    <p>A doen&#231;a cardiovascular ateroscler&#243;tica ainda &#233; uma das principais causas de morbidade e mortalidade em todo o mundo e seus fatores de risco j&#225; foram identificados. Dislipidemia, hipertens&#227;o arterial, tabagismo, diabetes e adiposidade est&#227;o frequentemente presentes, principalmente em associa&#231;&#227;o, em pacientes com doen&#231;a arterial coronariana (DAC).<xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B2">2</xref> Identificar pacientes com risco de desenvolver DAC e apresentar s&#237;ndromes coronarianas agudas &#233; de grande valia, e muitos escores foram criados com esse objetivo, como o escore de risco de Framingham, o escore ASSIGN e o escore de risco QRISKR2.</p>
    <p>O escore CHA2DS2VASc, que compreende insufici&#234;ncia card&#237;aca congestiva (C), hipertens&#227;o (H), idade 75 (A2), diabetes (D), acidente vascular cerebral ou ataque isqu&#234;mico transit&#243;rio (S2), doen&#231;a vascular (V), idade 65-74 anos (A) e sexo masculino (Sc), foi originalmente desenvolvido para prever o risco de acidente vascular cerebral em pacientes com fibrila&#231;&#227;o atrial. Recentemente, o escore CHA2DS2VASc tamb&#233;m foi associado a eventos adversos maiores em pacientes com infarto do mioc&#225;rdio com supradesnivelamento do segmento ST (IAMCSST),<xref ref-type="bibr" rid="B3">3</xref> s&#237;ndromes coron&#225;rias agudas sem supradesnivelamento do segmento ST<xref ref-type="bibr" rid="B4">4</xref> , <xref ref-type="bibr" rid="B5">5</xref> e em pacientes com doen&#231;a isqu&#234;mica cr&#244;nica est&#225;vel.<xref ref-type="bibr" rid="B6">6</xref> Al&#233;m disso, Tasolar et al.,<xref ref-type="bibr" rid="B4">4</xref> e Chua et al.,<xref ref-type="bibr" rid="B5">5</xref> demonstraram sua associa&#231;&#227;o com a gravidade da doen&#231;a arterial coronariana.</p>
    <p>O escore de SYNTAX residual (rSS) foi projetado para quantificar a incompletude da revasculariza&#231;&#227;o ap&#243;s interven&#231;&#227;o coron&#225;ria percut&#226;nea (ICP), calculando o escore SYNTAX (Synergy Between PCI With Taxus and Cardiac Surgery) restante, uma medida angiogr&#225;fica quantitativa de gravidade e complexidade anat&#244;mica ap&#243;s uma ICP. Um rSS alto foi associado a um progn&#243;stico ruim em 30 dias e 1 ano.<xref ref-type="bibr" rid="B7">7</xref></p>
    <p>Neste n&#250;mero dos Arquivos Brasileiros de Cardiologia, Kalkan et al.,<xref ref-type="bibr" rid="B8">8</xref> demonstraram pela primeira vez a associa&#231;&#227;o entre o escore CHA2DS2VASc e rSS em 688 pacientes submetidos &#224; ICP prim&#225;ria ap&#243;s IAMCSST.<xref ref-type="bibr" rid="B8">8</xref> Embora pare&#231;a &#243;bvio encontrar uma rela&#231;&#227;o entre um escore que inclua alguns dos principais fatores de risco para aterosclerose (CHA2DS2VASc) e a pr&#243;pria carga ateroscler&#243;tica (rSS), chama a aten&#231;&#227;o para a discuss&#227;o relevante e contempor&#226;nea sobre a import&#226;ncia da gravidade da doen&#231;a ateroscler&#243;tica.Possivelmente, a carga ateroscler&#243;tica &#233; o mecanismo por tr&#225;s da associa&#231;&#227;o do escore CHA2DS2VASc e eventos cardiovasculares encontrados em muitos estudos anteriores.<xref ref-type="bibr" rid="B3">3</xref> , <xref ref-type="bibr" rid="B5">5</xref> , <xref ref-type="bibr" rid="B6">6</xref></p>
    <p>Diretrizes internacionais sugerem a documenta&#231;&#227;o de isquemia antes de procedimentos invasivos eletivos para tratamento de doen&#231;a arterial coronariana, seja por eletrocardiograma de esfor&#231;o, ecocardiograma de estresse, cintilografia de perfus&#227;o mioc&#225;rdica ou resson&#226;ncia magn&#233;tica card&#237;aca. Al&#233;m disso, testes funcionais invasivos, como reserva de fluxo fracionada, s&#227;o recomendados antes da revasculariza&#231;&#227;o se a isquemia n&#227;o invasiva n&#227;o for demonstrada.<xref ref-type="bibr" rid="B9">9</xref> Controversamente, a superioridade das avalia&#231;&#245;es anat&#244;micas sobre os testes de isquemia tem sido repetidamente demonstrada em diferentes cen&#225;rios. Em um estudo randomizado de 3.283 pacientes, Singh et al.,<xref ref-type="bibr" rid="B10">10</xref> mostraram que a angiotomografia computadorizada (TC) teve uma associa&#231;&#227;o mais forte com doen&#231;a card&#237;aca coron&#225;ria de 5 anos, morte ou infarto do mioc&#225;rdio (IAM) n&#227;o fatal do que o eletrocardiograma de esfor&#231;o.<xref ref-type="bibr" rid="B10">10</xref> Da mesma forma, Hoffmann et al.,<xref ref-type="bibr" rid="B11">11</xref> demonstraram em um estudo randomizado de 9.102 pacientes que a angiotomografia teve uma capacidade discriminat&#243;ria maior do que o teste funcional na previs&#227;o de eventos cardiovasculares.<xref ref-type="bibr" rid="B11">11</xref> Em uma suban&#225;lise do estudo COURAGE, a carga anat&#244;mica avaliada pela angiografia coronariana foi um preditor consistente de morte, IAM e s&#237;ndromes coronarianas agudas sem supradesnivelamento do segmento ST, enquanto a carga isqu&#234;mica n&#227;o foi.<xref ref-type="bibr" rid="B12">12</xref> Uma suban&#225;lise recente do estudo ISCHEMIA revelou que a gravidade da DAC, avaliada pela angiotomografia, foi um preditor altamente significativo de mortalidade por todas as causas, morte cardiovascular e IAM, tanto espont&#226;neo quanto periprocedimento; novamente, a gravidade da isquemia n&#227;o foi associada a eventos adversos.<xref ref-type="bibr" rid="B13">13</xref></p>
    <p>A l&#243;gica por tr&#225;s dos testes anat&#244;micos serem superiores aos testes de isquemia &#233; simples. O IAM espont&#226;neo ocorre quando uma placa obstrutiva ou n&#227;o obstrutiva se rompe; os testes funcionais identificam apenas placas obstrutivas, enquanto a angiografia, invasiva ou por TC, identifica placas grandes e pequenas, que podem erodir ou romper, causando IAM e possivelmente a morte. Consequentemente, se voc&#234; tem mais placas, tem maior chance de instabilidade em uma delas e maior risco de eventos cardiovasculares. Um benef&#237;cio te&#243;rico da cirurgia de revasculariza&#231;&#227;o mioc&#225;rdica sobre a ICP baseia-se na ideia de que o enxerto protege longos segmentos de placas coron&#225;rias proximais, e o paciente estaria protegido de IAM se qualquer uma dessas se rompe, enquanto os stents protegem apenas o segmento com stent.<xref ref-type="bibr" rid="B14">14</xref></p>
    <p>Em conclus&#227;o, Kalkan et al.,<xref ref-type="bibr" rid="B8">8</xref> demonstraram que pacientes com maior escore CHA2DS2VASc tem maior rSS, o que significa maior gravidade da DAC. Com base em evid&#234;ncias recentes, esses pacientes podem, consequentemente, estar em maior risco de ruptura da placa e eventos cardiovasculares, como infarto do mioc&#225;rdio ou morte e, portanto, se beneficiariam de terapias estabilizadoras de placa mais agressivas. Ao prever a carga ateroscler&#243;tica, o escore CHA2DS<xref ref-type="bibr" rid="B2">2</xref> VASc pode ser mais uma ferramenta para predi&#231;&#227;o de risco em pacientes com DAC.</p>

    <body>
      <p>Atherosclerotic cardiovascular disease is still a major cause of morbidity and mortality worldwide, and its risk factors have already been identified. Dyslipidemia, high blood pressure, cigarette smoking, diabetes and adiposity are frequently present, mostly in combination, in patients with coronary artery disease (CAD).<xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B2">2</xref> Identifying patients at risk for developing CAD and presenting acute coronary syndromes is of great value, and many scores were created with this aim, such as the Framingham risk score, the ASSIGN score and the QRISKR2 risk score.</p>
      <p>CHA2DS2VASc score, which comprises congestive heart failure (C), hypertension (H), age &#8805; 75 (A2), diabetes (D), stroke or transient ischemic attack (S2), vascular disease (V), age 65-74 years (A) and male gender (Sc), was originally developed to predict stroke risk in patients with atrial fibrillation. Recently, the CHA2DS2VASc score was also associated with major adverse events in patients with ST-segment elevation myocardial infarction (STEMI)<xref ref-type="bibr" rid="B3">3</xref> non-ST elevation acute coronary syndromes<xref ref-type="bibr" rid="B4">4</xref> , <xref ref-type="bibr" rid="B5">5</xref> and in patients with chronic stable ischemic disease.<xref ref-type="bibr" rid="B6">6</xref> Moreover, Tasolar et al.<xref ref-type="bibr" rid="B4">4</xref> and Chua et al.<xref ref-type="bibr" rid="B5">5</xref> demonstrated its association with coronary artery disease severity.</p>
      <p>Residual syntax score (rSS) was designed to quantify the incompleteness of revascularization after percutaneous coronary intervention (PCI), calculating the remaining SYNTAX (Synergy Between PCI With Taxus and Cardiac Surgery) score, a quantitative angiographic measure of anatomic severity and complexity after a PCI. A high rSS was associated with a poor 30-day and 1-year prognosis.<xref ref-type="bibr" rid="B7">7</xref></p>
      <p>In this issue of Arquivos Brasileiros de Cardiologia , Kalkan et al.<xref ref-type="bibr" rid="B8">8</xref> demonstrated for the first time the association between CHA2DS2VASc score and rSS in 688 patients who underwent primary PCI after STEMI.<xref ref-type="bibr" rid="B8">8</xref> Although it seems obvious to find a relationship between a score that includes some of the main risk factors for atherosclerosis (CHA2DS2VASc) and the atherosclerotic burden itself (rSS), it brings our attention to the relevant and contemporary discussion about the importance of the severity of atherosclerosis. Possibly, the atherosclerotic burden is the mechanism behind the association of CHA2DS2VASc score and cardiovascular events found in many previous studies<xref ref-type="bibr" rid="B3">3</xref> , <xref ref-type="bibr" rid="B5">5</xref> , <xref ref-type="bibr" rid="B6">6</xref></p>
      <p>International guidelines suggest the documentation of ischemia before elective invasive procedures to treat coronary artery disease, either by exercise electrocardiogram testing, stress echocardiography, single-photon emission computed tomography or cardiac magnetic resonance. Moreover, invasive functional testing, such as fractional flow reserve, is recommended before revascularization if non-invasive ischemia is not demonstrated.<xref ref-type="bibr" rid="B9">9</xref> Controversially, the superiority of anatomic assessments over ischemia testing has been repeatedly demonstrated in different scenarios. In a randomized trial of 3283 patients, Singh et al.<xref ref-type="bibr" rid="B10">10</xref> showed that computed tomography (CT) angiography had a stronger association with 5-year coronary heart disease, death, or non-fatal myocardial infarction (MI) than exercise electrocardiogram.<xref ref-type="bibr" rid="B10">10</xref> Similarly, Hoffmann et al.<xref ref-type="bibr" rid="B11">11</xref> demonstrated in a randomized trial of 9102 patients that CT angiography had a greater discriminatory ability than functional testing in predicting cardiovascular events<xref ref-type="bibr" rid="B11">11</xref> . In a sub-analysis of the COURAGE trial, anatomic burden assessed by coronary angiography was a consistent predictor of death, MI and non-ST segment elevation acute coronary syndromes, whereas ischemic burden was not.<xref ref-type="bibr" rid="B12">12</xref> A recent sub-analysis of the ISCHEMIA trial revealed that CAD severity, evaluated by CT angiography, was a highly significant predictor of all-cause mortality, cardiovascular death and MI, both spontaneous and periprocedural; again, ischemia severity was not associated with adverse events.<xref ref-type="bibr" rid="B13">13</xref></p>
      <p>The rationale behind anatomic tests being superior to ischemia testing is simple. Spontaneous MI occurs when either an obstructive or non-obstructive plaque ruptures; functional tests identify only obstructive plaques, whereas angiography, invasive or CT, identifies big and small plaques, which can erode or rupture, causing MI and possibly death. Consequently, if you have more plaques, you have a bigger chance of instability in one of those and a higher risk of cardiovascular events. One theoretical benefit of coronary artery bypass graft over PCI relies on the idea that the graft bypasses long segments of proximal coronary plaques, and the patient would be protected from MI if any of those ruptures, whereas the stents protect only the stented segment.<xref ref-type="bibr" rid="B14">14</xref></p>
      <p>In conclusion, Kalkan et al.<xref ref-type="bibr" rid="B8">8</xref> demonstrated that patients with higher CHA2DS2VASc score have higher rSS, which means higher CAD severity. Based on recent evidence, those patients might consequently be at higher risk of plaque rupture and cardiovascular events, such as MI or death, and therefore would benefit from more aggressive plaque stabilizing therapies. By predicting atherosclerotic burden, the CHA2DS2VASc score might be another tool for risk prediction in patients with CAD.</p>
