos dados contempor&#226;neos sobre as caracter&#237;sticas e resultados desses procedimentos na realidade brasileira. O objetivo deste estudo foi descrever as caracter&#237;sticas cl&#237;nicas e angiogr&#225;ficas, os aspectos dos procedimentos, as complica&#231;&#245;es e os desfechos cl&#237;nicos de pacientes com OTC contemporaneamente submetidos &#224; ICP no Brasil em centros dedicados &#224; ICP em OTC.</p>

        <title>Coleta e monitoramento dos dados</title>
        <p>Os dados foram inseridos em um registro latino-americano multic&#234;ntrico de ICP em OTC iniciado pelo grupo de investigadores, com o apoio da Sociedade Brasileira de Hemodin&#226;mica e Cardiologia Intervencionista. O banco de dados foi gerenciado pelo centro coordenador por meio da plataforma Research Electronic Data Capture (REDCap),<xref ref-type="bibr" rid="B7">7</xref> ferramenta aprovada pela Ag&#234;ncia Nacional de Vigil&#226;ncia Sanit&#225;ria (Anvisa).</p>
        <p>Todos os investigadores receberam um manual com instru&#231;&#245;es padronizadas para a inclus&#227;o dos dados nas planilhas eletr&#244;nicas. O material teve como foco os objetivos de cadastro, o esclarecimento do processo de coleta e o armazenamento dos dados (computadores, tablets e/ou celulares, de acordo com a demanda de cada centro participante). Os centros receberam suporte on-line e telef&#244;nico para d&#250;vidas de inclus&#227;o ou conclus&#227;o de casos e para feedbacks mensais para dados faltantes e valores discrepantes. Foram utilizadas regras internas de qualidade para aprimorar a qualidade do banco de dados por meio de an&#225;lises descritivas dos dados, que eram resumidas e enviadas no formato de relat&#243;rios mensais de monitoria para os centros participantes.</p>

        <title>Defini&#231;&#245;es</title>
        <p>A isquemia de grau moderado a severa foi definida como a presen&#231;a de defeito perfusional por cintilografia, ecocardiografia de estresse ou resson&#226;ncia magn&#233;tica igual ou superior a 10%. A calcifica&#231;&#227;o moderada/severa foi definida como um envolvimento de pelo menos 50% do vaso pela angiografia. A tortuosidade moderada/severa foi considerada quando eram observadas duas angula&#231;&#245;es de pelo menos 70&#186; ou uma angula&#231;&#227;o de pelo menos 90&#186; no vaso-alvo, no segmento proximal &#224; OTC. Os cotos proximal e distal foram definidos como brusco ou afilado. Os vasos colaterais foram classificados como &#250;teis para abordagem se considerados pelo operador como pass&#237;veis de cruzamento por um fio-guia e um microcateter e tamb&#233;m atrav&#233;s da classifica&#231;&#227;o de Werner.<xref ref-type="bibr" rid="B8">8</xref> As defini&#231;&#245;es dos escores preditores de sucesso e complexidade J-CTO, PROGRESS, CL e ORA eram realizadas automaticamente conforme a inclus&#227;o das informa&#231;&#245;es angiogr&#225;ficas e cl&#237;nicas necess&#225;rias para seu c&#225;lculo.<xref ref-type="bibr" rid="B9">9</xref>-<xref ref-type="bibr" rid="B12">12</xref></p>
        <p>As seguintes estrat&#233;gias foram consideradas para a realiza&#231;&#227;o dos procedimentos: (a) fios anter&#243;grados: consistiu na tentativa de cruzar diretamente o segmento oclu&#237;do com o uso de diferentes fios-guia, de forma progressiva ou n&#227;o; (b) dissec&#231;&#227;o e reentrada anter&#243;grada: foi definida como um procedimento por abordagem anter&#243;grada durante o qual o operador intencionalmente usou o espa&#231;o subintimal para superar parcial ou totalmente o segmento oclu&#237;do, com fios-guia ou dispositivos dedicados, reentrando no l&#250;men verdadeiro distalmente &#224; oclus&#227;o; e (c) procedimento retr&#243;grado: foi definido como uma tentativa de recanaliza&#231;&#227;o por um vaso colateral ou enxerto (oclu&#237;do ou n&#227;o) que irriga um segmento distal &#224; oclus&#227;o, podendo ser com t&#233;cnicas de fios intraplacas ou dissec&#231;&#227;o e reentrada. O sucesso t&#233;cnico foi definido como recanaliza&#231;&#227;o da OTC com implante de stent, fluxo final do Thrombolysis in Myocardial Infarction (TIMI) II/III e estenose residual menor que 30%. O sucesso do procedimento foi definido como a obten&#231;&#227;o do sucesso t&#233;cnico sem eventos cardiovasculares adversos maiores (ECAM).</p>

      <sec>
        <title>Desfechos cl&#237;nicos e complica&#231;&#245;es</title>
        <p>Definiu-se ECAM como a combina&#231;&#227;o de morte, infarto agudo do mioc&#225;rdio (IAM) e acidente vascular cerebral (AVC). Caracterizou-se IAM de acordo com a vers&#227;o mais recente da defini&#231;&#227;o universal de IAM.<xref ref-type="bibr" rid="B13">13</xref> O AVC foi definido como um novo d&#233;ficit neurol&#243;gico focal de in&#237;cio s&#250;bito e de causa irrevers&#237;vel presumivelmente cerebrovascular dentro de 24 horas e n&#227;o causado por qualquer outra causa facilmente identific&#225;vel.</p>
        <p>As complica&#231;&#245;es do procedimento inclu&#237;ram sangramento maior, perfura&#231;&#227;o coron&#225;ria, tamponamento card&#237;aco e revasculariza&#231;&#227;o de urg&#234;ncia com ICP ou CRM. Um sangramento maior foi definido como qualquer sangramento com redu&#231;&#227;o na hemoglobina &gt; 3 g/dL, transfus&#227;o sangu&#237;nea ou interven&#231;&#227;o cir&#250;rgica. A perfura&#231;&#227;o coron&#225;ria foi definida atrav&#233;s da classifica&#231;&#227;o de Ellis.<xref ref-type="bibr" rid="B14">14</xref> O tamponamento card&#237;aco foi definido como comprometimento hemodin&#226;mico causado por ac&#250;mulo agudo de sangue no espa&#231;o peric&#225;rdico. A revasculariza&#231;&#227;o de urg&#234;ncia foi definida como um procedimento realizado durante a interna&#231;&#227;o sem planejamento pr&#233;vio, para tratamento de angina e/ou isquemia recorrente.</p>

      <title>Discuss&#227;o</title>
      <p>Neste estudo, relatamos dados contempor&#226;neas de ICP em OTC realizadas por centros brasileiros participantes do Registro Latino-Americano de OTC, incluindo dados cl&#237;nicos e angiogr&#225;ficos, os aspectos do procedimento e os ECAM. H&#225; amplo relato de dados desses procedimentos provenientes de pa&#237;ses da Am&#233;rica do Norte, Europa Ocidental e Jap&#227;o,<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B15">15</xref>-<xref ref-type="bibr" rid="B18">18</xref> por&#233;m h&#225; uma escassez de informa&#231;&#245;es em nosso meio. Nessa an&#225;lise contempor&#226;nea representativa da pr&#225;tica brasileira, encontramos resultados encorajadores, com taxas de sucesso acima de 80% e baixas taxas de complica&#231;&#245;es e de ECAM. Tamb&#233;m foi relevante a demonstra&#231;&#227;o de que as principais indica&#231;&#245;es de realiza&#231;&#227;o das interven&#231;&#245;es, angina e tratamento de isquemia mioc&#225;rdica significativa, assim como o tratamento cl&#237;nico recebido pelos pacientes antes das interven&#231;&#245;es, estavam de acordo com as diretrizes.<xref ref-type="bibr" rid="B15">15</xref>,<xref ref-type="bibr" rid="B16">16</xref> A presente an&#225;lise &#233; relevante por se tratar do primeiro relato da pr&#225;tica m&#233;dica de centros de refer&#234;ncia brasileiros no tratamento da ICP de OTC com as t&#233;cnicas contempor&#226;neas e a utiliza&#231;&#227;o das abordagens recomendadas pelos consensos internacionais.<xref ref-type="bibr" rid="B17">17</xref></p>
      <p>As indica&#231;&#245;es para realiza&#231;&#227;o de ICP de OTC foram recentemente questionadas por ensaios cl&#237;nicos randomizados que n&#227;o mostraram benef&#237;cio desses procedimentos em redu&#231;&#227;o de eventos cardiovasculares ou melhora da fun&#231;&#227;o ventricular, embora diversas limita&#231;&#245;es metodol&#243;gicas tenham sido observadas nesses estudos.<xref ref-type="bibr" rid="B19">19</xref>-<xref ref-type="bibr" rid="B21">21</xref> Por outro lado, a ICP em OTC melhorou significativamente os sintomas e a qualidade de vida quando comparada &#224; terapia medicamentosa ideal em dois estudos randomizados recentes.<xref ref-type="bibr" rid="B22">22</xref>,<xref ref-type="bibr" rid="B23">23</xref> Nesse contexto, &#233; animador perceber que 85% dos procedimentos relatados no presente estudo tenham sido realizados para al&#237;vio de sintomas, refletindo uma grande ader&#234;ncia dos centros brasileiros &#224;s boas pr&#225;ticas cl&#237;nicas e &#224;s diretrizes internacionais.</p>
      <p>Embora a presen&#231;a de uma OTC ocorra em at&#233; 18 a 52% das cineangiocoronariografias.<xref ref-type="bibr" rid="B19">19</xref>,<xref ref-type="bibr" rid="B24">24</xref>,<xref ref-type="bibr" rid="B25">25</xref> a maioria dos casos n&#227;o possui indica&#231;&#227;o cl&#237;nica para interven&#231;&#227;o. No entanto, um percentual desses pacientes apresenta angina significativa, refrat&#225;ria ao tratamento cl&#237;nico e com comprometimento da qualidade de vida, ou necessita de interven&#231;&#227;o na OTC como estrat&#233;gia de revasculariza&#231;&#227;o completa em pacientes multiarteriais. Acreditamos que o presente relato demonstre a factibilidade de oferecer um tratamento efetivo para esses pacientes no Brasil com taxas satisfat&#243;rias de sucesso e baixo &#237;ndice de complica&#231;&#245;es, em um contexto de mundo real.</p>
      <p>Na <xref ref-type="table" rid="t5">Tabela 5</xref>, comparamos os resultados do tratamento de OTC em nosso meio com os registros contempor&#226;neos da literatura, que categorizamos como Registros Nacionais de ICP em centros n&#227;o selecionados quanto &#224; expertise para ICP de OTC<xref ref-type="bibr" rid="B26">26</xref>,<xref ref-type="bibr" rid="B27">27</xref> e Registros de ICP de OTC em centros dedicados a essas interven&#231;&#245;es nos Estados Unidos, Europa, Jap&#227;o e Am&#233;rica Latina.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B10">10</xref>,<xref ref-type="bibr" rid="B28">28</xref>-<xref ref-type="bibr" rid="B31">31</xref> Os dados dos registros de centros dedicados &#224; ICP em OTC em pa&#237;ses desenvolvidos demonstram taxas de sucesso superiores ao presente estudo, fato que pode ser explicado por terem sido realizadas por operadores de refer&#234;ncia no contexto global e com ampla oferta de dispositivos e materiais para sua realiza&#231;&#227;o. Por outro lado, as taxas de sucesso em nosso meio, assim como no registro LATAM, foram superiores &#224;quelas de Registros Nacionais de pa&#237;ses desenvolvidos que n&#227;o selecionaram somente centros dedicados &#224; ICP de OTC, evidenciando j&#225; uma consider&#225;vel expertise na realiza&#231;&#227;o de ICP em OTC em nosso meio.</p>

      <p>A complexidade das oclus&#245;es em nosso registro, conforme avaliada pela pontua&#231;&#227;o J-CTO, foi semelhante &#224; de outros Registros dedicados a ICP de OTC, mas as informa&#231;&#245;es dos Registros Nacionais de pa&#237;ses desenvolvidos n&#227;o estavam dispon&#237;veis. As taxas de complica&#231;&#245;es e ECAM em nosso meio tamb&#233;m foram semelhantes entre os Registros dedicados ao tratamento de ICP de OTC. Essas compara&#231;&#245;es, embora coloquem em perspectiva a realidade brasileira e latino-americana com a pr&#225;tica m&#233;dica de outros pa&#237;ses, devem ser vistas com cautela em virtude de potenciais vieses na sele&#231;&#227;o de centros, aferi&#231;&#227;o de eventos e outros fatores de confus&#227;o.</p>
      <p>Conforme mencionado acima, nossas menores taxas de sucesso podem estar relacionadas a recursos mais limitados e diferentes fases da curva de aprendizado pelos centros e operadores participantes.<xref ref-type="bibr" rid="B32">32</xref> Diferentemente de outros registros dedicados a ICP em OTC (Jap&#227;o, Estados Unidos e Europa), em nosso meio e na experi&#234;ncia de centros da Am&#233;rica Latina (da qual o Brasil tamb&#233;m participa), n&#227;o estabelecemos um n&#250;mero m&#237;nimo de casos por operador ou por centro para participar. Nosso objetivo foi mostrar um quadro da pr&#225;tica cl&#237;nica em ICP de OTC no Brasil em servi&#231;os engajados no tratamento da OTC de forma dedicada, e todos os centros dispostos a participar foram inclu&#237;dos. Nossos resultados podem, portanto, ser mais generaliz&#225;veis e representar a realidade da maioria dos servi&#231;os de cardiologia intervencionista com interesse em ICP de OTC.</p>
      <p>O uso de microcateteres e inje&#231;&#245;es contralaterais s&#227;o consideradas boas pr&#225;ticas em diversos documentos.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B26">26</xref>,<xref ref-type="bibr" rid="B28">28</xref> O fato de apenas metade dos procedimentos em nosso estudo ter sido feita com inje&#231;&#227;o contralateral e apenas 2/3 usar microcateter pode refletir a curva de aprendizado em curso em nosso pa&#237;s. Embora um uso menor de microcateteres do que o recomendado possa estar relacionado a reembolso e quest&#245;es financeiras, n&#227;o existe esse problema em rela&#231;&#227;o &#224;s inje&#231;&#245;es contralaterais, exceto a escolha do operador. Essas observa&#231;&#245;es destacam a import&#226;ncia de uma educa&#231;&#227;o continuada e treinamento adequado dos operadores dispostos a realizar ICP em OTC.</p>

        <sec>
          <title>Background</title>
          <p> Major advances have been seen in techniques and devices for performing percutaneous coronary interventions (PCIs) for chronic total occlusions (CTOs), but there are limited real-world practice data from developing countries.</p>
        </sec>
        <sec>
          <title>Objectives</title>
          <p> To report clinical and angiographic characteristics, procedural aspects, and clinical outcomes of CTO PCI performed at dedicated centers in Brazil.</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p> Included patients underwent CTO PCI at centers participating in the LATAM CTO Registry, a Latin American multicenter registry dedicated to prospective collection of these data. Inclusion criteria were procedures performed in Brazil, age 18 years or over, and presence of CTO with PCI attempt. CTO was defined as a 100% lesion in an epicardial coronary artery, known or estimated to have lasted at least 3 months.</p>
        </sec>
        <sec>
          <title>Results</title>
          <p> Data on 1196 CTO PCIs were included. Procedures were performed primarily for angina control (85%) and/or treatment of moderate/severe ischemia (24%). Technical success rate was 84%, being achieved with antegrade wire approaches in 81% of procedures, antegrade dissection and re-entry in 9%, and retrograde approaches in 10%. In-hospital adverse cardiovascular events occurred in 2.3% of cases, with a mortality rate of 0.75%.</p>
        </sec>
        <sec>
          <title>Conclusions</title>
          <p> CTOs can be treated effectively in Brazil by using PCI, with low complication rates. The scientific and technological development observed in this area in the past decade is reflected in the clinical practice of dedicated Brazilian centers.</p>

        <title>Introduction</title>
        <p>The prevalence of chronic total occlusion (CTO) is high, with approximately one in three patients undergoing diagnostic coronary angiography, and may reach up to 50% in studies of patients with previous coronary artery bypass grafting (CABG). The condition is also one of the most frequent causes of incomplete revascularization. Percutaneous coronary intervention (PCI) for CTO has been traditionally associated with lower success rates and more complications compared to PCI for stenosis and antegrade flow. This is related to technical difficulties in crossing occlusion with the guidewire but also to greater angiographic complexity, higher risk profile, and presence of comorbidities.<xref ref-type="bibr" rid="B1">1</xref>-<xref ref-type="bibr" rid="B3">3</xref> In recent years, major advances in techniques and devices to recanalize CTOs have been seen, with international CTO PCI registries reporting success rates near 90%.<xref ref-type="bibr" rid="B4">4</xref> However, there are limited contemporary data on the characteristics and outcomes of these procedures in Brazil. This study aimed to describe the clinical and angiographic characteristics, procedural aspects, complications, and clinical outcomes of contemporary patients with CTO undergoing PCI at Brazilian centers dedicated to CTO PCI.</p>

          <title>Patients</title>
          <p>Included patients underwent PCI for treatment of CTO at Brazilian hospitals participating in the LATAM CTO Registry. The inclusion criteria were being 18 years of age or over and having CTO with PCI attempt indicated by the attending physician. There were no minimum procedural volume requirements for participating centers. CTO was defined as a 100% lesion in an epicardial coronary artery, known or estimated to have lasted at least 3 months.<xref ref-type="bibr" rid="B5">5</xref>,<xref ref-type="bibr" rid="B6">6</xref> All decisions regarding patient indications and clinical treatment were made by the attending physicians, with no interference from the researchers. Informed consent was obtained, and the study was approved by a research ethics committee.</p>

          <title>Data collection and monitoring</title>
          <p>Data were included in a multicenter Latin American CTO PCI registry initiated by the group of investigators with support of the Brazilian Society of Hemodynamics and Interventional Cardiology. The database was managed by the coordinating center using Research Electronic Data Capture (REDCap),<xref ref-type="bibr" rid="B7">7</xref> an application approved by the Brazilian Health Regulatory Agency (Anvisa).</p>
          <p>All investigators received a manual with standardized instructions for entering data into electronic spreadsheets. The material focused on registry objectives, data collection, and storage processes (computers, tablets, and/or cell phones, according to the needs of each participating center). The centers received online and telephone support for questions regarding inclusion or completion of cases and monthly feedback for missing data and outliers. Internal quality rules were used to improve the quality of the database, consisting of descriptive analyses of the data, which were summarized and submitted as monthly monitoring reports to the participating centers.</p>

          <title>Definitions</title>
          <p>Moderate-to-severe ischemia was defined as the presence of a perfusion defect detected by scintigraphy, stress echocardiography, or magnetic resonance imaging equal to or greater than 10%. Moderate/severe calcification was defined as at least 50% vessel involvement detected by angiography. Moderate/severe tortuosity was considered when two angulations of at least 70 degrees or one angulation of at least 90 degrees were observed in the target vessel, more specifically in the segment proximal to the CTO. Proximal and distal stumps were defined as blunt or tapered.Collateral vessels were classified as useful for the approach if considered, by the operator, crossable by a guidewire and a microcatheter, and also according to Werner classification.<xref ref-type="bibr" rid="B8">8</xref> Scoring systems for predicting success and complexity &#8212; J-CTO, PROGRESS, CL, and ORA scores &#8212; were used automatically according to the inclusion of angiographic and clinical information required for their calculations.<xref ref-type="bibr" rid="B9">9</xref>-<xref ref-type="bibr" rid="B12">12</xref></p>
          <p>The following strategies were considered for performing the procedures: (a) antegrade wires consisted of an attempt to directly cross the occluded segment using different guidewires, either progressively or not; (b) antegrade dissection and re-entry (ADR) was defined as an antegrade procedure during which the operator intentionally used the subintimal space to partially or completely pass the occluded segment with guidewires or dedicated devices, re-entering the true lumen distally to the occlusion; and (c) retrograde procedure was defined as an attempt at recanalization using a collateral vessel or graft (either occluded or not) that irrigates a segment distal to the occlusion, which could be done using intraplaque wire techniques or dissection and re-entry. Technical success was defined as CTO recanalization with stent implantation, final TIMI grade II/III flow, and residual stenosis lower than 30%. Procedural success was the achievement of technical success without major adverse cardiovascular events (MACEs).</p>
        </sec>
        <sec>
          <title>Clinical outcomes and complications</title>
          <p>MACE was defined as a composite of death, myocardial infarction (MI), and stroke. MI was characterized according to the most recent version of the universal definition.<xref ref-type="bibr" rid="B13">13</xref> Stroke was defined as a new, sudden-onset focal neurological deficit of irreversible, presumably cerebrovascular cause within 24 hours and not caused by any other easily identifiable cause.</p>
          <p>Procedural complications included major bleeding, coronary perforation, cardiac tamponade, and urgent revascularization with PCI or CABG. Major bleeding was defined as any bleeding with reduced hemoglobin &gt; 3 g/dl, blood transfusion, or surgical intervention. Coronary perforation was defined according to the Ellis classification.<xref ref-type="bibr" rid="B14">14</xref> Cardiac tamponade was defined as hemodynamic compromise caused by acute accumulation of blood in the pericardial space. Urgent revascularization was defined as an unplanned procedure performed during hospitalization for treatment of angina and/or recurrent ischemia.</p>

        <sec>
          <title>Statistical analysis</title>
          <p>A descriptive analysis of the data was performed. Parametric continuous variables were reported as mean &#177; standard deviation (SD); nonparametric variables as median (interquartile range); and categorical variables as absolute and relative frequencies. All analyses were performed on SPSS 27.0. The Kolmogorov-Smirnov test was used to ascertain data normality. The significance level was set at &lt;0.05.</p>

Technical success rate at Brazilian centers was 84%, and procedural success was 82%. The strategy showing the highest success rate was that of antegrade wires, with ADR and retrograde approaches being used in approximately 10% of cases each. A single femoral access was used in 26% of the procedures, and a single radial access in 20%. A contralateral injection was used in half of all cases, and the most commonly combined access sites were radial and femoral (27%), followed by bifemoral (22%). <title>Discussion</title>
        <p>In this study, we report contemporary CTO PCI data from Brazilian centers participating in the LATAM CTO Registry, including clinical and angiographic characteristics, procedural aspects, and MACEs. There is a broad collection of reports on these procedures from North America, Western Europe, and Japan,<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B15">15</xref>-<xref ref-type="bibr" rid="B18">18</xref> but Brazil still lacks information. In this contemporary analysis of Brazilian practice, we found encouraging results, with success rates above 80% and low rates of complications and MACEs. Also relevant was the demonstration that the main indications for performing interventions&#8212;angina control and treatment of significant myocardial ischemia&#8212;, as well as the clinical treatment received by patients before the interventions, were in accordance with current guidelines.<xref ref-type="bibr" rid="B15">15</xref>,<xref ref-type="bibr" rid="B16">16</xref> The present analysis is the first medical practice report of Brazilian centers of excellence in CTO PCI using contemporary techniques and approaches recommended by international consensus groups.<xref ref-type="bibr" rid="B17">17</xref></p>
        <p>The indications for CTO PCI were recently questioned by randomized clinical trials that did not show the benefit of these procedures in reducing cardiovascular events or improving ventricular function, although several methodological limitations were observed in those studies.<xref ref-type="bibr" rid="B19">19</xref>-<xref ref-type="bibr" rid="B21">21</xref> Conversely, CTO PCI significantly improved symptoms and quality of life compared to optimal drug therapy in two recent randomized trials.<xref ref-type="bibr" rid="B22">22</xref>,<xref ref-type="bibr" rid="B23">23</xref> Therefore, we are encouraged to report that 85% of the procedures in the present study were performed to relieve symptoms, which reflects substantial adherence of Brazilian centers to good clinical practices and international guidelines.</p>
        <p>Although CTO is found in up to 18-52% of coronary angiograms,<xref ref-type="bibr" rid="B19">19</xref>,<xref ref-type="bibr" rid="B24">24</xref>,<xref ref-type="bibr" rid="B25">25</xref> most cases have no clinical indication for intervention. However, some of these patients have significant angina, refractory to clinical treatment and impairing quality of life, while those with multivessel disease require CTO intervention as a complete revascularization strategy. We believe that the present report demonstrates feasibility of providing an effective treatment for these patients in Brazil, with satisfactory success rates and low complication rates in a real-world context.</p>
        <p><xref ref-type="table" rid="t5001">Table 5</xref> compares CTO treatment outcomes in our setting with those of contemporary literature, which we categorized as national registries of nondedicated CTO PCI centers<xref ref-type="bibr" rid="B26">26</xref>,<xref ref-type="bibr" rid="B27">27</xref> and registries of dedicated CTO PCI centers in the United States (US), Europe, Japan, and Latin America.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B10">10</xref>,<xref ref-type="bibr" rid="B28">28</xref>-<xref ref-type="bibr" rid="B31">31</xref> Data from dedicated CTO PCI centers in developed countries demonstrate success rates higher than those of the present study, which can be explained by procedures being performed by experienced operators in the global context using a wide range of devices and materials. Conversely, success rates in our study, as well as in the LATAM registry, were higher than those of national registries of developed countries not restricted to dedicated CTO PCI centers, which shows considerable CTO PCI expertise in our setting.</p>

        <p>Occlusion complexity in our registry, as assessed by the J-CTO score, was similar to that of other dedicated CTO PCI registries, but information from national registries of developed countries was not available. The rates of complications and MACEs in our setting were also similar to those of dedicated CTO PCI registries. These comparisons, although putting Brazilian and Latin American practices into perspective against medical practice in other countries, should be viewed with caution because of potential biases in center selection, event measurement, and other confounding factors.</p>
        <p>As we mentioned above, our lower success rates may be related to more limited resources and different stages of the learning curve shown by participating centers and operators.<xref ref-type="bibr" rid="B32">32</xref> Unlike other dedicated CTO PCI registries (Japan, US, and Europe), we did not establish a minimum number of cases per operator or per center to participate in our study, similar to the experience of other Latin American centers. Our objective was to show an overview of CTO PCI practice at Brazilian services dedicated to CTO treatment, and all centers willing to participate were included. Our results may therefore be more generalizable and represent the reality of most interventional cardiology services that perform CTO PCI.</p>
        <p>Microcatheters and contralateral injections are considered good practices in several reports.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B26">26</xref>,<xref ref-type="bibr" rid="B28">28</xref> The fact that only half of the procedures in our study were performed with a contralateral injection and only two thirds used a microcatheter may reflect the ongoing learning curve in our country. Although the use of microcatheters below recommendations may be related to reimbursement and financial issues, this is not the case with contralateral injections, which are used at the discretion of the operator. These observations highlight the importance of continuing education and adequate training for operators willing to perform CTO PCI.</p>

          <title>Limitations</title>
          <p>Included data were reported by the participating centers, with no external auditing or onsite monitoring, but we periodically checked the database for outliers, spurious data, and asymmetries in an effort to improve data quality. Additionally, a data dictionary and a detailed instruction manual were sent to all investigators to standardize data collection and minimize variability. We also provided continuous support to the centers that had questions and needed help with collections. Patient inclusion by each center was not necessarily consecutive; thus, we cannot exclude a potential selection bias. Angiographic and procedural characteristics were not independently evaluated by a central laboratory, which may also be considered a limitation. Evaluation of the scoring systems depends largely on the performance of dual injection angiography; however, this was used in only half of cases, which may have overestimated the scores. Clinical outcomes were not adjudicated centrally by a clinical event committee, but standardized definitions were provided to centers in the study manual.</p>
