      <title>Relato de Caso</title>
      <p>Apresenta-se um caso de um paciente do sexo masculino, 62 anos, fumante, com hist&#243;rico m&#233;dico de hipertens&#227;o e dislipidemia. Ele foi internado pela primeira vez devido a um infarto agudo do mioc&#225;rdio com supradesnivelamento do segmento ST (IAMCSST) da parede lateral. Uma angiografia coron&#225;ria emergente (realizada 2 horas ap&#243;s o in&#237;cio da dor tor&#225;cica) revelou estenose de 80% da art&#233;ria coronariana descendente anterior esquerda (DAE) m&#233;dia, oclus&#227;o total da Dg1 (primeiro ramo diagonal da DAE) em seu &#243;stio e estenose distal de 70% do ramo posterolateral da art&#233;ria coronariana circunflexa esquerda (PL). Foi realizada angioplastia com implanta&#231;&#227;o de stent farmacol&#243;gico na DAE e dilata&#231;&#227;o com bal&#227;o da Dg1. Dias depois foi efetuada angioplastia com implanta&#231;&#227;o de stent farmacol&#243;gico na PL.</p>
      <p>O ecocardiograma transtor&#225;cico mostrou fun&#231;&#227;o sist&#243;lica biventricular preservada com altera&#231;&#245;es da contractilidade das paredes anterior e lateral. O paciente permaneceu assintom&#225;tico durante o restante tempo de interna&#231;&#227;o e recebeu alta m&#233;dica.</p>
      <p>Dois meses ap&#243;s a alta, o paciente foi readmitido devido a dor tor&#225;cica pleur&#237;tica, ECG anormal mostrando eleva&#231;&#227;o difusa do segmento ST com concavidade superior e eleva&#231;&#227;o da prote&#237;na C reativa (199mg/L) e da troponina I de alta sensibilidade (2953 ng/L). O ecocardiograma transtor&#225;cico exibiu fun&#231;&#227;o sist&#243;lica biventricular preservada, com as altera&#231;&#245;es da contractilidade previamente relatadas e derrame peric&#225;rdico de pequeno volume. Com base nessa apresenta&#231;&#227;o, as hip&#243;teses diagn&#243;sticas levantadas foram a de s&#237;ndrome de Dressler versus outras causas de pericardite com les&#227;o mioc&#225;rdica associada.</p>
      <p>Foi realizada uma resson&#226;ncia magn&#233;tica card&#237;aca (RMC) para avalia&#231;&#227;o adicional, que revelou uma massa alongada intraperic&#225;rdica (medindo 25 x 13 x 40 mm) adjacente aos segmentos basais anterior e &#226;ntero-lateral e em estreito contato com o stent da DAE ( <xref ref-type="fig" rid="f01">Figura 1</xref> ). A massa apresentava sinal isointenso em imagens ponderadas em T1, hipersinal nas imagens ponderadas em T2, perfus&#227;o de primeira passagem e realce tardio heterog&#233;neo ap&#243;s administra&#231;&#227;o de gadol&#237;nio. A presen&#231;a de realce tardio subendoc&#225;rdico nos segmentos m&#233;dio-basais anteriores e antero-laterais confirmou o infarto pr&#233;vio no territ&#243;rio da DAE. Tamb&#233;m foi observada a presen&#231;a de realce peric&#225;rdico, devido &#224; presen&#231;a de atividade inflamat&#243;ria.</p>
      <p>

      </p>
      <p>Inicialmente, esses achados levantaram a preocupa&#231;&#227;o de uma complica&#231;&#227;o do procedimento endovascular previamente realizado envolvendo a DAE, como dissec&#231;&#227;o coron&#225;ria ou perfura&#231;&#227;o com hematoma organizado. Uma nova angiografia coronariana mostrou persist&#234;ncia do bom resultado em rela&#231;&#227;o ao stent da DAE, sem sinais de complica&#231;&#245;es relacionadas com o procedimento. Suspeitou-se ent&#227;o de uma origem neopl&#225;sica da massa. Foi realizada uma tomografia computadorizada (TC) do t&#243;rax que revelou uma les&#227;o suspeita no hilo esquerdo, junto ao br&#244;nquio do lobo superior esquerdo com invas&#227;o da veia pulmonar superior esquerda ( <xref ref-type="fig" rid="f02">Figura 2</xref> ). A bi&#243;psia da les&#227;o pulmonar esquerda demonstrou um tumor carcin&#243;ide do pulm&#227;o.</p>
      <p>

          <graphic xlink:href="1678-4170-abc-115-1suppl1-0022-90534-gf02.tif"/></fig>
      </p>
      <p>A presen&#231;a de linfadenopatias e n&#243;dulos pleurais apontou para uma natureza metast&#225;tica da massa adjacente &#224; DAE. A eleva&#231;&#227;o da troponina de alta sensibilidade foi interpretada como relacionada com a infiltra&#231;&#227;o mioc&#225;rdica. Apesar da presen&#231;a de doen&#231;a ateroscler&#243;tica em outras art&#233;rias coronarianas, n&#227;o foi poss&#237;vel excluir a hip&#243;tese de compress&#227;o externa da DAE pela massa metast&#225;tica como contribuinte para o IAMCSST pr&#233;vio da parede lateral.</p>
      <p>O diagn&#243;stico final foi de uma neoplasia pulmonar prim&#225;ria com envolvimento card&#237;aco secund&#225;rio.</p>
      <p>Investiga&#231;&#245;es posteriores revelaram doen&#231;a metast&#225;tica generalizada com envolvimento &#243;sseo, da gl&#226;ndula par&#243;tida, pancre&#225;tico e cerebral e o paciente iniciou quimioterapia direcionada e radioterapia. Aos dois anos de acompanhamento, o paciente encontrava-se livre de sintomas e eventos card&#237;acos e permanecia sob tratamento quimioter&#225;pico paliativo.</p>
    </sec>
    <sec sec-type="conclusions">
      <title>Conclus&#227;o</title>
      <p>Os sintomas relacionados com a doen&#231;a card&#237;aca metast&#225;tica, que podem ser inespec&#237;ficos e mimetizar outros dist&#250;rbios card&#237;acos, como doen&#231;a arterial coronariana ou pericardite, raramente representam a primeira manifesta&#231;&#227;o de uma malignidade previamente desconhecida. Enquanto o ecocardiograma &#233; o m&#233;todo de imagem mais utilizado para examinar o cora&#231;&#227;o e o peric&#225;rdio, a imagem multimodal com RMC e TC oferece vantagens no diagn&#243;stico da doen&#231;a card&#237;aca metast&#225;tica,<xref ref-type="bibr" rid="B6">6</xref> , <xref ref-type="bibr" rid="B7">7</xref> como foi demonstrado neste caso.</p>

        <title>Case Report</title>
        <p>We present a case of a 62-year-old male patient who had a medical history of hypertension and dyslipidemia and was a current smoker. He was first admitted to the hospital due to a lateral wall ST-segment elevation myocardial infarction (STEMI). Emergent coronary angiography (performed 2 hours after the onset of chest pain) revealed an 80% stenosis of the mid left anterior descending coronary artery (LAD), a total occlusion of the Dg1 (first diagonal branch of the LAD) on its ostium and a distal 70% stenosis of the left posterolateral branch of the left circumflex coronary artery (PL). Angioplasty with drug eluting stent (DES) implantation in the LAD and balloon dilatation of the Dg1 was performed. Percutaneous coronary intervention with DES implantation in the PL was conducted a few days later.</p>
        <p>The transthoracic echocardiogram (TTE) showed preserved biventricular systolic function with anterior and lateral wall motion abnormalities. The patient remained asymptomatic afterwards and was discharged home.</p>
        <p>Two months after discharge, the patient was readmitted due to pleuritic chest pain, abnormal ECG showing diffuse upward concave ST-segment elevation and elevated C-reactive protein (199 mg/L) and high-sensitive troponin I (2953 ng/L). The TTE exhibited preserved biventricular systolic function with the previously reported wall motion abnormalities and mild pericardial effusion. Based on this presentation, the diagnostic hypotheses raised were Dressler syndrome versus other causes of pericarditis with associated myocardial injury.</p>
        <p>A cardiac magnetic resonance imaging (cMRI) was performed for further evaluation, which revealed an intrapericardial elongated mass (measuring 25 x 13 x 40 mm) adjacent to the basal anterior and anterolateral segments and in close contact with the LAD stent ( <xref ref-type="fig" rid="f01001">Figure 1</xref> ). This mass had isointense signal intensity on T1-weighted images, high signal intensity on T2-weighted images, first-pass perfusion, and heterogeneous late gadolinium enhancement (LGE). Subendocardial LGE in the mid-basal anterior and anterolateral segments confirmed the previous infarction in the LAD territory. Contrast-enhanced pericardium was also noted, due to inflammatory activity.</p>
        <p>
          <fig id="f01001">
            <label>Figure 1</label>

(F) LGE with heterogeneous appearance of the mass and diffuse contrast-enhanced pericardium.</title>

        </p>
        <p>At first, these findings raised concerns about a complication of the previous endovascular procedure involving the LAD artery, such as coronary dissection or perforation with an organizing hematoma. A new coronary angiography showed persistence of the good result regarding the LAD stent, with no signs of procedure complications. A neoplastic origin of the mass was then suspected. A thoracic computed tomography (CT) was performed and unveiled a suspicious lesion in the left hilum, just next to the left superior lobe bronchus with invasion of the left superior pulmonary vein ( <xref ref-type="fig" rid="f02001">Figure 2</xref> ). A biopsy of the left pulmonary lesion revealed a carcinoid tumor of the lung.</p>
        <p>
          <fig id="f02001">
            <label>Figure 2</label>

        </p>
        <p>The presence of lymphadenopathy and pleural nodules pointed toward a metastatic nature of the mass adjacent to the LAD. High sensitive troponin elevation was interpreted as related to myocardial infiltration. Despite the presence of atherosclerotic disease in other coronary arteries, the hypothesis of external compression of the LAD by the metastatic mass as a contributor to the previous lateral wall STEMI could not be excluded.</p>
        <p>The final diagnosis was a primary lung malignancy with secondary involvement of the heart.</p>
        <p>Further investigation later unveiled widespread metastatic disease with bone, parotid gland, pancreatic and brain involvement and the patient started on targeted chemo- and radiotherapy. At two years of follow-up, the patient is free from cardiac symptoms and events and remains on palliative chemotherapy.</p>

        <title>Conclusion</title>
        <p>Symptoms related to metastatic heart disease, which can be nonspecific and mimic other cardiac disorders such as coronary artery disease or pericarditis, can rarely be the first manifestation of a previously unknown malignancy. Whereas echocardiography is the most frequently used imaging method to examine the heart and pericardium, multimodality imaging with cMRI and CT offers advantages in the diagnosis of metastatic heart disease,<xref ref-type="bibr" rid="B6">6</xref> , <xref ref-type="bibr" rid="B7">7</xref> as was demonstrated in this case.</p>
