    </disp-quote>
    <p>O ensino e a pr&#225;tica m&#233;dica brasileiros sofrem de in&#250;meras limita&#231;&#245;es, entre as quais destaca-se a deficiente forma&#231;&#227;o cient&#237;fica. Ultimamente surgiu a figura do m&#233;dico-pesquisador como elemento capaz de suprir, em parte, tal defici&#234;ncia. Aqui procuro avaliar seu papel.</p>
    <p>A medicina tradicional ocupa-se da aplica&#231;&#227;o do conhecimento m&#233;dico. Portanto, lida com as caracter&#237;sticas gerais das doen&#231;as, isto &#233;, sintomas, evolu&#231;&#227;o, comprometimento fisiopatol&#243;gico de &#243;rg&#227;os e sistemas, etiologia, m&#233;todos diagn&#243;sticos e terap&#234;utica.</p>
    <p>Requer estruturas hospitalares, ambulatoriais e laboratoriais cl&#225;ssicas que permitam fazer testes diagn&#243;sticos como imagens e perfil bioqu&#237;mico. No exerc&#237;cio da cl&#237;nica n&#227;o se permite experimenta&#231;&#227;o. O exerc&#237;cio da profiss&#227;o m&#233;dica baseia-se na aplica&#231;&#227;o judiciosa do conhecimento atual.</p>
    <p>Uma das grandes dificuldades nesse processo &#233; que as doen&#231;as n&#227;o ocorrem como puros fen&#244;menos biol&#243;gicos, bioqu&#237;micos ou f&#237;sicos. Ao contr&#225;rio, como as doen&#231;as ocorrem nas pessoas, suas manifesta&#231;&#245;es s&#227;o dependentes da individualidade de cada um. As escolas m&#233;dicas procuram mostrar aos alunos o estado atual das enfermidades e como elas s&#227;o investigadas e tratadas. Ocorre que as verdades biom&#233;dicas mudam com a evolu&#231;&#227;o do conhecimento, e isto esta ocorrendo atualmente com grande velocidade dado a evolu&#231;&#227;o das pesquisas e do conhecimento m&#233;dico. Um aspecto essencial do ensino m&#233;dico &#233; o conceito de que se deve n&#227;o apenas ensinar o que se sabe hoje, mas ensinar o aluno a pensar criticamente para poder julgar as novidades publicadas e aplicar apenas conhecimentos bem estabelecidos com base em pesquisas de alto n&#237;vel. Evidentemente, se o professor, ele mesmo, n&#227;o vivenciar este conceito, n&#227;o poder&#225; transmiti-lo aos alunos. Portanto, a qualidade da pr&#225;tica m&#233;dica depende n&#227;o apenas do ensino, mas tamb&#233;m das estruturas do sistema de sa&#250;de como um todo.</p>
    <p>A qualidade do ensino m&#233;dico atual no Brasil &#233; inadequada. Por exemplo, o exame do Conselho Regional de Medicina do Estado de S&#227;o Paulo (CREMESP), que &#233; feito voluntariamente por alunos ao final do curso m&#233;dico, mostra um &#237;ndice de aprova&#231;&#227;o de apenas 50%.<xref ref-type="bibr" rid="B1">1</xref> Ora, o CREMESP est&#225; restrito ao estado de S&#227;o Paulo, que tem justamente algumas das melhores escolas m&#233;dicas do pa&#237;s. Pode-se imaginar qual seria o resultado de exame semelhante se fosse aplicado a todas as escolas m&#233;dicas do pa&#237;s, muitas das quais n&#227;o t&#234;m estrutura t&#233;cnica nem corpo docente qualificado. A cria&#231;&#227;o desordenada de escolas m&#233;dicas nos &#250;ltimos anos &#233; causa de imensa preocupa&#231;&#227;o entre os profissionais dedicados ao ensino m&#233;dico. Evidentemente, pode-se esperar que grande n&#250;mero de m&#233;dicos pouco qualificados passe a exercer a profiss&#227;o nos pr&#243;ximos anos. Isso representa uma amea&#231;a ao atendimento m&#233;dico da popula&#231;&#227;o em geral.</p>
    <p>Para complicar, o n&#250;mero de hospitais de ensino &#233; notoriamente insuficiente. Assim, das 353 escolas m&#233;dicas apenas 69 (19,5%) t&#234;m hospitais de ensino.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref> Al&#233;m disso, os hospitais de ensino, em geral, n&#227;o possuem estruturas para treinamento em pesquisa.</p>
    <p>Em resumo, pode-se concluir que o ensino m&#233;dico no Brasil, em geral, &#233; de baixa qualidade. Por extens&#227;o a pr&#225;tica m&#233;dica &#233; tamb&#233;m de baixo n&#237;vel. Como n&#227;o h&#225; nenhum controle externo cada m&#233;dico exerce a profiss&#227;o segundo suas pr&#243;prias convic&#231;&#245;es. Da&#237; resultam aplica&#231;&#245;es &#8220;off-label&#8221; e in&#250;meros procedimentos question&#225;veis, tais como uso excessivo de exames laboratoriais e de imagens e prescri&#231;&#245;es inadequadas de medicamentos sem comprova&#231;&#227;o de efic&#225;cia. Isso, obviamente, onera o sistema de sa&#250;de sem vantagens para os pacientes. &#201; neste contexto que se insere o m&#233;dico-pesquisador, tanto de forma&#231;&#227;o em ci&#234;ncia b&#225;sica quanto cl&#237;nica.</p>
    <p>Para come&#231;ar, a forma&#231;&#227;o do pesquisador requer uma formula&#231;&#227;o especial. O pesquisador precisa entender de metodologia cient&#237;fica lato sensu. Especificamente, precisa compreender a import&#226;ncia dos modelos experimentais, sejam de bancada, experimenta&#231;&#227;o animal ou cl&#237;nicos. Registros baseados em prontu&#225;rios m&#233;dicos eletr&#244;nicos, com grande n&#250;mero de pacientes, s&#227;o hoje amplamente utilizados como complementa&#231;&#227;o aos estudos randomizados, e representam o chamado &#8220;mundo real&#8221;.<xref ref-type="bibr" rid="B4">4</xref> O pesquisador precisa entender princ&#237;pios b&#225;sicos de estat&#237;stica, que hoje se tornou uma disciplina por si s&#243;, tornando-se indispens&#225;vel. Precisa compreender bem a quest&#227;o de vieses diversos, como os de sele&#231;&#227;o de amostras e a presen&#231;a de fatores de confus&#227;o, sobretudo em estudos observacionais. &#201; obrigat&#243;rio o entendimento da adequa&#231;&#227;o das amostras e modelos para a resposta de perguntas espec&#237;ficas. Um exemplo muito ilustrativo &#233; o de Eric Kandel,<xref ref-type="bibr" rid="B5">5</xref> que no seu magn&#237;fico livro Em busca da mem&#243;ria relata que passou quatorze meses em Paris, somente estudando as caracter&#237;sticas da lesma marinha Aplysia, que se revelou o modelo ideal para seus estudos de mem&#243;ria. Ele ganhou o Pr&#234;mio Nobel de Medicina e Fisiologia em 2000, pelas suas descobertas sobre a mem&#243;ria. Al&#233;m disso, tecnologias modernas como biologia molecular, gen&#233;tica e epigen&#233;tica, randomiza&#231;&#227;o mendeliana, v&#225;rias t&#233;cnicas de imagem de &#243;rg&#227;os, tecidos e estruturas intracelulares como mol&#233;culas e sinaliza&#231;&#227;o paracelular e intracelular, acrescentaram recentemente instrumentos poderosos para a cria&#231;&#227;o de conhecimento.</p>
    <p>Diferentemente do que se observa na pr&#225;tica cl&#237;nica, os resultados de pesquisas s&#227;o em geral de longo prazo, podendo consumir anos ou at&#233; a vida inteira do investigador. Em resumo, a forma&#231;&#227;o do pesquisador &#233; um aprendizado em servi&#231;o, que envolve muitas disciplinas, e cuja finalidade prec&#237;pua &#233; a busca da verdade e a cria&#231;&#227;o de novos conhecimentos.</p>
    <p>A relev&#226;ncia do m&#233;dico-pesquisador fica clara dentro do escopo de associar cl&#237;nica e pesquisa; algu&#233;m que tenha vis&#227;o conjunta, integrada dos grandes problemas m&#233;dicos. Isso tem a ver com efici&#234;ncia, relev&#226;ncia, inova&#231;&#227;o e a no&#231;&#227;o de custo/benef&#237;cio. No entanto, &#233; preciso conhecimento m&#237;nimo da parte do m&#233;dico-pesquisador sobre aspectos essenciais da medicina de hoje: gen&#233;tica e epigen&#233;tica, biologia molecular, biologia vascular, sistemas de sinaliza&#231;&#227;o celulares e intracelulares, sistemas redox entre outros. Especificamente cabe a este: a) analisar criticamente os resultados de pesquisas sejam experimentais ou aplicadas, com o objetivo de verificar se os procedimentos empregados s&#227;o de alta qualidade e, portanto, confi&#225;veis. Exemplos recentes de publica&#231;&#245;es sem base cient&#237;fica s&#243;lida, s&#227;o os m&#250;ltiplos estudos relativos ao tratamento medicamentoso da Covid-19; v&#225;rias propostas terap&#234;uticas se revelaram totalmente in&#243;cuas ou at&#233; prejudiciais, causando s&#233;rios transtornos no tratamento dessa pandemia.<xref ref-type="bibr" rid="B6">6</xref> Parte desse problema se deve &#224;s publica&#231;&#245;es &#8220;pr&#233;-print&#8221;, sem aval de uma revis&#227;o por pares. Casos similares s&#227;o os de vitamina E,<xref ref-type="bibr" rid="B7">7</xref> reposi&#231;&#227;o hormonal<xref ref-type="bibr" rid="B8">8</xref> e antioxidantes.<xref ref-type="bibr" rid="B9">9</xref> cujas hip&#243;teses de benef&#237;cios n&#227;o se comprovaram; b) interpretar e divulgar os achados de investiga&#231;&#245;es para o cl&#237;nico, j&#225; salientando os pr&#243;s e contras desses achados; c) gerar protocolos relevantes que possam ser desenvolvidos no nosso meio; d) criar os meios adequados para o desenvolvimento de pesquisas, de novas tecnologias ou a implementa&#231;&#227;o de desenvolvimentos recentes. Esta talvez seja a miss&#227;o mais importante do m&#233;dico-pesquisador. Hospitais, com exce&#231;&#227;o dos de ensino, s&#227;o normalmente preparados para atendimento m&#233;dico. Portanto n&#227;o t&#234;m estruturas pr&#243;prias para pesquisas. Habitualmente gestores administrativos est&#227;o mais preocupados com finan&#231;as e efici&#234;ncia do complexo hospitalar. No entanto, conceitos modernos indicam que ao praticar investiga&#231;&#245;es cl&#237;nicas, a qualidade do atendimento m&#233;dico melhora. Assim, associar atendimento e pesquisas &#233; uma maneira de aprender com a pr&#243;pria experi&#234;ncia e maximizar os efeitos do trabalho. &#201; uma maneira de se evitar erros repetitivos, pela avalia&#231;&#227;o cr&#237;tica e constante dos procedimentos de uma institui&#231;&#227;o. Esta &#233; uma tend&#234;ncia da medicina moderna.</p>
    <p>Estas fun&#231;&#245;es do m&#233;dico-pesquisador, s&#227;o essenciais na medicina translacional,<xref ref-type="bibr" rid="B10">10</xref> compreendendo desde a divulga&#231;&#227;o de achados b&#225;sicos, passando pela elucida&#231;&#227;o de mecanismos fisiopatol&#243;gicos e s&#237;ndromes cl&#237;nicas e sobretudo encurtando o per&#237;odo entre descobertas de ci&#234;ncias b&#225;sicas e sua aplica&#231;&#227;o cl&#237;nica. Portanto, a atua&#231;&#227;o do m&#233;dico-pesquisador &#233; essencial para o exerc&#237;cio qualificado da profiss&#227;o m&#233;dica, bem como para o desenvolvimento cient&#237;fico do pa&#237;s.</p>
    <p>Em conclus&#227;o, o m&#233;dico-pesquisador pode exercer papel fundamental no aperfei&#231;oamento do ensino e da pr&#225;tica m&#233;dica. Seu conhecimento do m&#233;todo cient&#237;fico pode contribuir para o exerc&#237;cio de uma medicina criteriosa, baseada em investiga&#231;&#245;es realmente qualificadas.</p>
    <p>Felizmente, temos observado nos &#250;ltimos anos, no Brasil um n&#250;mero crescente de m&#233;dicos-pesquisadores. Por&#233;m, estes n&#250;meros s&#227;o ainda insuficientes. Portanto, devemos incentivar inicia&#231;&#245;es cient&#237;ficas e cursos de p&#243;s-gradua&#231;&#227;o qualificados no Brasil e no exterior e sobretudo adequar escolas m&#233;dicas para n&#227;o apenas exercer seu papel tradicional mas tamb&#233;m criar estruturas de pesquisa.</p>

      </disp-quote>
      <p>Medical education and practice in Brazil suffer from countless limitations, among which lacking scientific training stands out. Recently, the physician-researcher has emerged as a figure capable of partially correcting this gap. In this paper, I will seek to evaluate the physician-researcher's role.</p>
      <p>Conventional medicine is dedicated to the application of medical knowledge. Therefore, it deals with the general characteristics of diseases, namely, symptoms, evolution, pathophysiological impairment of organs and systems, etiology, diagnostic and therapeutic methods.</p>
      <p>This requires conventional hospital, outpatient, and laboratory structures that make it possible to conduct diagnostic tests, such as imaging and biochemical profile. Within clinical practice, experimenting is not permitted. Practice of the medical profession is based on the judicious application of current knowledge.</p>
      <p>One of the main difficulties in this process is that diseases do not occur as purely biological, biochemical, or physical phenomena. On the contrary, because diseases occur in people, their manifestations are dependent on the individuality of each person. Medical schools seek to show students the current state of illnesses and how they are investigated and treated. It so happens that biomedical truths change as knowledge evolves, and this is currently taking place at an amazing speed, given the evolution of research and medical knowledge. An essential aspect of medical education is the concept that we must not only teach what is known today, but also teach students to think critically in order to be able to judge published reports and to apply only well-established knowledge that is based on high-level research. Evidently, if educators themselves do not incorporate this concept, they will not be able to transmit it to students. Therefore, the quality of medical practice depends not only on teaching, but also on the structures of the health system as a whole.</p>
      <p>The quality of medical education in Brazil is currently inadequate. For example, the Regional Council of Medicine of the State of S&#227;o Paulo (CREMESP) exam, which is taken voluntarily by students at the end of their medical course, shows an approval rate of only 50%.<xref ref-type="bibr" rid="B1">1</xref> CREMESP is limited to the state of S&#227;o Paulo, which has some of the best medical schools in Brazil. It is possible to imagine what the results of a similar exam would be, were it applied to all Brazilian medical schools, many of which do not possess technical structures or qualified teaching staff. The unprecedent opening of new medical schools in the past years has been a cause of immense concern among professionals dedicated to medical education. Evidently, it is possible to expect that a large number of underqualified doctors will begin practicing the profession in the coming years. This poses a threat to the medical care of the general population.</p>
      <p>To complicate matters further, the number of teaching hospitals is notoriously insufficient. Thus, of the 353 medical schools, only 69 (19.5%) have teaching hospitals.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref> Moreover, teaching hospitals, in general, do not have structures for research training.</p>
      <p>To summarize, it is reasonable to conclude that medical education in Brazil is, in general, of low quality. By extension, medical practice is also low-level. As there is no external control, each doctor practices the profession according to their own convictions. This results in &#8220;off-label&#8221; applications and numerous questionable procedures, such as excessive use of laboratory and imaging tests and inappropriate medication prescriptions with no proof of efficacy. This obviously overloads the health system, without any benefits for patients. It is in this context that physician-researchers are positioned, regarding both training in basic science and clinical practice.</p>
      <p>To begin with, a researcher's training requires a special formulation. Researchers need to understand lato sensu scientific methodology. Specifically, they need to understand the importance of experimental models, whether benchside, animal, or clinical. Registries based on electronic medical records, with large numbers of patients, are now widely used to complement randomized studies, and they represent the so-called &#8220;real world&#8221;.<xref ref-type="bibr" rid="B4">4</xref> Researchers need to understand the basic principles of statistics, which has become a mandatory discipline on its own. They require a deep understanding of the issue of different biases, such as sample selection and the presence of confounding factors, especially in observational studies. It is mandatory to understand the adequacy of samples and models to answer specific questions. A very illustrative example is that of Eric Kandel,<xref ref-type="bibr" rid="B5">5</xref> who in his magnificent book In Search of Memory reports that he spent fourteen months in Paris, exclusively studying the characteristics of the Aplysia sea slug, which proved to be the ideal model for his studies of memory. He received the Nobel Prize in Medicine or Physiology in 2000 for his discoveries on memory. In addition, modern technologies, such as molecular biology, genetics and epigenetics, Mendelian randomization, as well as various imaging techniques for organs, tissues, and intracellular structures such as molecules and paracellular and intracellular signaling have recently added powerful tools for knowledge creation.</p>
      <p>Differently from clinical practice, research results are generally long-term, and they may take years or even a lifetime. A researcher's training is an apprenticeship in service, which involves many disciplines, whose foremost purpose is the search for the truth and the creation of new knowledge.</p>
      <p>The physician-researcher's relevance is evident within the scope of associating clinical practice and research to create a joint, integrated view of major medical problems. This is related to efficiency, relevance, innovation, and the notion of cost/benefit. However, a certain amount of knowledge is required on the part of the physician-researcher on essential aspects of contemporary medicine: genetics and epigenetics, molecular biology, vascular biology, cellular and intracellular signaling systems, redox systems, among others. They are specifically responsible for: a) critically analyzing the results of research, whether experimental or applied, with the objective of verifying that the procedures employed are of high quality and, therefore, reliable. Recent examples of publications without a solid scientific basis are the multiple studies related to medical treatment of Covid-19; several therapeutic proposals turned out to be totally innocuous or even harmful, causing serious disorder in the treatment of this pandemic.<xref ref-type="bibr" rid="B6">6</xref> Part of this problem is due to &#8220;pre-print&#8221; publications, which are not backed by peer review. Similar cases are those of vitamin E,<xref ref-type="bibr" rid="B7">7</xref> hormone replacement,<xref ref-type="bibr" rid="B8">8</xref> and antioxidants,<xref ref-type="bibr" rid="B9">9</xref> whose hypothetical benefits have not been proven; b) interpreting and disseminating the findings of investigations to clinicians, highlighting the pros and cons of these findings; c) generating relevant protocols that can be developed in our setting; d) creating adequate means for the development of research and new technology or for the implementation of recent developments. This is perhaps the most important mission of physician-researchers. Hospitals, with the exception of teaching hospitals, are normally prepared for medical care; therefore, they do not have their own research structures. Administrative managers are usually more concerned with the finances and efficiency of the hospital complex. However, modern concepts indicate that, when they practice clinical investigations, the quality of medical care improves. Accordingly, associating care and research is a means of learning from their own experience and maximizing the effects of their work. It is a way to avoid repetitive errors, through constant and critical evaluation of an institution's procedures. This is a trend in modern medicine.</p>
      <p>These functions of the physician-researcher are essential in translational medicine,<xref ref-type="bibr" rid="B10">10</xref> ranging from the dissemination of basic findings to the elucidation of pathophysiological mechanisms and clinical syndromes and, above all, shortening the period between discoveries of basic sciences and their clinical application. Therefore, the role of the physician-researcher is essential to qualified practice of the medical profession, as well as to the scientific development of the country.</p>
      <p>In conclusion, physician-researchers can play a fundamental role in improving teaching and medical practice. Their knowledge of the scientific method can contribute to judicious medical practice, based on truly qualified investigations.</p>
      <p>Fortunately, in recent years, we have observed an increasing number of physician-researchers in Brazil. However, these numbers are still insufficient. Therefore, we must encourage scientific initiation and qualified postgraduate courses in Brazil and abroad and, above all, adapt medical schools not only to exercise their traditional role, but also to create research structures.</p>
