        <sec>
          <title>Fundamento</title>
          <p> O interesse pela hipertens&#227;o em crian&#231;as e adolescentes aumentou desde a atualiza&#231;&#227;o do sistema de classifica&#231;&#227;o da press&#227;o arterial para comparar com o sistema de classifica&#231;&#227;o dos adultos, alterando a terminologia de &#8220;normal alta&#8221; para &#8220;pr&#233;-hipertens&#227;o&#8221;.</p>
        </sec>
        <sec>
          <title>Objetivo</title>
          <p> O objetivo deste estudo foi analisar a associa&#231;&#227;o da modula&#231;&#227;o auton&#244;mica card&#237;aca com os n&#237;veis press&#243;ricos dos adolescentes.</p>
        </sec>
        <sec>
          <title>M&#233;todos</title>
          <p> 203 adolescentes foram agrupados de acordo com a press&#227;o arterial sist&#243;lica (PAS) e a press&#227;o arterial diast&#243;lica (PAD). Um grupo foi caracterizado como pr&#233;-hipertens&#227;o, e o outro como normotenso. Foram coletadas caracter&#237;sticas antropom&#233;tricas, cardiovasculares e de qualidade do sono. Inicialmente, os dados foram submetidos ao teste de normalidade Kolmogorov-Smirnov . As vari&#225;veis quantitativas cont&#237;nuas foram analisadas por meio do teste T de Student n&#227;o pareado. Para an&#225;lise das vari&#225;veis categ&#243;ricas, foi utilizado o teste qui-quadrado. Um modelo de regress&#227;o log&#237;stica foi realizado. O n&#237;vel de signific&#226;ncia estabelecido foi p&lt;0,05. Os dados foram expressos como m&#233;dia &#177; desvio padr&#227;o e intervalo de confian&#231;a. O software R foi utilizado para an&#225;lise dos dados. O tamanho do efeito foi calculado com a f&#243;rmula de Cohen.</p>
        </sec>
        <sec>
          <title>Resultados</title>
          <p> O grupo pr&#233;-hipertens&#227;o apresentou aumento da entropia de Shannon e diminui&#231;&#227;o da vari&#226;ncia total. Al&#233;m disso, no modelo de regress&#227;o log&#237;stica, os adolescentes deste grupo tiveram 1,03 mais chances de ter a entropia de Shannon afetada quando a PAS foi ajustada ao g&#234;nero, matura&#231;&#227;o sexual, tempo escolar, idade, circunfer&#234;ncia da cintura e qualidade do sono.</p>
        </sec>
        <sec>
          <title>Conclus&#227;o</title>
          <p> Nossos dados mostram que a modula&#231;&#227;o auton&#244;mica pode desempenhar um papel importante no desenvolvimento da press&#227;o arterial elevada em adolescentes ao controlar fatores como tempo escolar e qualidade do sono.</p>

      <title>Introdu&#231;&#227;o</title>
      <p>No mais recente relat&#243;rio sobre as causas globais de morte, as doen&#231;as cardiovasculares novamente lideraram a mortalidade por todas as causas para doen&#231;as n&#227;o transmiss&#237;veis,<xref ref-type="bibr" rid="B1">1</xref> sendo a hipertens&#227;o uma das principais causas nesta categoria. Devido ao aumento de mortes relacionadas &#224;s doen&#231;as cardiovasculares, a triagem para o desenvolvimento dessas doen&#231;as cresceu e est&#225; sendo direcionada &#224; popula&#231;&#227;o mais jovem.<xref ref-type="bibr" rid="B2">2</xref></p>
      <p>O interesse pelo tema da hipertens&#227;o em crian&#231;as aumentou desde a publica&#231;&#227;o, em 2004, do Grupo de Trabalho Nacional sobre Educa&#231;&#227;o em Press&#227;o Alta ( National High Blood Pressure Education Program Working Group), que atualizou o sistema de classifica&#231;&#227;o da press&#227;o arterial em crian&#231;as e adolescentes para que fosse compar&#225;vel ao sistema de classifica&#231;&#227;o de adultos, mudando a terminologia de &#8220;normal alta&#8221; para &#8220;pr&#233;-hipertens&#227;o&#8221;.<xref ref-type="bibr" rid="B3">3</xref></p>
      <p>A classifica&#231;&#227;o da pr&#233;-hipertens&#227;o foi criada para alertar os m&#233;dicos e justificar a introdu&#231;&#227;o de mudan&#231;as de estilo de vida entre aqueles que podem correr o risco de desenvolver hipertens&#227;o ainda jovens. Atualmente, sabe-se que crian&#231;as com press&#227;o alta t&#234;m mais chances de desenvolver hipertens&#227;o na vida adulta.<xref ref-type="bibr" rid="B4">4</xref> Al&#233;m disso, estudos recentes sugerem que crian&#231;as com pr&#233;-hipertens&#227;o podem desenvolver a hipertens&#227;o at&#233; antes de atingirem a vida adulta.<xref ref-type="bibr" rid="B5">5</xref></p>
      <p>Reconhecendo as lacunas e a necessidade de uma revis&#227;o de literatura atualizada e relevante, um estudo de 2020 enfatizou que o diagn&#243;stico da hipertens&#227;o em crian&#231;as e adolescentes &#8211; diferentemente dos adultos &#8211; baseia-se em suas curvas de idade e altura.<xref ref-type="bibr" rid="B6">6</xref> Assim, &#233; importante ressaltar que, em 2017, a Academia Americana de Pediatria (AAP) e seu Conselho de Desenvolvimento e Seguran&#231;a do Paciente criaram diretrizes pr&#225;ticas para fornecer informa&#231;&#245;es atuais sobre t&#243;picos relevantes para o diagn&#243;stico, avalia&#231;&#227;o e tratamento da hipertens&#227;o em crian&#231;as e adolescentes. Agora, para crian&#231;as com idade &#8805; 13 anos, a press&#227;o alta &#233; caracterizada de 120/80 at&#233; 129/80 mmHg; hipertens&#227;o est&#225;gio 1, 130/80 at&#233; 139/89; hipertens&#227;o est&#225;gio 2, &#8805;140 / 90 mmHg.<xref ref-type="bibr" rid="B7">7</xref></p>
      <p>O sistema nervoso aut&#244;nomo tem papel importante na r&#225;pida regula&#231;&#227;o da press&#227;o, e tem sido estudado em rela&#231;&#227;o &#224; sua fun&#231;&#227;o no desenvolvimento da hipertens&#227;o.<xref ref-type="bibr" rid="B8">8</xref> Um dos principais fatores correlacionados com a hipertens&#227;o &#233; o desequil&#237;brio auton&#244;mico, que tamb&#233;m est&#225; ligado ao risco cardiovascular.<xref ref-type="bibr" rid="B8">8</xref> , <xref ref-type="bibr" rid="B9">9</xref></p>

      <title>Discuss&#227;o</title>
      <p>O objetivo deste estudo foi analisar a associa&#231;&#227;o entre a modula&#231;&#227;o auton&#244;mica card&#237;aca e os n&#237;veis de press&#227;o arterial em adolescentes. Os principais achados deste estudo mostram que os adolescentes no grupo pr&#233;-hipertens&#227;o tiveram aumento na entropia de Shannon e redu&#231;&#227;o na vari&#226;ncia total. Al&#233;m disso, no modelo de regress&#227;o log&#237;stica, a entropia de Shannon foi a vari&#225;vel que mais esteve associada a mudan&#231;as no padr&#227;o da press&#227;o arterial, mostrando que adolescentes no grupo pr&#233;-hipertens&#227;o tinham 1,03 mais chances de ter a entropia de Shannon afetada quando a PAS era ajustada por sexo, matura&#231;&#227;o sexual, per&#237;odo escolar, idade, circunfer&#234;ncia da cintura e qualidade do sono.</p>
      <p>Nossos dados corroboram os achados de outros estudos,<xref ref-type="bibr" rid="B22">22</xref> , <xref ref-type="bibr" rid="B23">23</xref> trazendo mais evid&#234;ncias para demonstrar que o aumento da PA entre adolescentes est&#225; associado ao desequil&#237;brio auton&#244;mico observado pela vari&#226;ncia total menor e pela entropia de Shannon maior. Esses achados s&#227;o altamente relevantes clinicamente, pois explicam a fisiopatologia da hipertens&#227;o, j&#225; que mudan&#231;as no padr&#227;o da VFC promovem um indicador sens&#237;vel e precoce do dist&#250;rbio de sa&#250;de. Como descrito por McCraty et al., a VFC alta &#233; sinal de boa adapta&#231;&#227;o cardiovascular, caracterizando um indiv&#237;duo saud&#225;vel, com mecanismos auton&#244;micos eficientes. Por outro lado, a baixa VFC costuma ser indicativa de adapta&#231;&#227;o anormal e insuficiente do barorreflexo, o que pode indicar a presen&#231;a de disfun&#231;&#227;o auton&#244;mica.<xref ref-type="bibr" rid="B24">24</xref></p>
      <p>Al&#233;m disso, o estudo de Farah et al.<xref ref-type="bibr" rid="B25">25</xref> demonstrou que a VFC pode ser afetada por fatores como sexo, idade, circunfer&#234;ncia da cintura e qualidade do sono, o que corrobora outros achados pr&#233;-existentes na literatura.<xref ref-type="bibr" rid="B26">26</xref> , <xref ref-type="bibr" rid="B27">27</xref> Por&#233;m, de acordo com nosso estudo, o grupo pr&#233;-hipertens&#227;o n&#227;o mostrou nenhuma diferen&#231;a significativa na an&#225;lise das seguintes vari&#225;veis: idade, altura, peso, circunfer&#234;ncia da cintura, percentual de gordura e qualidade do sono, o que exclui a possibilidade de esses fatores, isoladamente, influenciarem a VFC. Por&#233;m, no modelo de regress&#227;o log&#237;stica, esses fatores associados estiveram relacionados a mais chances de afetar a entropia de Shannon.</p>
      <p>O estudo de Amara et al.<xref ref-type="bibr" rid="B28">28</xref> publicou mais evid&#234;ncias para apoiar o fato de que o hist&#243;rico familiar de hipertens&#227;o &#233; um importante fator que influencia a VFC, at&#233; 30%, e que a hipertens&#227;o &#233; duas vezes mais frequente em pessoas que tem um ou dois pais hipertensos. Neste estudo, o hist&#243;rico familiar de hipertens&#227;o provavelmente n&#227;o mostrou nenhuma rela&#231;&#227;o com as mudan&#231;as na VFC devido &#224; aus&#234;ncia de fatores como perfil gen&#233;tico dos adolescentes e a presen&#231;a de polimorfismos, o que pode influenciar a express&#227;o e a produ&#231;&#227;o de componentes regulat&#243;rios que est&#227;o presentes no sistema end&#243;crino, como o sistema renina-angiotensina-aldosterona.</p>
      <p>Ent&#227;o, embora os mecanismos exatos envolvidos no desenvolvimento da pr&#233;-hipertens&#227;o em adolescentes n&#227;o tenham sido plenamente elucidados, estudos recentes, como os de Pal et al. e Wu et al., demonstraram uma associa&#231;&#227;o entre o desequil&#237;brio auton&#244;mico e a menor modula&#231;&#227;o vagal card&#237;aca com a pr&#233;-hipertens&#227;o.<xref ref-type="bibr" rid="B29">29</xref> , <xref ref-type="bibr" rid="B30">30</xref> Outros estudos corroboram esses achados, reportando que a redu&#231;&#227;o na modula&#231;&#227;o vagal card&#237;aca e a VFC s&#227;o as principais causas do maior risco cardiovascular em adolescentes com pr&#233;-hipertens&#227;o; al&#233;m disso, obesidade, estresse psicossocial e dislipidemia foram reportados como importantes fatores de risco para pr&#233;-hipertens&#227;o e hipertens&#227;o.<xref ref-type="bibr" rid="B29">29</xref> - <xref ref-type="bibr" rid="B31">31</xref></p>
      <p>Atualmente, de acordo com v&#225;rios estudos,<xref ref-type="bibr" rid="B2">2</xref> - <xref ref-type="bibr" rid="B8">8</xref> o desequil&#237;brio auton&#244;mico tem sido considerado essencial para a etiologia da hipertens&#227;o. Em adolescentes, este desequil&#237;brio auton&#244;mico &#233; causado principalmente pela atividade aumentada do sistema nervoso simp&#225;tico, que tamb&#233;m esteve associada ao maior risco de eventos cardiovasculares, obesidade e disfun&#231;&#245;es do sono.<xref ref-type="bibr" rid="B17">17</xref> - <xref ref-type="bibr" rid="B26">26</xref> Por&#233;m, embora a hiperatividade simp&#225;tica esteja relacionada a outros fatores, como obesidade e problemas no sono, nossos dados n&#227;o mostraram diferen&#231;as na qualidade do sono entre os grupos.</p>
      <p>Com rela&#231;&#227;o ao sexo dos adolescentes, alguns estudos observaram que meninos t&#234;m maior preval&#234;ncia de PA alta do que meninas.<xref ref-type="bibr" rid="B32">32</xref> , <xref ref-type="bibr" rid="B33">33</xref> Nesse sentido, os estudos de Sztajzel et al. e Jackson et al., utilizando monitoriza&#231;&#227;o ambulatorial de press&#227;o arterial em crian&#231;as, mostraram que a idade traz um aumento na press&#227;o arterial tanto para meninos quanto meninas. Por&#233;m, depois do in&#237;cio da puberdade, a press&#227;o arterial em meninos &#233; maior do que em meninas da mesma idade. Isso se deve ao fato de jovens mulheres terem t&#244;nus vagal card&#237;aco maior durante o dia e a noite em rela&#231;&#227;o a homens da mesma faixa et&#225;ria.<xref ref-type="bibr" rid="B34">34</xref> , <xref ref-type="bibr" rid="B35">35</xref></p>
      <p>Nosso estudo teve limita&#231;&#245;es, como a aus&#234;ncia de uma medida direta da qualidade do sono (polissonografia) e um n&#250;mero limitado de participantes, j&#225; que &#233; a fase preliminar do estudo. Apesar dessas limita&#231;&#245;es, conseguimos avaliar a associa&#231;&#227;o da modula&#231;&#227;o auton&#244;mica card&#237;aca e os diferentes n&#237;veis de press&#227;o nos adolescentes.</p>
    </sec>
    <sec sec-type="conclusions">
      <title>Conclus&#227;o</title>
      <p>Nossos dados mostram que a modula&#231;&#227;o auton&#244;mica pode ter um papel no desenvolvimento da press&#227;o alta, quando controlada por outros fatores, como tempo escolar e qualidade do sono. Este achado tem uma aplica&#231;&#227;o importante, considerando que o desequil&#237;brio auton&#244;mico pode ser um sinal precoce do desenvolvimento de altos n&#237;veis de press&#227;o arterial em adolescentes.</p>

          <title>Background</title>
          <p>The interest regarding hypertension among children and adolescents has increased since the blood pressure rating system was updated to be compared with the adult rating system, changing the terminology from &#8220;normal high&#8221; to &#8220;prehypertension&#8221;.</p>

          <title>Objective</title>
          <p>This study aimed to analyze the association between cardiac autonomic modulation and pressure levels of adolescents.</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p>203 adolescents were grouped according to systolic blood pressure (SBP) and diastolic blood pressure (DBP). One group was characterized as prehypertension, and the other as normotensive. Anthropometric, cardiovascular and sleep quality characteristics were collected. Initially, the data were submitted to the Kolmogorov-Smirnov normality test. Continuous quantitative variables were analyzed using the unpaired Student t-test. For the analysis of categorical variables, a chi-square test was used. A logistic regression model was performed. The level of significance was set at p&lt;0.05. The data were expressed as mean &#177; standard deviation and confidence interval. The R software was used for data analysis. The effect size was calculated using the Cohen&#8217;s formula.</p>
        </sec>
        <sec>
          <title>Results</title>
          <p>The prehypertension group showed an increase in Shannon entropy and a decrease in total variance. Also, in the logistic regression model, adolescents in this group were 1.03 times more likely to have Shannon entropy&#8217;s affected when SBP was adjusted for gender, sexual maturation, school time, age, waist circumference, and sleep quality.</p>
        </sec>
        <sec>
          <title>Conclusion</title>
          <p>Our data show that autonomic modulation may play an important role in the development of elevated blood pressure in adolescents, when controlling for other factors, such as school time and sleep quality.</p>

        <title>Introduction</title>
        <p>In the most recent report on the global cause of deaths, cardiovascular diseases once again led the all-cause mortality for non-communicable diseases,<xref ref-type="bibr" rid="B1">1</xref> and hypertension was one of the leading causes in this category. Due to the rise of deaths related to cardiovascular diseases, the screening for the development of these diseases has grown and is being addressed to the younger part of the population.<xref ref-type="bibr" rid="B2">2</xref></p>
        <p>The interest in childhood hypertension has increased since the publication, in 2004, of the National High Blood Pressure Education Program Working Group, which updated the blood pressure classification system in children and adolescents to match the classification system of adults, changing the terminology from &#8220;normal high&#8221; to &#8220;prehypertension&#8221;.<xref ref-type="bibr" rid="B3">3</xref></p>
        <p>The &#8220;prehypertension&#8221; classification was created to warn physicians and justify the introduction of lifestyle changes in those who may be at risk for the development of hypertension at a young age. Currently, it is known that children with high blood pressure are more likely to develop hypertension in adulthood.<xref ref-type="bibr" rid="B4">4</xref> Also, recent evidence suggests that prehypertensive children may develop hypertension even before they reach adulthood.<xref ref-type="bibr" rid="B5">5</xref></p>
        <p>Recognizing the ongoing evidence gaps and the need for an updated and relevant literature review, a 2020 study highlighted that the diagnosis of hypertension in children and adolescents &#8211; in contrast with adulthood &#8211; is based on their age percentile curves and height.<xref ref-type="bibr" rid="B6">6</xref> Therefore, it is worth mentioning that in 2017 the American Academy of Pediatrics (AAP) and its Council on Quality Improvement and Patient Safety developed practical guidelines to provide updated information on topics that are relevant for the diagnosis, assessment, and treatment of hypertension in children and adolescents. Now, for children aged &#8805; 13 years, high blood pressure is characterized as 120/80 to 129/80 mmHg; stage 1 hypertension is 130/80 to 139/89 mmHg; and stage 2 hypertension is &#8805;140 / 90 mmHg.<xref ref-type="bibr" rid="B7">7</xref></p>
        <p>The autonomic nervous system plays an important role in the rapid regulation of blood pressure, and has been long investigated regarding its function in the development of hypertension.<xref ref-type="bibr" rid="B8">8</xref> One of the most important factors correlated with hypertension is autonomic imbalance, which is also correlated to cardiovascular risk.<xref ref-type="bibr" rid="B8">8</xref> , <xref ref-type="bibr" rid="B9">9</xref></p>
        <p>One of the most used tools for the assessment of autonomic modulation is the Heart Rate Variability (HRV), mainly due to its noninvasive approach and great reliability.<xref ref-type="bibr" rid="B4">4</xref> - <xref ref-type="bibr" rid="B9">9</xref> Among all HRV indexes, the nonlinear ones measure the complexity of the ECG signal, standing out from more common HRV methods, such as the Frequency domain, that can be affected by noise. Shannon entropy is an indicator of the complexity of the signal, being widely correlated with autonomic modulation. However, adolescents are also affected by other cardiovascular risk factors, such as obesity<xref ref-type="bibr" rid="B5">5</xref> - <xref ref-type="bibr" rid="B10">10</xref> and poor sleep quality due to school routine.<xref ref-type="bibr" rid="B11">11</xref> , <xref ref-type="bibr" rid="B12">12</xref> Therefore, it is important to determine the real contribution of autonomic imbalance for the development of high blood pressure levels. This study aimed to evaluate the association of cardiac autonomic modulation with blood pressure levels in adolescents.</p>

          <title>Sample</title>
          <p>The sample size of this study was established based on statistical calculations using the G*power software, taking into account the two-tailed t-test, with an effect size of 0.30, confidence interval of 95% and probability of error of 5%. For these characteristics, a sample size of 134 participants and power of 0.95 were found.</p>
          <p>Thus, the present study was conducted with 203 adolescents (70 boys and 133 girls). aged between 11 and 18 years, grouped according to SBP and DBP. One group was characterized as pre-hypertension (44 adolescents; SBP&gt; 120/80 mmHg), and the other was characterized as normotensive (159 adolescents; SBP &lt;120/80 mmHg). All subjects were selected from a state public school (Centro Anil Rio Anil - CINTRA) in S&#227;o Lu&#237;s, Maranh&#227;o, Brazil. All methods used in this study were approved by the institutional ethics committee and followed the guidelines from the Declaration of Helsinki.</p>

          <title>Statistical analysis</title>
          <p>Initially, the data were submitted to the Kolmogorov-Smirnov test to assess the normality of its distribution. Then, a descriptive analysis was conducted and the data were expressed as mean, standard deviation and confidence interval for baseline characteristics and continuous variables, and as absolute frequencies and percentage rates for categorical variables. The unpaired Student t-test was chosen to analyze continuous quantitative variables with normal distribution. Categorical variables were analyzed using the chi-square test. Logistic regression was performed to understand which variable would be best associated with blood pressure in the sample. Point estimates for the associations were expressed as odds ratios (OR) and respective 95% confidence intervals (95%CI). The R software was used for data analysis. The effect size was calculated according to the Cohen&#8217;s formula.<xref ref-type="bibr" rid="B21">21</xref> The significance level established for this study was p &lt;0.05.</p>

        <title>Discussion</title>
        <p>This study aimed to analyze the association between cardiac autonomic modulation and blood pressure levels in adolescents. The main findings of the present study show that adolescents in the prehypertension group showed increased Shannon entropy and decreased total variance. Also, in the logistic regression model, Shannon entropy was the variable that was mostly associated with changes in blood pressure pattern, showing that adolescents in the prehypertension group had 1.03 more chances of having Shannon entropy affected when the SBP was adjusted by gender, sexual maturation, school time, age, waist circumference and sleep quality.</p>
        <p>Our data corroborate the findings of other studies,<xref ref-type="bibr" rid="B22">22</xref> , <xref ref-type="bibr" rid="B23">23</xref> providing more evidence to show that an increase in BP levels among adolescents is associated with the autonomic imbalance observed by the lower total variance and higher Shannon entropy. These findings are highly clinically relevant for understanding the pathophysiology of hypertension, since changes in HRV patterns provide a sensitive and early indicator of health impairment. As described by McCraty et al., high HRV is a sign of good cardiovascular adaptation, characterizing a healthy individual with efficient autonomic mechanisms. On the other hand, low HRV is generally an indicator of abnormal and insufficient adaptation of the baroreflex, which may indicate the presence of autonomic dysfunction.<xref ref-type="bibr" rid="B24">24</xref></p>
        <p>In addition, the study by Farah et al.<xref ref-type="bibr" rid="B25">25</xref> showed that HRV can be affected by factors including sex, age, waist circumference and sleep quality, which corroborates other pre-existing findings in the literature.<xref ref-type="bibr" rid="B26">26</xref> , <xref ref-type="bibr" rid="B27">27</xref> However, according to our study, the prehypertension group did not show any significant differences in the analysis of the following variables: age, height, weight, waist circumference, fat percentage and sleep quality, which excluded the possibility of these isolated factors influencing HRV. However, in the logistic regression model, these associated factors were related to increased chances of affecting Shannon entropy.</p>
        <p>In the study by Amara et al.,<xref ref-type="bibr" rid="B28">28</xref> they published more evidence supporting that family history of hypertension is an important factor influencing HRV, up to 30%, and that hypertension is twice as common in individuals who have one or two hypertensive parents. In the present study, the family history of hypertension probably did not show any relationship with changes in HRV due to the absence of factors such as the genetic profile of adolescents and the presence of polymorphisms, which can influence the expression and production of regulatory components that are present in the endocrine system, such as the renin-angiotensin-aldosterone system.</p>
        <p>Thus, although the precise mechanisms involved in the development of prehypertension in adolescents have not yet been clearly explained, recent studies, such as the ones by Pal et al. and Wu et al., have demonstrated an association between autonomic imbalance and decreased vagal modulation with prehypertension.<xref ref-type="bibr" rid="B29">29</xref> , <xref ref-type="bibr" rid="B30">30</xref> Other studies corroborate these findings, reporting that the decrease in vagal modulation and HRV are the main causes of increased cardiovascular risk in adolescents with prehypertension; besides, obesity, psychosocial stress and dyslipidemia have been reported as important risk factors for prehypertension and hypertension.<xref ref-type="bibr" rid="B29">29</xref> - <xref ref-type="bibr" rid="B31">31</xref></p>
        <p>Currently, according to several studies,<xref ref-type="bibr" rid="B2">2</xref> - <xref ref-type="bibr" rid="B8">8</xref> autonomic imbalance has been widely accepted as being fundamental to the etiology of hypertension. In adolescents, this autonomic imbalance is mainly caused by increased activity of the sympathetic nervous system, which has also been associated with an increased risk of cardiovascular events, obesity and sleep disorders.<xref ref-type="bibr" rid="B17">17</xref> - <xref ref-type="bibr" rid="B26">26</xref> However, although sympathetic hyperactivity is linked to other factors, such as obesity and sleep disorders, our data did not show differences in sleep quality between groups.</p>
        <p>Regarding the gender of adolescents, some studies have observed that boys have higher prevalence of high BP than girls.<xref ref-type="bibr" rid="B32">32</xref> , <xref ref-type="bibr" rid="B33">33</xref> In this sense, the studies by Sztajzel et al. and Jackson et al., using ambulatory BP monitoring in children, revealed that with age there is an increase in blood pressure among boys and girls. However, after the onset of puberty, the blood pressure in boys is higher than that of girls at the same age. This is because young women have higher day and night vagal tone than men of similar age.<xref ref-type="bibr" rid="B34">34</xref> , <xref ref-type="bibr" rid="B35">35</xref></p>
        <p>Our study faced limitations such as the absence of a direct measure of sleep quality (polysomnography) and a limited number of subjects, as it is a preliminary phase of the study. Despite these limitations, we were able to assess the association of cardiac autonomic modulation with the different pressure levels of adolescents.</p>
      </sec>
      <sec sec-type="conclusions">
        <title>Conclusion</title>
        <p>In conclusion, our data show that autonomic modulation may play a role in the development of elevated blood pressure, when controlling for other factors, such as school time and sleep quality. This finding has an important application considering that autonomic imbalance can be an early sign for the development of high levels of blood pressure in adolescents.</p>
