          <title>Fundamento</title>
          <p>O suco de laranja (SL) &#233; rico em polifen&#243;is com propriedades anti-inflamat&#243;rias e antioxidantes. Ap&#243;s o infarto do mioc&#225;rdio (IM), mudan&#231;as complexas ocorrem na estrutura e na fun&#231;&#227;o card&#237;acas, processo conhecido como remodela&#231;&#227;o card&#237;aca (RC). O estresse oxidativo e a inflama&#231;&#227;o podem modular esse processo. Nossa hip&#243;tese foi a de que o consumo de SL atenua a RC ap&#243;s o IM.</p>

        <sec>
          <title>Objetivos</title>
          <p>Avaliar a influ&#234;ncia do SL sobre a RC ap&#243;s IM pela an&#225;lise de vari&#225;veis funcionais, morfol&#243;gicas, de estresse oxidativo, de infla&#231;&#227;o, e de metabolismo energ&#233;tico.</p>

          <title>M&#233;todos</title>
          <p>Um total de 242 ratos machos pesando entre 200 e 250g foram submetidos a um procedimento cir&#250;rgico (liga&#231;&#227;o da art&#233;ria coron&#225;ria ou cirurgia simulada). Sete dia ap&#243;s a cirurgia, os animais sobreviventes foram divididos para um dos quatro grupos: 1) SM, animais sham que receberam &#225;gua e maltodextrina (n= 20); 2) SSL, animais sham que receberam SL (n= 20); 3) IM, animais infartados que receberam &#225;gua e maltodextrina (n= 40); e 4) ISL, animais infartados que receberam SL (n = 40). A an&#225;lise estat&#237;stica foi realizada pelo teste de ANOVA com dois fatores com o teste de Holm-Sidak. Os resultados foram apresentados em m&#233;dia &#177; desvio padr&#227;o, e o n&#237;vel de signific&#226;ncia adotado foi de 5%.</p>
        </sec>
        <sec>
          <title>Resultados</title>
          <p>Tr&#234;s meses depois, o IM levou &#224; hipertrofia do ventr&#237;culo esquerdo (VE), com disfun&#231;&#227;o sist&#243;lica e diast&#243;lica, e aumento nos mediadores inflamat&#243;rios e de estresse oxidativo. Os animais que consumiram SL apresentaram menor atividade da glutationa peroxidase e maior express&#227;o da heme-oxigenase-1 (HO-1).</p>
        </sec>
        <sec>
          <title>Conclus&#227;o</title>
          <p>O SL atenuou a RC, e a HO-1 pode exercer um importante papel nesse processo.</p>

      <title>Introdu&#231;&#227;o</title>
      <p>O nome polifen&#243;is , ou compostos fen&#243;licos , refere-se a um grande grupo de mol&#233;culas encontradas em verduras, frutas, cereais, ch&#225;, caf&#233;, cacau, soja, e suco de fruta. <xref ref-type="bibr" rid="B1">1</xref> Esses compostos t&#234;m sido estudados devido ao seu potencial efeito biol&#243;gico na preven&#231;&#227;o e tratamento de diferentes doen&#231;as. <xref ref-type="bibr" rid="B2">2</xref> , <xref ref-type="bibr" rid="B3">3</xref></p>
      <p>Em revis&#227;o da literatura, Hyson mostrou que o suco da fruta, definido como suco puro ou 100% suco, reteve a maioria dos nutrientes e fitoqu&#237;micos da fruta &#237;ntegra e, portanto, pode ser importante no benef&#237;cio e prote&#231;&#227;o da sa&#250;de humana. <xref ref-type="bibr" rid="B4">4</xref> O suco de laranja (SL) &#233; fonte de compostos fen&#243;licos na forma de diferentes flavonoides. O principal flavonoide de interesse &#233; a hesperidina e sua forma hidrolisada, a hesperetina. <xref ref-type="bibr" rid="B5">5</xref> O interesse na pesquisa sobre as propriedades do SL aumentou devido &#224; sua a&#231;&#227;o anti-inflamat&#243;ria e antioxidante nas doen&#231;as cr&#244;nicas. <xref ref-type="bibr" rid="B6">6</xref></p>
      <p>Por exemplo, na les&#227;o mioc&#225;rdica, suplementos de antioxidantes podem ter efeito ben&#233;fico na remodela&#231;&#227;o card&#237;aca (RC). Em estudos usando modelo de infarto do mioc&#225;rdio (IM), compostos bioativos presentes no alecrim, tomate, e ch&#225; verde, e antioxidantes tais como &#225;cido asc&#243;rbico, quercetina, alfa-tocoferol, e vitamina A mostram efeito protetor contra a RC. <xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B3">3</xref> , <xref ref-type="bibr" rid="B7">7</xref> - <xref ref-type="bibr" rid="B10">10</xref></p>
      <p>A doen&#231;a card&#237;aca isqu&#234;mica, incluindo o IM, &#233; uma causa importante de insufici&#234;ncia card&#237;aca e morte em todo o mundo. Ap&#243;s o IM, mudan&#231;as complexas no ventr&#237;culo esquerdo (VE) podem causar altera&#231;&#245;es no tamanho, na massa, e na geometria do cora&#231;&#227;o, e na fun&#231;&#227;o card&#237;aca. <xref ref-type="bibr" rid="B11">11</xref> , <xref ref-type="bibr" rid="B12">12</xref> Tais mudan&#231;as s&#227;o definidas como RC e podem levar &#224; insufici&#234;ncia card&#237;aca e aumento na mortalidade. <xref ref-type="bibr" rid="B13">13</xref> Muitos fatores podem estar envolvidos na RC, incluindo estresse oxidativo, inflama&#231;&#227;o, fibrose, e apoptose. <xref ref-type="bibr" rid="B14">14</xref> , <xref ref-type="bibr" rid="B15">15</xref></p>
      <p>No IM, a isquemia inicia a gera&#231;&#227;o de esp&#233;cies reativas de oxig&#234;nio (EROS). As EROS danificam diretamente as membranas celulares, ativam a resposta inflamat&#243;ria, e levam &#224; morte celular. Elas tamb&#233;m podem atuar como sinais de transdu&#231;&#227;o, estimulando o fator nuclear kappa B (NF-&#954;B), o qual estimula a s&#237;ntese de citocinas pr&#243;-inflamat&#243;rias. <xref ref-type="bibr" rid="B14">14</xref> - <xref ref-type="bibr" rid="B16">16</xref> Al&#233;m disso, o sistema KEAP-1/Nrf2 (prote&#237;na 1 associada a ECH tipo Kelch/fator nuclear eritroide 2 relacionado ao fator 2) poderia ser ativado durante o estresse oxidativo celular, e exercer papel cr&#237;tico na homeostase redox. Esse &#233; um mecanismo universal que atua nos genes-alvo do Nrf2, conhecidos como elementos de resposta antioxidante. As outras vari&#225;veis relativas &#224; fun&#231;&#227;o n&#227;o foram valorizadas devido &#224; elevada frequ&#234;ncia card&#237;aca <xref ref-type="bibr" rid="B30">30</xref> no grupo SL ( <xref ref-type="table" rid="t2">Tabela 2</xref> do material suplementar).</p>

        <p>Quanto aos mediadores inflamat&#243;rios, o grupo SSL apresentou IL-10 e INF-&#947; mais elevados em compara&#231;&#227;o ao grupo SM ( <xref ref-type="table" rid="t2">Tabela 2</xref> ).       <title>Discuss&#227;o</title>
      <p>No presente estudo, IM induzido por liga&#231;&#227;o da art&#233;ria coron&#225;ria em ratos resultou em hipertrofia do VE e disfun&#231;&#227;o diast&#243;lica, o que foi compat&#237;vel com altera&#231;&#245;es observadas no infarto cr&#244;nico. <xref ref-type="bibr" rid="B11">11</xref> Nossos dados tamb&#233;m mostraram aumento no estresse oxidativo e marcadores inflamat&#243;rios, bem como altera&#231;&#245;es no metabolismo energ&#233;tico, com defici&#234;ncia na &#946;-oxida&#231;&#227;o de &#225;cidos graxos. Essas altera&#231;&#245;es caracterizam o processo de RC. <xref ref-type="bibr" rid="B31">31</xref> - <xref ref-type="bibr" rid="B33">33</xref> Tamb&#233;m observamos redu&#231;&#227;o na express&#227;o de Nrf-2 e HO-1. Na fase cr&#244;nica do IM, a via do Nrf2 pode estar reduzida por express&#227;o anormal do gene-alvo do Nrf2, afetando a manuten&#231;&#227;o da homeostase redox via enzimas medidas por elementos de resposta antioxidante. <xref ref-type="bibr" rid="B34">34</xref> Em estudo pr&#233;vio conduzido em nosso laborat&#243;rio com modelo de IM, observamos express&#227;o mais baixa de Nrf2 e HO-1. <xref ref-type="bibr" rid="B3">3</xref> Esses achados sugerem ou uma menor express&#227;o ou um maior catabolismo da prote&#237;na Nrf2, levando, assim, &#224; menor s&#237;ntese de HO-1.</p>
      <p>No presente estudo, o consumo de SL resultou na atenua&#231;&#227;o da RC nos animais infartados. Essa atenua&#231;&#227;o pode ser observada na diminui&#231;&#227;o da cavidade do VE (DDVE e DSVE), e na melhora da fun&#231;&#227;o sist&#243;lica, caracterizada pelo aumento no S&#8217; m&#233;dio, <xref ref-type="bibr" rid="B35">35</xref> e na fun&#231;&#227;o diast&#243;lica (menor di&#226;metro do &#225;trio esquerdo). No estudo de Yu et al., <xref ref-type="bibr" rid="B36">36</xref> os ratos infartados por liga&#231;&#227;o da art&#233;ria coron&#225;ria esquerda, tratados com hesperidina por quatro semanas, apresentaram DDVE e DSVE mais baixos e melhor fun&#231;&#227;o sist&#243;lica que os animais infartados. Esses dados s&#227;o similares aos nossos, e podem indicar o efeito da hesperidina do SL sobre o processo de RC. Em outro estudo, outro composto fen&#243;lico, a hesperetina, tamb&#233;m apresentou efeito sobre o cora&#231;&#227;o. Em um modelo de sobrecarga de press&#227;o, Deng et al., <xref ref-type="bibr" rid="B37">37</xref> encontraram valores mais baixos de DDVE e de DSVE oito semanas ap&#243;s a administra&#231;&#227;o de hesperetina.</p>
      <p>O IM leva a desequil&#237;brio entre a produ&#231;&#227;o de EROS e defesas antioxidantes, levando a estresse oxidativo. Ap&#243;s isquemia, algumas EROS danificam membranas celulares, iniciando o processo de peroxida&#231;&#227;o lip&#237;dica. <xref ref-type="bibr" rid="B38">38</xref> Por exemplo, Bagatini et al., <xref ref-type="bibr" rid="B39">39</xref> descreveram aumento na peroxida&#231;&#227;o lip&#237;dica em pacientes com IM. Nossos resultados tamb&#233;m mostraram maior concentra&#231;&#227;o de hidroper&#243;xidos lip&#237;dicos nos animais infartados em compara&#231;&#227;o aos animais n&#227;o infartados. <xref ref-type="bibr" rid="B40">40</xref> A enzima SOD &#233; a primeira defesa do organismo contra EROS. Nosso estudo mostrou que os animais infartados, em compara&#231;&#227;o aos n&#227;o infartados, apresentaram maior atividade da enzima SOD, conforme descrito anteriormente. <xref ref-type="bibr" rid="B3">3</xref> , <xref ref-type="bibr" rid="B25">25</xref> Em rela&#231;&#227;o ao consumo de SL, observamos que os animais que consumiram SL apresentaram menor atividade da GSH-Px. Resultado semelhante foi apresentado por Selvaraj e Pugalendi <xref ref-type="bibr" rid="B41">41</xref> no modelo de isquemia induzida por isoproterenol: os ratos que receberam hesperidina apresentaram atividade mais baixa das enzimas antioxidantes, entre eles, a GSH-Px. <xref ref-type="bibr" rid="B41">41</xref></p>
      <p>Em rela&#231;&#227;o ao metabolismo energ&#233;tico, o cora&#231;&#227;o, assim como outros &#243;rg&#227;os, consegue adaptar-se e utilizar o melhor substrato energ&#233;tico em cada situa&#231;&#227;o. A PFK atua na regula&#231;&#227;o da glic&#243;lise, e catalisa a fosforila&#231;&#227;o de glicose em frutose-6-fosfato e subsequentemente em frutose 1,6-bisfosfato. <xref ref-type="bibr" rid="B42">42</xref> A PFK &#233; ativada quando as concentra&#231;&#245;es de ATP s&#227;o reduzidas e &#233; inibida quando as c&#233;lulas t&#234;m reserva suficiente de ATP e de outros substratos, tais como &#225;cidos graxos. <xref ref-type="bibr" rid="B42">42</xref> Nossos dados mostraram valores mais altos de PFK em animais infartados com ingest&#227;o de SL. Esses dados mostram que atividade aumentada da PFK pode levar &#224; regula&#231;&#227;o da via glicol&#237;tica, fornecendo mais substrato para produ&#231;&#227;o de energia. Outro achado importante que indica maior uso de substrato &#233; a maior atividade da ATP sintase em animais que receberam SL.</p>
      <p>Al&#233;m do estresse oxidativo e altera&#231;&#245;es metab&#243;licas, observamos que os animais infartados que receberam SL apresentaram valores mais baixos de IFN-&#947;. Uma vez que a fase cr&#244;nica da inflama&#231;&#227;o est&#225; relacionada a uma produ&#231;&#227;o aumentada de IFN-&#947;, <xref ref-type="bibr" rid="B43">43</xref> nossos resultados sugerem uma fase mais avan&#231;ada em dire&#231;&#227;o &#224; resolu&#231;&#227;o do processo inflamat&#243;rio. Um resultado interessante &#233; que animais sham que consumiram SL mostraram efeito imunomodulat&#243;rio, indicado pelos valores mais altos de IL-10 e INF-&#947;. Similar aos nossos achados em animais do grupo sham, os quais n&#227;o sofreram nenhuma les&#227;o card&#237;aca, estudos com humanos sadios, de meia idade, relataram que o SL alterou a express&#227;o g&#234;nica em leuc&#243;citos para um perfil anti-inflamat&#243;rio e antiaterog&#234;nico, <xref ref-type="bibr" rid="B44">44</xref> e promoveu uma prote&#231;&#227;o precoce de c&#233;lulas sangu&#237;neas mononucleares contra dano oxidativo no DNA. <xref ref-type="bibr" rid="B45">45</xref> Al&#233;m disso, o consumo de SL com a refei&#231;&#227;o rica em carboidratos preveniu o estresse oxidativo e inflamat&#243;rio induzido pela refei&#231;&#227;o. <xref ref-type="bibr" rid="B46">46</xref></p>
      <p>Outro achado interessante em nosso estudo foi os valores mais elevados de HO-1 em animais que consumiram SL. Lin et al., <xref ref-type="bibr" rid="B47">47</xref> em 2005, tamb&#233;m mostraram que a hesperetina induziu a express&#227;o proteica de HO-1. <xref ref-type="bibr" rid="B47">47</xref> A enzima HO-1 exerce a&#231;&#227;o importante na homeostase celular devido &#224; sua a&#231;&#227;o catab&#243;lica no grupo heme das hemoprote&#237;nas, gerando subprodutos tais como ferro, biliverdina e mon&#243;xido de carbono. Por meio desses subprodutos, a HO-1 exerce a&#231;&#227;o inflamat&#243;ria antioxidante, e antiapopt&#243;tica. <xref ref-type="bibr" rid="B48">48</xref> , <xref ref-type="bibr" rid="B49">49</xref> Al&#233;m dessa fun&#231;&#227;o cl&#225;ssica, a HO-1 participa na sinaliza&#231;&#227;o celular amplificando a a&#231;&#227;o dos indutores (heme, oxidantes, citocinas, for&#231;as hemodin&#226;micas, fatores de crescimento, hip&#243;xia, e horm&#244;nios) de fatores de transcri&#231;&#227;o. <xref ref-type="bibr" rid="B48">48</xref></p>
      <p>Wang et al., <xref ref-type="bibr" rid="B50">50</xref> mostraram que a HO-1 &#233; importante para a homeostase card&#237;aca, protegendo o &#243;rg&#227;o contra isquemia e les&#245;es induzidas pela reperfus&#227;o, e danos oxidativos. <xref ref-type="bibr" rid="B50">50</xref> Em outro estudo, a administra&#231;&#227;o de hemina em camundongos infartados induziu a ativa&#231;&#227;o de HO-1, o que causou uma mudan&#231;a nos macr&#243;fagos para um fen&#243;tipo anti-inflamat&#243;rio (M2), redu&#231;&#227;o da expans&#227;o da cicatriz do infarto, e melhora da fun&#231;&#227;o card&#237;aca. <xref ref-type="bibr" rid="B51">51</xref> Assim, HO-1 aumentada tamb&#233;m pode exercer importante papel na atenua&#231;&#227;o da RC pelo SL ( <xref ref-type="fig" rid="f02">Figura 2</xref> ). Ainda, esse aumento foi independente da via do Nrf2, uma vez que o SL n&#227;o causou altera&#231;&#245;es na express&#227;o dessa prote&#237;na. Similar aos nossos achados, Wang et al., <xref ref-type="bibr" rid="B52">52</xref> encontraram que a isoliquiritina e a isoliquiritigenina, flavonoides derivados do alca&#231;uz, induziram a express&#227;o de HO-1 independentemente da express&#227;o de Nrf2. <xref ref-type="bibr" rid="B52">52</xref> A express&#227;o de HO-1 pode ser induzida por diferentes vias e variar de acordo com o modelo e tratamento utilizado. <xref ref-type="bibr" rid="B47">47</xref></p>

        <title>Limita&#231;&#245;es</title>
        <p>O SL usado no estudo foi um suco comercial, pronto para consumo, pasteurizado, livre de conservantes e a&#231;&#250;car. A escolha por esse suco foi para garantir a padroniza&#231;&#227;o. No entanto, &#233; poss&#237;vel que o uso de outros tipos de sucos, elaborados com outros tipos de laranjas, poderia levar a respostas diferentes.</p>

      <title>Conclus&#227;o</title>
      <p>O SL atenuado por RC ap&#243;s IM, com diminui&#231;&#227;o do di&#226;metro do VE e melhora da fun&#231;&#227;o sist&#243;lica e diast&#243;lica; HO-1 pode desempenhar um papel importante neste processo.</p>

          <title>Background</title>
          <p>Orange juice (OJ) is rich in polyphenols with anti-inflammatory and antioxidant properties. After myocardial infarction (MI), complex changes occur in cardiac structure and function, which is known as cardiac remodeling (CR). Oxidative stress and inflammation can modulate this process. We hypothesized that the consumption of OJ attenuates the CR after MI.</p>

        <sec>
          <title>Objectives</title>
          <p>To evaluate the influence of OJ on CR after MI by analysis of functional, morphological, oxidative stress, inflammation, and energy metabolism variables.</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p>A total of 242 male rats weighing 200-250 g were submitted to a surgical procedure (coronary artery ligation or simulated surgery). Seven days after surgery, survivors were assigned to one of the four groups 1) SM, sham animals with water and maltodextrin (n= 20); 2) SOJ, sham animals with OJ (n= 20); 3) IM, infarcted animals with water and maltodextrin (n= 40); and 4) IOJ, infarcted animals with OJ (n = 40). Statistical analysis was performed by the two-way ANOVA supplemented by Holm-Sidak. Results are presented as mean &#177; standard deviation, the level of significance adopted was 5%.</p>
        </sec>
        <sec>
          <title>Results</title>
          <p>After 3 months, MI led to left ventricular (LV) hypertrophy, with systolic and diastolic dysfunction, and increased oxidative stress and inflammatory mediators. OJ intake reduced LV cavity and improved systolic and diastolic function. The OJ animals presented lower activity of glutathione peroxidase and higher expression of heme-oxygenase-1 (HO-1).</p>

          <title>Conclusion</title>
          <p>OJ attenuated CR in infarcted rats and HO-1 may be play an important role in this process.</p>

        <title>Introduction</title>
        <p>The name polyphenols , or phenolic compounds , refers to a large group of molecules found in leaf vegetables, fruits, cereals, tea, coffee, cocoa, wine, soy, and fruit juice.<xref ref-type="bibr" rid="B1">1</xref> These compounds have been studied because of their potential biological effect in prevention and treatment of different diseases.<xref ref-type="bibr" rid="B2">2</xref> , <xref ref-type="bibr" rid="B3">3</xref></p>
        <p>In a review of the literature, Hyson showed that fruit juices, defined as pure juice or 100% fruit juice, retained most of nutrients and phytochemicals of the whole fruit and therefore may have an important potential to benefit and protect human health.<xref ref-type="bibr" rid="B4">4</xref> Orange juice (OJ) is a source of phenolic compounds in the form of different flavonoids. The main flavonoid of interest is hesperidin and its hydrolyzed form, hesperetin.<xref ref-type="bibr" rid="B5">5</xref> The research interest in the properties of OJ has increased because of its anti-inflammatory and antioxidant action in chronic diseases.<xref ref-type="bibr" rid="B6">6</xref></p>
        <p>For example, in myocardial injury, antioxidant supplements may have beneficial effects in cardiac remodeling (CR). In studies using myocardial infarction (MI) model, bioactive compounds present in rosemary, tomato, green tea, and antioxidants such as ascorbic acid, quercetin, alpha-tocopherol, and vitamin A showed protection against CR.<xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B3">3</xref> , <xref ref-type="bibr" rid="B7">7</xref> - <xref ref-type="bibr" rid="B10">10</xref></p>
        <p>Ischemic heart disease, including MI, is a leading cause of heart failure and death worldwide. After MI, complex changes in the left ventricle can cause changes in cardiac size, mass, geometry, and function.<xref ref-type="bibr" rid="B11">11</xref> , <xref ref-type="bibr" rid="B12">12</xref> These changes are defined as CR and can lead to heart failure and increased mortality.<xref ref-type="bibr" rid="B13">13</xref> Many factors may be involved in CR, such as oxidative stress, inflammation, fibrosis, and apoptosis.<xref ref-type="bibr" rid="B14">14</xref> , <xref ref-type="bibr" rid="B15">15</xref></p>
        <p>In MI, ischemia initiates the generation of reactive oxygen species (ROS). ROS directly damage cell membranes, activate the inflammatory response, and lead to cell death. They can also act as transduction signals, stimulating nuclear factor &#954;B (NF-&#954;B), which, in turn, stimulates proinflammatory cytokines synthesis.<xref ref-type="bibr" rid="B14">14</xref> - <xref ref-type="bibr" rid="B16">16</xref> In addition, the Kelch-like ECH-associated protein 1/nuclear factor erythroid 2-related factor 2 (KEAP-1/Nrf2) system could be activated during cellular oxidative stress and play a critical role in redox homeostasis. This is a universal mechanism that acts in Nrf2 target genes called antioxidant response elements. Glutathione peroxidase (GSH-Px) and heme-oxygenase-1 (HO-1) are examples of proteins regulated by this system.<xref ref-type="bibr" rid="B17">17</xref></p>
        <p>Therapeutic strategies to attenuate CR after MI have been extensively studied.<xref ref-type="bibr" rid="B18">18</xref> , <xref ref-type="bibr" rid="B19">19</xref> Aldosterone blockers, angiotensin-converting enzyme inhibitors, and beta-blockers are some of these strategies.<xref ref-type="bibr" rid="B20">20</xref> In this context, bioactive compounds of natural products, with cardioprotective properties such as flavonoids, can be an important adjuvant in the treatment of MI. On the other hand, studies show that a focus on food and dietary patterns instead of individual nutrients or phytochemicals is better for cardiometabolic health.<xref ref-type="bibr" rid="B21">21</xref> Thus, the aim of this study was to evaluate the influence of OJ intake on CR after MI.</p>

When all groups were analyzed, a difference in mortality was observed between the groups (p=0.04). However, mortality was not different between the infarcted groups (p=0.836). After the period of OJ intake, euthanasia of surviving animals was performed. Then, histological analysis of the left ventricle of infarcted animals was performed to verify the infarction size ( <xref ref-type="fig" rid="f01001">Figure 1</xref> of the Supplementary material). These animals did not present a difference in infarct size (IM= 40.1&#177;7.41%; IOJ=38.1&#177;5.76%; p= 0.528). The final body weight (BW) was not different among the groups ( <xref ref-type="table" rid="t1001">Table 1</xref> ).</p>

          <title>Effect of MI in rat hearts</title>
          <p>MI led to adverse CR. Regarding morphological data, MI led to higher values of LV diastolic diameter/BW, LV systolic diameter/BW, left atrial diameter/aorta, LV mass index (LVMI), LV weight/BW, and right ventricular weight/BW ( <xref ref-type="table" rid="t1001">Table 1</xref> ), LV posterior wall thickness/BW, interventricular septum wall thickness/BW, and left atrial diameter/BW ( <xref ref-type="table" rid="t1001">Table 1</xref> of Supplementary material). These changes characterize the enlargement of the left cavities and LV hypertrophy. MI impaired cardiac systolic function, as shown by the lower values of fractional area change (FAC), S&#180; mean ( <xref ref-type="table" rid="t1001">Table 1</xref> ), endocardial fractional shortening, and ejection fraction ( <xref ref-type="table" rid="t2001">Table 2</xref> of Supplementary material). Cardiac diastolic function was also impaired, as shown by decreased mean E&#180; ( <xref ref-type="table" rid="t1001">Table 1</xref> ), E wave deceleration time, E&#180; lateral, and E&#180; septal ( <xref ref-type="table" rid="t2001">Table 2</xref> of Supplementary material) and increased A wave, A&#180; mean; E/E&#180; ratio ( <xref ref-type="table" rid="t1001">Table 1</xref> ), Tei index, E/A ratio, isovolumetric relaxation time adjusted by heart rate, A&#180; lateral, and A&#180; septal.</p>

          <title>Effect of OJ intake on rat hearts</title>
          <p>OJ intake reduced the LV cavity, with lower values of LV end-diastolic diameter (LVDD) and LV end-systolic diameter (LVSD); improved systolic function, with higher values of S&#180; mean; and improved diastolic function, with lower left atrial diameter adjusted for aorta diameter ( <xref ref-type="table" rid="t1001">Table 1</xref> )<xref ref-type="bibr" rid="B29">29</xref> after MI. No differences were observed for other echocardiographic variables ( <xref ref-type="table" rid="t1001">Table 1</xref> ). The other function variables were not valued because of the higher heart rate<xref ref-type="bibr" rid="B30">30</xref> in the IOJ group (presented in Supplementary material <xref ref-type="table" rid="t2001">Table 2</xref> ).</p>
          <p>In addition, the animals that consumed OJ presented lower activity of GSH-Px ( <xref ref-type="table" rid="t2001">Table 2</xref> ). No differences were observed for LH, SOD activity, catalase activity, or GSH-Px expression ( <xref ref-type="table" rid="t2001">Table 2</xref> and <xref ref-type="fig" rid="f02001">Figure 2</xref> of the Supplementary material).</p>
          <p>Regarding the inflammatory mediators, the SOJ group showed higher IL-10 and INF-&#947; than the SM group ( <xref ref-type="table" rid="t2001">Table 2</xref> ). Also, the IOJ group had lower INF-&#947; values than the IM group ( <xref ref-type="table" rid="t2001">Table 2</xref> ). No differences were observed for NF-&#954;B or Sirt-1 between animals that consumed or did not consume OJ ( <xref ref-type="table" rid="t2001">Table 2</xref> and <xref ref-type="fig" rid="f02001">Figure 2</xref> of the Supplementary material).</p>
          <p>An improvement in the use of substrate occurred in the animals that consumed OJ. We observed higher values in PFK activity in the IOJ group compared with the IM group and higher activity of ATP synthase in animals that consumed OJ. No difference was observed in the activity of other energy metabolism enzymes between the groups ( <xref ref-type="table" rid="t3001">Table 3</xref> ).</p>
          <p>Interestingly, the animals with OJ intake had a higher expression of HO-1 (Figure 1A), although they did not present a difference in Nrf2 expression (Figure 1B).</p>

These alterations characterize the CR process.<xref ref-type="bibr" rid="B31">31</xref> - <xref ref-type="bibr" rid="B33">33</xref> We also observe a decrease in Nrf2 and HO-1 expression. In the chronic phase of MI, the Nrf2 pathway may be diminished by abnormal expression of the Nrf2 target gene, affecting the maintenance of redox homeostasis via enzymes mediated by antioxidant response elements.<xref ref-type="bibr" rid="B34">34</xref> In a previous study conducted in our laboratory with the MI model, lower expression of Nrf2 and HO-1 was observed.<xref ref-type="bibr" rid="B3">3</xref> These findings suggest either a lower expression or greater catabolism of the Nrf2 protein, thus leading to the lower synthesis of HO-1.</p>
        <p>In the present study, OJ intake resulted in attenuation of CR in infarcted animals. This attenuation can be observed in the decrease of LV cavity (LVDD and LVSD) and in the improvement of systolic function, characterized by the increase in the S&#180; mean,<xref ref-type="bibr" rid="B35">35</xref> and diastolic function (lower left atrial diameter). In the study by Yu et al.,<xref ref-type="bibr" rid="B36">36</xref> infarcted rats, by left coronary ligation, treated with hesperidin for four weeks had lower LVDD and LVSD and improved systolic function than infarcted animals. These data are similar to ours and may show the effect of hesperidin of OJ on the CR process. In other study, another phenolic compound, hesperetin, also had an effect on the heart. In pressure-overload model, Deng et al.<xref ref-type="bibr" rid="B37">37</xref> found lower values of LVDD and LVSD at eight weeks of hesperetin administration.</p>
        <p>MI leads to an imbalance between the production of ROS and antioxidant defenses, leading to oxidative stress. After ischemia, some ROSs damage cell membranes, initiating the process of lipid peroxidation.<xref ref-type="bibr" rid="B38">38</xref> For example, Bagatini et al.<xref ref-type="bibr" rid="B39">39</xref> described an increase in lipid peroxidation in patients with MI. Our data also show a higher concentrations of lipid hydroperoxides in infarcted animals compared to non-infarcted animals. The SOD enzyme is the organism&#8217;s first defense against ROS.<xref ref-type="bibr" rid="B40">40</xref> Our data show that infarcted animals compared to non-infarcted animals showed greater activity of the SOD enzyme, as previously described.<xref ref-type="bibr" rid="B3">3</xref> , <xref ref-type="bibr" rid="B25">25</xref> Regarding OJ intake, we observed that the animals that consumed OJ presented lower GSH-Px activity. A similar result was shown by Selvaraj and Pugalendi<xref ref-type="bibr" rid="B41">41</xref> in myocardial ischemia model induced by isoproterenol: rats that received hesperidin presented lower activity of the antioxidant enzymes, among them GSH-Px.<xref ref-type="bibr" rid="B41">41</xref></p>
        <p>In relation to energy metabolism, the heart, like other organs, can adapt and use the best energy substrate in each situation. PFK acts in glycolysis regulation, and catalyzes the phosphorylation of glucose in fructose-6-phosphate and subsequently in fructose 1,6-bisphosphate.<xref ref-type="bibr" rid="B42">42</xref> PFK is activated when ATP concentrations become low and is inhibited when cells have sufficient supply of ATP and other substrates such as fatty acids.<xref ref-type="bibr" rid="B42">42</xref> Our data showed higher values of PFK in infarcted animals with OJ intake. These data show that increased PFK activity may lead to regulation of the glycolytic pathway, by providing more substrate for energy production. Another important finding that indicates a greater use of substrate is the higher ATP synthase activity in the animals that received OJ.</p>
        <p>In addition to oxidative stress and metabolic changes, we found that the infarcted animals that received OJ presented lower values of IFN-&#947;. Since a chronic phase of inflammation is related to an increased production of IFN-&#947;,<xref ref-type="bibr" rid="B43">43</xref> our findings suggest more advanced phase towards the resolution of the inflammatory process. An interesting result is that sham animals who consumed OJ demonstrated an immunomodulatory effect, as shown by higher values of IL-10 and INF-&#947;. Similar to our findings in animals in the sham group, which had no cardiac injury, studies in healthy, middle-aged humans showed that OJ altered leukocyte gene expression to an anti-inflammatory and anti-atherogenic profile<xref ref-type="bibr" rid="B44">44</xref> and provided an early protection of mononuclear blood cell against oxidative DNA damage.<xref ref-type="bibr" rid="B45">45</xref> In addition, OJ intake with the high-carbohydrate meal prevented meal-induced oxidative and inflammatory stress.<xref ref-type="bibr" rid="B46">46</xref></p>
        <p>Another intriguing finding in our study was the higher values of HO-1 in animals that consumed OJ. Lin et al.<xref ref-type="bibr" rid="B47">47</xref> in 2005 also showed that hesperetin induced protein expression of HO-1.<xref ref-type="bibr" rid="B47">47</xref> The HO-1 enzyme plays important role in cell homeostasis because of its catabolic action on the heme group of hemoproteins, generating by-products such as iron, biliverdin, and carbon monoxide. Through these by-products, HO-1 exerts anti-inflammatory, antioxidant, and antiapoptotic action.<xref ref-type="bibr" rid="B48">48</xref> , <xref ref-type="bibr" rid="B49">49</xref> In addition to this classical function, HO-1 participates in cell signaling as amplifier of inductors (heme, oxidants, cytokines, hemodynamic forces, growth factors, hypoxia, and hormones) of transcription factors.<xref ref-type="bibr" rid="B48">48</xref></p>
        <p>Wang et al.<xref ref-type="bibr" rid="B50">50</xref> showed that HO-1 is important for heart homeostasis by protecting it from ischemia and reperfusion-induced lesions and oxidative damage.<xref ref-type="bibr" rid="B50">50</xref> In another study, hemin administration in infarcted mice induced HO-1 activation, which caused a change in infarct macrophages toward a M2 anti-inflammatory phenotype, reduction of infarct scar expansion, and improvement of cardiac function.<xref ref-type="bibr" rid="B51">51</xref> Thus, increased HO-1 may also play an important role in CR attenuation by OJ ( <xref ref-type="fig" rid="f02001">Figure 2</xref> ). In addition, this increase was independent of the Nrf2 pathway, since OJ did not lead to alterations in the expression of this protein. Similar to our findings, Wang et al.<xref ref-type="bibr" rid="B52">52</xref> found that isoliquiritin and isoliquiritigenin, flavonoids derived from liquorice, induced HO-1 expression independent of Nrf2 expression.<xref ref-type="bibr" rid="B52">52</xref> The HO-1 expression can be induced by different pathway and may vary according to the model and treatment used.<xref ref-type="bibr" rid="B47">47</xref></p>

          <title>Limitations</title>
          <p>The OJ used in the study was a commercial, ready-to-eat, pasteurized juice, free of preservatives and sugar. The choice of ready juice was to ensure its standardization. However, the use of other types of juices, made with other types of oranges, could possibly show different responses.</p>
