  <body>
    <p>A pandemia da doen&#231;a de coronav&#237;rus 2019 (COVID-19), al&#233;m de repercuss&#245;es f&#237;sicas, emocionais e sociais gerou importantes altera&#231;&#245;es nas medidas n&#227;o farmacol&#243;gicas de aux&#237;lio ao tratamento de pacientes com doen&#231;as card&#237;acas, como &#233; o caso da Reabilita&#231;&#227;o Card&#237;aca Baseada em Exerc&#237;cios (RCBE). A incapacidade associada &#224; s&#237;ndrome p&#243;s-COVID-19 parece ter um impacto consider&#225;vel nos servi&#231;os de sa&#250;de, pois o tratamento desses pacientes implica uma demanda muito grande de cuidados e um alto custo econ&#244;mico.<xref ref-type="bibr" rid="B1">1</xref></p>
    <p>Evid&#234;ncias cient&#237;ficas demostraram uma associa&#231;&#227;o estreita do comportamento sedent&#225;rio com desenvolvimento e agravo de doen&#231;as cardiovasculares (DCV), al&#233;m de mortalidade precoce. Muitas dessas mortes poderiam ser evitadas ou postergadas com cuidados preventivos e medidas terap&#234;uticas, onde a RCBE &#233; um pilar fundamental.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref></p>
    <p>Os servi&#231;os de RCBE foram temporariamente suspensos pela pol&#237;tica crescente de distanciamento social que levou a v&#225;rios bloqueios sucessivos, tentando prevenir a propaga&#231;&#227;o da infec&#231;&#227;o. Um novo cen&#225;rio se estabeleceu pela impossibilidade de os pacientes irem de forma presencial aos centros de tratamento, o que j&#225; era limitado por v&#225;rios fatores, incluindo geografia, tempo, finan&#231;as e n&#250;mero de centros de reabilita&#231;&#227;o dispon&#237;veis no Brasil. Outras oportunidades sociais para pacientes com DCV praticarem exerc&#237;cios com quantidade e intensidade recomendadas de atividade f&#237;sica regular tamb&#233;m foram interrompidas.<xref ref-type="bibr" rid="B4">4</xref> O COVID-19 apresentou um risco duplo para esses indiv&#237;duos. Em primeiro lugar, comorbidades cardiovasculares pr&#233;-existentes expondo pacientes a alto risco de desfechos adversos em caso de infec&#231;&#227;o pelo coronav&#237;rus. Em segundo lugar, o combate aos fatores de risco modific&#225;veis que podem ser controlados com atividade f&#237;sica regular tamb&#233;m ficaram prejudicados.<xref ref-type="bibr" rid="B5">5</xref></p>
    <p>Os atendimentos em RCBE requerem transforma&#231;&#245;es que reinventem o modelo de relacionamento paciente-terapeuta, a fim de atender com qualidade as necessidades das pessoas, fazendo com que retornem ao n&#237;vel da melhor capacidade funcional e independ&#234;ncia poss&#237;vel da maneira mais eficaz, eficiente e segura, buscando reduzir as consequ&#234;ncias diretas e indiretas do COVID-19.<xref ref-type="bibr" rid="B6">6</xref> Essas transforma&#231;&#245;es, no entanto, n&#227;o s&#227;o uma tarefa f&#225;cil.</p>
    <p>O estudo de Ghisi et al.,<xref ref-type="bibr" rid="B7">7</xref> publicado nos ABC Cardiol, apesar de ter uma amostra pequena e utilizado um question&#225;rio online desenvolvido pelos autores, mostrou como as amea&#231;as econ&#244;micas experimentadas pelos profissionais de sa&#250;de e seus programas, e a incapacidade de muitos pacientes de ter acesso e poder navegar no mundo virtual afetou sobremaneira os participantes da RCBE. Os autores avaliaram participantes de programas de RCBE sobre alfabetiza&#231;&#227;o em sa&#250;de, uso de tecnologia e acesso &#224; internet e como eles perceberam sua sa&#250;de durante a pandemia, dentre outras coisas, 26% relataram agravo em sua condi&#231;&#227;o card&#237;aca e sintomas como dor no peito, falta de ar, palpita&#231;&#245;es card&#237;acas, ansiedade e depress&#227;o. Indo al&#233;m, tamb&#233;m alertaram para o fato de que as institui&#231;&#245;es e os profissionais de RCBE devem trabalhar para desenvolver uma melhor abordagem pr&#225;tica para monitorar os pacientes cardiopatas virtualmente e personalizar os cuidados de preven&#231;&#227;o, ajudando esses indiv&#237;duos em sua recupera&#231;&#227;o e na preven&#231;&#227;o de eventos.<xref ref-type="bibr" rid="B7">7</xref> Haskiah et al. tamb&#233;m encontraram redu&#231;&#227;o na capacidade de exerc&#237;cio e aumento no percentual de gordura em participantes que suspenderam seus tratamentos durante a pandemia de COVID-19. Sugerem que a reabilita&#231;&#227;o card&#237;aca remota possa ser uma alternativa eficaz &#224; reabilita&#231;&#227;o card&#237;aca ambulatorial em per&#237;odos como a pandemia de COVID-19 e mesmo para pacientes que optem por essa abordagem.<xref ref-type="bibr" rid="B8">8</xref> Apesar do encaminhamento ser recomenda&#231;&#227;o Classe I, apenas entre 30-50% dos pacientes eleg&#237;veis s&#227;o encaminhados para RCBE por seu cardiologista; e menos ainda, conseguem concluir um programa.<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B10">10</xref> Nakayama et al. identificaram no Jap&#227;o, o que ocorre tamb&#233;m no Brasil: o principal impedimento para frequentar programas de RCBE &#233; a dist&#226;ncia de casa para o local do programa e escassez de servi&#231;os estruturados.<xref ref-type="bibr" rid="B10">10</xref>,<xref ref-type="bibr" rid="B11">11</xref></p>
    <p>Um fator positivo deste novo cen&#225;rio da RCBE supervisionada &#224; dist&#226;ncia &#233; a implementa&#231;&#227;o da chamada fase 4 de reabilita&#231;&#227;o com exerc&#237;cios. Esta fase caracteriza-se pelo paciente n&#227;o mais estando vinculado aos centros de tratamento de forma presencial e sim remota. Ownbey et al.,<xref ref-type="bibr" rid="B12">12</xref> mostraram que n&#227;o h&#225; diferen&#231;as significativas nos resultados dos pacientes, entre h&#237;bridos e presenciais, e indicam que a RCBE remota &#233; uma adi&#231;&#227;o vi&#225;vel ao atendimento presencial.<xref ref-type="bibr" rid="B12">12</xref> No entanto, no Brasil, assim como em outros pa&#237;ses,<xref ref-type="bibr" rid="B13">13</xref> a maioria dos profissionais de sa&#250;de tem pouca ou nenhuma experi&#234;ncia na implementa&#231;&#227;o de RCBE domiciliar e remota, al&#233;m disso os conv&#234;nios m&#233;dicos e o pr&#243;prio sistema &#250;nico de sa&#250;de (SUS) normalmente n&#227;o cobrem essa modalidade de atendimento. S&#227;o necess&#225;rios maiores investimentos em treinamento de profissionais e implementa&#231;&#227;o de formas de monitoramento de exerc&#237;cios &#224; dist&#226;ncia, educa&#231;&#227;o para uma vida saud&#225;vel e modifica&#231;&#227;o de comportamento para melhorar a sa&#250;de geral.</p>
    <p>A RCBE encontra-se numa fronteira, necessitando novas pesquisas na &#225;rea, maiores investimentos que permitam manter um maior n&#250;mero de pacientes em atendimento tanto presencial quanto remoto, onde o maior impedimento se encontra no investimento financeiro, educa&#231;&#227;o do paciente para sua automonitoriza&#231;&#227;o, permitindo assim uma maior seguran&#231;a em rela&#231;&#227;o &#224; prescri&#231;&#227;o do exerc&#237;cio f&#237;sico &#224; dist&#226;ncia.</p>

      <p>The coronavirus disease 2019 (COVID-19) pandemic, in addition to physical, emotional, and social repercussions, has generated important changes in non-pharmacological measures to aid in treating patients with heart disease, such as Exercise-Based Cardiac Rehabilitation (ECBR). The disability associated with the post-COVID-19 syndrome seems to have a considerable impact on health services, as the treatment of these patients implies a very high demand for care and a high economic cost.<xref ref-type="bibr" rid="B1">1</xref></p>
      <p>Scientific evidence has shown a close association between sedentary behavior and the development and worsening of cardiovascular disease (CVD) and early mortality. Many of these deaths could be avoided or postponed with preventive care and therapeutic measures, where EBCR is a fundamental pillar.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref></p>
      <p>EBCR services were temporarily suspended by the increasing policy of social distancing, which led to several successive lockdowns, trying to prevent the spread of the infection. A new scenario was established by the impossibility of patients going to the treatment centers in person, which was already limited by several factors, including geography, time, finances, and the number of rehabilitation centers available in Brazil. Other social opportunities for CVD patients to exercise at the recommended amount and intensity of regular physical activity were also cut off.<xref ref-type="bibr" rid="B4">4</xref> COVID-19 presented a dual risk for these individuals. First, pre-existing cardiovascular comorbidities expose patients to a high risk of adverse outcomes in case of coronavirus infection. Second, the fight against modifiable risk factors that can be controlled with regular physical activity was also impaired.<xref ref-type="bibr" rid="B5">5</xref></p>
      <p>EBCR care requires transformations that reinvent the patient-therapist relationship model in order to meet people's needs with quality, making them return to the level of the best functional capacity and independence possible in the most effective, efficient, and safe way, seeking to reduce the direct and indirect consequences of COVID-19.<xref ref-type="bibr" rid="B6">6</xref> These transformations, however, are not an easy task.</p>
      <p>The study by Ghisi et al.,<xref ref-type="bibr" rid="B7">7</xref> published in the ABC Cardiol, despite having a small sample and using an online questionnaire developed by the authors, shows how the economic threats experienced by health professionals and their programs and the inability of many patients to have access and to be able to navigate in the virtual world greatly affected the participants of the EBCR. The authors assessed participants in EBCR programs on health literacy, technology use, and internet access and how they perceived their health during the pandemic, among other things, 26% reported worsening heart conditions and symptoms such as chest pain, shortness of breath, heart palpitations, anxiety, and depression. Going beyond also alerted to the fact that EBBR institutions and professionals should work to develop a better practical approach to monitor cardiac patients virtually and personalize preventive care, helping these individuals in their recovery and the prevention of events.<xref ref-type="bibr" rid="B7">7</xref> Haskiah et al. also found a reduction in exercise capacity and an increase in fat percentage in participants who suspended their treatments during the COVID-19 pandemic. They suggest remote cardiac rehabilitation may be an effective alternative to outpatient cardiac rehabilitation in periods like the COVID-19 pandemic and even for patients who choose this approach.<xref ref-type="bibr" rid="B8">8</xref> Although referral is a Class I recommendation, only 30-50% of eligible patients are referred for EBCR by their cardiologist, and even fewer manage to complete a program.<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B10">10</xref> Nakayama et al. They identified in Japan what also occurs in Brazil: the main impediment to attending EBCR programs is the distance from home to the program location and the scarcity of structured services.<xref ref-type="bibr" rid="B10">10</xref>,<xref ref-type="bibr" rid="B11">11</xref></p>
      <p>A positive factor in this new scenario of distance-supervised EBBR is the implementation of the so-called phase 4 of rehabilitation with exercises. This phase is characterized by the patient being no longer physically linked to the treatment centers but remotely. Ownbey et al.<xref ref-type="bibr" rid="B12">12</xref> showed no significant differences in patient outcomes between hybrid and face-to-face care, indicating that remote EBCR is a viable addition to face-to-face care.<xref ref-type="bibr" rid="B12">12</xref> However, in Brazil, as in other countries,<xref ref-type="bibr" rid="B13">13</xref> most healthcare professionals have little or no experience implementing home and remote RCBE; in addition, medical insurance and the unified health system (SUS) usually do not cover this type of care. Greater investments are needed in training professionals and implementing remote exercise monitoring, education for healthy living, and behavior modification to improve overall health.</p>
      <p>The EBCR is on a frontier, requiring further research in the area and greater investments that allow maintaining a greater number of patients in both face-to-face and remote care, where the greatest impediment is found in financial investment, patient education for self-monitoring, thus allowing greater safety concerning the prescription of physical exercise at a distance.</p>
