  <body>
    <p>Prezado Editor,</p>
    <p>Foi com grande interesse que tivemos acesso ao documento intitulado &#8220;Diretrizes da Sociedade Brasileira de Cardiologia sobre a An&#225;lise e Emiss&#227;o de Laudos Eletrocardiogr&#225;ficos &#8211; 2022&#8221;, publicado na Edi&#231;&#227;o de setembro dos Arquivos Brasileiros de Cardiologia (ABC),<xref ref-type="bibr" rid="B1">1</xref> elaborado por um grupo de especialistas brasileiros no assunto. &#201; nossa opini&#227;o que uma atualiza&#231;&#227;o se fazia necess&#225;ria, tendo sido a mesma detalhada e de agrad&#225;vel leitura.</p>
    <p>No entanto, chamamos aten&#231;&#227;o para duas informa&#231;&#245;es apresentadas sobre a interpreta&#231;&#227;o do eletrocardiograma do atleta que nos trouxeram preocupa&#231;&#227;o. A primeira se encontra na se&#231;&#227;o &#8220;Sobrecargas das C&#226;maras Card&#237;acas&#8221; (P&#225;gina 18), na qual os autores descrevem que o Crit&#233;rio de Sokolow-Lyon N&#195;O DEVE ser utilizado para avalia&#231;&#227;o da sobrecarga ventricular esquerda (SVE) EM ATLETAS.</p>
    <p>Discordamos dessa afirmativa e nos baseamos para tanto no International criteria for electrocardiographic interpretation in athletes: Consensus statement,<xref ref-type="bibr" rid="B2">2</xref>-<xref ref-type="bibr" rid="B4">4</xref> do qual o autor s&#234;nior desta carta foi um dos autores. Nesse documento, publicado simultaneamente em tr&#234;s peri&#243;dicos de alto impacto,<xref ref-type="bibr" rid="B2">2</xref>-<xref ref-type="bibr" rid="B4">4</xref> o Crit&#233;rio de Sokolow-Lyon (soma da onda S em V1 + onda R em V5 ou V6 &gt;3,5 mV) serve sim para definir SVE nesse grupo de indiv&#237;duos treinados (Tabela 1 do documento).<xref ref-type="bibr" rid="B2">2</xref> &#201; fato que embora existam v&#225;rios crit&#233;rios para definir a SVE em atletas, o de Sokolow-Lyon ainda &#233; o mais utilizado. Cabe salientar que uma s&#233;rie de estudos que avaliaram o eletrocardiograma do atleta tamb&#233;m utilizaram esse crit&#233;rio para avaliar tal sobrecarga.<xref ref-type="bibr" rid="B5">5</xref>-<xref ref-type="bibr" rid="B12">12</xref></p>
    <p>Neste ponto, nos permitimos fazer uma suposi&#231;&#227;o: Talvez os autores da &#8220;Diretrizes da Sociedade Brasileira de Cardiologia sobre a An&#225;lise e Emiss&#227;o de Laudos Eletrocardiogr&#225;ficos &#8211; 2022&#8221;<xref ref-type="bibr" rid="B1">1</xref> quiseram dizer que o crit&#233;rio de Sokolow-Lyon n&#227;o deve ser usado isoladamente para definir SVE em atletas, visto que estas altera&#231;&#245;es s&#227;o consideradas variantes da normalidade nesse cen&#225;rio? Contudo, ao escrever &#8220;n&#227;o deve ser utilizado em atletas&#8221;, nos parece que tal assertiva pode causar confus&#227;o ao leitor, trazendo uma informa&#231;&#227;o incorreta.</p>
    <p>A segunda preocupa&#231;&#227;o que veio &#224; tona ao lermos o manuscrito diz respeito &#224; se&#231;&#227;o 12.1.2 (P&#225;gina 29), na qual os autores das Diretrizes colocam um complexo QRS com dura&#231;&#227;o &#8805;160 ms como anormal. De fato, eles t&#234;m raz&#227;o. No entanto e em contraste, os crit&#233;rios internacionais para interpreta&#231;&#227;o do eletrocardiograma do atleta<xref ref-type="bibr" rid="B2">2</xref> destacam uma dura&#231;&#227;o do QRS como sendo anormal j&#225; quando &#8805;140 ms (Figura 1 do documento),<xref ref-type="bibr" rid="B2">2</xref> e n&#227;o somente ao alcan&#231;ar a dura&#231;&#227;o &#8805;160 ms.</p>
    <p>Por fim, &#233; importante destacar que os mesmos autores das recentes Diretrizes Brasileiras de Eletrocardiograma<xref ref-type="bibr" rid="B1">1</xref> fazem refer&#234;ncia aos crit&#233;rios internacionais para interpreta&#231;&#227;o do eletrocardiograma do atleta<xref ref-type="bibr" rid="B2">2</xref> em seu documento. Dessa forma, acreditamos que essas corre&#231;&#245;es pontuais por n&#243;s propostas possam ser consideradas, sem que isso desmere&#231;a em nada o importante e certamente &#225;rduo trabalho realizado na confec&#231;&#227;o dessa Diretriz.</p>

        <title>Dear Editor,</title>
        <p>It was with great interest that we had access to the document entitled &#8220;Brazilian Society of Cardiology Guidelines on the Analysis and Issuance of Electrocardiographic Reports &#8211; 2022&#8221;,<xref ref-type="bibr" rid="B1">1</xref> developed by Brazilian experts on the subject. It is our opinion that an update was necessary, as it is detailed and pleasant to read.</p>
        <p>However, we draw attention to two pieces of information presented about the interpretation of the athlete&#8217;s electrocardiogram that caused us concern. The first is found in the section &#8220; Cardiac Chambers Hypertrophy&#8221; (Page 18), in which the authors describe that the Sokolow-Lyon Criteria SHOULD NOT be used to determine left ventricular hypertrophy (LVH) IN ATHLETES.</p>
        <p>We disagree with this statement and based it on the International criteria for electrocardiographic interpretation in athletes: Consensus statement,<xref ref-type="bibr" rid="B2">2</xref>-<xref ref-type="bibr" rid="B4">4</xref> of which the senior author of this letter was one of the authors. In this document, published simultaneously in three journals with high impact factor,<xref ref-type="bibr" rid="B2">2</xref>-<xref ref-type="bibr" rid="B4">4</xref> the Sokolow-Lyon Criteria (S wave in V1 + R wave in V5 or V6 &gt;3.5 mV) does serve to define LVH in this group of trained individuals (Table 1 of the document).<xref ref-type="bibr" rid="B2">2</xref> It is a fact that although there are several criteria to define LVH in athletes, Sokolow-Lyon is still the most used. It should be noted that a series of studies that evaluated the electrocardiogram in athletes also used this criterion to assess such hypertrophy.<xref ref-type="bibr" rid="B5">5</xref>-<xref ref-type="bibr" rid="B12">12</xref></p>
        <p>At this point, we allow ourselves to make an assumption: could it be that the authors of the &#8220;Brazilian Society of Cardiology Guidelines on the Analysis and Issuance of Electrocardiographic Reports &#8211; 2022&#8221;,<xref ref-type="bibr" rid="B1">1</xref> meant that the Sokolow-Lyon criteria should not be used alone to define LVH in athletes, since these alterations are considered normal variants in this scenario? However, when writing &#8220;should not be used in athletes&#8221;, it seems to us that such an assertion could cause confusion to the reader, bringing incorrect information.</p>
        <p>The second concern that came to the fore when reading the manuscript relates to section 12.1.2 (Page 29), where the Guidelines authors place a QRS complex duration &#8805;160 ms as abnormal. Indeed, they are right. However, in contrast, the International criteria for electrocardiographic interpretation in athletes<xref ref-type="bibr" rid="B2">2</xref> highlight a QRS duration as being abnormal even when &#8805;140 ms (Figure 1 of the document),<xref ref-type="bibr" rid="B2">2</xref> and not only when reaching a duration &#8805;160 ms.</p>
        <p>Finally, it is important to highlight that the same authors of the recent Brazilian Guidelines of Electrocardiographic<xref ref-type="bibr" rid="B1">1</xref> make reference to the International criteria for electrocardiographic interpretation in athletes<xref ref-type="bibr" rid="B2">2</xref> in their document. Thus, we believe that these specific corrections proposed by us can be considered, without this in any way detracting from the important and certainly hard work carried out in the preparation of these Guidelines.</p>
