    <p>A hist&#243;ria recente da taquicardia atrial para-Hissiana (TAPH) se parece muito com a hist&#243;ria mais antiga da taquicardia de reentrada nodal atrioventricular (TRNAV). O debate sobre os limites anat&#244;micos precisos da reentrada nodal atrioventricular continua hoje no que diz respeito &#224; TRNAV,<xref ref-type="bibr" rid="B1">1</xref> mesmo 75 anos ap&#243;s a primeira suspeita de que alguns mecanismos de taquicardia supraventricular poderiam envolver a regi&#227;o do n&#243; atrioventricular.<xref ref-type="bibr" rid="B2">2</xref> Nesse tipo de arritmia, o primeiro alvo para a terapia ablativa por cateter foi a via r&#225;pida do n&#243; atrioventricular:<xref ref-type="bibr" rid="B3">3</xref> essa abordagem apresentou alta taxa de sucesso, mas a indu&#231;&#227;o do bloqueio atrioventricular foi encontrada em mais de um a cada cinco pacientes.<xref ref-type="bibr" rid="B4">4</xref> Ap&#243;s alguns anos, foi proposta uma abordagem por via lenta,<xref ref-type="bibr" rid="B5">5</xref> que se mostrou mais eficaz e segura do que a abordagem por via r&#225;pida.<xref ref-type="bibr" rid="B4">4</xref> Por alguns anos, essa nova abordagem gerou algum debate na comunidade de eletrofisiologia. Alguns autores chegam a sugerir a substitui&#231;&#227;o de uma t&#233;cnica por outra, desde que a TRNAV persistisse.<xref ref-type="bibr" rid="B6">6</xref> Atualmente, a abordagem pela via r&#225;pida foi definitivamente abandonada e, quando pensamos em abla&#231;&#227;o por cateter de TRNAV, pensamos apenas na abla&#231;&#227;o por via lenta.</p>
    <p>A TAPH &#233; um grupo de taquicardias atriais (ATs) com origem pr&#243;xima &#224; regi&#227;o do feixe de His. Sua preval&#234;ncia &#233; bastante elevada em algumas casu&#237;sticas,<xref ref-type="bibr" rid="B7">7</xref> por isso &#233; importante aprender a reconhec&#234;-las e trat&#225;-las. Hoje em dia, da mesma forma que a TRNAV, tamb&#233;m para a TAPH existe um debate tanto sobre o s&#237;tio anat&#244;mico de origem desse tipo de arritmia,<xref ref-type="bibr" rid="B8">8</xref> tanto sobre o mecanismo<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B10">10</xref> quanto sobre a melhor abordagem de abla&#231;&#227;o por cateter.<xref ref-type="bibr" rid="B11">11</xref>-<xref ref-type="bibr" rid="B14">14</xref> Alguns autores sugerem a presen&#231;a de um pequeno circuito reentrante adjacente ao anel tric&#250;spide;<xref ref-type="bibr" rid="B10">10</xref> outros descreveram a TAPH como ATs focais originando-se de v&#225;rios pontos ao redor do anel tric&#250;spide ou mitral.<xref ref-type="bibr" rid="B9">9</xref> As TAPH s&#227;o pass&#237;veis de abla&#231;&#227;o por cateter por v&#225;rias abordagens, incluindo o septo interatrial direito, septo interatrial esquerdo por pun&#231;&#227;o transeptal e seio n&#227;o coron&#225;rio (SNC) de Valsalva da raiz a&#243;rtica por abordagem transa&#243;rtica via art&#233;ria femoral.</p>
    <p>Nesta edi&#231;&#227;o dos Arquivos Brasileiros de Cardiologia, Chokr et al.<xref ref-type="bibr" rid="B15">15</xref>, &#233; importante descrever uma s&#233;rie de casos de pacientes submetidos &#224; abla&#231;&#227;o do seio de Valsalva. Um dos achados mais relevantes de seu trabalho foi que esse tipo de abla&#231;&#227;o &#233; vi&#225;vel com uma exposi&#231;&#227;o radiol&#243;gica relativamente baixa, tamb&#233;m sem um sistema de mapeamento eletroanat&#244;mico 3-D e sem a ecocardiografia intracard&#237;aca, m&#233;todo que a comunidade de eletrofisiologia atual parece n&#227;o conseguir deixar de fora. Por esse motivo, o m&#233;todo pode ser vi&#225;vel em todos os laborat&#243;rios de eletrofisiologia.</p>
    <p>No entanto, mesmo que j&#225; tenha sido comprovado que uma abordagem ablativa do seio de Valsalva &#233; mais eficaz e segura do que uma abordagem da regi&#227;o do feixe de His no septo interatrial direito e/ou esquerdo,<xref ref-type="bibr" rid="B12">12</xref>,<xref ref-type="bibr" rid="B14">14</xref> independentemente do local de ativa&#231;&#227;o atrial mais precoce, alguns autores sugerem como primeira escolha em algumas situa&#231;&#245;es a abla&#231;&#227;o do septo interatrial direito ou esquerdo,<xref ref-type="bibr" rid="B11">11</xref>,<xref ref-type="bibr" rid="B13">13</xref> apesar do risco de danos ao sistema de condu&#231;&#227;o atrioventricular. Provavelmente, como na hist&#243;ria do TRNAV, daqui a mais 25 anos, quando pensarmos em abla&#231;&#227;o para TAPH, pensaremos apenas na abordagem do seio de Valsalva, e a abordagem da regi&#227;o do feixe de His ser&#225; definitivamente esquecida!</p>

    <body>
      <p>The recent history of the para-hisian atrial tachycardia (PHAT) looks a lot like the older history of the atrioventricular (AV) nodal reentry tachycardia (AVNRT). The debate about precise anatomic boundaries of AV nodal reentry continues today again in the AVNRT,<xref ref-type="bibr" rid="B1">1</xref> even 75 odd years after the first suspect that some mechanisms of supraventricular tachycardia could involve the region of the AV node.<xref ref-type="bibr" rid="B2">2</xref> In this type of arrhythmia, the first target for the catheter ablative therapy was the fast pathway of the AV node:<xref ref-type="bibr" rid="B3">3</xref> this approach showed a high success rate, but induction of AV block was found in more than one every five patients.<xref ref-type="bibr" rid="B4">4</xref> After a few years, an approach was proposed from the slow pathway,<xref ref-type="bibr" rid="B5">5</xref> that proved to be more effective and safer than the approach from the fast pathway.<xref ref-type="bibr" rid="B4">4</xref> For some years, this new approach generated some debate in the electrophysiology (EP) community. Some authors even suggested to cross over from one technique to the other as long as AVNRT persisted.<xref ref-type="bibr" rid="B6">6</xref> Nowadays, the approach from fast pathway is definitely abandoned and when we think of catheter ablation of AVNRT, we think only about ablation of the slow pathway.</p>
      <p>PHAT are a group of atrial tachycardia (ATs) originating near the His-bundle (HB) region. Their prevalence is quite high in some casuistries,<xref ref-type="bibr" rid="B7">7</xref> therefore it is important learning to recognize and to treat them. Nowadays, in the same manner of AVNRT, also for PHAT a debate is present for both the anatomic site of origin of this type of arrhythmia,<xref ref-type="bibr" rid="B8">8</xref> both about the mechanism,<xref ref-type="bibr" rid="B9">9</xref>,<xref ref-type="bibr" rid="B10">10</xref> both about the best catheter ablation approach.<xref ref-type="bibr" rid="B11">11</xref>-<xref ref-type="bibr" rid="B14">14</xref> Some authors hypothesized the presence of a small re-entrant circuit adjacent to the tricuspid annulus;<xref ref-type="bibr" rid="B10">10</xref> in contrast, other authors described PHAT as focal ATs arising from various location around the tricuspid or mitral annuli.<xref ref-type="bibr" rid="B9">9</xref> PHAT are amenable to catheter ablation from multiple approaches including right inter-atrial septum, left inter-atrial septum by a transeptal puncture and non coronary sinus (NCS) of Valsalva of the aortic root by a transaortic approach via the femoral artery.</p>
      <p>In this issue of Arquivos Brasileiros de Cardiologia, Chokr et al.<xref ref-type="bibr" rid="B15">15</xref>importantly, describe a case-series of patients ablated from the NCS of Valsalva. One of the most relevant findings of their work is the fact the this type of ablation is feasible with a relatively low radiological exposure also without a 3-D electroanatomic mapping system and without the intracardiac echocardiography, of which the current EP community seems to be unable to do without. For this reason, this method can be feasible in every EP lab. However, even if it was already proved that an ablative approach from the NCS of Valsalva is more effective and safer than an approach from the HB region in the right and/or left inter-atrial septum,<xref ref-type="bibr" rid="B12">12</xref>,<xref ref-type="bibr" rid="B14">14</xref>regardless of the earliest atrial activation site, there are authors that in some situations suggest as first choice the ablation of the right or left inter-atrial septum,<xref ref-type="bibr" rid="B11">11</xref>,<xref ref-type="bibr" rid="B13">13</xref> despite the risk of creating damages to the AV conduction system. Probably, as in AVNRT history, in another 25 years, when we will think about PHAT ablation, we will think only about the NCS of Valsalva approach, and the approach from the HB region will be definitely forgotten!</p>
