    <p>&#8220;N&#227;o existe revela&#231;&#227;o mais n&#237;tida da alma de uma sociedade do que a forma como ela trata as suas crian&#231;as.&#8221;</p>
    <p>Nelson Mandela (8 maio 1995)</p>
    <p>Mundialmente, estamos vivendo uma era sobrecarregada por doen&#231;as card&#237;acas cong&#234;nitas (DCC).<xref rid="B1" ref-type="bibr">1</xref> Ao tirar uma fotografia instant&#226;nea no ano de 2018, vemos que houve 2.944.932 nascidos vivos no Brasil (Tabnet, DATASUS). Considerando a incid&#234;ncia das DCC, cerca de 9 a cada 1.000 nascidos vivos (&#237;ndice que tem permanecido est&#225;vel em todos os pa&#237;ses e popula&#231;&#245;es), seria de se esperar cerca de 26.000 novos pacientes com alguma forma de DCC em nosso pa&#237;s. Por outro lado, a taxa de sobreviv&#234;ncia mundial para as DCC durante as &#250;ltimas 3 d&#233;cadas tem sido pr&#243;xima a 98%.<xref rid="B2" ref-type="bibr">2</xref> Dito isso, as DCC continuam um problema de sa&#250;de p&#250;blica em todo o mundo.</p>
    <p>Durante os &#250;ltimos quatro s&#233;culos, a especialidade da cardiologia pedi&#225;trica tem crescido imensamente, alcan&#231;ando diversos marcos no cen&#225;rio cl&#237;nico e cir&#250;rgico, devido, principalmente, aos incr&#237;veis avan&#231;os nas ferramentas diagn&#243;sticas de imagem cardiovascular. Isso permite o diagn&#243;stico n&#227;o invasivo em todas as idades, desde a vida fetal at&#233; a idade adulta. A modalidade relativamente nova de imagem por resson&#226;ncia magn&#233;tica card&#237;aca (RMC) fornece informa&#231;&#245;es clinicamente &#250;teis em v&#225;rias situa&#231;&#245;es de pacientes com DCC. As diretrizes atuais para diagn&#243;stico e tratamento incluem ecocardiograma, RMC e cateterismo card&#237;aco para a maioria dos pacientes com DCC.<xref rid="B3" ref-type="bibr">3</xref></p>
    <p>Kozak et al.,<xref rid="B4" ref-type="bibr">4</xref> tiraram uma &#8220;fotografia instant&#226;nea&#8221; da situa&#231;&#227;o brasileira em rela&#231;&#227;o ao uso da RMC em popula&#231;&#245;es pedi&#225;tricas, entrevistando cardiologistas da maioria dos estados, incluindo centros com diferentes n&#237;veis de aten&#231;&#227;o e volumes cir&#250;rgicos. A &#8220;imagem&#8221; foi clara: a RMC n&#227;o est&#225; sendo plenamente utilizada! Aproximadamente metade dos cardiologistas (52%) raramente utilizam a RMC para crian&#231;as. Ao ampliarmos esta imagem, duas camadas principais s&#227;o reveladas. A primeira camada envolve o processo de mudan&#231;a de pr&#225;ticas e da incorpora&#231;&#227;o de novas tecnologias. O uso predominante da RMC ainda depende do uso para cardiomiopatias, tetralogia de Fallot p&#243;s-reparo e anomalias do arco a&#243;rtico, segundo esses dados. No entanto, a era atual da RMC em cardiologia vai al&#233;m da avalia&#231;&#227;o anat&#244;mica, permitindo acesso a dados hemodin&#226;micos e outros dados funcionais que impactam no atendimento aos pacientes, na tomada de decis&#227;o cir&#250;rgica e at&#233; mesmo nos fatores de risco progn&#243;stico. A segunda camada, n&#227;o surpreendentemente, &#233; o custo da RMC, que deve ser analisado no contexto dos custos de sa&#250;de para popula&#231;&#245;es com DCC. &#192; medida que os pa&#237;ses se desenvolvem economicamente, a carga das condi&#231;&#245;es relacionadas &#224; pobreza diminui e &#233; substitu&#237;da por necessidades de cuidados cr&#244;nicos e frequentemente complexos, como as DCC. A taxa de sobreviv&#234;ncia e a qualidade de vida desses pacientes, desde a inf&#226;ncia at&#233; a idade adulta, dependem da excel&#234;ncia do atendimento m&#233;dico. Devido &#224; sua complexidade, as les&#245;es card&#237;acas cirurgicamente reparadas requerem vigil&#226;ncia ao longo da vida.<xref rid="B5" ref-type="bibr">5</xref> Portanto, todos esses pontos devem ser levados em considera&#231;&#227;o no planejamento de cuidados ideais para pacientes com DCC. &#201; caro e requer investimento ao longo da vida. No entanto, o investimento n&#227;o &#233; a solu&#231;&#227;o &#250;nica; h&#225; uma tr&#237;ade que precisa ser abordada simultaneamente, &#8220;pessoas, processo e tecnologia&#8221;. Kozak et al.,<xref rid="B4" ref-type="bibr">4</xref> demonstrou que existem tr&#234;s limita&#231;&#245;es principais: 1) custo (65%), 2) necessidade de seda&#231;&#227;o (60%) e 3) n&#250;mero insuficiente de profissionais qualificados (55%). A estas limita&#231;&#245;es interp&#245;e-se a&#231;&#245;es para atender &#224; tr&#237;ade: o investimento em novas tecnologias na &#225;rea da sa&#250;de, a necessidade de desenvolvimento profissional, e um processo para tornar a tecnologia eficaz e impactar a qualidade do atendimento. O treinamento de cardiologistas pedi&#225;tricos no mundo de hoje requer uma sub-especialidade em diagn&#243;stico por imagem, que tem avan&#231;ado demasiadamente lenta, principalmente no que diz respeito ao treinamento de RMC.<xref rid="B6" ref-type="bibr">6</xref> O aumento do n&#250;mero e da qualidade de profissionais aptos a executar e interpretar os resultados gerar&#225; um novo ciclo, incluindo o uso correto da RMC em pacientes com DCC, especialmente na popula&#231;&#227;o pedi&#225;trica.</p>
    <p>Estes dados t&#234;m o poder de desencadear m&#250;ltiplas a&#231;&#245;es no cuidado aos pacientes com DCC, a saber, aloca&#231;&#227;o de recursos, capacita&#231;&#227;o profissional e mudan&#231;as nas pr&#225;ticas. Todas essas a&#231;&#245;es aliadas a fortes pr&#225;ticas de cuidado nos mover&#227;o na mesma dire&#231;&#227;o, promovendo uma assist&#234;ncia &#224; sa&#250;de de qualidade.</p>

        <p>&#8220;There can be no keener revelation of a society&#8217;s soul than the way in which it treats its children.&#8221;</p>
        <p>Nelson Mandela (8 May 1995)</p>
      </disp-quote>
      <p>Worldwide, we are living in an era burdened by congenital heart diseases (CHD).<xref rid="B1" ref-type="bibr">1</xref> If we take a snapshot in 2018, there were 2,944,932 live births in Brazil (Tabnet, DATASUS). Considering the incidence of CHD, about 9 per 1000 live births, which has been stable across countries and populations, we would expect around 26,000 new patients with some form of CHD in our country. On the other hand, worldwide survival rate for CHD in the last 3 decades has been close to 98%.<xref rid="B2" ref-type="bibr">2</xref> Having said that, CHD continues to be a public health problem worldwide.</p>
      <p>In the last four centuries, the specialty of pediatric cardiology has grown tremendously, achieving several milestones in clinical and surgical scenarios, mostly due the incredible advances in cardiovascular imaging diagnostic tools. This allows non-invasive diagnosis in all ages from fetal life until adulthood. The relatively new modality of cardiac magnetic resonance imaging (CMR) provides clinically useful information in various situations in patients with CHD. Current guidelines for diagnosis and management include echocardiogram, CMR and cardiac catheterization for the majority of CHD patients.<xref rid="B3" ref-type="bibr">3</xref></p>
      <p>Kozak et al.<xref rid="B4" ref-type="bibr">4</xref> took a snapshot of the Brazilian situation on the use of CMR in pediatric populations interviewing cardiologists from most states, including centers with different levels of care and surgical volumes. The big picture: it is not being fully used! About half of the cardiologists (52%) rarely use CMR for children. If we magnify this picture, two major layers are revealed. The first layer involves the process of changing practices and incorporating new technologies. The predominant use of CMR still relies on use for cardiomyopathies, tetralogy of Fallot post-repair, and aortic arch anomalies, according to this data. However, the current era of CMR in cardiology goes beyond anatomical assessment, allowing us to access hemodynamic and other functional data which impacts patient care, surgical decision making, and even prognostic risk factors. The second layer, not surprisingly, is the cost of CMR, which should be analysed in the context of healthcare costs for populations with CHD. As countries develop economically, the burden of poverty-related conditions diminishes and is substituted by chronic and often complex care needs, such as CHD. The survival rate and quality of life for these patients from childhood into adulthood depends on excellence of medical care. The complexity of surgically repaired heart lesions requires lifelong surveillance.<xref rid="B5" ref-type="bibr">5</xref> Therefore, all these points should be taken in consideration when planning ideal care for patients with CHD. It is expensive, and it requires investment during a lifelong journey. However, investment is not a single solution; it is a triad which needs to be addressed simultaneously, &#8220;people, process, and technology&#8221;. Kozak et al.<xref rid="B4" ref-type="bibr">4</xref> has shown that there are three major limitations: 1) cost (65%), 2) the need for sedation (60%), and 3) insufficient number of qualified professionals (55%). These address the triad: the investment in new technology in healthcare, the need for professional development, and a process to make the technology effective and impact the quality of care. The training of pediatric cardiologist in today&#8217;s world requires sub-specialty in imaging diagnosis, which has been advancing too slowly, especially with respect to CMR training.<xref rid="B6" ref-type="bibr">6</xref> The increasing number and quality of professionals able to perform and interpret results will generate a new cycle, including correct use of CMR in patients with CHD, especially in pediatric populations.</p>
      <p>This data has the power to trigger multiple actions in caring for patients with CHD, namely, allocation of resources, professional training, and changes in practices. All these actions coupled with strong care practices move us in the same direction, promoting high quality health care.</p>
