      Caro Editor,
    </p>
    <p>O estudo de Paiva et al. avaliou a incid&#234;ncia de pacientes infectados pelo v&#237;rus COVID-19, associado a doen&#231;as cardiovasculares (DCV), no Brasil. No estudo, conclui- se que a alta preval&#234;ncia de s&#237;ndrome respirat&#243;ria aguda grave (SRAG) em adultos e idosos relaciona-se com caracter&#237;sticas sociodemogr&#225;ficas, cl&#237;nicas, sinais e sintomas. Visto isso, reitera-se a import&#226;ncia da aten&#231;&#227;o prim&#225;ria &#224; sa&#250;de &#8211; a fim da manuten&#231;&#227;o das consultas peri&#243;dicas, visando controle da doen&#231;a e sintomatologia, ao passo que a presen&#231;a de comorbidades cardiovasculares aumenta em casos severos de COVID-19. <xref rid="B1" ref-type="bibr">1</xref> </p>
    <p>No estudo, a popula&#231;&#227;o estudada foi de 116.343 pacientes, dos quais 61,9% foram diagnosticados por SRAG causada por COVID-19. Ao mesmo tempo, o estudo demonstra que a presen&#231;a de doen&#231;as cr&#244;nicas pode ser considerada fator de risco &#224; infec&#231;&#227;o por COVID-19, devido a maiores vulnerabilidade e morbimortalidade. Sendo assim, pacientes com DCV pr&#233;via s&#227;o mais predisponentes a desenvolverem quadros mais graves. Entretanto, no sexo feminino, observou-se uma menor preval&#234;ncia de SRAG por COVID-19 devido ao fato de haver varia&#231;&#227;o entre a resposta imunol&#243;gica e a susceptibilidade a infec&#231;&#245;es virais entre os sexos, o que gera, assim, diferen&#231;as na gravidade e evolu&#231;&#227;o da doen&#231;a. <xref rid="B1" ref-type="bibr">1</xref> </p>
    <p>Em Wuhan, na China, uma metan&#225;lise com 46.248 pacientes infectados analisou as comorbidades mais prevalentes, estando a DCV (5&#177;4%) em terceiro lugar. Foram avaliados, por Wang e colaboradores, 2020, somente pacientes hospitalizados acometidos pela infec&#231;&#227;o viral, os quais apresentaram maior preval&#234;ncia &#8211; 19,6% &#8211; de DCV, um dado refor&#231;a o fato de que a comorbidade DCV contribui para o agravo da gravidade da COVID-19, visto a evidente necessidade de interna&#231;&#227;o. Al&#233;m disso, os pacientes demonstraram evolu&#231;&#227;o com maior hipoxemia e imprescind&#237;vel interna&#231;&#227;o em UTIs. <xref rid="B2" ref-type="bibr">2</xref> O estudo realizado por Melo evidenciou resultados semelhantes a achados de um estudo na It&#225;lia &#8211; ambos analisados no decorrer de sete dias no m&#234;s de mar&#231;o de 2020; constatou-se decr&#233;scimo de 13% de pacientes com infarto agudo do mioc&#225;rdio (IAM) quando associados na mesma semana do ano de 2019. Por outro lado, mesmo ocorrendo uma redu&#231;&#227;o de casos de IAM e da taxa de n&#250;meros de &#243;bitos hospitalares, houve um aumento na taxa de letalidade intra-hospitalar nas interna&#231;&#245;es por DCV. Ambos os estudos demonstraram a rela&#231;&#227;o da COVID-19 com a alta taxa de preval&#234;ncia de les&#245;es card&#237;acas e um grande potencial de gravidade da COVID-19 nas DCV, em que a letalidade em pacientes internados com DCV alcan&#231;ou parcela mais economicamente ativa da popula&#231;&#227;o &#8211; de 20 a 59 anos. <xref rid="B3" ref-type="bibr">3</xref> </p>
    <p>Visto isso, destaca-se a import&#226;ncia do acompanhamento m&#233;dico a pacientes portadores de doen&#231;as cr&#244;nicas, j&#225; que, segundo Askin et al., no ano de 2020, houve aumento acentuado no dano do mioc&#225;rdio de pacientes com COVID-19, aumentando o risco de morbimortalidade. Dessa forma, a valoriza&#231;&#227;o da DCV como complica&#231;&#227;o associada ao v&#237;rus COVID-19, devido ao agravamento da sintomatologia da doen&#231;a, &#233; de extrema signific&#226;ncia para a aten&#231;&#227;o b&#225;sica &#224; sa&#250;de. <xref rid="B4" ref-type="bibr">4</xref> </p>
    <p>A presente situa&#231;&#227;o exige estrat&#233;gias que visem a preven&#231;&#227;o de complica&#231;&#245;es associadas a doen&#231;as cr&#244;nicas, como DCV. Portanto, os dados atuais demonstram a necessidade de aten&#231;&#227;o especial aos pacientes do grupo de risco assim como um manejo adequado das complica&#231;&#245;es cardiovasculares, visando uma r&#225;pida identifica&#231;&#227;o e aplica&#231;&#227;o de tratamento adequado. Ademais, recomenda-se aos portadores de DCV a atualiza&#231;&#227;o das vacinas devido ao risco de infec&#231;&#227;o bacteriana secund&#225;ria pelo SARS-CoV-2, assim como a ades&#227;o &#224; dieta adequada, sono regular e atividade f&#237;sica, evitando o tabagismo e etilismo. <xref rid="B2" ref-type="bibr">2</xref> </p>

      <p>Agradecemos aos autores pela an&#225;lise submetida &#8220;Preval&#234;ncia e Fatores Associados &#224; SRAG por COVID-19 em Adultos e Idosos com Doen&#231;a Cardiovascular Cr&#244;nica: Uma An&#225;lise Cr&#237;tica&#8221;, em especial pela exposi&#231;&#227;o de dados que refor&#231;am os achados apresentados em nosso estudo. <xref rid="B5" ref-type="bibr">1</xref> Destaca-se tamb&#233;m a relev&#226;ncia da an&#225;lise em rela&#231;&#227;o ao aumento das taxas de letalidade intra-hospitalar nas interna&#231;&#245;es por doen&#231;as cardiovasculares (DCV) como fator potencializador da necessidade de valoriza&#231;&#227;o deste grupo de risco na an&#225;lise das complica&#231;&#245;es associadas &#224; COVID-19. <xref rid="B6" ref-type="bibr">2</xref>, <xref rid="B7" ref-type="bibr">3</xref> </p>
      <p>As DCV j&#225; s&#227;o estabelecidas como marcadores de risco na aten&#231;&#227;o prim&#225;ria &#224; sa&#250;de (APS) brasileira, com o intuito de garantir a longitudinalidade do cuidado ao usu&#225;rio e o desenvolvimento de estrat&#233;gias preventivas voltadas ao cuidado integral da comunidade, <xref rid="B8" ref-type="bibr">4</xref> por&#233;m &#233; essencial o questionamento quanto &#224; aplicabilidade destas a&#231;&#245;es na estrat&#233;gia de sa&#250;de da fam&#237;lia (ESF). O desafio do monitoramento ao usu&#225;rio como forma de gest&#227;o &#224;s DCV sugere ultrapassar o imperativo das a&#231;&#245;es que apoiam o sistema diagn&#243;stico e terap&#234;utico no cuidado e manejo &#224;s condi&#231;&#245;es cr&#244;nicas. <xref rid="B9" ref-type="bibr">5</xref> Neste sentido, um estudo publicado recentemente, com mais de 100 milh&#245;es de participantes de todo o mundo, destaca a import&#226;ncia da preven&#231;&#227;o, da detec&#231;&#227;o e do controle na aten&#231;&#227;o prim&#225;ria, inclusive em pa&#237;ses de baixa e m&#233;dia renda, com propostas de a&#231;&#245;es como uso de mensagens de texto para acompanhar os usu&#225;rios com fatores de risco. <xref rid="B10" ref-type="bibr">6</xref> </p>
      <p>No contexto atual de enfrentamento &#224; pandemia da COVID-19, esta discuss&#227;o torna-se ainda mais relevante, pois, conforme refor&#231;ado pelos autores que realizaram a an&#225;lise cr&#237;tica, o agravamento da sintomatologia da COVID-19 coloca as DCV como um fator de risco importante &#224;s a&#231;&#245;es de rastreio, manejo e interven&#231;&#227;o precoce.</p>
      <p>Estudos sugerem a cria&#231;&#227;o de propostas de modelos de cuidados &#224;s condi&#231;&#245;es cr&#244;nicas, como as DCV, envolvendo forma&#231;&#227;o de grupos focais, que obtiveram resultados positivos na ades&#227;o da comunidade, melhora de indicadores cl&#237;nicos durante o acompanhamento, ado&#231;&#227;o de pr&#225;tica de autocuidado e melhor an&#225;lise das prioridades no planejamento em sa&#250;de. <xref rid="B10" ref-type="bibr">6</xref> De forma similar, a Organiza&#231;&#227;o Mundial de Sa&#250;de (OMS) lan&#231;ou um guia de implementa&#231;&#227;o para gest&#227;o de DCV na aten&#231;&#227;o prim&#225;ria, com propostas de triagem, avalia&#231;&#227;o e manejo de riscos neste n&#237;vel de aten&#231;&#227;o e educa&#231;&#227;o em sa&#250;de para profissionais quanto ao rastreio de fatores de risco, interven&#231;&#245;es no estilo de vida e encaminhamentos necess&#225;rios. <xref rid="B11" ref-type="bibr">7</xref> Especialmente acerca do manejo em usu&#225;rios com DCV e sintomas persistentes p&#243;s-COVID-19, a complexidade cl&#237;nica e psicossocial proporcionada pela coexist&#234;ncia dessas condi&#231;&#245;es implica a necessidade de um cuidado multiprofissional e de longo prazo. Finalizamos, assim, agradecendo &#224; possibilidade de refor&#231;ar a necessidade de se rever as a&#231;&#245;es na aten&#231;&#227;o prim&#225;ria &#224; sa&#250;de, em especial em nosso atual contexto de sa&#250;de.</p>

        Dear Editor,
      </p>
      <p>The study by Paiva et al. evaluated the incidence of patients infected with the COVID-19 virus, associated with cardiovascular diseases (CVD), in Brazil. It is concluded that the high prevalence of severe acute respiratory syndrome (SARS) in adults and in the elderly is related to sociodemographic and clinical characteristics, signs and symptoms. In view of that, the importance of primary health care is reiterated &#8211; in order to maintain regular medical visits aiming at controlling the disease and symptoms, while the presence of cardiovascular comorbidities increases in severe COVID-19 cases. <xref rid="B1" ref-type="bibr">1</xref> </p>
      <p>The study included 116,343 patients, of whom 61.9% were diagnosed with SARS caused by COVID-19. At the same time, the study demonstrates that the presence of chronic diseases can be considered a risk factor for infection by COVID-19 due to greater vulnerability and morbimortality. Therefore, patients with previous CVD are more likely to develop more severe conditions. However, in females, there was a lower prevalence of SARS by COVID-19 because there is a variation between the immune response and the susceptibility to viral infections between sexes, which generates differences in disease severity and evolution. <xref rid="B1" ref-type="bibr">1</xref> </p>
      <p>In Wuhan, China, a meta-analysis with 46,248 infected patients analyzed the most prevalent comorbidities, with CVD (5&#177;4%) in third place. Wang et al., 2020 evaluated only hospitalized patients affected by viral infection, which has shown a higher prevalence &#8211; 19.6% &#8211; CVD, which reinforces the fact that the comorbidity of CVD contributes an increased severity of COVID-19, given the evident need for hospitalization. In addition, the patients evolved with higher levels of hypoxemia and urgent hospitalization in ICUs. <xref rid="B2" ref-type="bibr">2</xref> In the study conducted by Melo, the results were</p>
      <p>similar to those found in a study in Italy &#8211; both analyzed over seven days in March 2020; it found a decrease of 13% of patients with acute myocardial infarction (AMI) associated in the same week of 2019. On the other hand, even though there was a reduction in AMI cases and in the rate of hospital deaths, there was an increase in the in-hospital lethality rate in hospitalizations for CVD. Both studies demonstrated the relationship of COVID-19 with the high prevalence of cardiac lesions and a great potential for COVID-19 severity in CVD, in which mortality of hospitalized patients with CVD reached the most economically active portion of the population &#8211; from 20 to 59 years of age. <xref rid="B3" ref-type="bibr">3</xref> </p>
      <p>In view of that, the importance of medical follow-up of patients with chronic diseases is highlighted, since, according to Askin et al., in 2020, there was a marked increase in myocardial damage in patients with COVID-19, increasing the risk of morbimortality. Therefore, the appreciation of CVD as a complication associated with the COVID-19 virus, due to the increase of the disease symptoms, is of extreme significance for primary health care. <xref rid="B4" ref-type="bibr">4</xref> </p>
      <p>The current situation requires strategies aimed at preventing complications associated with chronic diseases, such as CVD. Therefore, current data demonstrate the need for special attention to patients at high risk as well as proper management of cardiovascular complications, aiming at quickly identifying and applying adequate treatment. Furthermore, it is recommended that patients with CVD get vaccinated &#8211; due to the risk of secondary bacterial infection by SARS-CoV-2 &#8211; and adopt a proper diet, regular sleep and physical activity, avoiding smoking and alcohol consumption.</p>

        <p>We would like to thank the authors for the analysis &#8220;Prevalence and Factors Associated with SARS caused by COVID-19 in Adults and Aged People with Chronic Cardiovascular Disease: A Critical Analysis&#8221;, especially due to the exposure of data reinforcing the findings presented in our study. <xref rid="B12" ref-type="bibr">1</xref> The analysis is also relevant because it includes information regarding the increase of in-hospital mortality rates in hospitalizations due to cardiovascular diseases (CVD) as a factor that further calls for the need to draw special attention to this group at risk in the analysis of complications associated with COVID-19. <xref rid="B13" ref-type="bibr">2</xref>, <xref rid="B14" ref-type="bibr">3</xref> </p>
        <p>CVDs are established as risk markers in the Brazilian primary health care (PHC) in order to guarantee the longitudinality of user care and the development of preventive strategies aimed at the integral care of the community, <xref rid="B15" ref-type="bibr">4</xref> but it is essential to question the applicability of these actions in the family health strategy (FHS). The challenge of monitoring the users as a way of managing CVD suggests overcoming the imperative of actions that support the diagnostic and therapeutic system in the care and management of chronic conditions. <xref rid="B16" ref-type="bibr">5</xref> A recently published study with more than 100 million participants from around the world highlights the importance of prevention, detection and control in primary care, including in low- and middle-income countries, with proposals for actions that include sending text messages to check on users with risk factors. <xref rid="B17" ref-type="bibr">6</xref> </p>
        <p>In the current context of facing the COVID-19 pandemic, this discussion is even more relevant because, as emphasized by the authors who conducted the critical analysis, the worsening of the symptoms of COVID-19 places CVDs as an important risk factor that affect screening, management and early intervention actions.</p>
        <p>Studies suggest the creation of proposals for models of care for chronic conditions, such as CVD, involving focus groups with positive results in the community, improvement of clinical indicators during follow-up, adoption of self-care practices and better analysis of the priorities in health planning. <xref rid="B17" ref-type="bibr">6</xref> Similarly, the World Health Organization (WHO) released an implementation guide for CVD management in primary care, with proposals for risk screening, assessment and management at this level of health care and education for professionals regarding the screening of risk factors, lifestyle interventions and the required referrals. <xref rid="B18" ref-type="bibr">7</xref> Especially regarding the management of users with CVD and persistent symptoms after COVID-19, the clinical and psychosocial complexity provided by the coexistence of these conditions calls for long-term multidisciplinary care. To conclude, we would like to express our gratitude for the possibility of reinforcing the need to review primary health care actions, especially in our current health context.</p>
