        <title>Resumo</title>
        <sec>
          <title>Fundamento</title>
          <p> A fibrila&#231;&#227;o atrial (FA) acomete cerca de 2% a 4% da popula&#231;&#227;o mundial. Nos pacientes internados em unidades de terapia intensiva, esta incid&#234;ncia pode chegar em at&#233; 23% naqueles com choque s&#233;ptico. O impacto da FA nos pacientes s&#233;pticos se reflete em piores desfechos cl&#237;nicos e o reconhecimento dos fatores desencadeantes pode ser alvo para estrat&#233;gias de tratamento e preven&#231;&#227;o futuras.</p>
        </sec>
        <sec>
          <title>Objetivos</title>
          <p> Verificar a rela&#231;&#227;o entre desenvolvimento de FA e mortalidade nos pacientes acima de 80 anos inclu&#237;dos no protocolo sepse e identificar fatores de risco que contribuam para o desenvolvimento de FA nesta popula&#231;&#227;o.</p>
        </sec>
        <sec>
          <title>M&#233;todos</title>
          <p> Estudo observacional retrospectivo, com revis&#227;o de prontu&#225;rios eletr&#244;nicos e inclus&#227;o de 895 pacientes com 80 anos ou mais, inclu&#237;dos no protocolo sepse de um hospital privado de alta complexidade em S&#227;o Paulo/SP, no per&#237;odo de janeiro de 2018 a dezembro de 2020. Todos os testes foram realizados com n&#237;vel de signific&#226;ncia de 5%.</p>
        </sec>
        <sec>
          <title>Resultados</title>
          <p> A incid&#234;ncia de FA na amostra foi de 13%. Ap&#243;s an&#225;lise multivariada por regress&#227;o log&#237;stica m&#250;ltipla, foi poss&#237;vel demonstrar associa&#231;&#227;o de mortalidade na popula&#231;&#227;o estudada, com o escore SOFA ( odds ratio [OR] 1,21 [1,09 &#8211; 1,35]), valores mais altos de prote&#237;na C-reativa (OR 1,04 [1,01 &#8211; 1,06]), necessidade de droga vasoativa (OR 2,4 [1,38 &#8211; 4,18]), uso de ventila&#231;&#227;o mec&#226;nica (OR 3,49 [1,82 &#8211; 6,71]) e principalmente FA (OR 3,7 [2,16 &#8211; 6,31).</p>
        </sec>
        <sec>
          <title>Conclus&#245;es</title>
          <p> No paciente grande idoso (80 anos ou mais) com sepse, o desenvolvimento de FA se mostrou como fator de risco independente para mortalidade intra-hospitalar.</p>

      <title>Introdu&#231;&#227;o</title>
      <p>A fibrila&#231;&#227;o atrial (FA) &#233; a arritmia card&#237;aca sustentada mais comum, acometendo cerca de 2% a 4% da popula&#231;&#227;o mundial, o que implica em grande morbimortalidade e altos custos para os servi&#231;os de sa&#250;de.<xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B2">2</xref> Dentre os pacientes que necessitam de interna&#231;&#227;o hospitalar por qualquer outro motivo, a FA segue como a arritmia card&#237;aca mais detectada, especialmente em pacientes cr&#237;ticos. O risco de desenvolvimento de FA em pacientes admitidos na unidade de terapia intensiva varia entre 4,5% e 11%, chegando at&#233; 23% naqueles com choque s&#233;ptico.<xref ref-type="bibr" rid="B3">3</xref> , <xref ref-type="bibr" rid="B4">4</xref></p>
      <p>Dentre os v&#225;rios fatores de risco estabelecidos para FA, talvez a idade seja o mais proeminente, e o aumento da longevidade da popula&#231;&#227;o deve produzir um crescente n&#250;mero de novos casos,<xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B2">2</xref> o que torna os idosos, principalmente os grandes idosos (aqueles com idade acima de 80 anos), mais suscet&#237;veis aos efeitos delet&#233;rios e riscos j&#225; conhecidos decorrentes da doen&#231;a, agravados inclusive por outras comorbidades, frequentes nesta popula&#231;&#227;o, e sua potencial fragilidade.</p>
      <p>Apesar de j&#225; existir uma rela&#231;&#227;o bem estabelecida entre idade e FA,<xref ref-type="bibr" rid="B5">5</xref> os mecanismos respons&#225;veis pelo desenvolvimento da arritmia em pacientes cr&#237;ticos n&#227;o s&#227;o totalmente compreendidos. Provavelmente decorrem de um remodelamento atrial acelerado em combina&#231;&#227;o com fatores desencadeantes da arritmog&#234;nese, habitualmente encontrados em doentes mais graves,<xref ref-type="bibr" rid="B4">4</xref> como inflama&#231;&#227;o, dist&#250;rbios hidroeletrol&#237;ticos e medica&#231;&#245;es pr&#243;-arr&#237;tmicas, dentre elas vasopressores e inotr&#243;picos.<xref ref-type="bibr" rid="B6">6</xref></p>
      <p>O impacto da FA nesta popula&#231;&#227;o se reflete em piores desfechos cl&#237;nicos, destacando-se por sua import&#226;ncia o aumento no tempo de interna&#231;&#227;o hospitalar, o que infere inclusive em custos, aumento no tempo de ventila&#231;&#227;o mec&#226;nica e suas consequ&#234;ncias, e maior mortalidade.<xref ref-type="bibr" rid="B7">7</xref> - <xref ref-type="bibr" rid="B10">10</xref> Entretanto, a sua import&#226;ncia no ambiente cr&#237;tico ainda n&#227;o est&#225; totalmente clara, ora funcionando como um determinante da piora do paciente, ora como um marcador de gravidade de doen&#231;as de base, podendo ser utilizada inclusive como um fator progn&#243;stico.<xref ref-type="bibr" rid="B3">3</xref> , <xref ref-type="bibr" rid="B6">6</xref></p>
      <p>Dentro ainda de v&#225;rias incertezas existentes no ambiente cr&#237;tico alusivas &#224; FA, a rela&#231;&#227;o entre a arritmia e sepse, principalmente em idosos, ainda suscita diversas teorias. Apesar de estudos crescentes sobre sepse na &#250;ltima d&#233;cada, poucos trabalhos abordaram sua rela&#231;&#227;o com FA em uma popula&#231;&#227;o muito idosa (80 anos ou mais). Estes pacientes talvez sejam os mais beneficiados em se manter em ritmo sinusal ou terem a FA revertida o mais breve poss&#237;vel, visto que s&#227;o habitualmente mais fr&#225;geis e t&#234;m menor reserva funcional.</p>
      <p>Diante do que foi posto anteriormente, torna-se importante e extremamente relevante um estudo que aborde o tema, trazendo informa&#231;&#245;es adicionais que possam contribuir com a literatura vigente. Baseado nisso, este trabalho tem como objetivo prim&#225;rio verificar a associa&#231;&#227;o de mortalidade intra-hopitalar com o desenvolvimento de FA nos pacientes com sepse e, entre os objetivos secund&#225;rios, identificar potenciais fatores de risco que contribuam para o desenvolvimento de FA nesta popula&#231;&#227;o e comparar o tempo de interna&#231;&#227;o entre os pacientes que desenvolveram FA e aqueles que permaneceram em ritmo sinusal.</p>

      <p>Trata-se de um estudo observacional retrospectivo, com coleta de dados secund&#225;rios a partir de prontu&#225;rios eletr&#244;nicos revisados de pacientes com 80 anos ou mais, inclu&#237;dos no protocolo sepse de um hospital privado de alta complexidade em S&#227;o Paulo/SP, no per&#237;odo de janeiro de 2018 a dezembro de 2020. <title>Resultados</title>
      <p>A m&#233;dia de idade da popula&#231;&#227;o estudada foi de 88,3 anos (&#177; 5,3), sendo 52,4% mulheres. Durante a interna&#231;&#227;o 118 pacientes evolu&#237;ram com FA, representando aproximadamente 13% do total de indiv&#237;duos acompanhados retrospectivamente ( <xref ref-type="fig" rid="f01">Figura 1</xref> ).</p>
      <p>Como esperado, os pacientes que desenvolveram FA tinham uma frequ&#234;ncia maior de insufici&#234;ncia card&#237;aca e FA pr&#233;via em seu hist&#243;rico, bem como um &#225;trio esquerdo aumentado e FEVE reduzida no ecocardiograma.</p>
      <p>Foi observado, entre os pacientes que evolu&#237;ram com FA, valores maiores do escore SOFA e n&#237;veis mais elevados de PCR.</p>
      <p>Estes pacientes com arritmia permaneceram mais tempo internados, necessitaram mais frequentemente de drogas vasoativas e ventila&#231;&#227;o mec&#226;nica. Todos os dados acima encontram-se detalhados na <xref ref-type="table" rid="t1">Tabela 1</xref> .</p>

      <title>Discuss&#227;o</title>
      <p>A incid&#234;ncia global de FA nova ou recorrente encontrada em nossa popula&#231;&#227;o, durante a interna&#231;&#227;o, foi de 13,2%. O que se percebe na literatura &#233; uma grande variabilidade de resultados, por exemplo, em uma metan&#225;lise conduzida por Kuipers et al.,<xref ref-type="bibr" rid="B14">14</xref> a incid&#234;ncia de FA em pacientes com sepse, sepse grave e choque s&#233;ptico foi respectivamente de 8%, 10% e 23%.<xref ref-type="bibr" rid="B15">15</xref> , <xref ref-type="bibr" rid="B16">16</xref> No estudo de Walkey et al.,<xref ref-type="bibr" rid="B17">17</xref> incluindo mais de 40 mil pacientes, mas apenas com sepse grave (classifica&#231;&#227;o antiga), foi encontrada uma incid&#234;ncia de 5,9% de FA nova. J&#225; em Meierhenrich et al.,<xref ref-type="bibr" rid="B9">9</xref> foram selecionados apenas pacientes com choque s&#233;ptico e foi encontrada uma incid&#234;ncia de 46% de FA nova, 10 vezes mais do que aqueles pacientes com sepse sem evolu&#231;&#227;o para choque. Entretanto, mais recentemente, a metan&#225;lise conduzida por Corica et al.,<xref ref-type="bibr" rid="B18">18</xref> demonstrou preval&#234;ncia de FA nova em pacientes s&#233;pticos de 13,5%, semelhante &#224; do presente estudo. Essa variabilidade pode ser explicada pelos in&#250;meros crit&#233;rios de inclus&#227;o e diferentes popula&#231;&#245;es abordadas. Este estudo, por sua vez, representa uma parcela muito espec&#237;fica da popula&#231;&#227;o idosa cuja incid&#234;ncia de FA &#233; maior, al&#233;m de n&#227;o diferenciar aqueles com sepse e choque s&#233;ptico.</p>
      <p>Dentre os fatores de risco que contribu&#237;ram para o desenvolvimento de FA durante a interna&#231;&#227;o, destacaram-se o hist&#243;rico pr&#233;vio de insufici&#234;ncia card&#237;aca e FA e achados ecocardiogr&#225;ficos que caracterizavam aumento do &#225;trio esquerdo e redu&#231;&#227;o da FEVE. Diversos trabalhos que abordaram o tema tamb&#233;m tiveram impress&#245;es semelhantes,<xref ref-type="bibr" rid="B4">4</xref> , <xref ref-type="bibr" rid="B6">6</xref> , <xref ref-type="bibr" rid="B15">15</xref> mas h&#225; controv&#233;rsias. Salman et al.,<xref ref-type="bibr" rid="B19">19</xref> analisando uma coorte prospectiva, n&#227;o demonstraram rela&#231;&#227;o entre a apresenta&#231;&#227;o de FA e o tamanho do &#225;trio esquerdo, apesar de associar a queda na FEVE com maior chance de evolu&#231;&#227;o para arritmia. J&#225; Shaver et al.,<xref ref-type="bibr" rid="B15">15</xref> em dire&#231;&#227;o oposta, conseguiram demonstrar associa&#231;&#227;o com o tamanho do &#225;trio esquerdo, mas n&#227;o com a FEVE. Apesar de n&#227;o ser alvo deste trabalho, ensaios laboratoriais com o pept&#237;deo natriur&#233;tico cerebral o colocam em posi&#231;&#227;o de marcador independente no desenvolvimento de FA, como cita Augusto et al.,<xref ref-type="bibr" rid="B20">20</xref> Isso mais uma vez corrobora que a insufici&#234;ncia card&#237;aca &#233; um preditor para o surgimento da arritmia tanto ambulatorialmente como no ambiente cr&#237;tico.<xref ref-type="bibr" rid="B21">21</xref> - <xref ref-type="bibr" rid="B23">23</xref></p>
      <p>Tamb&#233;m foi observado neste trabalho, entre os pacientes que evolu&#237;ram com FA, valores maiores do escore SOFA e n&#237;veis mais elevados de PCR. Hoje, o papel inflamat&#243;rio gerado pela sepse tem significativa import&#226;ncia no desenvolvimento e manuten&#231;&#227;o da FA. Steinber et al.,<xref ref-type="bibr" rid="B24">24</xref> destacam que o processo inflamat&#243;rio predisp&#245;e ao estresse oxidativo, apoptose e fibrose, gerando um substrato importante para desencadear a arritmia. Outro agravante nesse contexto &#233; o ambiente pr&#243;-tromb&#243;tico produzido pela inflama&#231;&#227;o, capaz de induzir disfun&#231;&#227;o endotelial, ativa&#231;&#227;o plaquet&#225;ria e a cascata de coagula&#231;&#227;o. Ambos os efeitos podem ser respons&#225;veis n&#227;o s&#243; pelo in&#237;cio e manuten&#231;&#227;o da FA, mas tamb&#233;m piorar os desfechos tromb&#243;ticos associados &#224; arritmia.<xref ref-type="bibr" rid="B24">24</xref> Estes achados foram refor&#231;ados por Harada et al.,<xref ref-type="bibr" rid="B25">25</xref> que tamb&#233;m associaram maiores valores de PCR e escore SOFA ao desenvolvimento de FA, inclusive a mortalidade, ap&#243;s an&#225;lise ajustada. Launey et al.,<xref ref-type="bibr" rid="B23">23</xref> tamb&#233;m encontraram valores maiores de escore de SOFA em pacientes que evolu&#237;ram com FA. Meierhenrich et al.,<xref ref-type="bibr" rid="B9">9</xref> evidenciaram que pacientes que desenvolveram FA, s&#233;pticos ou n&#227;o, apresentaram n&#237;veis mais altos de PCR antes do evento arr&#237;tmico. Chung et al.,<xref ref-type="bibr" rid="B26">26</xref> tamb&#233;m demonstraram associa&#231;&#227;o entre valores elevados de PCR e ocorr&#234;ncia e manuten&#231;&#227;o da FA. Nestes casos, o PCR elevado aponta para um estado inflamat&#243;rio que promove o desenvolvimento ou a persist&#234;ncia de FA.</p>
      <p>Inicialmente, sepse de foco abdominal ou outro (n&#227;o urin&#225;rio, pulmonar ou cut&#226;neo) foi associado a maior mortalidade, entretanto, na an&#225;lise multivariada, n&#227;o foi encontrada diferen&#231;a estat&#237;stica. Apesar de existirem dados que apontem maior incid&#234;ncia de FA com infec&#231;&#245;es do trato respirat&#243;rio e foco urin&#225;rio,<xref ref-type="bibr" rid="B10">10</xref> , <xref ref-type="bibr" rid="B14">14</xref> , <xref ref-type="bibr" rid="B16">16</xref> , <xref ref-type="bibr" rid="B23">23</xref> em nosso estudo, n&#227;o foi poss&#237;vel determinar a rela&#231;&#227;o entre os s&#237;tios de infec&#231;&#227;o e a incid&#234;ncia de FA.</p>
      <p>Al&#233;m de permanecerem mais tempo internados neste estudo, pacientes que desenvolveram FA tamb&#233;m necessitaram mais frequentemente de droga vasoativa e ventila&#231;&#227;o mec&#226;nica. O uso de ventila&#231;&#227;o mec&#226;nica j&#225; foi evidenciado como fator de risco para FA em diversos estudos,<xref ref-type="bibr" rid="B4">4</xref> , <xref ref-type="bibr" rid="B7">7</xref> , <xref ref-type="bibr" rid="B21">21</xref> mas ainda n&#227;o &#233; um consenso.<xref ref-type="bibr" rid="B15">15</xref> Em pacientes com sepse que evoluem para choque, a necessidade de droga vasoativa se torna imperativa pela falha na compensa&#231;&#227;o entre a demanda e a oferta de oxig&#234;nio aos tecidos, portanto, por si s&#243;, j&#225; &#233; um fator de pior progn&#243;stico. Esta instabilidade pode resultar em FA assim como a pr&#243;pria arritmia gera necessidade de doses mais altas de vasopressores. Esta rela&#231;&#227;o j&#225; foi reproduzida em alguns estudos.<xref ref-type="bibr" rid="B4">4</xref> , <xref ref-type="bibr" rid="B15">15</xref> , <xref ref-type="bibr" rid="B22">22</xref></p>
      <p>Diversos trabalhos anteriores falharam em encontrar uma associa&#231;&#227;o direta de FA nova ou recorrente com o desfecho de morte durante a interna&#231;&#227;o, mas os resultados encontrados aqui, sugerem que o desenvolvimento de FA em pacientes s&#233;pticos, nesta faixa et&#225;ria, tem forte impacto sobre a mortalidade intra-hospitalar. Tal achado vai de encontro a outros trabalhos recentes, incluindo metan&#225;lises de pacientes cr&#237;ticos, com sepse e/ou choque s&#233;ptico que desenvolveram FA nova, os quais exibiram resultados semelhantes.<xref ref-type="bibr" rid="B8">8</xref> , <xref ref-type="bibr" rid="B10">10</xref> , <xref ref-type="bibr" rid="B13">13</xref> , <xref ref-type="bibr" rid="B14">14</xref></p>
      <p>Houve sempre uma grande discuss&#227;o se a FA tinha um papel a desempenhar na evolu&#231;&#227;o ou desfecho da sepse, ou simplesmente refletia a severidade da doen&#231;a, como um marcador de gravidade. Como outros trabalhos referidos anteriormente aqui, a FA representou sim uma disfun&#231;&#227;o org&#226;nica que implicou em piora do desfecho cl&#237;nico. Diante do que foi exposto, a abordagem adequada da arritmia deve ser elevada a outro patamar de import&#226;ncia no contexto da evolu&#231;&#227;o deste grupo de pacientes, sendo necess&#225;rio o desenvolvimento de estrat&#233;gias adequadas de preven&#231;&#227;o e tratamento, visando redu&#231;&#227;o de danos.<xref ref-type="bibr" rid="B15">15</xref> Especificamente sobre este t&#243;pico, j&#225; existem resultados que mostraram que a falha na manuten&#231;&#227;o do ritmo sinusal em pacientes com sepse foi associada a piores taxas de mortalidade em compara&#231;&#227;o com aqueles pacientes que obtiveram sucesso na revers&#227;o da FA.<xref ref-type="bibr" rid="B9">9</xref> , <xref ref-type="bibr" rid="B16">16</xref></p>
      <p>Este estudo possui algumas limita&#231;&#245;es, como a defini&#231;&#227;o de FA, que foi baseada em registros cl&#237;nicos em prontu&#225;rios e, quando dispon&#237;veis, eletrocardiograma de 12 deriva&#231;&#245;es. Isto deixa espa&#231;o para poss&#237;veis epis&#243;dios de FA parox&#237;stica n&#227;o diagnosticada ou registrada pelo m&#233;dico respons&#225;vel. Tamb&#233;m foram inclu&#237;dos pacientes com FA recorrente, mas isto n&#227;o se mostrou como vi&#233;s de amostragem com implica&#231;&#227;o nos desfechos. Tal fato j&#225; tinha sido abordado em trabalhos realizados por Arrigo et al.,<xref ref-type="bibr" rid="B3">3</xref> e Shaver et al.,<xref ref-type="bibr" rid="B15">15</xref> que evidenciaram maiores taxas de mortalidade em pacientes com FA nova, provavelmente por menor toler&#226;ncia &#224;s altera&#231;&#245;es hemodin&#226;micas provocadas pela arritmia, diferente de pacientes com FA recorrente, melhor adaptados. Al&#233;m disso, por se tratar de estudo retrospectivo baseado em coleta de dados de prontu&#225;rios eletr&#244;nicos e, tamb&#233;m, por n&#227;o dispor da causa mortis definitiva dos pacientes que evolu&#237;ram a &#243;bito, s&#227;o necess&#225;rios novos estudos para elucida&#231;&#227;o dos resultados obtidos.</p>
    </sec>
    <sec sec-type="conclusions">
      <title>Conclus&#227;o</title>
      <p>No paciente grande idoso (80 anos ou mais) com sepse, o desenvolvimento de FA se mostrou como fator de risco independente para mortalidade intra-hospitalar. Devido &#224;s evid&#234;ncias, &#233; cada vez mais urgente abordar este tema nesta popula&#231;&#227;o em que a FA incide e tem seu maior impacto. Estes talvez sejam os mais beneficiados ao se manterem no ritmo sinusal ou terem a FA revertida o mais breve poss&#237;vel, visto que s&#227;o habitualmente mais fr&#225;geis e t&#234;m menor reserva funcional. Al&#233;m disso, a identifica&#231;&#227;o de fatores de risco associados &#224; FA no contexto cr&#237;tico pode servir para eventuais estrat&#233;gias de controle para preven&#231;&#227;o.</p>

        <title>Abstract</title>
        <sec>
          <title>Background</title>
          <p>Atrial fibrillation (AF) affects about 2% to 4% of the world population, and in patients hospitalized in intensive care units, this incidence can reach up to 23% in those with septic shock. The impact of AF in patients with sepsis is reflected in worse clinical outcomes, and the identification of the triggering factors can be a target for future prevention and treatment strategies.</p>
        </sec>
        <sec>
          <title>Objectives</title>
          <p>To verify the relationship between the development of AF and mortality in patients over 80 years of age included in the sepsis protocol and to identify the risk factors that contribute to the development of AF in this population.</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p>Retrospective observational study, with a review of electronic medical records and inclusion of 895 patients aged 80 years or older, included in the sepsis protocol of a high-complexity private hospital in S&#227;o Paulo, SP, from January 2018 to December 2020. All tests were performed with a significance level of 5%.</p>
        </sec>
        <sec>
          <title>Results</title>
          <p>The incidence of AF in the sample was 13%. After multivariate analysis, using multiple logistic regression, it was possible to demonstrate an association of mortality, in the studied population, with the SOFA score (odds ratio [OR] 1.21 [1.09 &#8211; 1.35]), higher values of C-reactive protein (OR 1.04 [1.01 &#8211; 1.06]), need for vasoactive drugs (OR 2.4 [1.38 &#8211; 4.18]), use of mechanical ventilation (OR 3.49 [1.82 &#8211; 6.71]), and mainly AF (OR 3.7 [2.16 &#8211; 6.31])</p>
        </sec>
        <sec>
          <title>Conclusion</title>
          <p>In very elderly patients (80 years of age and older) with sepsis, the development of AF was shown to be an independent risk factor for in-hospital mortality.</p>

      <sec sec-type="intro">
        <title>Introduction</title>
        <p>Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia, affecting approximately 2% to 4% of the world population, and it implies high morbidity and mortality, as well as high costs for health services.<xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B2">2</xref> Among patients requiring hospitalization for any other reason, AF remains the most detected cardiac arrhythmia, especially in critically ill patients. The risk of developing AF in patients admitted to the intensive care unit ranges from 4.5% to 11%, reaching up to 23% in those with septic shock.<xref ref-type="bibr" rid="B3">3</xref> , <xref ref-type="bibr" rid="B4">4</xref></p>
        <p>Among the several established risk factors for AF, age is perhaps the most prominent, and the increase in population longevity is expected to produce an increasing number of new cases,<xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B2">2</xref> which makes the elderly, especially the very elderly (those over 80 years of age), more susceptible to the deleterious effects and already known risks arising from the disease, aggravated further by other comorbidities, which are frequent in this population, and their potential fragility.</p>
        <p>Despite the existence of an already well-established relationship between age and AF,<xref ref-type="bibr" rid="B5">5</xref> the mechanisms responsible for the development of arrhythmia in critical patients are not yet fully understood. They probably result from accelerated atrial remodeling in combination with triggering factors for arrhythmogenesis, usually found in more severely ill patients, such as inflammation, hydroelectrolytic disorders, and pro-arrhythmic medications, including vasopressors and inotropes.<xref ref-type="bibr" rid="B6">6</xref></p>
        <p>The impact of AF in this population is reflected by worse outcomes, highlighting for its importance increased hospital stay, which also has an impact on costs, increased mechanical ventilation time and its consequences, and higher mortality.<xref ref-type="bibr" rid="B7">7</xref> - <xref ref-type="bibr" rid="B10">10</xref> However, its importance in the critical environment is still not completely clear, sometimes functioning as a determinant of patients&#8217; worsening, sometimes as a marker of the severity of underlying diseases, and it can even be used as a prognostic factor.<xref ref-type="bibr" rid="B3">3</xref> , <xref ref-type="bibr" rid="B6">6</xref></p>
        <p>Within the various uncertainties in the critical environment related to AF, the relationship between arrhythmia and sepsis, especially in the elderly, still raises several theories. Although studies on sepsis have increased in the last decade, few studies have addressed its relationship with AF in a very elderly population (80 years and over). These patients may benefit the most from maintaining sinus rhythm or having AF reversed as soon as possible, as they are usually more fragile and have less functional reserve.</p>
        <p>For these reasons, it is important and extremely relevant to conduct studies that address the topic, bringing additional information that can contribute to the current literature. Based on this, the primary objective of this study was to verify the association of in-hospital mortality with the development of AF in patients with sepsis and, as a secondary objective, to identify potential risk factors that contribute to the development of AF in this population and compare the duration of hospitalization between patients who developed AF and those who remained in sinus rhythm.</p>

        <title>Methods</title>
        <p>This is a retrospective observational study, with secondary data collection from reviewed electronic medical records of patients aged 80 years or older, included in the sepsis protocol of a high complexity private hospital in S&#227;o Paulo, SP, from January 2018 to December 2020. The study was approved by the institutional Research Ethics Committee by Hospital S&#237;rio Liban&#234;s, under CAAE protocol 47665721.9.0000.546.</p>
        <p>To define AF, cardiac rhythm data described in medical records and, when available, 12-lead electrocardiogram recordings were used.</p>
        <p>The sepsis protocol used at the institute, which was based on the definition of the 2016 Surviving Sepsis Campaign Guidelines (SEPSIS-3),<xref ref-type="bibr" rid="B11">11</xref> consisted of possible or probable focus of infection associated with two of the following systemic inflammatory response syndrome markers: heart rate &gt; 90 bpm, body temperature &gt; 38&#176;C or &lt; 36&#176;C, respiratory rate &gt; 20 bpm, leukocytes &gt; 12.000/mm3, or &lt; 4000/mm3; or at least one marker of organ dysfunction, characterized by: lactate &gt; 22 mg/dL, creatinine &gt; 2.0 mg/dL, bilirubin &gt; 2.0 mg/dL, international normalization index &gt; 1.5, activated thromboplastin time &gt; 60 seconds, or platelets &lt; 100.000/mm3.</p>
        <p>A total of 1339 medical records were eligible for inclusion, and 444 who had arrhythmia at admission or who had a cardiac pacemaker were excluded. There remained 895 patients admitted in sinus rhythm, among which 14.9% had previously presented paroxysmal AF ( <xref ref-type="fig" rid="f01001">Figure 1</xref> ).</p>

        <p>The following variables were analyzed: sex, age, body mass index, length of hospital stay, in-hospital mortality, associated comorbidities (systemic arterial hypertension, heart failure [HF], diabetes mellitus, stroke, chronic coronary arterial disease, previous AF, chronic kidney disease, obesity, or others [all comorbidities not mentioned]), previous use of antiarrhythmics, echocardiographic data such as left atrial size and left ventricular ejection fraction (LVEF), focus of sepsis, SOFA score (characterized by the sum of values assigned from 0 to 4 for each of the following variables: PaO2/FiO2ratio, platelet count, total bilirubin values, mean arterial pressure, Glasgow coma scale, creatinine levels or urine output),<xref ref-type="bibr" rid="B11">11</xref> C-reactive protein (CRP) value at admission, use of vasoactive drugs (noradrenaline, vasopressin, and/or dobutamine), and need for mechanical ventilation.</p>
        <p>The definition of HF was based on previous comorbidities described in medical records and/or on previous echocardiography exams of the patient demonstrating LVEF &lt;40% (Simpson or Teicholz), which, according to the Brazilian Guideline on Chronic and Acute Heart Failure, characterizes it as HF with reduced ejection fraction.<xref ref-type="bibr" rid="B12">12</xref> Enlarged left atrium was defined as linear measurement &gt; 40 mm (reference value between 28 and 40 mm). CRP results above 1 mg/dL, obtained by the ultrasensitive immunoturbidimetry method, should be interpreted as indicative of a possible infectious or inflammatory process.</p>

          <title>Statistical analysis</title>
          <p>Qualitative characteristics were described using absolute and relative frequencies, based on the development of AF, and the association of characteristics with groups was verified using chi-square or exact tests (Fisher&#8217;s exact test or likelihood ratio test).<xref ref-type="bibr" rid="B13">13</xref> Quantitative characteristics were described, according to the development of AF, using mean and standard deviation when data distribution was normal, or median and quartiles when data distribution was not normal. Normality was evaluated using the Kolmogorov-Smirnov test and compared using Student&#8217;s t-test (unpaired) or Mann-Whitney tests, respectively.</p>
          <p>The unadjusted odds ratios (OR) were estimated for each evaluated characteristic for the outcome using bivariate logistic regression, and the multiple logistic regression model was estimated, selecting the variables that in the bivariate tests presented significance levels below 0,20 (p &lt; 0.20), with all variables inserted in the model kept in the final model (full model).</p>
          <p>The analyses were performed using IBM-SPSS for Windows version 22.0 and tabulated using Microsoft-Excel 2010, and the tests were performed with a significance level of 5%.</p>

        <title>Results</title>
        <p>The mean age of the population studied was 88.3 years (&#177; 5.3), and 52.4% were women. During hospitalization, 118 patients developed AF, representing approximately 13% of the total number of individuals followed up retrospectively ( <xref ref-type="fig" rid="f01001">Figure 1</xref> ).</p>
        <p>As expected, patients who developed AF had a higher frequency of heart failure and previous AF in their history, as well as enlarged left atrium and reduced LVEF on echocardiogram.</p>
        <p>Among patients who developed AF, higher SOFA score values and higher levels of CRP were observed.</p>
        <p>These patients with arrhythmia remained hospitalized longer, and they required vasoactive drugs and mechanical ventilation more frequently. These data are detailed in <xref ref-type="table" rid="t1001">Table 1</xref> .</p>

        <title>Discussion</title>
        <p>The overall incidence of new or recurrent AF found in our population during hospitalization was 13.2%. The literature demonstrates a great variability of results; for instance, in a meta-analysis conducted by Kuipers et al.,<xref ref-type="bibr" rid="B14">14</xref> the incidence of AF in patients with sepsis, severe sepsis, and septic shock was respectively 8%, 10%, and 23%.<xref ref-type="bibr" rid="B15">15</xref> Walkey et al.,<xref ref-type="bibr" rid="B17">17</xref> including more than 40,000 patients, but only with severe sepsis (old classification), found an incidence of 5.9% for new AF. On the other hand, Meierhenrich et al.,<xref ref-type="bibr" rid="B9">9</xref> separating only patients with septic shock, found an incidence of 46% for new AF, 10 times more than patients with sepsis without evolution to shock. However, more recently, the meta-analysis conducted by Corica et al.<xref ref-type="bibr" rid="B18">18</xref> showed a 13.5% prevalence of new AF in patients with sepsis, similar to that of the present study. This variability can be explained by the numerous inclusion criteria and different populations addressed. This study, in turn, represents a very specific portion of the elderly population whose incidence of AF is higher, in addition to not differentiating between patients with sepsis and septic shock.</p>
        <p>Among the risk factors that contributed to the development of AF during hospitalization, the previous history of HF and AF and echocardiographic findings that characterized left atrial enlargement and LVEF reduction stood out. Several studies that addressed the topic also had similar findings,<xref ref-type="bibr" rid="B4">4</xref> , <xref ref-type="bibr" rid="B6">6</xref> , <xref ref-type="bibr" rid="B15">15</xref> but there are controversies. Salman et al.,<xref ref-type="bibr" rid="B19">19</xref> analyzing a prospective cohort, did not demonstrate a relationship between the presentation of AF and the size of the left atrium, despite associating a decrease in LVEF with a greater chance of progression to arrhythmia. Shaver et al.,<xref ref-type="bibr" rid="B15">15</xref> in contrast, managed to demonstrate an association with left atrial size, but not with LVEF. Although it was not the subject of this work, laboratory tests with brain natriuretic peptide place it as an independent marker in the development of AF, as mentioned by Augusto et al.<xref ref-type="bibr" rid="B20">20</xref> This once again confirms that HF is a predictor for the onset of arrhythmia, both on an outpatient basis and in a critical setting.<xref ref-type="bibr" rid="B21">21</xref> - <xref ref-type="bibr" rid="B23">23</xref></p>
        <p>In this study, among patients who developed AF, higher SOFA score values and higher CRP levels were also observed. Currently, the inflammatory role generated by sepsis is significant in the development and maintenance of AF. Steinber et al.<xref ref-type="bibr" rid="B24">24</xref> highlight that the inflammatory process predisposes to oxidative stress, apoptosis, and fibrosis, generating an important substrate to trigger arrhythmia. Another aggravating factor in this context is the prothrombotic environment produced by inflammation, capable of inducing endothelial dysfunction, platelet activation, and the coagulation cascade. Both effects may be responsible not only for the initiation and maintenance of AF, but also for worsening thrombotic outcomes associated with arrhythmia.<xref ref-type="bibr" rid="B24">24</xref> These findings were reinforced by Harada et al.,<xref ref-type="bibr" rid="B25">25</xref> who also associated higher CRP and SOFA scores with the development of AF, including mortality, after adjusted analysis. Launey et al.<xref ref-type="bibr" rid="B23">23</xref> also found higher SOFA scores in patients who developed AF. Meierhenrich et al.<xref ref-type="bibr" rid="B9">9</xref> showed that patients who developed AF, whether septic or not, had higher levels of CRP before the arrhythmic event. Chung et al.<xref ref-type="bibr" rid="B26">26</xref> also demonstrated an association between high CRP values and the occurrence and maintenance of AF. In these cases, elevated CRP points to an inflammatory state that promotes the development or persistence of AF.</p>
        <p>Initially, sepsis from an abdominal or other focus (non-urinary, pulmonary, or cutaneous) was associated with higher mortality; however, in the multivariate analysis, no statistical difference was found. Although there are data indicating a higher incidence of AF with respiratory tract infections and urinary focus,<xref ref-type="bibr" rid="B10">10</xref> , <xref ref-type="bibr" rid="B14">14</xref> , <xref ref-type="bibr" rid="B16">16</xref> , <xref ref-type="bibr" rid="B23">23</xref> in our study, it was not possible to determine the relationship between the sites of infection and the incidence of AF.</p>
        <p>In addition to longer hospital stay in this study, patients who developed AF also required vasoactive drugs and mechanical ventilation more often. The use of mechanical ventilation has already been evidenced as a risk factor for AF in several studies,<xref ref-type="bibr" rid="B4">4</xref> , <xref ref-type="bibr" rid="B7">7</xref> , <xref ref-type="bibr" rid="B21">21</xref> but there is still no consensus.<xref ref-type="bibr" rid="B15">15</xref> In patients with sepsis who progress to shock, vasoactive drugs become imperative due to the failure to compensate between the demand and supply of oxygen to the tissues; therefore, it is already a factor of worse prognosis. This instability can result in AF, just as the arrhythmia itself generates the need for higher doses of vasopressors. This relationship has already been reproduced in some studies.<xref ref-type="bibr" rid="B4">4</xref> , <xref ref-type="bibr" rid="B15">15</xref> , <xref ref-type="bibr" rid="B22">22</xref></p>
        <p>Several previous studies failed to find a direct association of new or recurrent AF with the outcome of death during hospitalization, but the results found here suggest that the development of AF in septic patients in this age group has a strong impact on in-hospital mortality. This finding is in agreement with other recent studies, including meta-analyses of critically ill patients with sepsis and/or septic shock who developed new AF, which showed similar results.<xref ref-type="bibr" rid="B8">8</xref> , <xref ref-type="bibr" rid="B10">10</xref> , <xref ref-type="bibr" rid="B13">13</xref> , <xref ref-type="bibr" rid="B14">14</xref></p>
        <p>There has always been great discussion as to whether AF plays a role in the course or outcome of sepsis, or simply reflects the severity of the disease, as a marker of severity. As in other previously mentioned studies, in this study, AF did represent an organic dysfunction that implied a worsening of the clinical outcome. Given the above, the appropriate approach to arrhythmia must be raised to another level of importance in the context of the evolution of this group of patients, requiring the development of adequate prevention and treatment strategies, with the aim of reducing health damage.<xref ref-type="bibr" rid="B15">15</xref> Specifically on this topic, there are already results showing that failure to maintain sinus rhythm in patients with sepsis was associated with worse mortality rates compared to patients whose AF was successfully reversed.<xref ref-type="bibr" rid="B9">9</xref> , <xref ref-type="bibr" rid="B16">16</xref></p>
        <p>This study has some limitations, such as the definition of AF, which was based on clinical records and, when available, 12-lead electrocardiogram. This leaves room for possible episodes of paroxysmal AF not diagnosed or recorded by the attending physician. Patients with recurrent AF (37.3%) were also included, but this was not shown to be a sampling bias with implications for outcomes. This fact had already been addressed in studies carried out by Arrigo et al.<xref ref-type="bibr" rid="B3">3</xref> and Shaver et al.,<xref ref-type="bibr" rid="B15">15</xref> showing higher mortality rates in patients with new AF, probably due to a lower tolerance of hemodynamic changes caused by arrhythmia, unlike patients with recurrent AF, who are better adapted. In addition, as this is a retrospective study based on data collection from electronic medical records and because the definitive causes of death of patients are unknown, further studies are needed to elucidate the results obtained.</p>

        <title>Conclusion</title>
        <p>In very elderly patients (80 years and over) with sepsis, the development of AF was shown to be an independent risk factor for in-hospital mortality. Due to the evidence, it is increasingly urgent to address this issue in this population, where AF has the greatest impact. These patients may benefit the most from maintaining sinus rhythm or having AF reversed as soon as possible, since they are usually more fragile and have less functional reserve. In addition, the identification of risk factors associated with AF in the critical context can serve for eventual control strategies for prevention.</p>
