          <title>Fundamento</title>
          <p>Algumas s&#237;ndromes t&#234;m caracter&#237;sticas espec&#237;ficas e facilmente reconhec&#237;veis, enquanto outras podem ser mais complexas de se identificar e podem apresentar diferentes manifesta&#231;&#245;es fenot&#237;picas, por exemplo. Um diagn&#243;stico etiol&#243;gico &#233; importante para entender a natureza da doen&#231;a, para estabelecer o progn&#243;stico e para come&#231;ar o tratamento, permitindo a inclus&#227;o de pacientes na sociedade e reduzindo o custo financeiro dessas doen&#231;as.</p>
        </sec>

          <title>Objetivo</title>
          <p>A proposta inicial deste estudo foi a triagem citogen&#233;tica para detectar a s&#237;ndrome de dele&#231;&#227;o 22q11.2 (SD22q11.2) em rec&#233;m-nascidos e crian&#231;as com doen&#231;a card&#237;aca cong&#234;nita utilizando a t&#233;cnica da amplifica&#231;&#227;o multiplex de sondas dependente de liga&#231;&#227;o (MLPA). Assim, por meio da pesquisa, outras mudan&#231;as gen&#244;micas foram identificadas nesses pacientes card&#237;acos. Nosso objetivo se estendeu a investigar essas outras mudan&#231;as citogen&#233;ticas.</p>
        </sec>
        <sec>
          <title>M&#233;todos</title>
          <p>Investigamos 118 rec&#233;m-nascidos com doen&#231;as card&#237;acas cong&#234;nitas nascidos consecutivamente durante um ano, utilizando a t&#233;cnica da MLPA.</p>
        </sec>
        <sec>
          <title>Resultados</title>
          <p>A t&#233;cnica da MLPA permitiu a detec&#231;&#227;o da SD22q11.2 em 10/118 pacientes (8,5%). Outras altera&#231;&#245;es gen&#244;micas foram identificadas em 6/118 pacientes (5%): 1p36 del, 8p23 del (2 casos), 7q dup, 12 dup e 8q24 dup.</p>
        </sec>
        <sec>
          <title>Conclus&#227;o</title>
          <p>Este estudo ressalta a relev&#226;ncia da detec&#231;&#227;o de altera&#231;&#245;es gen&#244;micas que est&#227;o presentes em rec&#233;m-nascidos e crian&#231;as com doen&#231;as card&#237;acas cong&#234;nitas por meio de ferramentas citogen&#244;micas.</p>

      <title>Introdu&#231;&#227;o</title>
      <p>As anomalias cong&#234;nitas s&#227;o detectadas em aproximadamente 6% dos rec&#233;m-nascidos &#8211; a literatura internacional descreve que 7,9 milh&#245;es de crian&#231;as nascem com doen&#231;as cong&#234;nitas graves por ano. No Brasil, as malforma&#231;&#245;es representam a segunda principal causa de mortalidade infantil. <xref ref-type="bibr" rid="B1">1</xref></p>
      <p>A doen&#231;a card&#237;aca cong&#234;nita (DCC), de acordo com a defini&#231;&#227;o proposta por Mitchell et al., consiste de uma anomalia estrutural macrosc&#243;pica do cora&#231;&#227;o ou dos grandes vasos intrator&#225;cicos com repercuss&#245;es funcionais significativas ou potencialmente significativas. Representa aproximadamente 40% de todas as malforma&#231;&#245;es cong&#234;nitas. De acordo com a Organiza&#231;&#227;o Mundial da Sa&#250;de (OMS), sua incid&#234;ncia varia de 0,8% em pa&#237;ses de alta renda a 1,2% em pa&#237;ses de baixa renda. A incid&#234;ncia m&#233;dia de 1% normalmente &#233; aceita no Brasil e em outros pa&#237;ses da Am&#233;rica Latina. <xref ref-type="bibr" rid="B2">2</xref></p>
      <p>O Brasil registra 2,8 milh&#245;es de nascidos vivo por ano, e estima-se que quase 29.000 novos casos de DCC ocorrem a cada ano. <xref ref-type="bibr" rid="B2">2</xref> Aproximadamente 20 a 30% dos rec&#233;m-nascidos (RN) com doen&#231;as card&#237;acas morrem no per&#237;odo neonatal. DCCs s&#227;o as malforma&#231;&#245;es com o maior impacto na morbidade e mortalidade infantis, assim como nos custos com sa&#250;de p&#250;blica. <xref ref-type="bibr" rid="B2">2</xref> , <xref ref-type="bibr" rid="B4">4</xref> , <xref ref-type="bibr" rid="B5">5</xref></p>
      <p>M&#250;ltiplos fatores ambientais e gen&#233;ticos, como muta&#231;&#245;es, altera&#231;&#245;es cromoss&#244;micas e altera&#231;&#245;es gen&#233;ticas, est&#227;o inter-relacionados e s&#227;o a base das DCCs. Considerando a variedade de genes que coordenam o desenvolvimento do cora&#231;&#227;o e os v&#225;rios mecanismos que simultaneamente guiam o desenvolvimento geral do embri&#227;o, &#233; razo&#225;vel imaginar que as mudan&#231;as que interferem nesta organiza&#231;&#227;o complexa podem levar a v&#225;rias malforma&#231;&#245;es. Do ponto de vista gen&#233;tico, &#233; importante avaliar se a DCC &#233; uma ocorr&#234;ncia isolada ou se est&#225; associada a outras caracter&#237;sticas, de forma a constituir uma s&#237;ndrome. <xref ref-type="bibr" rid="B6">6</xref> A SD22q11.2 &#233; considerada como a segunda mudan&#231;a cromoss&#244;mica mais associada &#224;s DCCs, com incid&#234;ncia de 1:4000-5000 nascidos vivos, e aproximadamente 5% dos pacientes com doen&#231;as card&#237;acas apresentam esta dele&#231;&#227;o. <xref ref-type="bibr" rid="B7">7</xref> - <xref ref-type="bibr" rid="B9">9</xref> Anomalias conotruncais, principalmente a tetralogia de Fallot, atresia pulmonar com defeito do septo ventricular, transposi&#231;&#227;o das grandes art&#233;rias, truncus arteriosus tipo 1 e arco a&#243;rtico interrompido (IAA) tipo B s&#227;o as DCCs mais associadas &#224; SD22q11.2. <xref ref-type="bibr" rid="B8">8</xref> - <xref ref-type="bibr" rid="B10">10</xref></p>
      <p>&#201; importante notar que outras dele&#231;&#245;es e/ou duplica&#231;&#245;es que podem ser encontradas em pacientes card&#237;acos e sua detec&#231;&#227;o permitem o acompanhamento multidisciplinar adequado, a avalia&#231;&#227;o do progn&#243;stico e o aconselhamento gen&#233;tico.</p>

        <p>A t&#233;cnica da amplifica&#231;&#227;o multiplex de sondas dependente de liga&#231;&#227;o (MLPA) foi realizada com cinco kits diferentes: SALSA P064 para as s&#237;ndromes de microdele&#231;&#227;o e microduplica&#231;&#227;o mais comuns; P036 e P070 para desequil&#237;brio subtelom&#233;rico; e P356 e P250, espec&#237;ficos para o cromossomo 22q e DiGeorge e Velocardiofacial (MRC-Holland, Amsterd&#227;, Holanda). As rea&#231;&#245;es da MLPA foram realizadas de acordo com o protocolo do fabricante. Analisamos os resultados com o software GeneMarker&#174; (Softgenetics, LLC, State College, Pa., EUA). Os resultados foram considerados anormais quando foram observados picos relativos, com tamanho menor que 0,75 (dele&#231;&#227;o) ou maior que 1,25 (amostras duplicadas em rela&#231;&#227;o ao normal).</p>

      <p>Apesar dos avan&#231;os recentes e do fato de a SD22q11.2 ser considerada como a microdele&#231;&#227;o cromoss&#244;mica mais comum em humanos, de acordo com estudos realizados na Europa e nos Estados Unidos, <xref ref-type="bibr" rid="B8">8</xref> esta s&#237;ndrome permanece subdiagnosticada no Brasil devido &#224; falta de informa&#231;&#227;o entre m&#233;dicos e &#224; dificuldade de se obter uma triagem espec&#237;fica no Sistema &#218;nico de Sa&#250;de (SUS). No Brasil, n&#227;o h&#225; estudos sobre a triagem da dele&#231;&#227;o 22q11.2 utilizando a t&#233;cnica da MLPA exclusivamente na faixa et&#225;ria que n&#243;s analisamos. Neste per&#237;odo da vida, o fen&#243;tipo nem sempre &#233; claro, o que dificulta o diagn&#243;stico precoce nesses pacientes. <xref ref-type="bibr" rid="B8">8</xref> , <xref ref-type="bibr" rid="B9">9</xref> , <xref ref-type="bibr" rid="B12">12</xref></p>
      <p>Nosso objetivo &#233; enfatizar que as doen&#231;as card&#237;acas cong&#234;nitas devem ser um sinal de alerta para a SD22q11.2, especialmente em pacientes com malforma&#231;&#245;es conotruncais, hipocalcemia e dismorfismo facial. Por&#233;m, ainda &#233; um grande desafio devido aos seus v&#225;rios aspectos cl&#237;nicos e comportamentais, que podem atrasar o diagn&#243;stico e o tratamento adequado. Al&#233;m disso, a import&#226;ncia de se detectar outras dele&#231;&#245;es e/ou duplica&#231;&#245;es relacionadas &#224; DCC &#233; clara, j&#225; que isso levaria ao aconselhamento gen&#233;tico, ao acompanhamento multidisciplinar e &#224; identifica&#231;&#227;o de outras malforma&#231;&#245;es associadas. <xref ref-type="bibr" rid="B10">10</xref> , <xref ref-type="bibr" rid="B12">12</xref> , <xref ref-type="bibr" rid="B16">16</xref> , <xref ref-type="bibr" rid="B22">22</xref></p>
      <p>Os resultados descritos aqui est&#227;o alinhados com a literatura, que confirma a presen&#231;a das malforma&#231;&#245;es conotruncais na maioria dos pacientes com dele&#231;&#227;o 22q11.2. <xref ref-type="bibr" rid="B9">9</xref> , <xref ref-type="bibr" rid="B16">16</xref></p>
      <p>Pacientes com SD 22q11.2 e DCC severa provavelmente morrem antes de ter acesso ao hospital terci&#225;rio de refer&#234;ncia, levando &#224; subnotifica&#231;&#227;o da sua taxa de ocorr&#234;ncia. &#201; importante notar que, no Brasil, diferentemente de pa&#237;ses desenvolvidos, o aborto em casos de malforma&#231;&#227;o n&#227;o &#233; permitido por lei. Isso tamb&#233;m justificaria a diferen&#231;a quando comparamos nossa s&#233;rie de casos com as taxas de detec&#231;&#227;o de dele&#231;&#227;o 22q11.2 descritas na literatura. <xref ref-type="bibr" rid="B9">9</xref> , <xref ref-type="bibr" rid="B10">10</xref> , <xref ref-type="bibr" rid="B17">17</xref> , <xref ref-type="bibr" rid="B19">19</xref> , <xref ref-type="bibr" rid="B20">20</xref></p>
      <p>Neonatologistas, cardiologistas pedi&#225;tricos e cirurgi&#245;es card&#237;acos devem estar cientes das especificidades e do cuidado associados &#224; SD22q11.2. Como se pode observar, pacientes afetados pela condi&#231;&#227;o normalmente apresentam mudan&#231;as em v&#225;rios sistemas do corpo e requerem muitas interven&#231;&#245;es cl&#237;nicas, envolvendo hospitaliza&#231;&#227;o ao longo da vida. Deve-se notar que pacientes com SD22q11.2 t&#234;m taxas de mortalidade mais altas em compara&#231;&#227;o a outras s&#237;ndromes gen&#233;ticas (como a S&#237;ndrome de Down) e casos n&#227;o-sindr&#244;micos de DCC. <xref ref-type="bibr" rid="B16">16</xref> , <xref ref-type="bibr" rid="B23">23</xref> , <xref ref-type="bibr" rid="B31">31</xref></p>
      <p>Dada a import&#226;ncia desta s&#237;ndrome altamente complexa, &#233; essencial fazer uma triagem e detect&#225;-la cedo no primeiro ano de vida, com base em um conjunto de caracter&#237;sticas cl&#237;nicas e laboratoriais. <xref ref-type="bibr" rid="B12">12</xref> Vale notar que, neste grupo et&#225;rio, as taxas de morbidade e mortalidade s&#227;o altas, principalmente devido a infec&#231;&#245;es recorrentes resultando de aplasia ou hipoplasia t&#237;mica em DCC. <xref ref-type="bibr" rid="B12">12</xref> , <xref ref-type="bibr" rid="B34">34</xref> , <xref ref-type="bibr" rid="B35">35</xref></p>
      <p>Dentre os pacientes avaliados, a dele&#231;&#227;o 8p23 foi observada em dois deles. As doen&#231;as complexas foram dupla via de sa&#237;da do ventr&#237;culo direito e hipoplasia tric&#250;spide no ventr&#237;culo direito, ambas diagnosticadas na primeira semana de vida. &#201; importante salientar que esses pacientes n&#227;o apresentavam nenhuma anomalia cl&#237;nica t&#237;pica.</p>
      <p>V&#225;rios autores sugeriram que a dele&#231;&#227;o 8p pode ser mais frequente do que o n&#250;mero relativamente baixo de casos descritos na literatura. Os resultados dos estudos da correla&#231;&#227;o gen&#233;tico-fen&#243;tipo de indiv&#237;duos com DCC enfatizam a regi&#227;o cromoss&#244;mica 8p23.1 como a regi&#227;o cr&#237;tica para a morfog&#234;nese card&#237;aca. A por&#231;&#227;o proximal da regi&#227;o p23.1 cont&#233;m o gene da prote&#237;na de liga&#231;&#227;o 4 (GATA4), que codifica um fator de transcri&#231;&#227;o que tem papel essencial no desenvolvimento dos cora&#231;&#245;es humanos. CCs neste tipo de dele&#231;&#227;o podem ser explicadas pela haploinsufici&#234;ncia do GATA4. <xref ref-type="bibr" rid="B36">36</xref></p>
      <p>A dele&#231;&#227;o 8p23.1 pode estar associada a uma ampla gama de anomalias cl&#237;nicas, incluindo dismorfismo facial, microcefalia, restri&#231;&#227;o de crescimento intrauterino, doen&#231;as psiqui&#225;tricas, d&#233;ficit intelectual e doen&#231;as card&#237;acas, sendo que as mais frequentes s&#227;o defeitos do septo atrioventricular e do septo atrial. <xref ref-type="bibr" rid="B36">36</xref> , <xref ref-type="bibr" rid="B38">38</xref> , <xref ref-type="bibr" rid="B39">39</xref> H&#225; relatos de dele&#231;&#227;o intersticial e terminal no bra&#231;o curto do cromossomo 8 (mais frequentemente com rupturas nas regi&#245;es 8p21 a 8p23), assim como dele&#231;&#245;es combinadas com duplica&#231;&#245;es. A vari&#225;vel status cl&#237;nico se deve &#224; extens&#227;o da dele&#231;&#227;o ou &#224; localiza&#231;&#227;o do ponto de ruptura. <xref ref-type="bibr" rid="B38">38</xref></p>
      <p>O d&#233;ficit intelectual &#233; o achado mais frequente para este tipo de dele&#231;&#227;o. Aparentemente h&#225; uma rela&#231;&#227;o entre o tamanho da regi&#227;o da dele&#231;&#227;o no cromossomo 8 e o n&#237;vel de defici&#234;ncia intelectual, segundo a qual as dele&#231;&#245;es terminais mais distais &#224; regi&#227;o 23.1 est&#227;o associadas ao dano intelectual menor. <xref ref-type="bibr" rid="B36">36</xref> , <xref ref-type="bibr" rid="B40">40</xref></p>
      <p>Tamb&#233;m foi poss&#237;vel observar d&#233;ficit motor, atraso lingu&#237;stico e mudan&#231;as comportamentais (hiperatividade, d&#233;ficit de aten&#231;&#227;o e agressividade). As dele&#231;&#245;es SOX7 tamb&#233;m t&#234;m um papel no atraso do desenvolvimento e, possivelmente, nas caracter&#237;sticas dism&#243;rficas de indiv&#237;duos com dele&#231;&#227;o 8p23. <xref ref-type="bibr" rid="B38">38</xref> , <xref ref-type="bibr" rid="B41">41</xref> , <xref ref-type="bibr" rid="B42">42</xref></p>
      <p>Este estudo detectou a duplica&#231;&#227;o 7q11.2 em um paciente. Diferentemente da dele&#231;&#227;o de 1p36 e 8p23, este tipo de altera&#231;&#227;o gen&#244;mica n&#227;o tem correla&#231;&#227;o com doen&#231;as card&#237;acas. O paciente inclu&#237;do no estudo tinha coarcta&#231;&#227;o da aorta, e o diagn&#243;stico foi feito aos sete dias de vida; n&#227;o havia nenhuma malforma&#231;&#227;o aparente.</p>
      <p>&#201; importante notar que esta &#233; uma patologia rara, j&#225; que h&#225; aproximadamente 54 casos descritos na literatura. Duplica&#231;&#245;es parciais 7q foram classificadas em quatro grupos de acordo com a regi&#227;o cromoss&#244;mica afetada. <xref ref-type="bibr" rid="B43">43</xref></p>
      <p>Para o paciente detectado com a duplica&#231;&#227;o 8p24, haver&#225; um acompanhamento neurol&#243;gico para monitorar o desenvolvimento cl&#237;nico, j&#225; que os genes associados &#224; epilepsia est&#227;o localizados na regi&#227;o 8p24, e altera&#231;&#245;es nesta regi&#227;o podem ser respons&#225;veis por convuls&#245;es t&#244;nicas, &#224;s vezes seguidas de convuls&#245;es cl&#244;nicas generalizadas. Um dos genes envolvidos &#233; o KCNQ3 (canais de pot&#225;ssio dependentes de voltagem, subfam&#237;lia Q, membro 3). O desenvolvimento costuma ser benigno, com remiss&#227;o no primeiro ano de vida. &#201; importante observar que neste tipo de altera&#231;&#227;o gen&#244;mica n&#227;o h&#225; maior correla&#231;&#227;o com alguns tipos espec&#237;ficos de doen&#231;as card&#237;acas. O paciente da nossa s&#233;rie tinha atresia pulmonar e nenhum outro achado cl&#237;nico. <xref ref-type="bibr" rid="B44">44</xref></p>
      <p>A outra altera&#231;&#227;o gen&#244;mica detectada em um paciente com IAA foi a duplica&#231;&#227;o 12p. A literatura demonstra que esta altera&#231;&#227;o gen&#244;mica est&#225; relacionada &#224; s&#237;ndrome de Pallister-Killian (SPK), pela qual pacientes apresentam altera&#231;&#245;es craniofaciais t&#237;picas, defeitos card&#237;acos cong&#234;nitos, atraso de desenvolvimento vari&#225;vel, dano intelectual, convuls&#245;es, hipotonia, surdez, l&#243;bulos da orelha grandes, pesco&#231;o curto, m&#227;os grandes com dedos pequenos, encurtamento desproporcional de bra&#231;os e pernas, defici&#234;ncias de pigmenta&#231;&#227;o da pele, h&#233;rnia diafragm&#225;tica cong&#234;nita e outras anomalias sist&#234;micas. A SPK est&#225; relacionada &#224; trissomia ou &#224; tetrassomia de genes espec&#237;ficos localizados na regi&#227;o duplicada. O fen&#243;tipo SPK normalmente &#233; observado em indiv&#237;duos com duplica&#231;&#245;es completas ou parciais de 12p (trissomia 12p, em vez de tetrassomia 12p) como resultado da duplica&#231;&#227;o intersticial ou transloca&#231;&#227;o desequilibrada. <xref ref-type="bibr" rid="B45">45</xref></p>
      <p>Nesses casos, a correla&#231;&#227;o fen&#243;tipo-gen&#243;tipo &#233; muito importante para o diagn&#243;stico cl&#237;nico e as abordagens terap&#234;uticas. Por&#233;m, alguns fen&#243;tipos parecem estar associados a duplica&#231;&#245;es de regi&#245;es cromoss&#244;micas espec&#237;ficas.</p>
      <p>Avan&#231;os m&#233;dicos permitiram mais precis&#227;o na correla&#231;&#227;o entre a estrutura do DNA e o fen&#243;tipo cl&#237;nico. Como demonstrado neste estudo, as t&#233;cnicas citogen&#233;ticas permitem o diagn&#243;stico precoce de diferentes s&#237;ndromes de dele&#231;&#227;o e duplica&#231;&#227;o em pacientes card&#237;acos. Deve-se notar que um conhecimento aprofundado de testes citogen&#233;ticos e sua aplica&#231;&#227;o em diferentes campos da medicina levar&#227;o &#224; escolha adequada para a triagem: MLPA, array , painel ou sequenciamento de exoma.</p>
      <p>Estudos recomendam que todos os rec&#233;m-nascidos ou crian&#231;as com doen&#231;as card&#237;acas e dismorfismo, ou outras anomalias cong&#234;nitas, sejam testadas para dele&#231;&#227;o 22q11.2. Al&#233;m disso, os desequil&#237;brios genot&#237;picos de outras regi&#245;es cromoss&#244;micas, incluindo 10p12-p15, 4q21-q35, 8p21-p23, 17p13 e 18q21, podem ser encontrados em pacientes com suspeita cl&#237;nica de SD22q11.2 e sem a dele&#231;&#227;o 22q11.2. <xref ref-type="bibr" rid="B46">46</xref> , <xref ref-type="bibr" rid="B47">47</xref></p>
      <p>A investiga&#231;&#227;o gen&#244;mica &#233; crucial para realizar um perfil correto e precoce da etiologia de pacientes card&#237;acos, al&#233;m de permitir o melhor entendimento da alta variabilidade fenot&#237;pica nesses pacientes.</p>
      <p>Por fim, acreditamos que este estudo vai ajudar a demonstrar a import&#226;ncia da investiga&#231;&#227;o citogen&#233;tica precoce em pacientes card&#237;acos para avaliar o progn&#243;stico e o risco de recorr&#234;ncia, ao mesmo tempo em que leva ao tratamento correto, acompanhamento e aconselhamento familiar.</p>

      <title>Conclus&#245;es</title>
      <p>Os autores demonstraram a import&#226;ncia de investigar e detectar altera&#231;&#245;es gen&#244;micas utilizando a t&#233;cnica da MLPA em rec&#233;m-nascidos e crian&#231;as com doen&#231;as card&#237;acas. O r&#225;pido progresso do uso de testes citogen&#233;ticos no ambiente cl&#237;nico leva os m&#233;dicos a gerenciar as DDCs e considerar que muitos pacientes podem sofrer de d&#233;ficits cromoss&#244;micos ou at&#233; gen&#233;ticos.</p>

        <sec>
          <title>Background</title>
          <p>Some syndromes have specific and easily recognizable features, while others may be more complex to identify and may present different phenotypic manifestations, for example. An etiological diagnosis is important to understand the nature of the disease, to establish the prognosis and to start the treatment, allowing the inclusion of patients in society and reducing the financial cost of such diseases.</p>
        </sec>
        <sec>
          <title>Objective</title>
          <p>The initial proposal of this study was cytogenetic screening for the detection of the 22q11.2 deletion syndrome in consecutive newborns and infants with congenital heart disease using the multiplex ligation-dependent probe amplification (MLPA) technique. Therefore, throughout our research, other genomic alterations were identified in these cardiac patients. Thus, our objective was extended to investigate these other cytogenetic alterations.</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p>We investigated 118 neonates with congenital heart diseases born consecutively during one year using the MLPA technique.</p>
        </sec>
        <sec>
          <title>Results</title>
          <p>The MLPA technique allowed the detection of 22q11.2DS in 10/118 patients (8.5%). Other genomic alterations were also identified in 6/118 patients (5%): 1p36 del, 8p23 del (2 cases), 7q dup, 12 dup and 8q24 dup.</p>
        </sec>
        <sec>
          <title>Conclusion</title>
          <p>This study highlights the relevance of detecting genomic alterations that are present in newborns and infants with congenital cardiac diseases using cytogenomic tools.</p>

        <title>Introduction</title>
        <p>Congenital anomalies are detected in approximately 6% of newborns &#8211; the international literature describes that 7.9 million children are born with severe congenital disorders per year. In Brazil, malformations represent the second-leading cause of child mortality. <xref ref-type="bibr" rid="B1">1</xref></p>
        <p>Congenital heart disease, according to the definition proposed by Mitchell et al., consists of a macroscopic structural abnormality of the heart or large intrathoracic vessels, with significant or potentially significant functional repercussions. It represents approximately 40% of all congenital malformations. Its incidence, according to the World Health Organization (WHO), ranges from 0.8% in high-income countries to 1.2% in low-income countries. The average incidence of 1% is usually accepted in Brazil and other Latin American countries. <xref ref-type="bibr" rid="B2">2</xref></p>
        <p>Brazil registers 2.8 million live births annually, and it is estimated that almost 29,000 new cases of congenital heart disease (CHD) occur each year <xref ref-type="bibr" rid="B2">2</xref> . Approximately 20% to 30% of newborns (NB) with heart diseases die in the neonatal period. CHDs are the malformations with the greatest impact on infant morbidity and mortality and on public health expenses. <xref ref-type="bibr" rid="B2">2</xref> , <xref ref-type="bibr" rid="B4">4</xref> , <xref ref-type="bibr" rid="B5">5</xref></p>
        <p>Multiple environmental and genetic factors such as mutations, chromosomal alterations and gene alterations are interrelated and underlie CHDs. Given the variety of genes that coordinate the development of the heart and the various mechanisms that simultaneously guide the overall embryo development, it is reasonable to imagine that changes that interfere with this complex organization can lead to many malformations. From a genetic point of view, it is essential to evaluate whether CHD is an isolated occurrence or if it is associated with other characteristics, in such a way to constitute a syndrome. <xref ref-type="bibr" rid="B6">6</xref> The 22q11.2 deletion syndrome is considered as the chromosomal change that is the second most associated with CHDs, with an incidence of 1:4000-5000 live births, and approximately 5% of the patients with heart diseases present with this deletion. <xref ref-type="bibr" rid="B7">7</xref> - <xref ref-type="bibr" rid="B9">9</xref> Conotruncal anomalies, particularly tetralogy of Fallot, pulmonary atresia with a ventricular septal defect, transposition of the great arteries, type I truncus arteriosus and type B interrupted aortic arch are the most common CHDs associated to 22q11.2 deletion syndrome. <xref ref-type="bibr" rid="B8">8</xref></p>
        <p>It is also important to note that other deletions and/or duplications that may be found in cardiac patients and their detection allow a proper multidisciplinary follow-up, prognostic assessment and genetic counseling.</p>

        <title>Patients and Methods</title>
        <sec>
          <title>Patients</title>
          <p>We investigated 118 newborns with congenital heart disease (69 males; mean age of 31.7 days) admitted for surgical correction from March 2012 to June 2014. They were included consecutively from the Pediatric Cardiology Unit of Instituto do Cora&#231;&#227;o, at Hospital das Clinicas, Medical School of Universidade de Sao Paulo (InCor &#8211; HCFMUSP), the Neonatal Center of Instituto da Crian&#231;a, Hospital das Clinicas, Medical School of Universidade de Sao Paulo (ICr &#8211; HCFMUSP) or the Intensive Neonatal Treatment Center (CTIN2) of ICr-HCFMUSP.</p>
          <p>The ethics committee of HCFMUSP approved the study, and an informed consent form was obtained from the families of all patients.</p>
        </sec>
        <sec>
          <title>DNA extraction</title>
          <p>Genomic DNA was isolated from 3 mL of peripheral whole blood using a commercially available DNA isolation kit (QIAamp DNA Blood Mini Kit &#174; , Qiagen, Hilden, Germany) according to the manufacturer&#8217;s instructions. The quality and quantity of DNA samples were determined using a Qubit &#174; 2.0 Fluorometer (Invitrogen, Carlsbad, California, USA), and the integrity was ascertained by agarose gel electrophoresis.</p>
        </sec>
        <sec>
          <title>MLPA studies</title>
          <p>The multiplex ligation-dependent probe amplification (MLPA) technique was performed using five different kits: SALSA P064 for the most common microdeletion and microduplication syndromes, P036 and P070 for subtelomeric imbalance, and P356 and P250, which are specific for chromosome 22q and Digeorge/Velocardial (MRC-Holland, Amsterdam, The Netherlands). The MLPA reactions were performed according to the manufacturer's protocol. We analyzed the results using the software GeneMarker&#174; (Softgenetics, LLC, State College, Pa., USA). The results were considered abnormal when relative peaks were observed, whose size was smaller than 0.75 (deletion) or larger than 1.25 (duplicate samples relative to normal).</p>

        <title>Discussion</title>
        <p>It is important to highlight that the early diagnosis of CHD has been increasing due to the improvement of prenatal diagnosis. The international literature shows that 43.6% of cardiac patients are diagnosed during the first week of life; 70%, up to 6 months of age; and up to 86% in up to two years. <xref ref-type="bibr" rid="B11">11</xref> - <xref ref-type="bibr" rid="B13">13</xref></p>
        <p>As chromosome disorders are more frequent in patients with cardiac malformations than in the general population, it is crucial to emphasize the importance of genetic investigation in the diagnosis of CHD. The specific molecular diagnosis of a genetic syndrome will allow a proper multidisciplinary follow-up, prognostic assessment and family awareness of the risks of reoccurrence. <xref ref-type="bibr" rid="B14">14</xref></p>
        <p>The microdeletion and/or microduplication syndromes associated with congenital heart disease are not detected in classical karyotyping, which may only be identified when we employ other molecular techniques, such as FISH (fluorescence in situ hybridization), MLPA, or, more recently, chromosomal microarray analysis (CMA). <xref ref-type="bibr" rid="B11">11</xref></p>
        <p>This study demonstrated the importance of using the MLPA technique for the early diagnosis of 22q11.2DS in patients with CHDs in the first year of life. We observed that this deletion was detected in 8.5% of the patients, exceeding the rates described in the literature. This finding may be owed to the characteristics of the patients included in the study, most of whom were treated at a national tertiary referral hospital. <xref ref-type="bibr" rid="B9">9</xref> , <xref ref-type="bibr" rid="B10">10</xref> , <xref ref-type="bibr" rid="B16">16</xref></p>
        <p>Despite recent advances and the fact that 22q11.2DS is considered as the most common chromosome microdeletion in humans, according to studies undertaken in Europe and in the United States, <xref ref-type="bibr" rid="B8">8</xref> this syndrome remains underdiagnosed in Brazil due to the lack of information among physicians and the difficulty to obtain a specific screening in the Brazilian Unified Health System (SUS). In Brazil, there are no studies on the screening of the 22q11.2 deletion using the MLPA technique exclusively in the age group that we address. In this period of life, the phenotype is not always clear, which makes early diagnosis very difficult in these patients. <xref ref-type="bibr" rid="B8">8</xref> , <xref ref-type="bibr" rid="B9">9</xref> , <xref ref-type="bibr" rid="B12">12</xref></p>
        <p>Our purpose is to highlight that congenital heart diseases should be a warning sign for the 22q11.2DS, particularly in patients with conotruncal malformations, hypocalcemia, and facial dysmorphisms. However, it still poses a great challenge due to its many clinical and behavioral features, which can often delay the diagnosis and proper treatment. In addition, the importance of detecting other CHD-related deletions and/or duplications is clear, which would allow genetic counseling, multidisciplinary follow-up and identification of other associated malformations. <xref ref-type="bibr" rid="B10">10</xref> , <xref ref-type="bibr" rid="B12">12</xref> , <xref ref-type="bibr" rid="B16">16</xref> , <xref ref-type="bibr" rid="B22">22</xref></p>
        <p>The results described here are in line with the literature, which confirms the presence of conotruncal malformations in most of the patients with 22q11.2 deletion. <xref ref-type="bibr" rid="B9">9</xref> , <xref ref-type="bibr" rid="B16">16</xref></p>
        <p>Patients with the 22q11.2DS and severe CHD probably die before having access to a tertiary referral hospital, leading to the underestimation of the occurrence rate. It is important to note that, in Brazil, unlike in some developed countries, abortion in cases of malformation is not allowed by law. This would also justify the difference when we compare our case series with detection rates of 22q11.2 deletion described in the literature. <xref ref-type="bibr" rid="B9">9</xref> , <xref ref-type="bibr" rid="B10">10</xref> , <xref ref-type="bibr" rid="B17">17</xref> , <xref ref-type="bibr" rid="B19">19</xref> , <xref ref-type="bibr" rid="B20">20</xref></p>
        <p>Neonatologists, pediatric cardiologists and cardiac surgeons should be aware of the specifics and the care associated with 22q11.2DS. As we can see, affected patients often present changes in several body systems and require many clinical interventions involving hospitalization throughout their lives. It should be noted that patients with 22q11.2DS have the highest mortality rates when compared to other genetic syndromes (such as Down syndrome) and nonsyndromic cases of CHD. <xref ref-type="bibr" rid="B16">16</xref> , <xref ref-type="bibr" rid="B23">23</xref> , <xref ref-type="bibr" rid="B31">31</xref></p>
        <p>Given the importance of this highly complex syndrome, it is crucial to perform a screen and detect it early in the first year of life, based on a set of clinical and laboratory characteristics. <xref ref-type="bibr" rid="B12">12</xref> It is worth noting that, in this age range, morbidity and mortality rates are high, mainly due to recurrent infections resulting from aplasia or thymic hypoplasia and CHD. <xref ref-type="bibr" rid="B12">12</xref> , <xref ref-type="bibr" rid="B34">34</xref> , <xref ref-type="bibr" rid="B35">35</xref></p>
        <p>Among the assessed patients, the 8p23 deletion was observed in two of them. The complex diseases were infundibular double outlet right ventricle and right ventricular tricuspid hypoplasia, both diagnosed in the first week of life. It is important to highlight that these patients did not have any typical clinical abnormalities.</p>
        <p>Several authors have suggested that 8p deletion may be more frequent than the relatively low number of cases described in the literature. The results of genotype-phenotype correlation studies of individuals with CHDs highlighted the chromosome region 8p23.1 as a critical region for cardiac morphogenesis. The proximal portion of the p23.1 band contains the GATA binding protein 4 (GATA4) gene, which encodes a transcription factor that plays a key role in the development of human hearts. CCs in this type of deletion can be explained by the GATA4 haploinsufficiency. <xref ref-type="bibr" rid="B36">36</xref></p>
        <p>8p23.1 deletion may be associated with a wide range of clinical abnormalities, including facial dysmorphism, microcephaly, intrauterine growth restriction, neuropsychiatric disorders, intellectual deficit and heart diseases, the most frequent of which are atrioventricular septal defects and atrial septal defects. <xref ref-type="bibr" rid="B36">36</xref> , <xref ref-type="bibr" rid="B38">38</xref> , <xref ref-type="bibr" rid="B39">39</xref> There are reports of interstitial and terminal deletions of the short arm of chromosome 8 (most often with breaks in the 8p21 to 8p23 bands), as well as deletions combined with duplications. The variable clinical status is due to the extent of deletion or to the location of the breakpoint region. <xref ref-type="bibr" rid="B38">38</xref></p>
        <p>Intellectual deficit is the most frequently described finding for this type of deletion. There appears to be a relationship between the size of the deleted region in chromosome 8 and the degree of intellectual impairment, in which terminal deletions more distal to band 23.1 are associated with lower intellectual impairment. <xref ref-type="bibr" rid="B36">36</xref> , <xref ref-type="bibr" rid="B40">40</xref></p>
        <p>We can also observe motor deficit, language delay and behavioral changes (hyperactivity, attention deficit and aggressiveness). SOX7 deletions have also been understood to have a role in the developmental delay and possibly in the dysmorphic features in individuals with 8p23 deletion. <xref ref-type="bibr" rid="B38">38</xref> , <xref ref-type="bibr" rid="B41">41</xref> , <xref ref-type="bibr" rid="B42">42</xref></p>
        <p>The present study detected 7q11.2 duplication in one patient. Unlike deletion of 1p36 and 8p23, this type of genomic alteration has no correlation with heart diseases. The patient included in the study had aortic coarctation, and the diagnosis was made at seven days of age, and he did not present with any apparent malformation.</p>
        <p>It is important to note that this is a rare pathology, as there are approximately 54 cases described in the literature. Partial 7q duplications were classified into four groups according to the affected chromosome region. <xref ref-type="bibr" rid="B43">43</xref></p>
        <p>For the patient detected with 8q24 duplication, there will be a neurological follow-up to monitor the clinical development, since epilepsy-associated genes are located in the 8q24 region, and alterations in this region may be responsible for tonic seizures, sometimes followed by the generalized clonic seizure. One of the genes involved is KCNQ3 (potassium channel, voltage-gated KQT-like subfamily Q member 3). The development is usually benign, with remission in the first year of life. It is important to note that in this type of genomic alteration, there is no greater correlation with some specific types of cardiac diseases. The patient in our series had pulmonary atresia and no other clinical findings. <xref ref-type="bibr" rid="B44">44</xref></p>
        <p>The other genomic alteration detected in a patient with IAA was 12p duplication. The literature demonstrates that this genomic alteration is related to Pallister-Killian syndrome (PKS), in which patients present typical craniofacial alterations, congenital heart defects, variable developmental delay, intellectual impairment, seizures, hypotonia, deafness, thick earlobes, short neck, large hands with short fingers, disproportionate shortening of arms and legs, skin pigmentation disorders, congenital diaphragmatic hernia and other systemic abnormalities. PKS is related to the trisomy or tetrasomy of specific genes located in the duplicated region. The PKS phenotype is often observed in individuals with complete or partial duplications of 12p (12p trisomy rather than 12p tetrasomy) as a result of interstitial duplication or unbalanced translocation. <xref ref-type="bibr" rid="B45">45</xref></p>
        <p>In these cases, the phenotype-genotype correlation is very important for the clinical, diagnostic and therapeutic approaches. However, certain phenotypes seem to be associated with duplications of particular chromosome regions.</p>
        <p>Medical advances have allowed an increasingly accurate correlation between DNA structure and clinical phenotype. As shown in this study, cytogenetic techniques allow the early diagnosis of different deletion and duplication syndromes in cardiac patients. It should be noted that a deep knowledge of cytogenetic tests and their application in different fields of medicine will allow the appropriate choice for the screen: MLPA, array genotyping, panel or exome sequencing.</p>
        <p>Studies recommend that all newborns or children with cardiac diseases and dysmorphism or other congenital anomalies be tested for deletion 22q11.2. In addition, genotypic imbalances of other chromosome regions, including 10p12-p15, 4q21-q35, 8p21-p23, 17p13 and 18q21, can be found in patients with clinical suspicion of SD22q11.2 and without 22q11.2 deletion. <xref ref-type="bibr" rid="B46">46</xref> , <xref ref-type="bibr" rid="B47">47</xref></p>
        <p>Genome investigation is crucial to allow an unequivocal and early profile of the etiology of cardiac patients, in addition to allowing a better understanding of the high phenotypic variability in these patients.</p>
        <p>Finally, we believe this study will help demonstrate the importance of early cytogenetic investigation in cardiac patients to assess the prognosis and risks of recurrence while allowing proper treatment, follow-up and family counseling.</p>

        <title>Conclusions</title>
        <p>The authors demonstrate the importance of investigating and detecting genomic alterations by the MLPA technique present in newborns and infants with cardiac diseases. The fast progress of the use of cytogenetic tests in a clinical environment leads physicians managing CHDs to consider that many patients may suffer from chromosomal or even genetic disorders.</p>
        <p>The detection of cytogenomic alterations will allow clinicians to evaluate the prognosis and the risks of recurrence while allowing proper follow-up and treatment, aiming at improving the children&#8217;s life quality.</p>
