          <title>M&#233;todos</title>
          <p> Estudo de coorte transversal no Registro Paulista de Cirurgia Cardiovascular II (REPLICCAR II). Os dados perioperat&#243;rios de 3122 pacientes foram agrupados pelo n&#250;mero de enxertos arteriais utilizados e seus desfechos foram comparados: reopera&#231;&#227;o, infec&#231;&#227;o profunda da ferida tor&#225;cica (IPFT), acidente vascular cerebral, les&#227;o renal aguda, intuba&#231;&#227;o prolongada (&gt;24 horas), tempo de interna&#231;&#227;o curta (&lt;6 dias), tempo de interna&#231;&#227;o prolongada (&gt;14 dias), morbidade e mortalidade. O Propensity Score Matching (PSM) correspondeu a 1062 pacientes, ajustado para o risco de mortalidade.</p>
        </sec>
        <sec>
          <title>Resultados</title>
          <p>Ap&#243;s PSM, o grupo enxerto arterial &#250;nico apresentou pacientes com idade avan&#231;ada, mais ex-fumantes, hipertensos, diab&#233;ticos, portadores de angina est&#225;vel e infarto do mioc&#225;rdio pr&#233;vio. Nos enxertos arteriais m&#250;ltiplos houve predom&#237;nio do sexo masculino, pneumonia recente e cirurgias de urg&#234;ncia. Ap&#243;s o procedimento, houve maior incid&#234;ncia de derrame pleural (p=0,042), pneumonia (p=0,01), reintuba&#231;&#227;o (p=0,006), IPFT (p=0,007) e desbridamento esternal (p=0,015) no grupo de enxertos multiarteriais, por&#233;m, menor necessidade de hemotransfus&#227;o (p=0,005), infec&#231;&#245;es de extremidades (p=0,002) e menor tempo de interna&#231;&#227;o (p=0,036). O uso bilateral da art&#233;ria tor&#225;cica interna n&#227;o foi relacionado ao aumento da taxa de IPFT, e sim a hemoglobina glicosilada &gt;6,40% (p=0,048).</p>
        </sec>
        <sec>
          <title>Conclus&#227;o</title>
          <p> Pacientes submetidos a t&#233;cnica multiarterial apresentaram maior incid&#234;ncia de complica&#231;&#245;es pulmonares e IPFT, sendo que a hemoglobina glicosilada &#8805;6,40% teve maior influ&#234;ncia no resultado infeccioso do que a escolha dos enxertos.</p>

      <title>Introdu&#231;&#227;o</title>
      <p>As doen&#231;as cardiovasculares s&#227;o a primeira causa de morte no mundo, sendo a doen&#231;a arterial coronariana a de maior impacto.<xref rid="B1" ref-type="bibr">1</xref> Neste cen&#225;rio, a cirurgia de revasculariza&#231;&#227;o mioc&#225;rdica (CRM) tem se tornado uma estrat&#233;gia fundamental resultando um dos procedimentos mais realizados no mundo,<xref rid="B2" ref-type="bibr">2</xref> , <xref rid="B3" ref-type="bibr">3</xref> aumentando a sobrevida em longo prazo. Sendo assim, a pat&#234;ncia dos enxertos escolhidos se torna um ponto fundamental na tomada de decis&#227;o.</p>
      <p>Evid&#234;ncias atuais t&#234;m recomendado o maior uso de enxertos arteriais principalmente para a segunda melhor art&#233;ria coron&#225;ria. Neste cen&#225;rio, estudos prospectivos com grandes amostras foram capazes de afirmar que o uso da art&#233;ria tor&#225;cica interna (ATI) bilateral &#233; melhor que a ATI &#250;nica em longo prazo, inclusive quando a revasculariza&#231;&#227;o for incompleta.<xref rid="B4" ref-type="bibr">4</xref> - <xref rid="B7" ref-type="bibr">7</xref> No entanto, a dificuldade da t&#233;cnica de retirada dos enxertos e consequentemente o aumento do tempo cir&#250;rgico fazem com que alguns cirurgi&#245;es optem pela utiliza&#231;&#227;o de enxertos venosos ao inv&#233;s de arteriais,<xref rid="B6" ref-type="bibr">6</xref> , <xref rid="B8" ref-type="bibr">8</xref> , <xref rid="B9" ref-type="bibr">9</xref> ao mesmo passo, a infe&#231;&#227;o profunda da ferida tor&#225;cica, aumento de sangramento intraoperat&#243;rio, maior tempo de cicatriza&#231;&#227;o entre outros se destacam como pontos negativos.<xref rid="B9" ref-type="bibr">9</xref> Motivos que podem ter colaborado para uma baixa ader&#234;ncia na pr&#225;tica.</p>
      <p>Por outro lado, an&#225;lises recentes mostrando os benef&#237;cios do uso do enxerto da art&#233;ria radial para a segunda melhor art&#233;ria coron&#225;ria mostram o impacto de uma boa alternativa. Os motivos potenciais seriam a diminui&#231;&#227;o do tempo da cirurgia, a diminui&#231;&#227;o do risco de infe&#231;&#227;o profunda na ferida tor&#225;cica e uma excelente pat&#234;ncia em longo prazo quando associada ao uso de antagonistas do c&#225;lcio.<xref rid="B10" ref-type="bibr">10</xref> Embora sua escolha para os diversos perfis de pacientes pare&#231;a maior, incluindo os diab&#233;ticos, alguns questionamentos relacionados &#224;s caracter&#237;sticas da art&#233;ria coron&#225;ria alvo e ao grau de obstru&#231;&#227;o s&#227;o manifestos.</p>
      <p>Nesta vertente quando analisamos os resultados do ART trial vemos que pacientes tratados com a op&#231;&#227;o multiarterial, independentemente de ser ATI bilateral ou ATI unilateral com enxerto radial, tiveram maior sobrevida livre de eventos quando comparados ao bra&#231;o uniarterial.<xref rid="B11" ref-type="bibr">11</xref> No entanto ainda existem controv&#233;rsias de que a op&#231;&#227;o multiarterial aumentaria o n&#250;mero de complica&#231;&#245;es imediatas.</p>
      <p>Desta forma, o intuito deste estudo foi comparar os resultados imediatos no que tange a morbimortalidade de pacientes submetidos a CRM uniarterial versus multiarterial no projeto Registro Paulista de Cirurgia Cardiovascular II (REPLICCAR II)</p>

      <p>Para esta an&#225;lise foram utilizados dados do REPLICCAR II, desenvolvido em parceria com a Escola da Sa&#250;de P&#250;blica de Harvard Medical School . Trata-se de um registro prospectivo, observacional e multic&#234;ntrico com dados de pacientes submetidos a cirurgia de revasculariza&#231;&#227;o mioc&#225;rdica prim&#225;ria e isolada em 5 hospitais do estado de S&#227;o Paulo entre julho de 2017 e junho de 2019. A coleta das vari&#225;veis perioperat&#243;rias foi online em um registro constru&#237;do na plataforma REDCap e dedicada ao projeto o mesmo que seguiu as defini&#231;&#245;es da vers&#227;o 2.9 do sistema de coleta de dados da Society of Thoracic Surgeons (STS).</p>
      <p>Foi avaliada a tend&#234;ncia do uso dos enxertos arteriais nas cirurgias de revasculariza&#231;&#227;o mioc&#225;rdica em centros no Estado de S&#227;o Paulo, ao mesmo passo que foram comparados os resultados imediatos utilizando 1 enxerto arterial (uniarterial) versus &#8805;2 enxertos arteriais (multiarterial) em rela&#231;&#227;o aos desfechos recomendados pelo STS. Duas an&#225;lises foram feitas, uma com a base total e outra ap&#243;s o ajuste com a t&#233;cnica de Propensity Score Matching que utilizou o risco esperado de mortalidade pelo STS resultando no pareamento de 1062 pacientes (531 em cada grupo uni versus multiarterial). O fluxograma de metodologias abordadas est&#225; disposto na <xref rid="f02" ref-type="fig">figura 1</xref> . A caracteriza&#231;&#227;o dos pacientes avaliados &#233; representada no suplemento 1. <p>Os dados perioperat&#243;rios e de seguimento (presencial ou via telef&#244;nica) realizados 30 dias ap&#243;s a cirurgia (se alta hospitalar pr&#233;via) foram inclu&#237;dos na plataforma constru&#237;da no REDCap para este projeto. Auditorias de qualidade foram realizadas periodicamente com o intuito de verificar a exatid&#227;o, integridade e consist&#234;ncia dos dados.<xref rid="B13" ref-type="bibr">13</xref></p>
        <p>As vari&#225;veis de desfecho analisadas foram: reopera&#231;&#227;o, infec&#231;&#227;o profunda de ferida tor&#225;cica, acidente vascular cerebral, les&#227;o renal, intuba&#231;&#227;o prolongada (&gt;24 horas), tempo de interna&#231;&#227;o curta (&lt;6 dias), tempo de interna&#231;&#227;o prolongada (&gt;14 dias), morbimortalidade e mortalidade operat&#243;ria.</p>
        <p>Todas as defini&#231;&#245;es das vari&#225;veis seguiram os crit&#233;rios estipulados pela vers&#227;o 2.9 da STS Adult Cardiac Surgery Database. <xref rid="B14" ref-type="bibr">14</xref> A vari&#225;vel morbidade foi um desfecho composto que incluiu os cinco desfechos importantes: acidente vascular cerebral, falha renal aguda, intuba&#231;&#227;o prolongada, infe&#231;&#227;o profunda da ferida tor&#225;cica e reopera&#231;&#227;o. Mortalidade operat&#243;ria foi definida como as mortes ocorridas ap&#243;s a alta hospitalar, mas antes do trig&#233;simo dia de p&#243;s-operat&#243;rio, assim como os &#243;bitos ocorridos durante a interna&#231;&#227;o em que a opera&#231;&#227;o foi realizada, mesmo que ap&#243;s 30 dias.</p>

        <p>O software R vers&#227;o 4.0.2 foi utilizado para a realiza&#231;&#227;o de todas as an&#225;lises neste estudo.</p>
        <p>Na an&#225;lise descritiva, as vari&#225;veis cont&#237;nuas foram expressas em m&#233;dia, mediana, desvio padr&#227;o e quartis (25% e 75%), enquanto as vari&#225;veis categ&#243;ricas foram expressas em termos de frequ&#234;ncias e porcentagens. Em raz&#227;o de missings data , as porcentagens foram calculadas em fun&#231;&#227;o do N de respostas obtidas por vari&#225;veis e n&#227;o do n&#250;mero total de pacientes.</p>
        <p>O Propensity Score Matching foi utilizado para o pareamento dos grupos utilizando o m&#233;todo &#8220;nearest&#8221;, dispon&#237;vel no pacote &#8220;matchit&#8221; do software R.</p>
        <p>Para a compara&#231;&#227;o de dois grupos nas vari&#225;veis cont&#237;nuas foi utilizado o teste t para vari&#225;veis que seguiam a distribui&#231;&#227;o normal (teste de Anderson-Darling) e para as demais foram utilizados os testes n&#227;o param&#233;tricos. No caso das vari&#225;veis homog&#234;neas foi utilizado teste de Mann-Whitney e para vari&#225;veis heterog&#234;neas foi utilizado teste de Brunner-Munzel. Em rela&#231;&#227;o, as vari&#225;veis categ&#243;ricas, foi utilizado o teste Exato de Fisher ou o teste Qui-Quadrado. O n&#237;vel de signific&#226;ncia adotado nos testes foi de 0,05. Foram consideradas hip&#243;teses bicaudais. Al&#233;m disso, os intervalos de confian&#231;a constru&#237;dos foram de 95%.</p>

      <title>Resultados</title>
      <p>O banco de dados geral mostrou que o tempo de circula&#231;&#227;o extracorp&#243;rea foi significativamente maior no grupo enxerto arterial &#250;nico (p=0,001), assim como o tempo de an&#243;xia (p=0,031); por outro lado, o tempo total de cirurgia foi maior no grupo de enxertos arteriais m&#250;ltiplos (p&lt;0,001). Percebe-se que o perfil do paciente do grupo uniarterial apresentou n&#237;veis mais baixos de hemat&#243;crito (p&#8804;0,001), hemoglobina (p&#8804;0,001), mas maior taxa de hemotransfus&#245;es durante o procedimento (p&#8804;0,001), comparado ao grupo multiarterial. No intraoperat&#243;rio, a art&#233;ria radial foi utilizada em 30,32% dos pacientes submetidos ao tratamento com multiarterial e a art&#233;ria tor&#225;cica interna direita em 79,28%, enquanto a art&#233;ria tor&#225;cica interna esquerda foi utilizada em 100% dos casos; assim, a art&#233;ria tor&#225;cica interna esquerda, a art&#233;ria tor&#225;cica interna direita e a art&#233;ria radial foram utilizadas concomitantemente em 9,06% dos pacientes (Material suplementar &#8211; <xref rid="t2" ref-type="table">Tabela 2</xref> ).</p>

      <p>Entre as vari&#225;veis pr&#233;-operat&#243;rias ap&#243;s PSM (Material suplementar &#8211; Tabela 4), houve maior preval&#234;ncia do sexo masculino no grupo de enxertos arteriais m&#250;ltiplos (p&lt;0,001). O perfil dos pacientes mostrou que os pacientes do grupo uniarterial tinham idade mais avan&#231;ada (p&lt;0,001), maior taxa de diabetes mellitus (p=0,008), hipertens&#227;o (p&lt;0,001), infarto do mioc&#225;rdio pr&#233;vio &gt;21 dias (p=0,004), e tabagismo (p=0,027). Em contrapartida, o grupo multiarterial apresentou maior taxa de cirurgias de urg&#234;ncia (p=0,022), hist&#243;ria familiar de doen&#231;a coronariana (p=0,011), pneumonia recente (p=0,007), angina inst&#225;vel ou infarto agudo do mioc&#225;rdio com ou sem eleva&#231;&#227;o do segmento ST e sintomas na admiss&#227;o hospitalar (p=0,03). Apesar do risco predito de mortalidade estar equilibrado entre os dois grupos, observou-se heterogeneidade de probabilidade de risco em rela&#231;&#227;o &#224;s vari&#225;veis dos desfechos avaliados.</p>
      <p>Em rela&#231;&#227;o aos achados intraoperat&#243;rios ( <xref rid="t1" ref-type="table">Tabela 1</xref> ), o grupo uniarterial apresentou n&#237;veis de hemat&#243;crito e hemoglobina ligeiramente menores em rela&#231;&#227;o ao grupo de enxertos arteriais m&#250;ltiplos (p=0,007 e p=0,006, respectivamente), o que pode ter influenciado na maior necessidade de vermelho transfus&#227;o de c&#233;lulas sangu&#237;neas (p=0,005). No grupo de enxertos arteriais m&#250;ltiplos, houve maior utiliza&#231;&#227;o da t&#233;cnica esqueletizada para dissec&#231;&#227;o da art&#233;ria tor&#225;cica interna (p&lt;0,001) e tempo cir&#250;rgico (p&lt;0,001).</p>
      <p>Com rela&#231;&#227;o aos resultados p&#243;s-operat&#243;rios ( <xref rid="t2" ref-type="table">Tabela 2</xref> ), o grupo de enxertos arteriais m&#250;ltiplos teve maior necessidade de drenagem tor&#225;cica devido a derrame pleural (p=0,042), IPFT (p=0,007), pneumonia (p=0,01), necessidade de reintuba&#231;&#227;o (p= 0,006), morbidade (p=0,028) e desbridamento esternal (p=0,015). No entanto, houve menores taxas de infec&#231;&#227;o no local da retirada da veia safena (p=0,002) e tempo de interna&#231;&#227;o &lt;6 dias (p=0,036). N&#227;o foram observadas diferen&#231;as estatisticamente significativas em rela&#231;&#227;o ao p&#243;s-operat&#243;rio de acidente vascular cerebral, les&#227;o renal, tempo de intuba&#231;&#227;o, mortalidade e tempo de interna&#231;&#227;o &gt; 14 dias ou mortalidade.</p>
      <p>Investigando os achados no grupo de enxertos arteriais m&#250;ltiplos, foi feita uma an&#225;lise relacionando o n&#237;vel de hemoglobina glicosilada entre os pacientes que desenvolveram IPFT versus aqueles que n&#227;o desenvolveram. No grupo de pacientes com enxertos multiarteriais, a hemoglobina glicosilada foi significativamente maior no grupo IPFT do que no grupo n&#227;o IPFT (p=0,048). <title>Discuss&#227;o</title>
      <p>O debate acerca da rela&#231;&#227;o de pr&#243;s e contras sobre o uso de enxertos arteriais m&#250;ltiplos persistem, onde os melhores resultados em longo prazo<xref rid="B9" ref-type="bibr">9</xref> s&#227;o confrontados por maiores taxas de infec&#231;&#227;o, dificuldade t&#233;cnica e o consequente aumento de tempo cir&#250;rgico.<xref rid="B15" ref-type="bibr">15</xref> , <xref rid="B16" ref-type="bibr">16</xref> Nossa an&#225;lise &#233; consistente com a literatura e mostrou que na rotina a maioria das equipes prefere utilizar um &#250;nico enxerto arterial nas cirurgias de revasculariza&#231;&#227;o mioc&#225;rdica (uniarterial N= 2591 versus multiarterial N=531). Neste estudo transversal do banco de dados REPLICCAR II, o grupo com enxertos multiarteriais foi associado ao desenvolvimento de complica&#231;&#245;es imediatas pulmonares e IPFT.</p>
      <p>Com rela&#231;&#227;o aos desfechos pulmonares, houve aumento na taxa de derrame pleural (p=0,042), pneumonia p&#243;s-operat&#243;ria (p=0,01) e necessidade de reintuba&#231;&#227;o (p=0,006), entretanto, &#233; importante ressaltar que este mesmo grupo tamb&#233;m apresentou maior incid&#234;ncia de pneumonia no per&#237;odo pr&#233;-operat&#243;rio (p=0,007), o que pode ter contribu&#237;do com o resultado negativo desta popula&#231;&#227;o, assim como a utiliza&#231;&#227;o de t&#233;cnica de pleuras abertas para dissec&#231;&#227;o das art&#233;rias tor&#225;cicas e o aumento de tempo cir&#250;rgico (p&gt;0,001), consequentemente levando a maior tempo de anestesia,<xref rid="B17" ref-type="bibr">17</xref> entretanto, n&#227;o foi observado impacto na mortalidade do grupo de pacientes submetidos a t&#233;cnica multiarterial.</p>
      <p>Rocha et. al.<xref rid="B18" ref-type="bibr">18</xref> avaliaram retrospectivamente a evolu&#231;&#227;o de pacientes que foram submetidos a CRM com 2 ou &gt;2 enxertos arteriais, dentre seus achados, o tempo de perman&#234;ncia hospitalar entre os grupos n&#227;o diferiu na popula&#231;&#227;o estudada (m&#233;dia de 6 dias em ambos os grupos). Em nosso estudo, avaliando pacientes submetidos a t&#233;cnica cir&#250;rgica uni e multiarterial, foi observado que o grupo multiarterial teve impacto significativo na redu&#231;&#227;o do tempo de perman&#234;ncia hospitalar (p=0,036). A justificativa pode ser devida a uma maior disponibilidade do paciente para deambular de maneira mais eficiente no p&#243;s-operat&#243;rio, implicando na redu&#231;&#227;o de edema, dor, est&#237;mulo a circula&#231;&#227;o sangu&#237;nea entre outros, o que pode ter influenciado em um menor tempo de interna&#231;&#227;o, tendo em vista de que a deambula&#231;&#227;o precoce &#233; um dos itens mais importantes que contribui &#224; evolu&#231;&#227;o positiva do paciente, como o recomendam os protocolos de r&#225;pida recupera&#231;&#227;o p&#243;s-cir&#250;rgica.<xref rid="B19" ref-type="bibr">19</xref></p>
      <p>Nesta an&#225;lise, o grupo uniarterial, mesmo com uma maior preval&#234;ncia de pacientes diab&#233;ticos (p=0,008) e maior risco de IPFT (p=0,003), teve menor taxa de IPFT (p=0,007) e de desbridamento (p=0,015) que o grupo de pacientes com enxertos multiarteriais, o que vai de encontro com a literatura atual.<xref rid="B15" ref-type="bibr">15</xref> , <xref rid="B16" ref-type="bibr">16</xref> Observamos que a hemoglobina glicosilada &gt;6,40% foi determinante para o desenvolvimento de IPFT, independentemente do enxerto utilizado no grupo multiarterial (p=0,048). Isto tamb&#233;m pode ter sido influenciado pelo fato de que no grupo multiarterial houve uma taxa maior de pacientes operados com status de urg&#234;ncia (p=0,022) e com idade mais avan&#231;ada (p&lt;0,001).</p>
      <p>Tendo em vista de que a hemoglobina glicosilada elevada influenciou no desenvolvimento de IPFT no grupo multiarterial, levanta-se o questionamento relacionado &#224; atual conduta das equipes m&#233;dicas, onde pacientes portadores de diabetes, ou com n&#237;veis aumentados de hemoglobina glicosilada s&#227;o gerenciados para o n&#227;o uso da ATI bilateral e sim para m&#233;todos alternativos como enxerto de art&#233;ria radial e/ou safena, com o intuito de reduzir a probabilidade de ocorrer o desfecho de infec&#231;&#227;o.<xref rid="B20" ref-type="bibr">20</xref> , <xref rid="B21" ref-type="bibr">21</xref> Entretanto, nossos achados mostram que para evitar IFPT, os valores glic&#234;micos precisam ser estritamente corrigidos antes, durante e ap&#243;s a cirurgia de forma soberana &#224; escolha dos enxertos utilizados, desta forma, acreditamos que a otimiza&#231;&#227;o mais criteriosa da glicemia<xref rid="B22" ref-type="bibr">22</xref> , <xref rid="B23" ref-type="bibr">23</xref> pode trazer mais benef&#237;cios ao paciente que simplesmente evitar o uso da ATI bilateral.</p>
      <p>Dorman et al.<xref rid="B24" ref-type="bibr">24</xref> compararam retrospectivamente ap&#243;s PSM, pacientes diab&#233;ticos controlados e assistidos submetidos a CRM uni e multi arterial em um seguimento de 30 anos e mostraram que, por mais que o paciente fosse diab&#233;tico o uso de ATI bilateral n&#227;o aumentou as taxas de morbidade e tampouco de mortalidade, Zhou et al.<xref rid="B25" ref-type="bibr">25</xref> tamb&#233;m mostraram benef&#237;cios no uso da t&#233;cnica mutliarterial para pacientes diab&#233;ticos devidamente controlados em sua metan&#225;lise, Estes achados refor&#231;am a teoria colocada pelos autores deste artigo, onde o controle da glicemia do paciente &#233; soberano &#224; escolha dos enxertos.</p>
      <p>Este estudo trata-se de uma coorte transversal em um banco de dados de pacientes submetidos a CRM no estado de S&#227;o Paulo, onde o impacto da escolha do tipo de enxertos utilizados n&#227;o seguiu uma padroniza&#231;&#227;o pr&#233;-cirurgia o que pode ter impactado nos achados. Para diminuir vieses foi feito um ajuste atrav&#233;s da t&#233;cnica de Propensity Score Matching a partir da mortalidade estimada pelo STS em nossos registros prospectivos e estruturados de forma robusta, portanto, os resultados precisam ser validados em futuras pesquisas, preferencialmente em estudos randomizados, considerados padr&#227;o-ouro para o perfil de an&#225;lise.</p>

      <p>Nesta presente an&#225;lise, foi identificado que o uso de enxertos multiarteriais para pacientes submetidos a cirurgia de revasculariza&#231;&#227;o mioc&#225;rdica apresentaram maior taxa de complica&#231;&#245;es pulmonares e infec&#231;&#227;o profunda de ferida tor&#225;cica, sendo que a hemoglobina glicosilada acima de 6,40% teve maior influ&#234;ncia no resultado do que a escolha dos enxertos propriamente dita. Recomenda-se o controle rigoroso da glicemia dos pacientes eletivos para n&#237;veis de hemoglobina glicada &#8805;6,40%, assim como um controle exaustivo nos pacientes operados de urg&#234;ncia, de forma de evitar desfechos infecciosos que podem impactar na evolu&#231;&#227;o do paciente de forma consider&#225;vel.</p>

          <title>Background</title>
          <p>The short-term results after using arterial grafts still raise questions and doubts for medical society.</p>
        </sec>
        <sec>
          <title>Objective</title>
          <p>To compare the immediate outcomes of patients undergoing single arterial graft versus multiple arterial grafts coronary artery bypass grafting surgery.</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p>Cross-sectional cohort study in the S&#227;o Paulo Registry of Cardiovascular Surgery II (REPLICCAR II). Perioperative data from 3122 patients were grouped by the number of arterial grafts used, and their outcomes were compared: reoperation, deep sternal wound infection (DSWI), stroke, acute kidney injury, prolonged intubation (&gt;24 hours), short hospital stay (&lt;6 days), prolonged hospital stay (&gt;14 days), morbidity and mortality. Propensity Score Matching (PSM) matched 1062 patients, adjusted for the mortality risk.</p>
        </sec>
        <sec>
          <title>Results</title>
          <p>After PSM, the single arterial graft group showed patients with advanced age, more former smokers, hypertension, diabetes, stable angina, and previous myocardial infarction. In the multiple arterial grafts, there was a predominance of males, recent pneumonia, and urgent surgeries. After the procedure, there was a higher incidence of pleural effusion (p=0.042), pneumonia (p=0.01), reintubation (p=0.006), DSWI (p=0.007), and sternal debridement (p=0.015) in the multiple arterial grafts group, however, less need for blood transfusion (p=0.005), extremity infections (p=0.002) and shorter hospital stays (p=0.036). Bilateral use of the internal thoracic artery was not related to increased DSWI rate, but glycosylated hemoglobin &gt;6.40% (p=0.048).</p>
        </sec>
        <sec>
          <title>Conclusion</title>
          <p>Patients undergoing the multiarterial technique had a higher incidence of pulmonary complications, and DSWI, where glycosylated hemoglobin &#8805;6.40%, had a greater influence on the infectious outcome than the choice of grafts.</p>

      <sec sec-type="intro">
        <title>Introduction</title>
        <p>Cardiovascular diseases are the leading cause of death in the world, and coronary artery disease has the greatest impact,<xref rid="B1" ref-type="bibr">1</xref>In this scenario, coronary artery bypass grafting (CABG) surgery has become a fundamental strategy, resulting in one of the most performed procedures in the world,<xref rid="B2" ref-type="bibr">2</xref> , <xref rid="B3" ref-type="bibr">3</xref> increasing long-term survival. Therefore, the patency of the chosen grafts becomes a fundamental point in decision-making.</p>
        <p>Current evidence has recommended the greater use of arterial grafts, especially for the second-best coronary artery. In this scenario, prospective studies with large samples were able to state that using a bilateral internal thoracic artery (ITA) is better than using a single ITA in the long term, even when revascularization is incomplete.<xref rid="B4" ref-type="bibr">4</xref> - <xref rid="B7" ref-type="bibr">7</xref> However, the difficulty of the graft harvesting technique and, consequently, the increase in surgical time make some surgeons choose to use venous grafts instead of arterial grafts;<xref rid="B6" ref-type="bibr">6</xref> , <xref rid="B8" ref-type="bibr">8</xref> , <xref rid="B9" ref-type="bibr">9</xref> at the same time, deep sternal wound infection (DSWI), increased intraoperative bleeding, and longer healing time, among others, stand out as negative points.<xref rid="B9" ref-type="bibr">9</xref>These reasons may have contributed to low adherence in practice.</p>
        <p>On the other hand, recent analyses showing the benefits of using a radial artery graft for the second-best coronary artery show a good alternative&#8217;s impact. Potential reasons would be reduced surgery time, decreased risk of DSWI, and excellent long-term patency associated with using calcium channel blockers.<xref rid="B10" ref-type="bibr">10</xref>Although its choice for different patient profiles, including people with diabetes, seems greater, some questions related to the characteristics of the target coronary artery and the degree of obstruction are evident.</p>
        <p>In this aspect, according to the results of the ART trial, patients treated with the multiple arterial grafts (MAG) option, regardless of whether it is bilateral or unilateral ITA with radial graft, had greater event-free survival when compared to the single arterial graft.<xref rid="B11" ref-type="bibr">11</xref>However, there are still controversies that the MAG option would increase the number of immediate complications.</p>
        <p>Thus, this study aimed to compare the immediate results regarding morbidity and mortality of patients undergoing CABG procedure with single arterial graft (SAG) versus multiple arterial grafts in the Registro Paulista de Cirurgia Cardiovascular II/S&#227;o Paulo Registry of Cardiovascular Surgery II (REPLICCAR II) project.</p>

        <title>Methods</title>
        <p>For this analysis, data from the REPLICCAR II were used. This is a prospective, observational, and multicenter registry with data from patients undergoing primary and isolated coronary artery bypass grafting surgery in 5 hospitals in the state of S&#227;o Paulo between July 2017 and June 2019. The collection of perioperative variables was online in a registry built on the Research Electronic Data Capture (REDCap) platform and dedicated to the project, which followed the definitions of version 2.9 of the Society of Thoracic Surgeons (STS) data collection system.</p>
        <p>The short-term results of using arterial grafts in CABG surgeries in hospitals in the state of S&#227;o Paulo were evaluated, while immediate results were compared using 1 arterial graft versus &#8805;2 arterial grafts concerning the STS outcomes. Two analyses were performed, one with the total base and the other after adjustment with the Propensity Score Matching (PSM) technique, which used the STS predicted risk of mortality, resulting in the matching of 1062 patients (531 in each group, single arterial graft vs. multiple arterial grafts). The flowchart of methodologies addressed is shown in <xref rid="f02001" ref-type="fig">Figure 1</xref> . <p>Perioperative and follow-up data (collected in person or by phone) performed 30 days after surgery (if previous hospital discharge) were included in the REDCap platform built for this project. Quality audits were carried out periodically to verify the accuracy, integrity, and consistency of the data.<xref rid="B13" ref-type="bibr">13</xref></p>
          <p>The outcome variables analyzed were: reintervention, DSWI, stroke, kidney injury, prolonged intubation (&gt;24 hours), short hospital stay (&lt;6 days), prolonged hospital stay (&gt;14 days), operative morbidity, and mortality.</p>
          <p>All variable definitions followed the criteria from STS Adult Cardiac Surgery Database version 2.9.<xref rid="B14" ref-type="bibr">14</xref>The morbidity variable was a composite endpoint that included the five major endpoints: stroke, acute kidney failure, prolonged intubation, DSWI, and reoperation. Operative mortality was defined as deaths that occurred after hospital discharge but before the 30thpostoperative day and deaths that occurred during the hospital stay in which the operation was performed, even after 30 days.</p>
          <p>All patients who underwent CABG surgery from July 2017 to 2019 in the 5 centers participating in this study were included in REPLICCAR II. Of this database (N=3122), 17% (N=531) received multiple arterial grafts. During the study, it was noticed that there was an increase in the use of radial and right thoracic artery grafts, as shown in Material Supplementary &#8211; <xref rid="f02001" ref-type="fig">Figure 1</xref> .</p>

          <title>Statistical analysis</title>
          <p>The R software version 4.0.2 was used to perform all of this study&#8217;s analyses.</p>
          <p>In the descriptive analysis, continuous variables were expressed as mean, median, standard deviation, and quartiles (25% and 75%), while categorical variables were expressed in frequencies and percentages. Due to missing data, the percentages were calculated as a function of the number (N) of responses obtained by variables instead of the total number of patients.</p>
          <p>Propensity Score Matching was used to match the groups using the &#8220;nearest&#8221; method, available in the Matchit package of R software, by the STS predicted risk of mortality of the patients analyzed, creating both paired groups with N=531 (single and multiple arterial grafts).</p>
          <p>For comparing two groups in continuous variables, the t-test was used for variables that followed the normal distribution (Anderson-Darling test), and non-parametric tests were used for the others. In the case of homogeneous variables, the Mann-Whitney test was used, and the Brunner-Munzel test was used for heterogeneous variables. Fisher&#8217;s exact or the Chi-Square test was used for categorical variables. The significance level adopted in the tests was 0.05. Two-tailed hypotheses were considered. Furthermore, the confidence interval constructed was 95%.</p>

        <title>Results</title>
        <p>The general database showed that the cardiopulmonary bypass time was significantly longer in the single arterial graft group (p=0.001), as well as the anoxia time (p=0.031); on the other hand, the total surgery time was longer in the multiple arterial graft group (p&lt;0.001). It is noticed that the profile patient in the SAG group had lower hematocrit (p&#8804;0.001) and hemoglobin (p&#8804;0.001) levels but a higher rate of blood transfusions during the procedure (p&#8804;0.001) compared to the MAG group. Intraoperatively, the radial artery was used in 30.32% of the patients who underwent MAG treatment, and the right internal thoracic artery in 79.28%, while the left internal thoracic artery was used in 100% of the cases; Thus, the left internal thoracic artery, the right internal thoracic artery and the radial artery were concurrently used in 9.06% of the patients (Material Supplementary &#8211; <xref rid="t2001" ref-type="table">Table 2</xref> ).</p>

        <p>The SAG treatment group had a significantly higher predicted risk than the MAG group at all assessments (p&lt;0.001), except for the prediction of hospital stay &lt;6 days (STS) (Material Supplementary &#8211; <xref rid="t1001" ref-type="table">Table 1</xref> ). Regarding the outcomes observed in the complete database (Material Supplementary &#8211; Table 3), in the single arterial graft group, there was an increase in acute kidney injury (p&#8804;0.001), new atrial fibrillation (p=0.006), sepsis (p=0.003), intraoperative and postoperative transfusion of blood products (p&#8804;0.001), need for intra-aortic balloon pump (p=0.008) and need for dialysis in the postoperative period (p=0.002), as well as increased length of hospital stay (&gt;14 days) (p=0.036) concerning the multiple arterial graft group.</p>
        <p>After the Propensity Score Matching, matched by the STS predicted mortality risk, outcome comparisons were performed between the 531 patients in both groups.</p>
        <p>Among the preoperative variables after PSM (Material Supplementary &#8211; Table 4), there was a higher prevalence of males in the multiple arterial graft group (p&lt;0.001). The patient profile showed that patients in the single arterial graft group had older age (p&lt;0.001), higher rate of diabetes mellitus (p=0.008), hypertension (p&lt;0.001), previous myocardial infarction &gt;21 days (p=0.004), and smoking (p=0.027). In contrast, the MAG group had a higher rate of surgeries with urgent status (p=0.022), family history of coronary heart disease (p=0.011), recent pneumonia (p=0.007), unstable angina or acute myocardial infarction with or without ST-segment elevation and symptoms at hospital admission (p=0.03). Despite the predicted risk of mortality being balanced between both groups, heterogeneity of risk probability was observed concerning the variables of evaluated outcomes.</p>
        <p>Concerning intraoperative findings ( <xref rid="t1001" ref-type="table">Table 1</xref> ), the single arterial graft group had slightly lower hematocrit and hemoglobin levels compared to the multiple arterial graft group (p=0.007 and p=0.006, respectively), which may have influenced a greater need for red blood cell transfusion (p=0.005). In the multiple arterial graft group, there was greater use of the skeletonized technique for dissection of the internal thoracic artery (p&lt;0.001) and duration of surgery (p&lt;0.001).</p>
        <p>Regarding postoperative results ( <xref rid="t2001" ref-type="table">table 2</xref> ), the multiple arterial graft group had a greater need for chest drainage due to pleural effusion (p=0.042), DSWI (p=0.007), pneumonia (p=0.01), need for reintubation (p=0.006), morbidity (p=0.028) and sternal debridement (p=0.015). However, there were lower rates of saphenous vein harvest site infection (p=0.002) and length of hospital stay &lt;6 days (p=0.036). No statistically significant differences were observed regarding postoperative stroke, kidney injury, intubation time, mortality, and length of stay &gt;14 days or mortality.</p>
        <p>Investigating the findings in the multiple arterial graft group, an analysis was carried out relating the glycosylated hemoglobin level between patients who developed DSWI versus those who did not. In the group of patients with MAG, glycosylated hemoglobin was significantly higher in the DSWI group than in the non-DSWI group (p=0.048). However, there was no difference between the associations of arterial grafts used, even in the subgroup with the bilateral internal thoracic artery ( <xref rid="f03001" ref-type="fig">Figure 2</xref> ).</p>

        <title>Discussion</title>
        <p>The debate over the pros and cons of using multiple arterial grafts persists, where the best long-term results<xref rid="B9" ref-type="bibr">9</xref>are confronted by higher infection rates, the technical difficulty that requires the surgeon&#8217;s experience, and the consequent increase in surgical time.<xref rid="B15" ref-type="bibr">15</xref> , <xref rid="B16" ref-type="bibr">16</xref> Our analysis is consistent with the literature and showed that most teams routinely prefer to use a single arterial graft in CABG surgeries (single arterial graft N=2591 vs. multi arterial graft N=531). In this cross-sectional study of the REPLICCAR II database, the group with MAG was associated with a higher rate of immediate pulmonary complications and DSWI.</p>
        <p>Regarding pulmonary outcomes, there was an increase in the rate of pleural effusion (p=0.042), postoperative pneumonia (p=0.01), and the need for reintubation (p=0.006); however, it is important to note that this same group also had a higher incidence of pneumonia in the preoperative period (p=0.007), which may have contributed to these outcomes, as well as the use of the open pleural technique for dissection of intern thoracic arteries, and the increase in surgical time (p&gt;0.001), consequently leading to longer anesthesia time,<xref rid="B17" ref-type="bibr">17</xref>however, it is important to emphasize that there was no impact on the mortality of this group and the time of hospitalization was reduced compared to the other analyzed group.</p>
        <p>Rocha et al.<xref rid="B18" ref-type="bibr">18</xref>retrospectively evaluated the evolution of patients who underwent CABG with 2 or &gt;2 arterial grafts; among its findings, the length of hospital stay between groups did not differ in the studied population (mean of 6 days in both groups). In our study, the multiple arterial graft group significantly reduced the length of hospital stay (p=0.036). This may be due to the greater availability of the patient to walk more efficiently in the postoperative period, resulting in the reduction of edema and pain, and stimulation of blood circulation, among others, which may have influenced a shorter hospital stay, considering that early ambulation is one of the most important items in contributing to the positive evolution of the patient, as recommended by protocols for rapid recovery after surgery.<xref rid="B19" ref-type="bibr">19</xref></p>
        <p>In this analysis, the single arterial graft group, even with a higher prevalence of diabetic patients (p=0.008) and a higher risk of DSWI (p=0.003), had a lower rate of DSWI (p=0.007) and debridement (p=0.015) than the group of patients with multiple arterial grafts, which is in line with the current literature.<xref rid="B15" ref-type="bibr">15</xref> , <xref rid="B16" ref-type="bibr">16</xref> We observed that glycosylated hemoglobin &gt;6.40% was determinant for the development of DSWI, regardless of the graft used in the MAG group (p=0.048). This could also have been influenced by the fact that in the multiarterial group, there was a higher rate of patients operated on with an urgent status (p=0.022) and with more advanced age (p&lt;0.001).</p>
        <p>Considering that high glycosylated hemoglobin influenced the development of DSWI in the multiple arterial graft group, the question arises regarding the current conduct of medical teams, where patients with diabetes or increased levels of glycosylated hemoglobin are managed for alternative methods for the use of bilateral ITA, such as radial artery and/or saphenous graft, to reduce the probability of an outcome of infection.<xref rid="B20" ref-type="bibr">20</xref> , <xref rid="B21" ref-type="bibr">21</xref> However, our findings show that to avoid DSWI, glycemic values need to be strictly corrected before, during, and after surgery, regardless of the choice of grafts. In this way, we believe that the most careful optimization of blood glucose<xref rid="B22" ref-type="bibr">22</xref> , <xref rid="B23" ref-type="bibr">23</xref> may bring more benefits to the patient than simply avoiding the use of bilateral ITA.</p>
        <p>Dorman et al.<xref rid="B24" ref-type="bibr">24</xref>retrospectively compared controlled and assisted diabetic patients after PSM who underwent SAG and MAG CABG surgery in a 30-year follow-up and showed that, even though the patients had diabetes, the use of bilateral ITA did not increase morbidity or mortality rates. Zhou et al.<xref rid="B25" ref-type="bibr">25</xref>also showed benefits in using the multiple arterial graft technique for properly controlled diabetic patients in their meta-analysis. These findings reinforce the theory put forward by the authors of this paper: the control of the patient&#8217;s blood glucose is more important than the choice of grafts.</p>
        <p>This study is a cross-sectional cohort in a database of patients undergoing CABG in the state of S&#227;o Paulo, where the impact of choosing the type of graft used did not follow a pre-surgery standardization, which may have affected the findings. An adjustment was made using the Propensity Score Matching technique based on the STS-predicted mortality risk in our prospective and robustly structured records to reduce biases. Therefore, the results need to be validated in future research, preferably in randomized studies, considered the gold standard for the analysis profile.</p>

        <title>Conclusion</title>
        <p>In the present analysis, it was identified that the use of multivessel grafts for patients undergoing coronary artery bypass graft surgery had a higher rate of pulmonary complications and deep chest wound infection, with glycosylated hemoglobin above 6.40% having a greater influence on the outcome of the than the choice of grafts itself. Strict blood glucose control is recommended in elective patients for glycated hemoglobin levels &#8805;6.40% and exhaustive control for patients operated under urgent conditions to avoid infectious outcomes that can significantly impact the patient&#8217;s evolution.</p>
