        <title>Resumo</title>
        <sec>
          <title>Fundamento</title>
          <p>O volume plaquet&#225;rio m&#233;dio (VPM), uma medida simples de ativa&#231;&#227;o plaquet&#225;ria, tornou-se recentemente um t&#243;pico interessante no campo da pesquisa cardiovascular. A reabilita&#231;&#227;o card&#237;aca (RC) baseada em exerc&#237;cios &#233; uma interven&#231;&#227;o abrangente que diminui a morbidade-mortalidade em pacientes com doen&#231;a arterial coronariana (DAC). Estudos sobre os efeitos do exerc&#237;cio f&#237;sico na ativa&#231;&#227;o plaquet&#225;ria t&#234;m produzido resultados conflitantes.</p>
        </sec>
        <sec>
          <title>Objetivo</title>
          <p>O objetivo deste estudo foi determinar o efeito de um programa de RC baseado em exerc&#237;cios sobre o VPM em pacientes com DAC est&#225;vel.</p>
        </sec>
        <sec>
          <title>M&#233;todos</title>
          <p>A amostra foi composta por 300 pacientes consecutivos com DAC est&#225;vel. Os pacientes foram divididos em dois grupos: grupo RC (n = 97) e grupo n&#227;o RC (n = 203). Foi feito um hemograma. As medidas de correla&#231;&#227;o ponto-bisserial foram tiradas para mostrar a correla&#231;&#227;o entre a altera&#231;&#227;o do VPM e a RC. Valor de p&lt;0,05 foi considerado estatisticamente significativo.</p>
        </sec>
        <sec>
          <title>Resultados</title>
          <p>A diminui&#231;&#227;o do VPM foi maior no grupo CR do que no grupo n&#227;o CR [(-1,10 (-1,40-(-0,90)) vs. (-0,10 (-2,00-0,00)); p&lt;0,001]. &#916;VPM teve correla&#231;&#227;o positiva com &#916; neutr&#243;filos (r = 0,326, p&lt;0,001), &#916;TG (r = 0,439, p&lt;0,001), &#916;LDL-c (r = 0,478, p&lt;0,001), &#916;GB (r = 0,412, p&lt;0,001) e &#916;PCR (r = 0,572, p &lt;0,001). Foi encontrada uma correla&#231;&#227;o significativa entre &#916;VPM% e CR (r = 0,750, p &lt;0,001).</p>
        </sec>
        <sec>
          <title>Conclus&#245;es</title>
          <p>Pudemos mostrar que a RC baseada em exerc&#237;cios tem forte rela&#231;&#227;o com a redu&#231;&#227;o do VPM em pacientes com DAC. Consideramos que a diminui&#231;&#227;o da ativa&#231;&#227;o plaquet&#225;ria com RC baseada em exerc&#237;cios pode desempenhar um papel importante na redu&#231;&#227;o do risco tromb&#243;tico em pacientes com DAC est&#225;vel. <title>Popula&#231;&#227;o do estudo</title>
        <p>O tamanho da amostra foi determinado com poder de 80% e margem de erro de 5% ap&#243;s avalia&#231;&#227;o preliminar de 5 a 10 casos. O estudo incluiu 300 pacientes ambulatoriais consecutivos que foram submetidos a angiografia coron&#225;ria (CAG) nos seis meses anteriores devido a angina de peito est&#225;vel e &gt;50% de estenose detectada em pelo menos uma art&#233;ria coron&#225;ria, ou tinham hist&#243;rico de interven&#231;&#227;o coron&#225;ria percut&#226;nea (ICP)/cirurgia coron&#225;ria de revasculariza&#231;&#227;o do mioc&#225;rdio (CRM), e foi encaminhado para um programa de RC de fase III. O grupo n&#227;o RC foi composto por 97 pacientes que n&#227;o concordaram em participar do programa de reabilita&#231;&#227;o. Foram exclu&#237;dos pacientes com doen&#231;as imunol&#243;gicas ou inflamat&#243;rias, doen&#231;as hematol&#243;gicas, sepse, infec&#231;&#245;es locais ou sist&#234;micas ativas, doen&#231;a renal cr&#244;nica (eTFG &lt;30Ml/min/1,73m 2 ), idade &#8804;18 e &gt;80, fra&#231;&#227;o de eje&#231;&#227;o do ventr&#237;culo esquerdo (FEVE) &lt;40%, ou com hist&#243;ria de malignidade.</p>
        <p>Os tratamentos dos pacientes foram otimizados antes da participa&#231;&#227;o, e nenhum deles teve altera&#231;&#245;es na medica&#231;&#227;o durante o estudo.</p>
        <p>Foi realizado o hemograma nos pacientes inclu&#237;dos no grupo RC um dia antes do in&#237;cio do programa e um dia ap&#243;s o t&#233;rmino do programa (que durou seis semanas), ap&#243;s jejum de 12 horas. Por outro lado, nos pacientes que n&#227;o foram inclu&#237;dos no programa, foi coletado sangue para hemograma no momento de inclus&#227;o no estudo e seis semanas depois, em jejum de 12 horas. Todas as amostras foram obtidas em tubos padronizados de &#225;cido etilenodiamino tetra-ac&#233;tico (EDTA). O hemograma foi medido em analisador hematol&#243;gico automatizado Advia 2120 (Siemens). Glicose em jejum, colesterol LDL (LDC-c), triglicer&#237;deos (TG), colesterol HDL (HDL-c), gl&#243;bulos brancos (GB), prote&#237;na C reativa (PCR), creatinina e hemoglobina foram medidos. O &#237;ndice de massa corporal (IMC) foi calculado como peso (kg)/altura (m 2 ).</p>
        <p>O c&#225;lculo do escore de Gensini foi iniciado atribuindo-se um escore de gravidade para cada estenose coron&#225;ria: 1 ponto para estreitamento &#8804;25%, 2 pontos para estreitamento de 26 a 50%, 4 pontos para estreitamento de 51 a 75%, 8 pontos para estreitamento de 76 a 90% estreitamento, 16 pontos para estreitamento de 91 a 99% e 32 pontos para oclus&#227;o total. A partir da&#237;, cada pontua&#231;&#227;o de les&#227;o foi multiplicada por um fator que leva em considera&#231;&#227;o a import&#226;ncia da posi&#231;&#227;o da les&#227;o na circula&#231;&#227;o coron&#225;ria (5 para o tronco da coron&#225;ria esquerda, 2,5 para o segmento proximal da coron&#225;ria descendente anterior esquerda, 2,5 para a proximal segmento da art&#233;ria circunflexa, 1,5 para o segmento m&#233;dio da art&#233;ria coron&#225;ria descendente anterior esquerda, 1,0 para a art&#233;ria coron&#225;ria direita, o segmento distal da art&#233;ria coron&#225;ria descendente anterior esquerda, a art&#233;ria p&#243;stero-lateral e a art&#233;ria marginal obtusa, e 0,5 para outros segmentos). Finalmente, o escore de Gensini foi calculado pela soma das pontua&#231;&#245;es dos segmentos coron&#225;rios individuais. <xref ref-type="bibr" rid="B10">10</xref></p>
        <p>O Ecocardiograma foi feito com base nas diretrizes da Sociedade Americana de Ecocardiografia. O volume diast&#243;lico final (VDF) e o volume sist&#243;lico final (VSF) do ventr&#237;culo esquerdo (VE) foram calculados a partir da visualiza&#231;&#227;o do corte apical de duas e quatro c&#226;maras usando o m&#233;todo de Simpson modificado. A FEVE foi calculada como FEVE=(VDF-VSF)/VSFX100.</p>
        <p>O estudo foi realizado nos termos da Declara&#231;&#227;o de Helsinque e com a aprova&#231;&#227;o do conselho de &#233;tica local.</p>

        <p>Um teste incremental em cicloerg&#244;metro foi aplicado antes do programa de RC em pacientes do grupo RC, para determinar a capacidade de exerc&#237;cio. Durante o teste, o objetivo era atingir a frequ&#234;ncia card&#237;aca m&#225;xima esperada de acordo com a idade (220 menos a idade). As indica&#231;&#245;es para encerrar o teste de esfor&#231;o foram: desvio do segmento ST, angina moderada a grave, queda na press&#227;o arterial sist&#243;lica &gt;10 mmHg (persistentemente abaixo da linha de base), apesar de um aumento na carga de esfor&#231;o, resposta hipertensiva (press&#227;o arterial sist&#243;lica &gt;250 mmHg e/ou press&#227;o arterial diast&#243;lica &gt;115 mmHg), sintomas do sistema nervoso central (por exemplo, ataxia, tontura ou quase s&#237;ncope), fadiga, falta de ar, respira&#231;&#227;o ofegante, c&#227;ibras nas pernas ou claudica&#231;&#227;o. Ap&#243;s descanso de dois minutos, a carga de esfor&#231;o foi aumentada em 25W a cada dois minutos. A frequ&#234;ncia card&#237;aca e a press&#227;o arterial foram monitoradas durante todo o teste. A carga m&#225;xima foi determinada pela capacidade m&#225;xima de exerc&#237;cio.</p>
        <p>O programa de RC foi realizado com a supervis&#227;o de uma equipe multidisciplinar, incluindo um cardiologista, um fisioterapeuta experiente como coordenador e um especialista em fisioterapia e reabilita&#231;&#227;o como diretor m&#233;dico. O programa foi realizado no centro de reabilita&#231;&#227;o card&#237;aca do nosso hospital de cardiologia e cirurgia cardiovascular.</p>
        <p>O programa de RC consiste em exerc&#237;cios aer&#243;bicos e exerc&#237;cios de relaxamento. Com base no resultado do teste ergom&#233;trico, a prescri&#231;&#227;o do exerc&#237;cio foi agendada individualmente. Os pacientes permaneceram no programa cinco dias por semana, por um total de seis semanas. Todos os pacientes do grupo RC conclu&#237;ram o programa. Cada sess&#227;o durava 30 minutos, incluindo o aquecimento de cinco minutos e o relaxamento final de cinco minutos. A intensidade do exerc&#237;cio aer&#243;bio foi prescrita de acordo com a capacidade de exerc&#237;cio de cada indiv&#237;duo.</p>
        <p>A intensidade come&#231;ou em 40-50% da reserva de frequ&#234;ncia card&#237;aca m&#225;xima e gradualmente aumentou para 70-85% da reserva de frequ&#234;ncia card&#237;aca m&#225;xima. A reserva de frequ&#234;ncia card&#237;aca foi avaliada pela f&#243;rmula de Karvonen (FCtreino = (FCm&#225;x. &#8211; FCrepouso) x Intensidade do exerc&#237;cio + FCrepouso). <xref ref-type="bibr" rid="B11">11</xref> FCtreino sendo a frequ&#234;ncia card&#237;aca durante o exerc&#237;cio aer&#243;bio, FCm&#225;x a frequ&#234;ncia card&#237;aca m&#225;xima alcan&#231;ada no teste de cicloerg&#244;metro, e FCrepouso a frequ&#234;ncia card&#237;aca em repouso. A Escala de Taxa de Percep&#231;&#227;o de Esfor&#231;o (RPE, Rate of Perceived Exertion ) de Borg foi usada, e os pacientes se exercitaram com RPE de 13-15. Os pacientes foram monitorados continuamente por eletrocardiografia (ECG) com um transmissor de ECG de 1 canal (Custo med, Ottobrunn, Alemanha) e as medi&#231;&#245;es de press&#227;o arterial sist&#243;lica/diast&#243;lica eram realizadas automaticamente a cada cinco minutos por meio de um sistema de software (Custo med, Ottobrunn, Alemanha). Durante o estudo, os pacientes tamb&#233;m foram encaminhados a um psic&#243;logo, um nutricionista e uma cl&#237;nica para parar de fumar.</p>

        <title>An&#225;lise estat&#237;stica</title>
        <p>As an&#225;lises estat&#237;sticas foram realizadas no software estat&#237;stico SPSS (vers&#227;o 21.0, SPSS, Chicago, IL, EUA). Vari&#225;veis cont&#237;nuas s&#227;o apresentadas em m&#233;dia e desvio-padr&#227;o. Vari&#225;veis categ&#243;ricas s&#227;o apresentadas como n&#250;meros e porcentagens. As vari&#225;veis foram comparadas usando o teste t de Student bicaudal para vari&#225;veis cont&#237;nuas de distribui&#231;&#227;o normal ou o teste U de Mann-Whitney para vari&#225;veis cont&#237;nuas de distribui&#231;&#227;o n&#227;o normal. O teste de Kolmogorov-Smirnov foi aplicado para verificar a normalidade da distribui&#231;&#227;o das vari&#225;veis cont&#237;nuas, e o teste do qui-quadrado para vari&#225;veis categ&#243;ricas. O teste relacionado ou teste dos postos sinalizados de Wilcoxon foi usado para comparar as vari&#225;veis antes e depois da terapia. A correla&#231;&#227;o de Spearman foi realizada para examinar a rela&#231;&#227;o entre as vari&#225;veis cont&#237;nuas. Medidas de correla&#231;&#227;o ponto-bisserial foram realizadas para mostrar a correla&#231;&#227;o entre as altera&#231;&#245;es do VPM e a RC. Valor de p&lt;0,05 foi adotado como estatisticamente significativo.</p>

      <title>Discuss&#227;o</title>
      <p>Este estudo mostrou que a RC baseada em exerc&#237;cios tem uma forte rela&#231;&#227;o com a redu&#231;&#227;o do VPM em pacientes com DAC.</p>
      <p>Estudos sobre o efeito do exerc&#237;cio na ativa&#231;&#227;o plaquet&#225;ria t&#234;m produzido resultados conflitantes at&#233; o momento. Em concord&#226;ncia com nosso estudo, Yaz&#305;c&#305;et al. <xref ref-type="bibr" rid="B7">7</xref> relataram que uma mudan&#231;a no estilo de vida que inclu&#237;sse pelo menos 180 minutos/semana de atividades f&#237;sicas de intensidade moderada diminuiu o VPM em pacientes pr&#233;-hipertensos. Al&#233;m disso, Rauramaa et al., <xref ref-type="bibr" rid="B12">12</xref> mostraram que o treinamento f&#237;sico de intensidade baixa a moderada se associou &#224; diminui&#231;&#227;o da agrega&#231;&#227;o plaquet&#225;ria. Contr&#225;rio a esses resultados, tamb&#233;m foi demonstrado que o teste ergom&#233;trico aumentou o VPM em pacientes com DAC. <xref ref-type="bibr" rid="B11">11</xref> , <xref ref-type="bibr" rid="B13">13</xref> Al&#233;m disso, h&#225; registros de que exerc&#237;cios resistidos graduais aumentam o VPM. <xref ref-type="bibr" rid="B12">12</xref> , <xref ref-type="bibr" rid="B14">14</xref></p>
      <p>O treinamento f&#237;sico pode trazer efeitos ben&#233;ficos &#224;s plaquetas por meio de diferentes mecanismos. Com o treinamento f&#237;sico, o fluxo puls&#225;til na aorta aumenta, e isso pode induzir uma libera&#231;&#227;o aguda e suprarregula&#231;&#227;o de &#243;xido n&#237;trico (NO), que &#233; um mediador potente dos efeitos antiplaquet&#225;rios e supress&#227;o da reatividade plaquet&#225;ria. <xref ref-type="bibr" rid="B7">7</xref> , <xref ref-type="bibr" rid="B15">15</xref> Sabe-se que a RC aumenta o colesterol HDL, o que pode estimular a produ&#231;&#227;o de plaquetas e, assim, diminuir a ativa&#231;&#227;o plaquet&#225;ria. <xref ref-type="bibr" rid="B9">9</xref> Em nosso estudo, o colesterol &#916;HDL foi correlacionado de forma independente com a diminui&#231;&#227;o do VPM.</p>
      <p>O VPM &#233; reconhecido como um marcador inflamat&#243;rio em doen&#231;as cardiovasculares, cerebrovasculares, reumatol&#243;gicas e gastroenterol&#243;gicas. <xref ref-type="bibr" rid="B16">16</xref> Recentemente, demosntrou-se que a presen&#231;a de megacari&#243;citos ativados na medula &#243;ssea se correlaciona com o aumento dos n&#237;veis circulantes de IL-6 em pacientes com aterosclerose. <xref ref-type="bibr" rid="B17">17</xref> Pesquisas anteriores forneceram evid&#234;ncias de que um estilo de vida fisicamente ativo est&#225; associado a biomarcadores inflamat&#243;rios inferiores em todo o corpo, e as a&#231;&#245;es anti-inflamat&#243;rias do treinamento f&#237;sico cr&#244;nico s&#227;o evidentes ap&#243;s 2 a 12 semanas de treinamento supervisionado. <xref ref-type="bibr" rid="B18">18</xref> Em nosso estudo, demonstramos que &#916;PCR foi um dos preditores independentes de &#916;MPV. A redu&#231;&#227;o do VPM ap&#243;s a RC pode ser explicada pelo efeito anti-inflamat&#243;rio dos programas de RC baseados em exerc&#237;cios.</p>
      <p>As contradi&#231;&#245;es nos resultados dos estudos podem ser explicadas por v&#225;rios aspectos do exerc&#237;cio que afetam as fun&#231;&#245;es das plaquetas, como diferentes intensidades de exerc&#237;cio, dura&#231;&#245;es e v&#225;rios n&#237;veis de aptid&#227;o dos indiv&#237;duos. <xref ref-type="bibr" rid="B7">7</xref> Est&#225; comprovado que o exerc&#237;cio intenso agudo aumenta as citocinas pr&#243;-inflamat&#243;rias plasm&#225;ticas. <xref ref-type="bibr" rid="B18">18</xref> O VPM pode refletir as mudan&#231;as na estimula&#231;&#227;o plaquet&#225;ria e no n&#237;vel de produ&#231;&#227;o de plaquetas. <xref ref-type="bibr" rid="B13">13</xref> Um aumento no VPM ap&#243;s o exerc&#237;cio pode ser atribu&#237;do &#224; libera&#231;&#227;o recente de plaquetas grandes jovens, particularmente do pool espl&#234;nico, para a circula&#231;&#227;o. <xref ref-type="bibr" rid="B7">7</xref> Neste estudo, implementamos o programa de RC cinco dias por semana por um total de seis semanas com densidade gradativa crescente, de acordo com a capacidade de exerc&#237;cio de cada indiv&#237;duo.</p>
      <p>A aterosclerose em si pode estimular megacari&#243;citos da medula &#243;ssea, que se mostrou associada &#224; s&#237;ndrome coronariana aguda, por induzir consumo de plaquetas circulantes durante a aterog&#234;nese. <xref ref-type="bibr" rid="B17">17</xref> - <xref ref-type="bibr" rid="B19">19</xref> Um dos mecanismos da RC baseada em exerc&#237;cios potencialmente envolvido na redu&#231;&#227;o de infarto e reinfarto do mioc&#225;rdio pode estar relacionado com a redu&#231;&#227;o do VPM. <xref ref-type="bibr" rid="B9">9</xref> , <xref ref-type="bibr" rid="B20">20</xref></p>
      <p>As plaquetas reticuladas s&#227;o maiores e possivelmente mais ativas do que as plaquetas n&#227;o reticuladas. <xref ref-type="bibr" rid="B1">1</xref> Al&#233;m disso, as reticuladas ou grandes exibem reatividade aumentada apesar da terapia antiplaquet&#225;ria. <xref ref-type="bibr" rid="B21">21</xref> , <xref ref-type="bibr" rid="B22">22</xref> Uma vez que foi demonstrado que o VPM alto &#233; um fator de risco independente para infarto do mioc&#225;rdio (IM) futuro e recorrente, estando associados &#224; s&#237;ndrome coronariana aguda ou a fatores de risco cardiovascular, <xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B19">19</xref> , <xref ref-type="bibr" rid="B23">23</xref> levantou-se o questionamento sobre como diminuir o VPM. <xref ref-type="bibr" rid="B17">17</xref> , <xref ref-type="bibr" rid="B23">23</xref> Embora tenha sido demonstrado anteriormente que o tratamento com estatinas diminui o VPM, <xref ref-type="bibr" rid="B21">21</xref> em nosso estudo a maioria dos pacientes j&#225; estava em uso de estatinas, ent&#227;o nos ativemos ao efeito aditivo da RC na redu&#231;&#227;o do VPM.</p>

        <title>Limita&#231;&#245;es do estudo</title>
        <p>O presente estudo teve algumas limita&#231;&#245;es. Primeiro, avaliamos o VPM apenas uma vez e n&#227;o checamos as mudan&#231;as nele ao longo do tempo. Em segundo lugar, alguns medicamentos antiplaquet&#225;rios podem afetar o tamanho das plaquetas.</p>

      <p>Este estudo mostrou que a RC baseada em exerc&#237;cios tem forte rela&#231;&#227;o com a redu&#231;&#227;o do VPM em pacientes com DAC.
          <title>Background</title>
          <p>Mean platelet volume (MPV), which is a simple measure of platelet activation, has recently become an interesting topic in cardiovascular research. Exercise-based cardiac rehabilitation (CR) is a comprehensive intervention that decreases mortality-morbidity in patients with coronary artery disease (CAD). Studies on the effects of exercise on platelet activation have yielded conflicting results.</p>
        </sec>
        <sec>
          <title>Objective</title>
          <p>The purpose of this study was to determine the effect of an exercise-based CR programs on MPV in patients with stable CAD.</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p>The sample was composed of 300 consecutive stable CAD patients. The patients were divided into two groups: CR group (n = 97) and non-CR group (n = 203). Blood analysis was performed. Point-Biserial correlation measures were performed to show correlation between MPV change and CR. A p value of &lt;0.05 was considered statistically significant.</p>
        </sec>
        <sec>
          <title>Results</title>
          <p>The decrease in MPV was greater in the CR group than in the non-CR group [(-1.10(-1.40-(-0.90)) vs. (-0.10 (-2.00-0.00)); p&lt; 0.001]. &#916;MPV had a positive correlation with &#916; neutrophil (r = 0.326, p &lt; 0.001), &#916;TG (r = 0.439, p &lt; 0.001), &#916;LDL-c (r = 0.478, p &lt; 0.001), &#916;WBC (r = 0.412, p &lt; 0.001), and &#916;CRP (r = 0.572, p &lt; 0.001). A significant correlation was found between &#916;MPV% and CR (r=0.750, p&lt;0.001).</p>
        </sec>
        <sec>
          <title>Conclusions</title>
          <p>We were able to show that exercise-based CR has a strong relationship with MPV reduction in patients with CAD. We consider that decreased platelet activation with exercise-based CR might play an important role in reducing thrombotic risk in patients with stable CAD. <title>Introduction</title>
        <p>Cardiovascular (CV) disease is one of the leading causes of mortality and disability and remains a major concern despite improved clinical outcomes with evidence-based treatment.<xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B2">2</xref> Platelets are essential for primary hemostasis and repair of the endothelium, but they also play a key role in the pathogenesis of atherosclerosis and arterial thrombosis.<xref ref-type="bibr" rid="B1">1</xref> Platelet activation is associated with CV events.<xref ref-type="bibr" rid="B3">3</xref> Monitoring the function of platelets may help to evaluate the prognosis of patients with coronary artery disease (CAD).<xref ref-type="bibr" rid="B4">4</xref></p>
        <p>However, because platelet function testing is a time-consuming, costly, and technically challenging process, it is not widely used.<xref ref-type="bibr" rid="B4">4</xref> Compared with smaller platelets, larger platelets contain dense granules, express more adhesion receptors, and induce higher thrombotic activity, which can reflect the degree of platelet activation.<xref ref-type="bibr" rid="B5">5</xref> Mean platelet volume (MPV) is a major parameter of platelet size that has been proposed as an indicator of platelet reactivity and is routinely determined by complete count analyzers at a relatively low cost.<xref ref-type="bibr" rid="B6">6</xref> Increased MPV levels have been reported to be associated with CAD, myocardial infarction, peripheral arterial disease, cerebrovascular disease, and poor outcome.<xref ref-type="bibr" rid="B7">7</xref></p>

        <title>Methods</title>
        <sec>
          <title>Study population</title>
          <p>Sample size was determined with 80% power and 5% margin of error after preliminary evaluation of 5-10 cases. The study included 300 consecutive outpatients who had undergone coronary angiography (CAG) in the previous six months due to stable angina pectoris and &gt;50% stenosis detected in at least one coronary artery, or had a history of percutaneous coronary intervention (PCI)/coronary artery bypass graft (CABG) surgery, and had been referred to a Phase-III CR program. The non-CR group was composed of 97 patients who did not agree to participate in the CR program. Patients with immunologic or inflammatory diseases, hematological diseases, sepsis, active local or systemic infections, chronic renal disease (eGFR&lt;30Ml/min/1.73m2), age &#8804;18 and &gt;80, left ventricular ejection fraction (LVEF) &lt;40%, or with history of malignancy were excluded.</p>
          <p>Patients&#8217; medical treatments were optimized before the participation, and none of them had changes in medication during the study.</p>
          <p>Blood analysis was performed in patients included in the CR group one day before the start of the program and one day after the end of the program (which lasted six weeks) after a 12-hour fasting. On the other hand, in patients who were not included in the CR program, blood analysis was performed upon their inclusion in the study and six weeks later, after a 12-hour fasting. All samples were obtained in standardized dipotassium ethylenediaminetetraacetic acid (EDTA) tubes. The blood counts were measured using an automated hematology analyzer Advia 2120 (Siemens). Fasting blood glucose, low-density lipoprotein cholesterol (LDC-c), triglycerides (TG), high-density lipoprotein cholesterol (HDL-c), white blood cell (WBC), C-reactive protein (CRP), creatinine, and hemoglobin values were measured. Body mass index (BMI) was calculated as weight (kg)/height (m2).</p>
          <p>Calculation of the Gensini score was initiated by giving a severity score to each coronary stenosis: 1 point for &#8804;25% narrowing, 2 points for 26 to 50% narrowing, 4 points for 51 to 75% narrowing, 8 points for 76 to 90% narrowing, 16 points for 91 to 99% narrowing, and 32 points for total occlusion. Thereafter, each lesion score was multiplied by a factor that takes into account the importance of the lesion&#8217;s position in the coronary circulation (5 for the left main coronary artery, 2.5 for the proximal segment of the left anterior descending coronary artery, 2.5 for the proximal segment of the circumflex artery, 1.5 for the mid-segment of the left anterior descending coronary artery, 1.0 for the right coronary artery, the distal segment of the left anterior descending coronary artery, the poster lateral artery, and the obtuse marginal artery, and 0.5 for other segments). Finally, the Gensini score was calculated by summation of the individual coronary segment scores.<xref ref-type="bibr" rid="B10">10</xref></p>
          <p>Echocardiographic measurements were taken according to the American Society of Echocardiography guidelines. The left ventricle (LV) end-diastolic volume (EDV) and end-systolic volume (ESV) were calculated from the apical two- and four-chamber views using the modified Simpson&#8217; method. The LVEF was calculated as LVEF= (EDV-ESV)/EDVX100.</p>
          <p>This study was performed in accordance with the Declaration of Helsinki and with the approval by the local ethics board.</p>

          <title>CR program</title>
          <p>A step incremental cycle ergometer test was applied before the CR program in patients in the CR group, so as to determine their exercise capacity. During the test, the aim was to reach the expected maximum heart rate according to age (220 minus age). The indications for terminating the exercise testing were: ST-segment deviation, moderate to severe angina, drop in systolic blood pressure &gt;10 mmHg (persistently below baseline) despite an increase in workload, hypertensive response (systolic blood pressure &gt;250 mmHg and/or diastolic blood pressure &gt;115 mmHg), central nervous system symptoms (e.g., ataxia, dizziness, or near syncope), fatigue, shortness of breath, wheezing, leg cramps, or claudication. After a two-minute resting period, the workload was increased by 25W every two minutes. Heart rate and blood pressure were measured during the whole test. The maximum workload was determined as the maximal exercise capacity.</p>
          <p>The CR program was performed with the supervision of a multidisciplinary team, including a cardiologist, an experienced physical therapist as a coordinator, and a physical therapy and rehabilitation specialist as the medical director. The rehabilitation program was performed in the CR center of our cardiology and cardiovascular surgery hospital.</p>
          <p>The CR program consists of aerobic exercise training and relaxation exercises. Based on the result of the exercise testing, exercise prescription was scheduled individually. Patients remained in the program five days a week, for a total of six weeks. All patients in the CR group completed the program. Each session lasted 30 minutes, including the five-minute warm-up and the final five-minute cool-down. The aerobic exercise intensity was prescribed according to the individual&#8217;s exercise capacity. The intensity began at 40-50% of maximal heart rate reserve and gradually increased to 70-85% of maximal heart rate reserve. Heart rate reserve was evaluated by the Karvonen Formula (HRtrain = (HRmax &#8211; HRrest) x ExerciseIntensity + HRrest).<xref ref-type="bibr" rid="B11">11</xref> HRtrain being the heart rate during the aerobic exercise, HRmax the maximum heart rate reached thorough the cycle ergometer test, and HRrest the heart rate at rest. The Borg Scale of Rate of Perceived Exertion (RPE) was used, and patients exercised at an RPE of 13-15. Patients were continuously monitored by electrocardiography (ECG) with a 1-channel ECG transmitter (Custo med, Ottobrunn, Germany), and systolic/diastolic blood pressure measurements were performed automatically every five minutes via a software system (Custo med, Ottobrunn, Germany). During the study, patients were also directed to a psychologist, a dietitian, and a smoking cessation clinic.</p>

          <title>Statistical analysis</title>
          <p>Statistical analyses were carried out using the SPSS statistical software (version 21.0, SPSS, Chicago, IL, USA). Continuous variables are presented as mean and standard deviation. Categorical variables are presented as numbers and percentages. The variables were compared using the two-tailed student t test for continuous variables of normal distribution or the Mann-Whitney U test for continuous variables of non-normal distribution. The Kolmogorov-Smirnov test was applied to verify the normality of the distribution of continuous variables. The chi-square test was used for categorical variables. The related test or Wilcoxon signed-rank test was used to compare variables before and after therapy. Spearman&#8217;s correlation analysis was performed to examine the relationship between continuous variables. Point-Biserial correlation measures were performed to show the correlation between MPV changes and the CR. A p-value of &lt;0.05 was defined as statistically significant.</p>

        <title>Discussion</title>
        <p>This study showed that exercise-based CR has a strong relationship with MPV reduction in patients with CAD.</p>
        <p>Studies on the effect of exercise on platelet activation have yielded conflicting results so far. In agreement with our study, Yaz&#305;c&#305;et al.<xref ref-type="bibr" rid="B7">7</xref> reported that a lifestyle change that includes at least 180 minutes/week of moderate-intensity physical activities decreased MPV in pre-hypertensive patients. Also, Rauramaa et al.<xref ref-type="bibr" rid="B12">12</xref> showed that low to moderate intensity exercise training was associated with diminished platelet aggregation. Contrary to these results, it was shown that exercise stress test increased MPV in patients with CAD.<xref ref-type="bibr" rid="B11">11</xref> , <xref ref-type="bibr" rid="B13">13</xref> Also, it has been shown that graded resistance exercises increase MPV.<xref ref-type="bibr" rid="B12">12</xref> , <xref ref-type="bibr" rid="B14">14</xref></p>
        <p>Exercise training can bring beneficial effects on platelets via different mechanisms. With exercise training, the pulsatile flow in the aorta increases, and this might induce an acute release and upregulation of nitric oxide (NO), which is a potent mediator of antiplatelet effects and suppression of platelet reactivity.<xref ref-type="bibr" rid="B7">7</xref> , <xref ref-type="bibr" rid="B15">15</xref> It is known that CR increases HDL cholesterol, which can stimulate platelet production and thereby decrease platelet activation.<xref ref-type="bibr" rid="B9">9</xref> In our study, &#916;HDL-cholesterol was found to be independently correlated with a decrease in MPV.</p>
        <p>MPV has been recognized as an inflammatory marker in cardiovascular, cerebrovascular, rheumatologic, and gastroenterological diseases.<xref ref-type="bibr" rid="B16">16</xref> Recently, it was shown that the presence of activated megakaryocytes in bone marrow correlates with increased circulating levels of IL-6 in patients with atherosclerosis.<xref ref-type="bibr" rid="B17">17</xref> Previous research provided evidence that a physically active lifestyle is associated with lower whole-body inflammatory biomarkers, and the anti-inflammatory actions of chronic exercise training are evident after as little as 2 to 12 weeks of supervised exercise training.<xref ref-type="bibr" rid="B18">18</xref> In our study, we demonstrated that &#916;CRP was one of the independent predictors of &#916;MPV. Reduction in MPV after CR might be explained by the anti-inflammatory effect of exercise-based CR programs.</p>
        <p>The contradictions in the results of studies can be explained by various aspects of exercise affecting platelet functions such as different exercise intensity, duration, and various fitness levels of the subjects.<xref ref-type="bibr" rid="B7">7</xref> Acute intense exercise is proven to increase plasma pro-inflammatory cytokines.<xref ref-type="bibr" rid="B18">18</xref> MPV can reflect the changes in platelet stimulation and the rate of platelet production level.<xref ref-type="bibr" rid="B13">13</xref> An increase in MPV after exercise may be attributed to the fresh release of young large platelets, particularly from the splenic pool, into the circulation.<xref ref-type="bibr" rid="B7">7</xref> In this study, we implemented the CR program five days per week for a total of six weeks with a gradually increasing density, according to the individual&#8217;s exercise capacity.</p>
        <p>Atherosclerosis itself can stimulate bone marrow megakaryocytes, which was shown to be associated with acute coronary syndrome, by causing circulating platelet consumption during athereogenesis.<xref ref-type="bibr" rid="B17">17</xref> - <xref ref-type="bibr" rid="B19">19</xref> One of the mechanisms of exercise-based CR potentially involved in the reduction of infarction and re-infarction may be related with reduction of MPV.<xref ref-type="bibr" rid="B9">9</xref> , <xref ref-type="bibr" rid="B20">20</xref></p>
        <p>Reticulated platelets are larger and possibly more active than non-reticulated platelets.<xref ref-type="bibr" rid="B1">1</xref> Moreover, reticulated or large platelets exhibit increased reactivity despite anti-platelet therapy.<xref ref-type="bibr" rid="B21">21</xref> , <xref ref-type="bibr" rid="B22">22</xref> Since it has been shown that high MPV is an independent risk factor for future myocardial infarction (MI) and recurrent MI, being associated with acute coronary syndrome or cardiovascular risk factors,<xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B19">19</xref> , <xref ref-type="bibr" rid="B23">23</xref> the questioning on how to decrease MPV has already been raised.<xref ref-type="bibr" rid="B17">17</xref> , <xref ref-type="bibr" rid="B23">23</xref> Although it was previously shown that statin treatment decreases MPV,<xref ref-type="bibr" rid="B21">21</xref> in our study most of the patients were already on statins, so we showed the additive effect of CR on MPV reduction.</p>

          <title>Study limitations</title>
          <p>The present study has some limitations. First, we evaluated MPV only once and did not asses changes in it over time. Second, some anti-platelet drugs might affect platelet size.</p>

        <title>Conclusion</title>
        <p>This study showed that exercise-based CR has a strong relationship with MPV reduction in patients with CAD. We consider that decreased platelet activation with exercise-based CR might play an important role in reducing thrombotic risk in patients with stable CAD.</p>
